ABSTRACT Assessing the bacterial community composition across cacao crops is important to understand its potential role as a modulator of cadmium (Cd) translocation to plant tissues under field conditions; Cd mobility between soil and plants is a complex and multifactorial problem that cannot be captured only by experimentation. Although microbes have been shown to metabolize and drive the speciation of Cd under controlled conditions, regardless of the link between soil bacterial community (SBC) dynamics and Cd mobilization in the rhizosphere, only a few studies have addressed the relationship between soil bacterial community composition (SBCC) and Cd content in cacao seeds (Cdseed). Therefore, this study aimed to explore the association between SBCC and different factors influencing the distribution of Cd across cacao crop systems. This study comprised 225 samples collected across five farms, where we used an amplicon sequencing approach to characterize the bacterial community composition. The soil Cd concentration alone (Cdsoil) was a poor predictor of Cdseed. Still, we found that this relationship was more apparent when the variation within farms was controlled, suggesting a role of heterogeneity within farms in modulating Cd translocation and, thus, seed Cd content. Our results provide evidence of the link between soil bacterial communities and the distribution of Cd across Colombian cacao crops, and highlight the importance of incorporating fine-spatial-scale studies to advance the understanding of factors driving Cd uptake and accumulation in cacao plants.IMPORTANCECadmium (Cd) content in cacao crops is an issue that generates interest due to the commercialization of chocolate for human consumption. Several studies provided evidence about the non-biological factors involved in its translocation into the cacao plant. However, factors related to this process, including soil bacterial community composition (SBCC), still need to be addressed. It is well known that soil microbiome could impact compounds’ chemical transformation, including Cd, on the field. Here, we found the first evidence of the link between soil bacterial community composition and Cd concentration in cacao soils and seeds. It highlights the importance of including the variation of bacterial communities to assess the factors driving the Cd translocation into cacao seeds. Moreover, the results highlight the relevance of the spatial heterogeneity within and across cacao farms, influencing the variability of Cd concentrations.

Abstract The Nav1.7 voltage-gated sodium channel plays a key role in nociception. Three functional variants in the SCN9A gene (encoding M932L, V991L, and D1908G in Nav1.7), have recently been identified as stemming from Neanderthal introgression and to associate with pain symptomatology in UK BioBank data. In 1000 genomes data, these variants are absent in Europeans but common in Latin Americans. Analysing high-density genotype data from 7594 Latin Americans, we characterized Neanderthal introgression in SCN9A. We find that tracts of introgression occur on a Native American genomic background, have an average length of ~123 kb and overlap the M932L, V991L, and D1908G coding positions. Furthermore, we measured experimentally six pain thresholds in 1623 healthy Colombians. We found that Neanderthal ancestry in SCN9A is significantly associated with a lower mechanical pain threshold after sensitization with mustard oil and evidence of additivity of effects across Nav1.7 variants. Our findings support the reported association of Neanderthal Nav1.7 variants with clinical pain, define a specific sensory modality affected by archaic introgression in SCN9A and are consistent with independent effects of the Neanderthal variants on Nav1.7 function.

Resumen: Objetivo: estimar la prevalencia de los factores de riesgo para enfermedad cardiovascular en hombres con disfunción eréctil atendidos en un centro especializado en diferentes ciudades de Colombia y evaluar su asociación con el grado de severidad de la enfermedad. Métodos: estudio de corte transversal analítico. Se revisaron los registros clínicos de pacientes con diagnóstico de disfunción eréctil que fueron atendidos durante el primer semestre del año 2018. Se analizaron las características sociodemográficas y clínicas de los pacientes, y se estimó la prevalencia de los factores de riesgo cardiovascular. Se exploró la asociación entre estos factores y el grado de severidad de la disfunción eréctil por medio de modelos multivariados logísticos ordinales. Resultados: se incluyeron 2.484 pacientes que cumplieron los criterios de selección, con una edad promedio de 48,7 años (± 14,1). 2.112 sujetos presentaron uno o más factores de riesgo cardiovascular, para una prevalencia global de 85,2%. La prevalencia estimada para factor evaluado fue la siguiente: sobrepeso y obesidad 70,9%, edad superior a 45 años 60,0%, hiperlipidemia 25,1%, hipertensión 21,4%, consumo de tabaco 17,7%, diabetes 16,7%, infarto agudo de miocardio 2,21% y antecedente de alguna enfermedad cardiovascular diferente a infarto 1,68%. En el análisis multivariado se observó que la diabetes y la edad son factores que aumentan el riesgo de severidad de la disfunción eréctil. Conclusiones: es alta la prevalencia de factores de riesgo cardiovascular en hombres colombianos con disfunción eréctil. En ese sentido, el sobrepeso u obesidad es el factor modificable más frecuente en esta población. El grado de disfunción puede estar asociado con la edad y la diabetes. Abstract: Objective: To estimate the prevalence of risk factors of cardiovascular disease in men with erectile dysfunction attending a specialist clinic in different cities in Colombia, as well as their association with the level of severity of the disorder. Methods: A cross-sectional analytical study was conducted using a review of the medical records of patients diagnosed with erectile dysfunction and seen during the first six months of 2018. An analysis was made of the socio-demographic and clinical characteristics of the patients, and the prevalence of cardiovascular risk factors was calculated. The association between these factors and the level of severity of the erectile dysfunction was determined using ordinal multivariate logistic models. Results: The study included a total of 2,484 patients that met the selection criteria. The mean age was 48.7 (± 14.1) years. The majority (2.112) of subjects had one or more cardiovascular risk factors, with an overall prevalence of 85.2%. The estimated prevalence for a factor evaluated was as follows: overweight and obesity; 70.9%, age greater than 45 years; 60.0%, hyperlipidaemia; 25.1%, hypertension; 21.4%, smoking; 17.7%, diabetes; 16.7%, acute myocardial infarction; 2.21%, and a history of any cardiovascular disease other than an infarction; 1.68%. In the multivariate analysis it was observed that diabetes and age are factors that increase the risk of severity of erectile dysfunction. Conclusions: The prevalence of cardiovascular risk factors is high in Colombian men with erectile dysfunction. As regards this, overweight or obesity is the most common modifiable factor in this population. The level of dysfunction may be associated with age and diabetes.

