AIM: To investigate the underlying factors by establishing a new paradigm for assessing control ability under stereopsis testing conditions. METHODS: This was a prospective observational study. We evaluated the control ability of intermittent exotropia (IXT) patients in three conditions: natural 2D optotype viewing, 2D optotype viewing with polarized glasses, and 3D optotype viewing with polarized glasses. Recording with a smartphone, we captured videos to analyze the accurate time of spontaneous exodeviation and subsequent realignment before and after breaking fusion. Additionally, the correlation of stereopsis were also analyzed. RESULTS: A total of 48 patients (age range: 4-33y; 54.17% male) participated in the study. When viewing 3D optotypes with polarized glasses, their median control scores were 1 (interquartile range, 0-4) at distance and 0 (0-1) at near. These scores were significantly better than those observed under natural viewing conditions, which were 2.5 (1-5) at a distance and 1 (0-3) at near (Friedman test, P=0.049). Furthermore, those subjects who exhibited exophoria (realignment within 2 seconds) while viewing 3D optotypes with polarized glasses were more likely to have measurable stereo vision (Kendall's τb=-0.344, P=0.018). CONCLUSION: IXT patients exhibit enhanced control ability when using polarized glasses to view 3D optotypes, notably improving realignment capabilities. This expands our understanding of current tests and offers a potentially sensitive method for assessing IXT severity.

One significant environmental element influencing the growth and yield of rice (Oryza sativa L.) is high temperature. Nevertheless, the mechanism by which rice responds to high temperature is not fully understood. A rice glycosyltransferase gene, OsDUGT1, was identified as a heat-responsive gene in this investigation. Its function was studied by overexpression and knockout methods. The results showed that under heat stress, OsDUGT1 overexpression lines (OsDUGT1-OE) increased the survival rate of rice, while Osdugt1 knockout lines (Osdugt1-ko) decreased the survival rate compared to wild type (ZH11). In addition to rice, heat stress tolerance was also improved by ectopic expression of OsDUGT1 in transgenic Arabidopsis thaliana plants. We observed that ROS scavenging ability, malondialdehyde accumulation, and the ion leakage are relevant to the expression level of OsDUGT1. Through enzyme activity analysis, we found that OsDUGT1 could glycosylate flavonoid compounds. Correspondingly, the loss of OsDUGT1 function caused a significant decrease in endogenous flavonoid accumulation in rice, which was demonstrated by our metabolomics analysis. Additionally, our transcriptomic analysis of Osdugt1 mutant lines under heat stress condition indicated that mutation of OsDUGT1 can reduce the transcriptional activity of heat response related genes, antioxidant enzyme genes and other genes involved in the flavonoid biosynthetic pathway. In summary, our work revealed that OsDUGT1 plays a crucial role in adjusting and balancing the overall plant metabolism and transcription under heat stress through glycosylation of flavonoids, and offers a key prospect gene for breeding efforts to enhance crop heat tolerance under the trend of climate warming all over the globe.

Abstract A combined model was developed using contrast-enhanced CT-based radiomics features and clinical characteristics to predict liver fibrosis stages in patients with chronic liver disease (CLD). We retrospectively analyzed multiphase CT scans and biopsy-confirmed liver fibrosis. 160 CLD patients were randomly divided into 7:3 training/validation ratio. Clinical laboratory indicators associated with liver fibrosis were identified using Spearman's correlation and multivariate logistic regression correlation. Radiomic features were extracted after segmenting the entire liver from multiphase CT images. Feature dimensionality reduction was performed using RF-RFE, LASSO, and mRMR methods. Six radiomics-based models were developed in the training cohort of 112 patients. Internal validation was conducted on 48 randomly assigned patients. Receptor Operating Characteristic (ROC) curves and confusion matrices were constructed to evaluate model performance. The radiomics model exhibited robust performance, with AUC values of 0.810 to 1.000 for significant fibrosis, advanced fibrosis, and cirrhosis. The integrated clinical-radiomics model had superior diagnostic efficacy in the validation cohort, with AUC values of 0.836 to 0.997. Moreover, these models outperformed established biomarkers such as the aspartate aminotransferase to platelet ratio index (APRI) and the fibrosis 4 score (FIB-4), as well as the gamma glutamyl transpeptidase to platelet ratio (GPR), in predicting the fibrotic stages. The clinical-radiomics model holds considerable promise as a non-invasive diagnostic tool for the assessment and staging of liver fibrosis in the patients with CLD, potentially leading to better patient management and outcomes.

