Rehabilitating thin jaws without reconstructive surgery entails using narrow implants. The proposed treatment adopted an innovative implant system, allowing the mini-screws to be parallel and immediately loaded. A mandible, wearing an overdenture, was functionalized contextually to the residual dental extraction and the placement of six 2.4-mm thick one-piece implants. Low-profile intermediate abutments, the LEMs, able to rotate over the spherical heads of the fixtures, were connected after suturing, oriented, and blocked in a mutual parallel position. The copings, engaging with a tapered juncture of the LEMs, resulted in their alignment to be intraorally wedged together. The provisional superstructure enclosed the copings and was immediately connected to the implants, and the definitive prosthesis was delivered after three months. No clinical signs of peri-implantitis or radiographically evident bone loss were recorded after a two-year follow-up without any prosthetic complication. Aim: The aim of this in vitro study is to present a debonding protocol developed to remove a screw-retained, monolithic, zirconia restoration from its titanium-base abutment, and to microscopically evaluate the abutment integrity at both the prosthetic and connection levels. Materials and Methods: A total of 30 samples were tested. Each sample consisted of a monolithic zirconia restoration bonded on a titanium link abutment. Five different shapes were designed and fabricated. Randomly, one-third of the Ti-link abutments were subjected to an anodizing process. Then, all the zirconia samples were bonded to the Ti-link abutments according to a pre-established protocol. Forty-eight hours later, the samples were debonded according to the experimental protocol. The outcomes were evaluated by a visual inspection with an optical microscope, scanning electron microscopy (SEM), and chemical composition analysis. Results: Thirty samples were collected and visually analyzed. Seven samples were randomly evaluated via scanning electron microscopy. In all the examinations, no relevant changes were reported. Chemical composition analysis also relieved no changes in the chemical structure of the titanium. Conclusions: The titanium-base abutments do not alter the structure and properties of the material, not creating phase changes or the birth of oxides such as to induce fragility. Further clinical studies with longer follow-up periods are needed to confirm these preliminary results.

Fibrous ridges on the edentulous maxillary arch are commonly found in combination syndrome or due to ill-fitting dentures. Often, these cases are managed conservatively using modified impression techniques to achieve better support and peripheral seal without displacing the movable tissue. Many impression techniques were proposed and justified with their respective ideologies, but some may complicate both the clinical and laboratory procedures. In this report, two simplified techniques are demonstrated to make an impression of the maxillary arch with fibrous ridges. Laboratory steps in custom tray fabrication are also emphasised for a successful and predictable impression. [ABSTRACT FROM AUTHOR]

Purpose: To determine the survival rate, incidence of prosthetic complications, and patient satisfaction of implant fixed complete dental prostheses (IFCDPs) after a mean observation period of 1.4 years. Materials and methods: Twenty‐eight (28) eligible participants were recruited according to specific inclusion and exclusion criteria. The definitive metal‐acrylic resin IFCDPs consisted of titanium bars veneered with acrylic resin and acrylic denture teeth. Prosthodontic complications, divided into major and minor, were monitored. Parameters such as gender, jaw location, bruxism, and occlusal scheme were evaluated. Moreover, a questionnaire was administered throughout the study to assess patient satisfaction. Poisson regression as well as repeated measures ANOVA were used for statistical analysis. Results: Fourteen (14) males and 14 females were enrolled and followed‐up at 3, 6, and 12 months. All IFCDPs survived (100% survival rate). The most frequent minor complication was the loss of material used to close the screw access hole (20% out of total complications). The most frequent major complication was chipping of the acrylic denture teeth (77.14% out of total complications). Gender (p = 0.008) and bruxism (p = 0.030) were significant predictors for the total major complications (major wear and major chipping) while occlusal scheme was a significant predictor for major chipping events (p = 0.030). Conclusions: While IFCDPs demonstrated high prosthetic survival rates, they also exhibited a high number of chipping events of the acrylic veneering material, especially in males, bruxers, and individuals with canine guidance occlusion. Purpose: To analyze peri-implant bone loss around two types of tissue-level implants (convergent and divergent transmucosal morphology) placed in the same region of either the maxilla or mandible in a single surgical session. Materials and Methods: This prospective study included 21 patients who each received two implants, one with a convergent transmucosal collar (n = 21) and the other with a divergent collar (n = 21). Implants were placed in a single surgical session, by the same clinician, in the same clinical setting at the Prosthodontic and Occlusion Unit, University of Valencia (Spain). The implants (n = 42) were restored with splinted crowns screwed directly onto the prosthetic platforms. Bone loss analysis consisted of measurements taken from periapical radiographs 24 months after loading. Statistical analysis evaluated the homogeneity of the groups using Pearson's chi-square test; bone loss was compared with the Wilcoxon test. Statistical significance was set at 5% (α = .05), with a confidence interval of 95% and a power of 57%. Results: Implants with convergent transmucosal morphology presented less mean peri-implant bone loss (0.29 ± 0.34 mm) than those with divergent transmucosal morphology (0.60 ± 0.63 mm), with a statistically significant difference between the two types of implants (P = .031). Abstract Background This study aimed to compare retention and fracture load in endocrowns made from translucent zirconia and zirconium lithium silicate. Methods Fifty-six intact human maxillary molars after being mounted in acrylic resin, were scanned to acquire biogeneric copies. Specimens underwent standard endodontic treatment and were prepared for endocrown up to 2 mm above the cementoenamel junction. The specimens were randomly divided into two groups of 28, and endocrowns were designed using biogeneric copies and milled from high-translucent zirconia disks (Zr) and zirconium lithium silicate blocks (ZLS). After cementation with dual-cure resin cement, all the specimens underwent thermomechanical aging, and pull-out retention test and compressive test were conducted (14 specimens were used for each test in each group, n = 14), and failure modes in both tests were evaluated. Results Independent samples t-test showed significant difference between the retention of Zr (271.5 N ± 114.31) and ZLS (654.67 N ± 223.17) groups (p value = 0.012). Compressive test results were also significantly different between Zr (7395.07 N ± 1947.42) and ZLS (1618.3 N ± 585) (p = 0.002). Failure mode of retention test was primarily adhesive failure at the cement-restoration interface in Zr group and cement-tooth interface in ZLS group. Failure modes of fracture test for Zr group were 7 non-restorable fractures and one restorable fracture while 6 specimens resisted compressive loads up to 8500 N without fracture. ZLS group showed 7 restorable and 7 non-restorable failures. Conclusions Zr endocrowns showed significantly lower retention and higher fracture strength. Both materials seem to be suitable for fabrication of endocrown in clinical setup.

Statement of the Problem: Poly-etheretherketone is a novel material used in the construction of the removable partial dentures frameworks instead of the metal frameworks. This material can be fabricated by various techniques. Most common methods are the injection molding or Computer Aided Design/Computer Aiding Manufacturing (CAD/CAM) milling techniques. The fabrication technique may affect the adaptation of the frameworks by influencing the retention. Purpose: To assess the effect of the processing techniques of high performance Poly-etheretherketone either by injection molding (pressing) or CAD/CAM milling techniques on removable partial denture frameworks retention for rehabilitation of upper class I Kennedy classification. Materials and Method: This in vitro study was performed on one epoxy resin model representing the partially edentulous maxillary arch with natural teeth extending from first premolar to first premolar. First premolars and canines were reduced to receive porcelain fused to metal crowns with 0.50mm mesio-buccal retentive undercuts, distal guiding planes and mesial occlusal rest seat on first premolars and cingulum rest seat on canines. Considering the construction technique of frameworks, twenty samples were divided into two groups. In the group I, ten frameworks were fabricated by injection molding, and in the group II, ten frameworks were fabricated by CAD/CAM. The removal and insertion was carried out at 120, 720 and 1440 cycles for both groups, respectively. The retention values were measured by using Universal Testing Machine before cycling and after each interval. Results: Independent t-test showed significant difference on retention at different simulation cycles between groups. Group II exhibited significantly less retention than group I (p< 0.001), while comparing the retention at different cycles within each group by paired sample t-test exhibited significant decrease of retention till the end of the cycling (p< 0.001). Conclusion: From the retention point of view, high performance poly-etheretherketone frameworks fabricated by injection molding technique provided a promising method over CAD/CAM technique milling method.

