6. A Multi-Center, Multi-Vendor Study to Evaluate the Generalizability of a Radiomics Model for Classifying Prostate cancer: High Grade vs. Low Grade

Radiomics applied in MRI has shown promising results in classifying prostate cancer lesions. However, many papers describe single-center studies without external validation. The issues of using radiomics models on unseen data have not yet been sufficiently addressed. The aim of this study is to evaluate the generalizability of radiomics models for prostate cancer classification and to compare the performance of these models to the performance of radiologists. Multiparametric MRI, photographs and histology of radical prostatectomy specimens, and pathology reports of 107 patients were obtained from three healthcare centers in the Netherlands. By spatially correlating the MRI with histology, 204 lesions were identified. For each lesion, radiomics features were extracted from the MRI data. Radiomics models for discriminating high-grade (Gleason score ≥ 7) versus low-grade lesions were automatically generated using open-source machine learning software. The performance was tested both in a single-center setting through cross-validation and in a multi-center setting using the two unseen datasets as external validation. For comparison with clinical practice, a multi-center classifier was tested and compared with the Prostate Imaging Reporting and Data System version 2 (PIRADS v2) scoring performed by two expert radiologists. The three single-center models obtained a mean AUC of 0.75, which decreased to 0.54 when the model was applied to the external data, the radiologists obtained a mean AUC of 0.46. In the multi-center setting, the radiomics model obtained a mean AUC of 0.75 while the radiologists obtained a mean AUC of 0.47 on the same subset. While radiomics models have a decent performance when tested on data from the same center(s), they may show a significant drop in performance when applied to external data. On a multi-center dataset our radiomics model outperformed the radiologists, and thus, may represent a more accurate alternative for malignancy prediction.

Historically, medicinal plants have always had an important place in medicine. Medicinal plants processing represents a great challenge, due to their compounds sensitive to the environmental conditions that surround and degrade them. Mostly of these plants require to be dry to preserve its safety and medicinal properties; therefore, for proper drying, it is necessary to use sustainable devices that protect the desirable characteristics of plants from direct radiation. In this work, the kinetics of dehydration of three medicinal plants are presented in an indirect solar dryer. In addition, the experimental results were adjusted to nine mostly used models, to estimate the drying conditions required to achieve a desired final moisture content. Modified Page and Page were the models with better fit to experimental results. Furthermore, a computational simulation of temperature evolution and distribution inside the dryer is presented. These results agree with those obtained experimentally.

Simple Summary Computer-aided diagnosis systems to improve significant prostate cancer (PCa) diagnoses are being reported in the literature. These methods are based on either deep learning or radiomics. However, there is a lack of scientific evidence comparing these methods on the same external validation sets. The aim of our study was to compare the performance of a deep-learning model with the performance of a radiomics model for significant-PCa diagnosis on various cohorts. We collected multiparametric magnetic resonance images and pathology data from four patient cohorts (644 patients in total). One of the cohorts was used to develop a deep-learning model and a radiomics model. Both models were tested on the three remaining cohorts. The comparison shows that whereas the performance of the deep-learning model was higher on the training cohort, the radiomics model outperformed the deep-learning model in all the testing cohorts, making it a more accurate tool with which to detect clinically significant prostate cancer. Abstract The computer-aided analysis of prostate multiparametric MRI (mpMRI) could improve significant-prostate-cancer (PCa) detection. Various deep-learning- and radiomics-based methods for significant-PCa segmentation or classification have been reported in the literature. To be able to assess the generalizability of the performance of these methods, using various external data sets is crucial. While both deep-learning and radiomics approaches have been compared based on the same data set of one center, the comparison of the performances of both approaches on various data sets from different centers and different scanners is lacking. The goal of this study was to compare the performance of a deep-learning model with the performance of a radiomics model for the significant-PCa diagnosis of the cohorts of various patients. We included the data from two consecutive patient cohorts from our own center (n = 371 patients), and two external sets of which one was a publicly available patient cohort (n = 195 patients) and the other contained data from patients from two hospitals (n = 79 patients). Using multiparametric MRI (mpMRI), the radiologist tumor delineations and pathology reports were collected for all patients. During training, one of our patient cohorts (n = 271 patients) was used for both the deep-learning- and radiomics-model development, and the three remaining cohorts (n = 374 patients) were kept as unseen test sets. The performances of the models were assessed in terms of their area under the receiver-operating-characteristic curve (AUC). Whereas the internal cross-validation showed a higher AUC for the deep-learning approach, the radiomics model obtained AUCs of 0.88, 0.91 and 0.65 on the independent test sets compared to AUCs of 0.70, 0.73 and 0.44 for the deep-learning model. Our radiomics model that was based on delineated regions resulted in a more accurate tool for significant-PCa classification in the three unseen test sets when compared to a fully automated deep-learning model.

