Background Marching band artists are a physically active population, composed of approximately 27,000 people in the United States. University marching band artists face many of the same physically active demands and mental stressors as student athletes, potentially predisposing them to injury, illness, and risk for eating disorders (EDs). The purpose of this study was to examine ED risk across sex in university marching band artists, and to determine the type of risk based on the Eating Disorder Inventory-3 (EDI-3) and Eating Disorder Inventory-3 Symptom Check List (EDI-3 SC). A secondary aim examined marching band artists and pathogenic weight control behavior use across sex. Methods This was a cross-sectional study. A total of 150 marching band artists (female: n = 84, male: n = 66, age = 19.9 ± 1.1 years) from three National Collegiate Athletic Association Division I university marching bands participated in the study. We screened for ED risk using the EDI-3, and the EDI-3 SC. Results Overall, marching band artists were at risk for EDs, using only the EDI-3, 45.3% (n = 68) were at risk, with females at significant higher risk than males [χ2 = 5.228, p = .022]; using only the EDI-3 SC, 54% (n = 81) were at risk and no significant differences were found across sex. Overall, 48% of all participants reported dieting and 20.7% engaged in excessive exercise to control weight. Significant differences were found between sex and purging to control weight [χ2 = 3.94, p = .047] and laxative use [χ2 = 4.064, p = .044], with females engaging in behavior more than males. Conclusions Eating disorder risk was prevalent for both female and male marching band artists, with females displaying higher risk for EDs than males. Furthermore, marching band artists are engaging in pathogenic behaviors to control their weight. Healthcare providers (e.g., physicians, athletic trainers, physical therapist, dietitians, etc.) working in this setting should be aware of the risk factors displayed in marching band artists, and be able to provide education, prevention, and clinical interventions to this population. Additionally, marching band administrators should be aware of all medical risk factors and the benefit of having a healthcare provider (e.g., athletic trainer) to oversee the healthcare and wellness of marching band artists.

Individuals at risk for schizophrenia-spectrum disorders display abnormalities related to motivational salience, or the ability of stimuli to elicit attention due to associations with rewards or punishments. However, the nature of these abnormalities is unclear because most focus on responses to stimuli from broad “pleasant” and “unpleasant” categories and ignore the variation of motivational salience within these categories. In two groups at risk for schizophrenia-spectrum disorders—a Social Anhedonia group and a Psychotic-like Experiences group—and a control group, the current study examined event-related potential components sensitive to motivational salience—the Early Posterior Negativity (EPN), reflecting earlier selective attention, and the Late Positive Potential (LPP), reflecting sustained attention. Compared to controls, the Social Anhedonia group showed smaller increases in the EPN in response to erotica and smaller increases in the LPP as the motivational salience of pleasant images increased (excitinglarger increases in LPP amplitudes as the motivational salience of pleasant images increased. Also, both at-risk groups showed larger increases in the LPP to threatening images but smaller increases to mutilation images. These findings suggest that examining abnormalities beyond those associated with broad categories may be a way to identify mechanisms of dysfunction.

Background: Studies examining the physiological consequences associated with deficits in energy availability (EA) for male athletes are sparse.Purpose: To examine male athlete triad components; low energy availability (LEA) with or without an eating disorder risk (ED), reproductive hormone [testosterone (T)], and bone mineral density (BMD) in endurance-trained male athletes during different training periods.Methods: A cross-sectional design with 14 participants (age: 26.4 ± 4.2 years; weight: 70.6 ± 6.4 kg; height: 179.5 ± 4.3 cm; BMI: 21.9 ± 1.8 kg/m2) were recruited from the local community. Two separate training weeks [low (LV) and high (HV) training volumes] were used to collect the following: 7-day dietary and exercise logs, and blood concentration of T. Anthropometric measurements was taken prior to data collection. A one-time BMD measure (after the training weeks) and VO2max-HR regressions were utilized to calculate EEE.Results: Overall, EA presented as 27.6 ± 10.7 kcal/kgFFM·d-1 with 35% (n = 5) of participants demonstrating increased risk for ED. Examining male triad components, 64.3% presented with LEA (≤ 30 kcal/kgFFM·d-1) while participants presented with T (1780.6 ± 1672.6 ng/dl) and BMD (1.31 ±.09 g/cm2) within normal reference ranges. No differences were found across the 2 training weeks for EI, with slight differences for EA and EEE. Twenty-five participants (89.3%) under-ingested CHO across both weeks, with no differences between weeks.Conclusion: Majority of endurance-trained male athletes presented with one compromised component of the triad (LEA with or without ED risk); however, long-term negative effects on T and BMD were not demonstrated. Over 60% of the participants presented with an EA ≤ 30 kcal/kgFFM·d-1, along with almost 90% not meeting CHO needs. These results suggest male endurance-trained athletes may be at risk to negative health outcomes similar to mechanistic behaviors related to EA with or without ED in female athletes.