Resumen: Introducción: estudios recientes han reportado fenómenos trombóticos o coagulopatía en pacientes con COVID-19. Hay posiciones divergentes en cuanto a la prevención, el diagnóstico y el tratamiento de estos fenómenos, y la práctica clínica actual está basada únicamente en deducciones por extensión a partir de estudios retrospectivos, series de casos, estudios observacionales y guías internacionales desarrolladas previas a la pandemia. Objetivo: establecer una serie de recomendaciones sobre prevención, diagnóstico y manejo de las complicaciones trombóticas asociadas a COVID-19. Métodos: se desarrolló una guía rápida en la que se aplicó el marco de la evidencia a la decisión (EtD) de GRADE y un sistema de participación iterativo, con análisis estadísticos y cualitativos de sus resultados. Resultados: se generaron 31 recomendaciones clínicas enfocadas a: a) Pruebas de coagulación en adultos sintomáticos con sospecha de infección o infección confirmada por SARS-CoV-2; b) Tromboprofilaxis en personas adultas con diagnóstico de COVID-19 (escalas de riesgo, tromboprofilaxis de manejo ambulatorio, intrahospitalario y duración de tromboprofilaxis después del egreso de hospitalización), c) Diagnóstico y tratamiento de las complicaciones trombóticas y d) Manejo de personas con indicación previa a usar agentes anticoagulantes. Conclusiones: las recomendaciones clínicas de este consenso orientan la toma de decisiones clínicas respecto a prevención, diagnóstico y tratamiento de fenómenos trombóticos en pacientes con COVID-19, y representan un acuerdo que ayudará a disminuir la dispersión en las prácticas clínicas acorde con el desafío que impone la pandemia. Abstract: Introduction: recent studies have reported the occurrence of thrombotic phenomena or coagulopathy in patients with COVID-19. There are divergent positions regarding the prevention, diagnosis, and treatment of these phenomena, and current clinical practice is based solely on deductions by extension from retrospective studies, case series, observational studies, and international guidelines developed prior to the pandemic. Objective: to generate a group of recommendations on the prevention, diagnosis and management of thrombotic complications associated with COVID-19. Methods: a rapid guidance was carried out applying the GRADE Evidence to Decision (EtD) frameworks and an iterative participation system, with statistical and qualitative analysis. Results: 31 clinical recommendations were generated focused on: a) Coagulation tests in symptomatic adults with suspected infection or confirmed SARS CoV-2 infection; b) Thromboprophylaxis in adults diagnosed with COVID-19 (Risk scales, thromboprophylaxis for outpatient, in-hospital management, and duration of thromboprophylaxis after discharge from hospitalization), c) Diagnosis and treatment of thrombotic complications, and d) Management of people with previous indication of anticoagulant agents. Conclusions: recommendations of this consensus guide clinical decision-making regarding the prevention, diagnosis, and treatment of thrombotic phenomena in patients with COVID-19, and represent an agreement that will help decrease the dispersion in clinical practices according to the challenge imposed by the pandemic.

Pigmentation variation in humans is influenced by complex genetic architecture in different populations. Here, the authors perform a genome-wide association analysis involving > 6,000 Latin Americans for pigmentation of skin and eyes, and identify known and novel genetic associations.

