Objective: To investigate the effects of dental anxiety on fluctuations in blood pressure (BP) and heart rate (HR) during tooth extraction in hypertensive patients under local anaesthesia, and how they are influenced by various confounding variables. Methods: This is a prospective repeated-measures cohort study involving 600 patients successively recruited from Peking University School and Hospital of Stomatology, Beijing, China. BP and HR were repeatedly measured at rest (T0), before anaesthesia (T1), during tooth extraction (T2) and after tooth extraction (T3). Anxiety status was measured prior to local anaesthesia using a modified dental anxiety scale (MDAS). Three groups were assigned: mild anxiety (Corah DAS score of 4 to 8), moderate anxiety (score of 9 to 12) and severe anxiety (score of 13 to 20). We used a generalised linear mixed model (GLMM) to analyse the effects of dental anxiety on fluctuations in BP and HR. Interaction analysis was used to further explore the correlationship between these interactive factors. Results: The mean anxiety scale score was 9.63 ± 2.88. Severe preoperative anxiety (score of 14 to 20) was associated with significantly increased HR during administration of anaesthesia. Patients with severe anxiety also displayed a significantly greater increase in HR during anaesthetic administration (P < 0.001). When analysing the joint effects of different anxiety statuses over time, blood pressure was significantly elevated in all patients with moderate and severe anxiety during tooth extraction at T2 (ß = 1.25, 95% CI 0.24 to 2.27). We also observed a significant decrease in HR in the moderate anxiety group at T3 (ß = -1.51, 95% CI -2.38 to -0.63) and a significant increase in HR in the severe anxiety group at T1, T2 and T3 (ß = 2.52, 95% CI 1.12 to 3.93; ß = 3.84, 95% CI 2.30 to 5.38; ß = 4.57, 95% CI 3.03 to 6.11, respectively). Conclusion: This study indicates that the effects of dental anxiety on BP and HR in middleaged and elderly patients with hypertension during local anaesthesia and tooth extraction were influenced by various confounding variables. Primary organic aerosols (POAs) are a major component of PM 2.5 in winter polluted air in the North China Plain (NCP), but our understanding of the atmospheric aging processes of POA particles and the resulting influences on their optical properties is limited. As part of the Atmospheric Pollution and Human Health in a Chinese Megacity (APHH-Beijing) program, we collected airborne particles at an urban site (Beijing) and an upwind rural site (Gucheng, Hebei province) in the NCP during 13–27 November 2016 for microscopic analyses. We confirmed that large numbers of light-absorbing spherical POA (i.e., tarball) and irregular POA particles with high viscosity were emitted from domestic coal and biomass burning at the rural site and were further transported to the urban site during regional wintertime hazes. During the heavily polluted period (PM 2.5 > 200 µ g m -3), more than 60 % of these burning-related POA particles were thickly coated with secondary inorganic aerosols (named as core–shell POA–SIA particles) through the aging process, suggesting that POA particles can provide surfaces for the heterogeneous reactions of SO 2 and NO x. As a result, during the heavily polluted period, their average particle-to-core diameter ratios at the rural and urban sites increased to 1.60 and 1.67, respectively. Interestingly, we found that the aging process did not change the morphology and sizes of POA cores, indicating that the burning-related POA particles are quite inert in the atmosphere and can be transported over long distances. Using Mie theory we estimated that the absorption capacity of these POA particles was enhanced by ∼ 1.39 times in the heavily polluted period at the rural and urban sites due to the "lensing effect" of secondary inorganic coatings. We highlight that the lensing effect on burning-related POA particles should be considered in radiative forcing models and authorities should continue to promote clean energy in rural areas to effectively reduce primary emissions. Purpose: To assess associations between the amount of physical activity and the prevalence of ocular diseases. Methods: The participants of the population‐based Beijing Eye Study underwent a detailed ophthalmological examination and an interview. Physical activity was assessed in a standardized questionnaire. Results: Out of 3468 study participants, information on their physical activity was available for 3031 (87.4%) individuals (age: 64.6 ± 9.7 years; range: 50–93 years). In multivariate analysis (regression coefficient r: 0.41), higher physical activity was associated with a lower prevalence of diabetic retinopathy [p = 0.009; standardized regression coefficient beta: −0.05; non‐standardized regression coefficient B: −15.7; 95% confidence interval (CI): −27.6, −3.90] after adjusting for younger age, rural region of habitation, lower level of education, lower blood concentrations of triglycerides and low‐density lipoproteins and higher blood concentrations of high‐density lipoproteins, higher systolic blood pressure, lower body mass index and lower depression score. Other major ocular diseases such as open‐angle glaucoma (p = 0.25), angle‐closure glaucoma (p = 0.59), nuclear cataract (p = 0.78), cortical cataract (p = 0.54), posterior subcapsular cataract (p = 0.96), retinal vein occlusions (p = 0.93) and central serous choroidopathy (p = 0.39) were not statistically associated with physical activity in that model. The association between higher physical activity and prevalence of age‐related macular degeneration (p = 0.04; β: 0.04; B: 4.87; 95% CI: 0.25, 9.50) was marginally significant. Conclusions: Higher physical activity and less sedentary lifestyle were associated with a lower prevalence of diabetic retinopathy, while the occurrence of other major ocular diseases such as any type of cataract and of glaucoma, retinal vein occlusions and central serous choroidopathy was statistically independent of physical activity or a more sedentary lifestyle. Purpose: To assess the 10-year incidence and progression of epiretinal membranes (ERMs). Methods: The population-based longitudinal Beijing Eye Study, which included 4439 subjects (age:40+years) in 2001, was repeated in 2011 with 2695 subjects participating (66.4% of the survivors). The study participants underwent a detailed ophthalmic examination, including retinal photography. Assessing fundus photograohs, ERMs were classified as cellophane macular reflex (CMR) without retinal folds, or as preretinal macular fibrosis (PMF) without or with retinal folds. Results: Fundus photographs were available for 2476 subjects with a mean age of 69.0±7.8 years (range:51–93 years) and mean axial length of 23.3±0.9 mm (range:19.92–26.33mm). The 10-year incidence of ERMs was 8.4% (208/2476 participants; 95% confidence interval (CI):7.4,9.5). ERMs developed bilaterally in 50 (24%) individuals and unilaterally in 158 (76%) persons. The incidence of PMFs with 2.5% (95% CI: 1.9, 3.1) was lower than the incidence of CMRs with 5.9% (95% CI: 5.0, 6.9). Higher 10-year incidence of ERMs was associated with older age (P<0.001; odds ratio (OR): 1.06; 95%CI:1.04,1.09), previous cataract surgery (P = 0.003;OR:3.32;95%CI:1.51,7.29) and presence of a complete