Establishment of dedicated Stroke Centers has shown to be effective on the outcome of patients with acute ischemic stroke, as well as mechanical thrombectomy (MTE) in acute large vessel occlusion. The cost-effectiveness of this treatment has also been proven in several countries, but so far not in Switzerland. We compare the pathways and economic impact of patients with acute large vessel occlusions causing acute ischemic stroke before the establishment of the stroke center and MTE in 2016 with the time afterwards in the years 2016–2020. Local data from the Swiss Stroke Registry and hospital accounting as well as economic data from a healthcare insurance company was used for evaluation in an economic model. Both payer and societal perspectives were considered, and probabilistic sensitivity analysis was undertaken to explore uncertainty. Establishment of a new Stroke Center in Central Switzerland increased the absolute number of thrombectomies from 0 in 2015 to 55 in 2016 to 83 in 2020, as well as the percentage of MTE in large vessel occlusions (LVO) from 50.9% in 2016 to 58.2% in 2020. Over a 15-year horizon, predicted average additional costs of CHF 7,978 were associated with the establishment of a new stroke center, as well as 0.60 quality-adjusted life-years (QALY) per patient and an additional survival of 0.59 years per patient. The calculated incremental cost-effectiveness ratio was therefore CHF 13,297 per QALY gained. When societal costs were included, the new stroke care model was predicted to dominate the old care model. Robustness of model results was confirmed via probabilistic sensitivity analysis. The results rely on data from a single stroke center and, therefore, cannot be generalized. Establishment of a new Stroke Center can be cost-effective and provide better outcomes in terms of functional independence as well as quality-adjusted life-years. [ABSTRACT FROM AUTHOR]

Multiparametric MRI, photographs and histology of radical prostatectomy specimens, and pathology reports of 107 patients were obtained from three healthcare centers in the Netherlands. By spatially correlating the MRI with histology, 204 lesions were identified. For each lesion, radiomics features were extracted from the MRI data. Radiomics models for discriminating high-grade (Gleason score ≥ 7) versus low-grade lesions were automatically generated using open-source machine learning software. The performance was tested both in a single-center setting through cross-validation and in a multi-center setting using the two unseen datasets as external validation. For comparison with clinical practice, a multi-center classifier was tested and compared with the Prostate Imaging Reporting and Data System version 2 (PIRADS v2) scoring performed by two expert radiologists. The three single-center models obtained a mean AUC of 0.75, which decreased to 0.54 when the model was applied to the external data, the radiologists obtained a mean AUC of 0.46. In the multi-center setting, the radiomics model obtained a mean AUC of 0.75 while the radiologists obtained a mean AUC of 0.47 on the same subset. While radiomics models have a decent performance when tested on data from the same center(s), they may show a significant drop in performance when applied to external data. On a multi-center dataset our radiomics model outperformed the radiologists, and thus, may represent a more accurate alternative for malignancy prediction.