Introducción: La enfermedad coronaria es una causa importante de la morbilidad y la mortalidad en el mundo. Dentro de este grupo, se encuentran los pacientes con angina refractaria. Objetivo: El objetivo de la investigación fue evaluar la eficacia y seguridad de la contrapulsación externa aumentada con 35 sesiones de una hora, en una población atendida desde diciembre de 2013 hasta noviembre de 2015. Materiales y métodos: Se realizó un estudio observacional analítico de cohorte concurrente. Se seleccionaron los pacientes que fueron remitidos para contrapulsación. A todos se les realizó una evaluación incial y a los seis meses de la clase funcional de angina según la Canadian Cardiovascular Society y disnea de la New York Heart Association, se valoraron los medicamentos recibidos y procedimientos realizados. Se aplicó el cuestionario de calidad de vida EQ – 5 D antes y a los seis meses. Para establecer la presencia de diferencias en el seguimiento en la clase funcional, la calidad de vida al inicio y a los seis meses, se utilizó la prueba no paramétrica de Wilcoxon para datos pareados, con un nivel de significación menor de 0,05. Las estadísticas fueron analizadas con el programa SPSS versión 20,0. Resultados: El análisis inicial de 74 pacientes, 50 hombres (67,6%), el promedio de edad 65,5 años. Al inicio de la terapia el 74,4% de los pacientes, se encontraban con disnea clase II – III de la New York Heart Association y el 77% con angina II – III de la Canadian Cardiovascular Society. La mediana de la fracción de eyección fue 50,5% (35,0 – 60,0). Los antecedentes más importantes fueron: la hipertensión arterial 62 (83,8%) y la diabetes mellitus 28 (37,8%). Medicamentos recibidos para la angina y procedimientos realizados: betabloqueadores 74 (100%), estatinas 71 (95,9%), aspirina 67 (90,5%), inhibidores de la enzima convertidora de angiotensina/bloqueadores de los receptores de angiotensina 64 (86,5%), trimetazidina 56 (75,7%), nitratos 37 (50%), calcioantagonistas 20 (27%), ivabradina 15 (20,3%), revascularización quirúrgica 41 (55,4%), revascularización percutánea 51 (68,9%), rehabilitación cardiovascular 45 (60,8%) y uso de dispositivos como el marcapasos, el cardiodesfibrilador o el cardiodesfibrilador con resincronizador 19 (25,6%). Aim of study: The development of a procedure to calibrate the LEACHM and EU-Rotate_N models for simulating water and nitrogen dynamics in cauliflower crops. Area of study: Calibration was performed using experimental data obtained from measurements in a cauliflower crop sited in Valencia (Spain) region. Material and methods: A procedure based on generalized sensitivity indices for time-dependent outputs was used to determine the most influencing model parameters, in order to reduce the number of parameters to be calibrated and to avoid overparameterization. The most influencing parameters were introduced in an optimization process that uses the experimental measurements of soil water and nitrate content to determine its optimal value and obtain calibrated models. Main results: After this analysis, the most important hydraulic parameters found were the coefficients of Campbell’s equation for the LEACHM model and the soil water content at field capacity and drainage coefficient for the EU-Rotate_N model. For the N cycle, the most influencing parameters were those related with the nitrification, humus mineralization rate and residue decomposition for both models. Both calibrated models provided good simulation of soil water content with an error between 5-7%. However, larger errors in soil-nitrate content simulation were found, mainly in the period corresponding to the crop residues incorporation. The prediction of the calibrated models in a different plot gave error values of about 7-9% for soil water content, but for soil nitrate content errors computed were 34% and 58%. Research highlights: After calibration, both models can be used to optimize the farmer water management and fertilization practices in horticultural crops, although in the N case further studies should be performed.

Fundamentos y objetivos: El sangrado es la principal complicación no cardiaca relacionada con la intervención coronaria percutánea y se asocia a un aumento en el riesgo de eventos adversos. Actualmente existen varias escalas para definir riesgo de sangrado y varias definiciones de sangrado. El objetivo es determinar los factores asociados con la presencia de sangrado en pacientes con síndrome coronario agudo sometidos a intervención coronaria percutánea, utilizando la escala de Bleeding Academic Research Consortium y posteriormente ofrecer recomendaciones. Materiales y Métodos: estudio observacional analítico tipo casos y controles en pacientes con síndrome coronario agudo sometidos a intervención coronaria percutánea a quienes se aplicó la escala de Bleeding Academic Research Consortium. Los casos fueron definidos por pacientes que presentaron sangrado posterior al procedimiento (Bleeding Academic Research Consortium tipo 1-5) y los controles por pacientes que no presentaron sangrado (Bleeding Academic Research Consortium tipo 0). Resultados: Se seleccionaron 366 pacientes (121 casos, 245 controles). El 33% de pacientes presentaron algún tipo de sangrado distribuidos así: tipo 1: 12%, tipo 2: 13,4%, tipo 3: 7,3% y tipo 5: 0,3%. En el análisis multivariado los predictores más importantes de sangrado estadísticamente significativos fueron: sexo femenino, tratamiento con warfarina y uso de tirofiban durante la intervención coronaria percutánea. Conclusiones: El sangrado es un evento frecuente en la intervención coronaria percutánea, encontrando que utilizando la clasificación de Bleeding Academic Research Consortium la mayoría de los sangrados fueron tipo 1 y 2. Los factores de riesgo para sangrado descritos a nivel mundial, son parecidos a los del estudio. El sexo femenino, el uso de warfarina y uso de tirofibán fueron estadísticamente significativos. Consideramos que un buen instrumento para definir tipo de sangrado es la escala de Bleeding Academic Research Consortium.

Humans show great diversity in facial appearance and this variation is highly heritable. Here, Andres Ruiz-Linares and colleagues examined facial features in admixed Latin Americans and identify genome-wide associations for 14 facial traits, including four gene loci (RUNX2, GLI3, DCHS2 and PAX1) influencing nose morphology.

By examining Latin American individuals of mixed European, Native American and African ancestry, Adhikari et al. identify novel loci influencing various features of facial and scalp hair. The study also provides experimental evidence that one of the implicated genes (PRSS53) is expressed in the hair follicle and that the top associated variant alters processing of this enzyme.

