This article discusses the dermoscopic findings of pseudomyogenic hemangioendothelioma (PHE), a rarely metastasizing vascular tumor. The case report describes a 45-year-old man with PHE on his back, presenting as two painful lesions with unclear borders and ulcerated surfaces. Dermoscopic features included a red background, bright yellow structureless areas, scattered scales, white radial streaks, brown streaks in a "polygonal" shape, thick arborizing vessels, and clusters of small serpentine vessels. The article also provides information on the clinical features, histopathological characteristics, and treatment options for PHE. This study is the first to report on the dermoscopic features of PHE.
