1. Course and predictors of supportive care needs among Mexican breast cancer patients

Abstract

Objective: This study examined the course and predictors of supportive care needs among Mexican breast cancer patients for different cancer treatment trajectories.Methods: Data from 172 (66.4% response rate) patients were considered in this observational longitudinal study. Participants were measured after diagnosis, neoadjuvant treatment, surgery, adjuvant treatment, and the first post-treatment follow-up visit. Psychological, Health System and Information, Physical and Daily Living, Patient Care and Support, Sexual, and Additional care needs were measured with the Supportive Care Needs Survey (SCNS-SF34). Linear mixed models with maximum-likelihood estimation were computed.Results: The course of supportive care needs was similar across the different cancer treatment trajectories. Supportive care needs declined significantly from diagnosis to the first post-treatment follow-up visit. Health System and Information care needs were the highest needs over time. Depressive symptoms and time since diagnosis were the most consistent predictors of changes in course of supportive care needs of these patients.Conclusions: Health system and information care needs of Mexican breast cancer patients need to be addressed with priority because these needs are the least met. Furthermore, patients with high depressive symptoms at the start of the disease trajectory have greater needs for supportive care throughout the disease trajectory.

2. Power, politics, and protest.

Abstract

Of all the influences on the development of New Granada's political life during the eighteenth century, Bourbon policies are the most obvious. We have seen how, throughout the century, ministers of Bourbon monarchs sought to enhance royal authority and improve Spain's command of the region's resources, with varying degrees of intensity and success. The first conjuncture of Bourbon reform in New Granada, in 1717–23, was ineffectual, if not entirely futile. It did, however, clear the ground for a second conjuncture of administrative and commercial reorganization in 1739–40, from which New Granada emerged with a new framework for government, under the command of the viceroys, and a new framework for its overseas trade, carried by individual register-ships. Then, after a long phase of piecemeal, incremental reform, the government of Charles III inaugurated Bourbon New Granada's third and most radical reformist conjuncture in 1778, when Gálvez's new colonial program was applied to the region in the visita general of 1778–83. New Granada's political system was therefore affected by three major moments of reform during the eighteenth century. In the first, the crown reasserted royal authority, but failed to sustain its reorganization of New Granada's system of government. In the second, Madrid established a permanent viceregal government, which strengthened the networks of royal command in New Granada by installing a strong authority at the heart of the territory. 3. Merchants and monopoly.

Abstract

At the beginning of the eighteenth century, New Granada lacked a substantial and influential merchant class of the kind found in Peru and Mexico. In 1695, a group of some twenty merchants in Bogotá had established a consulado de comercio modeled on those of Lima and Mexico City, by contracting with the crown to pay the royal tax known as the avería, due on goods imported by the galleons, in return for the right to a self-governing mercantile jurisdiction. The Consulado of Santa Fe de Bogotá did not survive for long, however. Its members were unable to fulfill their financial obligations, and when the galleon system itself collapsed during the War of the Spanish Succession the Consulado lost its rationale. It was suppressed in 1713, reflecting the inability of New Granadan merchants to sustain an institution of this kind. Many decades then passed before a consulado de comercio was reestablished in New Granada. When the institution was revived in 1795, it was at Cartagena de Indias, the country's leading port and home of its mercantile elite. 4. New Granada and the Spanish mercantile system, 1700–1778.

Abstract

So far, we have examined the structures of settlement, society, and economy in eighteenth-century New Granada and have seen something of the economic links that bound the colony to Spain. However, by focusing on the character of New Granada's social and economic structures, the foregoing chapters have tended to emphasize elements of continuity and evolution within the colony, without paying much attention to the external factors that affected its development. And yet, of course, the eighteenth century saw important changes in relations between Spain and her colonies, as the Bourbon kings who inherited the Hapsburg throne sought to reverse the seemingly inexorable process of decline that Spain had suffered during the seventeenth century. Not only did the new dynasty establish a more centralized and absolutist authority within the metropolis, but, by political, military, and economic reform, it also sought to bring about what has been called the “second conquest of America.” This was not a process that began immediately or proceeded smoothly. The first reforms were hesitant in tone and uneven in application, and the great age of reform did not arrive until the reign of Charles III. New Granada, however, felt the repercussions of change at the metropolitan center early in the century, when its trade and administration were touched by Madrid's earliest efforts to reassert control over the resources of its empire. Abstract