Background: Approval of hypomethylating agents in patients with chronic myelomonocytic leukaemia is based on trials done in patients with myelodysplastic syndromes. We aimed to investigate whether hypomethylating agents provide a benefit in subgroups of patients with chronic myelomonocytic leukaemia compared with other treatments. Methods: For this retrospective cohort study, data were retrieved between Nov 30, 2017, and Jan 5, 2019, from 38 centres in the USA and Europe. We included non-selected, consecutive patients diagnosed with chronic myelomonocytic leukaemia, who received chronic myelomonocytic leukaemia-directed therapy. Patients with acute myeloid leukaemia according to 2016 WHO criteria at initial diagnosis (ie, ≥20% blasts in the bone marrow or peripheral blood) or with unavailability of treatment data were excluded. Outcomes assessed included overall survival, time to next treatment, and time to transformation to acute myeloid leukaemia. Analyses were adjusted by age, sex, platelet count, and Chronic myelomonocytic leukaemia-Specific Prognostic Scoring System (CPSS). Patients were grouped by first received treatment with either hydroxyurea, hypomethylating agents, or intensive chemotherapy, and stratified by risk according to blast count, French-American-British subtype, CPSS, WHO 2016 subtype, and the eligibility criteria of the DACOTA trial (NCT02214407). Findings: 949 patients diagnosed with chronic myelomonocytic leukaemia between April 13, 1981, and Oct 26, 2018, were included. Median follow-up was 23·4 months (IQR 11·5–42·3) from diagnosis and 16·2 months (6·6–31·6) from start of first-line treatment. 412 (43%) of 949 patients received hypomethylating agents as first treatment, 391 (41%) hydroxyurea, and 83 (9%) intensive chemotherapy. Adjusted median overall survival for patients treated with hydroxyurea versus hypomethylating agents was 15·6 months (95% CI 13·1–17·3) versus 20·7 months (17·9–23·4); hazard ratio (HR) 1·39 (1·17–1·65; p=0·0002) and 14·0 months (9·8–17·2) versus 20·7 months (17·9–23·4; HR 1·55 [1·16–2·05]; p=0·0027) for those treated with intensive chemotherapy versus hypomethylating agents. In patients with myeloproliferative chronic myelomonocytic leukaemia (myeloproliferative CMML), median overall survival was 12·6 months (10·7–15·0) versus 17·6 months (14·8–21·5; HR 1·38 [1·12–1·70]; p=0·0027) for patients treated with hydroxyurea versus hypomethylating agents, and 12·3 months (8·4–16·6) versus 17·6 months (14·8–21·5; HR 1·44 [1·02–2·03]; p=0·040) for intensive chemotherapy versus hypomethylating agents. Background: Experimental and clinical data suggest that reducing inflammation without affecting lipid levels may reduce the risk of cardiovascular disease. Yet, the inflammatory hypothesis of atherothrombosis has remained unproved. Methods: We conducted a randomized, double-blind trial of canakinumab, a therapeutic monoclonal antibody targeting interleukin-1β, involving 10,061 patients with previous myocardial infarction and a high-sensitivity C-reactive protein level of 2 mg or more per liter. The trial compared three doses of canakinumab (50 mg, 150 mg, and 300 mg, administered subcutaneously every 3 months) with placebo. The primary efficacy end point was nonfatal myocardial infarction, nonfatal stroke, or cardiovascular death. RESULTS: At 48 months, the median reduction from baseline in the high-sensitivity C-reactive protein level was 26 percentage points greater in the group that received the 50-mg dose of canakinumab, 37 percentage points greater in the 150-mg group, and 41 percentage points greater in the 300-mg group than in the placebo group. Canakinumab did not reduce lipid levels from baseline. At a median follow-up of 3.7 years, the incidence rate for the primary end point was 4.50 events per 100 person-years in the placebo group, 4.11 events per 100 person-years in the 50-mg group, 3.86 events per 100 person-years in the 150-mg group, and 3.90 events per 100 person-years in the 300-mg group. The hazard ratios as compared with placebo were as follows: in the 50-mg group, 0.93 (95% confidence interval [CI], 0.80 to 1.07; P = 0.30); in the 150-mg group, 0.85 (95% CI, 0.74 to 0.98; P = 0.021); and in the 300-mg group, 0.86 (95% CI, 0.75 to 0.99; P = 0.031). The 150-mg dose, but not the other doses, met the prespecified multiplicity-adjusted threshold for statistical significance for the primary end point and the secondary end point that additionally included hospitalization for unstable angina that led to urgent revascularization (hazard ratio