Abstract Background Abdominal pain in adults represents a wide range of illnesses, often warranting immediate intervention. This study is to fill the gap in the knowledge about incidence, presentation, causes and mortality from abdominal pain in an established emergency department of a tertiary hospital in Tanzania. Methods This was a prospective cohort study of adult (age ≥ 18 years) patients presenting to the Emergency Medicine Department of Muhimbili National Hospital (EMD-MNH) in Dar Es Salaam, Tanzania with non-traumatic abdominal pain from September 2017 to October 2017. A case report form was used to record data on demographics, clinical presentation, management, diagnosis, outcomes and patient follow-up. The primary outcome of mortality was summarized using descriptive statistics; secondary outcome was, risks for mortality. Results Among 3381 adult patients present during the study period, 288 (8.5%) presented with abdominal pain, and of these 199 (69%) patients were enrolled in our study. Median age was 47 years (IQR 35–60 years), 126 (63%) were female, and 118 (59%) were referred from another hospital. Most common final diagnoses were malignancies 71 (36%), intestinal obstruction 11 (6%) and peptic ulcer disease 9 (5%). Most common EMD interventions given were intravenous fluids 57 (21%), analgesia 49 (25%) and antibiotics 40 (20%). 160 (80%) were admitted of which 15 (8%) underwent surgery directly from EMD. 24-h and 7-day mortality were 4 (2%) and 7 (4%) respectively, while overall in hospital-mortality was 16 (8%). Abstract Background Non-traumatic chest pain (NTCP) is a common reason for emergency department (ED) attendance in high-income countries, with the primary concern focused on life threatening cardiovascular diseases. There is general lack of data on aetiologies, diagnosis and management of NTPC in Sub Sahara African (SSA) countries. We aimed to describe evaluation, diagnosis and outcomes of adult patients presenting with NTCP to an urban ED in Tanzania. Method This was a prospective observational cohort study of consecutive adult (≥18 years) patients presenting with non-traumatic chest pain to the Emergency Medicine Department (EMD) of Muhimbili National Hospital (MNH) in Dar es salaam from September 2017 to April 2018. Structured case report form was used to collected demographics, clinical presentation, investigations, diagnosis, and EMD disposition and in hospital mortality. We determined frequency of NTCP among our patients, aetiologies, 24-h and 7-day in-hospital mortality, and predictors for mortality. Results We screened 29,495 adults attending EMD-MNH during the study and 389 (1.3%) presented with NTCP of these, 349 (90%) were enrolled. The median age was 45 (IQR 29–60) years and 177 (50.7%) were female. Overall, 69.1% patients received electrocardiography (ECG) in the EMD and 34.1% had a troponin test. Heart failure and pulmonary tuberculosis (PTB) were the leading hospital diagnoses (12.6% each), followed by chronic kidney disease (10%) and acute coronary syndrome (ACS) (9.6%). Total of 167 (48%) patients were admitted, and the 24-h and 7-day in-hospital mortality were 5 (3%) and 16 (9.6%) respectively. Univariate risk factors for mortality were a Glasgow Coma Scale of 0.04 ng/ml [RR 2.9 (95%CI 1.2–7.3)]. Features distinguishing cardiovascular from other causes were: bradycardia [RR = 2.6 (95%CI 2.1–3.2)], heart beat awareness [RR = 2.3 (95%CI 1.7–3.2)] and history of diabetic mellitus [RR = 2.2 (95% CI 1.6–3.0)]. Conclusion In this ED of SSA country, heart failure and pulmonary tuberculosis were the leading causes of NCTP, and ACS was present in 9.6%. NTCP in this setting carries high mortality, and ACS was the leading risk factor for death. ED providers in SSA must increasingly consider cardiovascular causes of NTCP.

Abstract Background Marburg virus disease (MVD) is a deadly illness caused by the zoonotic Marburg virus, which has led to outbreaks with fatality rates up to 100% in some African countries. On March 21, 2023, Tanzania had its first MVD outbreak, resulting in nine cases and six deaths, leading to a fatality rate of 66.7%. Following that, the Risk Communication and Community Engagement (RCCE) approach was promptly initiated to create community awareness regarding the MVD. A descriptive cross-sectional assessment was conducted in May 2023 in Bukoba District Council and Bukoba Municipal Council in the Kagera region to document the community awareness and sources of information regarding MVD during the outbreak. Data were collected using a structured questionnaire developed using the WHO COVID-19 RCCE Rapid Quantitative Assessment Tool. Descriptive analysis was conducted using Microsoft Excel 2021. Results There were a total of 714 community respondents, of whom 456 (63.9%) were from Bukoba District Council. The majority 628 (88%) were aged 18 years and above. There were 393 (55%) females, and 407 (57%) of respondents had completed primary education. All respondents reported being informed about MVD through different channels, with 588 (82.4%) receiving information from Community Health Workers. Most of them 573 (80.3%) were satisfied with implemented RCCE interventions, 651 (91.2%) perceived MVD to be an extremely severe disease, and 698 (97.76%) mentioned hand washing as one of the recommended preventive measures. Conclusions During the Marburg virus disease outbreak response, where risk communication and community engagement interventions were implemented, the community was aware of the Marburg virus disease and community health workers emerged as the most frequently mentioned channel of communication during the outbreak.