posterior vitreous detachment (P = 0.02;OR:1.84;95%CI:1.12,3.02). In the age groups of <60 years, 60–69 years, 70–79 years and 80+years, incidence of ERMs was 3.1%, 10.0%, 14.4% and 10.9%, respectively, with no significant gender difference. Conclusions: In Chinese aged 40+ years, the 10-year incidence of ERMs (8.4%) increased with older age, previous cataract surgery and complete posterior vitreous detachment. Purpose To determine the prevalence and associations of central serous chorioretinopathy ( CSC) in elderly Chinese. Methods The population-based cross-sectional Beijing Eye Study 2011 included 3468 individuals (age: 64.6 ± 9.8 years; range: 50-93 years), who underwent enhanced depth imaging of spectral-domain optical coherence tomography ( SD- OCT). CSC was defined as serous detachment of the retina in the macular region without signs of haemorrhages or signs of polypoidal choroidal vasculopathy, exudative age-related macular degeneration or other retinal vascular disorders, both on fundus photographs and on optical coherence tomography ( OCT) images. Results Central serous chorioretinopathy was diagnosed in 10 eyes (prevalence rate: 0.15 ± 0.05%; 95% confidence interval ( CI):0.06%, 0.25%) of 10 subjects (prevalence rate: 0.31 ± 0.10%; 95% CI: 0.12%, 0.50%). In five subjects, CSC was located foveally, and in five subjects, CSC was located extrafoveally. All subjects affected by foveal CSC were men, and three of the five individuals with extrafoveal CSC were men. In univariate analysis, subjects with CSC were significantly younger than the remaining study participants, and foveal CSC showed a significant (p = 0.02) predilection for men. After adjusting for age and gender, individuals with foveal CSC (383 ± 112 μm versus 270 ± 47 μm; p = 0.02) and the whole group of subjects with CSC had a significantly thicker subfoveal choroid. In a parallel manner, eyes contralateral to eyes with foveal CSC showed a significantly thicker subfoveal choroid than the age-adjusted control group (413 ± 74 μm versus 270 ± 47 μm; p = 0.001). Conclusions In Chinese aged 50+ years, the prevalence of CSC was 0.14% per subject. Purpose: To assess whether macular Bruch´s membrane gets lengthened in axial myopia. Methods: Using the enhanced depth imaging mode of spectral-domain optical coherence tomography and examining a subgroup of participants of the population-based cross-sectional Beijing Eye Study, we measured the length of Bruch´s membrane (“MacBMLength”) from the fovea to the temporal edge of parapapillary gamma zone, and the distance between the fovea and the temporal optic disc border. Parapapillary gamma zone was defined as the parapapillary region without Bruch´s membrane. We additionally measured ocular biometric parameters and assessed non-ophthalmologic variables. Results: Measurements of MacBMLength were performed on 322 individuals. MacBMLength (mean: 3.99±0.33 mm; range: 3.17–4.93 mm) was not significantly associated with any systemic parameter or ocular biometric parameter. Gamma zone width (mean: 0.18±0.30mm; range: 0.00–2.61mm) was associated (multivariate analysis; correlation coefficient r:0.80) with longer axial length (P<0.001; standardized correlation coefficient beta: 0.60; non-standardized correlation coefficient B:0.11; 95%CI: 0.09,0.14) and with longer fovea-optic disc border distance (P<0.001; beta:0.28; B:0.19; 95%CI:0.14,0.25), but not with MacBMLength (P = 0.42). Fovea-temporal disc border distance (mean: 4.16±0.44mm; range: 3.17–5.86mm) was associated (overall correlation coefficient: 0.68) with longer axial length (P<0.001; beta: 0.36; B: 0.10; 95%CI: 0.06, 0.13), after adjusting for flatter anterior chamber depth (P = 0.003; beta:-0.14; B:-0.14; 95%CI: -0.23,-0.05) and wider parapapillary gamma zone (P<0.001; beta:0.42; B:0.62; 95%CI:0.44,0.81). Conclusions: In contrast to parapapillary gamma zone width and fovea-disc border distance, MacBMLength was not significantly associated with axial length. Axial elongation associated increase in fovea-disc distance may predominantly occur through development or elongation of parapapillary gamma zone, while macular Bruch´s membrane may mostly be independent of axial elongation. Purpose: To evaluate prevalence and associated factors for myopia in high school students in Beijing. Methods: Grade 10 and 11 high school students were randomly selected from nine randomly selected districts of Beijing. The students underwent non-cylcoplegic auto-refractometry and an interview. Results: Out of 4798 eligible students, 4677 (93.4%) students (mean age:16.9±0.7years;range:16–18 years) participated. Mean refractive error of right eyes and left eyes was −2.78±2.29 diopters and −2.59±2.50 diopters, respectively. Prevalence of myopia (defined as ≤ −1.00 diopters in the worse eye) was 80.7% (95% Confidence Interval (CI): 79.6–81.8%). Out of 3773 students with myopia, 1525 (40.4%) wore glasses daily. In multiple logistic regression analysis, a higher prevalence of myopia was associated with female sex (odds ratio (OR) = 1.31;95%CI:1.11–1.55), Han ethnicity (OR = 1.64;95%CI:1.28–2.11), attending key schools (OR = 1.48;95%CI:1.24,1.77), higher family income (OR = 1.37;95%CI:1.09–1.71), longer time spent for near work (OR = 1.43;95%CI:1.06–1.93), shorter near work distance (OR = 1.87;95%CI:1.55–2.26), lower frequency of active rest during studying (OR = 1.40;95%CI:1.16–1.70), and parental myopia (OR = 2.28;95%CI:1.80–2.87). The interaction between distance from near work and time spent for near work was statistically (P = 0.03) significant. In multiple logistic regression analysis, higher prevalence of high myopia (≤-6.0 diopters) was associated with studying in key schools (OR = 1.38;95%CI:1.05,1.81), lower frequency of active rest during studying (OR = 1.40;95%CI:1.09,1.79), and a higher number of myopic parents (OR = 2.66;95%CI:2.08,3.40). Conclusions: A prevalence of about 80% for myopia and a prevalence of about 10% for high myopia in students aged 16 to 18 years and attending classes of grade 10 and 11 in a Chinese metropolitan region is another example of the high prevalence of moderate and high myopia in metropolitan areas of China. With this young myopic generation getting older, myopia as cause for visual impairment and blindness may further increase in importance. Objective: To assess the cumulative 10-year incidence of diabetic retinopathy (DR) and its associated factors in a population living in Greater Beijing. Methods: The population-based longitudinal Beijing Eye Study, which included 4439 subjects (age in 2001: 40+years) in 2001, was repeated in 2011 with 2695 subjects participating (66.4% of the survivors). The study participants underwent a detailed ophthalmic examination. Fundus photographs were examined for the new development of DR. Results: After excluding individuals with DR at baseline (n = 87) or no sufficient fundus photographs in 2011 (n = 6), the study included 2602 subjects with a mean age of 64.6±9.7 years (median: 64.0 years; range: 50 to 93 years). In the 10-year period, 109 subjects (39 men) developed new DR with an incidence of 4.2% (95% confidence interval (CI): 3.45,5.03). In multiple logistic regression analysis, incident DR was significantly associated with higher HbA1c value (P<0.001; Odds Ratio (OR): 1.73; 95% Confidence Interval (CI): 1.35,2.21), longer