A patient presented with overlapping clinical and laboratory features of 2 rare autoinflammatory diseases, NLRP1-associated autoinflammation with arthritis and dyskeratosis and familial multiple self-healing palmoplantar carcinoma. Her severe inflammatory attack was treated with the IL-1 receptor-α inhibitor anakinra along with the Janus kinase inhibitor ruxolitinib. Three years into the treatment, the patient’s inflammatory symptoms are completely in remission.

In this paper, we consider generalizations of the Galilean space endowed with an action of a nonlinear transformation group. Background: AxenfeldRieger syndrome is a genetically heterogeneous group of morphogenesis disorders associated with abnormal development of the anterior segment of the eye, teeth, the organ of hearing, and abdominal region. Сongenital glaucoma, which is refractory to standard methods of treatment due to pronounced changes in the structures of the anterior segment of the eyeball and other comorbidities, is frequent manifestation of this syndrome. There are several methods for treating glaucoma in patients with AxenfeldRieger syndrome: sinus trabeculectomy, implantation of drainage devices and transscleral cyclocoagulation. Сlinical case description: A patient with congenital glaucoma associated with AxenfeldRieger syndrome underwent sinus trabeculectomy with posterior scleral trepanation in the right eye and valve drainage implantation in the left eye at the S. Fyodorov Eye Microsurgery Federal State Institution, Moscow due to the intraocular pressure decompensation. In the postoperative period, an encapsulated cyst around the body of the drainage was detected, and then a revision of the operation area was performed. After the anti-glaucoma operations, the intraocular pressure compensation was achieved in the follow-up period up to 9 months. Conclusion: Depending on the degree of the changes in the anterior chamber angle structures, sinus trabeculectomy or valve drainage implantation are the methods of choice for the surgical treatment aimed at the intraocular pressure compensation and visual function preservation in patients with Axenfeld-Rieger syndrome.

Зазначено, що для вирішення задачі розробки інформаційної технології планування підвищення рівня зрілості підмножини процесів еталонної моделі зрілості SPICE, попередньо це питання необхідно розглянути на вербальному рівні як технологію (послідовність етапів) реалізації поставленої проблеми. На першому етапі проведено формалізацію структури еталонної моделі SPICE, яка складається з множини окремих процесів. Ця множина представлена як ієрархічна структура: перший рівень – множина процесів і підпроцесів; другий рівень – множина груп процесів; третій рівень – множина категорій процесів. На другому етапі розглянуто метод оцінки рівня можливості окремого процесу/підпроцесу еталонної моделі зрілості SPICE, який наведено з погляду двох вимірів моделі SPICE: призначення процесів; атрибути процесів (вимірні характеристики, необхідні для управління процесом і підвищення можливості його виконання). Третій етап технології присвячено синтезу моделі планування розвитку підмножини процесів моделі SPICE, яка визначає рівень якості окремої складової процесу розробки програмного забезпечення (ПР ПЗ), що в свою чергу позитивно впливає на рівень розробки програмних систем. Оцінка та планування рівня можливості підмножини процесів реалізується на деякому плановому періоді в умовах обмежених ресурсів на основі їх оптимального розподілу з урахуванням важливості окремих процесів та їх практик упродовж планового періоду. На четвертому етапі технології розглядається алгоритм планування розвитку підмножини процесів еталонної моделі зрілості SPICE на основі методу послідовного аналізу варіантів. Це обумовлено в першу чергу тим, що модель оптимізації має адитивну цільову функцію. Метод дозволяє під час його використання відкидати ті допустимі рішення, які не містять оптимальних. У подальшому при конкретизації моделі планується вибрати алгоритм, який відноситься до цього методу і адаптувати його до розв’язання поставленої задачі. П’ятий етап присвячено інформаційній технології реалізації розробленої моделі та алгоритму. На цьому етапі вирішується наступна множина задач. Аналіз бізнес-процесів технології покращення якості ПР ПЗ. Визначення вимог до ПЗ. Формування діаграми варіантів використання. Розробка моделей даних. Обґрунтування вибору інструментарію для розробки ПЗ. Вибір еталонної системної архітектури. Далі кодування і тестування ПЗ. На шостому етапі на основі інформаційної технології формується множина варіантів плану підвищення якості підмножини процесів моделі зрілості SPICE з метою підтримки прийняття рішення керівництвом ІТ-компанії. Для цього попередньо визначається множина ефективних рішень, яка пропонується для визначення остаточного варіанта, який реалізується у подальшому.