The cultivation of sugarcane represents an important part in the Colombian economy due to the diverse applications in industries like liquor, food, paper and biofuels. Sugarcane worldwide production is affected by the presence of phytopathogenic agents, mainly filamentous fungi such as Physalospora tucumanensis (red rot disease) and Fusarium spp. To date in Colombia, Pokka boheng disease whose causal agent is the fungus Fusarium verticillioides, has not been reported, which is why it is necessary to identify appropriately this microorganism, responsible for losses in productivity and food contamination. In order to isolate and identify the infectious agent from symptomatic tissues, disinfection and culture in liquid and solid culture mediums were performed in malt extract (2%) and yeast extract (0.2%) both liquid and solid, for 7 days. After several replicates in agar plate, a purification was made along with a morphological characterization based on the shape and color of the mycelium, as well as the type of spores generated. Additionally, the genetic material was extracted and gene markers (ITS, Elongation Factor 1-α (EF) and β-Tubulin (Btub)) were amplified by PCR. Then, DNA sequencing was used to obtain the data to make a phylogenetic reconstruction by probabilistic methods (Maximum Likelihood and Bayesian Inference). The isolated strain, named as EA-FP0013 was located in the Fujikuroi complex group, with high probable identity to Fusarium verticillioides. Thus, early and species-specific identification of these fungal isolates by molecular methods may allow the timely diagnosis of emerging pathophysiological diseases of interest in the region, and thus propose the respective control strategies.

Introduction: Breast cancer is one of the most frequent diseases in Colombia and worldwide. Thousands of women, who undergo treatment, survive and require timely and comprehensive occupational therapy intervention. This paper presents a rehabilitation case study that followed the biopsychosocial and quality of life in persons model. Case presentation: Intervention on a 64-year-old woman referred to the occupational therapy service with a diagnosis of infiltrating ductal carcinoma of the left breast with neoadjuvant radiotherapy, after modified radical mastectomy and stage III lymphedema. She presented with restricted participation and occupational performance, specifically in activities of daily living, with relevant psychosocial and socio-emotional consequences. An intervention focused on the individual, following a biopsychosocial approach, was proposed in order to apply strategies on restorative, empowerment and maintenance activities of occupational skills involved in activities of daily living. Emphasis was placed on socio-emotional, occupational biomechanics and education aspects with special care to involve the interests of women. Conclusions: Rehabilitation for breast cancer patients not only involves biomedical care but also approaches psychosocial aspects that sometimes have to be solved in advance to get results that are later evident in the health of the person. In this case, a breast prosthesis was elaborated by and for the person, using all kinds of strategies that responded to biomedical axes and well-being and health.

Estudios desarrollados en el grupo BIOMA de la Universidad de Antioquia, mostraron como el hongo Purpureocillium sp. (cepa UdeA0106) puede biocontrolar una plaga conocida como sínfilos (subfilo Myriapoda: clase Symphyla). Este trabajo buscó identificar géneros de hongos anamórficos, aislados de diferentes sustratos (sínfilos y suelos) para determinar la capacidad antagónica de estos, así como de cepas de una colección de hongos biocontroladores sobre Purpureocillium sp. (cepa UdeA0106). A partir de sínfilos se registraron 9 cepas distribuidas en 3 géneros, y en suelo, 17 cepas correspondientes a 10 géneros. Las pruebas de antagonismo consistieron en una prueba in vitro empleando la técnica de cultivo dual en cajas de Petri con agar sabouraud dextrosa (SDA) utilizando 16 cepas: 12 provenientes de los aislamientos y 4 del cepario del grupo BIOMA. Las variables evaluadas fueron el radio de crecimiento en los enfrentamientos y el porcentaje de inhibición del crecimiento radial (PICR) a los 7 y 14 días. El análisis se realizó con un modelo mixto lineal generalizado mediante Proc GLIMMIX SAS® de manera independiente. El hongo Purpureocillium sp. (cepa UdeA0106) fue inhibido por Volutella sp. (Vo.UdeA0109), Paecilomyces sp. aislado 2 (Pa.UdeA0109) y Peyronella sp. (Pe.UdeA0108) (p < 0,001), mientras que el resultado no fue estadísticamente significativo con las cepas Beauveria sp. (Be.UdeA0109), Dendrodochium sp. (De.UdeA0109), Myrothecium sp. (Mi.UdeA0109), Paecilomyces sp. aislado 1 (Pa.UdeA0309), Paecilomyces sp., aislado 3 (Pa.UdeA0209), Lecanicillium sp. (Le.UdeA0109) y Purpureocillium sp. (Hy.UdeA0109). El análisis muestra que pueden presentarse interacciones entre cepas de hongos con resultados no deseados a la hora de realizar actividades de control.