Background: To verify the influence of macrogeometry with healing chambers on the osseointegration of dental implants by analyzing implant stability quotient (ISQ) and evaluate the correlation between insertion torque and ISQ insertion with different macrogeometries., Methods: In total, 26 implants were installed in the posterior mandible of eight patients with sufficient bone height for the installation of implants measuring 3.5 mm in diameter and 9.0 mm in length. The implants were categorized according to two types of macrogeometry: a test group (GT) with 13 conical implants with healing chambers and a control group (GC) with 13 conical implants with conventional threads. To insert the implants, a bone drilling protocol was used up to a diameter of 3 mm with the last helical bur. The insertion torque of the implants was evaluated, followed by the measurement of ISQ at 0 (T-0), 7 (T-7), 14 (T-14), 21 (T-21), 28 (T-28), and 42 (T-42) days., Results: The mean insertion torque was 43 Ncm in both groups, without a significant difference. Moreover, no significant difference in the ISQ values was found between the groups at different time points (p > 0.05), except at T-7 (GT = 69.87±1.89 and GC = 66.48±4.49; p = 0.01). Although there was no significant difference, ISQ median values were higher in the GT group than GC group at 28 days (GT = 67.98 and GC = 63.46; p = 0.05) and 42 days (GT = 66.12 and GC = 60.33; p = 0.09). No correlation was found between the insertion torque and ISQ insertion (p > 0.05)., Conclusion: Furthermore, implants with a 3.5 mm diameter macrogeometry, with or without healing chambers, inserted with a drilling protocol up to 3 mm in diameter of the last helical bur, led to a similar secondary stability, with no difference in ISQ values. Although, implants with healing chamber demonstrates ascending values in the graph of ISQ, having a trend of faster osseointegration than implants without healing chambers. Background: Implant-retained obturators for maxillectomy cases have several advantages over traditional obturators but prosthetic design for specific conditions after maxillary resection has several challenges and the appropriate implant placement configuration is essential for improving retention and the stability of the implant-retained obturator., Objectives: The present study aimed to assess the retention force of using linear and nonlinear implant placement configurations using ball and socket attachment in implant-retained obturators at the initial retention and after simulation of six months of use., Materials and Methods: Two identical epoxy resin maxillary models of a completely edentulous unilateral maxillary defect (Brown's class IIb) were used for implant placement, in the first model three implants were arranged with linear placement configuration, and in the second model three implants were arranged in nonlinear placement configuration. For proper sample sizing, 26 models and obturator were used. Two equal groups of obturators (13 for each group) were constructed, each with a different implant placement configuration. Both groups used the same attachment design (a non-splinted ball attachment). Using a cyclic loading machine that served as a dental insertion and removal simulator, each study group was subjected to 500 tension-compression cycles simulating 6 months of use. Using the universal testing machine, each obturator was removed at a speed of 50 mm/min for the crosshead. peak load to dislodgement was measured at the initial retention and after the simulations of six months of use. Data were analyzed using independent and paired t-tests while percent change was analyzed using the Mann Whitney U test., Results: There were a statistically significant differences in retention between the nonlinear implant placement configuration for Brown's class IIb maxillectomy and the linear implant placement configuration at initial retention evaluation with p-value of < 0.0001 and after simulation of six months of usage with p-value of < 0.0001 Also, after simulation of 6 months of use group I lose - 24.87 (10.16) % of its retention while group II lose - 17.49 (7.78) %., Conclusions: Non-linear implant placement is more retentive at the initial retention and after simulation of six months of use than linear and loses less retention after usage., (© 2024. Purpose: To investigate how well an implant stability quotient (ISQ) represents resonance frequency., Materials and Methods: Benchtop experiments on standardized samples that replicated a mandibular premolar site were conducted to correlate an ISQ value and a resonance frequency to synthetic bone density and an incremental insertion torque; then, a frequency spectrum analysis was performed to check the validity of the resonance frequency analysis (RFA). Brånemark Mk III implants (4 × 11.5 mm; Nobel Biocare) were placed in Sawbones test models of five different densities (40, 30, 40/20, 20, and 15 PCF). An incremental insertion torque was recorded during implant placement. To perform stability measurements, the test models were partially clamped in a vise (unclamped volume: 10 × 20 × 34 mm). A MulTipeg (Integration Diagnostics) was attached to the implants, and a Penguin (Integration Diagnostics) RFA measured ISQ. Simultaneously, the MulTipeg motion was monitored via a laser Doppler vibrometer and processed by a spectrum analyzer to obtain the resonance frequency. Tightness of the clamp was adjusted to vary the resonance frequency. A statistical analysis produced a linear correlation coefficient (R) among the measured ISQ, resonance frequency, and incremental insertion torque., Results: The resonance frequency had high correlation to the incremental insertion torque (R &#61; 0.978), confirming the validity of using RFA for this study. Measured ISQ data were scattered and had low correlation to the resonance frequency (R &#61; 0.214) and the incremental insertion torque (R &#61; 0.386). The spectrum analysis revealed the simultaneous presence of multiple resonance frequencies., Conclusions: For the designed benchtop tests, resonance frequency does indicate implant stability in view of Sawbones density and incremental insertion torque. However, ISQ measurements do not correlate to the resonance frequency and may not reflect the stability when multiple resonance frequencies are present simultaneously.

Purpose: To evaluate the correspondence between output from a new artificial intelligence tool (AIT) and clinician evaluation regarding the immediate loading suitability of dental implants based on insertion torque curves recorded during implant placement in an in vitro test. The secondary aim was to analyze peak insertion torque (PIT) and variable torque work (VTW) values of the implants., Materials and Methods: The study was performed with four different densities of artificial bone blocks of solid rigid polyurethane without a cortical layer. Five types of implants with different macrogeometries were used. A total of 140 implants (7 implants of each type in the four polyurethane blocks) were inserted. Immediately after implant placement, the insertion torque curves were classified by the operator as suitable (S) or nonsuitable (NS) for immediate loading. The same curves were then analyzed by the new AIT, which classified them as belonging to the 'YES' or 'NO' class. For each implant, PIT and VTW values were also recorded., Results: The correspondence between clinician and AIT evaluation was 99.3%, with only one false negative reported by the algorithm analysis. The AIT was found to have a sensitivity of 98.95%, specificity of 100%, positive predictive value of 100%, and negative predictive value of 97.8%. Mean PIT of the whole sample was 34.19 ± 19.43 Ncm, while mean VTW was 2,266.89 ± 1,993.73 Ncm. Statistically significant differences were found between implant systems in the whole sample and according to density of the polyurethane block., Conclusions: The AIT showed a high level of accuracy in the prediction of immediate loading suitability of dental implants based on the provided insertion torque curves. All the implants used in the in vitro test achieved good levels of primary stability, except when inserted in the least-dense polyurethane block. Clinical studies conducted with larger samples and more clinicians are necessary to confirm these results.