Background. The research presents the results of a study of 212 clinical cases of a new coronavirus infection in children of Penza for 2021-2022. The results of the features of the clinical manifestations of SARS-CoV-2 infection in pediatric patients are presented, as well as data on the presence/absence of complications and the timing of treatment of patients at the State Medical Institution “N.F. Filatov Medical Center” in Penza. The purpose of the study is to analyze the clinical features of the course of COVID-19 in hospitalized children, depending on their age. Materials and methods. From 01.12.2021 to 30.11.2022, a study was conducted of 212 confirmed clinical cases of SARS-CoV-2 infection in children hospitalized at the State Medical Institution “N.F. Filatov Medical Center” in Penza. Etiological verification was carried out by PCR with the isolation of the SARS-CoV-2 virus. The method is based on the detection of a fragment of nucleic acid that is specific to the SARS-CoV-2 virus. The material for the study is a smear from the mucous membrane of the nasopharynx. Results and conclusions. The observation revealed an unreliable prevalence of cases of the disease of boys, compared with girls. In the age category up to 1 year, the incidence of the disease is higher than in children of other age groups. Among the clinical symptoms, the most frequent were fever, cough, nasal congestion. Uncharacteristic symptoms: repeated vomiting, abdominal pain, nausea, diarrhea. The course of COVID-19 in pediatric patients, in most cases, was accompanied by acute pharyngitis and pneumonia, less often – acute bronchitis, acute tracheitis, acute stenosing laryngotracheitis, acute tonsillitis and acute pharyngotracheitis.

Зазначено, що процесний підхід є засобом наукового дослідження і практичної зміни навколишнього світу, який дозволяє представляти та реалізовувати виробничі системи у вигляді множини процесів. У роботі розглядаються особливості використання такого підходу до дослідження людської діяльності, пов’язаної з розробкою програмного забезпечення (ПЗ). Представлено перелік науковців, які звернули увагу на необхідність реалізації процесного підходу до розробки ПЗ і на цій основі було введено поняття процесу розробки ПЗ (ПР ПЗ). Визначено поняття моделі життєвого циклу (МЖЦ). Підкреслено, що найбільш відомими наборами МЖЦ є моделі зрілості СММІ та SPICE. Під покращенням ПР ПЗ у роботі розуміється множина дій, спрямованих на поліпшення його характеристик шляхом виконання деякого набору заходів, які базуються на інженерії якості, у тому числі на TQM. Підкреслено, що моделі зрілості відносяться до однієї з двох головних категорій: безперервні (continuous) і дискретні (staged). Модель СММІ реалізується на основі двох підходів, а модель SPICE тільки на основі безперервного підходу. На теперішній час модель СММІ формалізовано, що дозволило планувати розвиток ПР ПЗ або його окремих процесів в умовах обмежених ресурсів. З погляду еталонної моделі SPICE представлена формалізація її структури у вигляді наступної ієрархії: множина груп процесів; множина категорій; множина процесів; множина практик. На основі наведеної структури проведена формалізація оцінки рівня можливості окремого процесу моделі SPICE. Розроблена модель стала основою для розробки оптимізаційної задачі планування розвитку підмножини процесів моделі зрілості SPICE. З метою аналізу цієї задачі і розробки алгоритму її реалізації в роботі вона представлена у скороченому вигляді. Її цільова функція є адитивною і визначає інтегральний показник збільшення рівня можливості підмножини процесів на всьому плановому періоді. Модель має ресурсне обмеження, яке характеризується тим, що ресурси, які не використано на заданому підперіоді, можуть бути реалізовані на наступних підперіодах планування. Показано, що для оптимізації такої моделі найбільш придатним є метод послідовного аналізу варіантів, який може бути реалізованим на основі різних алгоритмів. У роботі використано алгоритм «Київський віник», який є багатокроковим процесом, на кожному підперіоді планування якого реалізується «відмітання» деякої множини неконкурентоспроможних варіантів.

