The loss of different alkyl radicals from unsymmetrical tertiary amine radical cations is studied in order to systematically examine the competition between simple cleavage reactions. The reversal of the preference for the loss of small and large radicals observed when the internal energy of the reactant is increased shows that the rate of loss of the larger alkyl radical increases steeply with internal energy.

The purpose of this study was to assess the cognitive development of 27 children with nonhemorrhagic neonatal stroke (occurring within the first 28 days of life). The cognitive evaluation consisted of the Bayley Scales of Infant Development, administered at 12 and/or 24 months poststroke. Compared with the normative sample, children with neonatal stroke obtained significantly lower scores on the Bayley Psychomotor Development Index at 12 months poststroke and on the Bayley Mental and Psychomotor Development Indices at 24 months poststroke. Outcome did not differ based on stroke type or laterality of infarct. However, there was a trend toward higher scores on the Bayley Psychomotor Development Index at 24 months in the left hemisphere group compared to the right hemisphere group. Overall, children with neonatal stroke evidenced significant impairment within the first 2 years poststroke. Further research is required to confirm whether cognitive impairments in these children resolve, remain in the low-average range, or increase with development as more complex skills are learned. The need for and extent of thyroid surgery for a thyroid nodule is usually primarily based on fine-needle aspiration (FNA) and frozen section diagnosis. The relative role of these modalities in 66 patients who had undergone FNA and subsequent thyroidectomy with frozen section was investigated. Cases that demonstrated discordance between FNA and frozen section (n = 22; 33%) were identified, and all slides were re-reviewed by a cytopathologist using current established cytologic criteria. A change in diagnosis was made in 13 cases with an increase in accuracy of FNA from 71% to 88%. The accuracy of frozen section (92%) did not change appreciably with re-review (94%). The accuracy of FNA and frozen section combined was found to be 98%. The rate of discordance decreased from 33% to 13% with re-review. When strict histologic and cytologic criteria are applied, FNA and frozen section are accurate and complementary methods that help in determining the operative management of thyroid masses. However, when FNA yields a diagnosis of 'follicular neoplasm,' frozen section is unlikely to change this diagnosis. [ABSTRACT FROM AUTHOR]

Little is known about the cognitive sequelae of pediatric-onset multiple sclerosis (MS). Ten pediatric patients with MS were evaluated using a comprehensive neuropsychological battery. Neuropsychological deficits were identified on measures of general cognition, language, visuomotor integration, and verbal and visual memory. Cognitive impairment occurs in children with MS, and those with longer disease duration and younger age at MS onset appear to be at greatest risk.
