Purpose – The purpose of this paper is to provide a synthesis of current and previous government policies and strategies, in relation to people with learning disabilities and employment, to facilitate a better understanding of the current situation and future challenges. Design/methodology/approach – A search was completed to identify government policies relating to the employment of people with learning disabilities. Key policies were identified and their impact was discussed in the paper. Findings – It appears there is a necessity to identify how successful pilot projects can be replicated on a national scale, with clear targets and measures and initial financial support to set up these services. Alongside this there is a need for interventions targeting not just employers, but the general population, educating people about the importance of including and valuing people with learning disabilities in the workforce. Originality/value – It is important that policy is analysed and the impact of it is assessed to determine whether more action is necessary. Recent policy pronouncements emphasise the importance of engaging fathers with preventive primary care services. However, in England, there is a paucity of literature which examines African and African-Caribbean fathers' experiences of service provision. This paper reports a study that investigated African and African-Caribbean fathers' beliefs about fatherhood, health and preventive primary care services, with the aim of addressing the deficit in the literature. Nine focus groups involving 46 African and African-Caribbean fathers, recruited using purposive sampling, were undertaken between October 2008-January 2009. Fatherhood was seen as a core aspect of the participants' identities. The fathers enacted these identities in a number of ways, such as caring for and protecting children, which were influenced by spirituality, relationships with women, paid work and racism. The fathers had concerns about their bodies, medical conditions, physical activity and forms of consumption. However, their primary focus was on maintaining and improving the well-being of their children. This resulted in them neglecting their own health needs as they had to meet the obligations of family life and paid work. The fathers reported limited contact with preventive primary care services and were unaware of their purpose, function and availability. They identified ethnicity as a positive asset, and felt their families and communities had particular strengths. However they acknowledged that structural constraints, including racism, influenced their perceptions of and access to local health services. The engagement of African and African-Caribbean fathers needs to be addressed more specifically in policy as part of a broader programme of action to tackle health inequalities. In addition, child health services could build on fathers' commitment to children's well-being through practice that addresses fathers' as well as mothers' needs in families. [ABSTRACT FROM AUTHOR]

Objectives To explore similarities and differences in policy content and the political context of the three main English government reports on health inequalities: the Black Report (1980), the Acheson Enquiry (1998), and the Marmot Review (2010). Methods Thematic policy and context analysis of the Black Report (1980), the Acheson Enquiry (1998), and the Marmot Review (2010) in terms of: (i) underpinningtheoretical principles; (ii) policy recommendations; (iii) the political contexts in which each was released; and (iv) their actual or potential influence on research and policy. Results There were great similarities and very few differences in terms of both the theoretical principles guiding the recommendations of these reports and the focus of the recommendations themselves. However, there were clear differences in terms of the political contexts of each report, as well as their subsequent impacts on research and policy. Conclusion The paper calls into question the progress of health inequalities research, the use of evidence and of the links between research, politics and policy. [ABSTRACT FROM AUTHOR]

Purpose -- The aim of this article is to critically review the Autism Strategy and to discuss its implications. Design/methodology/approach -- This is a and critical review of the Autism Strategy and its implementation. This includes discussion of the political context, reactions to the strategy by stakeholders, economic considerations, equity, integration across health and social care and the role of diagnostic services and specialist interventions. Findings -- The Autism Strategy Fulfilling and Rewarding Lives was published in 2010. It aimed to increase awareness of autism, establish clear pathways for diagnosis and needs assessment, promote independent living and access to work, and help the development of local services. It focuses on intended outcomes, is not prescriptive about how those aims are achieved, and relies on existing legislation such as the Disability Discrimination Act. The emphasis on accessing mainstream services may limit the development of appropriate specialist services, especially in the current economic climate. Specialist interventions that may follow diagnosis are not prioritised, even though the economic case for them has been well made by the Audit Commission. Although the Department of Health has produced "outcomes and ambitions" to measure implementation of the Strategy, local authorities are not required to measure themselves against these targets or publish their results. However , organisations such as the National Autistic Society have already developed training materials to help with implementation, and the NICE guidelines for adults with autism due in 2012 may help the development of better services. Originality/value -- This article provides new insights into the implications of the strategy for service users, service managers and healthcare professionals.
