Abstract Background South-South Cooperation (SSC) is recognized as a vital component of goal 17 – ‘Partnerships for the goals’ within the SDGs and countries in the global south has been utilizing this cooperation mainly in research. We report the first health care professional exchange program through this south-to-south collaboration focusing on advancing quality of abortion care in the Sub-Saharan region. Methods We conducted a comprehensive mixed method study, incorporating both quantitative and qualitative approaches, to assess the impact of health care professionals’ exchange program within the obstetrics and gynecology departments of two teaching institutions in Ethiopia and Rwanda over the course of a year (February 1, 2023-January 31,2024). Our methodology involved administering a self-administered questionnaire on Google Form to health professionals who participated in the exchange program. Qualitative data were collected through an in-depth interview with representative subjects from these professionals. Simple descriptive statistics using SPSS version 23 was employed to analyze the quantitative data. For the qualitative data, recruitment of participants continued until thematic saturation was achieved, signifying no emergence of new themes. Thematic analysis, following initial coding and inter-coder agreement, was employed to identify key themes from the interviews. Finally, triangulation of qualitative and quantitative data was performed to provide a comprehensive understanding of the program’s impact. Results Among 60 health care professionals whom we have approached for this survey, 55 consented and participated in the survey, representing a response rate of 92%. Among them, 35 (63.6%) were from Rwanda, 20 (36.4%) were from Ethiopia, 23.6% were females, 80.0% were residents and 16.4% were midwives. The overall mean scores for knowledge, attitude, and practice transformation on safe abortion care by Rwandese professionals were 84.9%, 84.5%, and 84.6%, respectively. From the Ethiopian side, the overall score for knowledge and attitude, and intent to practice demonstration were 81.3, 83.4%. Essentially, they expressed that it allowed them to inspire Rwandese counterparts on the importance of women’s reproductive rights and implementation of global best practice (89%). On analysis of the qualitative data, we identified 3 overarching themes: 1) Opportunities created by the exchange 2) Inputs on safe abortion care quality improvement 3) Recommendations for future exchange programs. Most participants indicated that the exchange program was very helpful in advancing knowledge, attitude, and intent to practice quality abortion care. Participants from both sides agreed that the safe abortion care services provided in Ethiopia is far advanced, which can be reproduced in Rwanda by implementing the lessons learned from the exchange program. Conclusion South-to-south collaboration through health care professionals exchange programs among developing countries (including Sub-Saharan Africa) is feasible and helpful in transforming the level of reproductive health practice (mainly safe abortion care) as well as advocating for better reproductive health rights in these countries.

Abstract Background Half of countries in Africa lack access to radiation (RT), which is essential for standard treatment of locally advanced cervical cancers. We evaluated outcomes for patients treated with neoadjuvant chemotherapy (NACT) followed by radical hysterectomy in settings where no RT is available. Methods We performed a retrospective descriptive study of all patients with FIGO stage IB2-IIA2 and some exceptional stage IIB cases who received NACT and surgery at Kigali University Teaching Hospital in Rwanda. Patients were treated with NACT consisting of carboplatin and paclitaxel once every 3 weeks for 3–4 cycles before radical hysterectomy. We calculated recurrence rates and overall survival (OS) rate was determined by Kaplan-Meier estimates. Results Between May 2016 and October 2018, 57 patients underwent NACT and 43 (75.4%) were candidates for radical hysterectomy after clinical response assessment. Among the 43 patients who received NACT and surgery, the median age was 56 years, 14% were HIV positive, and FIGO stage distribution was: IB2 (32.6%), IIA1 (7.0%), IIA2 (51.2%) and IIB (9.3%). Thirty-nine (96%) patients received 3 cycles and 4 (4%) received 4 cycles of NACT. Thirty-eight (88.4%) patients underwent radical hysterectomy as planned and 5 (11.6%) had surgery aborted due to grossly metastatic disease. Two patients were lost to follow up after surgery and excluded from survival analysis. For the remaining 41 patients with median follow-up time of 34.4 months, 32 (78%) were alive with no evidence of recurrence, and 8 (20%) were alive with recurrence. One patient died of an unrelated cancer. The 3-year OS rate for the 41 patients who underwent NACT and surgery was 80.8% with a recurrence rate of 20%. Conclusions Neoadjuvant chemotherapy with radical hysterectomy is a feasible treatment option for locally advanced cervical cancer in settings with limited access to RT. With an increase in gynecologic oncologists skilled at radical surgery, this approach may be a more widely available alternative treatment option in countries without radiation facilities.

