Abstract

Background. The prevalence of depression in rural Ugandan communities is high and yet detection and treatment of depression in the primary care setting is suboptimal. Short valid depression screening measures may improve detection of depression. We describe the validation of the Luganda translated nine- and two-item Patient Health Questionnaires (PHQ-9 and PHQ-2) as screening tools for depression in two rural primary care facilities in Eastern Uganda. Methods. A total of 1407 adult respondents were screened consecutively using the nine-item Luganda PHQ. Of these 212 were randomly selected to respond to the Mini International Neuropsychiatric Interview diagnostic questionnaire. Descriptive statistics for respondents' demographic characteristics and PHQ scores were generated. The sensitivity, specificity and positive predictive values (PPVs), and area under the ROC curve were determined for both the PHQ-9 and PHQ-2. Results. The optimum trade-off between sensitivity and PPV was at a cut-off of ≥5. The weighted area under the receiver Operating Characteristic curve was 0.74 (95% CI 0.60-0.89) and 0.68 (95% CI 0.54-0.82) for PHQ-9 and PHQ-2, respectively. Conclusion. The Luganda translation of the PHQ-9 was found to be modestly useful in detecting depression. The PHQ-9 performed only slightly better than the PHQ-2 in this rural Ugandan Primary care setting. Future research could improve on diagnostic accuracy by considering the idioms of distress among Luganda speakers, and revising the PHQ-9 accordingly. Abstract

With 47 samples of commercially available oil paints, the applicability of Optical coherence tomography to noninvasive tomography of paint layers was examined. Two different instruments, utilizing near-infrared light with central wavelength of 823nm and, for the first time, 1.55 μm, were used to obtain cross-sectional images. Example tomograms are given; a ray tracing correction of images is also discussed. The tests revealed that applicability of OCT is limited to certain pigments and the longer wavelength is better suited for this application. [ABSTRACT FROM AUTHOR]

Abstract

Optical Coherence Tomography (OCT) is an emerging technology for high-resolution non-contact imaging of semi-transparent structures. Originally developed for medical diagnostics, we extend the OCT-technique to problems posed in material testing and characterisation. Layer thickness and refractive indices as well as internal structures of polymer parts have been determined within this study. An extension of OCT, namely polarisation-sensitive OCT (PS-OCT) has been used to identify regions of increased anisotropy within injection-moulded plastic parts and to determine quantitatively internal strain within translucent materials in a non-destructive way. Abstract

Background: Periodontal surgery stimulates osteoclast activity, leading to varying amounts of alveolar crest loss. We have established that topical application of 20 mg/ml of alendronate placed at the surgical mucoperiosteal site produced a striking reduction of alveolar bone loss in the rat model. The aim of this investigation was to examine the antiresorptive efficacy of 3 novel bisacylphosphonates topically delivered at the surgical site, in comparison to alendronate and etidronate which are in clinical use. Methods: Mucoperiosteal flap (MF) surgery was performed on the buccal and lingual aspects next to molars on both sides of the rat mandible. A gelatin sponge soaked in the bisphosphonate solution prepared by dissolving 20 mg of the bisphosphonate (alendronate, etidronate, VS-5, VS-6, ISA-13, SuBP) in 1 ml of saline was applied to exposed bone on the right side of the mandible (experimental, MF + BPs ) and the left side was treated with saline only (control, MF + S). Sections were evaluated for bone loss using microradiography pattern and amount. Results: The 3 novel bisacylphosphonates, VS-5 VS-6, and ISA-13 were more effective than etidronate, and less effective than alendronate. The most effective among this group was ISA-13 followed by VS-5 and VS-6. Conclusion: We conclude that ISA-13-like alendronate is effective in reducing alveolar bone loss when delivered at surgical sites. Since ISA-13 is well absorbed through mucose tissues, we suggest that ISA-13 efficacy on reducing bone loss should be tested by its application on the mucosal tissue. [ABSTRACT FROM AUTHOR]

A massive parasitic infestation with a general hypersensitivity reaction was seen, the possible significance of which as to the current deaths of harbour seals is discussed. 28. Distemper-like disease in Harbor Seals: Virus Isolation, further Pathologic and Serologic Findings*.