vs. placebo, 0.83; 95% CI, 0.73 to 0.95; P = 0.005). Canakinumab was associated with a higher incidence of fatal infection than was placebo. There was no significant difference in all-cause mortality (hazard ratio for all canakinumab doses vs. placebo, 0.94; 95% CI, 0.83 to 1.06; P = 0.31). Conclusions: Antiinflammatory therapy targeting the interleukin-1β innate immunity pathway with canakinumab at a dose of 150 mg every 3 months led to a significantly lower rate of recurrent cardiovascular events than placebo, independent of lipid-level lowering. We studied HLA class I (HLA-A, -B) and class II (HLA-DRB1, -DQB1) allele groups and alleles by PCR-SSP based typing in a total of 15,318 mixed ancestry Mexicans from all the states of the country divided into 78 sample sets, providing information regarding allelic and haplotypic frequencies and their linkage disequilibrium, as well as admixture estimates and genetic substructure. We identified the presence of 4268 unique HLA extended haplotypes across Mexico and find that the ten most frequent (HF > 1%) HLA haplotypes with significant linkage disequilibrium (Δ’≥0.1) in Mexico (accounting for 20% of the haplotypic diversity of the country) are of primarily Native American ancestry (A*02~B*39~DRB1*04~DQB1*03:02, A*02~B*35~DRB1*08~DQB1*04, A*68~B*39~DRB1*04~DQB1*03:02, A*02~B*35~DRB1*04~DQB1*03:02, A*24~B*39~DRB1*14~DQB1*03:01, A*24~B*35~DRB1*04~DQB1*03:02, A*24~B*39~DRB1*04~DQB1*03:02, A*02~B*40:02~DRB1*04~DQB1*03:02, A*68~B*35~DRB1*04~DQB1*03:02, A*02~B*15:01~DRB1*04~DQB1*03:02). Admixture estimates obtained by a maximum likelihood method using HLA-A/-B/-DRB1 as genetic estimators revealed that the main genetic components in Mexico as a whole are Native American (ranging from 37.8% in the northern part of the country to 81.5% in the southeastern region) and European (ranging from 11.5% in the southeast to 62.6% in northern Mexico). African admixture ranged from 0.0 to 12.7% not following any specific pattern. We were able to detect three major immunogenetic clusters correlating with genetic diversity and differential admixture within Mexico: North, Central and Southeast, which is in accordance with previous reports using genome-wide data. Our findings provide insights into the population immunogenetic substructure of the whole country and add to the knowledge of mixed ancestry Latin American population genetics, important for disease association studies, detection of demographic signatures on population variation and improved allocation of public health resources. To demonstrate the prevalence of cesearean sections (C-sections) in Ecuador and their distribution between private and public health centers.An observational population-based study was conducted of patients discharged from public and private hospitals in Ecuador after a C-section or vaginal delivery. Data were collected by the Ecuadorian National Institute of Statistics and Census (INEC) between 2001 and 2013.The overall national C-section rate in the private health care system is double the rate in the public health care system. Over the 13 years of the study, C-sections accounted for 57.5% of births in the private sector, while the public sector proportion did not exceed 22.3%. Countrywide, less than 36% of C-sections were found to be clinically justified by parallel analysis of absolute or relative indications. Acute fetal distress (AFD) was more frequently reported in private centers compared to public ones (446 per 10 000 live births versus 274 per 10 000). Since 2001, the number of births by cesarean section increased by more than 50% (RNational data show that C-sections are performed more frequently in Ecuador than the rate recommended by the World Health Organization, especially in the private health care system. Private centers also report higher rates of AFD, which implies that this diagnosis is either overused in private centers or underrecognized in public centers. Although several factors might be influencing these trends, no data are available to determine the relative importance of economics, practicality, and medical or personal concerns of mothers and physicians in deciding which method of delivery should be used.Demostrar la prevalencia de las cesáreas en Ecuador y su distribución entre centros privados y públicos de salud.Se realizó un estudio de observación basado en la población de pacientes dadas de alta de hospitales públicos y privados en Ecuador después de una cesárea o un parto vaginal. Los datos fueron recopilados por el Instituto Nacional de Estadísticas y Censo (INEC) de