As a Global South NGO from Bangladesh that has become one of the world's largest NGOs, BRAC's fiftieth anniversary was a momentous occasion for global development. This special issue celebrates BRAC's contributions to development knowledge and practice across five decades, highlighting BRAC's impact – and the challenges it has faced in achieving this – across several programs, ranging from humanitarian response to women's and young people's empowerment and its quest for inclusive poverty reduction in Bangladesh and beyond. We explore how BRAC's heavy investment in a culture of continuous learning enables the organisation to understand and improve its programs, ground these in local contexts and communities, and build external partnerships for spreading its impact, influence, and knowledge globally. We hope this special issue will advance understanding of and spread BRAC's strategies of working closely with poor and marginalised communities, of investing in transformative learning that starts with the self, of evolving and embracing mistakes, and of igniting hope over fate and despair. Since its inception, BRAC has combined emergency assistance with longer-term development interventions, grounding its approach in empowering local communities. Its experiences in navigating tensions across the humanitarian–development nexus and in debates around localising aid provide a useful perspective on the way in which these debates intersect, showcasing how prioritising a localised response is conducive of an approach that is "humanitarian in nature and developmental in solution". Through historical and contemporary perspectives, we explore how BRAC has adjusted its profile, adapted to new challenges, learned about new and changing settings, developed innovations, learned from mistakes, and dismantled boundaries to bridge humanitarian and developmental support in Bangladesh and beyond. These experiences highlight the importance of the "local" beyond geography, as evidenced by BRAC's engagement with the communities it serves and is part of, and its desire to move forward in an inclusive and socially just manner. Equitable futures depend upon better employment outcomes for young women. We use a skills ecosystems approach to explore how youth skills development programs can maximise their impact in highly gendered societies and labour markets. While improving employment outcomes and incomes for young women, BRAC's STAR program cannot withstand Bangladesh's deep-rooted and socially restrictive norms and practices. Short-term successes are diluted as family pressures and commitments take precedent over young women's economic lives. Adding a temporal dimension to the concept of skills ecosystems, these findings highlight that maintaining these impacts requires constant renegotiation and advocacy to challenge the structural obstacles within households and labour markets and within national policies and investments that constrain their longer-term economic empowerment. While urban geographies play a critical role in shaping youth development, existing literature has focused primarily on the ways in which young people use, navigate and shape cities. This has overlooked the depth and complexity of Africa's urban youth crisis. This paper introduces a new analytic framework, based on principles of Developmental Psychology, outlining the four critical psychosocial processes of building foundations, building blocks, building support structures and building aspirations that young people must draw upon as they navigate their futures. It then explores the ways in which the city and its economic, social and political landscapes have undermined these across the African continent. Cities shape poverty and hardship, but also transform social norms and practices in ways that constrain, undermine or erode these developmental processes. Where social support and socioeconomic status cannot protect young people in tough economic contexts, city life is detrimental to young people's psychosocial well-being, development and long-term futures. A paucity of systematic research into development NGO sectors means that we have underestimated their contributions to international development. In 2015 Britain's development NGOs spent nearly £7 billion, equivalent to over half of the UK government's Official Development Assistance (ODA) that year. Mapping the sector reveals its size and significance and gives new insight into its structural composition and how this it is influenced by changes to the funding landscape. Alongside new insights into an influential sector, this work highlights the importance of innovative research methods and techniques for a more systematic understanding of development NGOs globally. Across the Global South, the realities of urban informality are changing, with implications for how we understand this phenomenon across economic, spatial, and political domains. Recent accounts have attempted to recognise the diversity of informality across contexts and dimensions, as well as its everyday lived realities. Reviewing key debates in the sector, and drawing upon the new empirical studies in the papers presented here, we argue for a shift away from seeing urban informality narrowly as a setting, sector, or outcome. We suggest that reconsidering informality as a site of critical analysis offers a new perspective that draws on and extends political economy approaches, and