Despite recent advancements in antibiotics, hygienic measures, and peri-operative systemic antibiotics, post-operative infections in osteosynthesis remain prevalent and continue to be among the most common surgical complications, leading to delayed fracture healing, osteomyelitis, implant loosening, and loss of function. Osteosynthesis implants are routinely utilized in veterinary medicine and the current study investigates the microbial colonization of implants following osteosynthesis in small animals, along with its incidence and influencing factors. The results are analyzed in regard to correlations between infection, patient, disease progression, and radiographic images, as well as other factors that may promote infection. Seventy-one explants from sixty-five patients were examined and evaluated for microbial colonization. Factors like body weight and age, location and type of plate and additional injuries like lung lesions, the surgeon’s experience, or the number of people present during the surgical procedure seem to influence the development of an infection. Of the animals, 60% showed osteolytic changes and 73.3% of those with dysfunctional mobility had an implant infection. Microorganisms were detected in almost 50% of the explants, but a clinically relevant infection was only present in five patients (7.3%), suggesting that the presence of microorganisms on an implant does not necessarily lead to treatment complications.

Indications for implant removal after fracture healing are still under debate in both human and veterinary medicine. Although hardware removal is a common procedure, it should not be undertaken lightly. Intra and post-operative complications are common and a thorough evaluation of the risks and benefits should be performed. This review aimed to collect and summarise published data on the indications for implant removal in small animals, compare the collected data with human and equine medicine, and investigate the existence of guidelines for this purpose. There is no international consensual agreement for implant removal after fracture healing, neither in small animals nor in human orthopaedics. Decision-making processes are still controversial in some scenarios, thus clear evidence-based protocols for implant removal are needed.

A patellar luxation and concomitant cranial cruciate ligament rupture is a common pathology in dogs. Diagnosis is based on clinical evidence of a patellar luxation and stifle joint instability. However, diagnostic imaging is required to assess the number of skeletal deformities and signs of instability. Surgical options include both soft tissue and osseous techniques, although, in most cases, a combination of multiple procedures is necessary to correct the patellar luxation and restore the stifle joint stability. Complication rates are generally low, but can include reluxation and implant-associated complications. This article describes the patellar luxation and cranial cruciate ligament rupture signs in dogs, including the clinical presentation and diagnosis, and discusses current treatment options.

Brunnberg et al. establish a protocol that enables them to optically control translocation of the transporter associated with antigen processing (TAP), which plays a role in delivering proteasomal degradation products into the ER lumen. Their versatile approach provides insights into TAP function in the context of peptide-loading complex and stable peptide-MHC I complex trafficking in living cells, but has the potential to be applied to the investigation of other pathways.

For CNS lymphomas (CNSL), there is a high need for minimally invasive and easily obtainable diagnostic markers. Intrathecal IgM synthesis can easily be determined in routine CSF diagnostics. The aim of this study was to systematically investigate the diagnostic potential of intrathecal IgM synthesis in primary and secondary CNSL (PCNSL and SCNSL). In this retrospective study, patients with a biopsy‐proven diagnosis of PCNSL or SCNSL were compared with patients with other neurological diseases in whom CNSL was initially the primary radiological differential diagnosis based on MRI. Sensitivity and specificity of intrathecal IgM synthesis were calculated using receiver operating characteristic curves. Seventy patients with CNSL were included (49 PCNSL and 21 SCNSL) and compared to 70 control patients. The sensitivity and specificity for the diagnosis of CNSL were 49% and 87%, respectively, for the entire patient population and 66% and 91% after selection for cases with tumor access to the CSF system and isolated intrathecal IgM synthesis. In cases with MRI‐based radiological suspicion of CNSL, intrathecal IgM synthesis has good specificity but limited sensitivity. Because of its low‐threshold availability, analysis of intrathecal IgM synthesis has the potential to lead to higher diagnostic accuracy, especially in resource‐limited settings, and deserves further study. Antigen presentation via major histocompatibility complex class I (MHC-I) molecules is essential for surveillance by the adaptive immune system. Central to this process is the peptide-loading complex (PLC), which translocates peptides from the cytosol to the endoplasmic reticulum and catalyzes peptide loading and proofreading of peptide-MHC-I (pMHC-I) complexes. Despite its importance, the impact of individual PLC components on the presented pMHC-I complexes is still insufficiently understood. Here, we used stoichiometrically defined antibody-nanobody complexes and engineered soluble T cell receptors (sTCRs) to quantify different MHC-I allomorphs and defined pMHC-I complexes, respectively. Thereby, we uncovered distinct effects of individual PLC components on the pMHC-I surface pool. Knockouts of components of the PLC editing modules, namely tapasin, ERp57, or calreticulin, changed the MHC-I surface composition to a reduced proportion of HLA-A*02:01 presentation compensated by a higher ratio of HLA-B*40:01 molecules. Intriguingly, these knockouts not only increased the presentation of suboptimally loaded HLA-A*02:01 complexes but also elevated the presentation of high-affinity peptides overexpressed in the cytosol. Our findings suggest that the components of the PLC editing module serve a dual role, acting not only as peptide proofreaders but also as limiters for abundant peptides. Objectives: Clostridioides difficile infections (CDI) are common in autologous (auto-HSCT) or allogenic hematopoietic stem cell transplant (allo-HSCT) recipients. However, the impact of CDI on patient outcomes is controversial. We conducted this study to examine the impact of CDI on patient outcomes. Methods: We performed a retrospective single-center study, including 191 lymphoma patients receiving an auto-HSCT and 276 acute myeloid leukemia (AML) patients receiving an allo-HSCT. The primary endpoint was overall survival (OS). Secondary endpoints were causes of death and, for the allo-HSCT cohort, GvHD- and relapse-free survival (GRFS). Results: The prevalence of CDI was 17.6% in the AML allo-HSCT and 7.3% in the lymphoma auto-HSCT cohort. A higher prevalence of bloodstream infections, but no differences concerning OS or cause of death were found for patients with CDI in the auto-HSCT cohort. [AU] In the allo-HSCT cohort, OS and GRFS were similar between CDI and non-CDI patients. However, the leading cause of death was relapse among non-CDI patients, but it was infectious diseases in the CDI group with fewer deaths due to relapse. Conclusions: CDI was not associated with worse survival in patients receiving a hematopoietic stem cell transplantation, and there were even fewer relapse-related deaths in the AML allo-HSCT cohort.

