After stroke, the effects of focused scapulo-humeral training with simultaneous assessment of the changes in shoulder subluxation, related muscle thicknesses and upper limb performance have not been studied in the literature. This study aimed to investigate the effects of an 8-week scapulo-humeral training program in addition to conventional rehabilitation on upper extremity/trunk functions, shoulder pain, and sonographic measurements of the shoulder joint and periscapular muscles. Thirty stroke individuals were randomly separated into two groups as Group I-scapulo-humeral training (5F/10 M) and Group II – control (5F/10 M). Conventional rehabilitation program was applied to both groups, and a scapulo-humeral training exercise protocol was added for the scapulo-humeral group. All the treatments were applied for 1 hour/day, 3 days/week, 8 weeks. Clinical evaluations were made using the Fugl Meyer Assessment-Upper Extremity(FMA-UE), Action Research Arm Test(ARAT), ABILHAND, Visual Analog Scale, and Trunk Impairment Scale(TIS). Ultrasound was used to measure serratus anterior/lower trapezius muscle thicknesses, and acromion-greater tubercule/acromio-humeral distances. FMA-UE, ARAT, ABILHAND and TIS scores increased in both groups, with greater increases in most parameters in the scapulo-humeral training group. Shoulder pain decreased only in the scapulo-humeral training group. Subacromial distances were decreased on the paretic side, and muscle thicknesses increased on both sides in the scapulo-humeral training group, and in the control group, only serratus anterior muscle thickness increased on the paretic side (p < 0.05 for all). Additional scapulo-humeral exercises to conventional rehabilitation was seen to improve upper extremity/trunk performance and shoulder pain, and to increase scapula stabilizer muscle thicknesses in stroke individuals with mild-moderate upper extremity disability. Objective There is no consensus about the optimal frequency of patient visits during the rehabilitation program after arthroscopic rotator cuff repair (ARCR). This study aimed to investigate the short- and long-term effects of high-frequency (HF) and low-frequency (LF) visits of the patients in the first 12 weeks of rehabilitation after ARCR. Methods This was a quasi-randomized study with 2 parallel groups. Forty-seven patients with ARCR were included in 2 different patient visit frequency protocols (HF = 23, LF = 24) in 12 weeks of postoperative rehabilitation. Patients in the HF group visited the clinic twice a week, whereas patients in the LF group visited once every 2 weeks for the first 6 weeks and once a week for the following 6 weeks. Both groups performed the same exercise protocol. Outcome measurements were pain and range of motion measured at baseline; at the 3rd, 5th, 8th, 12th, and 24th weeks; and at 1-year follow-up. Shoulder function was assessed at the 12th and 24th weeks and at 1-year follow-up with an American Shoulder and Elbow Surgeons score. Results There was a significant group × time interaction in pain intensity during the activity between the groups. The activity pain intensity was higher in the LF group (4.2 points) at 8 weeks post surgery than in the HF group (2.7 points) (mean difference: 1.5 points, P  < .05), whereas it was similar in both groups at other time periods. The interaction term was not significant between the groups for pain intensity during rest and night through the 1-year follow-up. No group × time interactions were observed in shoulder range of motion and American Shoulder and Elbow Surgeons score over the postoperative period. Conclusion Both rehabilitation programs at different visit frequencies after ARCR showed similar clinical results in the long term. A supervised, controlled rehabilitation program with LF visits in the first 12 weeks after surgery can be sufficient to achieve optimal clinical results and reduce rehabilitation-related costs after ARCR. Impact This study highlights that LF treatment protocols under the supervision of the therapist can be adopted after the arthroscopic rotator cuff repair to achieve successful results while decreasing the treatment costs. Physical therapists should plan the treatment sessions efficiently for the compliance of the patients to the exercise treatment. Lay Summary If you are a patient with arthroscopic rotator cuff repair, a supervised, controlled rehabilitation program with low-frequency visits in the first 12 weeks after surgery could help you achieve the best outcome and help lower the costs of rehabilitation. The wall-slide exercise is commonly used in clinic and research settings. Theraband positioning variations for hip exercises have been investigated and used, but Theraband positioning variations for upper extremity wall-slide exercises, although not commonly used, have not been examined. To evaluate the effect of different Theraband positions (elbow and wrist) on the activation of the scapular and shoulder muscles in wall-slide exercises and compare these variations with each other and with regular wall-slide exercises for the upper