Abstract

BackgroundInitiation of anti-osteoporosis medications after hip fracture lowers the risk of subsequent fragility fractures. Historical biases of targeting secondary fracture prevention towards certain groups may result in treatment disparities. We examined associations of patient age, sex and race with anti-osteoporosis medication prescription following hip fracture.MethodsA cohort of patients with a hip fracture between 2016-2018 was assembled from the American College of Surgeons National Surgical Quality Improvement Program registry. Patients on anti-osteoporosis medications prior to admission were excluded. Multivariable logistic regression was used to determine adjusted associations between patient age, sex and race and their interactions with prescription of anti-osteoporosis medications within 30 days of surgery.ResultsIn total, 12,249 patients with a hip fracture were identified with a median age of 82 years (IQR: 73-87), and 67% were female (n = 8,218). Thirty days postoperatively, 26% (n = 3146) of patients had been prescribed anti-osteoporosis medication. A significant interaction between age and sex with medication prescription was observed (p = 0.04). Male patients in their 50s (OR:0.75, 95%CI:0.60-0.92), 60s (OR:0.81, 95%CI:0.70-0.94) and 70s (OR:0.89, 95%CI:0.81-0.97) were less likely to be prescribed anti-osteoporosis medication compared to female patients of the same age. Patients who belonged to minority racial groups were not less likely to receive anti-osteoporosis medications than patients of white race.InterpretationOnly 26% of patients were prescribed anti-osteoporosis medications following hip fracture, despite consensus guidelines urging early initiation of secondary prevention treatments. Given that prescription varied by age and sex, strategies to prevent disparities in secondary fracture prevention are warranted.

Abstract

Medial meniscal posterior root repair techniques have shown positive yet varied results in the literature. The decision to perform repair has improved clinical outcomes in many situations, although the healing rate is approximately 64% and the repair strength is roughly one-third of the native root strength, with meniscal extrusion being common. We present a technique based on biomechanical evidence to obtain a strong anatomic posterior root repair to restore nearly normal knee mechanics, combining an increased size of footprint under the lateral aspect of the medial meniscal horn for healing and a luggage-tag suture with a posteriorly placed horizontal mattress suture. The horizontal mattress suture is passed to capture the circumferential fibers of the meniscus and the luggage-tag suture is passed to capture the radial fibers of the meniscal body, through a single transtibial tunnel. The aim of this repair is to restore the normal meniscal function.

4. Common peroneal nerve injury in the setting of multi-ligament knee injury: incidence and recovery in a prospective cohort

Abstract

Objectives: Multiligament knee injuries (MLKI’s) are rare, but life-altering injuries which can be made worse with the presence of a concomitant nerve palsy or drop foot. Common peroneal nerve (CPN) palsy has previously been reported to be present in 5-40% of MLKI patients. The purpose of this study was to identify factors that predict CPN palsy at the time of MLKI presentation as well as determine variables that impact likelihood of neurologic recovery. Methods: Between 2006-2017, adults with MLKIs were identified at an academic level 1-trauma center and those with CPN palsy symptoms at presentation were identified. Patients were enrolled in prospective data collection and followed routinely after surgery. The primary outcome was MRC score to analyze associations between age, gender, BMI, injury severity (Schenck classification), surgical timing and presence of fibular head fracture. Secondarily, those with CPN palsy were also compared to a matched cohort of MLKI patients without CPN palsy for demographic as well as functional outcome differences using multiligament quality of life scores (MLQoL). Results: 152 patients were identified with MLKIs, of which 57 presented with a CPN palsy (38%). When comparing those with CPN palsy to those without, there were significantly more males in the CPN palsy cohort (p=0.001), significantly more lateral sided injuries – 84% versus 36% (p/=4). In comparing those with permanent CPN palsies to those who recovered, there was a trend towards significance for those with fibula head fractures being more likely to recover (Chi-squared 3.21, p=0.07). There was no significant difference in age, sex, BMI, sidedness, laterality, or time from presentation to surgery in terms of nerve recovery. There was no significant difference in MLQoL scores between those without CPN palsy, those with CPN palsy which recovered (MRC>/=4) and those with permanent CPN palsy (MRCConclusions: The rate of CPN palsy within MLKI identified in our study matches previously reported literature. Male gender, lateral sided injury and presence of a fibular head fracture were found to be risk factors for CPN palsy at the time of injury. We found no significant difference between demographic or injury mechanism between patients who developed permanent CPN palsy and those who recovered, though there was a trend towards better recovery in those with fibular head fracture. Additionally, we found no difference between patients with CPN palsy compared to those without with respect to MLQoL scores in any domain, which calls into question whether a MLKI patient report outcome measure should incorporate nerve specific questions given the high rate of nerve injuries. The authors intend to analyze the likelihood of recovery in partial versus complete CPN palsy at the time of presentation. To our knowledge this is the largest prospectively collected dataset on CPN palsies after MLKIs, and will provide important insight into neurologic prognostication, especially when combined with previously reported data elsewhere in the literature.