Abstract Background Increased global trade, while beneficial economically, can also increase the spread of vector-borne diseases, particularly those transmitted by Aedes mosquitoes spreading via trade routes. Given the heightened trade-induced activity at ports of entry, it is particularly crucial to assess the risk of mosquito-borne diseases in these settings. This study compared the risks of Aedes-borne disease in and around the eastern Tanzanian seaport of Tanga. Methods A 200 m × 200 m grid-based system was used to sample mosquitoes within the port area, and in surrounding areas at 2 km, 2.5 km, and 5 km away, between June and December 2023. We characterized mosquito breeding habitats, collected mosquito larvae using standard dippers and tested susceptibility of raised adult Aedes aegypti populations to different insecticides. Adult mosquitoes were collected using BG sentinel traps (daytime) and Centers for Disease Control (CDC) light traps (night-time). Additionally, more than 200 port users and neighboring residents were surveyed to assess their experiences with and perceptions of mosquito biting and disease risks. Results There were 2931 breeding sites, with (60.8%, n = 1782) positive for Aedes larvae. The percentage of water-holding containers infested with Aedes immatures, i.e., the container index (CI), was highest in the port area (66.2%), and lowest 5 km away (44.6%). The port area also had a greater proportion of temporary breeding sites (64.9%) than did the surrounding areas. The adult mosquito surveys revealed 20,449 mosquito species including: Culex quinquefasciatus (56.2%), Mansonia uniformis (38.6%), Ae. aegypti (5.1%), Anopheles gambiae (0.1%), and Anopheles funestus. Introduction: Physical exercise is a well-documented treatment for individuals with mental disorder. It helps improve symptoms and functioning of these individuals. Moreover, recent studies indicated that exercise improve emotional regulation which is one of the main target in borderline personality disorder (BPD) treatment. Therefore, exercise might have important benefits in this population. However, no previous study examined this effect. Objectives: This pilot study documents the facceptability of a protocol testing the effects of exercise on the response to a negative emotion in adults with BPD. Methods: 28 adults with a diagnosis of BPD have been recruited in a psychiatric hospital. Participants filled several questionnaires then viewed a scene from Silence of the Lambs to induce negative emotions. They were then assigned to 20 minutes of exercise or a neutral video of 20minutes. Affects were assessed 7 times during the protocol. Results: In this sample, 9 participants reported at least equal levels of affect after the induction than before. Preliminary results show a tendency of higher response of physical exercise than control on positive affects and no participant had any adverse effect fromexercise. Conclusions: This pilot study was the first to test the effects of exercise on symptoms of BPD. It also informs on the best way to conduct the principal study. First, the mood induction was poor, thus it will be changed for a stronger induction strategy. Then, the control intervention will be a placebo exercise. Les pneumopathies nosocomiales (PN) à Staphylocoque aureus (SA) résistant à la méticilline (SARM) en réanimation restent une problématique importante malgré l'émergence d'autres germes. L'objectif est de décrire la tendance des PN à SARM de 2003 à 2018 dans les services de réanimation d'un CHU. Les données de surveillance (Réa RAISIN) entre 2003 et 2018, de 10 services (total 131 lits) de réanimation adulte d'un CHU, ont été analysées. Les patients sélectionnés étaient des adultes hospitalisés plus de 2 jours et ayant présenté une PN. L'évolution au cours du temps de la proportion de PN à SA ainsi que de la proportion de SARM ont été décrites et évaluées à l'aide d'un test de Chi2 de tendance. Au total, 41 357 patients ont été inclus dont 3971 (9,6 %) PN entre 2003 et 2018. Sur cette période, une augmentation significative du taux de PN a été observée entre 2003 et 2009 (8,3 % contre 12,9 % respectivement, p < 10−3) puis une diminution entre 2010 et 2018 passant de 9,3 % à 6,8 % (p < 10−2). Chez les patients intubés : augmentation significative du taux de PN acquise par ventilation mécanique (PAVM) entre 2003 et 2009 (11,4 % contre 16,4 % respectivement, p < 10−2), puis diminution entre 2010 et 2018 (12,2 % contre 10,5 % respectivement, p < 10−2). Au total, 744 (16,5 %) PN à SA ont été identifiées dont 198 SARM représentant 26,6 % des PN à SA, soit 5 % des PN globale. Le taux de PN à SA diminuait significativement au cours du temps, passant de 35 % en 2003 à 15 % en 2018, p < 10−3. Le taux de SARM parmi les PN à SA diminuait de 37,5 % en 2003 à 12,5 % en 2018, p = 0,03. Nous retrouvions des résultats similaires chez les patients intubés avec un taux de PAVM à SA variant de 36,4 % à 16,6 % entre 2003 et 2018 (p < 10−3) et un taux de SARM parmi les PAVM à SA variant de 34,6 % à 13,0 % sur la même période (p < 10−3). Malgré un taux de PN quasi constant au cours du temps, la proportion des PN à SA et SARM semblent diminuer depuis 2003, confirmant l'implication des équipes de réanimation dans la prévention des infections nosocomiales. Health risks associated with poor sanitation behaviours continue to be reported mostly from low-income countries (LICs). Reports show that various factors limit many people from accessing and using improved latrines, forcing some to opt for sharing latrines with neighbours, others practicing open defecation. Meanwhile, debate prevails on whether shared latrines should be categorised as unimproved according to WHO/UNICEF-JMP criteria. We contribute to this debate based on results from a study undertaken in three regions, Tanzania.Data were collected through observations in 1,751 households with latrines, coupled with collection of opinions from heads of such households regarding the latrine-sharing practices. Bivariate and multivariate logistic regression analyses were performed to assess associations between the outcome and possible predictor variables.Of all 1,751 latrines, 14.6% were shared. Among the shared latrines, 74.2% were found being generally clean as compared to 69.2% of the non-shared ones. Comparing the shared and non-shared latrines, the non-shared latrines were significantly less likely to be found with floors built with permanent materials (OR = 0.73, 95% CI: 0.55, 0.98); washable floors (OR = 0.69; 95% CI: 0.51, 0.93); and lockable doors (OR = 0.73; 95% CI: 0.56, 0.95). Shared latrines were less likely to have floors with faecal matter, functional handwashing facilities (HWFs), HWFs with running water, and roofs; albeit the differences in all these scenarios were not statistically significant. Respondents expressed desire for improved latrines, but also did not find it wrong to share latrines if cleanliness was maintained.Having an 'improved' latrine remains important as JMP recommends, but based on our study findings, we argue that possessing a non-shared latrine neither guarantees safety to its users nor its categorisation as 'improved'. Instead, the state of the latrine, the construction technology used and the behaviours of the users may be more important.

L'infection à SARS-CoV-2 est à l'origine de syndromes respiratoires aigus sévères pouvant nécessiter un recours aux soins intensifs. Un des enjeux de la pandémie survenue début 2020 est de déterminer les facteurs associés à l'aggravation clinique. Le lien éventuel entre le délai d'accès aux soins et la survenue de formes sévères est mal connu. L'objectif de ce travail est de décrire les caractéristiques des patients hospitalisés pour infection à SARS-CoV-2 selon le délai entre le début des symptômes et l'admission en hospitalisation, afin de déterminer les caractéristiques associées à un délai court (< 48 h) ou long (> 10 jours). Ce travail repose sur les données de l'étude observationnelle, prospective, multicentrique NOSO-COR dont les objectifs étaient de décrire les cas d'infection par SARS-CoV-2 hospitalisés. L'analyse a été effectuée sur les données des cas communautaires d'infection à SARS-CoV-2 inclus au sein d'un CHU français du 08/02 au 17/05/2020. Les caractéristiques démographiques, les comorbidités sous-jacentes, les paramètres cliniques et biologiques ont été collectés. Le patient a été suivi jusqu'à la sortie de l'hôpital. Les comparaisons entre les groupes ont été effectuées en utilisant le test de Mann–Whitney (variables quantitatives) et le test du Chi2 (variables qualitatives). Les différentes caractéristiques associées au délai entre début des symptômes et admission en hospitalisation ont été évaluées à l'aide de régression logistique ajustée sur l'âge, le sexe, les comorbidités et les principaux symptômes. Huit cent vingt-deux patients ont été inclus dans l'analyse. L'âge médian était de 73 ans (IQR : 61–84), 453 (55,1 %) étaient des hommes. Comparés aux patients présentant un délai court entre le début des symptômes et l'hospitalisation, les patients présentant un délai long étaient plus jeunes (médiane 68 vs 82 ans, p < 0,001), avaient moins fréquemment de comorbidités cardiovasculaires (38,1 % vs 59,7 %, p < 0,001), neurologiques (9,9 % vs 34,7 %, p < 0,001) et présentaient plus fréquemment une hospitalisation directe en soins intensifs (24,3 % vs 5,6 %, p < 0,001). Ils présentaient plus fréquemment une toux (73,3 % vs 47,9 %, p < 0,001), des diarrhées (30,0 % vs 12,5 % p < 0,001), des difficultés à respirer (64,4 % vs 48,6 %, p = 0,004), une fatigue (71,7 % vs 57,6 %, p = 0,009), une anosmie (8,9 % vs 0,7 %, p < 0,001), une agueusie (11,7 % vs 1,4 %, p < 0,001), et un taux plasmatique de CRP plus élevé (médiane 107,0 mg/L vs 49,2, p < 0,001). Après analyse multivariée, les variables associées à un délai court entre début des symptômes et hospitalisation étaient l'âge > 75 ans (p < 0,001), les comorbidités cardiovasculaires (p = 0,043) et neurologiques (p = 0,009). Les variables associées à un délai long étaient la présence d'une toux (p = 0,019), le taux plasmatique de CRP > 100 mg/L (p = 0,003), l'anosmie (p = 0,008) et l'agueusie (p = 0,018). Le délai entre début des symptômes et hospitalisation semble être allongé chez les patients < 75 ans et ne présentant pas de comorbidités ; cette population est surreprésentée chez les patients bénéficiant d'une hospitalisation d'emblée en soins intensifs. La détection des signes de gravité et l'accès précoce à l'hospitalisation pourrait être un moyen de limiter la survenue de formes cliniques sévères. [ABSTRACT FROM AUTHOR]