Purpose: To develop the most compatible cementation protocol for ensuring minimal residual cement and optimal retention of cement-retained implant-supported fixed dental prostheses., Materials and Methods: A total of 30 custom implant abutments and zirconia crowns with bilateral wings were prepared. Three cement types were used for cementation: noneugenol resin cement (Premier Implant Cement; Group IC), dual-polymerizing self-adhesive resin cement (SmartCem2; Group SC), and zinc oxide eugenol cement (Temp-Bond; Group TB; n &#61; 30 per group). Three cementation methods were applied for each cement type, and the samples were divided into subgroups: (1) cement was injected using a graduated syringe (ICN, SC-N, and TB-N); (2) a cementa/on jig made with a silicone impression material and temporary resin material was used (IC-CJ, SC-CJ, and TB-CJ); (3) 3D-printed abutments were used as replicas for cementation (IC-3DP, SC-3DP, and TB-3DP). The amount of cement injected, surface area of the residual cement, and retentive strength were measured. Kruskal- Wallis and post-hoc Mann-Whitney tests were used for statistical analyses., Results: Excess cement was not observed when cementa/on jig or 3D-printed replicas were used. For IC and SC subgroups, non-use of these auxiliary tools resulted in significantly higher amounts of injected cement. The retentive strength differed significantly among the IC subgroups but not among the SC subgroups. The retentive strength of subgroups TB-N and TB-CJ was significantly higher than that of subgroup TB-3DP., Conclusions: To prolong the main purpose of each cement type, a cementation jig or 3D-printed replica is highly recommended regardless of the cement type.

Background: Effect of aging on tissue adaptability and retention of digital obturator is still under investigation., Methods: A maxillary Armany (class I) epoxy reference model was scanned to fabricate digital obturator fabricated from milled Co-Cr framework and 3D printed bulb. A color map of the scanned reference and digital obturator was made using Geomagic software to evaluate the accuracy of fit before and after cyclic loading using ROBOTA chewing simulator at 37,500, 75,000 and 150,000 cycles to simulate clinically 3-, 6- and 12-months chewing condition. Insertion-removal condition simulating the placement and removal of the obturator was done using repeated 360, 720 and 1440 cycles and retention was evaluated before and after the repeated cycles. Data were collected, tabulated and statistically analyzed using Statistical Package for Social Sciences (IBM SPSS Statistics 26). Student t-test and multi variable ANOVA test were used to detect significance. P-value < 0.05 was considered significant difference., Results: For retention test: There was a significant difference between baseline and 3, 6 and12 months. For the tissue surface adaptation test: There was significant difference at all measured areas (P-value < 0.05) before and after application of load., Conclusion: digitally designed and fabricated obturator was highly retentive and has excellent tissue surface adaptation upon fabrication, After application of load; reduction of retention and lack of tissue adaptation were resulted. THE CLINICAL IMPLICATION: of this manuscript is that digital obturator can be used successfully with the shortcomings of loosening retention and adaptation afterwhile. Objectives: To determine the correlation between the primary implant stability quotient and the implant percussion sound frequency., Materials and Methods: A total of 14 pigs' ribs were scanned using a dental cone beam computed tomography (CBCT) scanner to classify the bone specimens into three distinct bone density Hounsfield units (HU) value categories: D1 bone: >1250 HU; D2: 850-1250 HU; D3: <850 HU. Then, 96 implants were inserted: 32 implants in D1 bone, 32 implants in D2 bone, and 32 implants in D3 bone. The primary implant stability quotient (ISQ) was analyzed, and percussion sound was recorded using a wireless microphone connected and analyzed with frequency analysis software., Results: Statistically significant positive correlations were found between the primary ISQ and the bone density HU value (r = 0.719; p < 0.001), and statistically significant positive correlations between the primary ISQ and the percussion sound frequency (r = 0.606; p < 0.001). Furthermore, significant differences in primary ISQ values and percussion sound frequency were found between D1 and D2 bone, as well as between D1 and D3 bone. However, no significant differences were found in primary ISQ values and percussion sound frequency between D2 and D3 bone., Conclusion: The primary ISQ value and the percussion sound frequency are positively correlated., (© 2024 The Author(s). Aims: In the literature, it is still unclear if the decisions for selecting the type of implant crown-retaining system are based on scientific-based research or if the Universities' choices, Implant marketing trends, or finances could have a major influence on the private dentists' decisions., Objectives: Therefore, this study aimed to evaluate the crown-retaining system (cement- or screw-retained) used in dental schools and private dental practices., Methods: A 13-item questionnaire was sent to Canadian dental schools (n = 10) and dental offices in London (n = 298), Canada. The questionnaire included demographic questions and questions to reveal the dentists' perspectives on prosthetic implant treatment between the two-retaining systems. Results were analyzed using descriptive statistics and multinomial logistic regression (p = 0.05)., Results: Twenty-four private dentists and five dental schools responded to the survey - 62.5% of private practitioners and 60% of universities reported using both systems. A trend was observed in using screw-retained systems by dentists who graduated 5-10 years ago. Straumann, Astra, and Nobel Biocare were the private practices and dental schools' preferred implant systems. The use of platform switching for all cases was selected by 54.2% of the private practitioners and 40% of the dental schools. Resin cement was the private practice's preferred cementation method; the dental schools used glass ionomer and zinc phosphate cement. The multinomial logistic regressions showed no statistical difference between the crown-retaining system chosen and the decision factors. The laboratory technician's recommendations and cost influenced the decision-making process for private dentists. For the universities, perio-restorative outcome, implant position, survival rates, institute preferences, and evidence-based research influenced the crown-retaining system's decision-making process.  CONCLUSION: The Canadian dental schools and private practice reported using both screw- and cement-retaining systems. However, there was a difference in the selection criteria as the universities showed a tendency towards a more research-based approach in their decision, while for the private practices, the technicians' recommendations and cost played a major role in the decision process. Objective: To investigate the effects of sterilization, conditioning method, and thermal aging on the retentive strength of two-piece zirconia abutments., Materials and Methods: In total, 128 stock zirconia abutments were divided into four groups (n = 32) according to the conditioning parameters: (A) air-abrasion using 50 μm alumina particles/1.0 bar, (B) 50 μm/2.0 bar, (C) 100 μm/1.0 bar, and (D) 100 μm/2.0 bar. All abutments were bonded onto titanium bases using DTK adhesive resin and stored in water bath (37°C) for 72 h. Each group was subdivided into two subgroups (n = 16), group 1 was disinfected, whereas group 2 followed disinfection and autoclave sterilization. Half of the specimens of each subgroup (n&#x02009;=&#x02009;8) was directly subjected to the axial retention test (groups N), while the other half was first subjected to 150&#x02009;days of thermocycling followed by retention test (groups T). Statistical analysis was performed with three-way ANOVA, additional statistical analysis was performed by using separate one-way ANOVAs followed by the Tukey's post-hoc test for post hoc pairwise comparisons among groups., Results: The highest median retention strength was recorded for group B2N (1390 N), whereas the lowest strength was recorded for group C1T (688 N). No significant interaction (p ≥ 0.05) was detected between the different variables; conditioning method, sterilization, and the thermal cycling regarding the effect on the resulting retention. However, the sterilization always showed a positive effect. Thermocycling presented an adversely significant effect only in the absence of sterilization (p < 0.05), with the exception of subgroups A. For the sterilized groups, thermocycling had no statistically significant effect on the retention., Conclusion: Steam autoclaving increased the retention of hybrid zirconia abutments. Statement of Problem: Fractured prosthetic implant screws cannot be removed in all patients, ultimately leading to the removal of the implant. Whether an intentionally shortened prosthetic implant screw (SPIS) can provide adequate retention is unclear., Purpose: The purpose of this in vitro study was to evaluate the resistance to loosening of SPISs engaging the remaining coronal internal threads as a possible solution to maintaining both implant and restoration., Material and Methods: Fifty grade V titanium SPISs were used to tighten 50 titanium transepithelial abutments on implants to 30 Ncm. The specimens were distributed into 5 groups (n=10) according to the conditions under which the screws were secured to manufacturer-recommended preload: dry (D), moistened in saliva (AS), moistened in chlorhexidine (CLHX), wrapped in polytetrafluoroethylene tape (PTFE), and resin cemented (RE). All groups were subjected to a cyclic loading test (240 000 cycles). The reverse torque value (RTV) of the SPIS was registered twice: 24 hours after initial tightening (T1); and after retightening and the cyclic loading test (T2). The resultant RTV was compared with the 30-Ncm tightening torque to assess torque loosening. The Kruskal-Wallis and Mann-Whitney tests were used for the comparisons between groups and the Wilcoxon test for the intragroup comparisons (α=.05 with Bonferroni correction)., Results: At T1, all groups found lower mean±standard deviation RTVs than the reference tightening torque (30 Ncm) (D 24.82±2.34 Ncm, AS 25.56±2.89 Ncm, PTFE 26.02±2.26 Ncm, CLHX 26.26±1.82 Ncm), except the resin-cemented group, which increased its RTV (RE 44.01±19.94 Ncm). At T2, all the groups found lower RTVs than the reference tightening torque, and the torque values at T1 (D 19.81±6.59 Ncm, CLHX 18.98±6.36 Ncm, AS 21.28±7.32 Ncm), with the exception of PTFE (24.07±3.41 Ncm) and RE (41.47±21.68 Ncm), where RTV was similar to that recorded at T1. At T1, significant differences were found among the groups (P=.024). At T2, after cyclic loading, the RE group found the highest RTV, reporting significant differences with the D and CHLX groups (P<.05) and statistically similar to the AS group (P=.068)., Conclusions: PTFE-wrapped screws found similar RTVs after the fatigue test than dry, moistened with saliva, and moistened with chlorhexidine screws. The rotational movement of mini dental implants (MDIs) overdenture disturbs the function of the prosthesis. Many dentists place more MDIs to improve the overdenture stability; however, the influence of the MDIs number and distribution on the overdenture resistance to para-axial dislodgment has not been investigated. Seven resin models simulating atrophic mandibles housed twenty MDIs placed according to seven arrangements. Acrylic overdentures were fabricated for each cast and were dislodged five times in lateral, anterior and posterior directions, and the peak load dislodgment was measured. Each overdenture underwent 540 axial removal/placement cycles. The para-axial dislodgments were measured again, and data were compared. Dislodgment force values were measured in all directions, and the data were analysed using analysis of variance ANOVA and post hoc (p < 0.05). After six months of simulated placement/removal, increasing the MDI number showed a difference in resistance to para-axial dislodgment. The distribution affected the resistance to dislodgment in some directions. The inter-implant distance of 27 mm provided better resistance to posterior dislodgment than placing two MDIs close together at 19 mm. The placement of three MDIs at any distribution showed no significant difference except for resistance to posterior dislodgment. FourMDIs placed at any distribution showed a significant difference in all groups in all tested directions. Aims: This study aimed to assess the longevity of a fixed prosthesis in patients with intellectual disability (ID) and to investigate the risk factors associated with the failure of a prosthesis due to abutment tooth extraction or prosthesis dislodgement or removal. Methods: We studied past medical records to evaluate the longevity of 315 prostheses that were luted in 76 patients with ID. We calculated the survival rates and assessed 15 variables potentially associated with prosthetic failure using multivariate Cox regression analyses with shared frailty for patients. Results: Three‐quarters of our sample population had severe or profound ID. The maximum observation period was 31.0 years, and the corresponding survival ratio was 32.5%; the survival ratio at 10 years was 59.4%. The use of intravenous sedation significantly influenced the success of the prosthesis, with the hazard ratio (HR) being 0.49 times that of conventional treatment without behavior‐altering drug therapy. The most significant risk factor for prosthetic failure was age at placement; the HR for patients aged ≥31 years was 2.82 times that for patients aged ≤ 20 years. Conclusions: In patients with severe ID, appropriate intravenous sedation was effective in prolonging the longevity of a fixed prosthesis. [ABSTRACT FROM AUTHOR]