5. Early Versus Delayed Surgery Results In Similar Outcomes Following Multiligament Knee Injury; A Prospective Cohort

Abstract

Objectives: Multiligament knee injuries (MLKI’s) are rare, but life-altering injuries that have significant implications on patients’ functional status, quality of life and return to work and sport. While the treatment of these injuries is far from standardized, there exists good evidence to support operative intervention. Unfortunately there are no guidelines to indicate the ideal timing of surgery after injury to optimize clinical outcomes and patient satisfaction. With that in mind, the aim of this study was to assess injury specific functional outcomes following surgery for MKLIs and identify advantages to early versus delayed surgery. Methods: Between 2006-2017, adults with MLKIs were identified at an academic level 1-trauma center. Patients were enrolled in prospective data collection and followed routinely after surgery. Study participants were stratified into early (45 days from injury). The primary analysis was patient reported outcomes in the form of a specific and validated score (MLQOL) to compare early vs delayed surgical intervention. We secondarily analyzed associations between age, gender, BMI, injury severity (Schenck classification), associated nerve injuries, and reoperation rates to our primary outcome measure. Results: A total of 152 patients were identified, with 85 patients in the early group and 67 in the delayed. The mean time to surgery was 16.7 ± 7.9 days in the early group versus 253 ± 132.6 days in the delayed. Average follow-up was 46.7 months. There was no significant difference between groups with respect to age, gender distribution, BMI, injury mechanism, nor severity. The early surgery group was found to include more patients with lateral sided injuries (p=0.010) as well as nerve injuries at initial presentation (p=0.002). The delayed group was found to have more patients with PCL involvement compared to early surgery (P=0.003). We did not identify a significant difference in pain, stiffness, or instability patient reported outcome scores between the early and delayed surgical groups. 49 of the 152 patients underwent a repeat operation (32%) including manipulation under anesthesia, arthroscopic debridement, removal of hardware, revision and/or conversion to total knee arthroplasty. No significant difference was found in reoperation rates between the two groups. In our primary outcome, we did not identify any relationship between early or late surgery and MLQOL scores. In our secondary analysis, using a linear regression model, we determined older age to be independently predictive of poor outcomes with respect to pain (p=0.018), stiffness (p=0.048) and instability (p=0.004) as assessed through MLQOL questionnaire. Conclusion: In our analysis, stratifying patients to early vs delayed surgery had no effect patient reported outcomes following MLKI reconstruction. In the secondary outcome measures, we identified that regardless of time from injury to surgical intervention, older age was independently predictive of poor pain, stiffness, and instability scores in the MLQOL questionnaire. To our knowledge this is the largest prospectively collected dataset of injury specific outcomes in MLKIs. This analysis will allow for an evidence based approach to guide discussion, manage expectations, and predict outcomes of MLKI patients as well as provide the framework for how surgeons manage these significant injuries.