duration of diabetes mellitus (P<0.001; OR: 1.16; 95% CI: 1.10,1.22), higher serum concentration of creatinine (P = 0.02; OR: 1.01; 95% CI: 1.002,1.022), lower educational level (P = 0.049; OR: 0.74; 95% CI: 0.55,0.99), higher estimated cerebrospinal fluid pressure (P = 0.038; OR: 1.10; 95% CI: 1.01,1.22), and shorter axial length (P<0.001; OR: 0.48; 95% CI: 0.33,0.71). Conclusions: The cumulative 10-year incidence (mean: 4.2%) of DR in a North Chinese population was significantly associated with a higher HbA1c value, longer known duration of diabetes mellitus, higher estimated CSFP and shorter axial length (P<0.001). Shorter axial length (or hyperopia) and, potentially, higher CSFP may be additional risk factors to be taken into account when counseling and treating patients with diabetes mellitus. Purpose: To examine subfoveal choroidal thickness (SFCT) in eyes with glaucoma, using enhanced depth imaging spectral domain optical coherence tomography. Methods: The population-based Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years (range: 50–93 years). A detailed ophthalmic examination was performed including spectral-domain optical coherence tomography (SD-OCT) with enhanced depth imaging for measurement of SFCT, and assessment of fundus photographs for presence of glaucoma. In addition, the group of patients with chronic angle-closure glaucoma (ACG) from the Beijing Eye Study (n = 37) was merged with a group of patients with chronic ACG from the Tongren hospital (n = 52). Results: Assessments of SFCT and glaucoma were available for 3232 (93.2%) subjects. After adjusting for age, axial length, gender, anterior chamber and lens thickness, SFCT was not significantly associated with presence of glaucoma (P = 0.08; regression coefficient B:−15.7). As a corollary, in logistic regression analysis with adjustment for age, axial length and intraocular pressure, presence of glaucoma was not significantly associated with SFCT (P = 0.20). If only open-angle glaucoma was considered, multivariate analysis revealed no significant association between SFCT and presence of open-angle glaucoma (P = 0.44). As a corollary, in logistic regression analysis, open-angle glaucoma was not significantly associated with SFCT (P = 0.91). In a similar manner if only ACG was taken into account, SFCT was not significantly associated with the presence of ACG (P = 0.27) in multivariate analysis. As a corollary in binary regression analysis, presence of ACG was not significantly associated with SFCT (P = 0.27). Conclusions: In multivariate analysis with adjustment for age, axial length, gender, anterior chamber and lens thickness, neither OAG nor ACG was associated with an abnormal SFCT. Purpose: To examine a potential association between intraocular pressure (IOP) and cerebrospinal fluid pressure (CSFP) in a population-based setting. Methods: The population-based Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years (range: 50–93 years). A detailed ophthalmic examination was performed. Based on a previous study with lumbar cerebrospinal fluid pressure (CSFP) measurements, CSFP was calculated as CSFP [mm Hg] = 0.44×Body Mass Index [kg/m2]+0.16×Diastolic Blood Pressure [mm Hg]–0.18×Age [Years]. Results: In multivariate analysis, IOP was associated with higher estimated CSFP (P<0.001; standardized correlation coefficient beta: 0.27; regression coefficient B: 0.20; 95% confidence interval (CI): 0.16, 0.24), after adjusting for thinner central corneal thickness (P<0.001; beta: 0.45; B: 0.04;95%CI: 0.04,0.04), smaller corneal curvature radius (P<0.001; beta:−0.11; B:−1.13;95%CI:−1.61,−0.64), shallower anterior chamber depth (P = 0.01; beta:−0.05; B:−0.33;95%CI:−0.59,−0.08) and longer axial length (P = 0.002; beta: 0.08; B: 0.20;95%CI: 0.08,0.32)), and after adjusting for the systemic parameters of higher pulse rate (P<0.001; beta: 0.08; B: 0.02;95%CI: 0.01,0.03), higher prevalence of arterial hypertension (P = 0.002; beta: 0.06; B: 0.32;95%CI: 0.12,0.53)), frequency of drinking alcohol (P = 0.02; beta: 0.04; B: 0.09;95%CI: 0.01,0.17), higher blood concentration of triglycerides (P = 0.001; beta: 0.06; B: 0.06;95%CI: 0.02,0.10) and cholesterol (P = 0.049; beta: 0.04; B: 0.08;95%CI: 0.00,0.17), and body mass index (P<0.001; beta:−0.13; B:−0.09;95%CI:−0.13,−0.06). In a parallel manner, estimated CSFP (mean: 10.8±3.7 mm Hg) was significantly associated with higher IOP (P<0.001; beta: 0.13; B: 0.18;95%CI: 0.13,0.23) after adjusting for rural region of habitation (P<0.001; beta:−0.37; B:−2.78;95%CI:−3.07,−2.48), higher systolic blood pressure (P<0.001; beta: 0.34; B: 0.06;95%CI: 0.05,0.07), higher pulse rate (P = 0.003; beta: 0.05; B: 0.02;95%CI: 0.01,0.03), taller body height (P<0.001; beta: 0.11; B: 0.05;95%CI: 0.03,0.07), higher blood concentration of cholesterol (P = 0.003; beta: 0.05; B: 0.17;95%CI: 0.06,0.28) and higher level of education (P = 0.003; beta: 0.09; B: 0.30;95%CI: 0.16,0.45). Conclusions: IOP was positively associated with estimated CSFP after adjusting for other ocular and systemic parameters. As a corollary, higher estimated CSFP was significantly associated with higher IOP in multivariate analysis. Purpose: To examine characteristics of ocular hypertensive subjects and potential associations with estimated cerebrospinal fluid pressure (estCSFP). Methods: The population-based Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years. Ocular hypertension was defined as intraocular pressure (IOP) >21 mmHg, normal optic nerve head appearance and normal retinal nerve fiber layer thickness. IOP was corrected for its dependence on central corneal thickness (CCT) and corneal curvature radius. Estimated CSFP was calculated as CSFP [mmHg] = 0.44×Body Mass Index [kg/m2]+0.16×Diastolic Blood Pressure [mmHg]−0.18×Age [Years]−1.91. Estimated trans-lamina cribrosa pressure difference (estTLCPD) was IOP–estCSFP. Results: EstCSFP (10.5±3.6 mmHg versus 9.0±3.7 mmHg; P = 0.003) and estTLCPD (12.0±4.4 mmHg versus 5.4±3.8 mmHg; P<0.001) were higher in the ocular hypertensive group than in the normotensive group. In binary regression analysis, ocular hypertension was associated with increased estCSFP (P = 0.03; odds ratio (OR): 1.08; 95% confidence interval (CI): 1.01, 1.17) after adjusting for prevalence of arterial hypertension (P = 0.07; OR: 1.79; 95%CI: 0.96, 3.34), retinal nerve fiber layer thickness (P = 0.03; OR: 0.97; 95%CI: 0.95, 0.997) and blood glucose concentration (P = 0.006; OR: 1.17; 95%CI: 1.04, 1.30). Conclusions: Ocular hypertensive subjects (with IOP correction for CCT and corneal curvature) as compared to ocular normotensive subjects had a significantly higher estCSFP in univariate analysis and in multivariate analysis. Purpose: The cerebrospinal fluid pressure (CSFP) is a major determinant of central retinal vein pressure and thus of retinal capillary pressure. We tested the hypothesis whether prevalence and severity of diabetic retinopathy are associated with CSFP. Methods: The population-based Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years. A detailed ophthalmic examination was performed including fundus photography for the assessment of diabetic retinopathy according. Conclusions: Higher prevalence and severity of diabetic retinopathy were associated with higher estimated CSFP after adjusting for systemic parameters. Purpose: To examine