Concomitant cranial cruciate ligament rupture (CCLR) is a common complication in small breed dogs with patellar luxation (PL) with an elusive pathogenesis. Surgical treatment is available and commonly includes remodelling osteotomies. While these modern techniques have shown good functional results, access is limited due to the high costs for owners and the need for special surgical equipment. The objectives of the study were to evaluate the frequency and risk factors for concomitant CCLR in small breed dogs with PL. To study the outcome and complications of the combination of the Wedge recession osteotomy (WR) and Tibial tuberosity transposition (TT) with a Fascia over-the-top (OT) or a Capsular and fascial imbrication technique (CFI) for the simultaneous treatment of PL and concomitant CCLR. A retrospective study is presented here. The signalment, body weight, luxation grade and direction, affected side, bilateral or unilateral PL, CCLR and meniscal status were analysed. The surgical treatment for PL and concomitant CCLR, outcome and complications were investigated. Of 233 small breed dogs with PL, 52 (22.31%) had a concomitant CCLR. Maltese dogs were more likely to have concomitant CCLR. The mean age for the dogs with PL only was 5.32 years, which was significantly younger than the mean age of dogs with PL and concomitant CCLR (7.39 years). Overweight dogs with PL were prone to develop concomitant CCLR. Surgical stabilisation with a combination of WR, TT and OT or CFI had excellent or good results in 86.63% of the cases, while 16.67% of the cases developed complications. In conclusion, PL and concomitant CCL ruptures can be managed successfully by performing a combination of WR, TT and OT or CFI. The outcomes and complication rates are comparable to remodelling osteotomies. Moreover, these techniques are less expensive and can be performed with standard surgical equipment. These findings should be beneficial for clinical diagnosis, client education and treatment.

This report describes a rare case of a presumed local cutaneous drug reaction in a 10-year old, male miniature short-haired dachshund after subcutaneous injection of meloxicam and maropitant. The dog was presented with cubital joint lameness and meloxicam was injected subcutaneously. Bloody diarrhoea and bloody emesis developed over the next three days. After a repeated subcutaneous application of meloxicam and maropitant into the left thoracic wall, the skin and subcutis at the injection site indurated and became painful over an area of approx. 8x8cm. The skin became locally necrotic and was demarcated within a few days. Histopathology of deep skin biopsies revealed severe, necrosuppurative dermatitis, panniculitis and vasculitis with marked edema and deep sequestrating pannicular necrosis. Perivascular eosinophils and vascular thrombosis were present in deeper areas of the panniculus and cutaneous muscle. Microbiological culturing failed to identify bacterial growth. Based on the treatment history, clinical changes, histopathology and the lack of laboratory data suggestive of relevant differential diagnoses, a local cutaneous adverse drug reaction (CADR) was favoured. Surgical removal of affected areas and an open wound management were successful and the dog fully recovered. This rare case of localized canine CADR illustrates the importance of a precise medical history, a strategie to exclude relevant differential diagnoses and the difficulties in ultimately confirming localized CADR in veterinary medicine.

