Objectives: Increased epicardial adipose tissue (EAT) thickness is a risk factor for cardiovascular diseases. Previous studies have demonstrated that EAT thickness is increased in patients with hypertension compared with normotensive individuals. In the current study, we aimed to evaluate whether echocardiographically measured EAT thickness differs among patients with normotension, prehypertension, hypertension, and the relation between EAT thickness and blood pressure levels in prehypertensives. Study design: Patients with prehypertension (n=50) and hypertension (n=50) and normotensive healthy subjects (n=50) according to the American Hypertension Guidelines (Joint National Committee 7) were enrolled in the study. All participants underwent transthoracic echocardiographic examination. EAT thickness was measured from the parasternal long-axis view at end-systole. Angiosarcoma, the most common primary malignant neoplasm of the heart in adults, usually presents as pericardial effusion or right-sided heart failure. Rupture of an angiosarcoma-infiltrated cardiac chamber as a cause of hemothorax is very rare in the literature. In this report, we describe a 34-year-old male patient, who presented to emergency service with sudden chest pain and dyspnea. The diagnostic work-up revealed spontaneous right-sided hemothorax and a large right atrial (RA) mass with suspicious atrial perforation. An urgent surgery showed a vascularized irregular RA mass invading the parietal pericardium and pleura and a perforation of the RA free wall. Histopathologic examination confirmed the diagnosis of angiosarcoma, and the patient was subsequently referred for radiotherapy and chemotherapy.

Introduction: Coronary angiography is a principle diagnostic and therapeutic procedure in modern cardiology practice. The transradial access for cardiac catheterization has overtaken the transfemoral approach because of many advantages. However, some patients suffer radial pain after the procedure. Unfortunately, this complication has been poorly evaluated in previous studies. The present study aimed to determine the prevalence of radial pain after transradial coronary angiography and investigate factors that influence post-procedural pain. Methods: This is a cross-sectional study in which a total of 100 consecutive patients who underwent elective transradial coronary angiography ± percutaneous coronary intervention between January 2015–2016 were evaluated. The patients were asked about presence of disturbing pain in the forearm especially wrist region after the procedure. Verbal rating scale was used to evaluate pain assessment. The patients were divided as early pain group (two hours after the procedure) and prolonged pain group (one month after the procedure) and analysis was performed. Results: A total of 55 patients suffered from pain in the early phase (after two hours), and 26 of patients had prolonged pain at one month after the radial intervention. Independent pain predictors in the early pain group were male operator (p = 0.004, OR = 3.386, 95% CI: 1.484–7.725) and experience of operator (OR = 4.147, 95% CI: 1.637–10.506, p = 0.003). On the other hand, the younger age of patients (OR = 0.955, 95% CI: 0.915–0.966, p = 0.032) and experience of operator (OR = 3.947, 95% CI: 1.547–10.047, p = 0.004) were the independent predictors of prolonged radial pain. Conclusion: Pain after radial coronary angiography is not uncommon. Experience, operator gender, and age of the patients were independent predictors of pain after transradial coronary angiography.

Objectives: In this study, we evaluated aortic stiffnessand echocardiographic and laboratory factors affectingaortic stiffness in patients with metabolic syndrome(MetS).Materials and methods: Forty-six patients (25 male,mean age 47.3±6.5 years) with the diagnosis of MetS accordingto the Adult Treatment Panel III Final Report criteriawere included. Forty-four age and gender matchedhealthy subjects (18 male, mean age 46.0±6.1 years)were recruited as the control group. Aortic strain, distensibilityand stiffness index were calculated by M-modeechocardiography and diastolic parameters were measured.Results: Left ventricular mass index (LVMI), decelerationtime (DT), isovolumic relaxation time (IVRT) wereincreased and mitral E/A ratio was decreased in patientswith MetS compared to controls. Purpose – This study aims to better understand the phenomenon of travel itch by revealing the travel motives and travel attributes of “travel itchers” within the framework of motivational theories. Design/methodology/approach – This is an exploratory study. An online questionnaire was used to collect data from individuals who have first-hand experience of travel itch (N: 30). Data were analysed using content analysis within the framework of motivational theories. Findings – This exploratory study finds that travel itch means a desire to travel and findings revealed that the motives of travel itchers are novelty, escape, host-site involvement, self-actualization, stimulation in line with the Travel Career Pattern (TCP) except travel itself. Travel itself is both one of the main motives and coping methods that distinguish those from other travellers. For coping methods, itchers do travel-related activities apart from travel itself. Nonetheless, travel itch is a recurrent cycle even after having a holiday. Consequently, an attempt was made to redefine the concept of ‘travel itch’ in light of all these findings. Originality of the research – Travel itch is an interesting concept newly introduced into tourism literature. And in order to obtain data this study puts the travel itchers themselves who use this concept. Since this is one of the first studies to examine the concept of travel itch, this study can be a guide for future research and place the concept in the context of travel motivation.

