2. Prognostic value of adenosine stress cardiovascular magnetic resonance in patients with low-risk chest pain

Abstract Background Approximately 5% of patients with an acute coronary syndrome are discharged from the emergency room with an erroneous diagnosis of non-cardiac chest pain. Highly accurate non-invasive stress imaging is valuable for assessment of low-risk chest pain patients to prevent these errors. Adenosine stress cardiovascular magnetic resonance (AS-CMR) is an imaging modality with increasing application. The goal of this study was to evaluate the negative prognostic value of AS-CMR among low-risk acute chest pain patients. Methods We studied 103 patients, mean 56.7 ± 12.3 years of age, with chest pain and no electrocardiographic evidence of ischemia and negative cardiac biomarkers of necrosis, who were admitted to the Cardiac Decision Unit of our institution. All patients underwent AS-CMR. A negative AS-CMR was defined as absence of all the following: regional wall motion abnormalities at rest; perfusion defects during stress (adenosine) and rest; and myocardial scar on late gadolinium enhancement images. The patients were followed for a mean of 277 (range 161-462) days. The primary end point was defined as the combination of cardiac death, nonfatal acute myocardial infarction, re-hospitalization for chest pain, obstructive coronary artery disease (>50% coronary stenosis on invasive angiography) and coronary revascularization. Results In 14 patients (13.6%), AS-CMR was positive. The remaining 89 patients (86.4%), who had negative AS-CMR, were discharged. No patient with negative AS-CMR reached the primary end-point during follow-up. The negative predictive value of AS-CMR was 100%. Conclusion AS-CMR holds promise as a useful tool to rule out significant coronary artery disease in patients with low-risk chest pain. Patients with negative AS-CMR have an excellent short and mid-term prognosis.

Abstract Background In the past two decades, genetic testing for cancer risk assessment has entered mainstream clinical practice due to the availability of low-cost panels of multiple cancer-associated genes. However, the clinical value of multiple-gene panels for cancer susceptibility is not well established, especially in cases where panel testing identifies more than one pathogenic variant. The risk for specific malignancies as a result of a mutated gene is complex and likely influenced by superimposed modifier variants and/or environmental effects. Recent data suggests that the combination of multiple pathogenic variants may be fewer than reported by chance, suggesting that some mutation combinations may be detrimental. Management of patients with “incidentally” discovered mutations can be particularly challenging, especially when established guidelines call for radical procedures (e.g. total gastrectomy in CDH1) in patients and families without a classic clinical history concerning for that cancer predisposition syndrome. Case presentation We present two cases, one of an individual and one of a family, with multiple pathogenic mutations detected by multi-gene panel testing to highlight challenges practitioners face in counseling patients about pathogenic variants and determining preventive and therapeutic interventions. Conclusions Ongoing investigation is needed to improve our understanding of inherited susceptibility to disease in general and cancer predisposition syndromes, as this information has the potential to lead to the development of more precise and patient-specific counseling and surveillance strategies. The real-world adoption of new or improved technologies into clinical practice frequently requires medical decision-making in the absence of established understanding of gene-gene interactions. In the meantime, practitioners must be prepared to apply a rationale based on currently available knowledge to clinical decision-making. Current practice is evolving to rely heavily on clinical concordance with personal and family history in making specific therapeutic decisions.

International audience; Exploring Knowledge Graphs (KGs, in short) to discover facts and links is tedious even for experts with knowledge of SPARQL due to their unfamiliarity with the structure and labels of entities, classes and relations. Some KG applications require finding the connections between groups of nodes, even if users ignore the shape of these connections. However, SPARQL only allows checking if paths exist, not returning them. A recent property graph query language, GPML, allows also returning connecting paths, but not connections between three or more nodes. We propose to demonstrate RelSearch, a system supporting extended SPARQL queries, featuring standard Basic Graph Patterns (BGPs) as well as novel Connecting Tree Patterns (CTPs); each CTP requests the connections (paths, or trees) between nodes bound to variables. RelSearch evaluates such extended queries using novel algorithms which, unlike prior keyword search methods, return connections regardless of the edge directions and are independent of how we measure the quality (score) of each connection. We will demonstrate RelSearch's expressivity and efficiency using a variety of RDF graphs, user-selected score functions, and search exploration orders.