Ecuador entre el 2001 y el 2013.La tasa nacional de cesáreas en el sistema privado de atención de salud es el doble de la tasa que se observa en el sistema público de salud. Durante los 13 años que duró el estudio, las cesáreas representaron 57,5% de los nacimientos en el sector privado, mientras que la proporción en el sector público no superó 22,3%. A nivel de todo el país, se observó que menos de 36% de las cesáreas estaban clínicamente justificadas con un análisis paralelo de indicaciones absolutas o relativas. Se notificaron casos de sufrimiento fetal agudo con mayor frecuencia en los centros privados comparados con los públicos (446 por 10 000 nacidos vivos frente a 274 por 10 000 n.v.). Desde el 2001, el número de nacimientos por cesárea aumentó más de 50% (RLos datos nacionales muestran que las cesáreas se realizan en Ecuador con una frecuencia mayor a la tasa recomendada por la Organización Mundial de la Salud, especialmente en el sistema privado de atención de salud. BACKGROUND:Thrombin potently activates platelets through the protease-activated receptor PAR-1. Vorapaxar is a novel antiplatelet agent that selectively inhibits the cellular actions of thrombin through antagonism of PAR-1.METHODS:We randomly assigned 26,449 patients who had a history of myocardial infarction, ischemic stroke, or peripheral arterial disease to receive vorapaxar (2.5 mg daily) or matching placebo and followed them for a median of 30 months. The primary efficacy end point was the composite of death from cardiovascular causes, myocardial infarction, or stroke. After 2 years, the data and safety monitoring board recommended discontinuation of the study treatment in patients with a history of stroke owing to the risk of intracranial hemorrhage.RESULTS:At 3 years, the primary end point had occurred in 1028 patients (9.3%) in the vorapaxar group and in 1176 patients (10.5%) in the placebo group (hazard ratio for the vorapaxar group, 0.87; 95% confidence interval [CI], 0.80 to 0.94; P

Large variability in dissolved organic carbon (DOC) uptake rates has been reported for headwater streams, but the causes of this variability are still not well understood. Here we assessed acetate uptake rates across 11 European streams comprising different ecoregions by using whole‐reach pulse acetate additions. We evaluated the main climatic and biogeochemical drivers of acetate uptake during two seasonal periods. Our results show a minor influence of sampling periods but a strong effect of climate and dissolved organic matter (DOM) composition on acetate uptake. In particular, mean annual precipitation explained half of the variability of the acetate uptake velocities (VfAcetate) across streams. Temperate streams presented the lowest VfAcetate, together with humic‐like DOM and the highest stream respiration rates. In contrast, higher VfAcetate were found in semiarid streams, with protein‐like DOM, indicating a dominance of reactive, labile compounds. This, together with lower stream respiration rates and molar ratios of DOC to nitrate, suggests a strong C limitation in semiarid streams, likely due to reduced inputs from the catchment. Overall, this study highlights the interplay of climate and DOM composition and its relevance to understand the biogeochemical mechanisms controlling DOC uptake in streams. That degradation can be assessed by measuring the uptake of simple compounds of carbon or nitrogen such as acetate or nitrate. Most small-scale fisheries of large floodplain rivers are still managed under conventional top-down regulations that limit the application of an ecosystem approach to fisheries ( EAF) due to inappropriate legal frameworks. Using the Parana-Paraguay River fisheries (Argentina) as an example, this study examines the extent to which existing provincial legislations can be prepared for the adoption of an EAF. An Ecosystem Fishing Legal Approach ( EFLA) framework is proposed based on different criteria across an environmental-ecological, fishing, social, economic and institutional template. Policy Component Scores ( PCS) and an Integrated Policy Legal Index ( IPLI) were applied to assess the degree of compliance by current provincial legislations to EAF implementation. Cluster analysis was used to recognise the potential for articulating a legal framework at a basin scale. The EFLA framework, which provided an accurate picture of how provinces were poorly prepared to adopt an EAF for the Paraguay-Parana fisheries, and represents a suitable tool that can be adapted and extended to other basins around the world. This