Background: This study aimed to compare retention and fracture load in endocrowns made from translucent zirconia and zirconium lithium silicate. Methods: Fifty-six intact human maxillary molars after being mounted in acrylic resin, were scanned to acquire biogeneric copies. Specimens underwent standard endodontic treatment and were prepared for endocrown up to 2 mm above the cementoenamel junction. The specimens were randomly divided into two groups of 28, and endocrowns were designed using biogeneric copies and milled from high-translucent zirconia disks (Zr) and zirconium lithium silicate blocks (ZLS). After cementation with dual-cure resin cement, all the specimens underwent thermomechanical aging, and pull-out retention test and compressive test were conducted (14 specimens were used for each test in each group, n = 14), and failure modes in both tests were evaluated. Results: Independent samples t-test showed significant difference between the retention of Zr (271.5 N ± 114.31) and ZLS (654.67 N ± 223.17) groups (p value = 0.012). Compressive test results were also significantly different between Zr (7395.07 N ± 1947.42) and ZLS (1618.3 N ± 585) (p = 0.002). Failure mode of retention test was primarily adhesive failure at the cement-restoration interface in Zr group and cement-tooth interface in ZLS group. Failure modes of fracture test for Zr group were 7 non-restorable fractures and one restorable fracture while 6 specimens resisted compressive loads up to 8500 N without fracture. ZLS group showed 7 restorable and 7 non-restorable failures. Conclusions: Zr endocrowns showed significantly lower retention and higher fracture strength. Purpose: This study was designed to investigate the effect of post space conditioning with different solutions on the bond strength of glass FRC posts and evaluate the mode of resin failure., Methods: Sixty extracted human single rooted teeth were root filled using a resin sealer and suitable gutta-percha cones. Post spaces were prepared, and the teeth randomly allocated into 5 groups according to their irrigation regimens which included the following solutions: 17 % EDTA, 2 % CHX, 3 % NaOCl, 10 % ascorbic acid and QMix solutions. Post spaces were irrigated with 5 ml of the solution for 15 s and subsequently washed with distilled water and dried with paper points. Glass FRC posts were cemented into their spaces using a self-adhesive resin cement. The specimens were mounted in plexiglass molds using autopolymerizing acrylic resin. A universal testing machine was used to measure post retention at a crosshead speed of 2 mm/min. The results were analyzed by one-way ANOVA followed by Tukey HSD test (α = 0.05). Dislodged posts and post spaces were examined microscopically to evaluate retention failure., Results: The Ascorbic acid group exhibited the highest mean retentive strength value at 229 N, followed by QMix at 198 N, NaOCl at 186 N, CHX at 170 N, and EDTA at 124 N. The mean value of the ascorbic acid group was significantly higher than EDTA group, p = 0.012. The failure category was primarily mixed., Conclusions: Rinsing post spaces with ascorbic acid exhibited significantly superior bond strength. The failure mode was mixed., Clinical Significance: Irrigating post spaces with ascorbic acid solution before luting FRC posts significantly improves their bond strength compared to irrigation with EDTA solution. Statement of the Problem: Poly-etheretherketone is a novel material used in the construction of the removable partial dentures frameworks instead of the metal frameworks. This material can be fabricated by various techniques. Most common methods are the injection molding or Computer Aided Design/Computer Aiding Manufacturing (CAD/CAM) milling techniques. The fabrication technique may affect the adaptation of the frameworks by influencing the retention. Purpose: To assess the effect of the processing techniques of high performance Polyetheretherketone either by injection molding (pressing) or CAD/CAM milling techniques on removable partial denture frameworks retention for rehabilitation of upper class I Kennedy classification. Materials and Method: This in vitro study was performed on one epoxy resin model representing the partially edentulous maxillary arch with natural teeth extending from first premolar to first premolar. First premolars and canines were reduced to receive porcelain fused to metal crowns with 0.50mm mesio-buccal retentive undercuts, distal guiding planes and mesial occlusal rest seat on first premolars and cingulum rest seat on canines. Considering the construction technique of frameworks, twenty samples were divided into two groups. In the group I, ten frameworks were fabricated by injection molding, and in the group II, ten frameworks were fabricated by CAD/CAM. The removal and insertion was carried out at 120, 720 and 1440 cycles for both groups, respectively. The retention values were measured by using Universal Testing Machine before cycling and after each interval. Results: Independent t-test showed significant difference on retention at different simulation cycles between groups. Group II exhibited significantly less retention than group I (p< 0.001), while comparing the retention at different cycles within each group by paired sample t-test exhibited significant decrease of retention till the end of the cycling (p< 0.001). Conclusion: From the retention point of view, high performance poly-etheretherketone frameworks fabricated by injection molding technique provided a promising method over CAD/CAM technique milling method. ABSTRACT Introduction: There is no consensus as to which cemented or screwed retention system is best to avoid bone loss around the implant from a fixed implant-supported restoration. Objective: To evaluate the prosthesis retention systems on screw and cemented implants, regarding: bone loss, survival and failure rate, biological complications and microbiological analysis. Methods: A search was made for scientific articles that contemplated the subject through the databases Pubmed and SciELO, without period restriction. The titles, abstracts and then access to the full text has been verified. Results: It was found that excess cement may play an important role in the development of peri-implant disease. Technical failures are most seen in prosthesis retained by screws, and biological complications in cemented crowns. The success rate for both restraint systems is high, and retention-independent implant prosthesis treatment provides predictability. Conclusion: The appropriate retention system for the patient depends on several factors, including indication, advantages and disadvantages, retention provided, aesthetics and clinical performance.