National audience; Graph data management and querying has many practical applications. When graphs are very heterogeneous and/or users are unfamiliar with their structure, they may need to find how two or more groups of nodes are connected in a graph, even when users are not able to describe the connections. This is only partially supported by existing query languages, which allow searching for paths, but not for trees connecting three or more node groups. The latter is related to the NP-hard Group Steiner Tree problem, and has been previously considered for keyword search in databases. In this work, we formally show how to integrate connecting tree patterns (CTPs, in short) within a graph query language such as SPARQL or Cypher, leading to an Extended Query Language (or EQL, in short). We then study a set of algorithms for evaluating CTPs; we generalize prior keyword search work, most importantly by (i) considering bidirectional edge traversal and (ii) allowing users to select any score function for ranking CTP results. To cope with very large search spaces, we propose an efficient pruning technique and formally establish a large set of cases where our algorithm, MOLESP, is complete even with pruning. Our experiments validate the performance of our CTP and EQL evaluation algorithms on a large set of synthetic and real-world workloads.

Although there are limited data regarding the formation of coronary artery aneurysms (CAAs) after drug-eluting stent (DES) implantation, CAAs appear to be a rare complication of coronary stenting. The exact mechanism of CAA formation is unknown, but several hypotheses have been proposed. As the use of DES increases, the clinical significance of these findings will become clearer. We report on a patient who developed multiple CAAs in 2 different locations after sirolimus-eluting stent implantation.

International audience; Investigative Journalism (IJ, in short) requires combining highly heterogeneous digital datasets coming from a wide variety of sources. We have developed ConnectionLens, a system that integrates such sources into a single heterogeneous graph and enables users to query the graph using keywords. The first iteration of the system [7] followed a mediator architecture which severely constrained its query scalability. Thus, we fully re-engineered the system, moving it to a warehouse architecture, and replacing its core components (information extraction, data querying, and interactive interfaces), which allowed us to handle uses cases orders of magnitude larger than the previous platform. In a consortium of computer scientists and investigative journalists, we propose to demonstrate ConnectionLens' capability to integrate arbitrary heterogeneous datasets and query them flexibly by means of keywords. Among several scenarios, our main focus will be on a real-world journalistic use case about situations which may lead to Conflicts of Interest between biomedical experts and various organizations, such as corporations, lobbies, etc. The demonstration will showcase the end-to-end data analysis pipeline, illustrate each system component, and the different parameters governing graph creation and querying.

Digital data is a gold mine for modern journalism. However, datasets which interest journalists are extremely heterogeneous, ranging from highly structured (relational databases), semi-structured (JSON, XML, HTML), graphs (e.g., RDF), and text. Journalists (and other classes of users lacking advanced IT expertise, such as most non-governmental-organizations, or small public administrations) need to be able to make sense of such heterogeneous corpora, even if they lack the ability to define and deploy custom extract-transform-load workflows, especially for dynamically varying sets of data sources. We describe a complete approach for integrating dynamic sets of heterogeneous datasets along the lines described above: the challenges we faced to make such graphs useful, allow their integration to scale, and the solutions we proposed for these problems. International audience; Investigative Journalism (IJ, in short) is staple of modern, democratic societies. IJ often necessitates working with large, dynamic sets of heterogeneous, schema-less data sources, which can be structured, semi-structured, or textual, limiting the applicability of classical data integration approaches. In prior work, we have developed ConnectionLens, a system capable of integrating such sources into a single heterogeneous graph, leveraging Information Extraction (IE) techniques; users can then query the graph by means of keywords, and explore query results and their neighborhood using an interactive GUI. Our keyword search problem is complicated by the graph heterogeneity, and by the lack of a result score function that would enable pruning of the search space. In this work, we describe an actual IJ application studying conflicts of interest in the biomedical domain, and we show how ConnectionLens supports it. Then, we present novel techniques addressing the scalability challenges raised by this application: one allows us to reduce the significant IE costs while building the graph, while the other is a novel, parallel, in-memory keyword search engine, which achieves orders of magnitude speed-up over our previous engine. Our experimental study on the realworld IJ application data confirms the benefits of our contributions.

Background In the past two decades, genetic testing for cancer risk assessment has entered mainstream clinical practice due to the availability of low-cost panels of multiple cancer-associated genes. However, the clinical value of multiple-gene panels for cancer susceptibility is not well established, especially in cases where panel testing identifies more than one pathogenic variant. The risk for specific malignancies as a result of a mutated gene is complex and likely influenced by superimposed modifier variants and/or environmental effects. Recent data suggests that the combination of multiple pathogenic variants may be fewer than reported by chance, suggesting that some mutation combinations may be detrimental. Management of patients with “incidentally” discovered mutations can be particularly challenging, especially when established guidelines call for radical procedures (e.g. total gastrectomy in CDH1) in patients and families without a classic clinical history concerning for that cancer predisposition syndrome. Case presentation We present two cases, one of an individual and one of a family, with multiple pathogenic mutations detected by multi-gene panel testing to highlight challenges practitioners face in counseling patients about pathogenic variants and determining preventive and therapeutic interventions. Conclusions Ongoing investigation is needed to improve our understanding of inherited susceptibility to disease in general and cancer predisposition syndromes, as this information has the potential to lead to the development of more precise and patient-specific counseling and surveillance strategies. The real-world adoption of new or improved technologies into clinical practice frequently requires medical decision-making in the absence of established understanding of gene-gene interactions. In the meantime, practitioners must be prepared to apply a rationale based on currently available knowledge to clinical decision-making. Current practice is evolving to rely heavily on clinical concordance with personal and family history in making specific therapeutic decisions.