Objective - To investigate the immediate and 12-months effects of a school-based intervention aiming to reduce risk and enhance protective factors for eating disorder development in high school boys and girls. Method - In total, 4,149 adolescents from 30 high schools were eligible for inclusion and 2,446 consented to participate and were randomly allocated to the Healthy Body Image (HBI) intervention or a control group (classes as usual). The HBI intervention is multicomponent consisting of three workshops targeting body image, social media usage and lifestyle. Linear mixed model and intention-to-treat analyses were applied to investigate the effects of group, time, and gender at posttest, 3-, and 12-months follow-up. The main outcome variable was eating disorder sympomatology, and secondary outcome variables were self-esteem, mental distress, body image flexibility, thin internalization, muscular internalization, drive for leanness, perceived media pressure, protein- and creatine supplement use, and diet aid use. Results - The HBI intervention significantly reduced eating disorder risk factor scores related to eating disorder sympomatology, thin internalization and perceived pressure from media, which was particularly pronounced in girls. Positive intervention effects on body image flexibility were only observed at posttest for boys but grew increasingly larger for girls across the 12-month follow-up time span. Favorable intervention effects on protein and creatine supplement use were only present at 3-months follow-up in boys solely. A general favorable intervention effect was observed for self-esteem, mental distress, muscular internalization, and drive for leanness. Conclusion - The HBI intervention produced consistent reductions in risk factors and enhancements in protective factors associated with eating disorder development in adolescents.

The majority of advertisements contain thin-ideal imagery that have been digitally modified. A robust body of research has suggested that exposure to these retouched images has negative effects on body image and increases eating disorder risk. Furthermore, these concerns are known to be highly gendered both in nature and in their extent, with women revealing higher levels of concerns predominantly related to thinness. Although not supported as a useful approach by empirical data, in 2017, France introduced a law requiring advertisers to label images featuring models whose weight and/or shape have been altered. These images must bear the label “photographie retouchée”, or “retouched image”. However, this legislation has been difficult to enforce, as unlike other French legislation related to labeling advertising, its lack of specificity makes it difficult to identify violations. Paradoxically, given its intentions, where applied, uses of the label disproportionately focus on women’s bodies in the media, as compared to men’s bodies. These findings highlight the need for legislation that is enforceable and supported by the allocation of sufficient resources. In addition, findings highlight the importance of grounding legislation and policy in the extant relevant data and involving strategic stakeholders in its creation.

OBJECTIVES: Both mental health and eating disorders are increasing health concerns for college students. This study aimed to examine associative links between psychological distress, eating disorder risk, food addiction, and weight related concerns among first semester college students. METHODS: Freshman students 18 years or older in all majors in a southeastern university were invited to take an online survey. The measures included demographic characteristics, psychological behaviors and eating disorder risks. Pearson's correlation analysis was used to assess associations. RESULTS: Ninety-two students (79.3% female, 61.5% white) completed the survey. The passion of bodybuilding athletes for a symmetric, lean, heavily muscled body leads them to carry out exhausting exercise programs and restrictive eating regimens, sometimes resulting in disordered eating behaviors. This study investigates potential exacerbators on the development of disordered eating in bodybuilding and strength athletes. The study involved 103 Cypriot bodybuilding athletes of both sexes, performing at three levels: professional, recreational and strength athletes. The Eating Attitude Test 26 (EAT-26) and The Three Factor Eating Questionnaire (TFEQ-R21) were used to evaluate disordered eating and eating behaviors respectively. The current study was performed under the auspices of the Hellenic Center of Education & Treatment of Eating Disorders (KEADD). The degree of deviation between the perceived ideal body weight and the actual body weight was associated with increased risk of eating disorder. Athletes who desired a lower body weight recorded higher scores on EAT-26 overall, and the subscales of dieting and bulimia. Cognitive restraint and emotional eating scales of TFEQ-R21 were more pronounced in the non-professional athletes. The emotional eating score was higher in women. There is a need for appropriate sport-specific, gender-specific preventive intervention to deescalate the risk of eating disorder, in both professional and non-professional bodybuilding athletes.