Mast cells within the tumor microenvironment have controversial roles. Here, the authors show, using genetic mouse models, that in gastric cancer, mast cells at the periphery of the tumors are activated via cancer cell produced-IL33 and promote tumorigenesis by recruiting macrophages within the tumors.

To promote the development of an optimally functional total hip prosthesis for medium and large dog breeds, accurate measurements of the normal anatomy of the proximal femur and acetabular retroversion are essential. The aim of the current study was to obtain precise normal values of the femoral anteversion angle using computed tomography on cadavers of mature dogs with normal hip joints of both medium and large breeds. Based on the length of their femora 58 dogs were allocated either to group I: ≤195 mm or group II: >195 mm. In the study the femoral anteversion angle (FAA) was measured on each femur using multi-slice spiral computed tomography (CT). The data were processed as multi-planar and three-dimensional reconstructions using Advantage Workstation software. The CT measurements showed that the mean ± standard deviation (SD) FAA of group I was 31.34 ± 5.47° and in group II it was 31.02 ± 4.95°. There were no significant mean difference associations between the length of the femur and the femoral neck angle in either group (P > 0.05). The data suggest that a prosthesis FAA of 31 degrees would be suitable for a wide range of dog sizes.

Autologous hematopoietic stem cell transplantation (auto-HSCT) provides a potentially curative treatment option for relapsed and refractory lymphomas. Obesity displays an emerging epidemic risk factor for global mortality and is associated with an increased mortality in cancer patients. To date, the impact of obesity on the outcome of lymphoma patients undergoing auto-HSCT is understudied. We conducted a retrospective single-center study assessing 119 lymphoma patients who underwent auto-HSCT. Overall survival (OS) served as the primary endpoint whereas progression free survival (PFS), cumulative incidence of non-relapse related mortality (NRM) and cumulative incidence of relapse were analyzed as secondary endpoints. This study shows that students, teachers, and parents in Swedish schools ascribe differing meanings and significance to students' participation in school in relation to academic achievement. Students see participation as mainly related to social interaction and not academic achievement, whilst teachers view students' participation as more closely related to activity and academic performance. Despite these differences, teachers and students are in close agreement regarding activities of a social nature. Teachers' and parents' ratings of parents' involvement in school demonstrate a higher agreement, but also correlate negatively with the academic achievement of the student. This is likely because communication is more frequent with parents of underachieving students than students demonstrating high academic performance. The partly inconsistent results in previous research regarding the relation between participation and academic achievement can here be explained by the choice of raters, as this connection only exists in ratings carried out by teachers.

An 8-year-old male Jack Russell crossbreed dog was admitted to our hospital with dyspnoea and shock following a dog-bite injury on the ventral neck. Radiographs revealed subcutaneous emphysema and bilateral thyrohyoid bone fractures. Intra-operatively, rupture of both sternohyoid muscles, both hyoepiglotticus muscles, both thyrohyoid muscles and a partial cranial rupture of the superficial sphincter colli muscle were detected. Part of the epiglottis was detached from the thyroid cartilage. The patient’s severed muscles and torn epiglottis were reattached using a simple interrupted suture pattern. Hyoepiglotticus muscles could not be identified. The bilateral thyrohyoid bone fractures were repaired with intraosseous wire suture. A temporary tracheostomy tube and an esophageal feeding tube were placed postoperatively. The dog was discharged after 8 days, re-examined at 2 and 6 months and laryngeal and pharyngeal function were evaluated as normal. To the authors’ knowledge, this is the first report of a dog that presented with laryngeal trauma with hyoid bone fracture and acute dyspnea who underwent surgical treatment resulting in an acceptable outcome.

The aims of this study are first to compare the incidence of force on the first occasion of sexual intercourse reported by participants with disabilities to that of students without disabilities; second to determine whether there are significant differences in mental health, substance abuse, and school performance as reported by participants forced into their sexual debut as opposed to those who were not forced, analysed by gender; and finally to identify the significant variables that predict girls reporting force at sexual debut as opposed to girls not reporting force, as well as to identify similar variables within the male group. There were no data on sexual abuse prior to the first occasion of full sexual intercourse. Method: This cross-sectional study is based on 2 surveys: Life and Health--Young People 2005 and 2007. All 17/18-year-old adolescents in upper-secondary schools in a county in Sweden were asked the same questions both years. A total of 2,254 students completed the survey in 2005 and 2,641 in 2007. Results: The main finding is that force at sexual debut (intercourse) is more common among adolescents with a disability (4.0%) than those not reporting any disability (1.6%), and is most common among those reporting multiple disabilities (10.4%). This was found both for girls and boys, even if the rates for girls were several times higher. Other findings are that girls and boys reporting force at sexual debut (disability and non-disability groups taken together) reported different profiles. For girls, their country of origin and who they live with are significant. This background data is not significant for boys. Boys report a strong psychosomatic reaction. Conclusion: Culture-, functionality-, and gender-sensitive studies of adolescents' reactions to sexual abuse are needed to help determine relevant and effective interventions. (Contains 6 tables.)