Abstract Optical techniques are utilized for the non-invasive analysis of the zebrafish cardiovascular system at early developmental stages. Being based mainly on conventional optical microscopy components and image sensors, the wavelength range of the collected and analyzed light is not out of the scope of 400–900 nm. In this paper, we compared the non-invasive optical approaches utilizing visible and near infrared range (VISNIR) 400–1000 and the shortwave infrared range (SWIR) 900–1700 nm. The transmittance spectra of zebrafish tissues were measured in these wavelength ranges, then vessel maps, heart rates, and blood flow velocities were calculated from data in VISNIR and SWIR. An increased pigment pattern transparency was registered in SWIR, while the heart and vessel detection quality in this range is not inferior to VISNIR. Obtained results indicate an increased efficiency of SWIR imaging for monitoring heart function and hemodynamic analysis of zebrafish embryos and larvae and suggest a prolonged registration period in this range compared to other optical techniques that are limited by pigment pattern development.

Background: Implantation of various drainage devices is one of the treatments for refractory glaucoma, resistant to traditional surgical methods. The most commonly used glaucoma drainage device for refractory glaucoma is the Ahmed glaucoma valve. However, the long-term success rate of the surgery may be limited because of the development of complications during the late postoperative period. The complications do not allow the control of the achieved intraocular pressure (IOP) level. In such cases, a repeated implantation of the Ahmed valve may be one of the options for the surgical treatment. Clinical case description: Second Ahmed valves were implanted to two patients with refractory repeatedly operated glaucoma at the Department of surgical glaucoma treatment of the S. Fyodorov Eye Microsurgery Federal State Institution in Moscow. Previously, both patients had the Ahmed valves installed at their places of residence. During the postoperative period, one patient was found to have an encapsulated cyst around the drainage body, and later the revision of the operation area was performed. As a result of the repeated Ahmed valve implantation, the IOP compensation was achieved 9 months after the surgery. Conclusion: A repeated implantation of the Ahmed valve may be an option for the surgical treatment, if the first implant failed to control the IOP, in order to compensate the IOP and preserve the visual functions in patients with refractory repeatedly operated glaucoma.

The central part of the Kuril Archipelago (the Russian Far East) remains the least studied due to its poor accessibility. Such insufficient floristic data lead to uncertainties in the floristic subdivision of the Kuril Islands. We aimed to explore the vascular flora of the Simushir island, the largest one among the Middle Kurils. We revealed 24 new species for the flora of Simushir or Middle Kurils in general, including three species (Callitriche anceps, Carex meyeriana, and Ribes nigrum) that are new for the whole Kuril Archipelago. One-third of species new for Simushir are alien. Almost all the rest of the new species are associated with special types of habitats and can be found only after a detailed study of the island. We reported six new aquatic species for the Middle Kurils, refuting the earlier idea of extreme poverty of the Middle Kurils’ aquatic flora. We found no grounds for drawing a floristic border within the Northern and Middle Kurils, as the floristic similarity between Onekotan (oceanic Northern Kurils), continental Northern Kurils, and Simushir has the same magnitude.

The magnetic environment may influence the functioning of the cardiovascular system. It was reported that low-frequency and static magnetic fields affect hemodynamics, heart rate, and heart rate variability in animals and humans. Moreover, recent data suggest that magnetic fields affect the circadian rhythms of physiological processes. The influence of the magnetic environment on heart functionating during early development has been studied insufficiently. We utilized transparent zebrafish embryos to evaluate the effect of the hypomagnetic field on the characteristics of cardiac function using a noninvasive optical approach based on photoplethysmographic microscopic imaging. The embryos were exposed to the geomagnetic and hypomagnetic fields from the second to the 116th hour post fertilization under a 16 h light/8 h dark cycle or constant illumination. The exposure of embryos to the hypomagnetic field in both lighting modes led to increased embryo mortality, the appearance of abnormal phenotypes, and a significant increase in the embryo’s heartbeat rate. The difference between maximal and minimal heartbeat intervals, maximal to minimal heartbeat intervals ratio, and the coefficient of variation of heartbeat rate were increased in the embryos exposed to the hypomagnetic field under constant illumination from 96 to 116 h post fertilization. The dynamics of heartbeat rate changes followed a circadian pattern in all studied groups except zebrafish exposed to the hypomagnetic field under constant illumination. The results demonstrate the importance of natural magnetic background for the early development of zebrafish. The possible mechanisms of observed effects are discussed.