IntroductionStudies exploring the clinical and imaging characteristics of juvenile-onset back pain (JOBP) are scarce. The purpose of this study was to assess the clinical presentation, imaging findings, and factors associated with JOBP.MethodsA retrospective record-based study was conducted among all pediatric cases presenting to the Rheumatology unit of the Douala General Hospital, Cameroon, from January 2014 to December 2023. The study did not include children whose back pain began after 16 years of age.ResultsOf the 216 records of patients under 18 examined, 67 children (31 girls) were diagnosed with JOBP. The median age was 15 [13–16] years. More than two-third of the children included in this study had chronic pain (n = 46; 68.65%). Pain was mild to moderate in intensity in 48 children (71.6%). Radiculopathy was present in 10 children (14.92%). The most common location of back pain was the lumbar region (n = 64; 95.52%). Some children experienced pain in more than one location. The location of the pain was not associated with gender, duration of the pain, radiculopathy, or practice of a competitive sport (p > 0.05). Musculoskeletal abnormalities on imaging were found in 38 children (62.29%). In multivariate analysis, peripheral joint involvement [aOR = 0.253 (0.073–0.878); p = 0.030] and mild pain intensity [aOR = 0.077 (0.014–0.422); p = 0.003], were independently associated with the presence of musculoskeletal abnormalities on imaging.ConclusionJOBP affects a third of children and adolescents of our clinic population. The common musculoskeletal abnormalities found on spine imaging are disc diseases and scoliosis.

Abstract Background Autism spectrum disorder (ASD) is a neurodevelopmental disability associated with deficiency in social interaction, unusual development of social communication, and restricted or repetitive behaviors, interests and activities. This study aimed to describe management of pediatric ASD in Cameroon, a resource-constrained Central Africa country. Methods A retrospective study was conducted between December 2021 and May 2022 at the Pediatrics department of a reference hospital in the town of Douala. Data of interest of children with ASD were collected through eligible medical records and telephone discussions with their parents/guardians. Results Medical records of 145 children with ASD aged 2–15 years were included in the study, giving a hospital ASD prevalence of 3.7%. Time delay between parental concerns and hospital management was specified in 69 (47.58%) children, and among them 38 (55.07%) had a mean delay ± SD was less than five months. Children were mainly males (76%) and aged 4–5 years (37.93%), with mean age ± SD of 44.4 ± 22.2 months old. The main consultation reason was delayed language development (100%). Mean time delay between parental concerns and the first medical consultation was 18 months (range 1–60 month). Attention deficit hyperactivity disorder were found in 68.18% of children aged ≥ 6 years old. Neuropsychology (66.2%) was the most frequently used intervention. Some children were treated using traditional medicine. Conclusions Management of pediatric ASD is strongly influenced by socioeconomic and cultural context. It is crucial to implement behavioral change campaigns in community, organize training sessions to medical staff on diagnosis and treatment of ASD, and provide specialized centers with skilled staff and equipped material.

BackgroundResearch efforts in Rwanda to improve sexual and reproductive health and rights (SRHR) are increasing; however, comprehensive literature reviews on SRHR are limited. This scoping review examines individual and contextual factors shaping knowledge, attitudes, and practices in the domains of: 1) family planning, 2) abortion care, and 3) other SRHR in Rwanda. Recognizing that individual, community, and societal factors influence RH, this review is guided by Bronfenbrenner's Ecological Systems Theory.MethodsEligible studies were conducted in Rwanda, included males and/or females of any age, and were published within the past 20 years. Studies reporting views of only healthcare or other professionals were excluded.ResultsThirty-six studies were included. The majority addressed individual and contextual considerations. At the individual level, studies explored knowledge about SRHR problems while at the interpersonal level, the support and attitudes of men and community members for adolescent SRHR were investigated. In terms of healthcare organization, maternal health practices, increased access to family planning programs, and the need for sexually transmitted infection programs was explored. At the social and cultural level, researchers investigated beliefs and traditional gender roles. Regarding public health policy, studies mentioned promoting and increasing funding for SRHR and reducing gender inequities.ConclusionOur findings can inform SRHR research programs, public health campaigns, and policy advances in Rwanda.