Le virus du syndrome respiratoire aigu sévère à coronavirus 2 (SRAS-CoV-2), détecté pour la première fois en décembre 2019 dans la province chinoise du Hubei, a été qualifié de pandémique le 11 mars 2020. La COVID-19 est la maladie infectieuse émergente due au SRAS-CoV-2, principalement associée à une infection des voies respiratoires inférieures. Une connaissance approfondie des caractéristiques cliniques des patients COVID-19 symptomatiques lors de leur admission hospitalière est cruciale pour optimiser leur prise en charge dans ce contexte pandémique. L'objectif principal de l'étude était de décrire les caractéristiques cliniques des patients COVID-19 à l'admission et d'identifier les déterminants associés à l'admission directe ou au transfert dans les unités de soins intensifs (USI) des différents hôpitaux participants. Cette étude est un projet ancillaire du projet NOSO-COR, une étude prospective, observationnelle, en milieu hospitalier, dont l'objectif principal est d'estimer le risque de la COVID-19 nosocomiale. Les caractéristiques démographiques, les comorbidités sous-jacentes, ainsi que les paramètres cliniques et biologiques ont été collectés. Les patients ont été suivis jusqu'à la sortie de l'hôpital ou le décès. Les comparaisons entre les groupes ont été effectuées en utilisant le test de Mann–Whitney ou Kruskall–Wallis et le test du Chi2 ou exact de Fisher, le cas échéant. Un modèle de régression multivariée a été appliquée à 374 patients pour lesquels la durée de séjour d'hospitalisation était complète et sans données manquantes. Sur les 412 patients inclus, 325 (78,9 %) sont sortis vivants de l'hôpital et 87 sont décédés à l'hôpital au moment de l'analyse. L'âge médian était de 72,0 ans [IQR : 57–83] et 56,3 % étaient des hommes. Au total, 286 patients (69,4 %) avaient un ou plusieurs comorbidités dont les plus fréquents étaient les maladies cardiovasculaires (47,6 %) et le diabètes (19,9 %). Le sexe masculin (odds ratio [OR] ajusté, 1,99 [intervalle de confiance à 95 % (IC95 %) : 1,07–3,72]), la fièvre (OR : 1,37 [IC95 % : 1,01–1,88]), une auscultation pulmonaire anormale à l'admission (OR ajusté : 2,62 [IC95 % : 1,40–4,90]) et un taux élevé de la protéine C réactive (CRP) (OR ajusté : 6,96 [IC95 % : 1,45–33,35] pour CRP > 100 mg/L vs CRP < 10 mg/L) étaient associés à un risque d'admission/transfert en USI. L'allongement du délai entre l'apparition des symptômes et l'admission à l'hôpital a été associé à un risque d'admission directe/transfert en USI (OR ajusté : 4,82 [IC95 % : 1,61–14,43] pour un délai > 10 jours vs un délai < 3 jours). La présence d'une monocytopénie était également associée à un risque d'admission directe/transfert en USI (OR ajusté : 2,49 [IC95 % : 1,29–4,82]). La présentation clinique de l'infection par le SRAS-CoV-2 lors de l'admission à l'hôpital est associée au pronostic. Le délai entre le début des symptômes et l'admission à l'hôpital doit être considéré comme un marquer potentiel d'aggravation de l'état des patients. La COVID-19, pathologie associée au syndrome respiratoire aigu sévère du coronavirus 2 (SRAS-CoV-2) a émergé fin 2019 et est à l'origine d'infections des voies respiratoires inférieures. Cette étude ancillaire, ancrée dans le projet NOSO-COR destiné à l'étude du risque nosocomial du SRAS-CoV-2, a pour but d'étudier les caractéristiques cliniques et biologiques à l'admission des patients COVID-19 hospitalisés. Une étude observationnelle, prospective et multicentrique a été conduite dans un centre hospitalier universitaire de 5300 lits. Tout patient diagnostiqué positif à la COVID-19, présentant de la toux et/ou de la fièvre supérieure à 37,8 ° C, admis à l'hôpital et y ayant séjourné plus de 24 heures a été inclus dans l'étude. Les caractéristiques sociodémographiques, les comorbidités sous-jacentes ainsi que les paramètres cliniques et biologiques à l'admission ont été recueillis. Une analyse descriptive et stratifiée selon l'âge (inférieur 60 ans, entre 60 et 80 ans et supérieur 80 ans) a été effectuée et réalisée avec le logiciel STATA 14.0®. Au total, 1121 cas ont été inclus dans l'étude NOSO-COR entre le 8/02 et le 10/06/2020 avec un pic épidémique atteint au 26/03/2020. L'âge médian était de 77 ans [IQR : 64–87], avec un sex-ratio de 1,06. Les patients étaient à 79,1 % hospitalisés en service de médecine, chirurgie et gynéco-obstétrique et 7,3 % d'entre eux ont été secondairement transférés en service de réanimation. Le taux d'admission directe en réanimation était de 18,4 %. Deux cent cinquante patients, dont 166 (66,4 %) âgés de plus de 80 ans, sont décédés (toutes causes confondues), soit un taux brut de létalité observée de 22,3 %. Les patients présentaient de la fièvre supérieure à 37,8 °C, de la toux, une auscultation pulmonaire anormale et une dyspnée dans 82,5 %, 62,4 %, 59,3 % et 48,3 % des cas respectivement. De plus, 1031 (90,2 %) des patients souffraient de comorbidités, dont 566 (50,4 %) avec des antécédents cardiologiques. La moitié des patients inclus présentaient une lymphopénie, et 1/3 une protéine C réactive supérieure à 100 mg/L. La durée médiane de séjour d'hospitalisation était différente selon les catégories d'âge, avec un séjour plus long chez les patients âgés de plus de 80 ans (4 jours [IQR : 2–9] vs 8 jours [IQR : 4–14] vs 9 jours [IQR : 4–15], p < 0,001). Les patients âgés de plus de 80 ans décédaient plus fréquemment (6,3 % vs 17,0 % vs 34,2 %, p < 0,001). Les comorbidités cardiologiques, neurologiques et néoplasiques apparaissaient comme déterminantes selon l'âge des patients COVID-19 (p = 0,001). Background: Influenza can be associated with nonrespiratory disease presentation, but these are less well documented due to the lack of routine testing for influenza in the healthcare system, especially if patients do not present with influenza-like illness (ILI). We aimed to measure the proportion of influenza cases seeking care at emergency department (ED) for a nontraumatic cause, to describe their clinical presentation and their ED-discharge diagnosis., Methods: The study was conducted at seven hospitals in France, Spain, Italy and Germany during the 2019-20 influenza season, for a period of 10 weeks. Patients (≥ 18 years) consulting for nontraumatic causes at the ED were invited to participate. Consenting patients provided upper respiratory swab samples for influenza testing by reverse transcription polymerase chain reaction. Clinical and demographic data were collected., Results: There were 8678 patients included, 50.7% were female and the median age was 57 years. Among them, 494 (5.7%) were laboratory-confirmed influenza (LCI) cases. Nonetheless, only 24.3% of LCI cases had an ED-discharge of influenza. Of all cases confirmed as influenza, 47.6% had a nonrespiratory discharge diagnosis, which frequency increased with age. ILI case definition from the European Centre for Disease Prevention and Control was the most frequently met among influenza cases (68.6%). Older patients (≥ 65 years) were less frequently identified based on any ILI signs/symptoms., Conclusion: Our findings indicate that the impact of influenza among patients seeking care at the ED cannot be easily assessed based on clinical presentation and medical records alone. Background: Literature is limited on the impact of patient vaccination on the risk of hospital-acquired influenza (HAI). This test negative case-control study nested in a surveillance program aimed at evaluating the effectiveness of influenza vaccination in reducing the risk of HAI in hospitalized patients during 15 influenza seasons (2004-05 to 2019-20)., Methods: HAI cases were those who developed influenza like illness (ILI) symptoms at least 72 h after hospitalization and had a positive reverse transcriptase-polymerase chain reaction (RT-PCR). Controls were those with ILI symptoms and a negative RT-PCR test. A nasal swab as well as socio-demographic, clinical data and information on influenza vaccination were collected., Results: Of the 296 patients included, 67 were confirmed HAI cases. Influenza vaccine coverage was significantly higher among controls compared to HAI cases (p = 0.002). Individuals with personality disorders (PDs) have a decreased life expectancy compared with the general population in part due to physical illnesses. Many hypotheses have been suggested to explain those physical illnesses such as hormone imbalance, medication, lack of physical activity, and unhealthy diet. However, little is known about the relation between lifestyle and PDs. The purpose of this scoping review is to regroup the available information on this topic. We searched the literature up to February 2021 using four databases and found 21 articles analyzing the relation between lifestyle and PDs in observational studies including 153,081 participants from diverse populations going from general population to adults in psychiatric care. Most studies used measures of lifestyle as control variables or did not use lifestyle variables at all. Moreover, the instruments used to measure lifestyle variables lacked precision at best. Two studies demonstrated a relation between early malnutrition and further development of PDs, but those results may be influenced by confounding variables and cannot indicate a clear link between nutrition and personality disorder. The lack of solid evidence we observed is surprising, considering the multiple benefits individuals with PDs could get from a healthy lifestyle. Background: Exposure to infected healthcare workers (HCWs) is a source of hospital-acquired (HA) influenza. We estimated the risk of HA influenza for hospitalized patients by rate of influenza vaccine coverage (IVC) of HCWs., Methods: A case-case negative control study nested in a prospective cohort was conducted in 2 French university hospitals during 2 influenza seasons. Each inpatient with influenza-like illness (ILI) provided a nasal swab sample that was systematically analyzed for influenza virus by polymerase chain reaction (PCR) testing. An HA influenza case was a patient with a virological confirmation of influenza with onset of symptoms ≥72 hours after admission to the ward. The IVC rate of HCWs in each participating ward was calculated from the data provided by the occupational health departments. A mixed-effect logistic regression was performed with adjustments on patient sex, age, the presence of a potential source of influenza on the ward in the 5 days prior to the start of the ILI, type of ward and influenza season., Results: The overall HA influenza attack rate was 1.9 per 1,000 hospitalized patients. In total, 24 confirmed HA influenza cases and 141 controls were included. The crude odds ratio (OR) of HA influenza decreased from 0.52 (95% confidence interval [CI], 0.21-1.29) to 0.14 (95% CI, 0.03-0.63) when the IVC of HCWs increased from 20% to 40%. After adjustment, IVC ≥40% was associated with a risk reduction of HA influenza (aOR, 0.07; 95% CI, 0.01-0.78)., Conclusions: Considering a limited sample size, influenza vaccination of HCWs is highly suggestive of HA flu prevention among hospitalized patients.Trial Registration: clinicaltrials.gov identifier: NCT02198638.