Purpose: To describe the SpO2 in wakefulness, sleep and during the apnea–hypopnea in adults living in Bogotá, located at 2640 m above sea level. Methods: Descriptive observational study in adults referred for polysomnogram (PSG). A normal Apnea hypopnea index (AHI) was defined as ≤5 and obstructive sleep apnea (OSA) was classified as mild (AHI 5–15), moderate (AHI 15–30), and severe (AHI >30). T-test or ANOVA test for SpO2 differences between groups was used. Results: 1799 patients, 33% women. 222 (12.8%) did not have OSA (normal IAH), 268 (14.9%) mild OSA, 315 (17.5%) moderate ,and 993 (55.2%) severe. In all cases a low SpO2 (SpO2

INTRODUCCIÓN: las complicaciones relacionadas con el sangrado constituyen un problema importante en los pacientes que ingresan con síndrome coronario agudo, pues se asocian con morbilidad y mortalidad importantes. El puntaje "CRUSADE", estudiado y validado, identifica ocho predictores de sangrado mayor en el hospital y constituye una herramienta útil para comprobar el riesgo de sangrado. OBJETIVO: establecer las características operativas del puntaje de sangrado "CRUSADE" para determinar el riesgo de sangrado mayor en pacientes con síndrome coronario agudo sin elevación del ST. Establecer cual es el mejor punto de corte del puntaje "CRUSADE" en nuestro medio, con base en la curva de características operativas del receptor (ROC). MATERIALES Y MÉTODOS: se agruparon pacientes desde el 1º. de julio de 2011 hasta el 30 de noviembre del mismo año, detallando diferentes variables. Se realizó la curva ROC y se determinó: punto de corte, sensibilidad, especificidad, valores predictivos positivos y negativos y razones de verosimilitud. RESULTADOS: se presenta el análisis de 330 pacientes, con edad promedio de 66,5 años, 68,4% de género masculino. El promedio de puntaje "CRUSADE" fue de 27,9; el 18,18% presentó sangrado mayor, el 56,9% angina inestable y el 43,0% infarto agudo del miocardio sin elevación del ST. Se determinó el área bajo la curva cuyo resultado fue 0,8114 (IC 95%; 0,75 – 0,87). Utilizando un punto de corte mayor o igual a 35, los resultados aportaron una sensibilidad del 70% (IC 95%; 55,57 – 82, 43), una especificidad del 77,41% (IC 95%; 72,23 – 82,58), un valor predictivo positivo de 40,78% (IC 95%; 30,80 – 50,75), un valor predictivo negativo de 92,07% (IC 95%; 88,34 – 95,81), una razón de verosimilitud positiva 3,10 (2,35 – 4,08) y una razón de verosimilitud negativa 0,39 (0,26 – 0,57). CONCLUSIONES: el punto de corte para el puntaje "CRUSADE" propuesto, es mayor o igual a 35. Dada la importancia de realizar una estratificación del riesgo de sangrado, estos resultados sirven para recomendar la implementación sistemática del puntaje de sangrado "CRUSADE", y si este es mayor o igual a 35, establecer las recomendaciones para disminuir el riesgo de sangrado, y por ende, la morbilidad y la mortalidad.

OBJETIVO: La exposición a humo de leña es factor de riesgo para EPOC. A diferencia de la EPOC por cigarrillo (EPOC-C), para un mismo nivel de obstrucción, en la EPOC por leña (EPOC-L), la DLCO está menos disminuida, sugiriendo menos enfisema. Por tanto, el objetivo de este estudio fue comparar los hallazgos en la TCAR en mujeres con EPOC-L y con EPOC- C. MÉTODOS: Veintidós mujeres con EPOC severa (VEF1/CVF < 70% y VEF1 < 50%) fueron divididas en dos grupos: las expuestas a leña (EPOC-L; n = 12) y las expuestas a cigarrillo (EPOC-C; n = 10). Se compararon los dos grupos con respecto al puntaje de enfisema y el compromiso de la vía aérea en la TCAR, las anormalidades funcionales en la espirometría, la DLCO, los volúmenes pulmonares y la resistencia específica de la vía aérea (sRaw). RESULTADOS: Los dos grupos tuvieron VEF1, sRaw e hiperinflación pulmonar similares. En el grupo EPOC-C, hubo mayor disminución de la DLCO y de la DLCO/VA y mayor puntaje de enfisema. En el grupo EPOC-L, no encontramos enfisema significativo en la TCAR. Los hallazgos principales fueron engrosamiento peribronquial, dilataciones bronquiales y atelectasias subsegmentarias. CONCLUSIONES: En pacientes con EPOC-L severa no hay enfisema en la TCAR. El hallazgo más importante es el compromiso severo de la vía aérea. For a given degree of airway obstruction, the reduction in DLCO is smaller in individuals with wood smoke-related COPD than in those with smoking-related COPD, suggesting that there is less emphysema in the former. The objective of this study was to compare HRCT findings between women with wood smoke-related COPD and women with smoking-related COPD. METHODS: Twenty-two women with severe COPD (FEV1/FVC ratio < 70% and FEV1 < 50%) were divided into two groups: those with wood smoke-related COPD (n = 12) and those with smoking-related COPD (n = 10). The two groups were compared regarding emphysema scores and airway involvement (as determined by HRCT); and functional abnormalities-spirometry results, DLCO, alveolar volume (VA), the DLCO/VA ratio, lung volumes, and specific airway resistance (sRaw). Results: There were no significant differences between the two groups in terms of FEV1, sRaw, or lung hyperinflation. Decreases in DLCO and in the DLCO/VA ratio were greater in the smoking-related COPD group subjects, who also had higher emphysema scores, in comparison with the wood smoke-related COPD group subjects. In the wood smoke-related COPD group, HRCT scans howed no significant emphysema, the main findings being peribronchial thickening, bronchial dilation, and subsegmental atelectasis. CONCLUSIONS: Female patients with severe wood smoke-related COPD do not appear to develop emphysema, although they do show severe airway involvement. The reduction in DLCO and VA, with a normal DLCO/VA ratio, is probably due to severe bronchial obstruction and incomplete mixing of inspired gas during the determination of single-breath DLCO.