Missed opportunities for vaccination (MOV) reflect quality of immunization service. The objective of this study was to assess vaccination timeliness, prevalence, and characteristics of MOVs among children aged 0-23 months, as well as knowledge, attitude and practice of health workers towards immunization. An exit interview method was used to select caregivers and health personnel. Selection took place in 26 health facilities within 14 health areas in the Dshcang Health district. Data were collected using two face-to-face questionnaires adapted from the World Health Organization (WHO) tools. We conducted an evaluation of all free vaccines in the Expanded Programme on Immunisation (EPI). We studied timeliness, assessed MOV, and knowledge, behaviour and attitude of health workers on immunization. Basic statistical tests were used to study the association between MOV and socio demographic characteristics. A total of 363 children aged 0 to 23 months were surveyed. A total of 88 (91.66%) of health personnel agreed to participate in our study. A total of 298 (82.1%) children had vaccination cards with dates, leading to 18% not completely vaccinated. Vaccination timeliness ranged from 20% to 77%. Overall MOV estimated was 23.83%, range from 0% to 16.4% among all vaccines. Among health workers, 70.45% (62/88) had insufficient knowledge on vaccination, 73.86% assessed the vaccination status of children during any routine visit and 74% ask parents to bring the child's vaccination record to any health facility visit. The study highlighted presence of MOV among children. Strategies for remedying this includes strengthening parents' knowledge, organizing refresher courses for health workers on vaccination, and systematically assessing children's vaccination status.

Abstract Background In the midst of the COVID-19 pandemic, to palliate to the lockdown and cover academic programs, the faculty of medicine and pharmaceutical sciences (FMPS) of the university of Dschang (UDs) in Cameroon has implemented e-learning using WhatsApp®. Aim Describe the opinion of students and lecturers after its implementation of e-learning at the FMPS of UDs. Methods We designed a uniform teaching scheme using WhatsApp® during the university lockdown. Students and members of the teaching staff of the FMPS of UDs were enrolled after receiving clear information on the study implementation. At the end of the online-teaching period of two and a half months, we surveyed our students and teaching staff. Sociodemographic characteristics and opinions about e-learning were collected using a standard questionnaire. Results We enrolled 229 students and 40 lecturers of the FMPS. Students reported a decremented quality of internet connection (p 0.05). While lecturers were mainly more comfortable conveying the contents of their lectures, students tended to be less prone to actively participate. The motivation and satisfaction of the latter group toward e-learning were modest compared to classroom-based lectures while their feedback about the organization was positive. Conclusions E-learning using WhatsApp® could be an effective alternative to conventional classroom-based lessons in the context of COVID-19 pandemic. The use of a blended-learning program including classroom-based sessions could help improve its limitations.

Maternal mortality remains critically high in low- and middle-income countries (LMIC), particularly in sub-Saharan Africa. Rwanda’s leading causes of maternal death include postpartum hemorrhage and obstructed labor. Maternal recognition of obstetrical danger signs is critical for timely access to emergency care to reduce maternal mortality.To assess maternal knowledge of obstetrical danger signs among pregnant women attending antenatal care services in Kigali, Rwanda. We conducted a cross-sectional study between September and December 2018. The outcome of interest was maternal knowledge of ODS during pregnancy, labor and delivery, and the immediate postpartum period. We recruited pregnant women at five health centers, one district hospital, and one referral hospital, and we had them complete a structured questionnaire. Reporting three correct ODS was defined as having good knowledge of ODS. A total of 382 pregnant women responded to the survey. Most women (48.9%) were aged 26–35, and 50.5% had completed secondary or higher education. The knowledge of ODS was 56%, 9%, and 17% during pregnancy, labor and delivery, and postpartum, respectively. Women aged 26 to 35 had two times (OR: 1.80, 95% CI: 1.05, 3.06) higher odds of ODS knowledge during pregnancy than women aged 16 to 25. Attending three antenatal care visits was associated with 2.6 times (OR: 2.59, 95% CI: 1.17, 5.66) higher odds of ODS knowledge during pregnancy than not attending any visit. Longer distances to the nearby health facility were associated with significantly lower knowledge during pregnancy, and Muslim women had substantially higher postpartum ODS knowledge than any other religion. In conclusion, women’s knowledge of ODS associated with labor and delivery and postpartum was low. Antenatal care must be encouraged and its content revised to ensure it covers potential late pregnancy complications.

Our objective was to assess the health facility related factors that cause delays in cervical cancer diagnosis at a primary healthcare level in Rwanda. Healthcare providers in outpatient clinics at 10 health centers in Kigali city and the Eastern province of Rwanda were surveyed. Eighty-five healthcare providers participated; 83.5% were nurses and the remainder were midwives. Only 15 (17.6%) reported prior training on visual inspection with acetic acid (VIA) cervical cancer screening, and they were distributed among 6 of the 10 health centers surveyed. However, 76.5% of respondents reported that at least one person was trained in VIA at their health center. The basic equipment necessary for cervical cancer evaluation was reported to be generally available. Overall, only 31.8% of participants had good basic knowledge level on cervical cancer screening. No association was found between respondents’ knowledge about cervical cancer screening and profession, education level, work experience or reported prior training on VIA. There is a gap in the number of primary healthcare providers with the skills to perform pelvic exam and VIA cervical cancer screening at health centers in Rwanda. As health centers are the first point of contact for patients with the healthcare system, there is a need to improve their knowledge and skills in performing cervical cancer screening and detection.