Context Research exists on energy balances (EBs) and eating disorder (ED) risks in physically active populations and occupations by settings, but the EB and ED risk in athletic trainers (ATs) have not been investigated. Objective To assess ATs' energy needs, including the macronutrient profile, and examine ED risk and pathogenic behavioral differences between sexes (men, women) and job statuses (part time or full time) and among settings (college or university, high school, nontraditional). Design Cross-sectional study. Setting Free living in job settings. Patients or Other Participants Athletic trainers (n = 46; male part-time graduate assistant ATs = 12, male full-time ATs = 11, female part-time graduate assistant ATs = 11, female full-time ATs = 12) in the southeastern United States. Main Outcome Measure(s) Anthropometric measures (sex, age, height, weight, body composition), demographic characteristics (job status [full- or part-time AT], job setting [college/university, high school, nontraditional], years of AT experience, exercise background, alcohol use), resting metabolic rate, energy intake (EI), total daily energy expenditure (TDEE), EB, exercise energy expenditure, macronutrients (carbohydrates, protein, fats), the Eating Disorder Inventory-3, and the Eating Disorder Inventory-3 Symptom Checklist. Results The majority of participants (84.8%, n = 39) had an ED risk, with 26.1% (n = 12) engaging in at least 1 pathogenic behavior, 50% (n = 23) in 2 pathogenic behaviors, and 10.8% (n = 5) in >2 pathogenic behaviors. Also, 82.6% of ATs (n = 38) presented in negative EB (EI < TDEE). Differences were found in resting metabolic rate for sex and job status (F1,45 = 16.48, P = .001), EI (F1,45 = 12.01, P = .001), TDEE (F1,45 = 40.36, P < .001), and exercise energy expenditure (F1,38 = 5.353, P = .026). No significant relationship existed between Daily Reference Intakes recommendations for all macronutrients and sex or job status. Conclusions These athletic trainers experienced negative EB, similar to other professionals in high-demand occupations. Regardless of sex or job status, ATs had a high ED risk and participated in unhealthy pathogenic behaviors. The physical and mental concerns associated with these findings indicate a need for interventions targeted at ATs' health behaviors.

Introduction: It is difficult to detect athletes at risk of eating disorders. Objective: The aim of this study was to create a predictive equation for eating disorder risk behaviors (EDRB) in female athletes. Methods: 484 athletes participated in the study. The Eating Attitudes Test (EAT-26) was used to assess EDRB. The Sociocultural Attitudes Towards Appearance Questionnaire-3 was used to analyze the sociocultural influence on body image. The Body Shape Questionnaire and the Body Checking and Avoidance Questionnaire were used to assess body dissatisfaction in relation to thinness, and body checking, respectively. Skinfold thickness, body weight and height were also measured. The demographic data collected were age and weekly training regime. Results: The findings indicate that body mass index (BMI), body fat percentage (BF%), sociocultural influence, body checking and body dissatisfaction in relation to thinness explain 23% of the EDRB variance (p = 0.001). The results showed no difference between the EAT-26 scores and the predictive equation (p = 0.27). Conclusion: It was concluded that the predictive equation is reliable for evaluating EDRB in female athletes. Level of evidence III; study case-control.