limbs. Descriptive laboratory study. University laboratory. A total of 20 participants (age = 23.8 ± 3 years, height = 176.5 ± 8.14 cm, mass = 75.3 ± 12.03 kg, body mass index = 24.23 ± 4.03) with healthy shoulders. Participants performed wall-slide exercises (regular and 2 variations: Theraband at the elbow and Theraband at the wrist) in randomized order. Surface electromyographic activity of the trapezius (upper trapezius [UT], middle trapezius [MT], and lower trapezius [LT]), infraspinatus, middle deltoid (MD), and serratus anterior (SA) muscles. Regular wall-slide exercises elicited low activity in the MD and moderate activity in the SA muscles (32% of maximal voluntary isometric contraction [MVIC] in the SA), whereas the Theraband-at-elbow and Theraband-at-wrist variations elicited low activity in the MT, LT, infraspinatus, and MD muscles and moderate activity in the SA muscle (46% and 34% of MVIC in the SA, respectively). The UT activation was absent to minimal (classified as 0% to 15% of MVIC) in all wall-slide exercise variations. The Theraband-at-wrist variation produced lower UT:MT, UT:LT, and UT:SA levels compared with the regular wall-slide exercise and Theraband-at-elbow variation. In shoulder rehabilitation, clinicians desiring to activate the scapular stabilization muscles should consider using the Theraband-at-wrist variation. Those seeking more shoulder-abduction activation and less scapular stabilization should consider using the Theraband-at-elbow variation of the upper extremity wall-slide exercise. The aim of this study is to investigate the effects of scapular taping on scapular kinematics by three-dimensional electromagnetic system during shoulder elevation in facioscapulohumeral muscular dystrophy patients. A total of 11 patients with facioscapulohumeral muscular dystrophy were included in the study. Scapular anterior-posterior tilt, upward-downward rotation, and internal-external rotations were evaluated using the three-dimensional electromagnetic system during the elevation of the upper limbs in the scapular plane before and after kinesio taping. For maximum humerothoracic elevation, there were no differences between the patients before and after taping on both dominant (p = 0.72) and non-dominant sides (p = 0.64). For scapular internal rotation, upward rotation, and posterior tilt, there were no differences between patients before and after taping during humerothoracic elevation on both dominant and non-dominant sides (p > 0.05). These results showed us that the excessive and abnormal movements of the scapula observed during the humeral elevation in facioscapulohumeral muscular dystrophy patients cannot be supported with flexible methods like kinesio taping. Therefore, we recommend to evaluate the scapula position by applying flexible and rigid taping to the patients who can reach over 90o in humerothoracic elevation in future studies. Aim: The aim of the study is to define the profile of the children with neurodevelopmental disabilities who are referred to a pediatric rehabilitation clinic and to identify the risk factors related to their problems. Material and Method: Three hundred and ninety-six children and their parents who were referred to a rehabilitation clinic for special education, physiotherapy and rehabilitation between the years April 2000 and April 2012 were included in this study. All their records were analyzed retrospectively. Results: The 52.8% (n=209) of the children were boys and 47.2% (n=187) were girls. Number of children with Cerebral Palsy (CP) were in the first raw and children with Mental Retardation (MR) were the second in children with neurodevelopment disabilities. Quadriparetic type of CP was the most both in spontaneous cervicovaginal and caesarian deliveries. There were no statistical difference between the gestational age and types of CP (p<.05). Types of CP were different according to the type of delivery (z=-3.021, p=0.003). The 32.8% of mothers and 73.5% of the fathers were working. The 92.2% of mothers underwent regular doctor control during pregnancy, 97% of them delivered in an hospital, 51% did normal cervicovaginal delivery and 49% caesarian section. Conclusions: This is the first and only study which defines the profile of the children with neurodevelopment disorders who are referred to pediatric rehabilitation clinics. The results of this study will contribute to define the needs of the children and their families who apply to a pediatric rehabilitation clinic and will help to identify the framework and concept of the special education, physiotherapy and rehabilitation according to their status and needs. The article focuses on Lymphedema, a common problem for patients diagnosed with breast cancer. It states that static postural stability and fall risk were evaluated by the Biodex Balance System. It compares on the static postural stability and the fall risk in pre-elderly women with unilateral secondary lymphedema after breast cancer and health controls. It concludes that it is the first study that shows how lymphedema effected the postural stability.