Background/Purpose: Biomaterial implants are emerging as a treatment choice for pleurodesis; however, the optimal biomaterial and form for managing spontaneous pneumothorax, particularly post-video-assisted thoracic surgery, remain under investigation. This study evaluated the mechanical and biological properties of the poly-ε-caprolactone (PCL) membrane as a sclerosing agent for pleurodesis in Landrace pigs. Methods: Twenty-four Landrace pigs were split into two groups for mechanical abrasion and PCL membrane pleurodesis, with the latter group's PCL meshes inserted using video-assisted thoracic surgery. The mechanical and biological properties of the PCL membrane were assessed in pigs at three, six, and 12 months after the procedure. This assessment involved a range of techniques, such as the T-Peel test, macroscopic evaluation with a scoring scale, microscopic examination, and biomechanical and molecular weight analysis. Results: The PCL membrane group outperformed the traditional abrasion group, with stronger adhesions seen over longer implantation durations. This group also showed superior and more consistent results in both macroscopic and microscopic evaluations compared to the control group. The membrane-based method was easier and faster to perform than the control group's method, and importantly, no mortality occurred following membrane implantation. Conclusion: This study is the pioneering effort to present long-term findings regarding the mechanical and biological properties of the PCL membrane in an in vivo animal model. The membrane demonstrated better adhesion ability than that of traditional abrasion and showed reassuring biocompatibility in both the pig model, suggesting its potential as treatment for patients with primary spontaneous pneumothorax. Further clinical studies are needed to support these observations.

AIM: To investigate the clinical characteristics, treatment methods and outcomes of rhegmatogenous retinal detachment (RRD) in highly myopic eyes with implantable collamer lens (ICL). METHODS: High myopia patients who received treatment for nontraumatic RRD after ICL implantation surgery at the Retinal Department of Zhongshan Ophthalmic Center from Jan 2018 to Dec 2022 were reviewed. Comprehensive ophthalmologic examinations including visual acuity measurement and digital fundus photography were performed in each patient. RESULTS: A total of nine RRD eyes from nine patients who received V4c-ICL implantation were included. The mean time from ICL implantation surgery to the diagnosis of RRD was 32.44±22.56mo (range, 1-60mo). At the initial visit for RRD, giant retinal tear (GRT), horseshoe tear, simple round hole, and horseshoe tear combined with round hole were detected in 3, 3, 2, and 1 eye(s), respectively, with macula-off in eyes. Eight patients received surgical treatment, and one patient was treated by retinal laser photocoagulation alone. The ICL was preserved in 7 eyes. At the last follow-up, the mean best corrected visual acuity (BCVA) improved significantly from 1.76±1.06 logMAR at presentation to 0.81±1.01 logMAR (P=0.035), and no case of recurrent retinal detachment was found. CONCLUSION: The morphological presentation of retinal breaks is diverse in this study. The ICL can be preserved in most cases during the course of retinal detachment repair surgery in our data, companied with acceptable visual and anatomical outcomes.

Synovial sarcoma, a rare malignant neoplasm with a poor prognosis, accounts for approximately 5%-10% of all primary soft-tissue malignancies worldwide. Typically affecting adolescents and young adults, it primarily manifests near the joints of the lower extremities. This study aimed to demonstrate that this tumor can also affect the prevertebral space. A 32-year-old male patient presented at our outpatient clinic with a 2-month history of upper limb numbness and a 1-month complaint of palpable neck mass. Imaging studies revealed a bulky, lobulated, and heterogeneous mass exhibiting heterogeneous enhancement. Furthermore, the mass caused expansion of the neuroforamen in the neck, initially suggesting a diagnosis of malignant schwannoma. However, a histopathologic examination suggested synovial sarcoma. The article provided a comprehensive review of the clinical, pathological, and radiological features of this condition. Additionally, it explored current treatment options and prognoses by referencing relevant literature.