Background: Physical exercise is an evidence-based treatment to reduce symptoms and negative affect in several psychiatric disorders, including depressive, anxiety, and psychotic disorders. However, the effect of physical exercise on negative affect in patients with borderline personality disorder (BPD) has not yet been investigated. In this pilot study, we tested the safety, acceptability, and potential acute effects on negative affect of a single session of aerobic physical exercise in adults with BPD., Method: After completing a negative mood induction procedure, 28 adults with BPD were randomly assigned to a 20-minute single session of stationary bicycle or a control condition (emotionally neutral video)., Results: No adverse effects attributed to the physical exercise were reported and it was considered acceptable to patients. Following the negative mood induction, both conditions decreased the level of negative affect with a medium effect size but there was no significant difference between them., Conclusion: The results suggest that a single 20-minute session of physical exercise does not produce a reduction of negative affect in BPD. The superior longitudinal fasciculus (SLF) is part of the longitudinal association fiber system, which lays connections between the frontal lobe and other areas of the ipsilateral hemisphere. As a dominant association fiber bundle, it should correspond to a well-defined structure with a clear anatomical definition. However, this is not the case, and a lot of confusion and overlap surrounds this entity. In this review/opinion study, we survey relevant current literature on the topic and try to clarify the definition of SLF in each hemisphere. After a comparison of postmortem dissections and data obtained from diffusion MRI studies, we discuss the specifics of this bundle regarding its anatomical landmarks, differences in lateralization, as well as individual variability. We also discuss the confusion regarding the arcuate fasciculus in relation to the SLF. Inaccurate perceptions, such as under- or over-estimation of body size are often found in clinical eating disorder populations but have recently been shown also in healthy people. However, it is not yet clear how body size perception may be affected when the internal body representation is manipulated. In this study, visual adaptation was used to investigate whether exposure to distorted visual feedback alters the representation of body size and how long any such effects might last. Participants were exposed for five minutes to a distorted life-size image of themselves that was either 20% wider or 20% narrower than their normal size. Accuracy was measured using our novel psychophysical method that taps into the implicit body representation. The accuracy of the representation was assessed at 6, 12 and 18 min following exposure to adaptation. Altered visual feedback caused changes in participants' judgements of their body size: adapting to a wider body resulted in size overestimation whereas underestimations occurred after adapting to a narrower body. These distortions lasted throughout testing and did not fully return back to normal within 18 min. Inhaled corticosteroids (ICS) have been associated with increased risk of pneumonia. Their impact on respiratory virus infections is unclear. We performed a post-hoc analysis of the FLUVAC cohort, a multicenter prospective cohort study of adults hospitalized with influenza-like illness (ILI) during six consecutive influenza seasons (2012-2018). All patients were tested for respiratory virus infection by multiplex PCR on nasopharyngeal swabs and/or bronchoalveolar lavage. Risk factors were identified by logistic regression analysis. Among the 2658 patients included, 537 (20.2%) were treated with ICS before admission, of whom 282 (52.5%, 282/537) tested positive for at least one respiratory virus. Patients on ICS were more likely to test positive for non-influenza respiratory viruses (25.1% vs. 19.5%, P = 0.004), especially for adenovirus (aOR 2.36, 95% CI 1.18-4.58), and respiratory syncytial virus (aOR 2.08, 95% CI 1.39-3.09). Complications were reported in 55.9% of patients on ICS (300/537), primarily pneumonia (171/535, 32%). Among patients on chronic ICS who tested positive for respiratory virus, 14.2% (40/282) were admitted to intensive care unit, and in-hospital mortality rate was 2.8% (8/282). Objectives: The purpose of this study was to describe the clinical characteristics and in-hospital and post-discharge outcomes of respiratory syncytial virus (RSV) infection among adults hospitalised with influenza-like illness (ILI) and compared against patients admitted for influenza., Methods: Adults hospitalised with ILI were prospectively included from five French university hospitals over two consecutive winter seasons (2017/2018 and 2018/2019). RSV and influenza virus were detected by multiplex reverse transcription PCR on nasopharyngeal swabs. RSV-positive patients were compared to RSV-negative and influenza-positive hospitalised patients. Poisson regression models were used to estimate the adjusted prevalence ratio (aPR) associated with in-hospital and post-discharge outcomes between RSV and influenza infections. The in-hospital outcome was a composite of the occurrence of at least one complication, length of stay ≥7 days, intensive care unit admission, use of mechanical ventilation and in-hospital death. Post-discharge outcome included 30- and 90-day all-cause mortality and 90-day readmission rates., Results: Overall, 1428 hospitalised adults with ILI were included. RSV was detected in 8% (114 of 1428) and influenza virus in 31% (437 of 1428). Patients hospitalised with RSV were older than those with influenza (mean age 73.0 versus 68.8 years, p=0.015) with a higher frequency of chronic respiratory or cardiac disease (52% versus 39%, p=0.012, and 52% versus 41%, p=0.039, respectively) and longer hospitalisation duration (median stay 8 versus 6 days, p<0.001). Anti-influenza therapies were less prescribed among RSV patients than influenza patients (20% versus 66%, p<0.001). In-hospital composite outcome was poorer in RSV patients (aPR 1.5, 95% CI 1.1-2.1) than in those hospitalised with influenza. Introduction: Delay between symptom onset and access to care is essential to prevent clinical worsening for different infectious diseases. For COVID-19, this delay might be associated with the clinical prognosis, but also with the different characteristics of patients. The objective was to describe characteristics and symptoms of community-acquired (CA) COVID-19 patients at hospital admission according to the delay between symptom onset and hospital admission, and to identify determinants associated with delay of admission., Methods: The present work was based on prospective NOSO-COR cohort data, and restricted to patients with laboratory confirmed CA SARS-CoV-2 infection admitted to Lyon hospitals between February 8 and June 30, 2020. Long delay of hospital admission was defined as ≥6 days between symptom onset and hospital admission. Determinants of the delay between symptom onset and hospital admission were identified by univariate and multiple logistic regression analysis., Results: Data from 827 patients were analysed. Patients with a long delay between symptom onset and hospital admission were younger (p<0.01), had higher body mass index (p<0.01), and were more frequently admitted to intensive care unit (p<0.01). Their plasma levels of C-reactive protein were also significantly higher (p<0.01). The crude in-hospital fatality rate was lower in this group (13.3% versus 27.6%), p<0.01. Multiple analysis with correction for multiple testing showed that age ≥75 years was associated with a short delay between symptom onset and hospital admission (≤5 days) (aOR: 0.47 95% CI (0.34-0.66)) and CRP>100 mg/L at admission was associated with a long delay (aOR: 1.84 95% CI (1.32-2.55))., Discussion: Delay between symptom onset and hospital admission is a major issue regarding prognosis of COVID-19 but can be related to multiple factors such as individual characteristics, organization of care and severe pathogenic processes. Information gathered so far from published studies attest the existence of a complex relationship between tobacco smoking and the severity of COVID-19. We investigated the association between smoking habits and the severity of COVID-19 in patients hospitalized in university-affiliated hospitals in Lyon, France. Baseline sociodemographic, clinical and biological characteristics of adult COVID-19 hospitalized patients presenting from the community were prospectively collected and analyzed. Tobacco exposure was documented at admission. Characteristics of patients hospitalized in medical wards to those admitted or transferred to intensive care units (ICUs) were compared using Mann-Whitney and Χ 2 or Fisher's exact test. A composite endpoint including admission or transfer to ICU or death was created as a proxy for severe outcome. Adjusted odds ratio (aOR) and 95% confidence interval (95% CI) were calculated to identify variables independently associated with a severe outcome. Of the 645 patients with documented information on smoking habits, 62.6% were never-smokers, 32.1% ex-smokers, and 5.3% active smokers. Past tobacco use was independently associated with an increased risk of severe outcome (aOR: 1.71; 95% CI: 1.12-2.63), whereas a nonsignificant protective trend was found for active smoking. The results suggest that past smoking is associated with enhanced risk of progressing toward severe COVID-19 disease in hospitalized patients., (© 2021 Wiley Periodicals LLC.)

