Background Fear of cancer recurrence (FCR) affects virtually all patients who have been treated for cancer, to varying degrees. Breast cancer survivors who carry a BRCA1 or BRCA2 gene mutation are at high risk of cancer recurrence. No study has yet assessed FCR specifically in this population. Objectives This cross-sectional study, conducted in women who were treated for breast cancer and carrying a BRCA1/2 mutation, aimed to: (1) assess the mean level of FCR and estimate the proportion of patients with clinical levels of FCR; (2) examine the relationships between FCR and selected psychological variables (e.g., avoidance, intolerance to uncertainty) and quality of life; (3) explore whether FCR levels vary as a function of the past preventive treatment received; and (4) to assess the associations between FCR and the presence of decisional conflict or regret regarding the various preventive options. Method Participants were recruited through an e-mail sent to an oncogenetic network mailing list (Réseau ROSE). Participants were asked to complete a battery of questionnaires online assessing FCR and other psychological and quality of life variables. Results A total of 89 women completed the survey. Most participants had undergone a preventive mastectomy (62.9%) and a preventive salpingo-oophorectomy (75.3%) at the time of the study. The mean Fear of Cancer Recurrence Inventory-severity score was 16.8, which exceeds the clinical cut-off score of 13, and 70.8% of the participants showed a clinical level of FCR. FCR was significantly associated with higher levels of anxiety and depression, and higher avoidance and intolerance of uncertainty, but not with quality of life. No significant difference was observed on the total FCR score between women who had received preventive surgery (mastectomy and/or salpingo-oophorectomy) and those considering it, and those not considering it. The association was significant between higher FRC scores and greater decisional conflicts and regrets about choosing to undergo preventive surgery. Conclusion These data suggest that FCR is a significant problem for breast cancer survivors carrying a BRCA1/2 genetic mutation, even after undergoing a prophylactic surgery. This highlights the importance of providing these women with specific psychological intervention focusing on FCR. Context. Over 70% of cancer survivors live with additional comorbidities requiring coordinated care across primary care and oncology. Interventions to improve survivorship care planning have shown mixed results and few studies are focused on improving care coordination, specifically in the context of real-world practice. Objective: To examine effect of a care coordination intervention (Project CONNECT) for underserved breast and colorectal cancer survivors on change in patient-reported outcomes. Study Design and Analysis: Using a pre-post quasi-experimental design, eligible patients were administered a telephone survey pre-intervention and 6- and 12-months post-intervention to measure patient-reported care coordination. Summary statistics described patient characteristics and Generalized Estimating Equation assessed population-average changes in care coordination adjusted for patient characteristics. Setting: An integrated, safety-net health system with community-oriented primary care clinics and specialty tertiary care in Dallas, TX providing care to under- and uninsured patients. Population: 285 patients diagnosed with breast or colorectal cancer (stages I-III) with at least one other chronic condition (diabetes, hypertension, heart disease, chronic kidney disease, or chronic obstructive pulmonary disease). Intervention: Project CONNECT included an EMR-based registry to identify eligible cancer survivors, a nurse coordinator embedded in oncology connecting patients to primary care, and a coach to support nurse coordinator in integrating oncology and primary care. Outcome Measure: Adapted Picker Patient Experience of Care (coordination subscale) measure scores on a 1-3 scale, with a higher score representing worse perceived care coordination. Results: 70% of patients had breast cancer and 30% had colorectal cancer. Average age of eligible patients was 55 years (SD: 15.1). Our sample was predominantly women (79%) and over half were Hispanic ethnicity. After adjusting for patient characteristics, patient-reported care coordination improved significantly after intervention ([beta] = -0.062 [0.014 - -0.108]). Chronic glomerular and tubular nephrotoxicity is reported in 20-50% and 20-25%, respectively, of children and adolescents treated with ifosfamide and 60-80% and 10-30%, respectively, of those given cisplatin. Up to 20% of children display evidence of chronic glomerular damage after unilateral nephrectomy for a renal tumour. Overall, childhood cancer survivors have a ninefold higher risk of developing renal failure compared with their siblings. Such chronic nephrotoxicity may have multiple causes, including chemotherapy, radiotherapy exposure to kidneys, renal surgery, supportive care drugs and tumour-related factors. These cause a wide range of chronic glomerular and tubular toxicities, often with potentially severe clinical sequelae. Many risk factors for developing nephrotoxicity, mostly patient and treatment related, have been described, but we remain unable to predict all episodes of renal damage. This implies that other factors may be involved, such as genetic polymorphisms influencing drug metabolism. Although our knowledge of the long-term outcomes of chronic nephrotoxicity is increasing, there is still much to learn, including how we can optimally predict or achieve early detection of nephrotoxicity.