Abstract

Objectives: Arthroscopic partial meniscectomy is one of the most commonly performed procedures in orthopaedic surgery. However, information on the threshold at which patients consider themselves to be well for patient reported outcome measures (PROMs) after this surgery remains limited. Our goal was to determine the patient acceptable symptomatic state (PASS) for the Knee Injury and Osteoarthritic Outcome Score (KOOS), the International Knee Documentation Committee (IKDC) Subjective Knee Form, the Western Ontario Meniscal Evaluation Tool (WOMET) and the Marx Activity Scale (MAS) in patients with knee meniscal pathology who treated with partial knee meniscectomy. Methods: A consecutive series of patients with knee meniscal pathology treated with arthroscopic partial meniscectomy plus or minus intra-articular debridement were eligible. Other inclusion criteria were: a Kellegren-Lawrence Grade of 0-2, and ligamentous integrity. The KOOS (0-100, 5 subscales), IKDC (0-100), WOMET (0-100) and MAS (0-16) were administered at baseline and 12 months postoperatively. An external anchor question at 1 year postoperatively was utilized to determine PASS values: “Taking into account all the activities you have during your daily life, your level of pain, and also your functional impairment, do you consider that your current state is satisfactory?” A receiver operator curve analysis was used to determine the PASS value at which patients considered their status to be satisfactory. Results: There were 115 patients (mean ± SD age, 53.8 ± 12.0 years), and 57.3% were male. Based on a receiver operator curve analysis, the PASS values - at which patients considered their status to be satisfactory - at 1 year after surgery were 43 (KOOS-symptoms subscale), 83 (KOOS-pain subscale), 84 (KOOS-functions of daily living subscale), 75 (KOOS-function, sport and recreational activity subscale), 56 (KOOS-quality of life subscale), 56 (IKDC), 61 (WOMET), 7 (MAS). The PASS threshold was not affected by baseline scores across the different instruments and there was no relationship between baseline score and likelihood of achieving the PASS. Age and sex were not significantly related to the odds of achieving the PASS for any of the PROMs. Conclusion: This is the first study to determine PASS in four commonly used knee-related PROMs in patients undergoing arthroscopic partial meniscectomy. The findings can allow researchers and clinicians to determine if partial meniscectomy is meaningful to patients and will be helpful for responder analysis in future trials related to knee arthroscopy and the treatment of meniscal pathology.

Item does not contain fulltext BACKGROUND: The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) has been extensively evaluated in groups of patients with osteoarthritis, yet not in patients with a femoral neck fracture. This study aimed to determine the reliability, construct validity, and responsiveness of the WOMAC compared with the Short Form-12 (SF-12) and the EuroQol 5D (EQ-5D) questionnaires for the assessment of elderly patients with a femoral neck fracture. METHODS: Reliability was tested by assessing the Cronbach alpha. Construct validity was determined with the Pearson correlation coefficient. Change scores were calculated from ten weeks to twelve months of follow-up. Standardized response means and floor and ceiling effects were determined. Analyses were performed to compare the results for patients less than eighty years old with those for patients eighty years of age or older. RESULTS: The mean WOMAC total score was 89 points before the fracture in the younger patients and increased from 70 points at ten weeks to 81 points at two years postoperatively. In the older age group, these scores were 86, 75, and 78 points. The mean WOMAC pain scores before the fracture and at ten weeks and two years postoperatively were 92, 76, and 87 points, respectively, in the younger age group and 92, 84, and 93 points in the older age group. Function scores were 89, 68, and 79 points for the younger age group and 84, 71, and 73 points for the older age group. The Cronbach alpha for pain, stiffness, function, and the total scale ranged from 0.83 to 0.98 for the younger age group and from 0.79 to 0.97 for the older age group. Construct validity was good, with 82% and 79% of predefined hypotheses confirmed in the younger and older age groups, respectively. Responsiveness was moderate. No floor effects were found. Moderate to large ceiling effects were found for pain and stiffness scales at ten weeks and twelve months in younger patients (18% to 36%) and in the older age group (38% to 53%). CONCLUSIONS: The WOMAC showed good reliability, construct validity, and responsiveness in both age groups of elderly patients with a femoral neck fracture who had been physically and mentally fit before the fracture. The instrument is suitable for use in future clinical studies in these populations. CLINICAL RELEVANCE: The results are based on two clinical trials. The questionnaires used concern pure, clinically relevant issues (ability to walk, climb stairs, etc.). Moreover, the results can be used for future research comparing clinical outcomes (or treatments) for populations with a femoral neck fracture.