Purpose: To determine the relationship between bone loss that occurs during the peri-implantitis process and variations in implant stability using resonance frequency analysis (RFA) measurement methods., Materials and Methods: Forty selftapping implants were placed in cow ribs, and study scenarios were established according to the affected implant side and bone loss depth (n = 10 implants per group): Case 1 = bone loss on one side (vestibular); Case 2 = bone loss on two opposite sides (buccal and lingual); Case 3 = bone loss on two adjacent sides (buccal and mesial); and Case 4 = foursided bone loss (circumferential). For each group of 10 implants, first a bone loss of 0 mm was evaluated, then 4-mm defects (simulating 1/3 of bone loss) were created and evaluated, and finally 8-mm defects (simulating 2/3 of bone loss) were created and evaluated. Osteotomy measurements were made with a periodontal probe. For each implant, RFA was measured by the same operator using the Beacon system (Osstell)., Results: The initial implant stability quotient (ISQ) values of the 40 implants exceeded 70, reflecting an average of 73 in the buccolingual (VL) and 74.8 in the mesiodistal (MD) directions. ISQ measurements in the 10 implants in which bone dehiscence was performed on the vestibular aspect reflected a decrease in ISQ values as bone loss increased. When generating bone loss in two opposite sides (buccal and lingual), a greater decrease in ISQ values was observed when 2/3 of the implant were affected. The average VL ISQ measurement was less than 70 when at sites with 2/3 of bone loss., Conclusions: When bone loss occurs on only one side of the implant, the ISQ values decrease, but the implant maintains good stability. The same occurs when two opposite sides of the implant are affected, as the unaffected side has the least decrease in ISQ value.

A technique is outlined for utilising a polymeric composite reinforced with glass fibres in a three-dimensional mesh as a post-core in aesthetic cases. The clinical procedure involves obtaining an impression of the root canal space, scanning the definitive cast, and milling a fibre-reinforced composite post-core. Subsequently, the intra-radicular post-core is cemented using an adhesive resin cement. Background: Different methods have been proposed to investigate the fixation stability of dental implants, each of which has its limitations. Among these methods, resonance frequency analysis (RFA) has been widely utilized to measure dental implant stability. This study aimed to assess dental implants with two non-destructive RFA and acoustic modal analysis (AMA) validated with a finite element simulation of the fundamental natural frequency (NF) of the bone analog-implant structure., Material and Methods: A total number of 18 implants were inserted into two Polyurethane (PU) bone blocks with different densities (0.16 g/cc and 0.32 g/cc). AMA was used to measure NF; First, the sound originating from the axial tapping of the implant was recorded with a simple microphone. Secondly, a fast Fourier transformation algorithm was conducted to determine the NF of the implant-bone analog structure. In parallel, the ISQ (Implant Stability Quotient) value was measured using the Osstell® device. Finally, using finite element analysis (FEA), the implant-bone analog structure was modeled for validation., Results: Doubling the bone analog density resulted in an average increase of 82% and 47% in the NF and ISQ using AMA and Osstell®, respectively (P-value<0.05). Furthermore, a strong linear relationship (R2= 0.93) was observed between the measured NF and ISQ values in the linear regression analysis. The NF of the dental implant predicted by FEA was overestimated by about 15.2% and 15.0% than those in the low- and high-density PUs, respectively. Moreover, the FEA predicted an increase of 83% in NF by increasing the bone analog density from 0.16 to 0.32 g/cc., Conclusions: Having required the minimum process combined with easily available equipment makes it an ideal method for fixation strength studies. The good correspondence between the ISQ values and NFs, in addition to the good accuracy and reliability of the later method, confirms its application for fixation stability assessment.

Purpose: To evaluate and compare the difference in retention between implant-supported restorations with and without surface modification of the implant abutments., Materials and Methods: A total of 30 patients with singletooth implants were restored with cement-retained (Multilink N, Ivoclar) restorations using titanium base abutments (Variobase, Straumann) and randomly assigned surface modifications. Group 1 used nonmodified abutments, group 2 used sandblasted abutments, and group 3 used sandblasted abutments followed by an application of metal primer. All patients were recalled for a baseline examination 6 months after crown placement. The pull-out strength and intergroup distribution of mean pull-out strength were assessed. To assess differences between the three groups, intergroup statistical comparison of continuous variables was done using one-way ANOVA with Tukey correction for multiple group comparisons., Results: The results of the intergroup mean pull-out strength distribution revealed that the distribution of mean ± SD pull-out strength in group 1, group 2, and group 3 were 220.79 ± 94.23, 488.64 ± 84.12, and 705.46 ± 112.75 Ncm, respectively., Conclusions: Sandblasting followed by the application of metal primer produced the highest retention of porcelain-fused-to-metal (PFM) crowns to titanium base abutments, followed by sandblasting alone, with the least retention being observed with no surface treatment.

