Abstract

STUDY QUESTION Does BMI of gestational carriers (GCs) affect perinatal outcomes after embryo transfer? SUMMARY ANSWER Overweight and class I obesity in GCs does not affect the rate of good perinatal outcomes. WHAT IS KNOWN ALREADY The use of GCs is increasing, but uniform guidance regarding optimal BMI for GCs is lacking. Women with obesity who conceive without fertility treatment or through autologous or donor in vitro fertilization are at higher risk of adverse maternal and fetal outcomes, but data on obesity in GCs are very limited. STUDY DESIGN, SIZE, DURATION We performed a retrospective cohort study of 1121 GC cycles from January 2015 to December 2020 at US Fertility, the largest national partnership of fertility practices in the USA. PARTICIPANTS/MATERIALS, SETTING, AND METHODS All GC cycles performed at a large network of fertility practices were reviewed. Same-sex partners undergoing co-IVF were excluded. The primary outcome was good perinatal outcome from the first embryo transfer, defined as a singleton live birth at ≥37 weeks of gestation with birth weight between 2500 and 4000 g. Secondary outcome measures included frequencies of live birth, clinical pregnancy, miscarriage, full-term birth, low birth weight, large for gestational age, and cesarean delivery. A generalized linear model (log-binomial) was used for each to compare outcomes across BMI groups using normal BMI (20–24.9 kg/m2) as the reference group. Risk ratios and 95% CIs were estimated for each category group relative to normal BMI. MAIN RESULTS AND THE ROLE OF CHANCE We identified 1121 cycles in which GCs underwent first embryo transfer, of which 263 (23.5%) were in GCs with BMI >30. Demographics and reproductive history for GCs did not differ by BMI groups. The age of intended parents, use of frozen eggs, and fresh embryo transfers were higher with increasing BMI group. There were no statistically significant associations between BMI and good perinatal outcomes, live birth, clinical pregnancy, biochemical, spontaneous abortion, or low birth weight. However, among live births, higher BMI was significantly associated with birth by cesarean (P  = 0.015) and large for gestational age infants (P  = 0.023). LIMITATIONS, REASONS FOR CAUTION This was a retrospective study, and there may be unmeasured confounders. The number of patients with BMI <20 or ≥35 was small, limiting the power for these groups. We were not able to assess all maternal and fetal outcomes. WIDER IMPLICATIONS OF THE FINDINGS In this study, we did not identify any significant impact of BMI on the chances of having a good perinatal outcome. 3. Dimensional Tolerances: Back to the Basics

Abstract

Students often have difficulty grasping the principles of dimensional tolerances and frequently fail to recognize that dimensioning practice has a significant impact on the tolerance of part features. This observation may be attributed to several factors, not the least of which are changes in prior student education and life experiences and increasing pressure in academia to add course content to cover new technologies, sometimes at the expense of fundamental concepts. This paper presents some back-to-basics instructional methods designed to help students improve their understanding of tolerances, including a description of some hands-on instructional activities that were implemented in the Engineering Technology program at Illinois State University. Abstract

Genetic alterations in signaling pathways that control cell-cycle progression, apoptosis, and cell growth are common hallmarks of cancer, but the extent, mechanisms, and co-occurrence of alterations in these pathways differ between individual tumors and tumor types. Using mutations, copy-number changes, mRNA expression, gene fusions and DNA methylation in 9,125 tumors profiled by The Cancer Genome Atlas (TCGA), we analyzed the mechanisms and patterns of somatic alterations in ten canonical pathways: cell cycle, Hippo, Myc, Notch, Nrf2, PI-3-Kinase/Akt, RTK-RAS, TGFβ signaling, p53 and β-catenin/Wnt. We charted the detailed landscape of pathway alterations in 33 cancer types, stratified into 64 subtypes, and identified patterns of co-occurrence and mutual exclusivity. Eighty-nine percent of tumors had at least one driver alteration in these pathways, and 57% percent of tumors had at least one alteration potentially targetable by currently available drugs. Thirty percent of tumors had multiple targetable alterations, indicating opportunities for combination therapy. Abstract