study aims to assess operating room efficiency for resident-performed elective phacoemulsification surgeries done under local anesthesia by measuring different key performance indicators and comparing this with international benchmark data.Patients and Methods: This is a prospective cross-sectional study done in the Department of Ophthalmology of the Philippine General Hospital, the National University Hospital. The operating room milestones were noted and recorded by a single third-party observer in randomly selected operating rooms from April to June 2019.Results: Fifty-six phacoemulsification cases in randomly selected rooms fulfilling both inclusion and exclusion criteria were observed. None of the cases started on or before the scheduled 6:30 a.m. cutting time, with an average of 34 (SD 8.53) minutes late. Entry lag was above the median, while exit lag and turnover time were above the 95th percentile compared to benchmarking data. Segment analysis also showed an increased entry lag (35.11% vs 21.5%), significantly higher than benchmarks (t: 10.99, df: 55, p< 0.01). Comparison with proposed targets in other studies also showed an increased time for entry lag.Conclusion: This study determined that entry lag is the performance indicator that should be addressed to improve efficiency. Nowadays, data fusion is one of the trends in geomatics sciences, due to the necessity of merging data from different kind of sensors and periods of time. Also, to extrract the maximum information from data and useful multitemporal analysis, an exact geoconnection of all datasets in a common and stable reference system is essential. The results of the application of a methodology for an integrated orientation into a common reference system using data obtained by LiDAR systems, digital and historical photogrammetric flights dataset, used for proper analysis in multitemporal studies, are presented in this paper. In order to analyse the results of the presented methodology, several photogrammetric datasets have been used. This data corresponds with digital and analogic data. The most current flight (2010) combines data obtained with digital photogrammetric camera and LiDAR sensor which will be used as reference model for all subsequent photogrammetry flights. The philosophy of the methodology consists of orientating all photogrammetric flights to the DEM obtained by LiDAR data. All the models obtained from every photogrammetric block are comparable in terms of the geometric resolution of each one. For that reason, altimetric stable points are extracted automatically from the LiDAR points cloud to use these points such as altimetric control point in the different flights that must be oriented. Using LiDAR control points, we demonstrate the improvement in the results between initial orientation and final results. Also it is possible to improve the planimetric correspondence between different photogrammetric blocks using only altimetric control points iteratively. [ABSTRACT FROM AUTHOR]

Nowadays, the development of UAS has allowed the obtaining of high resolution and accurate cartographic products, such as DSMs and orthoimages. These products can be used in studies of the evolution of landslides. The stability of slopes is a main issue because, among others, it can suppose a serious risk to infrastructures. Until this moment, some studies for analysing slope movements have been carried out using the comparison of positions of well-defined points or comparing two surfaces obtained from two DSMs. In this paper we present a methodology for analysing landslides based on some linear elements located on the terrain. More specifically, we analyse some lines of road sections which are located on an unstable slope, checking the movements of the landslide and their effect on the infrastructure. The methodology includes the obtaining of high resolution orthoimages and DSMs which correspond to two or more epochs of the same landslide, the 3D digitizing of common linear elements, and the computing of the displacements of matched lines (from two epochs) using positional control methods based on lines. The proposed methodology has been tested using two DSMs and two orthoimages (corresponding to two epochs) obtained from two photogrammetric projects developed with an UAS. This real case was applied to an unstable slope with landslides which affected several sections of a road. The results have demonstrated the viability of the proposed methodology in analysing the behaviour of the landslide and more specifically, the effects on these infrastructures. [ABSTRACT FROM AUTHOR]