BackgroundEsophageal cancer is the seventh most frequently diagnosed cancer with a high mortality rate and the sixth leading cause of cancer deaths in the world. Early detection of esophageal cancer is very vital for the patients. Traditionally, contrast computed tomography (CT) was used to detect esophageal carcinomas, but with the development of deep learning (DL) technology, it may now be possible for non-contrast CT to detect esophageal carcinomas. In this study, we aimed to establish a DL-based diagnostic system to stage esophageal cancer from non-contrast chest CT images.MethodsIn this retrospective dual-center study, we included 397 primary esophageal cancer patients with pathologically confirmed non-contrast chest CT images, as well as 250 healthy individuals without esophageal tumors, confirmed through endoscopic examination. The images of these participants were treated as the training data. Additionally, images from 100 esophageal cancer patients and 100 healthy individuals were enrolled for model validation. The esophagus segmentation was performed using the no-new-Net (nnU-Net) model; based on the segmentation result and feature extraction, a decision tree was employed to classify whether cancer is present or not. We compared the diagnostic efficacy of the DL-based method with the performance of radiologists with various levels of experience. Meanwhile, a diagnostic performance comparison of radiologists with and without the aid of the DL-based method was also conducted.ResultsIn this study, the DL-based method demonstrated a high level of diagnostic efficacy in the detection of esophageal cancer, with a performance of AUC of 0.890, sensitivity of 0.900, specificity of 0.880, accuracy of 0.882, and F-score of 0.891. Abstract The liver is a multifaceted organ that is responsible for many critical functions encompassing amino acid, carbohydrate, and lipid metabolism, all of which make a healthy liver essential for the human body. Contemporary imaging methodologies have remarkable diagnostic accuracy in discerning focal liver lesions; however, a comprehensive understanding of diffuse liver diseases is a requisite for radiologists to accurately diagnose or predict the progression of such lesions within clinical contexts. Nonetheless, the conventional attributes of radiological features, including morphology, size, margin, density, signal intensity, and echoes, limit their clinical utility. Radiomics is a widely used approach that is characterized by the extraction of copious image features from radiographic depictions, which gives it considerable potential in addressing this limitation. It is worth noting that functional or molecular alterations occur significantly prior to the morphological shifts discernible by imaging modalities. Consequently, the explication of potential mechanisms by multiomics analyses (encompassing genomics, epigenomics, transcriptomics, proteomics, and metabolomics) is essential for investigating putative signal pathway regulations from a radiological viewpoint. In this review, we elaborate on the principal pathological categorizations of diffuse liver diseases, the evaluation of multiomics approaches pertaining to diffuse liver diseases, and the prospective value of predictive models. Accordingly, the overarching objective of this review is to scrutinize the interrelations between radiological features and bioinformatics as well as to consider the development of prediction models predicated on radiobioinformatics as integral components of clinical decision support systems for diffuse liver diseases.

Background/purposeBiomaterial-based implants are gaining traction as an option for pleurodesis treatment, yet the search for the best biomaterial or the most suitable shape to handle spontaneous pneumothorax continues. This forward-looking research assessed the use of a poly-ε-caprolactone membrane for its safety when applied as a sclerosant in pleurodesis procedures in human patients.MethodsFrom July 2017 to February 2018, we conducted a Phase I trial in which 10 patients with primary spontaneous pneumothorax were treated using video-assisted thoracoscopic surgery with a poly-ε-caprolactone membrane. These procedures encompassed bleb resection and mechanical pleurodesis through parietal pleura scrubbing. After resection, a 150 × 150 mm poly-ε-caprolactone membrane was applied to the apex. The primary outcome measures were the adverse events and laboratory outcomes.ResultsAfter surgery, we observed no cardiopulmonary-related adverse events or indications of systemic inflammation. Furthermore, no episodes of hypothermia or hyperthermia occurred. Chest radiographs showed no evident pneumonitis or effusion associated with tissue reactions. The average follow-up duration was 31.7 ± 17.7 months, during which two patients exhibited recurrence.ConclusionThis study is the first to show the biocompatibility of poly-ε-caprolactone in humans, suggesting its potential as a treatment option for patients with primary spontaneous pneumothorax. Despite the relatively small number of patients, we maintain confidence in the reliability and safety profile of the PCL membrane, bolstered by its previously established efficacy in applications involving other organs. Phase II and phase III clinical studies are needed to support these observations.