Foam rolling have gained popularity among elite athletes, but the effect of the speed parameter of foam rolling has not yet been determined. Our objective was to investigate the impact of different application speeds of foam roller on the mechanical properties of the quadriceps femoris muscle. Eighteen male professional basketball athletes (age 23 ± 4 years, body mass index 24.43 ± 1.59 kg/m²) participated in this study. We used a crossover design to randomize the order of the treatment speeds (30 beats per minute-FAST, 15 beats per minute-SLOW, and a self-determined speed-SELF) with a one-week washout period between each session. We measured dominant quadriceps femoris muscle tone, elasticity, and stiffness using the Myoton device before and after the interventions. We found that the average rate for SELF was 33±10 beats per minute, making SELF the fastest. All application speeds showed similar results in pre-intervention measurements of the mechanical properties of the tissues (P > 0.05). However, post hoc analysis revealed that a decrease was evident in SLOW compared to SELF in muscle tone in post-intervention measurements (P = 0.037). Also, we noted that comparison of pre- and post-intervention on FAST and SLOW showed a significant reduction in muscle tone (P = 0.002, P = 0.008). Slower foam rolling prior to training or competition may lead to a delay in the reaction time due to the reduction in tonus, that can increase the injury risks. Background: Shoulder pain is a common symptom for non-traumatic shoulder pathologies and affects 4.7-66.7% of entire population. Even with the latest technological advantages, palpation still is a cost and time efficient tool to evaluate the shoulder pathologies. Our aim was to investigate and compare palpation findings in different shoulder pathologies.Method: We used first assessments of impingement syndrome (IS) (n = 205), rotator cuff tear (RCT) (n = 185) and frozen shoulder (FS) (n = 210) patients who received treatment between 2010 and 2019 years. Two experienced physiotherapists palpated predefined points: long head of biceps brachii, pectoralis minor, lateral intermuscular septum, proximal tendons of extensor group. These points categorized into "proximal", "mid-proximal", "mid-distal", "distal", "irregular". We also grouped patients by their BMIs.Results: We found RCT patients had higher BMI than IS and FS patients (p < 0.001, p = 0.001); more tender intermuscular septum points and night pain than IS patients (p = 0.001, p = 0.003) and more extensor group proximal tendon points than IS and FS patients (p < 0.001, p = 0.001). "No tenderness" group had lesser resting pain than distal group (p < 0.001) and lesser activity and night pain than middle-proximal, middle-distal, and distal groups (all ps < 0.002). Distal group had higher night pain than middle-proximal group (p = 0.003). Morbidly obese patients had higher night pain than normal and overweight patients (p = 0.003 and p = 0.009).Conclusions: Tender point distribution varies depending on the pathology with biceps brachii being the most common. RCT patients had higher night pain related to more distal tender points. Patients with higher night pain had increased tenderness in septum intermuscular and extensor muscles' proximal tendons. Background: In air pistol shooters, the number of studies evaluating the physical characteristics is very few. The purpose of this study was to examine the relationship of core, scapula, and shoulder girdle muscles endurance on performance in air pistol shooters and to determine the importance of the endurance parameters., Methods: Twenty-eight male air pistol shooters were included. The Closed Kinetic Chain Upper Extremity Stability Test, Upper Quarter Y Balance Test, Prone Bridge Test, Side Bridge Endurance Test, and The Scapular Muscle Endurance Test were used to evaluate the endurance of shoulder girdle, core, and scapular muscles. The relationships between endurance and shooting performance parameters were investigated., Results: A positive moderate correlation was found between the scapular muscle endurance test and the shooting performance (P=0.002 r=0.638). A significant regression equation was found for shooting scores (F(1.26)=11.975, P=0.02, R2=0.315). It was observed that the shooting score increased by 0.375 points for each unit increase of the Scapular Muscle Endurance Test. No statistically significant relationship was observed in other parameters (P>0.05)., Conclusions: In this study, core and shoulder girdle muscle endurances were not associated with shooting performance. Scapular muscle endurance may contribute positively to shooting performance. An exercise program to increase scapular muscles endurance can be beneficial for the performance of shooters.