Hydrocarbons abound in the environment and microorganisms are often capable of assimilating and degrading these normally recalcitrant molecules. In order to achieve this, bacteria have developed specific adaptive mechanisms; much of the adaptive response is brought about by transcriptional activation of genes and is controlled by one or two component regulatory systems, global regulators and DNA binding proteins. The expressed gene products are then able to degrade the molecules and often take advantage of the stored energy imparted by the physicochemical properties of the hydrocarbon structure. The response of gene regulators to the presence of hydrocarbons such as toluene in the environment allows initiation or inhibition of transcription, so that the rate of synthesis of metabolically important gene products is adaptively modified. Microorganisms which mount the most appropriate physiological adaptation are then able to proliferate in the changing environment. Poly-(3-hydroxybutyrate) [ P(3 HB)] is a polyester synthesized as a carbon and energy reserve material by a wide number of bacteria. This polymer is characterized by its thermo-plastic properties similar to plastics derived from petrochemical industry, such as polyethylene and polypropylene. Furthermore, P(3 HB) is an inert, biocompatible and biodegradable material which has been proposed for several uses in medical and biomedical areas. Currently, only few bacterial species such as C upriavidus necator, A zohydromonas lata and recombinant E scherichia coli have been successfully used for P(3 HB) production at industrial level. Nevertheless, in recent years, several fermentation strategies using other microbial models such as A zotobacter vinelandii, A. chroococcum, as well as some methane-utilizing species, have been developed in order to improve the P(3 HB) production and also its mean molecular weight. Abstract: Little is known about the litterfall dynamics and the effects of stand-management practices under semiarid Mediterranean conditions. Our aim was to provide data on the annual amount and the seasonal distribution of litterfall in a 15-year-old afforestation area of Aleppo pine under three different intensities of thinning applied 5years before the study, and to analyse the relationships between litterfall and tree and stand characteristics. Three different but typical overstory thinning regimes [75% of mean basal area removed (T75), 60% (T60), and 48% (T48)] were performed on 16 randomly established 20×20m plots [4 per treatment+4 control unthinned plots (T0)]. Two trees per plot (8 trees per stand) were randomly assigned for monthly litterfall measurements from June 2009 to May 2010. In each tree, three circular traps were hung in random positions below its canopy. In total, 32 trees were monitored by means of 96 traps (24 traps per stand). Annual litterfall showed two seasonal peaks, ranging from 0.95Mgha
−1 yr−1 in T75 to 2.28Mgha−1 yr−1 in the unthinned control stands (T0). However, at the tree level, litterfall ranged from 4.0kg per treeyr−1 of total litterfall in T75 to 2.0kg per treeyr−1 for T0. Trees in T0 tended to shed more needles and twigs measured per m−2 yr−1 than those at low densities, despite having less mean crown projection (T75=86gm−2 yr−1 ; T60=91gm−2 yr−1 ; T48=94gm−2 yr−1 ; T0=102gm−2 yr−1 ). Also, control trees shed a greater proportion (%) of needles and twigs with respect to the total amount of litterfall, mainly from August to November in all cases. Annual litterfall and needle fall showed a significant linear relationship with regard to tree size and stand density, canopy cover, basal area, and annual biomass production. Although only oneyear of data are shown and relative caution should be taken into account until further research is concluded, our results show that greater needle fall could be stimulated at high tree densities by tree competition for resources. Simple Summary: Computer-aided diagnosis systems to improve significant prostate cancer (PCa) diagnoses are being reported in the literature. These methods are based on either deep learning or radiomics. However, there is a lack of scientific evidence comparing these methods on the same external validation sets. The aim of our study was to compare the performance of a deep-learning model with the performance of a radiomics model for significant-PCa diagnosis on various cohorts. We collected multiparametric magnetic resonance images and pathology data from four patient cohorts (644 patients in total). One