Breast cancer has become the number one cancer in the world, and intestinal flora may be closely linked to it. Geographic location also has an important impact on human intestinal flora. We conducted the first study on the intestinal flora of breast cancer patients and non-breast cancer patients in a tropical region - Hainan Province in China. At the same time, Pacbio platform based on third-generation sequencing was used for the first time to conduct 16S full-length sequencing of fecal microorganism DNA. We completed the species diversity analysis and differential species analysis of the intestinal flora between the two groups, inferred their functional genetic composition and performed functional difference analysis. There were statistically significant differences in alpha diversity between the two groups in Hainan Province. By species composition difference analysis, at the phylum level, Bacteroidales (P = 0.006) and Firmicutes (P = 0.002) was differed between the two groups, and at the genus level, 17 breast cancer-related differential species such as Bacteroides were screened. There are serious drilling fluid loss problems in fractured reservoirs during drilling and completion. Indoor evaluation of the drilling fluid lost control effect is an important basis for on-site plugging formula design, but there are some problems in drilling fluid lost control evaluation, such as the inability to evaluate specific loss types. Therefore, based on the classification of loss causes, this paper defines the main control factors of drilling fluid lost control efficiency of different loss types and puts forward a method for recognizing loss types. The influence of fracture module and experimental steps on the drilling fluid lost control efficiency was evaluated through laboratory experiments. Based on the analysis method of indoor and field drilling fluid lost control efficiency, the best laboratory experimental conditions of different loss types were recommended, and then, the experimental evaluation method of the drilling fluid lost control efficiency considering various loss types was established. This method can comprehensively evaluate and grade the lost control ability of the plugging formula. Through the verification in Block K of the Tarim Basin, the test results are closer to the field lost control results, and the evaluation results of the drilling fluid lost control efficiency are better, which can guide the field leakage control evaluation.

Abstract Background NLRP3 inflammasome-mediated neuroinflammation plays a critical role in the pathogenesis and development of Alzheimer’s disease (AD). Microglial autophagic degradation not only decreases the deposits of extracellular Aβ fibrils but also inhibits the activation of NRLP3 inflammasome. Here, we aimed to identify the potent autophagy enhancers from Penthorum chinense Pursh (PCP) that alleviate the pathology of AD via inhibiting the NLRP3 inflammasome. Methods At first, autophagic activity-guided isolation was performed to identify the autophagy enhancers in PCP. Secondly, the autophagy effect was monitored by detecting LC3 protein expression using Western blotting and the average number of GFP-LC3 puncta per microglial cell using confocal microscopy. Then, the activation of NLRP3 inflammasome was measured by detecting the protein expression and transfected fluorescence intensity of NLRP3, ASC, and caspase-1, as well as the secretion of proinflammatory cytokines. Finally, the behavioral performance was evaluated by measuring the paralysis in C. elegans, and the cognitive function was tested by Morris water maze (MWM) in APP/PS1 mice. Results Four ellagitannin flavonoids, including pinocembrin-7-O-[4″,6″-hexahydroxydiphenoyl]-glucoside (PHG), pinocembrin-7-O-[3″-O-galloyl-4″,6″-hexahydroxydiphenoyl]-glucoside (PGHG), thonningianin A (TA), and thonningianin B (TB), were identified to be autophagy enhancers in PCP. Among these, TA exhibited the strongest autophagy induction effect, and the mechanistic study demonstrated that TA activated autophagy via the AMPK/ULK1 and Raf/MEK/ERK signaling pathways. In addition, TA effectively promoted the autophagic degradation of NLRP3 inflammasome in Aβ(1–42)-induced microglial cells and ameliorated neuronal damage via autophagy induction. In vivo, TA activated autophagy and improved behavioral symptoms in C. elegans. Furthermore, TA might penetrate the blood-brain barrier and could improve cognitive function and ameliorate the Aβ pathology and the NLRP3 inflammasome-mediated neuroinflammation via the AMPK/ULK1 and Raf/MEK/ERK signaling pathways in APP/PS1 mice. Conclusion We identified TA as a potent microglial autophagy enhancer in PCP that promotes the autophagic degradation of the NLRP3 inflammasome to alleviate the pathology of AD via the AMPK/ULK1 and Raf/MEK/ERK signaling pathways, which provides novel insights for TA in the treatment of AD.