9. The Protective Effect of Kevlar ® Socks Against Hockey Skate Blade Injuries: A Biomechanical Study

Abstract

Objectives: Several recent high profile injuries to elite players in the National Hockey League (NHL) secondary to skate blade lacerations have generated significant interest in these injuries and possible methods to protect against them. These injuries are typically due to direct contact of the skate blade of another player with posterior aspect of the calf resulting in a range of potential injuries to tendons or neurovascular structures. The Achilles tendon is most commonly involved. Kevlar® reinforced socks have recently become available for hockey players to wear and are cited as providing possible protection against such injuries. However, there has been no investigation of the possible protective effects of Kevlar® reinforced socks against skate blade injuries, and it is currently unknown what protective effects, if any, that these socks provide against these injuries. The proposed study sought to address this by conducting a biomechanical investigation of the protective effects of Kevlar® reinforced socks against Achilles tendon injuries in a simulated model of skate blade injury using human cadaver limbs. This novel investigation is the first to address the possible benefits to hockey players of wearing Kevlar® reinforced socks. Methods: Seven matched pairs of human cadaver lower limbs were fitted with a Kevlar ® reinforced sock comprised of 60% Kevlar®/20% Coolmax® polyester/18 % Nylon/12% Spandex ( Bauer Elite Performance Skate Sock) on one limb and a standard synthetic sock comprised of 51% polyester/47% nylon/2% spandex ( Bauer Premium Performance Skate Sock) on the contralateral limb as a control. Each limb was then mounted on a Materials Testing System (MTS) with the ankle dorsiflexed to 90° and the knee held in full extension using a custom designed jig. Specimens were then impacted with a hockey skate blade directed at the posterior calf, 12 cm above the heel, at an angle of 45° and a speed of 31m/s, to a penetration depth of 4.3 cm, to simulate a typical ice hockey injury. Peak force, energy and power were calculated from the force-displacement data collected from the 7 matched pair trials. The cadavers were then dissected to identify the extent of the injury the skin and Achilles tendon from blade impact. Analysis of variance was used to test for a significant difference between the groups. Results: None (0/7) of the achilles tendons were lacerated when protected with Kevlar® reinforced socks; whereas all (7/7) achilles tendons tested using the standard synthetic sock were completely severed (Figure 1). Peak force (4030 +/- 1191 N vs. 2037 +/- 729 N), energy (81.4 +/- 38.9 J vs. 26.3 +/- 13.2 J) and power (471.2 +/- 166.7 W vs. 258.3 +/- 93.5 W) were all significantly (pConclusion: The Kevlar® reinforced socks provided significantly more cut resistance and were able to withstand a significantly larger peak force, energy and power from skate blade impact and prevent achilles tendon laceration when compared to standard synthetic hockey socks in a biomechanical testing model using human cadaver limbs. This is the first investigation to address the benefits of wearing Kevlar® reinforced hockey socks in a simulated model of hockey skate injuries and our results suggest a significant protective effect from the use of Kevlar® reinforced socks against hockey skate injuries.