Context:   Performing shoulder-abduction exercises with scapular retraction has been theorized to reduce the potential for shoulder impingement. However, objective data to support this premise are lacking., Objective:   To evaluate the influence of active scapular retraction on acromiohumeral distance (AHD) at 4 shoulder-abduction angles using real-time ultrasound., Design:   Cross-sectional study., Setting:   University laboratory., Patients or Other Participants:   Twenty asymptomatic individuals (10 men, 10 women; age = 22.9 ± 2.8 years, height = 169.3 ± 9.5 cm, mass = 65.5 ± 12.9 kg) were recruited., Main Outcome Measure(s):   Real-time ultrasound images of AHD were obtained during nonretracted and retracted scapular conditions at 0°, 45°, 60°, and 90° of shoulder abduction. A 2-factor analysis of variance with repeated measures was used to evaluate the influence of shoulder retraction on AHD across shoulder-abduction angles., Results:   A scapular-retraction condition × shoulder-abduction-angle interaction for AHD was found ( F 3,57 = 4.56, P = .006). The AHD was smaller at 0° (10.5 versus 11.2 mm, respectively; t 19 = 2.22, P = .04) but larger at 90° (9.4 versus 8.7 mm, respectively; t 19 = -2.30, P = .04) of shoulder abduction during the retracted than the nonretracted condition. No differences in AHD were observed between conditions at 45° ( t 19 = 1.45, P = .16) and 60° ( t 19 = 1.17, P = .86) of abduction., Conclusions:   The observed differences in AHD at 0° and 90° of shoulder abduction were small and did not exceed the established minimal detectable change for either angle. Our findings suggest that active scapular retraction during shoulder abduction has a minimal influence on AHD at 0° and 90° in healthy individuals. Further investigations are needed to determine whether scapular retraction influences AHD in individuals with subacromial impingement.

Background/aim: Our aim was to investigate the association of kinesiophobia with pain, quality of life, clinical measures, and depression in patients with ankylosing spondylitis (AS). Materials and methods: Patients with AS (n: 163) were enrolled in the study. Kinesiophobia was evaluated with the Tampa Scale of Kinesiophobia (TSK), pain with a visual analog scale (VAS), mobility with Bath Ankylosing Spondylitis Metrology Index (BASMI), disease activity with the Bath AS Disease Activity Index (BASDAI), functional status with the Bath AS Functional Index (BASFI), depression level with Beck's Depression Inventory (BDI), and quality of life with the AS Quality of Life Questionnaire (ASQoL). Besides correlation analysis, outcome measures were compared between patients with (TSK ? 37) and without (TSK < 37) kinesiophobia. Results: Kinesiophobia was found to be common with a percentage of 66.6%. TSK scores were correlated with VAS, BASFI, ASQoL, and BDI scores (r = 0.259, r = 0.294, r = 0.392, and r = 0.398, respectively; P < 0.05 for each). There were no correlations between TSK and the BASDAI and BASMI scores (r = 0.142, r = 0.198, respectively; P > 0.05). Patients with kinesiophobia had more pain and poorer BASFI, ASQoL, and BDI scores than patients without kinesiophobia (P < 0.05).Conclusion: Our study is the first study that investigated the relationship between kinesiophobia and other clinical measures in AS patients. Pain and psychological status have an impact on fear of movement and thus functional status and quality of life.

Objective: The aim of this study was to compare body fat mass, muscular endurance, multi-joint coordination and proprioception between sedentary adult women with and without knee pain., Methods: This cross-sectional study included 149 women. All participants were evaluated using the Visual Analog Scale to determine knee pain during specific functions and divided into 2 groups accordingly. The knee pain group (n=52; mean age: 42.6±4.1 years; mean height: 1.56±5.11 m; mean weight: 75.2±14.1 kg) included patients with mild to moderate knee pain scores. The without knee pain group (n=97; mean age: 41.7±4.2 years; mean height: 1.58±5.06 m; mean weight: 73.4±10.6 kg) included cases who reported no pain. Body composition was assessed using the TANITA Body Composition Analyzer. Concentric maximal torque of the knee flexor and extensor muscles was recorded using an Isomed 2000 isokinetic dynamometer at 180 deg/s. Coordination and proprioception were assessed using the Functional Squat System., Results: There was no significant difference in terms of physical characteristics, body composition parameters and coordination results between groups (p>0.05). Peak torque of flexion (0.65±0.21 N/kg) and extension (0.88±0.23 N/kg) of the affected side in the knee pain group were significantly lower than the without knee pain group dominant side flexion (0.74±0.19 N/kg) and extension (0.98±0.19 N/kg) (p<0.05). A significant difference in knee extension was observed between the affected (0.88±0.23 N/kg) and non-affected sides (0.93±0.21 N/kg) of the knee pain group (p<0.05). There were no significant differences for both legs between groups in terms of coordinative concentric side-to-side deficits and eccentric deficits (p>0.05). The deviation on visible movement for proprioception was significantly higher in the knee pain group (3.23±1.01 cm) than in the without knee pain group (2.78±1.03 cm) (p=0.012)., Conclusion: Knee pain impairs flexor and extensor peak torques of knee endurance and multi-joint proprioceptive accuracy in sedentary woman. No differences were observed in terms of body composition and joint coordination of the groups with or without knee pain.

