3. Artificial coastal defence structures as surrogate habitats for natural rocky shores : giving nature a helping hand

Engineered coastal defence structures are proliferating around coastlines globally to protect expanding urban developments from predicted sea level rise and extreme weather events. In response to evolving marine planning policies, it is becoming increasingly necessary to incorporate ecologically-sensitive design into coastal developments, not only to minimise their environmental impacts, but also to maximise potential ecological and socio-economic secondary benefits. I investigated coastal defence structures as surrogate habitats for rocky shore biodiversity, and the potential for the design of structures to be manipulated to achieve more beneficial outcomes. I focused on three major knowledge gaps that must be addressed in order to effectively incorporate ecologically-sensitive design into coastal defences: (i) the capacity to predict ecological responses to different engineering designs for coastal defence structures; (ii) the potential for ecological engineering interventions to enhance biodiversity on structures; and (iii) stakeholder perceptions regarding the desirability of potential secondary benefits that can be built-in to developments. To address the first knowledge gap, I surveyed 125 intertidal coastal defence structures around the coast of Wales, UK, and modelled the relationship between a number of physico-environmental parameters and the biological communities colonising each structure. Using these data I developed a predictive tool and demonstrated that, given the nature of the shoreline on which a new coastal defence was required (i.e. the surrounding sediments and level of exposure to prevailing wind and waves), it would be possible to predict (with up to 62% confidence) the characteristic community that could be expected to colonise a structure, based on its broad shape, position in the intertidal zone, and abundance of microhabitats. To address the second knowledge gap, I explored the potential for a novel ecological engineering intervention (drill-cored artificial rock pools) to enhance biodiversity on an intertidal riprap breakwater. Over a 30- month period, I found that the artificial pools performed an important ecological function on the structure. They were utilised by numerous species that were not otherwise recorded on surrounding emergent rock surfaces, including taxa that have frequently been reported to be absent or scarce on coastal defences previously (e.g. mobile fauna, lower-shore taxa and proportionally-rarer taxa). Furthermore, the artificial pools were just as productive as natural rock pools and supported a comparable number of species. The composition of communities in artificial and natural pools, however, was different, largely on account of differences in sessile assemblages (i.e. algae and encrusting fauna). The intervention, nevertheless, was an effective and affordable means of ecological enhancement, and has received considerable interest from industry and practitioners. To address the third knowledge gap, I investigated stakeholder attitudes regarding desirability of different potential secondary benefits that may be built-in to coastal developments. Although this study revealed complex and nuanced perceptions across sectors, there was unanimous support for implementing multi-functional coastal defence structures in place of traditional single-purpose ones, and in general the most desirable secondary benefits were ecological ones (prioritised over social, economic and technical benefits). In this thesis I evaluate these outcomes in the context of their application to marine planning and conservation management. I finally conclude by outlining the steps that are necessary to achieve wide-scale and effective implementation of ecologically sensitive and multi-functional design for artificial coastal defence structures that are becoming ubiquitous features of urban coastlines globally.

6. A systematic review of flurbiprofen 8.75mg dose and risk of adverse events (excluding haemorrhagic) resulting from drug-drug interactions.

Background: Flurbiprofen 8.75 mg lozenges and oromucosal sprays are used for symptomatic relief of sore throat in patients aged 12 years and over. The documented adverse events of flurbiprofen use include those related to its pharmacological actions, namely, increased risk of haemorrhagic events, however other adverse events (such as nephrotoxicity and cardiac failure) have been known to occur. The likelihood of occurrence of adverse events increases when flurbiprofen is used concomitantly with some other medications. Therefore, the objective of this systematic review was to collate the current evidence on adverse events which occur with flurbiprofen 8.75 mg dose (any formulation), in particular as a result of interaction with other medicinal products, with a focus on non-haemorrhagic events. Methods: Systematic searches of the literature were conducted to identify literature on any formulation of flurbiprofen 8.75 mg up to the date of the electronic database search (data lock: 28 April 2020). Publications were screened to identify studies reporting non-haemorrhagic adverse events with flurbiprofen 8.75 mg and/or non-haemorrhagic adverse events in the comparator arm. Data extraction was performed for eligible studies according to pre-defined criteria and summarised in narratives, tables and figures. Risk of bias and certainty of evidence assessments were planned for each included study where results relating to the primary objective of the systematic review were available. Results: Of 1,528 publications identified by systematic literature searches, 26 met the inclusion criteria and were included in this review. None of these 26 studies contained information on non-haemorrhagic adverse events occurring as a result of a drug-drug interaction (interaction with concomitant medication used with flurbiprofen 8.75 mg), as per the primary objective and secondary objectives of the systematic review. Conclusion: Results from this systematic review on the risk of non-haemorrhagic events did not provide evidence for these events occurring as a result of interaction with other medicinal products. Additional appropriately designed studies would be required to confirm whether these findings suggest a true absence of risk or limitations in reporting. [ABSTRACT FROM AUTHOR]