The aim of this study was to describe the proportion of multidrug-resistant microorganisms (MDROs) involved in ventilator-associated pneumonia (VAP) as the first hospital-acquired infection in 536 adults with restricted risk factors for MDRO-related infection. Background: Hospital-acquired influenza potentially leads to significant morbidity and mortality in already vulnerable patients, but its overall burden is not fully understood. We undertook this study to estimate the incidence and trends of hospital-acquired laboratory-confirmed influenza among adults, and to compare clinical characteristics between hospital-acquired and community-acquired influenza cases., Methods: This was a prospective surveillance study over 11 years of adults with influenza-like-illness (ILI) hospitalized in surgery, medicine and geriatric wards in a tertiary acute-care hospital in Lyon, France. Nasal swabs were systematically collected from those with ILI and tested for influenza by reverse transcriptase-polymerase chain reaction at the national influenza reference laboratory (Lyon, France)., Results: Influenza was laboratory confirmed at a rate of 1 in 13 patients who developed ILI during their hospitalization. Having an underlying disease was an important characteristic of hospital-acquired ILI cases. Cardiovascular disease was the most frequent underlying condition in both influenza-positive and influenza-negative patients. Complications were more frequent for influenza-positive than influenza-negative patients. Background: Influenza is an important cause of viral hospital-acquired infection involving patients, healthcare workers (HCW), and visitors. The frequency of asymptomatic influenza among HCW with possible subsequent transmission is poorly described. The objective is to determine the cumulative incidence of asymptomatic, paucisymptomatic, and symptomatic influenza among HCW., Method: A multicenter prospective cohort study was done in 5 French university hospitals, including 289 HCW during the 2016-2017 influenza season. HCW had 3 physical examinations (time [T] 0, before epidemic onset; T.1, before epidemic peak; T.2, T.3, after epidemic peak). A blood sample was taken each time for influenza serology and a nasal swab was collected at T1 and T2 for influenza detection by polymerase chain reaction (PCR). Positive influenza was defined as either a positive influenza PCR, and/or virus-specific seroconversion against influenza A, the only circulating virus, with no vaccination record during follow-up. Symptoms were self-reported daily between T1 and T2. Cumulative incidence of influenza was stratified by clinical presentation per 100 HCW., Results: Of the 289 HCW included, 278 (96%) completed the entire follow-up. Overall, 62 HCW had evidence of influenza of whom 46.8% were asymptomatic, 41.9% were paucisymptomatic, and 11.3% were symptomatic. Cumulative influenza incidence was 22.3% (95% confidence interval [CI]: 17.4%-27.2%). Cumulative incidence of asymptomatic influenza was 5.8% (95% CI: 3.3%-9.2%), 13.7% (95% CI: 9.9%-18.2%) for paucisymptomatic influenza, and 2.9% (95% CI: 1.3%-5.5%) for symptomatic influenza., Conclusions: Asymptomatic and paucisymptomatic influenza were frequent among HCW, representing 47% and 42% of the influenza burden, respectively. Vection is a perceptual phenomenon that describes the visually induced subjective sensation of self-motion in the absence of physical motion. Previous research has discussed the potential involvement of top-down cognitive mechanisms on vection. Here, we quantified how cognitive manipulations such as contextual information (i.e., expectation) and plausibility (i.e., chair configuration) alter vection. We also explored how individual traits such as field dependence, depersonalization, anxiety, and social desirability might be related to vection. Fifty-one healthy adults were exposed to an optic flow stimulus that consisted of horizontally moving black-and-white bars presented on three adjacent monitors to generate circular vection. Participants were divided into three groups and given experimental instructions designed to induce either strong, weak, or no expectation with regard to the intensity of vection. In addition, the configuration of the chair (rotatable or fixed) was modified during the experiment. Vection onset time, duration, and intensity were recorded. Results showed that expectation altered vection intensity, but only when the chair was in the rotatable configuration. Positive correlations for vection measures with field dependence and depersonalization, but no sex-related effects were found. Our results show that vection can be altered by cognitive factors and that individual traits can affect the perception of vection, suggesting that vection is not a purely perceptual phenomenon, but can also be affected by top-down mechanisms.