Objective: The aim of the study was to investigate the effects of the early initiation of passive and active range of motion exercises following arthroscopic rotator cuff repair., Methods: The study included 40 patients who underwent arthroscopic rotator cuff repair. Patients were quasi-randomly assigned into accelerated (ACCEL) protocol (n=19) and slow (SLOW) protocol (n=21) groups. Patients in both groups were treated with the same protocol. Active range of motion was begun at the 3rd week in the ACCEL group and the 6th week in the SLOW group. Range of motion was recorded at postoperative weeks 3, 5, 8, 12, and 24., Results: While active range of motion for all measurements improved across weeks, there were no differences between groups, with the exception of active total elevation which was greater at all time point measurements in the ACCEL group (p<0.05)., Conclusion: The early initiation of passive and gentle controlled active motion exercise following rotator cuff repairs does not appear to affect range of motion in the first 6 postoperative months.

Objectives: This study aims to compare the efficacy of manual therapy in the frozen shoulder patients with or without diabetes mellitus., Patients and Methods: Between May 2006 and January 2008, 50 patients (10 males, 40 females; mean age 52 ± 10 years; range 40 to 65 years) orthopedic surgeons referred to the Sports Physiotherapy Unit were included in the study. The patients were divided into two groups, including patients with primary frozen shoulder with type II diabetes mellitus (n=12) and non diabetics (n=38). All patients underwent a rehabilitation program including cold application, manual therapy and exercises twice a week. A total of 16 treatment sessions were performed. The most important part of the manual therapy included scapular mobilization and posterior capsule stretching. The range of motion was measured by goniometry. Functional activity status was assessed by Constant's score. The pain level was evaluated by visual analog scale, while muscle strength was evaluated by hand-held dynamometer. Student t-test was used to compare between the parameters of groups, while Paired sample t-test was used to compare pre- and post-treatment parameters of the patients., Results: The range of motion, functional activity status and muscular strength were improved and the pain level was reduced after rehabilitation in all of the patients in both groups (p<0.05). There was no difference in duration of the treatment between the groups (p>0.05). There were no differences in range of motion, functional activity status, pain level, and muscle strength before and after rehabilitation between the groups (p>0.05)., Conclusion: Manual therapy approaches may be safely applied in diabetic patients with frozen shoulder.

Objectives: In this study, we sought to compare the effects of the slow and accelerated protocols on pain and functional activity level after arthroscopic rotator cuff repair., Methods: The study included 29 patients (3 men, 26 women) who underwent arthroscopic repair of stage 2 and 3 rotator cuff tears. Patients were randomized in two groups: the accelerated protocol group (n=13) and slow protocol group (n=16). Patients in the accelerated protocol group participated in a preoperative rehabilitation program for 4-6 weeks. Patients were evaluated preoperatively and for 24 weeks postoperatively. Pain was assessed by visual analog scale, and functional activity level was assessed by The Disabilities of The Arm Shoulder and Hand (DASH) questionnaire. The active range of motion was initiated at week 3 after surgery for the accelerated rehabilitation protocol and at week 6 for the slow protocol. The rehabilitation program was completed by the 8th week with the accelerated protocol and by the 22nd week with the slow protocol., Results: There was no significant difference between the slow and accelerated protocols with regard to pain at rest (p>0.05). However, the accelerated protocol was associated with less pain during activity at weeks 5 and 16, and with less pain at night during week 5 (p<0.05). The accelerated protocol was superior to the slow protocol in terms of functional activity level, as determined by DASH at weeks 8, 12, and 16 after surgery (p<0.05)., Conclusion: The accelerated protocol is recommended to physical therapists during rehabilitation after arthroscopic rotator cuff repair to prevent the negative effects of immobilization and to support rapid reintegration to daily living activities.