This survey, "Life and Health--Young People 2005," included all 15/16-year-old adolescents in mainstream schools in the county of Orebro, Sweden. Just students with a slight/mild or moderate hearing loss were included. There were 56 (1.9%) "hard-of-hearing (HH) students with multiple disabilities," 93 (3.1%) students who were "just HH," 282 (9.7%) students with some "other disability than HH," and 2,488 (85.2%) students with "no disability." "HH with multiple disabilities" reported considerably higher scores for mental symptoms, substance use, and school problems than the "no disability" group. Those with "just HH" and those with "other disability than HH" had more mental symptoms and school problems than the "no disability" group but no significant differences in substance use. In conclusion, the combination of a hearing loss and some other disability strongly increases the risk for mental symptoms, school problems, and substance use. This group, thus, is an important target for preventive measures.

The aims of this study are to analyse the concept of mental health from the perspective of adolescent girls and boys and to describe what adolescent girls and boys regard as important determinants of mental health. Interviews with 48 children, 13 and 16 years old, in Sweden were held individually or in focus groups. The adolescents perceived mental health as an emotional experience, where positive as well as negative health is part of the concept. Family is the most important determinant for young people's mental health, closely followed by friends. Neither girls nor boys believed that there were any large differences in mental health between girls and boys. Age differences seemed to be more important than gender in the perception of mental health by children. (Contains 4 tables.)

Long term differences in survival after elective repair of abdominal aortic aneurysms (AAAs) between open surgical repair (OSR) and endovascular aneurysm repair (EVAR) are unclear, and hitherto artificial intelligence has not been used for this purpose. The aim was to compare the precision of survival estimates between the Kaplan–Meier (KM) method and the artificial intelligence derived method Neural Multi-Task Logistic Regression (N-MTLR), and to compare survival estimates as a function of patient age and time since surgery between OSR and EVAR using N-MTLR. All AAAs between 2003 and 2018 in Denmark were identified in the Danish vascular registry. Survival was estimated using the KM and N-MTLR methods, and prediction performance was estimated with the Brier score. 7 912 patients were included in the study, n = 6 569 (83%) men, median age 72 years (range 35 – 92), with a median follow-up time of 45.7 months (range 0 – 120). The two treatment cohorts, OSR n = 5 495 (69%), and EVAR n = 2 417 (31%), differed significantly in patient characteristics. The Brier score for KM increased from 0.044 to 0.244, and for N-MTLR from 0.044 to 0.206, from 90 days to 10 years. The N-MTLR method was more accurate than KM from 90 days to 10 years after surgery, p ≤.025. N-MTLR demonstrated significant increased probability for survival for OSR in patients aged 58 – 76 years at five years, and 65 – 73 at 10 years after surgery, and the opposite was found for the benefit of EVAR in patients aged 72 – 85 years at one year, 85 – 90 years at five years, and for 85 – 90 year olds at 10 years after surgery. N-MTLR outperforms KM for the entire post-operative follow-up time. This N-MTLR model has the potential to render more precise patient specific survival estimates and establish survival differences between subgroups of patients that KM is unable to detect, demonstrated here for different age groups. This study explores the process of identity construction for hard-of-hearing (HH) children in Sweden. Twenty-nine children aged 9–16 years who attended special classes for HH students were interviewed. During this longitudinal study, all classes were moved from an oral to a signing school environment. The findings support the position that a bilingual HH identity exists. HH children often construct their identity by widening their reference group to include not just HH but also those who are ‘almost the same’. They can have a sense of belonging either to deaf or hearing children, or both. In the development of identity HH children make distinctions between subgroups within their reference group. There were also children in crisis or with an unclear identity. This needs to be further explored to determine if the crisis is a productive part of identity construction or a problem requiring support. Gender construction also needs to be further explored.

The school playground is the main social arena for children attending classes for hard of hearing pupils, where they can meet and mix with other children. Their social interplay with other children is rather restricted during their spare time. For the hard of hearing children in this study, their school environment has shifted from one dominated by spoken language (at a mainstream compulsory school) to one dominated by the use of sign language (at a special school). Video-camera observations of the children playing during school breaks showed that, at the mainstream school, the hearing and hard of hearing children used the school playground in an unequal way. The hard of hearing children played at the periphery of the playground and the hearing children in the central interaction areas. In the special school the hard of hearing children played in the central interaction areas and often with deaf schoolmates. Thus, in the integrated environment the hard of hearing children were socially excluded and in the segregated environment they were socially included.