The expression of facial asymmetries has been recurrently related with poverty and/or disadvantaged socioeconomic status. Departing from the developmental instability theory, previous approaches attempted to test the statistical relationship between the stress experienced by individuals grown in poor conditions and an increase in facial and corporal asymmetry. Here we aim to further evaluate such hypothesis on a large sample of admixed Latin Americans individuals by exploring if low socioeconomic status individuals tend to exhibit greater facial fluctuating asymmetry values. To do so, we implement Procrustes analysis of variance and Hierarchical Linear Modelling (HLM) to estimate potential associations between facial fluctuating asymmetry values and socioeconomic status. We report significant relationships between facial fluctuating asymmetry values and age, sex, and genetic ancestry, while socioeconomic status failed to exhibit any strong statistical relationship with facial asymmetry. These results are persistent after the effect of heterozygosity (a proxy for genetic ancestry) is controlled in the model. Our results indicate that, at least on the studied sample, there is no relationship between socioeconomic stress (as intended as low socioeconomic status) and facial asymmetries.

Objetivos: verificar el nivel de cumplimiento de las metas en colesterol LDL, colesterol no-HDL, triglicéridos y colesterol HDL, de acuerdo con lo indicado por la guía del NCEP-ATPIII y las recomendaciones actuales, en una población de pacientes hospitalizados por causa no cardiovascular pero con antecedente de enfermedad coronaria. Analizar, además, los hipolipemiantes que utiliza esta población, así como la prevalencia de síndrome metabólico, glucemia anormal en ayunas y diabetes mellitus tipo 2. Metodología: el porcentaje de cumplimiento de estas metas se evaluó mediante el programa de análisis estadístico STATA. Se compararon los hallazgos del perfil lipídico con la propuesta de la guía del NCEP-ATPIII y las recomendaciones actuales (punto final primario). Igualmente, se analizaron los hipolipemiantes utilizados, así como la prevalencia de glucemia anormal en ayunas, diabetes mellitus tipo 2 y síndrome metabólico (punto final secundario). Resultados: se identificaron 281 pacientes que cumplían con los criterios de inclusión. En la tabla 2 se resumen los resultados del punto final primario. De acuerdo con el NCEP-ATPIII, en el riesgo alto, el porcentaje de pacientes que cumplían con las metas en el cLDL (objetivo principal en el tratamiento de las dislipidemias), era de 57,2 y en el riesgo muy alto de 23,5%. En la actualidad, a los pacientes con enfermedad coronaria, independiente de la presencia de síndrome metabólico o diabetes mellitus, se les recomienda una meta para el cLDL menor de 70 mg/dL (Recomendación IIa), la cual se observó en 21,7% de los pacientes estudiados. La prevalencia de glucemia anormal en ayunas fue de 25,6%, la de diabetes mellitus tipo 2 de 20% y la de síndrome metabólico de 45,2%. Con respecto a los hipolipemiantes utilizados, se observó que 56% usaba lovastatina, 14% no utilizaba hipolipemiantes y ninguno tenía asociación de estos fármacos. Conclusiones: después de nueve años de la publicación de las guías del NCEP-ATPIII y de las recomendaciones posteriores sobre las metas del perfil lipídico en pacientes con enfermedad coronaria y a pesar de la divulgación de las mismas a través de conferencias, congresos y publicaciones, estas metas las cumple un porcentaje bajo de pacientes, tal como lo demuestra este estudio observacional en sujetos hospitalizados por causas diferentes a la enfermedad coronaria pero con este antecedente. Una de las explicaciones para este pobre resultado es el uso de lovastatina, estatina de baja efectividad para reducir el cLDL. Se analizan otras posibles causas y se hacen propuestas para lograr los objetivos.Objectives: to verify the level of compliance with the targets in LDL cholesterol, non-HDL cholesterol, triglycerides, HDL cholesterol, in accordance with the recommendations of the NCEP-ATPIII guide and current recommendations in a population of patients hospitalized for non-cardiovascular causes but with a history of coronary disease. Analyze also lipid-lowering drugs used by this population as well as the prevalence of metabolic syndrome, abnormal fasting glucose and type 2 diabetes mellitus. Methodology: the percentage of fulfillment of these goals was assessed using the STATA statistical analysis program. Findings of the lipid profile were compared with the proposal from the NCEP-ATPIII guide and current recommendations (primary endpoint). Similarly, lipid-lowering drugs used were analyzed as well as the prevalence of abnormal fasting glucose, diabetes mellitus type 2 and metabolic syndrome (secondary endpoint). Results: we identified 281 patients who met the inclusion criteria. Table 2 summarizes the results of the primary endpoint. According to the NCEP-ATPIII, in high risk, the percentage of patients who met the LDL cholesterol goals (main goal in the treatment of dyslipidemias), was 57,2% and in the very high risk, 23,5%. Currently, patients with coronary heart disease independent of the presence of metabolic syndrome or diabetes mellitus, a goal for LDL cholesterol below 70 mg/dL is recommended (Recommendation IIa); this was observed in 21.7% of patients studied. The prevalence of abnormal fasting glucose was 25.6%, that of type 2 diabetes mellitus was 20% and that of metabolic syndrome 45.2%. With respect to lipid-lowering drugs used, we observed that 56% used lovastatin, 14% did not use lipid-lowering drugs and none had any association of these drugs. Conclusions: after nine years of the publication of NCEP-ATPIII guidelines and subsequent recommendations on the goals of the lipid profile in patients with coronary disease and despite the dissemination of these through lectures, conferences and publications, these goals are met by a low percentage of patients, as evidenced by this observational study in patients hospitalized for causes other than coronary heart disease but with this history. One explanation for this poor performance is the use of lovastatin, that is a low efficacy statin to reduce LDL cholesterol. Other possible causes are analyzed and proposals to achieve the objectives are made.