Aim: Nonalcoholic fatty liver disease (NAFLD) is the most common liver disorder which is reported as the hepatic manifestation of metabolic syndrome with an increased risk of cardiovascular events. Patients with NAFLD are also at risk of future cardiac events independently of metabolic syndrome. The aim of this study was to examine serum concentrations of heart type fatty acid binding protein (H-FABP) in NAFLD and to investigate its correlations with metabolic parameters and subclinical atherosclerosis. Patients and methods: A total of 34 patients with NAFLD and 35 healthy subjects were enrolled in the study. NAFLD patients had elevated liver enzymes and steatosis graded on ultrasonography. Healthy subjects had normal liver enzymes and no steatosis on ultrasonography. H-FABP levels were measured using an enzyme linked immunosorbent assay (ELISA) method and correlations with metabolic parameters and subclinical atherosclerosis were examined. Subclinical atherosclerosis was determined with carotid artery intima-media thickness (CIMT) which was measured by high resolution B mode ultrasonography. Results: H-FABP levels were elevated in patients with NAFLD (16.3 ± 4.0 ng/ml) when compared with healthy controls (13.8 ± 2.1 ng/ml; p < 0.001). NAFLD patients had significantly higher CIMT than the controls had (0.64 ± 0.17 mm vs. 0.43 ± 0.14 mm, p = 0.009). The H-FABP concentrations were significantly positively correlated with body mass index (r = 0.255, p = 0.042), fasting blood glucose level (r = 0.300, p = 0.013), CIMT (r = 0.335, p = 0.043), and homeostasis model assessment-estimated insulin resistance (HOMA-IR; r = 0.156, p = 0.306). In multiple linear regression analysis, H-FABP levels were only independently associated with CIMT (p = 0.04) Conclusion: Serum H-FABP concentrations increase in patients with NAFLD. Our results may not only suggest that H-FABP is a marker of subclinical myocardial damage in patients with NAFLD but also of subclinical atherosclerosis, independent of metabolic syndrome and cardiac risk factors. © 2013 Urban & Vogel.

Mühendislik jeolojisinde tasarıma yönelik ön çalışmalarda zeminlerin sınıflandırılması önemli bir aşamadır. Bir zemine atanabilecek birkaç harf ya da sayı uygulayıcı mühendislere çok hızlı olarak bir fikir sağlamaktadır. Ulusal ve uluslararası zemin sınıflama sistemlerinde ince taneli zeminlerin sınıflandırılması plastisite abağı ile yapılmaktadır. Ancak, likit limit ve plastik limit deneylerini etkileyen bazı faktörlere bağlı olarak zemin sınıfının plastisite abağı ile belirlenmesi sırasında belirsizlikler oluşmaktadır. Ayrıca, tanımlama noktalarının kimi zaman sınıfları ayıran çizgiler üzerine veya yakınına düşmesi durumunda belirsizlik seviyesi artmaktadır. Bu çalışmada söz konusu belirsizlikleri en aza indirmek için daha önce geliştirilen bir bulanık sınıflama sürecinin uygulaması yapılmıştır. Söz konusu bulanık sınıflama sürecinde plastisite abağındaki tanımlama noktalarının sınırları ayıran çizgilere olan dik mekansal mesafeleri dikkate alınmıştır. Sigmoid fonk siyonu kullanılarak tanımlama noktalarının bulanık plastisite ve bulanık zemin değerleri hesaplanmıştır. Son olarak bulanık işlemciler yardımı ile bulanık zemin sınıfları elde edilmiştir. Önerilen bulanık sınıflama süreci 100 adet laboratuvar verisi üzerinde uygulanmış ve elde edilen bulanık zemin sınıfları klasik zemin sınıfları ile karşılaştırılmıştır.

