Background: Ongoing legal and social discrimination, and stigmatization of people with lived experience of drug use (PWLE) continues to contribute to overdose-related deaths in Canada. The involvement of PWLE working in harm reduction services has proven effective in decreasing drug-related harms among PWLE; however, there exist unintended negative impacts. PWLE working in harm reduction services risk overextending themselves beyond employment parameters (e.g., emotional labor) with few systems in place (e.g., employment advocacy) for support. While meaningful participation of PWLE in harm reduction programs is critical to addressing the overdose crisis, their labor in Canada's overdose response commands further investigation and recognition. This paper examines some of the benefits and negative aspects of working in harm reduction among PWLE. Methods: Fifty qualitative surveys were completed by PWLE working in harm reduction services from across Canada at the National 2018 Stimulus conference held in Edmonton, Alberta. The surveys focused on the benefits and negatives of 'peer' employment and recommendations for organizational transformation through short answer written sections. Surveys were analyzed thematically using NVivo, informed by critical perspectives on substance use, with attention to key re-occurring themes on employment equity. Results: While participants described multiple benefits of working in harm reduction services, such as the valuing of their expertise by fellow 'peers,' growing skill sets, countering stigma, and preventing overdose deaths, issues of workplace equity were significantly identified. Stigma, tokenism, workplace discrimination, including power and pay inequities, as well as lack of worker compensation and benefits were identified as key factors persisting in the everyday experiences of participants. Conclusion: Continued exposure to stigma, workplace discrimination, and/or power imbalances, combined with the impact of high stakes employment (e.g., dealing with overdose deaths), can have significant consequences for PWLE working in harm reduction, including burn out. Policy recommendations include large-scale structural changes that address inequities of hierarchical 'peer' employment for PWLE, including increased leadership roles for diverse PWLE, pay equity and benefits, unionization, as well as more supportive working environments attentive to the intersecting social-structural factors (poverty, criminalization, racism, gendered violence) impacting the everyday lives of PWLE working in harm reduction. The Eastside Illicit Drinkers Group for Education has been supportive of research dedicated to establishing the effectiveness of Managed Alcohol Programs since its inception. Dedicated funding for MAPs in Vancouver, British Columbia is contingent on the research demonstrating that alcohol-related harms are reduced while participating in a Managed Alcohol Program. The Eastside Illicit Drinkers Group for Education understands the need for evidence-based research but we are critical of how much research is needed before sustainable funding can be established for these innovative programs. The article discusses the case study of a 54-year-old male patient who presented with raised eosinophils and full blood count. He was diagnosed with myeloproliferative neoplasms and hematological response; repeated cytogenetics and poor quality metaphases was observed; and morphological assessment and cytogenetic remission.

Food security is a challenge facing many African-American low-income communities nationally. Community and university partners have established urban agriculture programs to improve access to high quality affordable fruits and vegetables by growing, distributing, and selling food in urban neighborhoods. While the challenge of food security is within communities of color, few studies have described these urban agriculture programs and documented their impact on the crew members who work in the programs and live in the low-income communities. More information is needed on the program impact for crew and community health promotion. Using a survey and focus group discussion from the crew and staff we describe the program and activities of four Chicago Urban Agriculture programs. We summarized the impact these programs have on crew members’ perception of urban agriculture, health habits, community engagement, and community health promotion in low-income African-American neighborhoods. This case study describes the Cook County Green Corps program, a green job training program serving African American young adults from a low-income neighborhood. The program was implemented by an interdisciplinary organizing team to build knowledge, skills, and participation in sustainable jobs and urban agriculture among young adults. The trainees' experience was documented by a program evaluation survey, environmental knowledge survey, and 1 year reflection interview. We summarize the experiential design, implementation and evaluation of the program. We discuss the limitations and the benefits of the program for trainees and the neighborhood. We share recommendations for future green job training programs that can best serve urban neighborhoods. [ABSTRACT FROM AUTHOR]

Objective: To provide a more comprehensive view than previously available of US physician preparedness for public health emergencies, this study examined physicians' assessments of their preparedness, training, participation in institutional activities, information practices, and experiences with patient education. Four kinds of public health emergencies were considered: natural disasters, major airborne infections, major foodborne illness outbreaks, and chemical, biological, radiological, nuclear, or explosives (CBRNE) incidents., Methods: Between October 19, 2011, and January 11, 2012, researchers conducted a national poll among 1603 practicing physicians in a range of specialties in hospital and nonhospital settings., Results: More than one-half of physicians felt prepared to handle a natural disaster, a major outbreak of an airborne infection, or a major foodborne illness outbreak, whereas one-third (34%) felt prepared to handle a CBRNE incident. About one-half of physicians (55%) had participated in training or a conference related to emergencies in the past 2 years. Sizable fractions of physicians were unaware of emergency response tools in their care setting. For example, nearly one-half in hospitals (44%) did not know whether their care setting had an emergency response plan, and less than one-quarter had participated in a drill using such a plan in the past 2 years. Less than one-third (31%) of physicians had signed up to receive alerts in the case of future emergencies. One in 10 reported sharing emergency information with patients at least "sometimes.", Conclusions: Significant gaps remain in physician preparedness for public health emergencies, as well as in related training and participation in institutional activities. New efforts, with a focus on possible collaborations between public health institutions and health system leaders combined with effective use of online resources, are needed to bring more physicians on board and to develop relevant and useful key tools. New approaches, including those that rely on different types of care providers, may be needed to enhance patient education regarding emergency preparedness.