OBJETIVO: establecer el impacto de la rehabilitación cardiaca en pacientes con falla cardiaca de origen isquémico con fracción de eyección menor del 40% en términos de síntomas, a través de la percepción subjetiva de la escala de Borg, VO2, MET y distancia en millas antes y al finalizar la terapia de rehabilitación cardiovascular. DISEÑO: estudio de cohorte concurrente. POBLACIÓN: 224 pacientes que ingresaron al programa de rehabilitación cardiaca de La Fundación Clínica Abood Shaio en el periodo comprendido entre el 10 de diciembre de 2004 y el 2 de mayo de 2008. MEDICIONES: se revisó la base de datos de rehabilitación cardiaca clasificando los parámetros de actividad física al ingreso y al egreso de la terapia. RESULTADOS: en el impacto de la terapia de rehabilitación cardiaca al inicio, 63% de los pacientes se encontraba en las escalas uno y dos de Borg modificada, y al final 85% (p < 0,001) se ubicó en las escalas tres y cuatro; el VO2 al inicio fue de 7,79 y al final 19,04 (p < 0,001); los MET al inicio 2,22 y al final 5,44 (p < 0,001) y la distancia en millas por hora al inicio 1,33 y al final 2,58 (p < 0,001). CONCLUSIONES: se observó que la rehabilitación cardiaca tuvo un impacto significativo en los pacientes con falla cardiaca de origen isquémico con fracción de eyección del ventrículo izquierdo menor a 40% en términos de percepción de síntomas en forma subjetiva en la escala de Borg, VO2 indirecto, MET y distancia en millas por hora.OBJECTIVE: to establish the impact of heart rehabilitation in patients with heart failure of ischemic origin with ejection fraction

Introducción: el propósito del estudio fue evaluar el eficacia de la cirugía de Maze III modificada mediante una pinza de crioablación reutilizable en comparación con el Maze III clásico de corte y sutura, sólo en pacientes con fibrilación auricular permanente, y a la vez realizar cirugía mitral. Métodos: desde mayo de 1993 a abril de 2005, se realizó cirugía de Maze a 134 pacientes, de los cuales 92 presentaban fibrilación auricular permanente y además tenían cirugía de la válvula mitral. En el grupo clásico de Maze se incluyeron 71 pacientes (grupo I) y en el nuevo grupo 21 (grupo II). Se realizó un análisis multivariable para analizar estos resultados. Resultados: la edad promedio fue 48,1 años (24 a 67 años), en un total de 24 hombres y 68 mujeres. La duración de la fibrilación fue de 3,8 años ± 4,3 años (DE 0,5-20) y el promedio del diámetro de la aurícula izquierda fue de 57 (26-82). No se hallaron diferencias significativas en ambos grupos. El tiempo de isquemia del grupo I fue de 83 min y del grupo II de 77 min (p = 0,4). El sangrado post-quirúrgico fue de 762 mL en el grupo I y 680 para el grupo II (p= 0,6). La mortalidad en el grupo I fue de 5,6% y en el grupo II de 0%. El ritmo sinusal se recobró en el 87% de los pacientes del grupo I y en el 85,7% del grupo II. Conclusiones: la cirugía de Maze III modificada con pinza de crioablación reutilizable, es tan efectiva como el Maze clásico, que se considera el patrón de oro. Se encontró una disminución en el tiempo de isquemia y en el riesgo de sangrado post-quirúrgico, la cual fue no significativa estadísticamente. Hay diferencia en la mortalidad de los dos grupos. Esta modificación es segura y puede aplicarse por un mayor número de cirujanos en cualquier institución. En países que se encuentran en vía de desarrollo este aparato puede utilizarse como un método alterno de energía.Introduction: the purpose of this study was to evaluate the efficacy of the modified Maze III surgery using a reusable cryoablation clamp compared with the classic cut and suture Maze III only in patients with permanent atrial fibrillation and mitral surgery. Methods: in the period between May 1993 and April 2005, 134 patients underwent Maze surgery. 92 had permanent atrial fibrillation and previous mitral surgery. 71 patients had had the classic Maze surgery (group I) and 21 the modified Maze III with reusable cryoablation clamp. A multivariable analysis is realized in order to analyze these results. Results: mean age was 48,1 years (24 to 67 years). 24 were men and 68 were women. Mean duration of atrial fibrillation was 3,8 ± 4.3 years (standard deviation 0,5-20). Mean left atrial diameter was 57 mm (26-82 mm). No significant differences were found between the two groups. Mean ischemic time of group I was 83 minutes and that of group II was 77 minutes (p = 0,4). Post-chirurgic bleeding was of 762 cc in group I and 680 cc in group II (p = 0,6). Mortality was 5,6% in group I and 0% in group II. Sinus rhythm was recovered in 87% patients in group I and in 85,7% patients in group II. Conclusions: modified Maze III surgery with cryoablator reusable clamp is as effective as classic Maze, considered as the gold standard. We found a decrease in the ischemic time as well as in the post-chirurgic bleeding, being not statistically significant. There is a difference in mortality between the two groups. This is a safe modification and can be applied by more surgeons in any institution. In developing countries, this device can be used as an alternative energy method.