In the acute phase of the COVID-19 outbreak, focus is usually made on the fact that children are less susceptible to severe forms. However, the COVID-19 pandemic has numerous other issues in children’s health that maybe minimized in the Cameroonian context. A survey of pediatricians practicing in Cameroon showed a significant drop in pediatric outpatient consultations, from 60.4% of pediatricians seeing more than 30 patients per week before the pandemic to 9.9% during the pandemic. This had many consequences among which the delay in urgent care with an increased mortality, postponed routine immunization with the risk of an upsurge in vaccine-preventable diseases. Another study showed that mortality doubled during the months of April and May 2020 with 9.9% and 11.2% of hospital deaths respectively, compared to 4.9% (p = 0.0008) and 5.1% (p = 0.0001) during the same period of the previous year. The closure of schools and universities, by imposing an unusual sedentary lifestyle, could result in highest exposure to risk related to insufficient physical activity. Another major issue is the psychological impact of the outbreak on child’s mental health. Regarding the therapeutic and acute aspects of the care of children during the COVID-19 pandemic should not obscure a more global approach with other issues. The organization of care should be inclusive and take into account the physical, somatic impact as well as the psychological and socio-cultural aspects.

ObjectiveMalaria in pregnancy is associated with adverse perinatal outcomes. The objective was to compare outcomes of simple and severe malaria and to determine whether they vary by trimester or severity of infection.MethodsProspective cohort study performed in 3 hospitals in Rwanda. Both hospitalized and non-hospitalized pregnant patients with confirmed malaria were enrolled and followed until 7 days postpartum. Demographic, clinical manifestations and perinatal outcomes were recorded.ResultsThere were 446 pregnant women with confirmed malaria and outcome data; 361 (80.9%) had simple malaria. Severe malaria was more common as pregnancy progressed; out of 85 with severe malaria, 12.9%, 29.4% and 57.6% were in the 1st, 2nd and 3rd trimesters (pConclusionOverall poor outcomes are seen in malaria with significant differences in perinatal outcomes between simple and severe malaria and by trimester of infection. In addition to vector control and exposure prevention, efforts need to be made in screening, treatment education and monitoring pregnancies affected by malaria.

Despite some cases of severe or critical manifestations of the coronavirus disease 2019 (COVID-19) described among children, the prevalence of this infection in the pediatric population is quite low worldwide, particularly in sub-Saharan Africa. Current data suggest indeed that, independent of the population considered overall, severe and critical cases of COVID-19 are rare among children. This observation prompted us to discuss the possible hypotheses which could explain the low prevalence of COVID-19 among children; amongst others, we discuss (1) immunomodulation by the Bacillus Calmette–Guerin vaccine or by some parasitic infections such as malaria, schistosomiasis, and helminthiasis and (2) cross immunization with other coronaviruses commonly found in the sub-Saharan African setting.

La dilatation aiguë de l'estomac est une pathologie rare. Elle est classiquement observée en psychiatrie dans les troubles du comportement alimentaire. Le diagnostic préopératoire est difficile et fait appel à la radiologie. La mortalité liée aux complications varie entre 80% et 100%. Nous rapportons 2cas de dilatation aiguë de l'estomac prises en charge au service de Chirurgie Générale de l'Hôpital Aristide Le Dantec de Dakar dont l'une était compliquée de nécrose et l'autre d'une rupture gastrique. Il s'agissait de 2 patients dont l'un était de sexe masculin âgé de 32 ans et l'autre de sexe féminin âgée de 36 ans. Ils étaient reçus dans un tableau de douleurs abdominales aiguës et un état de collapsus cardiovasculaire. L'examen avait retrouvé un syndrome d'irritation péritonéale chez les 2 patients. A la biologie, on notait une anémie chez tous les patients. A la radiographie de l'abdomen sans préparation, on notait un pneumopéritoine massif chez le patient et un gros niveau hydro-aérique chez la patiente. Le diagnostic préopératoire était une péritonite par perforation d'organe creux chez le patient et une occlusion intestinale aiguë chez la patiente. Après une réanimation, la laparotomie avait permis de retrouver une dilatation énorme de l'estomac avec une large rupture au niveau de la petite courbure chez le patient et une dilatation importante de l'estomac avec une nécrose du fundus chez la patiente. Une suture de la petite courbure était réalisée chez le patient et une gastrectomie atypique fundique chez la patiente. Les suites opératoires étaient marquées par un décès chez le patient au deuxième jour post-opératoire et une sténose gastrique chez la patiente nécessitant une gastrectomie totale.La dilatation aiguë de l'estomac est une pathologie rare. Son diagnostic aux urgences est difficile car les signes ne sont pas spécifiques. Les formes compliquées donnent un tableau d'abdomen chirurgical aigu. L'exploration chirurgicale pose le diagnostic. Le traitement des complications va de la suture à la gastrectomie. La mortalité dans les formes compliquées est élevée.