This report presents the results of an annual survey of housestaff stipends, benefits, and funding for physicians at teaching hospitals in 1992. The data, presented in 48 tables and 4 figures, are based on responses from 325 members of the Council of Teaching Hospitals (COTH), an 83 percent response rate to the survey. Chapter I contains stipend data for all respondents, reported by hospital region, ownership, and city. Data are unadjusted for regional wage differences and only in Table 2 are data adjusted for inflation. Chapter II contains data on benefits provided to housestaff and their dependents. New questions this year include recruitment incentives, provision of day care, and infection control policies. Chapter III contains data on teaching hospital expenditures and sources of funding for housestaff stipends and benefits. The cross-tabular format displays benefit and expenditure data by region and ownership. Appended are recent articles published in the bimonthly "COTH Report" newsletter. These articles, which are based on data collected in the last year's housestaff survey, address alternative scheduling for housestaff, disability insurance, and immunization policies. Other appendices include the survey response rate by region and ownership and the 1992 COTH housestaff survey questionnaire. (JB)

This methodological investigation set out to examine the use of the Internet as a data collection tool: its potential for sampling gay and bisexual men and gathering data on sexual behaviour. The research took place in the context of the Internet and HIV study, an examination of high risk sexual behaviour among gay and bisexual men seeking sex on the Internet. The Internet and HIV study collected quantitative data using selfadministered questionnaires completed online or by pen-and-paper, and qualitative data using one-to-one interviews conducted face-to-face or online. Its design provides an invaluable opportunity for investigating Internet-based research methods. Methodological issues were explored through collection of primary data from respondents who had participated in the Internet and HIV study and secondary analysis of data collected for the study. The first area of exploration was the use of web surveys for sampling gay and bisexual men. The results indicated that participation in the Internet and HIV web survey was driven by an altruistic desire to contribute to research into sexual health promotion and HIV prevention. Drop out was most likely to occur in the early stages of the survey and minor differences in sub-group drop out were identified. Men who participated in the web survey were broadly similar to the MSM drawn from a national probability sample, but were more likely to participate in high risk sexual activity. Differences were more pronounced when the samples were restricted to London men. The second area of exploration was the effect of the Internet mode on data collection. The web survey reduced item nonresponse in comparison to the pen-and-paper survey but did not improve response to sensitive questions over and above this general mode effect. The results indicated that online synchronous interviews may be suited to a more structured format, given the difficulty of expressing complex ideas through typing in real time. It was concluded that the Internet has excellent potential for researching sexual behaviour among gay and bisexual men. It provides effective access to this population and more complete survey data than the traditional alternative. The application of online interviewing requires further exploration.

The teaching and research of engineering ethics are value-laden endeavors closely tied to the values of academic communities and engineering practice. At the core of this chapter is an appeal to academic leaders, teachers, and researchers to engage explicitly with the value-laden nature of engineering. The chapter expands the scope of established approaches to engineering ethics education by focusing on global and culturally inclusive practices. It is grounded in a reflection on what is meant by the term global and our positionality as researchers in this context. The chapter (1) identifies regional authorship patterns as well as the major institutional and national actors in engineering ethics education research, (2) maps the methods used to conduct research on global and culturally inclusive practices in engineering ethics education, and (3) surveys recent pedagogical and institutional practices to broaden engineering ethics education towards a global and culturally inclusive vision. Finally, the chapter puts forward a list of recommendations for engineering ethics education and research. Through this contribution, we strive to encourage aspirations and endeavors for the identification and incorporation of local contributions and culturally specific topics in the research and teaching of engineering ethics.