Background:Within young individuals, mood disorder onset may be related to changes in trajectory of brain structure development. To date, however, longitudinal prospective studies remain scarce and show partly contradictory findings, with a lack of emphasis on changes at the level of global brain patterns. Cross-sectional adult studies have applied such methods and show that mood disorders are associated with accelerated brain ageing. Currently, it remains unclear whether young individuals show differential brain structure aging trajectories associated with onset of mood disorder and/or presence of familial risk.Methods:Participants included young individuals (15-30 years, 53%F) from the prospective longitudinal Scottish Bipolar Family Study with and without close family history of mood disorder. All were well at time of recruitment. Implementing a structural MRI-based brain age prediction model, we globally assessed individual trajectories of age-related structural change using the difference between predicted brain age and chronological age (brain-predicted age difference (brain-PAD)) at baseline and at 2-year follow-up. Based on follow-up clinical assessment, individuals were categorised into three groups: (i) controls who remained well (C-well,n= 93), (ii) high familial risk who remained well (HR-well,n= 74) and (iii) high familial risk who developed a mood disorder (HR-MD,n= 35).Results:At baseline, brain-PAD was comparable between groups. Results showed statistically significant negative trajectories of brain-PAD between baseline and follow-up for HR-MD versus C-well (β= -0.60,pcorrected< 0.001) and HR-well (β= -0.36,pcorrected= 0.02), with a potential intermediate trajectory for HR-well (β= -0.24 years,pcorrected= 0.06). Conclusions:These preliminary findings suggest that within young individuals, onset of mood disorder and familial risk may be associated with a deceleration in brain structure aging trajectories. Extended longitudinal research will need to corroborate findings of emerging maturational lags in relation to mood disorder risk and onset.

Injury risk is multifactorial including non-modifiable and modifiable factors such as nutrition and mental health. The purpose of this study was to estimate eating disorder risk and body image (BI) dissatisfaction among Reserve Officers Training Corps (ROTC) cadets. A total of 102 (male: n = 75, female: n = 27, age: 20 &plusmn, 2 years) ROTC cadets self-reported height, current and ideal weight, and completed the Eating Attitudes Test-26 (EAT-26) and self-perceived BI current and perceived sex-specific figural stimuli. The overall eating disorder risk for ROTC cadets was 32.4%. No significant differences were found when comparing sex, ethnicity, or military branch. Overall risk of pathogenic behaviors included 11.8% who reported binge eating, 8.8% who used laxatives, diuretics, or diet pills, 8.8% who exercised for &gt, 60 min to control their weight, and 8.8% who lost 9.1 kg or more within the last 6 months. We identified significant interactions (p &le, 0.01) between sex of the solider, overall perceptions of male and female soldiers, and BI self-perceptions. The ROTC cadets in this study displayed eating disorder risk and BI dissatisfaction, which is concerning for tactical readiness, long-term behavioral health issues, and injury from pathogenic behaviors. Education and quality healthcare are necessary to mitigate the increased risk of eating and BI dissatisfaction within this population.

Background Occupational groups operating in dangerous environments may witness the development of acute stress reaction (ASR) in team members. Witnessing ASR in team members may increase the risk of developing subsequent post-traumatic stress disorder (PTSD) symptoms. Aims To describe ASR symptoms that individuals witness, assess the relationship between witnessing a team member exhibiting ASR symptoms and an individual's own PTSD symptoms, and describe common intervention responses by peers. Method Cross-sectional, anonymous surveys were conducted with US soldiers who were previously deployed (sample 1; n = 176) and currently deployed sample 2; n = 497). Surveys assessed combat experiences, PTSD (PTSD Checklist-5), ASR exposure and intervention responses. Analyses included frequencies and binary logistic regression. Results Witnessing at least one ASR symptom during a combat-related event was reported by 51.7% in sample 1 and 42.4% in sample 2; the most commonly observed symptoms were being unable to function or being detached. Controlling for combat experiences, high levels of witnessing a team member exhibit ASR symptoms was associated with increased risk of subthreshold PTSD or PTSD in sample 1 (odds ratio (OR) = 8.69, 95% CI 2.29–42.60) and approached significance in sample 2 (OR = 1.67, 95% CI 0.98–2.81). Common intervention responses included providing a directive or yelling; many also reported being unsure how to respond. Conclusions Witnessing team members who exhibit ASR symptoms appears to be associated with screening positive for subthreshold PTSD or PTSD. Results suggest the need for further research into how to prepare individuals to manage ASR in team members and to examine ASR in other high-risk occupations.