Abstract

Pathologic-histologic and immunocytologic as well as serologic findings from a total number of 50 recently dead or euthanatized moribund harbor seals ( Phoca vitulina) are described. In 14 of 31 histologically examined brains of affected seals alterations corresponding to canine distemper encephalitis (CDE) were found. Immunofluorescence-serologic tests using polyclonal as well as monoclonal antibodies against canine distemper virus (CDV) resulted in positive findings in organ sections of 24 dead seals. An agent was isolated from organs of dead seals and subjected to several passages in vero cell cultures as well as in primary seal lung-cell cultures. The agent was able to be identified as canine distemper-like virus with both antibodies. CDV neutralising serum-antibodies were detected in 13 out of 19 serum specimens. Further interdisciplinary investigations are required to determine whether the virus induced alterations - still to be interpretated only in a purely qualitative sense - should be considered as the manifestation of contributing ecological factors, or as the primary cause of the still present deaths of harbor seals. Zusammenfassung Staupeähnliche Erkrankung beim Seehund: Virusisolierung, weitere pathologische und serologische Untersuchungsergebnisse Es werden pathologisch-histologische, immunozytologische sowie serologische Untersuchungsergebnisse von insgesamt 50 frischtoten oder moribund euthanasierten Seehunden ( Phoca vitulina) beschrieben. In 14 der 31 histologisch untersuchten Gehirne finden sich der caninen Staupeenzephalitis (CDE) entsprechende morphologische Veränderungen. Unter Verwendung eines polyklonalen (CDV Gamakon) sowie eines monoklonalen (D 110) Antikörpers gegen CDV wird bei immunozytologischen Untersuchungen an Organschnitten in 24 Fällen CDV-Antigen nachgewiesen. Sowohl in Vero-Zellen als auch in primären Seehundslungenzellkulturen gelingt die Isolierung eines Agens, das mit beiden Antikörpern als CDV-ähnlich identifiziert werden kann. In 13 von 19 Fällen werden im Mikroneutralisationstest CDV neutralisierende Antikörper mit einem Titer ≥ 1:10 nachgewiesen. 30. Disposition of Alendronate Following Local Delivery in a Rat Jaw.

Abstract

Background: Recently, we have shown that local delivery of alendronate reduced significantly bone resorption activated by surgical separation of periosteum from bone. These results advocate the use of local application of alendronate in bone surgeries to prevent regional bone resorption at the surgery site. Here we investigated the efficacy of absorbtion of alendronate by the bone from a gelatin sponge soaked with radiolabeled alendronate applied topically at the surgical site. Methods: Following elevation of the mucoperiosteal flap next to premolars and molars of the rat mandible, a gelatin sponge soaked with 10 µl of radiolabled alendronate (1µCi/mg) was applied to exposed bone on one side. The local absorbtion of alendronate and its disposition in the contralateral side of the mandible as well as in the tibia bone were analyzed. Results: The results show that 10% of total alendronate content of the gelatin sponge was absorbed in the bone locally (in the surgical site), while 0.2% was disposed in the tibia. Of interest is the fact that the surgical wound in the contralateral side increased the disposition of alendronate up to 2%. This finding is most likely the result of extravasation and diffusion of alendronate due to surgical wounding. Conclusion: This study strongly supports our notion that local delivery of alendronate and its affinity to bone may become a very important treatment modality to prevent resorption of bone during dental and orthopedic procedures. Abstract

Background: Y-box binding protein-1 (YB-1) is the prototypic member of the cold shock protein family that fulfills numerous cellular functions. In the nucleus YB-1 protein orchestrates transcription of proliferation-related genes, whereas in the cytoplasm it associates with mRNA and directs translation. In human tumor entities, such as breast, lung and prostate cancer, cellular YB-1 expression indicates poor clinical outcome, suggesting that YB-1 is an attractive marker to predict patients' prognosis and, potentially, is suitable to individualize treatment protocols. Given these predictive qualities of YB-1 detection we sought to establish a highly specific monoclonal antibody (Mab) for diagnostic testing and its characterization towards outcome prediction (relapse-free and overall survival).Methods: Hybridoma cell generation was carried out with recombinant YB-1 protein as immunogen and Mab characterization was performed using immunoblotting and ELISA with recombinant and tagged YB-1 proteins, as well as immunohistochemistry of healthy and breast cancer specimens. Breast tumor tissue array staining results were analyzed for correlations with receptor expression and outcome parameters.Results: YB-1-specific Mab F-E2G5 associates with conformational binding epitopes mapping to two domains within the N-terminal half of the protein and detects nuclear YB-1 protein by immunohistochemistry in paraffin-embedded breast cancer tissues. Prognostic evaluation of Mab F-E2G5 was performed by immunohistochemistry of a human breast cancer tissue microarray comprising 179 invasive breast cancers, 8 ductal carcinoma in situ and 37 normal breast tissue samples. Nuclear YB-1 detection in human breast cancer cells was associated with poor overall survival (p = 0.0046). We observed a close correlation between nuclear YB-1 detection and absence of progesterone receptor expression (p = 0.002), indicating that nuclear YB-1 detection marks a specific subgroup of breast cancer. Likely due to limitation of sample size Cox regression models failed to demonstrate significance for nuclear YB-1 detection as independent prognostic marker.Conclusion: Monoclonal YB-1 antibody F-E2G5 should be of great value for prospective studies to validate YB-1 as a novel biomarker suitable to optimize breast cancer treatment.
