Abstract

Aims: The COVID‐19 pandemic caused significant disruption to routine activity in primary care. Medication reviews are an important primary care activity ensuring safety and appropriateness of prescribing. A disruption could have significant negative implications for patient care. Using routinely collected data, our aim was first to describe codes used to record medication review activity and then to report the impact of COVID‐19 on the rates of medication reviews. Methods: With the approval of NHS England, we conducted a cohort study of 20 million adult patient records in general practice, in‐situ using the OpenSAFELY platform. For each month, between April 2019 and March 2022, we report the percentage of patients with a medication review coded monthly and in the previous 12 months with breakdowns by regional, clinical and demographic subgroups and those prescribed high‐risk medications. Results: In April 2019, 32.3% of patients had a medication review coded in the previous 12 months. During the first COVID‐19 lockdown, monthly activity decreased (−21.1% April 2020), but the 12‐month rate was not substantially impacted (−10.5% March 2021). The rate of structured medication review in the last 12 months reached 2.9% by March 2022, with higher percentages in high‐risk groups (care home residents 34.1%, age 90+ years 13.1%, high‐risk medications 10.2%). The most used medication review code was Medication review done 314530002 (59.5%). Conclusions: There was a substantial reduction in the monthly rate of medication reviews during the pandemic but rates recovered by the end of the study period. Abstract

The intracellular coral–dinoflagellate symbiosis is the engine that underpins the success of coral reefs, one of the most diverse ecosystems on the planet. However, the breakdown of the symbiosis and the loss of the microalgal symbiont (i.e. coral bleaching) due to environmental changes are resulting in the rapid degradation of coral reefs globally. There is an urgent need to understand the cellular physiology of coral bleaching at the mechanistic level to help develop solutions to mitigate the coral reef crisis. Here, at an unprecedented scope, we present novel models that integrate putative mechanisms of coral bleaching within a common framework according to the triggers (initiators of bleaching, e.g. heat, cold, light stress, hypoxia, hyposalinity), cascades (cellular pathways, e.g. photoinhibition, unfolded protein response, nitric oxide), and endpoints (mechanisms of symbiont loss, e.g. apoptosis, necrosis, exocytosis/vomocytosis). The models are supported by direct evidence from cnidarian systems, and indirectly through comparative evolutionary analyses from non‐cnidarian systems. With this approach, new putative mechanisms have been established within and between cascades initiated by different bleaching triggers. In particular, the models provide new insights into the poorly understood connections between bleaching cascades and endpoints and highlight the role of a new mechanism of symbiont loss, i.e. 'symbiolysosomal digestion', which is different from symbiophagy. This review also increases the approachability of bleaching physiology for specialists and non‐specialists by mapping the vast landscape of bleaching mechanisms in an atlas of comprehensible and detailed mechanistic models. We then discuss major knowledge gaps and how future research may improve the understanding of the connections between the diverse cascade of cellular pathways and the mechanisms of symbiont loss (endpoints). Abstract

Electronic health records (EHRs) and other administrative health data are increasingly used in research to generate evidence on the effectiveness, safety, and utilisation of medical products and services, and to inform public health guidance and policy. Reproducibility is a fundamental step for research credibility and promotes trust in evidence generated from EHRs. At present, ensuring research using EHRs is reproducible can be challenging for researchers. Research software platforms can provide technical solutions to enhance the reproducibility of research conducted using EHRs. In response to the COVID‐19 pandemic, we developed the secure, transparent, analytic open‐source software platform OpenSAFELY designed with reproducible research in mind. OpenSAFELY mitigates common barriers to reproducible research by: standardising key workflows around data preparation; removing barriers to code‐sharing in secure analysis environments; enforcing public sharing of programming code and codelists; ensuring the same computational environment is used everywhere; integrating new and existing tools that encourage and enable the use of reproducible working practices; and providing an audit trail for all code that is run against the real data to increase transparency. Abstract