Purpose: To investigate the relationship between the structural parameters of trabecular bone obtained from CBCT imaging and the primary stability of dental implants., Materials and Methods: Sixty patients underwent implant placement followed by primary stability evaluation via measurement of the insertion torque (IT) and the implant stability quotient (ISQ). Gray values (GV) and the fractal dimension (FD) were also measured using pretreatment CBCT images., Results: FD values showed a positive and significant relationship with ISQ and IT values (P = .017 and P = .004, respectively). Additionally, there was a positive and significant correlation between GV and IT (P = .004) as well as between GV and ISQ (P = .010). FD and GV showed a considerable difference between the maxillary and mandibular jaws and were higher in the mandible. Only FD was significantly different between men and women and was higher in men. In the two age groups (older and younger than 45 years), only GV was considerably higher in people older than 45 (P < .05)., Conclusions: Both fractal dimension and gray values obtained from CBCT are efficient methods for predicting the primary stability of the implant due to their relationship with ISQ and IT values.

Purpose: The aim of this in vitro study was to assess the effects of using different cements and titanium copings designs on the retention of implant-supported fixed dental prostheses (IFDPs) using a pull-out test., Materials and Methods: Fifty zirconia (ZirCAD; Ivoclar Vivadent) and 20 prepolymerized denture acrylic resin (AvaDent) rectangular (36 mm × 12 mm × 8 mm) specimens were milled to mimic the lower left segmental portion of the All-on-Four IFDPs. Cylindrical titanium copings (Variobase; Straumann) (V) were used in 2 prepolymerized denture acrylic resin groups (n = 10) while conical titanium copings (Straumann) (C) were used as a control group for zirconia with 4 groups using cylindrical titanium copings. Before cementation, the outer surfaces of all titanium copings and the intaglio bonding surface of prosthetic specimens were airborne-particle abraded. All specimens were cemented following the manufacturer's recommendations and instructions according to the experimental design. After artificial aging (5000 cycles of 5°C 55°C, dwelling time 20 s; 150 N, 1.5 Hz in a 37°C water bath), all specimens were subjected to retention force testing using a pull-out test using a universal testing machine and a custom fixture with a crosshead speed 5 mm/min. Modes of failure were classified as Type 1, 2, or 3. Retention force values were analyzed by the t-test for the prepolymerized denture acrylic resin specimen groups, and 1-way ANOVA and the Tukey test for the zirconia groups at α = 0.05., Results: Mean and standard deviation retention force values varied from 101.1 ± 67.1 to 509.0 ± 65.2 N for the prepolymerized denture acrylic resin specimen groups. The zirconia groups ranged from 572.8 ± 274.7 to 1416.1 ± 258.0 N. There is no statistically significant difference in retention force values between V and C specimens cementing to zirconia with Panavia SA cement (Kuraray Noritake) (p = 0.587). The retention forces and failure modes were influenced by the cement used (p < 0.05). Modes of failure were predominantly Type 2 (mixed failure) and Type 1 (adhesive fracture from prosthetic materials) except for the quick-set resin group (Type 3, adhesive failure from coping)., Conclusions: When bonding IFDPs onto titanium copings, quick-set resin provided significantly higher retention force for prepolymerized denture acrylic resin prostheses. Conical and cylindrical titanium copings performed similarly when cemented to zirconia with Panavia SA cement under the same protocol. Background: The influence of dental implant length and diameter on primary stability in various bone densities is not well understood., Aim: To in vitro study the effect of length and diameter on resonance frequency analysis (RFA), insertion torque (IT) and displacement (DP) measurements of dental implants in different implant site densities., Materials and Methods: Dental implants of four different diameters (Ø 3.5, 4.0, 4.5 and 5.0 mm) and three different lengths (7, 11 and 15 mm) (Neoss Ltd, Harrogate, UK) were placed in polyurethane blocks of three different densities (Sawbones Europe AB, Malmö, Sweden). The primary stability was assessed by RFA (ISQ) (Osstell, Osstell AB, Gothenburg, Sweden) and insertion torque measurements (IT max in N cm) (iChiropo™, Bien-Air Dental SA, Bienne, Switzerland). In addition, the blocks were mounted in a rig and a lateral force of 25 N cm was applied to the implants and the DP was measured in μm with a micrometer gauge placed on the opposite side of the load transducer. Statistical analyses using linear and quadratic models were applied., Results: Implant length, diameter and block density were found to be significant independent predictors of RFA, IT max , and DP measurements. Purpose: To evaluate the influence of resin cement type, surface pretreatment and autoclave sterilization on the retention of Y-TZP crowns to Ti-base abutments. Methods: Y-TZP crowns were designed and milled to fit Ti-base abutments. Crowns were cemented using either a conventional resin cement (conventional) with a universal adhesive or a self-adhesive resin cement (self-adhesive), both following no surface pretreatment (No) or Ti-base abutment sandblasting (SB) (n=20/group). Half of the cemented samples were subjected to in-office autoclave sterilization. Pullout testing was performed in a universal testing machine at a speed of 1 mm/min until crown displacement. Data were statistically evaluated through a linear mixed model following post hoc comparisons by LSD test. Results: Pullout data as a function of cement type demonstrated higher retention for conventional relative to self-adhesive cement (p<0.001). Ti-base sandblasting (SB) favored crown retentiveness over No pretreatment (p<0.001). Sterilized crowns exhibited higher pullout values than non-sterile (p=0.036). All the two- and three-factor interaction analyses corroborated with the superior adhesive strength of conventional compared to self-adhesive cement (all, p<0.011), as well as, SB relative to No pretreatment (all, p<0.024). While autoclave sterilization maximized bond strength when self-adhesive cement (data collapsed over surface pretreatment, p<0.050) and No pretreatment were evaluated (data collapsed over surface pretreatment, p<0.013), no significant difference was observed for conventional resin cement (p=0.280) and SB (p=0.878) groups. Conclusions: Conventional resin cement and/or Ti-base sandblasting increased Y-TZP crown retentiveness, with no significant influence of autoclave sterilization. Objectives: Implant-supported restorations are generally used for the replacement of the lost teeth. Stability against masticatory forces and proper retention are critical for optimal durability of restorations. The aim of this experimental study was to compare the retention of cobalt-chromium (Co-Cr) copings made by different techniques. Materials and Methods: Twenty-four solid abutment analogs were mounted and scanned with a desktop scanner. They were divided into two groups (n=12) and received metal copings fabricated by either soft or hard Co-Cr alloy. Soft Cera­mill Sintron Co-Cr patterns were milled and sintered. Hard Co-Cr blocks were milled in a milling machine. The copings were sandblasted, polished, adjusted, and placed on the respective abutments. The frequency of adjustments was recorded for each abutment. The copings were cemented with zinc phosphate cement and underwent tensile test by a universal testing machine. The Mann-Whitney test and t-test were used to compare the two groups (α=0.05). Results: There was no significant difference in retention of copings between the experimental groups. The mean retentive force was 559.58±115.66 N and 557.13 ±130.48 N for the soft and hard metal groups, respectively (P=0.96). Alveolar bone quality at the implantation site affects the initial stability of dental implant treatment. However, the relationship between bone quality and osseointegration has yet to be evaluated. Herein, we aimed to investigate the effect of bone quality on dental implant stability in osseointegration formation changes. Patients underwent computed tomography imaging before dental implantation at the posterior. Hounsfield units were measured at the platform, middle, and tip sites. Implant stability was measured using resonance frequency analysis immediately and at 3 months postoperatively, in which the difference in implant stability quotients (ISQ) was defined as the change between primary and secondary fixation. In multiple regression analysis, the dependent variable was the change between the immediate and secondary fixations. We included 81 implants that conformed to the criteria. Primary fixation yielded the following results: R2 = 0.117, F = 2.529, and P = .047. The difference between the maxilla and mandible of the implantation site (P = .02) and the platform-site Hounsfield units (P = .019) were identified as significant factors. The following results were obtained regarding the change between the immediate and secondary fixation: R2 = 0.714, F = 40.964, and P < .001. The difference between diameter (P = .008) and the immediate ISQ (P < .001) were identified as significant factors. Overall, the bone quality of the implantation site affected initial fixation; however, it had limited effect on secondary fixation. Our findings clarified the period where bone quality affects dental implant treatment and is expected to advance dental implant treatment.