Background: Growth hormone (GH)--secreting pituitary adenomas can be aggressive and difficult to manage. Surgical resection for GH-secreting tumors remains the gold standard with increasing use of expanded endoscopic endonasal (EEA) techniques. Certain anatomical considerations make postsurgical biochemical remission challenging. Case Description: We describe the case of a 43-year-old male presenting with acromegaly after a lack of biochemical remission from a previous surgery. Resection of the residual tumor invading the retrogenu compartment of the cavernous sinus was challenging for several reasons: (a) its location adjacent to the right parasellar horizontal internal carotid artery (ICA) with involvement of the medial wall, (b) the large kissing bilateral ICAs reducing the intercarotid distance, and (c) potential scar tissue. EEA was undertaken with key surgical steps, including wide bilateral sphenoidotomies, right middle clinoidectomy to access the clinoidal ICA and the retrogenu compartment, identification of the top of the paraclival ICA by drilling across the sella floor, division of the sellar floor dura to increase the intercarotid distance and transcavernous mobilization of medial wall, and the tumor capsule away from the horizontal parasellar ICA and across to the diaphragm and pituitary gland. Postoperatively, biochemical remission was achieved with no new endocrine deficits. Conclusion: These surgical nuances permit biochemical remission in complex revisional cases with acromegaly. The Engineering Education Research Network in Africa (EERN-Africa) was created to enable connections between practitioners and researchers with a shared interest in African engineering education contexts. Recognising the importance of developing an African voice in the engineering education research space, the EERN-Africa community has interacted in a dynamic and dialogic way with our own teaching and research practices across diverse African contexts, with an ethical commitment to democratic and inclusive community-building. The objective of this paper is to reflect on the current status of the Community of Practice (CoP), and the challenges and opportunities in sustaining and growing the CoP. A collaborative analysis of perspectives on this emerging identity is presented, using an Appreciative Inquiry (AI) methodology and drawing on collective written reflections and discussions. Six broad themes on the value that the CoP has for both individuals and the group were identified: networking, capacity development, emotional support, impact on professional identity, social and environmental impact, and breaking borders. This report is devoted to the role and effectiveness of the state: what the state should do, how it should do it, and how it can do it better in a rapidly changing world. The pathways to an effective state are many and varied. The report provides a framework for guiding these efforts, in the form of a two-part strategy. The first part of the strategy is to focus the state's activities to match its capability. Many states try to do too much with few resources and little capability. Getting governments better focused on the core public activities that are crucial to development will enhance their effectiveness. The second part of the strategy is to look for ways to improve the state's capability by reinvigorating public institutions. The report puts particular emphasis on mechanisms that give public officials the incentive to do their jobs better and to be more flexible, but that also provide restraints to check arbitrary and corrupt behavior. The report is divided into the following parts: (1) "Rethinking the State--The World Over"; (2) "Matching Role to Capability"; (3) "Reinvigorating Institutional Capability"; and (4) "Removing Obstacles to Change." An estimated 77 probands with arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) are screened for mutations in desmocollin-2 (DSC2), a gene coding for a desmosomal cadherin. The heterozygous mutations, which are identified in four probands, result in frameshifts and premature truncation of the desmocollin-2 protein, which is consistent with the hypothesis that ARVD/C is a disease of the desmosome.