Symbiotic dinoflagellates in the family Symbiodiniaceae release mobile compounds (e.g., glucose, glycerol, amino acids, and lipids) to their host's tissues. Little is known about how different symbionts affect quantitative and qualitative differences in these compounds. We tested how symbiont identity affects glycerol and glucose pools in the tissues of the sea anemone Exaiptasia pallida ('Aiptasia'). We infected symbiont-free anemones with three different symbiotic dinoflagellates: Breviolum minutum isolated from our Aiptasia stock, B. minutum isolated from a different Aiptasia population, and the free-living Effrenium voralum. We measured free glycerol and glucose levels in host tissues under photosynthetic conditions, as well as metabolite release by these algae when freshly isolated from Aiptasia and incubated in a host tissue homogenate. Anemone tissues containing the stock B. minutum accumulated glycerol at a higher symbiont cell-specific rate than those containing the alternative B. minutum or E. voratum (e.g., at 9 h of light incubation: stock B. minutum, 4.05 * [10.sup.-5] nmol per algal cell; alternative B. minutum, 0.90 * [10.sup.-5] nmol per algal cell; E. voratum: 1.14 * [10.sup.-5] nmol per algal cell). All symbiotic hosts accumulated glucose between 1 and 12 h of light incubation. At 12 h, the symbiont cell-specific glucose content was 6-fold higher in anemone tissues that contained stock B. minutum than in those containing the alternative B. minutum (1.73 * [10.sup.-6] vs. 0.30 * [10.sup.-6] nmol per algal cell, respectively). All freshly isolated algae released glycerol and glucose when incubated in host homogenate except E. voratum, which did not release glycerol. These trends mirrored those in hospite. Abstract

Marine organisms that rely on environmental cues for reproduction are likely to experience shifts in reproductive phenology and output due to global climate change. To assess the role that the environment may play in the reproductive timing for temperate sponges, this study examined sexual and asexual reproduction in New Zealand sponge species (Tethya bergquistae and the Tethya burtoni complex) and correlated reproductive output with temperature, chlorophyll-u concentration, and rainfall. Histological analyses of sponges collected monthly (from February 2015 to February 2017) revealed that these sponges are oviparous and gonochoristic and that they sexually reproduce annually during the austral summer. Both monthly collections and in situ monitoring revealed that Tethya spp. asexually bud continuously, but with greater intensity in the austral spring and summer. Temperature was positively associated with both sexual reproduction and budding, with seasonal cues appearing important. Abstract

Objectives: To investigate the effect of the COVID‐19 pandemic on prostate cancer incidence, prevalence, and mortality in England. Patients and Methods: With the approval of NHS England and using the OpenSAFELY‐TPP dataset of 24 million patients, we undertook a cohort study of men diagnosed with prostate cancer. We visualised monthly rates in prostate cancer incidence, prevalence, and mortality per 100 000 adult men from January 2015 to July 2023. To assess the effect of the pandemic, we used generalised linear models and the pre‐pandemic data to predict the expected rates from March 2020 as if the pandemic had not occurred. The 95% confidence intervals (CIs) of the predicted values were used to estimate the significance of the difference between the predicted and observed rates. Results: In 2020, there was a drop in recorded incidence by 4772 (31%) cases (15 550 vs 20 322; 95% CI 19 241–21 403). In 2021, the incidence started to recover, and the drop was 3148 cases (18%, 17 950 vs 21 098; 95% CI 19 740–22 456). By 2022, the incidence returned to the levels that would be expected. During the pandemic, the age at diagnosis shifted towards older men. In 2020, the average age was 71.6 (95% CI 71.5–71.8) years, in 2021 it was 71.8 (95% CI 71.7–72.0) years as compared to 71.3 (95% CI 71.1–71.4) years in 2019. Conclusions: Given that our dataset represents 40% of the population, we estimate that proportionally the pandemic led to 20 000 missed prostate cancer diagnoses in England alone. The increase in incidence recorded in 2023 was not enough to account for the missed cases. The prevalence of prostate cancer remained lower throughout the pandemic than expected. As the recovery efforts continue, healthcare should focus on finding the men who were affected. The research should focus on investigating the potential harms to men diagnosed at older age. Abstract