Bu çalışmanın amacı, canlılık ve öğrenme boyutlarını içeren, işte gelişim olgusunu ölçmek amacıyla Porath vd.,’nin (2012) geliştirdiği İşte Gelişim (Thriving at Work) ölçeğini Türkçeye uyarlamaktır. Bu amaçla üç ayrı örneklem grubundan (akademisyen, turizm çalışanı ve öğretmen) toplam 455 anket toplanmıştır. Ölçek maddelerinin dil geçerliliğini sağlamak amacıyla maddeler, ilk aşamada beş akademisyen tarafından İngilizce’den Türkçe’ye sonraki aşamada aralarında iki dilli (bilingual) uzmanların da bulunduğu farklı beş akademisyen tarafından Türkçe’den İngilizce’ye çevrilmiştir. Dil geçerliliği sağlanan ölçeğe ardından güvenilirlik ve geçerlilik analizleri uygulanmıştır. Yapı geçerliliğini test etmek için uygulanan açıklayıcı faktör analizi (AFA) sonuçlarına göre, açıklanan varyansı %57,943 olan ölçeğin, orijinal yapılanmasına uygun şekilde öğrenme ve canlılık boyutları olmak üzere iki faktörlü bir yapıya sahip olduğu tespit edilmiştir. Elde edilen yapının doğrulayıcı faktör analizi (DFA) ile model uyumu test edilmiştir. Buna göre iki faktörlü işte gelişim ölçeğinin (X2/sd= 3,86; RMSEA=0,080; SRMR= 0.05, GFI= 0.95, NFI= 0.96, RFI= 0.94, CFI= 0.97 ve IFI= 0.97) iyi uyuma sahip olduğu ve modelin verilerle doğrulandığı görülmüştür. 11. Do young women with polycystic ovary syndrome show early evidence of preclinical coronary artery disease?

BACKGROUND: It is thought that women with polycystic ovary syndrome (PCOS) are at increased risk of developing cardiovascular diseases. METHODS: In this study, we used transthoracic echocardiography to measure coronary flow reserve (CFR) in 28 women with PCOS and in 26 healthy women. RESULTS: The PCOS and the control groups were similar in terms of age (27.1 ± 4.5 versus 28.8 ± 4.4 years) and BMI (26.6 ± 5.7 versus 24.7 ± 4.4 kg/m2). Fasting insulin levels and homeostasis model assessment insulin resistance index were higher in the PCOS group. LH, the LH/FSH ratio, total testosterone, free testosterone and androstenedione were higher in the PCOS group. FSH, estradiol, prolactin, progesterone, cholesterol, triglyceride and high-sensitive C-reactive protein were similar between the two groups, but homocysteine levels were higher in the PCOS group. Baseline diastolic peak f low velocity (DPFV) (25.0 ± 4.6 versus 23.3 ± 2.7 cm/s, P > 0.05), hyperaemic DPFV (71.2 ± 12.8 versus 73.0 ± 12.9 cm/s, P > 0.05) and CFR (2.8 ± 0.8 versus 3.2 ± 0.8 cm/s, P > 0.05) of the left anterior descending coronary artery were similar between the two groups. CONCLUSION: We conclude that in young women with PCOS and without cardiovascular risk factors, CFR is preserved. [ABSTRACT FROM AUTHOR]