potential associations between body height, cerebrospinal fluid pressure (CSFP), trans-lamina cribrosa pressure difference (TLCPD) and prevalence of open-angle glaucoma (OAG) in a population-based setting. Methods: The population-based Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years (range:50–93 years). A detailed ophthalmic examination was performed. Based on a previous study with lumbar cerebrospinal fluid pressure (CSFP) measurements, CSFP was calculated as CSFP[mmHg] = 0.44×Body Mass Index[kg/m2]+0.16×Diastolic Blood Pressure[mmHg]-0.18×Age[Years]-1.91 Results: Data of IOP and CSFP were available for 3353 (96.7%) subjects. Taller body height was associated with higher CSFP (P<0.001; standardized correlation coefficient beta:0.13; regression coefficient B:0.29; 95% confidence interval (CI):0.25,0.33) after adjusting for male gender, urban region of habitation, higher educational level, and pulse rate. If TLCPD instead of CSFP was added, taller body height was associated with lower TLCPD (P<0.001;beta:−0.10;B:−0.20;95%CI:−0.25,−0.15). Correspondingly, higher CSFP was associated with taller body height (P = 0.003;beta:0.02;B:0.01;95%CI:0.00,0.02), after adjusting for age, gender, body mass index, pulse, systolic blood pressure, and blood concentration of cholesterol. If IOP was added to the model, higher CSFP was associated with higher IOP (P<0.001;beta:0.02;B:0.02;95%CI:0.01,0.03). TLCPD was associated with lower body height (P = 0.003;beta:−0.04;B −0.02,95%CI:−0.04,−0.01) after adjusting for age, body mass index, systolic blood pressure, pulse, blood concentrations of triglycerides, axial length, central corneal thickness, corneal curvature radius, and anterior chamber depth. Adding the prevalence of OAG to the multivariate analysis revealed, that taller body height was associated with a lower OAG prevalence (P = 0.03;beta:−0.03;B:−1.20;95%CI:−2.28,−0.12) after adjusting for educational level and gender. Conclusions: Taller body height was associated with higher CSFP and lower TLCPD (and vice versa), after adjusting for systemic and ocular parameters. Purpose: To examine a potential association between longitudinal changes in intraocular pressure (IOP), arterial blood pressure and body mass index (BMI) in a population-based setting. Methods: The longitudinal population-based Beijing Eye Study included 2355 subjects with an age of 45+ years who were examined in 2006 and in 2011. The participants underwent a detailed ophthalmic examination including tonometry and measurement of arterial blood pressure and BMI. Results: Data on IOP, arterial blood pressure and BMI measured in 2006 and in 2011 were available for 2257 (95.8%) subjects with a mean age of 59.5±9.7 years. The mean change in IOP was −1.25±2.26 mm Hg, mean change in mean blood pressure −7.4±12.1 mmHg, and mean change in BMI was 0.01±2.04 kg/m2. In multivariate analysis, the 5-year change in IOP was significantly associated with a higher change in mean blood pressure (P<0.001; standardized regression coefficient Beta:0.11; regression coefficient B:0.02; 95% confidence interval (CI):0.01,0.03) after adjusting for younger age (P<0.001;Beta:−0.18;B:−0.04;95% CI:−0.05,−0.03), shorter body stature (P = 0.002;Beta:−0.06;B:−0.06;95% CI:−0.03,−0.01), thicker central corneal thickness (P<0.001;Beta:0.19;B:0.02;95% CI:0.01,0.02), deeper anterior chamber depth (P = 0.01;Beta:0.05;B:0.33;95% CI:0.07,0.60), and lower intraocular pressure at baseline (P<0.001;Beta:−0.56;B:−0.42;95% CI:−0.45,−0.39). If the analysis included only longitudinal parameters, the change in IOP was significantly associated with a higher change in mean arterial blood pressure (P<0.001;Beta:0.10;B:0.02;95% CI:0.01,0.03) and a higher change in body mass index (P<0.04;Beta:0.04;B:0.04;95% CI:0.01,0.09). Conclusions: In the 5-year follow-up of our population-based sample, a change in IOP was associated with a corresponding change in arterial blood pressure and with a corresponding change in body mass index. These longitudinal data support the notion of a physiological relationship between arterial blood pressure, intraocular pressure and body mass index. Objective: To assess the prevalence of pseudoexfoliation syndrome (PEX) and its associations in a population-based setting. Design: Population-based, cross-sectional cohort study. Participants: Of 4403 eligible subjects with an age of ≥50 years, 3468 individuals (78.8%) participated in the Beijing Eye Study 2011 (mean age, 64.6±9.8 years; range, 50–93 years). Methods: All study participants underwent a detailed ophthalmologic examination. After medical pupil dilation, PEX was assessed by an experienced ophthalmologist using slit-lamp-based biomicroscopy. Main Outcome Measures: Prevalence and associations of PEX. Results: Slit-lamp examination results were available for 3022 study participants (87.1%). Definite pseudoexfoliation was observed in 72 of the 3022 subjects, with a prevalence of 2.38% (95% confidence interval [CI], 1.84–2.93). Suspected PEX was detected in 104 of the subjects (3.44%; 95% CI, 2.8–4.1). The overall prevalence of PEX (definite and suspected) was 176 of 3022 or 5.82% (95% CI, 4.99–6.66). In 80 subjects (45.5%), PEX was detected in both eyes, whereas it was detected only in the right eye in 42 subjects (23.9%) and only in the left eye in 54 (30.7%). The prevalence of PEX increased from 1.1% in among those 50 to 54 years old, to 3.5%, 5.7%, and 11.8% among those 60 to 64 years, 70 to 74 years, and ≥80 years, respectively. In multivariate analysis, presence of PEX was significantly associated with older age (P<0.001; odds ratio [OR], 1.08; 95% CI, 1.04–1.10), shorter axial length (P = 0.03; OR, 0.82; 95% CI, 0.68,0.98), and shallower anterior chamber (P = 0.03; OR, 0.59; 95% CI, 0.36–0.95). We found that PEX was not associated (all P>0.05) with sex, diabetes mellitus, blood pressure, psychological depression, smoking, dyslipidemia, body mass index, central corneal thickness, corneal diameter, optic nerve head measurements, choroidal thickness, retinal vessel diameters, early age-related macular degeneration, or retinal vein occlusion. Conclusions: In a North Chinese population aged ≥50 years, the prevalence of definite PEX was 2.38% (95% CI, 1.84–2.93), suspect PEX was 3.4% (95% CI, 2.8–4.1) and overall PEX was 5.82% (95% CI, 4.99–6.66). We found PEX to be associated with older age, shorter axial length, and shallower anterior chamber. The relationship between PEX and glaucomatous optic neuropathy remained inconclusive among our population. Objective: To assess the prevalence of localized retinal nerve fiber layer defects (LRNFLD) and associated factors in adult Chinese. Methods: The population-based Beijing Eye Study 2011 included 3468 individuals (mean age: 64.6±9.8 years (range: 50–93 years)). The study participants underwent a detailed ophthalmological examination including spectral-domain optical coherence tomography (SpectralisR-OCT) assisted measurement of the RNFL. A LRNFLD was defined as a sector in which the RNFL contour line dipped into the red zone for a length of <180°. Results: Readable OCT images were available for 3242 (93.5%) subjects. LRNFLDs were detected in 640 eyes (9.9±0.4%) of 479 subjects (14.8±0.6%). In the age groups of 50–59 years, 60–69 years, 70–79 years, and 80+ years, the prevalence of LRNFLD per person