28. RELIGIOSITY AND CORONARY HEART DISEASE IN GREEK ADULTS.

Background: In developed countries, cardiovascular disease is the most common cause of both death and functional disability. The last decades, scientific community shows an increasing interest exploring the relationship between religious and health factors. Aim: The aim of the present study was to explore associations between factors related to religiosity and self-reported Coronary Heart Disease (CHD). Material and Method: This was a descriptive observational study, conducted during a 2-year period. The final studied sample consist- ed of 1062 citizens of Greece. Data collection was performed by using a special designed questionnaire based on relevant literature review. Results: Half of the studied sample was men, (n=529, 49.9%). The majority of participants (77.1%) was living in urban areas of the coun- try and was mean educated (67.1%). When asked about their employment status, most of them (65%) answered that they were profes- sionally active. As far as their medical history is concerned, the majority of participants (88%) had no CHD history, no history of diabetes mellitus (90.3%), no history of dyslipidemias (71.8%) and no history of hypertension (71.8%). Moreover, 29.9% of the participants were overweight. Religious fasting follows 51.4% of the sample and the reason for fasting is because the church imposes fasting rules. The factor "fasting" was also related to CHD occurrence, as those who were following religious fasting had no history of CHD. A significant difference was found between responders with conscious belief in God and CHD occurrence. Conclusion: The findings of this study suggest that factors related to religiosity, such as doubt in God belief and religious fasting may influence CHD incidence. Prospective cohort studies and clinical trial studies that specifically designed to assess the effect of religiosity on CHD outcome are necessary to be conducted. [ABSTRACT FROM AUTHOR]

Data journalism is the field of investigative journalism which focuses on digital data by treating them as first-class citizens. Following the trends in human activity, which leaves strong digital traces, data journalism becomes increasingly important. However, as the number and the diversity of data sources increase, heterogeneous data models with different structure, or even no structure at all, need to be considered in query answering. Inspired by our collaboration with Le Monde, a leading French newspaper, we designed a novel query algorithm for exploiting such heterogeneous corpora through keyword search. We model our underlying data as graphs and, given a set of search terms, our algorithm nds links between them within and across the heterogeneous datasets included in the graph. We draw inspiration from prior work on keyword search in structured and unstructured data, which we extend with the data heterogeneity dimension, which makes the keyword search problem computationally harder. We implement our algorithm and we evaluate its performance using synthetic and real-world datasets., Informal publication only