helps us to understand processes of stratification and disadvantage. We seek to highlight the significance of the informal-formal continuum at the same time as challenging this dichotomy, and to explore emerging theoretical and empirical developments, including changing attitudes to informality; the increasing salience of agency; and informality as strategy both for elite and subaltern groups. Background: The purpose of this study was to gain an in‐depth understanding on the subject of relapse from the perspectives of family members of young people receiving services for a first‐episode psychosis (FEP). Methods: A qualitative descriptive approach, using focus group methods, was used to elicit experiences, understandings, and knowledge of relapse in FEP. Family members were recruited from 4 specialized early intervention programmes for psychosis in Canada. A total of 24 (6 male, 18 female) family members participated in the study. Thematic analysis was used to examine the data. Results: The core underlying theme in all focus groups was worrying about relapse, which was often accompanied by significant levels of fear and anxiety, and was influenced by: (1) impact of an episode of psychosis; (2) limited confidence in recognizing and coping with relapse; (3) unmet needs for coping skills and emotional support and (4) unmet needs regarding frequency and continuity of communication with clinicians. Conclusions: Family members' unmet needs for relapse‐focused education, support and communication with service providers and peers, can have a negative impact on relapse prevention. Addressing family members' education and support needs in a tailored manner (including preferences for types of peer support) can contribute positively to their confidence and ability to recognize and respond to relapse. This can help reduce fear and anxieties about relapse, and positively influence the ability to function as caregivers. ABSTRACT Research in Bangladesh reveals the limitations of actor-oriented frameworks for understanding urban poverty that assess household livelihoods on the basis of a household's portfolio of assets or capitals. The narrow focus of these frameworks on households and their depoliticized definition of social capital overlook the political roots of urban poverty. The informal systems of governance that dominate resource distribution within low-income settlements ensure that the social resources necessary for long-term household improvement are confined to a small elite. Only through extending our analysis beyond the household level, to explore their position within this local political economy of employment and enterprise, can we recognize the limitations placed on household efforts to improve their livelihoods. Objective: In many countries, including Canada, a small proportion of people with psychotic disorders receive long-acting injectable (LAI) antipsychotics (APs), despite their demonstrated effectiveness and possible advantages for improving adherence rates. Attitudes regarding LAIs among physicians may influence their prescribing practices and thereby contribute to the underuse of LAIs. Here, we report on a qualitative study of perceptions and attitudes toward LAIs among psychiatrists in Canada. Method: Focus groups were conducted with 24 psychiatrists in 4 Canadian provinces. The focus groups inquired about experiences with and attitudes toward LAI APs. The sessions were audiotaped and transcribed verbatim, and transcripts were coded using a hybrid process of deductive and inductive methods. A brief pre-focus group questionnaire was administered. Results: The pre-focus group questionnaires indicated that psychiatrists in our study prescribed the oral formulation of APs most of the time and had limited experience with LAIs. The focus groups yielded 4 main themes: limited knowledge about and experience with LAIs; attitudes toward LAIs (beliefs about negative perceptions of patients regarding LAIs, personal bias against needles, and consensus about some advantages of LAIs); prescribing practices around LAIs (generally seen as a last-resort option for patients with a history of nonadherence); and pragmatic barriers to using LAIs (for example, cost, storage, and staffing). Conclusion: Several factors may be contributing to the underuse of LAIs and the continuing stigmatized and coercive image of LAIs. Psychiatrists may benefit from better education about LAIs, and from self-examination of their attitudes to LAIs and their prescribing practices. [ABSTRACT FROM AUTHOR]

Objective: Despite the well-acknowledged problem of poor adherence to antipsychotic (AP) medication, long-acting injectables (LAIs) that could improve adherence are underused in Canada. Attitudes concerning LAIs among patients and psychiatrists may contribute to this underuse. Our objective was to investigate perceptions of and attitudes toward LAIs among patients in Canada. Method: Focus groups were conducted with 34 patients with a diagnosis of schizophrenia spectrum psychoses in 4 Canadian provinces. The focus groups inquired about experiences with and attitudes toward LAI APs. The sessions were audiotaped and transcribed verbatim, and transcripts were coded using a combination of deductive and inductive methods. Results: Four themes emerged: awareness of and knowledge about LAIs; perceptions about LAIs; cost and convenience considerations; and issues arising from the