Background Body dissatisfaction influences women’s mental and physical health. To date, most research has focused on body dissatisfaction in relation to the ‘thin-ideal’. Thus, the association between body dissatisfaction, eating disorder symptomatology and muscularity-ideal in women is less clear. Lack of understanding is underpinned by the lack of reliable and valid muscularity-related assessments for women. To address this need, we developed, tested and re-tested two new body dissatisfaction scales: The Female Body Scale (FBS; adiposity dimension) and Female Fit Body Scale (FFITBS; muscularity dimension). Methods One hundred and fifty-two women in the United Kingdom rated which body figure best represented their current and ideal body, completed the Eating Disorder Examination Questionnaire (EDE-Q 6.0), and their body composition was measured. During re-test, the EDE-Q 6.0 and Drive for Muscularity Scale (DMS) were completed. Results Both the FBS and the FFITBS were found to be valid and reliable, and distinct types of body dissatisfaction were identified. Higher EDE-Q scores corresponded with greater body dissatisfaction scores on both the FBS and FFITBS. Thin-ideal (FBS) and larger/muscularity-ideal (FFITBS) body dissatisfaction predicted higher scores on the DMS. The muscularity scale (FFITBS) uniquely revealed that 28% of participants indicated body dissatisfaction toward the larger-muscularity-ideal. Conclusions Results reveal distinct dimensions of body dissatisfaction. These new, validated scales may be utilized to quickly identify eating disorder risk in women as a preventative assessment for clinicians and inform female-focused body-image and eating disorder research.

The current COVID‐19 pandemic has created a global context likely to increase eating disorder (ED) risk and symptoms, decrease factors that protect against EDs, and exacerbate barriers to care. Three pathways exist by which this pandemic may exacerbate ED risk. One, the disruptions to daily routines and constraints to outdoor activities may increase weight and shape concerns, and negatively impact eating, exercise, and sleeping patterns, which may in turn increase ED risk and symptoms. Relatedly, the pandemic and accompanying social restrictions may deprive individuals of social support and adaptive coping strategies, thereby potentially elevating ED risk and symptoms by removing protective factors. Two, increased exposure to ED‐specific or anxiety‐provoking media, as well as increased reliance on video conferencing, may increase ED risk and symptoms. Three, fears of contagion may increase ED symptoms specifically related to health concerns, or by the pursuit of restrictive diets focused on increasing immunity. In addition, elevated rates of stress and negative affect due to the pandemic and social isolation may also contribute to increasing risk. Evaluating and assessing these factors are key to better understanding the impact of the pandemic on ED risk and recovery and to inform resource dissemination and targets.

BackgroundAccelerated biological ageing has been proposed as a mechanism underlying mood disorder, but has been predominantly studied cross-sectionally in adult populations. It remains unclear whether differential ageing/maturation trajectories emerge earlier in life, in particular during the neurodevelopmental period of adolescence, and whether they are associated with onset of mood disorder and/or presence of familial risk.MethodsParticipants were young individuals (16-25 years) from the prospective longitudinal Scottish Bipolar Family Study (SBFS) with and without family history of mood disorder. All were well at time of recruitment. Implementing a structural MRI-based brain age prediction model, individual maturational trajectories were captured by the difference between predicted brain age and chronological age (brain-PAD) at baseline and two-year follow-up. Based on clinical assessment at follow-up, individuals were categorised into three groups: (i) controls who remained well (C-well,n=94), (ii) high familial risk who remained well (HR-well,n=73) and (iii) high familial risk who developed a mood disorder (HR-MD,n=38).ResultsResults showed no differences in brain-PAD between groups at baseline or follow-up. However, we found negative trajectories of brain-PAD for HR-MD versus C-well (β= −0.68 years,pp=.01), and for HR-well versus C-well (β= −0.30 years,p=.03).ConclusionsThese findings suggest that within young individuals, onset of mood disorder and familial risk may be associated with a deceleration in brain maturation trajectory. However, without significantly differential status of brain maturation at follow-up, extended longitudinal research will need to show whether this marks the emergence of maturational lag.