Introduction: A new respiratory virus, SARS-CoV-2, has emerged and spread worldwide since late 2019. This study aims at analysing clinical presentation on admission and the determinants associated with admission in intensive care units (ICUs) in hospitalized COVID-19 patients., Patients and Methods: In this prospective hospital-based study, socio-demographic, clinical and biological characteristics, on admission, of adult COVID-19 hospitalized patients presenting from the community for their first admission were prospectively collected and analysed. Characteristics of patients hospitalized in medical ward to those admitted in ICU were compared using Mann-Whitney and Chi-square or Fisher exact test when appropriate. Univariate logistic regression was first used to identify variables on admission that were associated with the outcome i.e. admission to an ICU versus total hospital stay in a medical ward. Forward selection was then applied beginning with sex, age and temperature in the multivariable logistic regression model., Results: Of the 412 patients included, 325 were discharged and 87 died in hospital. Multivariable regression showed increasing odds of ICU hospitalization with temperature (OR, 1.56 [95% CI, 1.06-2.28] per degree Celsius increase), oxygen saturation <90% (OR, 12.45 [95% CI, 5.27-29.4]), abnormal lung auscultation on admission (OR, 3.58 [95% CI, 1.58-8.11]), elevated level of CRP (OR, 2.7 [95% CI, 1.29-5.66for CRP>100mg/L vs CRP<10mg/L). and monocytopenia (OR, 3.28 [95% CI, 1.4-7.68]) were also associated with increasing odds of ICU hospitalization. Older patients were less likely to be hospitalized in ICU (OR, 0.17 [95%CI, 0.05-0.51]., Conclusions: Age and delay between onset of symptoms and hospital admission were associated with the risk of hospitalisation in ICU. Surgical site infection (SSI) surveillance methods are not standardised and are often time-consuming. We compared an active method, based on orthopaedic department staff reporting suspected SSI, with a semi-automated method, based on computerised extraction of surgical revisions, after total hip and knee arthroplasty. Both methods allowed finding the same SSI cases. We found the same sensitivity but higher specificity with a straightforward time gain using the passive method. Motivational theory-based interventions are known to be effective for increasing physical activity (PA) in the general population but their effects in people with severe mental illness are poorly understood. Therefore, we conducted a meta-analysis on the effect of these interventions on PA and cardiometabolic risk factors. A systematic search of randomized controlled trials through 6 databases was carried out from inception to March 2019. Analyses were conducted using random-effect models. Weighted mean difference (WMD) were used as effect size when outcomes had the same units, otherwise Hedge's g was used. Fourteen articles including 2128 participants were identified. Motivational theory-based interventions were effective in increasing PA (g = 0.27, 95%CI[0.03; 0.51], p = .003), reducing weight (WMD = -1.87 kg, 95%CI[-2.98; -0.76], p = .001), body mass index (WMD = -0.82 kg/m 2 , 95%CI[-1.23; -0.41], p = .009), waist circumference (WMD = -1.91 cm, 95%CI[-3.63; -0.18], p = .03) and fasting glucose (g = -0.17, 95%CI[-0.34; -0.001], p = .04). Larger effect sizes were found in interventions based on only one theoretical model of motivation. In conclusion, interventions using motivational theories are effective to improve PA levels and the cardiometabolic health profile of people with severe mental illness. Purpose: To describe the burden, and characteristics, of influenza-like illness (ILI) associated with non-influenza respiratory viruses (NIRV)., Methods: We performed a prospective, multicenter, observational study of adults admitted with ILI during three influenza seasons (2012-2015). Patients were screened for picornavirus, respiratory syncytial virus (RSV), coronavirus, human metapneumovirus, adenovirus, bocavirus, parainfluenza virus, and influenza, by PCR on nasopharyngeal samples. We excluded patients coinfected with NIRV and influenza., Results: Among 1421 patients enrolled, influenza virus was detected in 535 (38%), and NIRV in 215 (15%), mostly picornavirus (n = 61), RSV (n = 53), coronavirus 229E (n = 48), and human metapneumovirus (n = 40). In-hospital mortality was 5% (NIRV), 4% (influenza), and 5% (no respiratory virus). As compared to influenza, NIRV were associated with age (median, 73 years vs. 68, P = 0.026), chronic respiratory diseases (53% vs. 45%, P = 0.034), cancer (14% vs. 9%, P = 0.029), and immunosuppressive drugs (21% vs. 14%, P = 0.028), and inversely associated with diabetes (18% vs. 25%, P = 0.038). On multivariable analysis, only chronic respiratory diseases (OR 1.5 [1.1-2.0], P = 0.008), and diabetes (OR 0.5 [0.4-0.8], P = 0.01) were associated with NIRV detection., Conclusions: NIRV are common in adults admitted with ILI during influenza seasons. Outcomes are similar in patients with NIRV, influenza, or no respiratory virus.