Background: Experimental and clinical data suggest that reducing inflammation without affecting lipid levels may reduce the risk of cardiovascular disease. Yet, the inflammatory hypothesis of atherothrombosis has remained unproved. Methods: We conducted a randomized, double-blind trial of canakinumab, a therapeutic monoclonal antibody targeting interleukin-1β, involving 10,061 patients with previous myocardial infarction and a high-sensitivity C-reactive protein level of 2 mg or more per liter. The trial compared three doses of canakinumab (50 mg, 150 mg, and 300 mg, administered subcutaneously every 3 months) with placebo. The primary efficacy end point was nonfatal myocardial infarction, nonfatal stroke, or cardiovascular death. RESULTS: At 48 months, the median reduction from baseline in the high-sensitivity C-reactive protein level was 26 percentage points greater in the group that received the 50-mg dose of canakinumab, 37 percentage points greater in the 150-mg group, and 41 percentage points greater in the 300-mg group than in the placebo group. Canakinumab did not reduce lipid levels from baseline. At a median follow-up of 3.7 years, the incidence rate for the primary end point was 4.50 events per 100 person-years in the placebo group, 4.11 events per 100 person-years in the 50-mg group, 3.86 events per 100 person-years in the 150-mg group, and 3.90 events per 100 person-years in the 300-mg group. The hazard ratios as compared with placebo were as follows: in the 50-mg group, 0.93 (95% confidence interval [CI], 0.80 to 1.07; P = 0.30); in the 150-mg group, 0.85 (95% CI, 0.74 to 0.98; P = 0.021); and in the 300-mg group, 0.86 (95% CI, 0.75 to 0.99; P = 0.031). The 150-mg dose, but not the other doses, met the prespecified multiplicity-adjusted threshold for statistical significance for the primary end point and the secondary end point that additionally included hospitalization for unstable angina that led to urgent revascularization (hazard ratio vs. placebo, 0.83; 95% CI, 0.73 to 0.95; P = 0.005). Canakinumab was associated with a higher incidence of fatal infection than was placebo. There was no significant difference in all-cause mortality (hazard ratio for all canakinumab doses vs. placebo, 0.94; 95% CI, 0.83 to 1.06; P = 0.31). Conclusions: Antiinflammatory therapy targeting the interleukin-1β innate immunity pathway with canakinumab at a dose of 150 mg every 3 months led to a significantly lower rate of recurrent cardiovascular events than placebo, independent of lipid-level lowering. Heart failure (HF) is rapidly growing, conferring considerable mortality, morbidity, and costs. Dedicated HF clinics improve patient outcomes, and the development of a national HF clinics network aims at addressing this need at national level. Such a network should respect the existing health care infrastructures, and according to the capacities of hosting facilities, it can be organized into three levels. Establishing the continuous communication and interaction among the components of the network is crucial, while supportive actions that can enhance its efficiency include involvement of multidisciplinary health care professionals, use of structured HF‐specific documents, such as discharge notes, patient information leaflets, and patient booklets, and implementation of an HF‐specific electronic health care record and database platform.

Background: In the past two decades, genetic testing for cancer risk assessment has entered mainstream clinical practice due to the availability of low-cost panels of multiple cancer-associated genes. However, the clinical value of multiple-gene panels for cancer susceptibility is not well established, especially in cases where panel testing identifies more than one pathogenic variant. The risk for specific malignancies as a result of a mutated gene is complex and likely influenced by superimposed modifier variants and/or environmental effects. Recent data suggests that the combination of multiple pathogenic variants may be fewer than reported by chance, suggesting that some mutation combinations may be detrimental. Management of patients with "incidentally" discovered mutations can be particularly challenging, especially when established guidelines call for radical procedures (e.g. total gastrectomy in CDH1) in patients and families without a classic clinical history concerning for that cancer predisposition syndrome. Case presentation: We present two cases, one of an individual and one of a family, with multiple pathogenic mutations detected by multi-gene panel testing to highlight challenges practitioners face in counseling patients about pathogenic variants and determining preventive and therapeutic interventions. Conclusions: Ongoing investigation is needed to improve our understanding of inherited susceptibility to disease in general and cancer predisposition syndromes, as this information has the potential to lead to the development of more precise and patient-specific counseling and surveillance strategies. The real-world adoption of new or improved technologies into clinical practice frequently requires medical decision-making in the absence of established understanding of gene-gene interactions. In the meantime, practitioners must be prepared to apply a rationale based on currently available knowledge to clinical decision-making. In this paper, we propose a solution to support big data processing with MapReduce inside a wireless sensor network (WSN). The proposed framework provides a simple MapReduce interface, whose implementation can be dynamically loaded into the appropriate nodes. The execution of map and reduce functions inside the network is achieved by leveraging software defined networking principles and in particular by enhancing the SDN-WISE protocol on both the Controller and the sensor node sides. On the Controller side, the proposed framework introduces a functionality, which evaluates the optimal deployment of the processing functions in the network taking the typical WSN requirements into account and enforces it by instructing the flow tables to forward traffic from the mappers to the appropriate reducers. Then, on the node side, SDN-WISE is integrated with Contiki, a widely popular operating system for sensor nodes, which has the capability to load and execute new functions without service disruption, i.e., on the fly . The proposed framework is evaluated considering different network topologies and approaches about the data processing location.

The heterogeneity characterizing the Internet of Things (IoT) landscape can be addressed by leveraging network operating systems (NOS)s. Current NOS solutions have been developed for traditional infrastructured networks: they do not take the specific features of fundamental IoT components such as wireless sensor and actuator networks into account. Accordingly, in this paper an innovative integrated network operating system for the IoT which is obtained as the evolution of the Open Network Operating System (ONOS) is proposed. This has been appropriately extended to enhance the recently proposed SDN-WISE platform for supporting SDN in wireless sensor networks and let it interact with standard OpenFlow (OF) switches. The proposed integrated system has been prototyped.

