Abstract

Hygiene, together with epidemiology, represent the integral, biomedical fundaments of public health. The threat of epidemics depopulating both rural population and expanding urban centres, compelled medicine to develop these two new disciplines in the 19th century. Hygiene is the science of health preservation. Originally, it dealt with all factors affecting the physical and mental health and well-being of the population; it was rooted in the medical knowledge of disease incidence and disease prevention. The firm link between hygienic theories and practice with that of health status promote the prevention and control over infectious diseases. Initially, epidemiological focus was on communicable diseases, later it expanded to non-communicable ones. This text will support the preparation for a state exam at a pre-graduate level, providing thus a starting point for acquiring the desirable knowledge. Second revised edition

This publication analyses current literary knowledge on selected topics in the fields of oral health and pathology, with a particular emphasis on the potential roles of the oral microbiome and preventative approaches to oral afflictions. An important association with floral dysbiosis has been documented in important oral conditions, sometimes as a predisposing factor and at other times as a result thereof. However, much remains to be elucidated regarding the specific mechanisms at play, as well as their meaning in clinical practice. Continued research into the pathophysiology of certain oral diseases is of particular importance considering how widespread they are. A specific emphasis should be placed on understanding the exact mechanisms through which the microbiota facilitates health, and when disturbed, sickness. Abstract

The North Atlantic Treaty Organization (NATO) is the premier and only security alliance uniting 30 countries and growing with many partner states in the provision of collective security and against threats posed by conflict and natural disasters. Security of countries and communities is increasingly threatened by a broad spectrum of unconventional types of war and disease threats - from hybrid and asymmetric to multi-domain and peer-to-peer/near-peer conflict. The NATO Centre of Excellence for Military Medicine (MILMED COE) is the centre of gravity for medical best practices and promotion of medical doctrine across the NATO alliance. Disaster medicine is multidisciplinary and in NATO, multinational, requiring best practices that are driven by data and evidence to prevent death on the battlefield and prepare for future conflicts. "Vigorous Warrior" is a live military and disaster medicine exercise series using both civilian and military actors across all sectors of health focused on health security and identifying lessons learned to ready the alliance for future threats. In this brief report, we make the case that the Vigorous Warrior exercise exposes gaps, highlights challenges and generates an evidence base to make NATO military medicine systems more robust, more efficient and in provision of best medical practices. We specifically argue that clinical data capture must be duplicated and continuous across the alliance to ensure evidence-based medicine stays current in NATO military medical doctrine. Abstract

Objective: Acrylamide is a toxic compound found in occupational and non-occupational environment. It originates from industrial use, technological processes in construction as well as food production, water purification, and laboratory use. Adverse effects including neurotoxicity, probably carcinogenicity, genotoxicity, teratogenicity, and reproductive toxicity were reported in many studies. Our study is focused on acrylamide content in selected food products in the Czech Republic.Method: In this study, we determined acrylamide content in selected samples of commonly consumed food products (potato chips, biscuits, popcorn, corn flakes, breakfast cereals, and baked muesli).Results: The concentration of acrylamide in most samples of potato chips, biscuits and popcorn exceeded the benchmark limits. The acrylamide content in samples of potato chips from one manufacturer in 4 cases out of 5 analysed samples exceeded benchmark limit. On the other hand, no corn flakes sample acrylamide content from the same manufacture exceeded the benchmark limit. With 20 of all analysed samples exceeding the benchmark levels for acrylamide, it can be concluded that 47.6% of samples did not comply with the benchmark level.Conclusion: Dietary intake of frequently consumed food products with the acrylamide content exceeding the recommended comparative value could have adverse effects on the human health. Our study confirmed high amount of acrylamide in some selected food samples. Abstract

Traditional approaches and study design in cancer epidemiology have not been very successful in identifying and evaluating adequately the potential risk and/or protective factors associated with the disease. The main reasons for the failure are often due the small study sample size, and inadequate exposure information. In this paper, issues and approaches relevant to these two challenges are discussed. Multicentre study is proposed as a way to increase study size and to mitigate criticism about meta-analysis of independent studies. A multicentre study of large cohort or case-control studies also offer an exciting opportunity to study the contribution of epigenetic events that may be associated with lifestyle and environmental risk factors for human health. Optimizing methods for exposure assessment and how to reduce exposure to misclassification represent a difficult component in epidemiological studies. A potentially useful approach for improving exposure estimation is to rely on biomarkers of exposures. An example is provided to demonstrate how biomarkers of exposures could provide valuable information in addition to exposure measurements in traditional epidemiological studies. Finally, it is argued that risk assessment and the precautionary principle should not be viewed as conflicting paradigms but, rather, as a complementary approach for developing appropriate policies to address risks posed by exposure to carcinogens and a wide spectrum of other health hazards. Neurotoxicity was reported in workers exposed to acrylamide in the occupational environment. Abstract