10. Pre-operative Femoral Nerve Block in Hip Arthroscopy—A Retrospective Review of 108 Consecutive Cases

Abstract

Objectives: To evaluate the safety and efficacy of pre-operative femoral nerve block for post-operative pain control in patients undergoing hip arthroscopy. Methods: A retrospective chart review of 108 consecutive hip arthroscopy cases (in 103 patients) was carried out. All patients underwent hip arthroscopy at a single academic centre by two orthpaedic surgeons with advanced fellowship training in hip arthroscopy. All patients underwent arthroscopy under general anaesthetic with our without pre-operative femoral nerve block. Patients were separated into two groups based on the presence or absence of a pre-operative femoral nerve block prior to induction of general anesthesia. Groups were compared with respect to patient gender, age, body mass index (BMI), ASA Physical Status classification, procedure performed, operative time, total intra-operative morphine-equivalent dose, post-anesthesia care unit (PACU) pain scores (0 to 10) recorded at 0, 15, 30, 45, and 60 minutes post-operatively, total PACU morphine-equivalent dose, presence of nausea or vomiting in the PACU, time to discharge from PACU, oxycodone consumption in the surgical day care unit (SDCU), and highest pain score (0 to 10) in the SDCU prior to discharge. Results: Twelve cases were excluded from analysis for a total of 96 cases (in 92 patients). Forty patients had general anesthesia alone (Group A) and 56 patients had a pre-operative femoral nerve block prior to the induction of general anesthesia (Group B). There was no significant difference between the groups with regard to gender, age, weight, height, BMI, ASA classification, or type of procedure performed. Patients who received a femoral nerve block had a significantly shorter operative time and received a significantly lower total intra-operative morphine-equivalent dose than those patients who did not receive a block. Post-operative pain scores were lower at all time points for the femoral nerve block group, however statistical significance was seen only at the 60 minute post-operative time point (p < 0.05). Patients who did not receive a block had significantly higher morphine-equivalent doses in the PACU. There was no difference in rates of nausea and vomiting and time to discharge from the PACU between the two groups. Oxycodone consumption in the surgical day care unit (SDCU) was similar between groups, but the femoral nerve block patients had significantly lower pain scores prior to discharge from the SDCU. Two patients in Group A group were admitted to hospital post-operatively due to inadequate post-operative pain control. No complications were noted in any patient with regard to the femoral nerve block. Conclusion: A pre-operative femoral nerve block is a safe procedure that may decrease the requirement for intra-operative morphine while providing effective post-operative pain control in patients undergoing hip arthroscopy.

Abstract

Purpose To address the factors that surgeons use to decide between 2 options for treatment when the evidence is inconclusive. Methods We tested the null hypothesis that the factors surgeons use do not vary by training, demographics, and practice. A total of 337 surgeons rated the importance of 7 factors when deciding between treatment and following the natural history of the disease and 12 factors when deciding between 2 operative treatments using a 5-point Likert scale between "very important" and "very unimportant." Results According to the percentages of statements rated very important or somewhat important, the most popular factors influencing recommendations when evidence is inconclusive between treatment and following the natural course of the illness were "works in my hands," "familiarity with the treatment," and "what my mentor taught me." The most important factors when evidence shows no difference between 2 surgeries were "fewer complications," "quicker recovery," "burns fewer bridges," "works in my hands" and "familiarity with the procedure." Europeans rated "works in my hands" and "cheapest/most resourceful" of significantly greater importance and "what others are doing," "highest reimbursement," and "shorter procedure" of significantly lower importance than surgeons in the United States. Observers with fewer than 10 years in independent practice rated "what my mentor taught me," "what others are doing" and "highest reimbursement" of significantly lower importance compared to observers with 10 or more years in independent practice. Conclusions Surgeons deciding between 2 treatment options, when the evidence is inconclusive, fall back to factors that relate to their perspective and reflect their culture and circumstances, more so than factors related to the patient's perspective, although this may be different for younger surgeons. Clinical relevance Hand surgeons might benefit from consensus fallback preferences when evidence is inconclusive. It is possible that falling back to personal comfort makes us vulnerable to unhelpful commercial and societal influences.

Abstract

Acute knee dislocation is rare but has a high rate of associated neurovascular injuries and potentially limb-threatening complications. These include the substantial morbidity associated with peroneal nerve injury: neuropathic pain, decreased mobility, and considerably reduced function, which not only impairs patient function but complicates treatment.We therefore identified and quantified the risks associated with specific factors for peroneal nerve injury and recovery in patients with knee dislocations.We retrospectively reviewed the charts of 26 patients, from among a cohort of all 91 knee dislocations, with a peroneal nerve palsy over a 5-year period. We then used univariable and multivariable statistics to identify risk factors predicting peroneal nerve injury and recovery.Gender (odds ratio, 5.47), body mass index (odds ratio, 1.14), and fibular head fracture (odds ratio, 4.77) were associated with peroneal nerve injury. Only younger age was associated with peroneal nerve recovery.Knowledge of the risk factors for peroneal nerve injury and the predictors of recovery in knee dislocation allows the treating surgeon to have a better understanding of the nature of the neurologic injury and modify management based on the anticipated return of nerve function.Level II, prognostic study.