Cysticercosis is an infestation of Cysticercus cellulosae. When it occurs in the brain, chronic neurological complications can ensue, most commonly seizures. Neurocysticercosis is usually diagnosed by neuroimaging, a technique not available in most endemic countries. Hence immunological tests are valuable for diagnosis and epidemiological surveys. We evaluated the suitability of paper for storing blood and cerebrospinal fluid (CSF) until subsequent testing by enzyme-linked immunosorbent assay (ELISA), by testing whole blood samples on filter paper from 305 patients and CSF samples from 117 patients stored on ordinary white typing paper and on filter paper. Optimal preservation of biological samples is achieved when whole blood is stored on filter paper, CSF on white paper, and when samples are frozen within 1 week after collection. Our results could improve diagnostic capabilities and facilitate epidemiological surveys in endemic countries where immunodiagnostic tests cannot be rapidly performed because of inadequate laboratory infrastructure. Abstract

A total of 1280 banknotes were obtained from food outlets in 10 different countries (Australia, Burkina Faso, China, Ireland, the Netherlands, New Zealand, Nigeria, Mexico, the United Kingdom, and the United States), and their bacterial content was enumerated. The presence of bacteria on banknotes was found to be influenced by the material of the notes, and there was a strong correlation between the number of bacteria per square centimeter and a series of indicators of economic prosperity of the various countries. The strongest correlation was found with the “index of economic freedom,” indicating that the lower the index value, the higher the typical bacterial content on the banknotes in circulation. Other factors that appear to influence the number of bacteria on banknotes were the age of the banknotes and the material used to produce the notes (polymer-based vs. cotton-based). The banknotes were also screened for the presence of a range of pathogens. It was found that pathogens could only be isolated after enrichment and their mere presence does not appear to be alarming. In light of our international findings, it is recommended that current guidelines as they apply in most countries with regard to the concurrent hygienic handling of foods and money should be universally adopted. This includes that, in some instances, the handling of food and money have to be physically separated by employing separate individuals to carry out one task each; whereas in other instances, it could be advantageous to handle food only with a gloved hand and money with the other hand. If neither of these precautions can be effectively implemented, it is highly recommended that food service personnel practice proper hand washing procedures after handling money and before handling food.

7. Preferences for Hereditary Breast and Ovarian Cancer Information among Mexican, Cuban and Puerto Rican Women at Risk

Abstract

Background: Little is known about the preferences of at-risk Hispanic women to gain information on hereditary breast and ovarian cancer (HBOC). Aims: This study sought to qualitatively explore preferences for HBOC information among at-risk Mexican, Puerto Rican and Cuban women and to pilot a mock brochure aimed at Hispanic women. Methods: Hispanic women aged 18–65 years with a personal or family history of breast or ovarian cancer participated in a semistructured interview. Data were analyzed using a combination of open-coding and content analysis. Results: Fifty-three women participated in the study. For the majority of content areas, there were no major differences between the subethnicities. All women reported discussing cancer with a doctor after a family member had been diagnosed and discussing cancer within their families; however, the content of the discussion varied. Cuban and Puerto Rican women reported using the Internet routinely for health care information while Mexican women said they did not have access to computers and did not use them. All women liked the content and photos in the brochure but Mexican women thought the reading level was too high. Preferences for the spokesperson focused on the need for Spanish-speaking health care providers. Conclusions: While the data show some similarities, such as patterns of cancer discussion and appreciation of the mock brochure, there were differences between the groups on information preferences. In designing HBOC education information for Hispanic audiences, it is important to consider varied channels for dissemination and preferences for specific types of information across subethnicities.
