Background: Snapping scapula syndrome (SSS) can result in crepitus and painful scapulae during motion and may be treated with bursectomy and/or superomedial angle resection. The medial scapula corpus angle (MSCA) measures blade curvature on a transverse plane below the suprascapular fossa and may indicate SSS, yet a large overlapping range in MSCA exists between patients with and without SSS. This study quantified the effects of 3-dimensional scapula orientation in the imaging field, and the resulting variability in scapula type and MSCA. Methods: Computed tomography scans from 10 healthy controls (non-SSS) and 8 SSS patients were used to create 3-dimensional scapula models. The scapula type and MSCA were measured on a controlled reference imaging plane, and ones translated and rotated below the supraspinatus fossa to create 19 planes simulating variations due to scapulothoracic orientation. Planes translated and rotated above the reference plane also generated 13 modified MSCA planes to test areas modified during surgical resection. Statistical analyses compared the scapula type and MSCA between the reference and alternate planes within groups. Results: Scapula type commonly changed and the MSCA varied up to 104° within a subject depending on the imaging plane, regardless of location below or above the reference plane. Numerous statistical differences were detected in MSCA between the reference plane and those translated and rotated below that plane in both non-SSS and SSS groups. Planes translated above the reference plane showed consistent statistical differences in MSCA to the reference plane, but only in the SSS group. Discussion: Although scapula type and MSCA were previously shown to differentiate patients, the effect of viewing perspective was not considered. Differences in scapula orientation relative to the imaging plane dramatically varied the scapula type and MSCA, far exceeding differences between groups described previously. Herein, scapula type and MSCA often differed in planes translated above the reference plane, suggesting that scapular abnormalities contributing to SSS are largely at or close to the superomedial angle. Conclusion: The MSCA as defined previously likely lacks the sensitivity and specificity to reliably be used as a clinical diagnostic tool for SSS. The blade showed consistent differences when translated above the reference plane; however, it was still highly variable. Sensitivity and specificity of planes above the reference plane should be investigated further as they may provide reliable differentiation of non-SSS and SSS patients.

A 15-year-old boy presented with left-sided hip pain and imaging consistent with the diagnosis of femoroacetabular impingement. Following hip arthroscopy, which included an osteochondroplasty, labral repair, and capsular repair, the patient's anterior hip pain improved. However, his deep aching hip pain persisted until an ischial osteoid osteoma was identified and treated with radiofrequency ablation. At 3 years follow-up, the patient reports high satisfaction and minimal pain. We present this case to illustrate the importance of considering all potential causes of persistent hip pain following hip arthroscopy, including benign bone tumors which may be difficult to visualize on plain radiographs.

Purpose: To quantify the magnetic resonance arthrography (MRA) capsular morphologic findings associated with postarthroscopy hip instability. Methods: Among patients with clinically significant iatrogenic hip instability at a single center, patients with preindex and postindex surgery MRAs were identified. These MRAs were compared regarding effective intracapsular volume calculated by semi-automated 3-dimensional pixel intensity region segmentation, 2-dimensional anterior proximal intracapsular area in the femoral neck axial plane reconstruction, maximal anterior fluid pocket depth, capsule retraction distance, and capsular instability grade. Morphological measurements were conducted using Horos image processing software. Paired t-test, paired Wilcoxon signed rank test, and the McNemar test were used for identifying statistical significance. Results: In 42 patients, mean effective intracapsular volume was significantly greater in the postindex surgery MRAs (19.44 cm3 vs 17.26 cm3; P = .006). Proximal anterosuperior (12-3 o'clock) intracapsular area was also significantly greater after index surgery (2.84 cm2 vs 1.43 cm2; P < .001. Iatrogenic hip instability is increasingly recognized as a cause of persistent pain and disability after hip arthroscopy. Many authors currently advocate capsular repair to reduce postoperative instability. However, anatomic deficiencies in the anterosuperior capsule can prevent a functional capsular repair, particularly in the revision setting. Capsular reconstruction has been shown to restore biomechanical stabilization in cadaveric models and improve short-term patient outcomes in patients with primary hip arthroscopy failure. Arthroscopic hip capsular reconstruction is technically challenging, largely owing to complex suture management and difficulties with graft placement and sizing. This article describes the capsular reconstruction technique, detailing the technical aspects of anterosuperior capsular defect identification; capsular preparation; suture management; and dermal allograft sizing, preparation, and positioning.

