Context: Previous studies demonstrated faster correction of metabolic derangement associated with hypertrophic pyloric stenosis with pre-operative intravenous (IV) histamine-2 receptor antagonists.Aims: We investigated if similar outcomes are achieved with IV pantoprazole, a proton-pump inhibitor (PPI), including the subgroup of delayed presenters in the South African setting.Settings and Design: A 5-year retrospective record review (January 2014-December 2018) compared the rate of metabolic correction in patients with hypertrophic pyloric stenosis at two tertiary centres.Subjects and Methods: One centre routinely administers IV pantoprazole (1 mg/kg daily) preoperatively (PPI group) and the other does not (non-PPI group). Fluid administration, chloride supplementation and post-operative emesis were evaluated.Statistical Analysis: Spearman's rank correlation coefficient was used to calculate statistical significance for discrete dependent variables. Continuous variables were compared between the groups using the Student t-test. Fisher's exact contingency tables were used to classify categorical data and to assess the significance of outcome between two treatment options. P < 0.05 was considered statistically significant.Results: Forty-two patients received IV pantoprazole and 24 did not. The mean time of metabolic correction was 8 h shorter in the PPI group (P = 0.067). Total pre-operative chloride administration correlated to the rate of metabolic correction in both cohorts (P < 0.0001). Profound hypochloraemia (chloride <85 mmol/l) was corrected 23 h faster in the PPI group (P < 0.004). Post-operative emesis was noted: 0.45 episodes/patient in the PPI group and 0.75 episodes/patient in the non-PPI group (P = 0.01).Conclusions: Pre-operative IV pantoprazole administration showed a faster correction of metabolic derangements, and in profound hypochloraemia, the correction occurred substantially faster in the PPI group. Background Postlicensure studies have shown an association between rotavirus vaccination and intussusception. We assessed the risk of intussusception associated with Rotarix (RV1) administration, at 6 and 14 weeks of age, in an upper-middle-income country, South Africa. Methods Active prospective surveillance for intussusception was conducted in 8 hospitals from September 2013 through December 2017. Retrospective case enrollment was done at 1 hospital from July 2012 through August 2013. Demographic characteristics, symptom onset, and rotavirus vaccine status were ascertained. Using the self-controlled case-series method, we estimated age-adjusted incidence rate ratios within 1–7, 8–21, and 1–21 days of rotavirus vaccination in children aged 28–275 days at onset of symptoms. In addition, age-matched controls were enrolled for a subset of cases (n = 169), and a secondary analysis was performed. Results Three hundred forty-six cases were included in the case-series analysis. Post–dose 1, there were zero intussusception cases within 1–7 days, and 5 cases within 8–21 days of vaccination. Post–dose 2, 15 cases occurred within 1–7 days, and 18 cases within 8–21 days of vaccination. There was no increased risk of intussusception 1–7 days after dose 1 (no cases observed) or dose 2 (relative incidence [RI], 1.71 [95% confidence interval {CI}.83–3.01]). Similarly, there was no increased risk 8–21 days after the first (RI, 4.01 [95% CI,.87–10.56]) or second dose (RI,.96 [95% CI,.52–1.60]). Results were similar for the case-control analysis. Conclusions The risk of intussusception in the 21 days after the first or second dose of RV1 was not higher than the background risk among South Africa infants. The human-specific roundworm Ascaris lumbricoides continues to cause significant morbidity and mortality across the world, especially within Asia, South America, and Africa, despite the success of mass eradication programs. Most childhood worm infestations are relatively asymptomatic. Children can, however, be burdened by ascariasis and its negative nutritional impact. Heavy worm loads are complicated by intestinal obstruction, bowel necrosis, and bowel perforation. Less frequently, biliary obstruction and its complications may also occur. Radiological detection of complications allow for early institution of the largely successful nonoperative management followed by curative antihelminthics. Imaging is essential in the monitoring and detection of ominous signs that direct emergency surgical intervention and follow-up. [ABSTRACT FROM AUTHOR]

In an exchange of memories and references to writers, theorists and visual artists, Arnold and Meskimmon discuss the consequences of migration in terms of nationality, citizenship, denizenship, and art practice. The dialogue generates personal testimony about ‘be(com)ing’ transcultural and making oneself at home to effect a sense of belonging. Encounters with place and the roles of cultural artefacts are cited as mediators in negotiating relationships with ‘home’, and the concept of denizenship is advocated as a means of thinking through kinds of citizenship that might better describe the experiences of women as they make themselves at home in a global world. The spaces and tensions between races, ethnic groups, and communities in late Apartheid and post-1994 South African society, and the co-existence of different languages, religions and cultures, generated a society so fractured that cultural translation became a formidably difficult task. The concept of translation in a transforming society is examined through an analysis of two-dimensional language as a means of translating political events and experiences into visual forms, which attempt to communicate across cultural gaps. Iconic documentary photographs by Sam Nzima (1976) and Truth and Reconciliation Commission (TRC) photographers (1997), struggle posters by co-operatives and formally trained designers, and artworks by Kevin Brand, Sue Williamson and Marion Arnold are discussed. The images reveal that different forms of visual representation encode different relationships of signifying content and aesthetic form to offer alternatives to speech and writing in communicating some implications of Apartheid politics, leaving a legacy that validates art and design as tools of political activism. This article presents an interview of John Onians, a professor at the University of East Anglia, and the editor "Atlas of World Art," a publication which fuses the history and geography of art. Onians' passion for art is very evident. So too is the fact that he is a man whose philosophy of life plays itself out in the way he looks at and interprets art objects. While not denying the great importance of culture in shaping behavior and creative endeavor, he distances himself from cultural politics. According to him, he thinks political approaches are dreadfully limited. Thinking about how to articulate his global perspective and his fascination with the essential nature of human beings, Onians developed the idea of double-page spreads which focus on a place and a time.