increased from 9.9±0.9%, 11.6±1.0% and 20.6±1.4% to 33.0±3.2%, respectively. In multivariate analysis, prevalence of LRNFLDs was significantly associated with older age (P = 0.001; Odds Ratio (OR): 1.03; 95% Confidence Interval (CI): 1.01,1.05), myopic refractive error (P<0.001;OR:0.79;95%CI:0.74,0.85), larger beta zone of parapapillary atrophy (P<0.001; OR:1.34;95%CI:1.20,1.50), presence of glaucomatous optic neuropathy (P<0.001;OR:7.02;95%CI:3.87,12.7), presence of non-glaucomatous optic nerve damage (P = 0.001;OR:43.3;95%CI:8.24,227.1), and presence of diabetic retinopathy (P = 0.003;OR:2.79;95%CI:1.43,5.44). Conclusions: OCT-defined LRNFLDs were present in a prevalence of 14.8±0.6% in a population-based study sample of subjects aged 50+ years. Prevalence of LRNFLDs increased with higher age, myopic refractive error, and larger parapapillary beta zone. Purpose: To examine the retinal nerve fiber layer (RNFL) ophthalmoscopically, to search for localized RNFL defects, and to assess factors associated with RNFL visibility in a population-based setting. Methods: The population-based cross-sectional Beijing Eye Study 2006 included 3251 subjects. Using color fundus photographs, RNFL visibility was assessed in grades from 0 to 8 in 8 fundus sectors. Localized RNFL defects were defined as wedge-shaped defects running towards the optic disc. Results: After exclusion of subjects with optic media opacities, 2602 subjects (mean age:58.1±9.0 years) were included. RNFL visibility score was highest (P<0.001) in the temporal inferior region, followed by the temporal superior region, nasal superior region, and nasal inferior region. In multivariate analysis, prevalence of localized RNFL defects was associated with higher blood pressure (P<0.001; odds ratio (OR):1.07;95%CI:1.03,1.10), higher concentration of low-density lipoproteins (P = 0.01;OR:1.42;95%CI:1.08,1.85), higher prevalence of glaucomatous optic neuropathy (P<0.001;OR:46.8;95%CI:19.4,113) and diabetic retinopathy (P = 0.002;OR:3.20;95%CI:1.53,6.67), and lower total RNFL visibility (P<0.001;OR:0.92;95%CI:0.88,0.96). Conclusions: In Chinese aged 45+ years, a decreased RNFL visibility was associated with older age, male gender, dyslipidemia, hyperglycemia, myopia, larger optic disc, and glaucomatous or non-glaucomatous optic neuropathy. Localized RNFL defects (prevalence:3.7±0.45%) were correlated mainly with higher blood pressure, higher concentration of low-density lipoproteins, glaucomatous optic neuropathy and diabetic retinopathy. Purpose: To study subfoveal choroidal thickness (SFCT) in adult Chinese subjects and its correlation with ocular biometric parameters, refractive error, and age. Design: Population-based longitudinal study. Participants: The population-based Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years (range, 50–93 years). Methods: A detailed ophthalmic examination was performed, including spectral-domain optical coherence tomography (SD-OCT) with enhanced depth imaging for measurement of SFCT. Main Outcome Measures: Subfoveal choroidal thickness. Results: The SFCT measurements were available for 3233 subjects (93.2%). Mean SFCT was 253.8±107.4 μm (range, 8–854 μm). In multivariate analysis, SFCT increased with younger age (P<0.001; correlation coefficient r = 4.12; beta coefficient = 0.37), shorter axial length (P<0.001; r = 44.7; beta coefficient = 0.46), male gender (P<0.001; r = 28.5; beta coefficient = −0.13), deeper anterior chamber depth (P<0.001; r = 39.3; beta coefficient = 0.13), thicker lens (P<0.001; r = 26.8; beta coefficient = 0.08), flatter cornea (P<0.001; r = 46.0; beta coefficient = 0.11), and better best-corrected visual acuity (BCVA) (logarithm of minimal angle of resolution; P = 0.001; r = 48.4; beta coefficient = 0.06). In multivariate analysis, SFCT was not significantly associated with blood pressure, ocular perfusion pressure, intraocular pressure, cigarette smoking, alcohol consumption, serum concentrations of lipids and glucose, diabetes mellitus, and arterial hypertension. In the myopic refractive error range of more than −1 diopter (D), SFCT decreased by 15 μm (95% confidence interval [CI], 11.9–18.5) for every increase in myopic refractive error of 1 D, or by 32 μm (95% CI, 37.1–26.0) for every increase in axial length of 1 mm. For each year increase in age, the SFCT decreased by 4.1 μm (95% CI, 4.6–3.7) (multivariate analysis). Conclusions: Subfoveal choroidal thickness with a mean of 254±107 μm in elderly subjects with a mean age of 65 years decreased with age (4 μm per year of age) and myopia (15 μm per diopter [D] of myopia). It was also associated with male gender and the ocular biometric parameters of a deeper anterior chamber and thicker lens. The association between SFCT and BCVA indicates a functional aspect of SFCT. Purpose: To examine the incidence of age-related macular degeneration (AMD) and its associated factors in an adult Chinese population. Design: Population-based study. Participants: The Beijing Eye Study, which included 4439 subjects (age ≥40 years) in 2001, was repeated in 2006 with 3251 (73.2%) subjects participating. Methods: Fundus photographs were graded using the International Age-related Maculopathy Epidemiological Study Group grading system. Main Outcome Measures: Incidence of AMD. Results: Gradable slides were available on 3049 (93.9%) subjects who participated in the survey of 2001 and again in 2006. The incidence of early, late, and neovascular AMD per eye was 2.6% (95% confidence interval [CI], 2.2–3.0), 0.1% (95% CI, 0.00–0.2), and 0.1% (95% CI, 0.00–0.2), respectively. The incidence of early, late, and neovascular AMD per person was 4.2±0.4% (95% CI, 3.5–5.0), 0.1±0.1% (95% CI, 0.0–0.2), and 0.1±0.1% (95% CI, 0.0–0.2), respectively. By multivariate analysis, incident early AMD was associated significantly with greater age at baseline (P = 0.01; odds ratio [OR], 1.03; 95% CI, 1.01–1.06), smaller optic disc size (P = 0.007; OR, 0.50; 95% CI, 0.30–0.83), smaller scleral spur distance (P = 0.04; OR, 0.59; 95% CI, 0.36–0.98), and hyperopic refractive error (P = 0.057; OR, 1.15; 95% CI, 1.00–1.33), with the latter being significant only marginally. It was not associated with the systemic parameters of gender, body height, body mass index, region of habitation, level of education, profession, smoking, arterial blood pressure, diabetes mellitus, fasting blood concentrations of glucose, triglycerides, high-density or low-density lipoproteins; or the ocular parameters of intraocular pressure, retinal arterial and vein diameters, retinal microvascular abnormalities, amount of nuclear cataract, cortical cataract or subcapsular cataract, pseudophakia, glaucoma, nonglaucomatous optic neuropathy, retinal vein occlusions, size of the beta zone of parapapillary atrophy, or progression of the zone of atrophy during the follow-up from 2001 to 2006. Conclusions: Hyperopia, short interscleral spur distance, and small optic disc size were, beside older age, the main factors associated with incident early AMD. This may point to a small globe size, potentially in relation to a firmly attached vitreous, playing a role in early incident AMD. Purpose: The Beijing Eye Public Health Care Project was designed to screen all elderly subjects (age 55–85 years) of the rural region of Greater Beijing. It was developed as a