The purpose of the present study was to examine the association among the risk of eating disorder (ED), psychological adjustment, empathy, attachment style and thin-ideal internalization among adolescents with anorexia nervosa (AN), as compared to a healthy control (HC) group. The Questionnaire of General Data, the Eating Disorders Inventory-3 (EDI-3), the Questionnaire of Sociocultural Attitudes Towards Appearance-3 (SATAQ-3), the Experiences in Close Relationships-Revised (ECR-R) and the Basic Empathy Scale (BES) were administered to 35 female adolescents with AN and 35 HC adolescents. Eating disorder risk was found to be highest in AN females with higher levels of general psychological maladjustment (GPMC), followed by HC females with higher thin-ideal internalization. In the AN group, attachment style and thin-ideal internalization in combination with GPMC were not indicated as significant predictors of eating disorder risk. Study data provide support to conceptualizations of EDs that emphasize the role of general psychological maladjustment in the development of EDs., Cilj ovoga istraživanja bio je ispitati povezanost između rizika za razvoj poremećaja u jedenju (ED), psihološke prilagodbe, empatije, privrženosti i internalizacije mršavosti među adolescentima s anoreksijom nervozom (AN) u odnosu na zdravu kontrolnu skupinu (HC). Upitnik općih podataka, Upitnik o poremećaju jedenja-3 (EDI-3), Upitnik sociokulturnih stavova prema izgledu-3 (SATAQ-3), Revidirani upitnik o iskustvima u bliskim odnosima (ECR-R) i Upitnik o empatiji (BES) ispunilo je 35 adolescentica s dijagnosticiranim poremećajem u jedenju (anoreksija nervoza) i 35 zdravih adolescentica koje su činile kontrolnu skupinu. Rizik za razvoj poremećaja u jedenju se pronašao najvećim u adolescentica s anoreksijom nervozom koje su imale značajne teškoće u općoj psihološkoj prilagodbi, zatim u zdravih adolescentica kod kojih se potvrdila značajna internalizacija ideala mršavosti. U djevojaka s utvrđenom dijagnozom anoreksije nervoze, stil privrženosti i internalizacija ideala mršavosti se nisu pokazali značajnim prediktorom, u kombinaciji s faktorom opće psihološke neprilagodbe, za rizik za razvoj poremećaja u jedenju. Rezultati podupiru konstrukt poremećaja u jedenju koji naglašava ulogu opće psihološke prilagodbe u razvoju poremećaja u jedenju.

BackgroundEndophenotypes are laboratory-based measures hypothesized to lie in the causal chain between genes and clinical disorder, and to serve as a more powerful way to identify genes associated with the disorder. One promise of endophenotypes is that they may assist in elucidating the neurobehavioral mechanisms by which an associated genetic polymorphism affects disorder risk in complex traits. We evaluated this promise by testing the extent to which variants discovered to be associated with schizophrenia through large-scale meta-analysis show associations with psychophysiological endophenotypes.MethodWe genome-wide genotyped and imputed 4905 individuals. Of these, 1837 were whole-genome-sequenced at 11× depth. In a community-based sample, we conducted targeted tests of variants within schizophrenia-associated loci, as well as genome-wide polygenic tests of association, with 17 psychophysiological endophenotypes including acoustic startle response and affective startle modulation, antisaccade, multiple frequencies of resting electroencephalogram (EEG), electrodermal activity and P300 event-related potential.ResultsUsing single variant tests and gene-based tests we found suggestive evidence for an association between contactin 4 (CNTN4) and antisaccade and P300. We were unable to find any other variant or gene within the 108 schizophrenia loci significantly associated with any of our 17 endophenotypes. Polygenic risk scores indexing genetic vulnerability to schizophrenia were not related to any of the psychophysiological endophenotypes after correction for multiple testing.ConclusionsThe results indicate significant difficulty in using psychophysiological endophenotypes to characterize the genetically influenced neurobehavioral mechanisms by which risk loci identified in genome-wide association studies affect disorder risk.