Background: Despite the high success rate of radiofrequency catheter ablation (RFCA) in Wolff-Parkinson-White Syndrome (WPW), localizing the successful ablation site can be challenging and may require multiple radiofrequency (RF) applications., Objective: This study aims to evaluate the efficacy of a novel workflow for the automatic and precise identification of accessory pathway ablation site, named Delta Wave Automatic Mapping., Methods: Patients undergoing a first procedure for RF ablation of a manifest accessory pathway were included. Electro-Anatomical Mapping (EAM) was performed with the CARTO 3 system (Biosense Webster, Johnson & Johnson Medical S.p.a., Irvine, CA) leveraging auto-acquisition algorithms already present in the CARTO 3 software. Mapping and ablation were performed with an irrigated tip catheter with contact force sensor. Procedure success was defined as loss of pathway function after ablation. The number of RF applications required and time to effect were measured for each patient. Recurrences were evaluated during follow-up visits. Additionally, at the end of each procedure, historical predictors of ablation success were measured offline to evaluate their relationship with the successful ablation site found with the novel workflow., Results: A total of 50 patients were analysed (62% APs right and 38% APs left). All 50 APs were successfully eliminated in each procedure with a median Time-to-effect (TTE) of 2.0 (IQR 1.2-3.5) seconds. No AP recurrences during a median follow-up of 10 (IQR 6-13) months were recorded. Offline analysis of successful ablation site revealed a pre-ablation delta/ventricular interval of ≤-10 msec in 52% of the patients and in 100% of the patients the signal related to the Kent bundle was identified., Conclusions: The novel workflow efficiently localizes APs without requiring manual annotations. Historical endocardial parameters predicting success were measured offline for each case and they corresponded to the ablation target automatically annotated by the proposed workflow. Background: Two-dimensional speckle tracking evaluation (2D-STE) is a useful tool to evaluate the complexity of atrial function by the analysis of the different phases of atrial deformation and by the combination with Doppler measurements of diastolic function., Aim of the Study: To evaluate the role of the left atrial (LA) strain parameters to predict worsening chronic heart failure (CHF)., Methods: We enrolled outpatients affected by CHF referred to our heart failure unit. Each patient underwent a medical visit, an electrocardiogram (ECG), and an echocardiographic examination. LA function was assessed by 2D-STE. The three phases of LA strain, that is, the reservoir (LAr), the conduit (LAcd), and the contraction (LAct)-were evaluated. Moreover, the ratio between E and LAr (E/LAr) and those between LAr and septal (LAr/Ees), lateral (LAr/Eel), and septal-lateral (LAr/Eem) E/e' were measured. During follow-up, the events related to worsening of heart failure were evaluated., Results: Two hundred eleven patients were enrolled. During a mean follow-up of 14 ± 7 months, 37 patients showed at least one event related to heart failure worsening. At univariate Cox regression analysis, LAr, LAcd, LAct, E/LAr, LAr/Ees, LAr/Eel, and LAr/Eem were all associated with events related to heart failure worsening, but at multivariate regression analyses, only LAr (Hazard Ratio, HR: .95; 95% Confidence Interval, CI: .92-.99; p: .031), LAct (HR: 1.06; 95% CI: 1.01-1.12; p: .027), E/LAr (HR: 1.10; 95%CI: 1.0-1.16; p < .001), LAr/Ees (HR: .57; 95% CI: .37-.87; p: .010), and LAr/Eem (HR: .71; 95% CI: .53-.96; p: .026) remained significantly associated with the events. Finally, in a predictive model including the other relevant echocardiographic parameters LAr < 18%, LAct > -10.0%, LAr/Ees < 1.28, and E/LAr > 3.70 were associated with a statistically significant overall net reclassification improvement., Conclusions: In CHF patients, the measure of the LA reservoir and contraction by 2D-STE is independently associated with heart failure worsening, but the accuracy in predicting the events is even greater when the reservoir is combined with the Doppler measures of diastolic function., (© 2023 The Authors.