preparatory step for a telemedicine-based public health care system in ophthalmology in China. Design: Population-based public health care project. Participants: Elderly subjects (age 55–85 years) of the rural region of Greater Beijing. Methods: Project participants were visited, interviewed, and examined by 2500 high school graduates trained as ophthalmic technicians. If visual acuity was <0.30, subjects were referred to primary health care centers where ocular photographs were taken. Using telemedicine, the photographs were transmitted to a reading center and causes for visual impairment were diagnosed. Main Outcome Measures: Practicability of a mass screening system in ophthalmology; prevalence of visual impairment and causes for it. Results: Out of 692 323 eligible inhabitants, 562 788 (81.3%) subjects participated. Visual impairment in ≥1 eye was detected in 54 155 (9.62%) subjects, and among them, 30 164 (5.36%) subjects had bilateral visual impairment. Ocular fundus photographs were taken for 37 281 subjects. Cause for visual impairment was cataract in 19 163 (3.41%) of all screened subjects, glaucoma in 1606 (0.29%) subjects, diabetic retinopathy in 905 (0.16%) subjects, other macular diseases in 2700 (0.48%) subjects, pterygium in 1381 (0.25%) subjects, and corneal leukoma in 283 (0.05%) subjects. For 5853 (1.04%) subjects, a diagnosis of premature or mature cataract was made showing the urgent need of cataract surgery. After cataract surgery, visual acuity was ≥0.30 in 1464 (91.7%) of 1596 postoperatively reexamined subjects. Conclusions: Using a telemedicine approach, the Beijing Eye Public Health Care Project developed, applied and tested an infrastructure for ophthalmic mass screening of >500 000 elderly inhabitants with a response rate of >80%. Beside cataract, retinal diseases including diabetic retinopathy and glaucoma were major causes for visual impairment. Purpose: To estimate the 5-year incidence of cataract and cataract surgery in an adult Chinese population. Design: Population-based study. Participants: The Beijing Eye Study 2006 included 3251 (73.2%) subjects (≥45 years) of 4439 subjects who participated in the 2001 survey and returned for re-examination. Methods: Nuclear, cortical, and posterior subcapsular lens opacities were assessed based on standardized slit-lamp–based photographs and retroilluminated photographs of the lens using a modification of the grading score of the Age-Related Eye Disease Study. Main Outcome Measures: Incidence of any type of cataract and of cataract surgery. Results: The 5-year incidence of nuclear cataract was 5.98% (95% confidence intervals [CI], 5.96%–6.00%), that of cortical cataract was 11.14% (95% CI, 11.12%–11.17%), that of posterior subcapsular cataract was 5.47% (95% CI, 5.45%–5.48%), and that of cataract surgery was 2.01% (95% CI, 2.00%–2.02%). The incidence of any cataract (16.82%; 95% CI, 16.79%–16.86%) was significantly associated with higher age (P<0.001) and female gender (P<0.001). It was not significantly associated with the area of habitation (P = 0.78), smoking (P = 0.95), or alcohol consumption (P = 0.12). Differentiation into the 3 cataract types revealed that the incidence of nuclear cataract additionally was associated with rural region (P<0.001) and smoking (P<0.001). The incidence of cortical cataract additionally was associated with nonsmoking (P = 0.02). The incidence of cataract surgery was significantly (P<0.001) associated with age only. Conclusions: As in white persons, the age-adjusted incidence of all types of cataract types increased with age and it was associated with female gender. When compared with data in the literature, the incidence rates of all cataract types did not vary markedly between Chinese in greater Beijing and white persons. Purpose: To assess the prevalence of glaucoma in adult Chinese. Design: Population-based study. Methods: The Beijing Eye Study in 2001 included 4439 subjects with an age of 40+ years. Glaucoma was determined using the ISGEO (International Society of Geographical and Epidemiological Ophthalmology) classification scheme. Results: Glaucoma was detected in 158 subjects (3.7%, 95% confidence interval [CI] 3.1%–4.2%), in which open-angle glaucoma (OAG), primary angle-closure glaucoma (PACG), and secondary glaucoma (SG) accounted for 2.6% (95% CI 2.1%–3.0%), 1.0% (95% CI 0.7%–1.3%), and 0.07% (95% CI 0%–0.1%), respectively. The overall glaucoma prevalence for the age groups of 40 to 49 years, 50 to 59 years, 60 to 69 years, and 70+ years was 2.2%, 2.5%, 4.4%, and 9.5%, respectively. Glaucoma prevalence increased significantly with age (P = .001), myopic refractive error (P < .001), and intraocular pressure (P < .001). The age-standardized prevalence of OAG, PACG, and SG was 2.5%, 1.0%, and 0.1%, respectively. Prevalence of glaucoma-related bilateral blindness or unilateral blindness was significantly (P = .02 and P = .03) higher in PACG than in OAG. Frequency of glaucoma-associated blindness and low vision was significantly higher in the rural area than in the urban region (6/62 vs 2/96, P = .04). Conclusions: In the adult population of Greater Beijing, glaucoma prevalence was 3.6% and increased with age, myopic refractive error, and intraocular pressure. Glaucoma prevalence of 3.6% was comparable with figures from Caucasian populations. The ratio of OAG to PACG of 2.6:1 agrees with recent other studies from East Asia. To determine associations between educational level and ophthalmic diseases in Chinese. The population-based Beijing Eye Study, performed in 2006, enrolled 3,251 participants (age: 45+ years) out of 4,439 subjects invited to participate (response rate: 73.2%). The participants underwent an interview including questions concerning their educational level, and a detailed ophthalmic examination. Data on the level of education were available for 3,221 (99.1%) subjects, with 1,484 (46.1%) subjects living in the rural region. The mean age was 60.4 ± 10.1 years (range: 45–89 years). In a multivariate analysis, a higher level of education was significantly associated with myopic refractive error, higher best-corrected visual acuity, lower degree of nuclear cataract, and lower prevalence of angle-closure glaucoma, and with the systemic parameters of lower age, male gender, urban region, taller body height, and lower body mass index. It was not significantly associated with intraocular pressure, amount of subcapsular cataract and cortical cataract, cataract surgery, and the prevalences of diabetes mellitus, retinal vein occlusions, chronic open-angle glaucoma, and age-related macular degeneration, and with the systemic parameters of fasting serum concentrations of glucose, high-density lipoproteins, low-density lipoproteins, cholesterol and triglycerides, systolic and diastolic blood pressure. In the Greater Beijing area, a higher level of education was associated with myopic refractive error, higher best-corrected visual acuity, and lower prevalence of nuclear cataract and angle-closure glaucoma, after adjusting for the systemic parameters of younger age, male gender, urban region, taller body height, lower body mass index less smoking and less alcohol consumption. Objective: To examine the associations between alcohol consumption and ocular diseases in the adult population of mainland China. Design: Population-based