of the cohorts was used to develop a deep-learning model and a radiomics model. Both models were tested on the three remaining cohorts. The comparison shows that whereas the performance of the deep-learning model was higher on the training cohort, the radiomics model outperformed the deep-learning model in all the testing cohorts, making it a more accurate tool with which to detect clinically significant prostate cancer. The computer-aided analysis of prostate multiparametric MRI (mpMRI) could improve significant-prostate-cancer (PCa) detection. Various deep-learning- and radiomics-based methods for significant-PCa segmentation or classification have been reported in the literature. To be able to assess the generalizability of the performance of these methods, using various external data sets is crucial. While both deep-learning and radiomics approaches have been compared based on the same data set of one center, the comparison of the performances of both approaches on various data sets from different centers and different scanners is lacking. The goal of this study was to compare the performance of a deep-learning model with the performance of a radiomics model for the significant-PCa diagnosis of the cohorts of various patients. We included the data from two consecutive patient cohorts from our own center (n = 371 patients), and two external sets of which one was a publicly available patient cohort (n = 195 patients) and the other contained data from patients from two hospitals (n = 79 patients). Using multiparametric MRI (mpMRI), the radiologist tumor delineations and pathology reports were collected for all patients. During training, one of our patient cohorts (n = 271 patients) was used for both the deep-learning- and radiomics-model development, and the three remaining cohorts (n = 374 patients) were kept as unseen test sets. The performances of the models were assessed in terms of their area under the receiver-operating-characteristic curve (AUC). Whereas the internal cross-validation showed a higher AUC for the deep-learning approach, the radiomics model obtained AUCs of 0.88, 0.91 and 0.65 on the independent test sets compared to AUCs of 0.70, 0.73 and 0.44 for the deep-learning model. Our radiomics model that was based on delineated regions resulted in a more accurate tool for significant-PCa classification in the three unseen test sets when compared to a fully automated deep-learning model. This study was designed to determine which single bolus dose of remifentanil in combination with propofol and nitrous oxide is best to control the haemodynamic, autonomous and somatic responses in patients scheduled for dilatation and curettage of the uterine cervix. We evaluated the adequacy of different bolus doses of remifentanil, associated with propofol and nitrous oxide, for dilatation and curettage in a prospective double-blind study. The groups were similar with regard to biometric data and duration of surgery. The poor results using the lowest dose of remifentanil obliged us to abandon this dose. The total dose of remifentanil was larger in Group C(100 ± 5.7 μg vs. 65 ± 4.1 μg in Group B; P < 0.05), but more patients required extra bolus injections in Group B (69% vs. 38% in Group C; P < 0.01). Recovery times were significantly shorter in Group C. Aldrete scores when leaving the operation room was similar. Remifentanil 1.5 μgkg

Study design: A prospective double blind cross over trial of intravenous 4-Aminopyridine (4-AP).OBJECTIVE:&EMSP;:To determine the efficacy of this drug in the treatment of spinal cord injured (SCI) patients for neurologic impairment, pain and spasticity. SETTING:&EMSP;:The post anesthesia care unit (PACU) of a tertiary care acute hospital. METHODS:&EMSP;:Twelve paraplegic patients were enrolled in a double blind cross over intravenous trial of 4-Aminopyridine (4-AP). Thirty milligrams of 4-AP or placebo were administered over a 2 h period. Patients were serially examined during and after the infusion clinically for pain, sensorimotor function, hypertonicity and motor control using electromyography (EMG). Samples of blood and cerebrospinal fluid (CSF) were also analyzed at similar intervals. RESULTS:&EMSP;:Despite penetration of 4-AP into the CSF, no significant differences were noted in the clinical and EMG parameters at the times measured. Individual changes in sensory function were reported by some patients in both the placebo and 4-AP trials, however mean values were not robust. Frequently, patients complained of unpleasant symptoms during the 4-AP infusion.