coercive context under which LAIs were often prescribed. Nine patients had never heard about LAIs, and some others reported not having understood what was discussed with them regarding LAIs. Patients had typically heard about LAIs in either a context of coercion or of medication nonadherence. Patients had positive and negative perceptions concerning LAIs. The positive perceptions centred on relapse prevention and reduced effort in ensuring adherence, and the negative perceptions centred on financial costs and the inconvenience of appointments to receive injections. Conclusion: To enhance LAI usage, some of the issues that need to be addressed are the inadequacy of information given to patients, the element of coercion involved in LAI introduction, the pragmatic barriers to LAI uptake by patients, and negative subjective perceptions about LAIs. As urbanization changes the face of poverty in Bangladesh, endemic insecurities within the urban environment force low-income households to deploy new strategies of labour mobilization that challenge traditional patriarchal ideologies, and in the process, gender dynamics. The research reveals the complex balance male household heads face in meeting economic and social priorities. While the majority of households depend on female labour mobilization as a short-term means of survival, this comes at the cost of displacing longer-term goals of household advancement, given that status, prestige and social networks are dependent on the ability to uphold patriarchal norms that forbid women from working. While most men hold on to patriarchal beliefs – viewing the mobilization of female labour as a “necessary evil” that dampens household honour and prestige and threatens masculinity – women are aware of the importance of their work and the centrality of their contributions to income. These opposing perspectives generate tensions within the household, leaving women to face a complex balance between managing the household, their jobs and the marital relationship. A wife’s labour is often viewed as a threat to male dominance and authority and can lead to various negative behaviours by the household head, including reducing working hours and income contributions or taking a second wife. A paradox is visible, in which men are aware of these negative tendencies but do not associate them with their own marital problems, instead blaming wives for their “disobedience”. This may be one reason for the persistence of patriarchal social norms that frown upon sending married women to work while, at the same time, it has become widely acceptable to send young, unmarried daughters, who do not offer the same challenge to authority, to work in Bangladesh’s thriving export-oriented garments sector. In Bangladesh, urban poverty is neglected in research, policy and action on poverty reduction. This paper explores the underlying reasons for this relative neglect, which include national identity and image, the political economy of urban poverty and the structuring of knowledge creation. It argues for more comprehensive policy and programmes for the urban poor given Bangladesh’s increasingly urban future and the growing magnitude of urban poverty. The impact of climate change will accelerate Bangladesh’s ongoing urbanization as well as deepen the scale and severity of urban poverty. The fact that reducing urban poverty will be increasingly important in meeting national goals for poverty reduction means that policy and action must pay more attention to the urban poor. This is contingent upon two factors: first, a better understanding of the scale and nature of urban poverty and vulnerability; and second, the confrontation of powerful interests necessary to secure a national commitment to urban poverty reduction. Objective: To review the evidence for the role of long-acting injectable (LAI) antipsychotics (APs), especially the second-generation AP (SGA) LAIs, in the treatment of schizophrenia and to discuss the use rates of LAIs in Canada.Method: A search of online medical databases was conducted of the published literature (1995-2012) of the effects of LAIs on the domains of remission, adherence, relapse, and hospitalization. Results obtained from randomized controlled trials (RCTs), systematic reviews, meta-analyses, and large-scale observational studies were included. Expert consensus data were also solicited on LAI use within a Canadian context.Results: While the efficacy of LAIs, compared with placebo, is well established, the evidence from RCTs is equivocal for any specific advantage for SGA LAIs, compared with oral medications, probably owing to challenges in conducting such RCTs. Evidence from methodologically less rigorous studies and from clinical practice suggests some advantages in achieving and maintaining remission, risk of relapse, and hospitalization. The rate of LAI (first-generation AP and SGA) use from published outpatient studies is low at 6.3% in Canada, compared with 15% to 80% worldwide. However, there is a relatively high rate of use in specific early psychosis programs and in conjunction with community treatment orders in Canada.Conclusions: LAIs are at least as effective as oral APs in the treatment of psychotic disorders. The former may have specific advantages for patients who demonstrate covert nonadherence. The underuse of LAIs in Canada needs to be better understood and addressed. [ABSTRACT FROM AUTHOR]