Background: There is increasing understanding of the significance of early neurodevelopment in establishing risk for the range of mental disorders. Models of the early aetiology of mental disorders are complex with a range of potential factors from genetic and epigenetic to environmental influencing neurological and psychological development. Whilst the mechanisms are not fully understood, this paper provides an overview of potential biological and neurobiological factors that might be involved. Method: An aetiological model is presented and discussed. The discussion includes a range of risk factors for mental disorder. Maternal anxiety disorder is presented and reviewed as an example of the interaction of placental, epigenetic and early parenting factors elevating risk of poor neonatal outcome. Results: Available evidence points to the importance of in-utero influences as well as the role of early attachment and emotional care. Transgenerational mechanisms such as the impact of maternal mental disorder on foetal development are important models for examination of early risk. Maternal anxiety, as an example, is a significant risk factor for compromised mental health. Conclusions: Development of models for understanding the early origins of mental disorder is an important step in elaborating risk reduction strategies. Comprehensive early identification of risk raises the possibility of preventive interventions

Over the past two decades, the field of eating disorders has made remarkable strides in identifying, evaluating, and disseminating successful prevention programs. The current review identifies and discusses nine distinct eating disorders prevention programs that reduce existing eating disorder pathology or prevent the onset of future pathology. Each program was evaluated in one or more controlled trial with a follow-up period of at least six months. We review the evidence base for these nine successful programs and discuss their common and unique features. Based on authors' descriptions of their programs in published trials, we found that all programs were theory-driven, targeted one or more eating disorder risk factor (e.g., body dissatisfaction), were delivered across multiple group sessions, and included at least some interactive content. Most programs included content related to healthy eating/nutrition, media literacy/sociocultural pressures, and body acceptance/body satisfaction. Notably, there was wide variation in some participant features (e.g., participant age, sex, risk status) and intervention features (e.g., setting and format, length and dose, providers), suggesting that a variety of programs are beneficial in impacting eating disorder pathology. Implications and directions for future research are discussed, including an increased focus on universal and indicated prevention programs, expanding programs to a wider age range and a broader spectrum of weight-related problems, and rigorous evaluation of programs through efficacy, effectiveness, and implementation research.

ObjectiveTo estimate the risk for developing a defined neurodegenerative syndrome in a large cohort of idiopathic REM sleep behavior disorder (IRBD) patients with long follow-up.MethodsUsing the Kaplan-Meier method, we estimated the disease-free survival rate from defined neurodegenerative syndromes in all the consecutive IRBD patients diagnosed and followed-up in our tertiary referal sleep center between November 1991 and July 2013.ResultsThe cohort comprises 174 patients with a median age at diagnosis of IRBD of 69 years and a median follow-up of four years. The risk of a defined neurodegenerative syndrome from the time of IRBD diagnosis was 33.1% at five years, 75.7% at ten years, and 90.9% at 14 years. The median conversion time was 7.5 years. Emerging diagnoses (37.4%) were dementia with Lewy bodies (DLB) in 29 subjects, Parkinson disease (PD) in 22, multiple system atrophy (MSA) in two, and mild cognitive impairment (MCI) in 12. In six cases, in whom postmortem was performed, neuropathological examination disclosed neuronal loss and widespread Lewy-type pathology in the brain in each case.ConclusionsIn a large IRBD cohort diagnosed in a tertiary referal sleep center, prolonged follow-up indicated that the majority of patients are eventually diagnosed with the synucleinopathies PD, DLB and less frequently MSA. IRBD represented the prodromal period of these conditions. Our findings in IRBD have important implications in clinical practice, in the investigation of the early pathological events occurring in the synucleinopathies, and for the design of interventions with potential disease-modifying agents.