Purpose: To establish a quantitative relationship between the Blackburne–Peel index and posterior tibial slope in both skeletally mature and skeletally immature individuals and to evaluate the rate at which variation in tibial slope influences changes in patellar height categorization as normal, patella alta, and patella baja. Methods: A consecutive series of lateral knee radiographs were retrospectively reviewed. Radiographs were excluded for rotation, inadequate visible proximal tibia length, and obstructive hardware/pathology. Modified tibial slopes of 0°, 5°, 10°, and 15° were projected anteriorly from the medial tibial plateau as described by Blackburne–Peel. The Blackburne–Peel index was determined at each modified tibial slope interval. Caton–Deschamps and Insall–Salvati indices also were measured for comparison. The rate of Blackburne–Peel index change with increase in posterior tibial slope was quantitatively analyzed. Results: Fifty skeletally mature and 50 skeletally immature radiographs were included. In the skeletally mature, Blackburne–Peel indices decreased on average by 0.037, 0.044, and 0.049 as posterior tibial slope increased from 0-5°, 5-10°, and 10-15°, respectively. In the skeletally immature, Blackburne–Peel indices decreased on average by 0.045, 0.053, and 0.059 as posterior tibial slope increased from 0-5°, 5-10°, and 10-15°, respectively. Overall, 29 individuals with 0° of tibial slope were categorized as patella alta by the Blackburne–Peel index, and only 16 (55%) remained categorized as patella alta after increasing their posterior tibial slope to 15°. Conclusions: This study quantitatively demonstrates the relationship between posterior tibial slope and the Blackburne–Peel index. As expected, as posterior tibial slope increases, the Blackburne–Peel index decreases. While the change in the Blackburne–Peel index per 5° change in tibial slope appears to be small, nearly half (45%) of individuals categorized as patella alta with 0° of tibial slope were categorized as normal when their posterior tibial slope was systematically increased from 0° to 15°. When evaluating patellar height, it is important to understand how tibial slope affects the Blackburne-Peel Index measurement. Clinical Relevance: As posterior tibial slope increases, the numerator of the Blackburne-Peel ratio decreases, and vice versa. This relationship can lead to incorrect assessment of patellar height. Objectively placing individuals into patella alta and baja categories may influence patient care and decision making.

Case. An adolescent male developed fat embolism syndrome 24 hours after sustaining a closed right tibial shaft fracture in a football game. The patient was treated with emergent external fixator application due to declining respiratory and mental status and experienced swift recovery after stabilization. He was treated with an intramedullary nail within 1 week of injury. Conclusion. Pediatric fat embolism syndrome is uncommon, and a high index of suspicion is required to facilitate appropriate orthopaedic involvement. External fixation can be performed emergently with minimal fracture manipulation. Rapid provisional fixation appears to have facilitated recovery in this example.

Purpose: To evaluate the effectiveness of 2 weeks of naproxen prophylaxis for heterotopic ossification (HO) after primary hip arthroscopy for femoroacetabular impingement (FAI). Methods: All consecutive hip arthroscopy cases by a single surgeon from January 1, 2015, to December 31, 2016, were retrospectively reviewed. Cases were included if they met the following criteria: (1) Primary hip arthroscopy, including femoral neck osteoplasty, for FAI; (2) naproxen prophylaxis for 2 weeks’ duration; and (3) radiographic follow-up at a minimum of 3 months postoperatively. Radiographic imaging for each patient was evaluated for the presence of HO, and independent radiology reports were used to confirm patients with HO. Maximal size of HO was measured and classified according to the Brooker criteria. Demographic and operative variables of the patients included in this study who received naproxen prophylaxis for 2 weeks were formally compared with a cohort of patients who received naproxen prophylaxis for 3 weeks in a previous randomized controlled trial. Results: A total of 185 patients who received naproxen prophylaxis for 2 weeks were included in the study, 5 of whom developed HO (3%). A total of 48 patients who received naproxen prophylaxis for 3 weeks were included for comparison, 2 of whom developed HO (4%). There was no significant difference in the rate of HO formation between 2 weeks and 3 weeks of naproxen prophylaxis (P = .597). All 7 cases of HO were classified as Brooker class 1. Conclusions: The observed rate of postoperative heterotopic ossification after 2 weeks of naproxen prophylaxis in this study was equivalent to that observed after 3 weeks’ prophylaxis in a previously published study. Two weeks of naproxen prophylaxis may be a sufficient treatment course for the prevention of heterotopic ossification after primary hip arthroscopy for FAI. Hip arthroscopy and femoral osteochondroplasty are commonly used in the treatment of femoroacetabular impingement (FAI). Determining the correct resection depth of the femoral head–neck cam lesion intraoperatively can be challenging. Both inadequate resection and over-resection may result in complications, underlying the importance of using a consistent and accurate technique when resecting and reshaping the proximal femur. Osseous resection to a depth of the subchondral cortical–cancellous bone margin in individuals with FAI has been shown to restore proximal femoral anatomy to within submillimeter differences when compared with control subjects without FAI. This bony boundary may be used as an intraoperative guide to consistently achieve appropriate resection depth. The sclerotic margin indicating the extent of the cam-type deformity can be evaluated with preoperative radiographs and recreated fluoroscopically, giving the surgeon a reliable intraoperative template. In addition, changes in clinical appearance during arthroscopy, particularly identification of the underlying trabecular structure at the cortical–cancellous border during resection, provides a visible, reliable intraoperative guide to resection depth.
