1. An ambivalent prostate nodule after Bacillus Calmette-Guérin therapy.

Abstract

A 65-year-old patient without specific associated pathology was treated for a high-grade non-invasive papillary urothelial carcinoma by surgery associated with repeated intravesical Bacillus Calmette-Guérin (BCG) instillations. During follow-up, magnetic resonance imaging (MRI) found a clinically indurated prostate nodule with suspected extensive capsular invasion. Prostatic biopsies showed epithelioid and giant-cell granuloma associated with a single focus of adenocarcinoma. Urinary culture test and specific PCR confirmed the involvement of Mycobacterium bovis . The patient was treated first by rifampin, isoniazid and ethambutol and then by rifampin and isoniazid for a total duration of 9 months, with MRI reassessment at various intervals. After BCG therapy, systemic infectious complications but also local complications such as granulomatous disease have been reported, but prostatic abscesses with M. bovis mimicking cancer on MRI are rare. Consequently, we advise specific local urinary and prostate samples to test for mycobacteria (staining, culture, PCR) in order to avoid aggressive high-risk prostatic surgery., (© 2021 The Authors.)

Abstract

We report an exceptional case of renal cell carcinoma (RCC) associated with ALK translocation in an adult. The tumor was located in the left kidney and measured 4cm. The tumor was composed of sheets of large eosinophilic cells with frequently intracytoplasmic vacuoles. The nuclei were large with a nucleolar grade 3. At immunohistochemistry, tumor cells were diffusely positive for PAX8 and vimentin and focally stained with CK7. ALK immunostaining was diffuse and fluorescent in situ hybridization showed a rearrangement of ALK in numerous tumor cells. To date, less than 17 cases of RCCs with ALK rearrangements are reported in children and adults. The prognosis remains uncertain but ALK rearrangement may be of potential value for target therapy with crizotinib., (Copyright © 2016 Elsevier GmbH. All rights reserved.)

Abstract

Sarcoidosis malignancy syndrome is a rare phenomenon which remains controversial. We report here the case of a 46-year-old woman presenting with multisystem sarcoidosis 12 months after the completion of combined treatment for stage III squamous cell carcinoma of the uterine cervix; at the time she was still in complete remission of the tumor. The outcome was rapidly favorable under oral corticosteroid therapy. The time interval between the two illnesses as well as patient's age strongly suggest a relationship. Possible pathophysiologic mechanisms and the literature regarding uterine tumors are briefly reviewed.

Abstract

Subpleural emphysema induction was attempted by injecting elastase intrapleurally into rats and cats. Microscopic evaluation of emphysema (alveolar dilation and septal rupture) was compared with a quantification of alveolar dilation (Fawell-Newman method). The emphysema found in animals treated with an intrapleural injection of porcine pancreatic elastase did not appear to be specifically located in a subpleural zone, and was identical to the focal emphysema induced by an enzyme instillation into the airways. It should be noted that a placebo instillation into the feline airways sometimes induced alveolar dilation without septal disruption presumably by causing bacterial contamination. Cats were seen to be very sensitive to pleural stress since this enzyme injection produced an alveolar dilation (increased mean intercept) as significant as that in cats treated by an intrabronchial instillation of elastase (P less than 0.001).