GC-MS on the Viking 1976 Mars missions did not detect organic molecules on the Martian surface, even those expected from meteorite bombardment. This result suggested that the Martian regolith might hold a potent oxidant that converts all organic molecules to carbon dioxide rapidly relative to the rate at which they arrive. This conclusion is influencing the design of Mars missions. We reexamine this conclusion in light of what is known about the oxidation of organic compounds generally and the nature of organics likely to come to Mars via meteorite. We conclude that nonvolatile salts of benzenecarboxylic acids, and perhaps oxalic and acetic acid, should be metastable intermediates of meteoritic organics under oxidizing conditions. Salts of these organic acids would have been largely invisible to GC-MS. Experiments show that one of these, benzenehexacarboxylic acid (mellitic acid), is generated by oxidation of organic matter known to come to Mars, is rather stable to further oxidation, and would not have been easily detected by the Viking experiments. Approximately 2 kg of meteorite-derived mellitic acid may have been generated per m(2) of Martian surface over 3 billion years. How much remains depends on decomposition rates under Martian conditions. As available data do not require that the surface of Mars be very strongly oxidizing, some organic molecules might be found near the surface of Mars, perhaps in amounts sufficient to be a resource. Missions should seek these and recognize that these complicate the search for organics from entirely hypothetical Martian life.

Abstract

A recent asymptotics-based thermomechanical model is adapted and applied to the mould region in the continuous casting of round steel billets, with a view to describing the complex interplay between air-gap formation, mould taper, cooling channel width and cooling water velocity. Although the situation is steady state, the analysis leads to what is mathematically a dual moving-boundary problem for the solid–melt and solid–air interfaces, where the distance from the top of the mould region is the time-like variable in the problem. Moreover, the two interfaces are initiated at different locations. In addition, the thermal and mechanical problems are found to decouple and it is possible to solve the first ahead of the second. The model equations are solved numerically using a finite-difference method, and the approach is subsequently successfully validated against a previous finite-element model and experimental data from temperature measurements taken within the mould. It turns out that three different outcomes are possible: (I) the flux remains in molten state and no solid flux ever forms; (II) both molten and solid flux are present, and the profile of the oscillation mark is continuous with respect to the space variable in the casting direction; (III) both molten and solid flux are present, and the profile of the oscillation mark is discontinuous with respect to the space variable in the casting direction. Although (I) gave good agreement with experimental data, it suffered the drawback that solid flux is typically observed during actual continuous casting; this has been rectified in this work via alternative (II). On the other hand, alternative (III) can occur as a result of hysteresis-type phenomenon that is encountered in other flows that involve temperature-dependent viscosity; in the present case, this manifests itself via the possibility of multiple states for the oscillation-mark profile at the instants in time when solid flux begins to form and when it ceases to form. Abstract

Zipfian laws for frequency distributions of word pairs and longer phrases are derived from text sample analysis. From crossing of Zipfian curves, it is deduced that number of multi-word phrases that occur frequently in text is surprisingly small, of same order of magnitude as number of individual word-types. 24. Introduction: Gender, creativity and education in digital musics and sound art

Abstract

This special issue examines the politics of gender in relation to higher education, creative practices and historical processes in electronic music, computer music and sound art. The starting point is a summary of research findings on the student demographics associated with the burgeoning of music technology undergraduate degrees in Britain since the mid 1990s. The findings show a clear bifurcation: the demographics of students taking British music technology degrees, in comparison to traditional music degrees and the national average, are overwhelmingly male, from less advantaged social backgrounds, and slightly more ethnically diverse. At issue is the emergence of a highly (male) gendered digital music field. The special issue sets these findings into dialogue with papers by practitioners and scholars concerned with gender in relation to educational, creative and historical processes. Questions addressed include: What steps might be taken to redress gender inequalities in education, and in creative, compositional and curatorial practices? How can we combat the tendency to focus exclusively on the ‘problem of women’ while at the same time ignoring the challenges posed by the marked styles of masculinity evident in these fields? Is the gendering of electronic and digital musics and sound art evident in certain aesthetic directions? And what musical futures are augured by such imbalances?