The aim of this study was to evaluate the mechanical behavior of UCLA and Mini-conical abutments for implants with Tri-channel connections regarding torque loss and vertical misfit. Twenty 3-element metal-ceramic fixed partial dentures (FPD) supported by 2 implants were manufactured and divided into 2 groups (n = 10): UCLA (group 1) and Mini-conical Abutments (group 2). The evaluation of torque loss was carried out before and after mechanical cycling, while the vertical fit was evaluated throughout the different stages of manufacturing the prostheses, as well pre- and postcycling (300,000 cycles, 30 N). Statistical analyses of torque loss and vertical misfit were performed using the linear mixed effects model. Both groups showed torque loss after mechanical cycling (P < .05); however, there was no significant percentage differences between them (P = .795). Before cycling, the groups showed a significant difference in terms of vertical misfit values (P < .05); however, this difference was no long observed after cycling (P = .894). Both groups showed torque loss after the cycling test, with no significant difference (P > .05). There was no significant difference in vertical misfit after mechanical cycling; however, in group 1 (UCLA) there was accommodation of the implant-UCLA abutment interface, while group 2 (Mini-conical abutment) did not show changes in the interface with the implant after the test. Both groups behaved similarly regarding the torque loss of the prosthesis retention screws pre- and postmechanical cycling, with greater loss after the test.

Dental implants have gained popularity in recent years. The most important variable in determining the effectiveness of the implant's primary stability is bone density. The success of the implant depends on proper procedure and implant stability. With this background, the aim of the present study was to study the correlation between primary stability and bone density. The present retrospective study was conducted among 2,440 patients who had undergone implant treatment in the Department of Implantology, Saveetha Dental College and Hospitals, Chennai, from June 2021 to February 2022. Data regarding patient's age, gender, implant location, bone density, and primary stability were taken into consideration. Association between primary stability and bone density was assessed using the Chi-square test. Of the subjects, 33.42% had D2 bone density in the lower posterior region; 13.98% had D3 density in the lower posterior region. Primary stability of 30-40 Ncm was seen in the majority of the subjects; 32.64% who had primary stability of 30-40 Ncm had D2 bone density. There was a statistically significant association between implant site and bone density (p = 0.04) and primary stability and bone density (p = 0.03). Within the limitations of the study, it can be concluded that there is a strong association between implant primary stability and bone density.

Introduction: The radiographic examination of alveolar bone using 3D radiographic examination is essential in dental implant treatment planning. Our study aimed to systematically review and quantitatively analyze the correlation between alveolar bone parameters, specifically bone density and cortical bone thickness, assessed using cone beam computed tomography (CBCT) and/or multidetector computed tomography (MDCT); and primary implant stability (PIS) determined using implant stability quotient (ISQ), Periotest® value (PTV), and insertion torque value (ITV)., Methods: This review was registered in the PROSPERO database (registration number CRD42022307245). An electronic literature search was conducted on the PubMed, SCOPUS, and Web of Science databases for papers published until February 2022. The Quality Assessment in Prognostic Studies (QUIPS) tool was used to assess risk of bias. Meta-analyses were conducted to calculate the estimated average correlation coefficient based on a multilevel random-effects model, followed by subgroup analysis., Results: Twenty-six studies were included in this review, consisting of 17 prospective cohort studies, eight retrospective cohort studies, and one nonrandomized controlled trial. A total of 3109 implants placed in 1171 subjects were analyzed. Twenty-three studies were evaluated using meta-analysis. The alveolar bone condition was significantly correlated with ISQ (r = 0.60; p < .001), IT (r = 0.52; p < .001), and PTV (r = -0.42; p < .05)., Conclusion: Alveolar bone condition is significantly associated with PIS. Low bone density and thin cortical bone can lead to low PIS; therefore, modification of treatment planning and surgical procedures might be needed to avoid poor osseointegration., (© 2023 John Wiley & Sons A/S. Following the attainment of new positions of misaligned teeth after fixed orthodontic appliance therapy they need sustainment of new position mechanically so that supporting structures and bone attains concrete position and maturity. Retention is the period of Orthodontic treatment that keeps teeth in new position after treatment with orthodontic treatment. Without period of retention, there is a propensity for teeth to new position for their underlying position causing relapse. Hence it is very necessary to prevent relapse which depends on many factors. Every orthodontic patient must wear retention appliances to prevent relapse. Purpose: To determine the prevalence and distribution of prosthetic complications affecting implant‐supported fixed dental prostheses (ISFDPs). Materials and Methods: Subjects previously treated with one or more ISFDP(s) were identified from an electronic health record search and recalled for comprehensive clinical examination. Past prosthesis failures and complications were identified from the patient records while any existing complications, not previously recorded, were assessed during examination. ISFDP survival and failure rates were calculated with Kaplan‐Meier curves and life table analysis, while regression Poisson analysis was used to identify associations between outcomes and possible patient‐ and prosthesis‐based risk factors. Results: Seventy‐four subjects with 107 ISFDPs were enrolled in the study with a mean time between prosthesis delivery and exam of 3.14 years (range: 1.00‐9.00 years). Four prostheses failed, resulting in a cumulative prosthesis survival rate of 96.26%. Prosthetic complications affected 48.59% of ISFDPs, the majority (94.87%) of them minor complications. Only the use of a nightguard was associated with a lower prevalence of prosthetic screw loosening (HR 0.11, 95% CI 0.02‐0.59, p = 0.007) while no outcome differences were noted for other variables. Patient satisfaction was high regardless of presence or number of complications. Conclusions: ISFDPs demonstrated a high survival rate and overall high, patient‐reported satisfaction. Purpose: This is a single center, retrospective study to assess the prevalence of peri‐implant disease and biologic complications in a cohort of partially edentulous subjects in relation to selected prosthetic factors. Materials and methods: Subjects previously treated with one or more implant‐supported fixed dental prosthesis (ISFDPs) were recalled for a comprehensive examination. Clinical and radiographic records were taken and questionnaires were administered. The prevalence of implant failure, peri‐implant disease and other biologic complications were correlated with selected prosthetic, clinical and patient‐related factors using chi‐square and multiple regression analyses. Results: A convenience sample of 71 subjects with 100 prostheses supported by 222 dental implants were enrolled in the study. The mean follow‐up time after prosthesis delivery was 3.3 ± 1.5 years (range of 1‐9 years). The cumulative implant survival rate was 99.1%. Peri‐implantitis was the most frequent major biologic complication (5% of implants), while the most frequent minor biologic complication was peri‐implant mucositis (84.10% of implants). A diagnosis of peri‐implant mucositis was more likely associated with cement‐retained prostheses compared to screw‐retained prostheses (OR 6.8, 95% CI 1.1‐78.6, p = 0.045) and for short‐span prostheses (≤3 prosthetic units) (OR 2.3, 95% CI 1.1‐5.0, p = 0.034). Subject‐reported quality of life measures were high regardless of the existence of major and/or minor complications, but decreased with increasing number of minor and total biologic complications. Conclusions: Peri‐implant mucositis and other minor biologic complications were highly prevalent. Purpose: This study evaluated the effects of abutment height, airborne-particle abrasion, and type of cement on the tensile resistance to dislodgement of cement-retained implant restorations. Materials and Methods: Three groups of 12 standardized abutments each were prepared with different heights (4 mm, 5 mm, and 6 mm) using a milling machine. Crowns were cast in cobalt-chrome using the lost-wax technique, airborne particle-abraded using 50-µm aluminum oxide, and cleaned with acetone. Restorations were cemented using a noneugenol acrylic urethane cement, a resin-modified glass ionomer, or a zinc oxide-noneugenol cement. A 5-kg load was applied for 10 minutes. Samples were kept at 37°C and 100% humidity overnight. A tensile force was applied to the crown using a testing machine at a crosshead speed of 5 mm/minute until failure occurred. Next, the abutments were airborne particle-abraded with 50-µm aluminum oxide, and the cementation and testing procedures were repeated. The effects of cement, abutment height, and surface treatment were evaluated statistically. Results: There were significant differences among the cements. The resin-modified glass ionomer provided the greatest retention in all the tested conditions, while the zinc oxide-noneugenol cement produced the lowest retention values. Significant differences were also detected between 4-mm and 6-mm abutments, with the 6-mm abutments being more retentive. No differences were found between 4-mm and 5-mm abutments or between 5-mm and 6-mm abutments. The effect of airborne-particle abrasion was also found to be significant. A maximum increase of 90 N in retention force was observed after airborne-particle abrasion for the 5-mm abutments cemented with the acrylic urethane cement. Conclusions: Cement, airborne-particle abrasion, and abutment height can significantly influence retention of implant-supported crowns. Purpose: It has been suggested that the CAD/CAM Procera custom abutment may be universally applied with multiple implant systems. An acceptable fit between the internal hexagon of an abutment and the external hexagon of various implant systems, along with true interchangeability of the Procera abutment screw, would support this concept. This study determined the precision of fit of the CAD/CAM-produced Procera abutment onto the external hexagon and bearing surfaces of implants from 6 implant manufacturers and the interchangeability of the Procera abutment screw with these systems. Materials and Methods: This investigation consisted of 3 parts: (1) direct measurement of the internal hexagon and bearing surface of each Procera abutment and the external hexagon and the bearing surface of 6 implants from 6 different systems, (2) radiographic examination of 30 Procera abutment-implant junctions following tightening to 32 Ncm to determine the precision of fit between the bearing surfaces and the top of the external hexagon of the implant with the superior surface of the internal hexagon of the abutment, and (3) examination of 3 abutment screws and 3 implants from the various manufacturers for interchangeability based on American National Standards. Results: The mean flat-to-flat external hexagons of the implants measured between 2.67 and 2.69 mm. The Procera abutment's flat-to-flat internal hexagon measured 2.73 mm. The height of the various implant systems' external hexagon ranged from 0.69 to 0.81 mm. The height of the Procera abutment blanks was 0.90 mm. Radiographic examination demonstrated that not all of the manufacturers' screws fit appropriately within the internal screw bore of the Procera abutment. The internal bore of all implant systems studied had a metric thread designation of M2 x 0.4 - 6H. The metric thread designation of all abutment screws examined was M2 x 0.4 - 6g. The greatest variations in the dimensions of the abutment screws measured were seen in the diameter of the screw head, which ranged from 2.12 to 2.69 mm. Discussion: The Procera abutment's internal hexagon fit the external hexagon of all implant systems evaluated. The Procera abutment screw fit the internal screw bore of the implant systems tested. Conclusion: The Procera abutment with its screw can be universally applied to the implant systems studied. This fact, plus the CAD/CAM feature of this system, would thus provide a dynamic approach to satisfying the design and spatial needs of implant placements observed clinically. [ABSTRACT FROM AUTHOR]