The perception of the body and its parts has traditionally been studied using the conscious body image. Here, we determine the implicit representation of the hand. Participants were sequentially shown two life-size images of either the dorsal or palmar surface of their hand. In one interval either the horizontal or vertical dimension of the image was varied using an adaptive staircase, while the other interval contained the full-size, undistorted image. Participants reported which image most closely matched their hand. The staircase honed in on the distorted image that was equally likely to be judged as matching their own hand as the accurate image. The implicit representation was taken as midway between these two images. The experiment was repeated with different hand orientations. Perceived width depended on the orientation, with differences found between the upright and right orientations. Interestingly, the perceived length of the dorsum and palm were different from each other-length of the dorsum was overestimated whereas palm length was perceived accurately. Perceived body size is a fundamental construct that reflects our knowledge of self and is important for all aspects of perception, yet how we perceive our bodies and how the body is represented in the brain is not yet fully understood. In order to understand how the brain perceives and represents the body, we need an objective method that is not vulnerable to affective or cognitive influences. Here, we achieve this by assessing the accuracy of full-body size perception using a novel psychophysical method that taps into the implicit body representation for determining perceived size. Participants were tested with life-size images of their body as seen from different viewpoints with the expectation that greater distortions would occur for unfamiliar views. The Body Shape Questionnaire was also administered. Using a two-alternative forced choice design, participants were sequentially shown two life-size images of their whole body dressed in a standardized tight-fitting outfit seen from the front, side, or back. In one image, the aspect ratio (with the horizontal or vertical dimension fixed) was varied using an adaptive staircase, while the other was undistorted. Participants reported which image most closely matched their own body size. The staircase honed in on the distorted image that was equally likely as the undistorted photo to be judged as matching their perception of themselves. From this, the perceived size of their internal body representation could be calculated. Underestimation of body width was found when the body was viewed from the front or back in both sexes. However, females, but not males, overestimated their width when the body was viewed from the side. Height was perceived accurately in all views. These findings reveal distortions in perceived size for healthy populations and show that both viewpoint and sex matter for the implicit body representation. Though the back view of one's body is rarely-if ever-seen, perceptual distortions were the same as for the front view. Visually induced motion sickness (VIMS) is a well-known sensation in virtual environments and simulators, typically characterized by a variety of symptoms such as pallor, sweating, dizziness, fatigue, and/or nausea. Numerous methods to reduce VIMS have been previously introduced; however, a reliable countermeasure is still missing. In the present study, the effect of airflow and seat vibration to alleviate VIMS was investigated. Eighty-two participants were randomly assigned to one of four groups (airflow, vibration, combined airflow and vibration, and control) and then exposed to a 15 min long video of a bicycle ride shot from first-person view. VIMS was measured using the Fast Motion Sickness Scale (FMS) and the Simulator Sickness Questionnaire (SSQ). Results showed that the exposure of airflow significantly reduced VIMS, whereas the presence of seat vibration, in contrast, did not have an impact on VIMS. Additionally, we found that females reported higher FMS scores than males, however, this sex difference was not found in the SSQ scores. Our findings demonstrate that airflow can be an effective and easy-to-apply technique to reduce VIMS in virtual environments and simulators, while vibration applied to the seat is not a successful method.

Self-motion information can be used to update spatial memory of location through an estimate of a change in position. Viewing optic flow alone can create Illusory self-motion or "vection." Early studies suggested that peripheral vision is more effective than central vision in evoking vection, but controlling for retinal area and perceived distance suggests that all retinal areas may be equally effective. However, the contributions of the far periphery, beyond 90°, have been largely neglected. Using a large-field Edgeless Graphics Geometry display (EGG, Christie, Canada, field of view ±112°) and systematically blocking central (±20° to ±90°) or peripheral (viewing through tunnels ±20° to ±40°) parts of the field, we compared the effectiveness of different retinal regions at evoking forwards linear vection. Fifteen participants indicated when they had reached the position of a previously presented target after visually simulating motion down a simulated corridor. The amount of simulated travel needed to match a given target distance was modelled with a leaky spatial integrator model to estimate gains (perceived/actual distance) and a spatial decay factor. When optic flow was presented only in the far periphery (beyond 90°) gains were significantly higher than for the same motion presented full field or in only the central field, resulting in accurate performance in the range of speeds associated with normal walking. The increased effectiveness of optic flow in the peripheral field alone compared to full-field motion is discussed in terms of emerging neurophysiological studies that suggest brain areas dedicated to processing information from the far peripheral field.

Perceptual body size distortions have traditionally been studied using subjective, qualitative measures that assess only one type of body representation-the conscious body image. Previous research on perceived body size has typically focused on measuring distortions of the entire body and has tended to overlook the face. Here, we present a novel psychophysical method for determining perceived body size that taps into implicit body representation. Using a two-alternative forced choice (2AFC), participants were sequentially shown two life-size images of their own face, viewed upright, upside down, or tilted 90°. In one interval, the width or length dimension was varied, while the other interval contained an undistorted image. Participants reported which image most closely matched their own face. An adaptive staircase adjusted the distorted image to hone in on the image that was equally likely to be judged as matching their perceived face as the accurate image. When viewed upright or upside down, face width was overestimated and length underestimated, whereas perception was accurate for the on-side views. These results provide the first psychophysically robust measurements of how accurately healthy participants perceive the size of their face, revealing distortions of the implicit body representation independent of the conscious body image.

