Advances in engineering and health science have brought a significant improvement in health care and increased life expectancy. As a result, there has been a substantial growth in the number of older adults around the globe, and that number is rising. According to a United Nations report, between 2015 and 2030, the number of adults over the age of 60 is projected to grow by 56%, with the total reaching nearly 2.1 billion by the year 2050 [1]. Because of this, the cost of traditional health care continues to grow proportionally. Additionally, a significant portion of the elderly have multiple, simultaneous chronic conditions and require specialized geriatric care. However, the required number of geriatricians to provide essential care for the existing population is four times lower than the actual number of practitioners, and the demandsupply gap continues to grow [2]. The strategic Context in East Asia has changed considerably since the US first proposed theatre-missile-defence (TMD) cooperation with Japan and South Korea. North Korea's missile programme continues unabated, and China's assertiveness has increasingly become a cause of regional concern. Although the direct missile threat to South Korea is greater than that to Japan, the latter seems closer to a positive decision on deployment than the former. TMD deployments by both Japan and South Korea would enhance rather than undermine deterrence, and would therefore be in the interests of the US, Japan and South Korea. NATO enlargement has been an important issue on the international security agenda in 1997 and as the Czech Republic, Hungary and Poland have been invited to join NATO the US Senate is preparing for the ratification of this enlargement. Dr. Stephen A Cambone here looks at the debate in the Senate on NATO enlargement and at the worries which Senators have voiced concerning the process. The concerns fall into three categories: the problem of burdensharing—are the Europeans willing to con‐tribute their share of the cost? Future enlargement—if the costs of the first enlargement creates problems, will it be possible to enlarge even further in a second round? And lastly, Russia's relations to the West and its reaction to the enlargement.