Objective: To evaluate eating disorders risk behaviors and unhealthy weight control practices among adolescents who consider themselves as vegetarians and those who are omnivorous.Material and Methods: A cross-sectional study was conducted in 12 technical schools in the city of São Paulo, Brazil. The sample included 1167 adolescents (51% female), aged 14 to 19 (mean age, 16). Adolescents stated whether they were currently vegetarian, and the sample was dichotomized as vegetarian and non-vegetarian. The two groups were compared as regards weight status, eating disorder risk behavior, unhealthy weight control methods and the perception of healthy eating.Results: About 4% of the sample was currently vegetarian, most of them female (70.8%), and females were 2.89 times more likely to be vegetarian than males. No relationship was found between the vegetarian status and unhealthy weight control behavior; however the vegetarian group considered their diet to be healthier than the non-vegetarian group (P=.04).Conclusions: The frequency of vegetarianism, as well the frequency of eating disorder risk behaviors had no association in this adolescent sample from São Paulo, Brazil.

Cilj ovog istraživanja bio je ispitati relativan doprinos rizicnih cimbenika iz razlicitih domena (odnosi u obitelji i s vrsnjacima te osjecaj pripadnosti skoli) u objasnjavanju eksternaliziranih ponasanja (agresivnoga i rizicnoga spolnog ponasanja) i odstupajucih navika hranjenja mladica (n=429) i djevojaka (n=307) iz opce populacije između 15 i 20 godina. Kako bi se ispitala prediktivna snaga ispitivanih varijabli za predviđanje eksternaliziranih ponasanja i odstupajucih navika hranjenja kao kriterijskih varijabli, proveden je niz hijerarhijskih regresijskih analiza. Opcenito, kvaliteta odnosa s vrsnjacima i osjecaj pripadnosti skoli pokazali su se negativnim prediktorima agresivnoga ponasanja i pozitivnim prediktorima odstupajucih navika hranjenja. Nadalje, mladi s boljim skolskim uspjehom rjeđe su izvjestavali o rizicnom spolnom ponasanju, ali su cesce navodili simptome odstupajucih navika hranjenja. Ucinak kvalitete odnosa s obitelji na eksternalizirana ponasanja i odstupajuce navike hranjenja posredovan je kvalitetom odnosa s vrsnjacima. Nisu utvrđene spolne razlike u rizicnim cimbenicima za razlicita ponasanja.

This study explored eating disorder risk factors and possible psychosocial predictors of this risk in overweight and obese treatment-seeking adolescents. Prior to commencing treatment 108 overweight and obese adolescents aged 11 to 17 years (M = 14.31, SD = 1.57; 55% female) completed self-report measures of psychosocial factors. Females reported elevated levels of bulimic tendencies, body dissatisfaction, drive for thinness (p ≤.001) and males reported elevated body dissatisfaction (p < .001). Age, sex and BMI-for-age z-score explained 15% (p < .001) of the variance in eating disorder risk and psychosocial predictors an additional 25%. Sex did not have a moderating effect on these relationships (p = .21). Among overweight and obese treatment-seeking adolescents, those experiencing lower self-esteem and elevated depression and anxiety symptomatology are at increased eating disorder risk. This highlights the need to consider psychosocial factors in preventing and treating overweight and obesity.

The aim of this study is to explore the factor structure and internal consistency of the Eating Disorder Inventory-3 (EDI-3) among female adolescents from Buenos Aires (Argentina). 725 high school students, aged 13 to 19 years, completed the EDI-3 and a sociodemographic questionnaire. Different factor solutions were evaluated. The most satisfactory solution was reached to explore, first, the items of the three Eating Disorder Risk Scales (good structure, variance explained = 46.7%) and on the other, items of the Psychological Scales (with partial divergence from theoretical structure, variance explained = 44.1%). The computation of Composites (combining different scales proposed by the author) fitted more closely to the original version. Internal consistency coefficients for most scales were adequate. In analyzing the discriminatory power of the items, it was found a “floor effect” on some scales, this being expected in general population. We conclude that the EDI-3 is a valid instrument for the assessment of risk for eating disorders in general population and, consequently, its early detection. We discuss the use of this instrument in general population.