study. Participants: The Beijing Eye Study, performed in 2001, included 4439 subjects (age 40+ years) of 5324 individuals invited to participate (response rate 83.4%). The study was conducted in both a rural region (1973 subjects) and an urban region of Greater Beijing (2466 subjects). Methods: All participants underwent an interview, including questions about alcohol consumption and a detailed ophthalmic examination, including photography of the cornea, lens, and fundus. Main Outcome Measures: Consumption of alcohol and systemic and ophthalmic parameters. Results: Information on alcohol consumption was obtained on 4141 subjects (93.3%), of whom 549 (13.3%) reported they consumed beer or wine. In multivariate analysis, alcohol consumption was significantly associated with the systemic parameters of lower age (P = 0.001), male gender (P<0.001), rural region (P<0.001), lower level of education (P = 0.01), and smoking (P<0.001). Alcohol consumption was not a significant risk factor for the prevalences of age-related macular degeneration (P = 0.24), open-angle glaucoma (P = 0.51), angle-closure glaucoma (P = 0.75), diabetic retinopathy (P = 0.35), retinal vein occlusion (P = 0.39), pterygium (P = 0.08), trachoma (P = 0.053), epiretinal membrane (P = 0.09), non-glaucomatous optic nerve atrophy (P = 0.55), dry eye (P = 0.86), cortical cataract (P = 0.67), subcapsular posterior cataract (P = 0.62), or nuclear cataract (P = 0.76), or with the ocular parameters of refractive error (P = 0.99), intraocular pressure (P = 0.19), retinal artery diameters (temporal inferior: P = 0.60), retinal vein diameters (temporal inferior: P = 0.41), or size of alpha zone and beta zone of parapapillary atrophy (P = 0.68). Conclusions: When adjusted for the systemic parameters of age, gender, rural/urban region, level of education, and smoking, self-reported moderate consumption of alcohol does not have a significant effect on the prevalence of major ocular diseases or the physiologic parameters of intraocular pressure and refractive error. Purpose: To determine the prevalence of cataract surgery and the postoperative visual outcome in the adult Chinese population living in rural and urban regions of Greater Beijing. Design: Population-based study. Participants: The Beijing Eye Study 2006 included 3251 subjects (73.3%) (aged 45+ years) of 4439 subjects who participated in the 2001 survey and returned for reexamination. Methods: The prevalence of surgical pseudophakia/aphakia was assessed, and postoperative visual acuity and reasons for decreased postoperative vision were examined. Visual impairment was defined by a presenting visual acuity (PVA) <6/18 or a best-corrected visual acuity (BCVA) <6/18. Main Outcome Measures: Prevalence of cataract surgery, lens status, and causes of visual impairment. Results: Of the 3215 (98.9% of 3251) subjects for whom data on the lens status were available, 92 (41 male, 133 eyes) had undergone cataract surgery in 1 or both eyes (prevalence rate, 2.86%; 95% confidence interval [CI], 2.28–3.44). In multivariate analysis, this rate was statistically associated with higher age (P<0.001), presence of angle-closure glaucoma (P = 0.02), and higher level of hemorrhagic retinopathy (P<0.001). The prevalence of postoperative visual impairment was 27/133 (20.3%), as defined by a PVA <6/18, and 14/133 or 10.5%, as defined by a BCVA <6/18. Aphakia (odds ratio [OR], 6.71; P = 0.02) and rural residence (OR, 4.05; P = 0.01) were significantly associated with postoperative visual impairment. The most common causes of postoperative visual impairment were uncorrected refractive error followed by hemorrhagic retinopathy and posterior capsule opacification. By comparing 2001 with 2006, the rate of cataract surgery increased, and the rate of postoperative visual impairment decreased significantly (P = 0.049). Conclusions: In the adult population of Greater Beijing, approximately 3% of the population aged 45+ years underwent cataract surgery, the prevalence of which was associated with age, angle-closure glaucoma, and hemorrhagic retinopathy. One of 10 subjects who underwent cataract surgery had a BCVA visual of less than 6/18, and 1 of 5 subjects had a PVA of less than 6/18. Low postoperative visual acuity was associated with rural region, undercorrection of refractive error, and hemorrhagic retinopathy. Objective: To examine the relationship between mortality and major ocular diseases. Design: Population-based study. Participants: At baseline in 2001, the Beijing Eye Study examined 4439 subjects with an age of 40 years or more. The mean age was 56.2±10.6 years (range, 40–101 years). In 2006, all study participants were invited for a follow-up examination. Methods: The participants underwent a detailed ophthalmic examination and answered questions regarding their socioeconomic background. Rate of mortality was determined in the follow-up survey of 2006. Main Outcome Measures: Factors associated with mortality. Results: Of the 4439 subjects examined in the 2001, 3251 (73.2%) subjects returned for the follow-up examination, whereas 143 (3.2%) subjects had died and 1045 (23.5%) subjects were alive but did not agree to be reexamined. In binary logistic regression analysis, mortality was significantly associated with the systemic parameters of higher age (P<0.001; odds ratio [OR], 1.07), male gender (P = 0.01; OR, 0.55), lower level of education (P<0.001; OR, 0.65), smoking status (P = 0.023; OR, 1.25), and with the ocular parameters of level of diabetic-like retinopathy (P = 0.036; OR, 1.02), presence of angle-closure glaucoma (P = 0.013; OR, 3.74), and presence of nonglaucomatous optic nerve damage (P = 0.027; OR, 3.41). Presence of retinal vein occlusions was associated marginally with mortality (P = 0.059; OR, 2.59). Mortality was not significantly associated with best-corrected visual acuity (P = 0.14) in multivariate analysis, nor with age-related macular degeneration, open-angle glaucoma, trachoma, any type of cataract, visual field defects, intraocular pressure, or refractive error. Conclusions: If socioeconomic parameters, age, gender, and smoking status were taken into account, ocular parameters associated with an increased mortality were diabetic-like retinopathy, angle-closure glaucoma, and nonglaucomatous optic nerve damage. Retinal vein occlusions were marginally associated. Other major ocular disorders such as any form of cataract, open-angle glaucoma, age-related macular degeneration, trachoma, pterygia, and high myopia or high hyperopia were not significantly related to mortality. The article offers information on a study which examines the prevalence of diabetic retinopathy as cause for visual impairment in the Beijing Eye Public Health Care Project in China. Trained ophthalmic technicians assessed habitual visual acuity of all participants. The diagnosis of the major causes of visual impairment was made at the Beijing Institute of Ophthalmology. It shows that the prevalence of diabetic retinopathy was associated with younger age in multivariate analysis.

The article presents a study to examine the associations between disc size and level of education and time necessary in performing a standardized automatic visual field test as surrogates of cognitive function in Beijing, China. The study included 4089 subjects whom the assessable disc photographs and data of education level and perimetric test time have been available. Results of the study show optic nerve head size was associated with education level and perimetric test time.