Beryllium has an impact on the human health of professionally or non-occupationally exposed people. Current evidence suggests that beryllium acts as a hapten with limited antigenic properties and is presented by antigen presenting cells to CD4+ T cells, which possess specific antigen receptors. The immunological changes in humoral immunoreactivity were considered biomarkers of beryllium exposure. In the present, due to the development of immunologic knowledge, tests of cellular immunity have promising potential for further research in this field. The historical view of the immune response to beryllium in acute and/or chronic beryllium disease is an example of the development of the interaction between mechanisms of innate and adaptive (specific), humoral and cellular immunity. Abstract

Human biomonitoring (HBM) has a wide range of applications and long tradition both in health care and the field of public health. Its major advantage is the integration of all exposure routes and sources. Since HBM information is an integrated exposure finding it offers the opportunity to trace and mimic a realistic exposure scenario. It reduces the number of assumptions that need to be made when estimating exposure and thus helps to reduce the uncertainties in exposure science. In spite of some challenges, such as further harmonization in the area of HBM, necessity of deriving equivalents of markers of external exposure and addressing the ethical and political aspects of its application, HBM is an efficient and cost-effective way to measure exposure levels of the human body to xenobiotics. 41. Retrospective assessment of specific effects of exposure of workers to PCBs in Slovakia.

Abstract

Introduction and objective. Polychlorinated biphenyls (PCBs) have been used commercially since 1929 as dielectric and insulating non-flammable substances, additives for paints, etc. In Slovakia, 60 chemical production workers highly exposed to PCBs (mainly to Delor 103) were studied with duration of exposure ranging from 3 months to 19 years. Materials and method. Clinical examinations of skin, skin histology and laboratory tests concerning lipid metabolism, iron metabolism and levels of copper provided comparisons with a control group of healthy individuals and/or the upper limits of normal values. Results. Skin changes were found in 47 % of individuals. In most cases, milia-like efflorescences (57.14 %) occurred, as well as comedones (55.35 %); other symptoms occurred in a smaller number of workers. Hyperkeratosis, acanthosis of the epidermis (particularly in hair follicles), and a cellular infiltration of the corium were all found through histology. The intensity of cutaneous affections was associated with the changes in laboratory tests. Elevated triglycerides, cholesterol, and phospholipids were found in exposed workers. After a further two years, a significant increase in triglycerides was found in exposed individuals when compared with the control group. In addition, a significant increase in serum levels of copper, and total and unsaturated iron-binding capacity was detected. Conclusions. Anamnesis showed that some people directly exposed to PCBs may develop skin changes after three months of occupational exposure. The results represent a unique snapshot of worker exposure at a given location, representing the basis for comparison with the population who grew up in the area and still live there today. 42. Fish and fish products as risk factors of mercury exposure.

Abstract

Introduction and objective. Mercury is ubiquitous in the biosphere, occurring in the air, water, land, and soil, as well as in living organisms. Excessive exposure to mercury is associated with a wide range of adverse health effects including damage to the central nervous system and the kidneys. Mercury exists in many different forms in the environment which produce various patterns of toxicity. Protection of the food chain from contamination by mercury is an important task in the protection of health of the human population. Objective. The aim of the study was to monitor the concentrations of mercury in fish and fish products from food retail in Eastern Slovakia, and from the Ružín water reservoir, Košice district. Materials and method. A total of 384 samples of fish and fish products were collected for the study. Atomic absorption spectrometry standard solutions for mercury were used at a wavelength of 254 nm. Results. The majority of countries and global organizations now enforce a maximum concentration of mercury in fish of approximately 0.5 mg.kg-1. All of the 184 samples (50.52 % of the total fish samples studied) were above the maximum level set by the European Commission Regulation for mercury in fish. Conclusions. The systematic analytical control of contaminants in food is important. Mercury is concentrated in seafood, products of prey and marine fish, fish from rivers and lakes in the areas contaminated by mercury. According to the findings of this study with analyzer AMA 254, the consumption is not recommended of fish, especially seafood (meat of shark, swordfish and king mackerel), for selected groups of the population: children, women of childbearing age, pregnant women and nursing mothers. Abstract