This chapter describes CONVERGENCE’s licensing scheme and its governance, based on the MPEG-21 part 5 standard and on the specific content protection and rights management requirements, identified in the CONVERGENCE use scenarios. In the digital media value chain, Rights Expression Languages (RELs) are used to enable controlled access to digital resources, addressing several different issues from the description of licenses to access and usage control, payments, etc. A REL is an essential component of any security infrastructure supporting differentiated controlled access to digital resources, and providing adequate protection of intellectual property rights. Among these, the project has selected the MPEG-21 part 5 open standard, which can be implemented in XML, and is one of the main current contenders for a general-purpose REL. Our scheme is designed in the light of CONVERGENCE’s ability to distribute and manage any kind of digital resource in a large distributed environment, and this chapter explains how REL data is embedded into the CONVERGENCE data unit, the Versatile Digital Item (VDI) and introduces a basic set of security features, based on digital certificates, for the enforcement of the rights and conditions expressed in CONVERGENCE licenses.

Cerebrotendinous Xanthomatosis (CTX) is an autosomal recessive disorder of bile acid synthesis1. At least 50 different causative mutations have been identified in the CYP27A1 gene encoding for a sterol 27-hydroxylase important in bile acid synthesis2. Sterol 27-hydroxylase deficiency leads to 5α-cholestanol accumulation in the blood and tissues of affected patients1, including brain, often leading to severe neurological dysfunction that can incapacitate patients by the 4th–5th decade of life1. CTX generally presents clinically in the 2nd–3rd decade; childhood signs and symptoms can include chronic diarrhea, juvenile cataracts, school failure1,3, and potentially xanthomas1. Eighty-five percent of CTX patients develop cataracts of multiple types2, reported to occur as young as age 5–6 years1, with cataracts and chronic diarrhea commonly presenting before neurologic disease3. For this reason, and since diarrhea is non-specific, the clinician may not consider a diagnosis of CTX when cataracts are identified. Treatment for CTX is available in the form of chenodeoxycholic acid (CDCA)1,4. The FDA recently reapproved CDCA (Manchester Pharmaceuticals, Inc). As treatment of CTX from the preclinical stage prevents the onset of disease complications4, the value of early diagnosis for this disorder cannot be overstated. We describe two cases of CTX affected children enrolled in an IRB approved study at OHSU with bilateral cataracts as presenting signs of disease. Case 1 A 9 year old female was referred to an ophthalmologist for decreased vision over several months. Past ocular history was unremarkable; full ocular exam 2 years prior was normal. Family history was unremarkable. Past medical history was significant for diarrhea and nonverbal learning disorder. On examination uncorrected distance visual acuity was 20/80 and uncorrected near acuity was 20/50 in each eye. Manifest refraction was 0.75+0.50X90 in each eye, with corrected distance acuity of 20/60 in the right and 20/80 in the left. Complete dilated exam was remarkable for the presence of cataract in each eye and described as a diffuse nuclear haze on slit lamp examination, with mild posterior capsular opacification (figure 1). Retroillumination highlighted posterior capsular findings (figure 2). Sequential bilateral cataract surgery with intraocular lens placement resulted in a corrected visual acuity of 20/20 in each eye. A metabolic workup indicated plasma 5α-cholestanol was markedly elevated at 3.7 mg/dl (normal < 0.2 mg/dl1), suggestive of CTX. Urine testing confirmed CTX with elevated bile alcohol glucuronides present (30.1 μg/ml). CDCA treatment was initiated and diarrhea resolved within one month, with accompanying weight gain. There is increased interest in the role of dobutamine stress echocardiography (DSE) and adenosine magnetic resonance imaging (AMRI) performed in the chest pain unit as a diagnostic method to rule out Coronary Artery Disease (CAD) as the cause of the chest pain in this population.

- Abstract
Objective: We examined the association between climatologic parameters and daily admissions for non-fatal acute coronary syndromes (ACS) to emergency units of hospitals in the greater Athens area, from January 2001 to August 2002. Methods: Daily mean, maximum and minimum temperatures, relative humidity, wind speed, barometric pressure and a thermo-hydrological index (T.H.I.) were measured at the meteorological station of the Laboratory of Climatology of the Geology Department of the University of Athens. In addition, the daily number of admissions for acute myocardial infarction or unstable angina in the five major general hospitals in the greater Athens area was recorded. Generalized additive models (GAM) were applied to regress-time-series of daily numbers of outpatients with acute cardiac events against climatological variations, after controlling for possible confounders and adjustment for over dispersion and serial correlation. Results: Five thousand four hundred fifty-eight Athenians with non-fatal acute cardiac events were admitted to the selected hospitals during the period of the study, 4093 (75%) males and 1365 (25%) females.