Objetivo: describir las características cardíacas de pacientes sometidos a cambio valvular aórtico con ampliación del anillo para implantación de una prótesis mayor, mediante seguimiento clínico y ecocardiográfico. Materiales y métodos: entre 1997 y 2001 se tomaron 26 pacientes, de los cuales 13 (50%), con edad promedio de 57 años, se sometieron a seguimiento. La ecocardiografía se practicó en el control, obteniéndose área valvular aórtica, masa ventricular y gradientes transprotésicos para luego establecer los índices respectivos. Resultados: hubo mejoría en la clase funcional de los 13 pacientes controlados, con 92.3% en clase I (p= 0.002). La evolución postoperatoria fue de 25 meses en promedio. El índice de masa ventricular y los gradientes valvulares (medio) presentaron reducción de 149 a 113 g/m2 y de 67 a 12.2 mm Hg respectivamente (p= 0.001); también se evidenció aumento en el índice de área efectiva valvular protésica que varió de 0.37 a 1 cm/m2 (p= 0.001). Discusión: los resultados evidencian involución en los trastornos ventriculares y mejoría en la clase funcional de los pacientes sometidos a este procedimiento; sin embargo, no se logró establecer comparación con pacientes que presentaran una prótesis menor a 21 mm de diámetro, lo que permitiría confirmar las ventajas de una válvula mayor. Conclusión: la cirugía de cambio valvular aórtico con ampliación del anillo para la implantación de una prótesis mayor, parece ser una alternativa para los pacientes con anillo aórtico nativo pequeño, evidenciándose mejoría clínica y ecocardiográfica en su evolución.Objective: to describe the cardiac characteristics of patients submitted to aortic valvular change with ring widening for a bigger prosthesis implantation, through clinical and echocardiographic follow-up. Material and methods: 26 patients were chosen between 1997 and 2001. Follow-up was possible in 13 patients. Mean age was 57 years. A control echocardiogram was performed, obtaining the aortic valvular area, ventricular mass and the transprosthetic gradients in order to establish the respective index. Results: there was an improvement in the functional class of the controlled 13 patients, with 92.3% in class I (p: 0.002). Average postoperative evolution time was 25 months. Ventricular mass index and valvular gradients (Middle) showed a decrease from 149 to 113 gr/mt² and from 67 to 12.2 mmHg respectively (p: 0.001). An increase in the efficient prosthetic valvular area was also evidenced; it varied from 0.37 to 1 cm/mt² (p: 0.001). Discussion: the results evidence an involution of ventricular disorders and improvement in the functional class of patients who underwent this procedure; however, it was not possible to establish a comparison with patients that would have a prosthesis with a diameter smaller than 21 mm, which would allow to assert the advantages of a bigger valve. Conclusions: in a patient who comes to the emergency ward with non traumatic thoracic pain there are no significant changes in the clinical and electrocardiographical presentation respect to the new myocardial infarction definition.

The current genetic makeup of Latin America has been shaped by a history of extensive admixture between Africans, Europeans and Native Americans, a process taking place within the context of extensive geographic and social stratification. We estimated individual ancestry proportions in a sample of 7,342 subjects ascertained in five countries (Brazil, Chile, Colombia, México and Perú). These individuals were also characterized for a range of physical appearance traits and for self-perception of ancestry. The geographic distribution of admixture proportions in this sample reveals extensive population structure, illustrating the continuing impact of demographic history on the genetic diversity of Latin America. Significant ancestry effects were detected for most phenotypes studied. However, ancestry generally explains only a modest proportion of total phenotypic variation. Genetically estimated and self-perceived ancestry correlate significantly, but certain physical attributes have a strong impact on self-perception and bias self-perception of ancestry relative to genetically estimated ancestry.

Abstract  Chagas disease is a severe public health problem in Latin-American countries. In Colombia, the predominance of Trypanosoma cruzi I has been described in the literature, with a broad heterogeneity between strains. However, most of the studies carried out centered on isoenzyme analysis, with a smaller number that focus on other molecular methods. In this report, we discuss the results of a molecular analysis of T. cruzi I strains, isolated from the domestic cycle, from the department of Santander, one of the territorial divisions where the prevalence of infection is highest. Internal transcribed spacer–restriction fragment length polymorphism and random amplification of polymorphic DNA were used to characterize 16 strains from human and vector (Triatominae) hosts. The data reveal a clustering based on the biological origin. Human and vector strains grouped separately; however, three vector strains clustered together with human strains. These results indicate that genetic differences exist in the strains that infect both hosts. We conclude that T. cruzi I populations in the domestic cycle of transmission of Chagas disease in Santander are heterogeneous and are composed of different clones. The epidemiological and biological implications are discussed. [ABSTRACT FROM AUTHOR]