Masking, in which one stimulus affects the detection of another, is a classic technique that has been used in visual, auditory, and tactile research, usually using stimuli that are close together to reveal local interactions. Masking effects have also been demonstrated in which a tactile stimulus alters the perception of a touch at a distant location. Such effects can provide insight into how components of the body's representations in the brain may be linked. Occasional reports have indicated that touches on one hand or forearm can affect tactile sensitivity at corresponding contralateral locations. To explore the matching of corresponding points across the body, we can measure the spatial tuning and effect of posture on contralateral masking. Careful controls are required to rule out direct effects of the remote stimulus, for example by mechanical transmission, and also attention effects in which thresholds may be altered by the participant's attention being drawn away from the stimulus of interest. The use of this technique is beneficial as a behavioural measure for exploring which parts of the body are functionally connected and whether the two sides of the body interact in a somatotopic representation. This manuscript describes a behavioural protocol that can be used for studying contralateral tactile masking.

Long-range tactile masking has been reported between mirror symmetric body locations. This suggests a general principle of contralateral inhibition between corresponding points on each side of the body that may serve to enhance distinguishing touches on the two halves of the body. Do such effects occur before or after posture is added to the body schema? Here, we address this question by exploring the effect of arm position on long-range tactile masking. The influence of arm position was investigated using different positions of both the test and masking arms. Tactile sensitivity was measured on one forearm, while vibrotactile-masking stimulation was applied to the opposite arm or to a control site on the shoulder. No difference was found in sensitivity when test arm position was varied. Physical contact between the arms significantly increased the effectiveness of a masking stimulus applied to the other arm. Long-range masking between the arms was strongest when the arms were held parallel to each other and was abolished if the position of either the test arm or the masking arm was moved from this position. Modulation of the effectiveness of masking by the position of both the test and masking arms suggests that these effects occur after posture information is added to the body's representation in the brain.

Incorporating the fact that the senses are embodied is necessary for an organism to interpret sensory information. Before a unified perception of the world can be formed, sensory signals must be processed with reference to body representation. The various attributes of the body such as shape, proportion, posture, and movement can be both derived from the various sensory systems and can affect perception of the world (including the body itself). In this review we examine the relationships between sensory and motor information, body representations, and perceptions of the world and the body. We provide several examples of how the body affects perception (including but not limited to body perception). First we show that body orientation effects visual distance perception and object orientation. Also, visual-auditory crossmodal-correspondences depend on the orientation of the body: audio "high" frequencies correspond to a visual "up" defined by both gravity and body coordinates. Next, we show that perceived locations of touch is affected by the orientation of the head and eyes on the body, suggesting a visual component to coding body locations. Additionally, the reference-frame used for coding touch locations seems to depend on whether gaze is static or moved relative to the body during the tactile task. The perceived attributes of the body such as body size, affect tactile perception even at the level of detection thresholds and two-point discrimination. Next, long-range tactile masking provides clues to the posture of the body in a canonical body schema. Finally, ownership of seen body parts depends on the orientation and perspective of the body part in view. Together, all of these findings demonstrate how sensory and motor information, body representations, and perceptions (of the body and the world) are interdependent.

To accurately interpret tactile information, the brain needs to have an accurate representation of the body to which to refer the sensations. Despite this, body representation has only recently been incorporated into the study of tactile perception. Here, we investigate whether distortions of body representation affect tactile sensations. We perceptually altered the length of the arm and the width of the waist using a tendon vibration illusion and measured spatial acuity and sensitivity. Surprisingly, we found reduction in both tactile acuity and sensitivity thresholds when the arm or waist was perceptually altered, which indicates a general disruption of low-level tactile processing. We postulate that the disruptive changes correspond to the preliminary stage as the body representation starts to change and may give new insights into sensory processing in people with long-term or sudden abnormal body representation such as are found in eating disorders or following amputation.

We assessed the perception and attitudes of university staff, including medical school and other science specialties, toward the 2009 A/H1N1 influenza pandemic and influenza vaccination program. A cross-sectional online survey was conducted among 4,529 university personnel on October 19-20, 2009. Seven hundred (15%) employees participated in the study. Only 18% were willing to be vaccinated, men more than women (29% versus 9%, P < 0.001), and professors/researchers more than administrative/technical staff (30% vs. 6%, P < 0.001). Intention to be vaccinated was insufficient. Additional efforts are needed to improve information dissemination among university staff. Medical university personnel should receive more information to increase vaccine coverage and protect them as well as patients.

Touches on one hand or forearm can affect tactile sensitivity at contralateral locations on the opposite side of the body. These interactions suggest an intimate connection between the two sides of the body. Here, we explore the effect of masking not across the body but through the body by measuring the effect of a masking stimulus on the back on the tactile sensitivity of the corresponding point on the front. Tactile sensitivity was measured on each side of the stomach, while vibrotactile masking stimulation was applied to one side of the front and to points on the back including the point directly behind the test point on the front. Results were compared to sensitivity, while vibrotactile stimulation was applied to a control site on the shoulder. A reduction in sensitivity of about .8 dB was found that required the masking stimulus to be within about 2 cm of the corresponding point on the back.

Masking effects have been demonstrated in which tactile sensitivity is affected when one touch is close to another on the body surface. Such effects are likely a result of local lateral inhibitory circuits that sharpen the spatial tuning of a given tactile receptor. Mutually inhibitory pathways have also been demonstrated between cortical tactile maps of the two halves of the body. Occasional reports have indicated that touches on one hand or forearm can affect tactile sensitivity at contralateral locations. Here, we measure the spatial tuning and effect of posture on this contralateral masking effect. Tactile sensitivity was measured on one forearm, while vibrotactile masking stimulation was applied to the opposite arm. Results were compared to sensitivity while vibrotactile stimulation was applied to a control site on the right shoulder. Sensitivity on the forearm was reduced by over 3 dB when the arms were touching and by 0.52 dB when they were held parallel. The masking effect depended on the position of the masking stimulus. Its effectiveness fell off by 1 STD when the stimulus was 29 % of arm length from the corresponding contralateral point. This long-range inhibitory effect in the tactile system suggests a surprisingly intimate relationship between the two sides of the body.

Simulation of a given stimulus situation should produce the same perception as the original. Rogers et al (2009 Perception 38 907-911) simulated Wheeler's (1982, PhD thesis, Rutgers University, NJ) motion parallax stimulus and obtained quite different perceptions. Wheeler's observers were unable to reliably report the correct direction of depth, whereas Rogers's were. With three experiments we explored the possible reasons for the discrepancy. Our results suggest that Rogers was able to see depth from the simulation partly due to his experience seeing depth with random dot surfaces.

The aim of this study was to estimate influenza vaccine effectiveness (IVE) against laboratory-confirmed influenza among hospitalized patients. A case-control investigation was based on the prospective surveillance of influenza-like illness (ILI) during five flu seasons. We compared influenza-positive cases and influenza-negative controls. Unadjusted overall IVE was 62% (95% confidence interval 24% to 81%). We found that IVE was lower during the 2004-05 flu season (11%; 95% CI -232% to 76%) when the vaccine and circulating viruses were mismatched.