Objective: To evaluate the causes of visual impairment and blindness in adult Chinese in an urban and rural region of Beijing, China. Design: Population-based prevalence survey. Participants: From a rural region and an urban region of Greater Beijing, 4439 of 5324 ≥40-year-old invited subjects participated in the study (response rate, 83.4%). Using the World Health Organization (WHO) standard and the United States standard, blindness was defined as best-corrected visual acuity (BCVA) in the better-seeing eye of <20/400 and of <2/20, respectively, and visual impairment was defined as best-corrected vision of <20/60 and ≥20/400, and of <20/40 and ≥2/20, respectively. Methods: Determination of BCVA, pneumotonometry, frequency doubling perimetry, evaluation of photographs of the fundus and lens, and clinical examination. Main Outcome Measure: Causes of visual impairment and blindness. Results: Visual acuity measurements were available for 8816 eyes of 4409 subjects (99.3%). Using the WHO standard and the U.S. standard, 49 (1.1%) subjects and 95 (2.2%) subjects, respectively, had low vision, and 13 (0.3%) subjects and 15 (0.3%) subjects, respectively, were blind by definition. Taking the whole study population, the most frequent cause of low vision/blindness was cataract (36.7%/38.5%), followed by degenerative myopia (32.7%/7.7%), glaucoma (14.3%/7.7%), corneal opacity (6.1%/15.4%), and other optic nerve damage (2.0%/7.7%). Age-related macular degeneration (AMD) (2.0%/7.7%) and diabetic retinopathy (0%/7.7%) were responsible for a minority of cases. In subjects 40 to 49 years old, the most frequent cause of low vision and blindness was degenerative myopia. In the 50- to 59-year age group, the most frequent cause was cataract, followed by degenerative myopia. In the 60- to 69-year-old subjects and the ≥70-year group, the most frequent cause of low vision and blindness was cataract, followed by degenerative myopia and glaucoma. Conclusions: The most frequent cause of low vision and blindness in adult Chinese is cataract, followed by degenerative myopia and glaucomatous optic neuropathy, with degenerative myopia dominating in younger groups and cataract dominating in elder groups. In contrast to studies in Western countries, AMD and diabetic retinopathy appear to play a minor role as a cause of visual impairment in elderly Chinese. [Copyright &y& Elsevier]

Purpose To examine the prevalence of fundus tessellation and its associated factors. Design Population-based study. Participants The Beijing Eye Study 2011 included 3468 individuals with a mean age of 64.6±9.8 years (range, 50–93 years). Methods Participants underwent a comprehensive ophthalmic examination. By using 45° color fundus photographs of the macula and optic disc, fundus tessellation, defined as variation in the visibility of the large choroidal vessels, was differentiated into 3 grades. Main Outcome Measures Fundus tessellation. Results Assessment of fundus tessellation was available for 3442 individuals (99.6%) or 6789 eyes (98.6%). In multivariate analysis, a higher degree of fundus tessellation (mean, 0.84±0.79) was associated with older age ( P < 0.001; standardized correlation coefficient beta, 0.14), male sex ( P < 0.001; beta, −0.08), lower body mass index ( P = 0.04; beta, 0.03), worse best-corrected visual acuity ( P < 0.001; beta, 0.05), thinner subfoveal choroidal thickness ( P < 0.001; beta, −0.51), longer axial length ( P < 0.001; beta, 0.11), larger parapapillary beta zone ( P < 0.001; beta, 0.08), lower prevalence of intermediate age-related macular degeneration (AMD) ( P = 0.02; beta, −0.04), and lower prevalence of late AMD ( P = 0.007; beta, −0.04). If parapapillary beta zone was dropped, higher glaucoma prevalence ( P = 0.003) was associated with a higher degree of fundus tessellation. Prevalence of diabetes mellitus and retinal vein occlusions, mean blood pressure, and intraocular pressure were not ( P > 0.10) associated with fundus tessellation. In a reverse manner, thinner subfoveal choroidal thickness was associated with a higher degree of fundus tessellation ( P < 0.001; beta, −0.49) in the multivariate analysis. Subfoveal choroidal thickness decreased from 322±90 μm (95% confidence interval [CI], 317–327) in eyes without fundus tessellation to 229±80 μm in eyes with grade 1, to 122±52 μm in eyes with grade 2, and to 81±37 μm in eyes with grade 3 of fundus tessellation. Conclusions Fundus tessellation is a surrogate for choroidal thinness and may be a clinical sign for a leptochoroid. After adjusting for ocular and systemic parameters, fundus tessellation also is associated with a larger parapapillary beta zone and higher glaucoma prevalence, and a lower prevalence of intermediate and late AMD. Objective: To assess associations among outdoor activity, ocular biometric parameters, and myopia among grade 1 and grade 4 primary students in Beijing. Design: School-based, cross-sectional study. Participants: A total of 382 grade 1 and 299 grade 4 children participated in the study. Methods: The children underwent a comprehensive eye examination, including ocular biometry by optical low-coherence reflectometry and noncycloplegic refractometry. Parents and children participated in a detailed interview, including questions on time spent indoors and outdoors. Main Outcome Measures: Factors associated with myopia. Results: The study included 681 children, with 382 (56.1%) students from grade 1 (mean age, 6.3±0.5 years; range, 5–8 years) and 299 students from grade 4 (mean age, 9.4±0.7 years; range, 8–13 years); 370 students (54.3%) lived in the urban region. The mean daily time spent outdoors was 1.6±0.8 hours (range, 0.5–5.1 hours). In multivariate analysis, axial length was significantly associated with older age (P<0.001; standardized β coefficient, 0.28), taller body height (P = 0.001; β, 0.18), maternal myopia (P = 0.03; β, 0.09), and urban region of habitation (P<0.001; β, −0.21), or alternatively to the region of habitation, with less time spent outdoors (P = 0.001; β, −0.16) and more time spent indoors studying (P = 0.02; β, 0.10). The axial length-to-corneal curvature radius ratio was associated with older age, urban region of habitation, maternal and paternal myopia, and paternal level of education. Presence of myopia (defined as refractive error ≤−1 diopters in the right eye) was associated with older age (P<0.001; odds ratio [OR], 1.45; 95% confidence interval [CI], 1.24–1.69), maternal myopia (P<0.001; OR, 2.99; 95% CI, 1.94–5.35), and urban region of habitation (P<0.001; OR, 0.17; 95% CI, 0.11–0.26), or alternatively to the region of habitation, with less time spent outdoors (P<0.001; OR, 0.32; 95% CI, 0.21–0.48) and more time spent indoors studying (P<0.001; OR, 1.38; 95% CI, 1.09–1.75). Conclusions: Less outdoor activity, more indoor studying, older age, maternal myopia, and urban region of habitation were associated with longer ocular axial length and myopia in grade 1 and grade 4 primary school children in Greater Beijing. Remaining outdoors more (e.g., during school) may reduce the high prevalence of myopia in the young generation in Beijing.