Grid computing is the concept of harnessing the power of many computational resources in a transparent manner. It is currently an active research area, with significant challenges due to the scale and level of heterogeneity involved. One of the key challenges in implementing grid systems is resource allocation. Currently, centralised approaches are employed that have limited scalability and reliability, which is a key factor in achieving a usable grid system. The field of economics is the study of allocating scarce resources using economic mechanisms. Such systems can be highly scalable, robust and adaptive and as such are a potential solution to the grid allocation problem. There is also a natural fit of the economic allocation metaphor to grid systems, given the diversity of autonomy of grid resources. We propose that an economic system is a suitable mechanism for grid resource allocation. We propose a simple market mechanism to explore this idea. Our system is a fully decentralised economic allocation scheme, which aims to achieve a high degree of scalability and reliability, and easily allows resources to retain their autonomy. We implement a simulation of a grid system to analyse this system, and explore its performance and scalability, with a comparison to existing systems. We use a network to facilitate communication between participating agents, and we pay particular attention to the topology of the network between participating agents, examining the effects of different topologies on the performance of the system.

Abstract

The aim of this study was to investigate the specificity of temperate dinoflagellate - anthozoan symbioses and the influence of any specificity on the percentage contribution of photosynthetically fixed carbon to the host's daily respiratory carbon budget (CZAR). The British symbiotic Anthozoa Cereus pedunculatus (Pennant) I Anthopleura ballii (Cocks), Anemonia viridis (Forskal) and Isozoanthus sulcatus (Gos3e) and the sub-tropical anthozoan Aiptasia pallida (Verrill) and their algal symbionts were investigated. The characteristics of freshly isolated and cultured symbionts were studied to determine whether the algal cells residing in each host species appeared different and how symbiont characteristics may be determined by the host environment. Horphology, division behaviour and photosynthetic pigment composition were examined using SEMO TEMO LMO TLC and HPLC respectively. The symbionts of each host species were identified as dinoflagellates of the genus Symbiodinium, even though there was an absence of a motile phase in the cell cycle of the British hosts. Differences between symbionts of different host species and conspecific hosts were evident with respect to thylakoid arrangement, cell size and cell division rate, and possibly pigment concentrations. Host determined phenotypic plasticity was observed with respect to the absence of a "pellicle". Host-symbiont recognition and specificity were investigated - by reinfecting aposymbiotic C. _pedunculatus with symbionts from different host species and measuring the density and biomass of symbionts at different time intervals. The presence of different strains or species of Symbiodinium was evident, with recognition of these different symbionts being predominantly post-endocytotic. 'Homologous' symbionts were the most 'successful' at repopulating C. pedunculatus. After 9 months, the population densities and biomasses, and division rates of the different 'strains' Of 3ymbionts were the same, suggesting a greater symbiotic integration with 'heterologous, 'strains' with time. The reflection of specificity by the photosynthetic fixation of carbon in vivo was measured using an 02 electrode. 1110mologous, symbiontW photosynthesized at a greater rate than 'heterologous' symbionts in vivo, despite the higher photosynthetic rates of some of these 'heterologous' symbionts in their original hosts. Photosynthate translocation by symbionts in response to different host environments was investigated using in vivo and in vitro 14C labelling and the 'growth rate method'. The percentage translocation was host-symbiont specific and appeared to be controlled by different mechanisms in different host species and conspecific hosts from different locations. The CZAR in 'natural' and reestablished symbioses was calculated. Values ranged from 25.7 to 112.2 % in the 'natural' symbi T9 es and 58.1 to 76.7 % in the reestablished symbioses when measured using C. When measured using the 'growth rate method' values were 73.2 to 188.4 % and 88.3 to 146.5 % respectively. The CZAR in the reestablished symbioses and consequently their ability to survive autotrophically reflected the specific host-symbiont relationship. Symbiosis specificity and the reduced availability of carbon to host growth and reproduction in 'heterologous' symbioses may limit 'novel' symbioses in the field.