The objective of this paper is an analysis of two main attributes of healthcare systems. First of the main attributes is the trend of ever growing expenditures of healthcare systems all across the world. Second attribute is the efficiency of chosen mixed healthcare systems, where mixed system is one which features involvement of both private and public sector. Countries chosen for analysis are USA as the country with high private sector influence on healthcare, France with its mediocre influence and Japan, where the private companies participate in health care but are very strictly regulated by a zero profit rule, and the Czech Republic, where public sector dominates the health care. The result is that the systems with higher influence of the private sector tend to have lesser occupancy, not significantly better performance and higher expenditures. This raise doubts whether the private sector brings anything of value for the patients within the healthcare system. Abstract

Previous genome-wide association studies (GWAS) have identified six risk loci for renal cell carcinoma (RCC). We conducted a meta-analysis of two new scans of 5,198 cases and 7,331 controls together with four existing scans, totalling 10,784 cases and 20,406 controls of European ancestry. Twenty-four loci were tested in an additional 3,182 cases and 6,301 controls. We confirm the six known RCC risk loci and identify seven new loci at 1p32.3 (rs4381241, P=3.1 × 10−10), 3p22.1 (rs67311347, P=2.5 × 10−8), 3q26.2 (rs10936602, P=8.8 × 10−9), 8p21.3 (rs2241261, P=5.8 × 10−9), 10q24.33-q25.1 (rs11813268, P=3.9 × 10−8), 11q22.3 (rs74911261, P=2.1 × 10−10) and 14q24.2 (rs4903064, P=2.2 × 10−24). Expression quantitative trait analyses suggest plausible candidate genes at these regions that may contribute to RCC susceptibility.

46. Hearing Changes in Children Exposed to Arsenic in Neurotoxicity Context.

Abstract

Arsenic is element ubiquitously distributed in nature. Human exposure to this element is mediated by drinking water (e.g. in Bangladesh,Taiwan, Argentine, Chile, United States, Hungary), via local food grown in highly contaminated regions e.g. due to burning arsenic rich coal or in the vicinity of smelters, especially around copper and lead smelters and after a mass use of pesticides e.g. defoliant-dimethylarsinic acid- in a mass scale used by US troops in Vietnam as the Agent Blue or by see food items. In copper, lead and stibium smelters there are frequent chances for inhalation exposure which was described as well in connection with application of pesticides containing this metaloid. There were described as well iatrogenic and accidental intoxications with sever clinical consequences even with fatalities. The environmental and occupational toxicology of arsenic has been extensively reviewed (e.g. EPA, 1984; Clarkson, 2001; Bencko and Slamova, 2007; ATSDR, 2007). Abstract

Coronavirus disease COVID-19 is highly infectious disease caused by SARS-CoV-2 virus as a novel coronavirus led to pandemic. Due to fast transmission COVID-19 has become a global problem. The virus is spread by aerosol from infected people and persists in the air for a long period. SARS-CoV-2 affects the lungs, but it can also affect digestiv or cardiovascular systems, can attact the brain, damage vessels and can lead to neurological manifestation. COVID-19 has several clinical symptoms and can lead to multiorgan dysfunction and death. On the other hand, in some infected persons COVID-19 can take place asymptomatic. Due to the risk of rapid spread of COVID-19, the early prediction of the onset of the epidemic is important. The Water Based Epidemiology (WBE) is an effective tool for monitoring the number of infections and can serve as a tool to monitoring various human activity including health status of population in given area. One of the advantage of WBE in last pandemic is their capability to reveal the outbreaks at an early stage, including presymptomatic or asymptomatic transmission of SARS-CoV-2. The use of WBE is based on the principle of viral shedding in stool samples and on the detection of SARS-CoV-2 viral mRNA by PCR method in wastewater samples. On the other hand, some non-specific markers from COVID-19 infected persons can also be used in WBE. Viral infections are associated with inflammation. Neopterin, a pteridine derivative, is produced in the metabolism after some stimulus. Neopterin was found in urine and other body fluids, as blood serum, cerebral spinal fluid in COVID-19 infected persons in high levels. Through urinary excretion, neopterin was detected in wastewater samples. The other markers, as toxin-like peptides, similar or identical to toxic components of venoms from animals, such as conotoxins, phospholipases, phosphodiesterases, zinc metal proteinases, and bradykinins, were identified in blood plasma, urine and faecal samples from COVID-19 patients. These markers, including neopterin, can be potencially useful in WBE as markers of rapid prediction of the onset of the epidemic. The early detection of the presence of SARS-CoV-2 within communities can also give healthcare authorities time to prepare for potential outbreaks and time to prepare measures to protect population.