Bile acids (BAs) are crucial for maintaining intestinal epithelial homeostasis. However, the metabolic changes in BAs and the communication between intestinal epithelial cells (IECs) in infants after birth remain unclear. This study aims to elucidate the BA profiles of newborn piglets (NPs) and suckling piglets (SPs), and to investigate their regulatory effects on IEC proliferation and barrier integrity, as well as the potential underlying mechanisms. In this study, compared with NPs, there were significant increases in serum triglycerides, total cholesterol, glucose, and albumin levels for SPs. The total serum BA content in SPs exhibited an obvious increase. Moreover, the expression of BA synthase cytochrome P450 27A1 (CYP27A1) was increased, and the ileal BA receptor Takeda G-coupled protein receptor 5 (TGR5) and proliferation marker Ki-67 were upregulated and showed a strong positive correlation through a Spearman correlation analysis, whereas the expression of farnesoid X receptor (FXR) and occludin was markedly downregulated in SPs and also revealed a strong positive correlation. These findings indicate that the increased synthesis and metabolism of BAs may upregulate TGR5 and downregulate FXR to promote IEC proliferation and influence barrier function; this offers a fresh perspective and evidence for the role of BAs and BA receptors in regulating intestinal development in neonatal pigs.

This paper investigates the design problem of fuzzy dynamic output feedback $H_\infty $ controller for nonlinear networked systems via mismatched membership functions and adaptive event-triggered (AET) mechanism. Firstly, an AET mechanism is introduced to save communication resources, which causes the controller and the original system's premise variables to be asynchronous. Then, considering the influence of AET and mismatched membership functions, a model of fuzzy control system is established. In addition, utilizing the Lyapunov-Krasovskii(L-K) functional, sufficient conditions for the global exponential stability of the closed-loop system with $H_\infty $ performance are derived. Besides, the controller parameters and event-triggered (ET) weight matrix are solved by a set of linear matrix inequalities (LMIs). Finally, an example is given to demonstrate the effectiveness of the proposed control method.

In this paper, the problem of secure state estimation for cyber physical systems (CPSs) with state delay and sparse sensor attacks is studied. An algorithm combining set cover approach and adaptive switching mechanism is proposed, which can realize off-line acquisition of candidate set and accurately locate the real attack mode. The contributions of this algorithm are that it can greatly reduce the search space, eliminate the impact of attacks on state estimation, improve the estimation speed and ensure the real-time performance of state estimation under the premise of effective estimation. The sufficient condition for the existence of the observer is obtained. Finally, the rapidity and effectiveness of the designed observer are verified by two examples.

The underground powerhouse of China’s Wudongde hydropower station is one of the world’s underground powerhouses with the largest span and height. Because the underground powerhouse is all located in steeply-inclined layered rock masses, and the bedding plane strike of the rock mass has a small angle with the axis of the main caverns, there is a serious risk of high sidewall instability. According to the basic principles of the structurally-controlled concept, the mechanical properties of bedding planes and other discontinuities are largely attributed to this problem. A thorough investigation was conducted into the Wudongde underground powerhouse throughout the excavation process to clarify the possible mechanisms behind these failures and to realize the dynamic adjustment of construction. First, the geological conditions and mechanical properties of the rock masses in the study area were introduced in detail. The stability of the right bank underground powerhouse’s surrounding layered rock mass was then investigated using a discontinuum-based numerical method. Special attention was paid to the excavation-induced failure mechanisms of the steeply-inclined small-angle layered rock masses at the upstream high sidewall. The results show that the failure of steeply-inclined small-angle layered surrounding rock masses presents significant three-stage characteristics, i.e., 1) rock plate forms due to the unloading opening of the bedding plane; 2) rock plate cracks due to bending; and 3) rock plate buckles or slides due to crack penetration. Based on the understanding of the deformation and failure mechanisms of steeply-inclined small-angle layered rock masses, the excavation sequence needs to be optimized according to local geological conditions. In practice, it is vital to insist on the principles of excavation by thin layers and finish all the supports before the next-step excavation.