Purpose: This report involves the retrospective analysis of ITI implants placed and/or restored by a group of 7 clinicians located throughout the United States (5 periodontists and 2 prosthodontists). Materials and Methods: Six hundred seventy-five posterior single-tooth implants were restored in 471 patients (average time of loading 21.30 months, with a range of 1 to 78 months). Three hundred seventy implants and 71 implants were placed in mandibular and maxillary molar sites, respectively, and 108 and 126 were placed in mandibular and maxillary premolar sites, respectively. Results: A cumulative survival rate of 99.1% was obtained for all sites (6 failures). The survival rates for individual sites were as follows: 98.4% mandibular molars, 100% maxillary molars, 100% mandibular premolars, and 100% maxillary premolars. "At-risk" implants (1 to 2 mm of radiographic bone loss) were noted at 5 sites. Discussion: Minimal restorative problems were found with either screw-retained (n = 71) or cemented restorations on solid abutments (n = 600); 80.3% of screw-retained and 98.2% of cemented restorations were free of complications, respectively. Patient satisfaction scores were high (97.4%) as determined by the Patient Satisfaction Questionnaire. Conclusion: The data suggest that solid-screw (4.1 or 4.8 mm wide) ITI implants can be a satisfactory choice for posterior single-tooth restorations. [ABSTRACT FROM AUTHOR]

Aim: To compare the effects of cast partial denture with conventional all acrylic denture in respect to retention, stability, masticatory efficiency, comfort and periodontal health of abutments. Methods: 50 adult partially edentulous patient seeking for replacement of missing teeth having Kennedy class I and II arches with or without modification areas were selected for the study. Group-A was treated with cast partial denture and Group-B with acrylic partial denture. Data collected during follow-up visit of 3 months, 6 months, and 1 year by evaluating retention, stability, masticatory efficiency, comfort, periodontal health of abutment. Results: Chi-square test was applied to find out differences between the groups at 95% confidence interval where p = 0.05. One year comparison shows that cast partial denture maintained retention and stability better than acrylic partial denture (p< 0.05). The masticatory efficiency was significantly compromising from 3rd month to 1 year in all acrylic partial denture groups (p< 0.05). To evaluate silane influence on the interfacial fracture toughness (IFT) of composite cement, with the two sub-classes of CAD-CAM composites, polymer-infiltrated ceramic networks (PICN) and dispersed fillers (DF), after hydrofluoric acid etching (HF) or airborne-particle abrasion (AB). A secondary objective was to correlate results with developed interfacial area ratio (Sdr) and surface wettability. Experimental PICN and DF blocks were cut into equilateral half-prisms, which were treated with HF or AB, then treated with an experimental silane or not and bonded to their counterparts with an experimental light-cure resin cement. After thermocycling, samples (n = 30 per group) were tested for IFT using the notchless triangular prism test in a water bath at 36 °C. Moreover, profilometry and contact angle measurement were performed on rectangular samples of each group. Finally, bonding interface was analysed by SEM. PICN-HF treated with silane showed the highest IFT significantly. Three-way ANOVA revealed the influence of silane, material class and surface pre-treatment (HF or AB) on IFT (p < 0.05). When silane was used, IFT was correlated with Sdr, while surface wettability was increased. Silane application significantly increased IFT for PICN but not for DF, while PICN performed better with HF and DF with AB. Silane increases IFT of composite cement with PICNs, but not with DF materials. Results suggest that silane increases the micromechanical bond by promoting resin cement spreading and penetration in surface roughness. This roughness is significantly higher for pre-treated PICNs than for DF due to their specific honeycomb microstructure when etched, which explains their better bonding properties.