Abstract

Outside In Acquisition and analysis of large data sets promises to move us toward a greater understanding of the mechanisms by which biological systems are dynamically regulated to respond to external cues. Now, two papers explore the responses of a bacterium to changing nutritional conditions (see the Perspective by Chalancon et al. ). Nicolas et al. (p. 1103 ) measured transcriptional regulation for more than 100 different conditions. Greater amounts of antisense RNA were generated than expected and appeared to be produced by alternative RNA polymerase targeting subunits called sigma factors. One transition, from malate to glucose as the primary nutrient, was studied in more detail by Buescher et al. (p. 1099 ) who monitored RNA abundance, promoter activity in live cells, protein abundance, and absolute concentrations of intracellular and extracellular metabolites. In this case, the bacteria responded rapidly and largely without transcriptional changes to life on malate, but only slowly adapted to use glucose, a shift that required changes in nearly half the transcription network. These data offer an initial understanding of why certain regulatory strategies may be favored during evolution of dynamic control systems.

Abstract

We present Very Large Array observations of ammonia (NH3) (1,1), (2,2), and CCS (2_1-1_0) emission toward the Infrared Dark Cloud (IRDC) G19.30+0.07 at ~22GHz. The NH3 emission closely follows the 8 micron extinction. The NH3 (1,1) and (2,2) lines provide diagnostics of the temperature and density structure within the IRDC, with typical rotation temperatures of ~10 to 20K and NH3 column densities of ~10^15 cm^-2. The estimated total mass of G19.30+0.07 is ~1130 Msun. The cloud comprises four compact NH3 clumps of mass ~30 to 160 Msun. Two coincide with 24 micron emission, indicating heating by protostars, and show evidence of outflow in the NH3 emission. We report a water maser associated with a third clump; the fourth clump is apparently starless. A non-detection of 8.4GHz emission suggests that the IRDC contains no bright HII regions, and places a limit on the spectral type of an embedded ZAMS star to early-B or later. From the NH3 emission we find G19.30+0.07 is composed of three distinct velocity components, or "subclouds." One velocity component contains the two 24 micron sources and the starless clump, another contains the clump with the water maser, while the third velocity component is diffuse, with no significant high-density peaks. The spatial distribution of NH3 and CCS emission from G19.30+0.07 is highly anti-correlated, with the NH3 predominantly in the high-density clumps, and the CCS tracing lower-density envelopes around those clumps. This spatial distribution is consistent with theories of evolution for chemically young low-mass cores, in which CCS has not yet been processed to other species and/or depleted in high-density regions., 29 pages, 9 figures, accepted for publication by ApJ. 37. Evidence-based Assessment of Coping and Stress in Pediatric Psychology

Abstract

Objective To review selected measures of stress and coping in pediatric populations. Stress and coping are presented within a risk and resiliency framework. Methods The Society of Pediatric Psychology (SPP) surveyed the membership to identify the most frequently used assessment instruments. Twelve measures of coping and three measures of stress were reviewed. These instruments were evaluated using the Stress and Coping workgroup's modification of the criteria developed by the SPP Assessment Task Force (SPP-ATF). Results One of the three measures of stress and five of the 12 measures of coping were Well-established measures that broaden understanding. Additionally, one of the coping measures was categorized as a Well-established measure that guides treatment. Merits of the individual measures are discussed. Conclusions Recommendations for future research are provided, including suggestions for the construction and use of measures to inform treatment research.