Oral non-steroidal anti-inflammatory drugs (NSAIDs) are known to be associated with an increased risk of bleeding. The NSAID, flurbiprofen, in the form of 8.75 mg lozenge or oromucosal spray is indicated for the symptomatic relief of sore throat. Despite the low dose as compared to alternative flurbiprofen preparations, concerns have been raised regarding its safety in terms of haemorrhagic events. This systematic review was conducted to identify existing evidence on the risk of haemorrhagic events with flurbiprofen 8.75 mg dose (any formulation), particularly where this may be due to potential interactions with other medicinal products. The systematic review examined studies reporting haemorrhagic events in patients receiving flurbiprofen 8.75 mg dose. Six individual electronic databases were searched up to 28th April 2020. Records were initially screened for relevance followed by further review of potentially eligible studies. Data extraction was performed for eligible studies and risk of bias in studies was assessed. The search strategy identified 1093 individual records. Of these, 1038 records were excluded after initial review; the majority of these records related to flurbiprofen in alternative formulations with alternative doses (e.g., eye drops, skin patches, oral tablets) thus were not considered relevant for further review. The 55 remaining records related to flurbiprofen 8.75 mg dose (any formulation) or flurbiprofen lozenge/oromucosal spray where the dose was not specified. After further review, 52 of these records were not considered eligible. Thus, only three records were included in this systematic review. The three studies reported a total of five haemorrhagic events in patients taking flurbiprofen 8.75 mg lozenge; the corresponding risk in each of the studies was 8.33, 1.98 and 1.96%. Where possible, comparison of flurbiprofen 8.75 mg lozenge to placebo produced risk ratios of 0.96 (95% CI 0.07, 13.25) and 2.00 (95% CI 0.10, 118.0). This systematic review found limited evidence on the risk of haemorrhagic events with flurbiprofen when used at a dose of 8.75 mg. Counts were low across all studies and results comparing flurbiprofen and placebo treatment arms were non-significant. However, scarcity of studies and low certainty of evidence for the outcome of haemorrhagic events limits the conclusions of this systematic review. [ABSTRACT FROM AUTHOR]

The integration of ethics in engineering education has largely been focused at the curriculum design level. The authors posit that this integration be done at the accreditation level and investigate how ethics may be more extensively incorporated in the documentation of a particular engineering accreditation body's qualification standards. The paper proceeds, by means of a narrative review, to justify an expanded conception of the teaching of ethics within engineering education. It builds a synthesis of contrasting conceptual approaches to the teaching of ethics within engineering and proposes a conceptual framework to guide both regulators and educators to identify and engage with different elements of the ethics across the curriculum within an engineering programme. The South African case study provides a context to engage with existing policy formulation around programme accreditation and to demonstrate the application of the proposed conceptual framework across the graduate attributes so to indicate how ethics might be more comprehensively integrated within a programme. This demonstrates that ethics needs to be repositioned at the centre of the preparation of engineers, rather than at the periphery. The expected consequence of this integration is the more extensive incorporation of ethics within and across accredited engineering programmes. [ABSTRACT FROM AUTHOR]

Abstract Background Following the expansion of the European Union, there has been a large influx of Central and East European (CEE) migrants to the UK. CEE men who have sex with men (MSM) represent a small minority within this population that are none-the-less important to capture in sexual health research among the CEE migrant community. This paper examines the feasibility of recruiting CEE MSM for a survey of sexual behaviour in London using respondent driven sampling (RDS), via gay websites and in GUM clinics. Methods We sought CEE MSM to start RDS chain referral among GUM clinic attendees, our personal contacts and at gay events and venues in central London. We recruited CEE MSM (n = 485) via two popular websites for gay men in Britain (March-May 2009) and at two central London GUM clinics (n = 51) (July 2008-March 2009). Results We found seventeen men who knew other CEE MSM in London and agreed to recruit contacts into the study. These men recruited only three men into the study, none of whom recruited any further respondents, and RDS was abandoned after 7 months (July 2008-January 2009). Half of the men that we approached to participate in RDS did not know any other CEE MSM in London. Men who agreed to recruit contacts for RDS were rather more likely to have been in the UK for more than one year (94.1% vs 70.0%, p = 0.052). Men recruited through gay websites and from GUM clinics were similar. Conclusions The Internet was the most successful method for collecting data on sexual risk behaviour among CEE MSM in London. CEE MSM in London were not well networked. RDS may also have failed because they did not fully understand the procedure and/or the financial incentive was not sufficient motivation to take part.

Abstract Background Depression is a common and distressing mental health problem that is responsible for significant individual disability and cost to society. Medication and psychological therapies are effective for treating depression and maintenance anti-depressants (m-ADM) can prevent relapse. However, individuals with depression often express a wish for psychological help that can help them recover from depression in the long-term. We need to develop psychological therapies that prevent depressive relapse/recurrence. A recently developed treatment, Mindfulness-based Cognitive Therapy (MBCT, see http://www.mbct.co.uk) shows potential as a brief group programme for people with recurring depression. In two studies it has been shown to halve the rates of depression recurring compared to usual care. This trial asks the policy research question, is MBCT superior to m-ADM in terms of: a primary outcome of preventing depressive relapse/recurrence over 24 months; and, secondary outcomes of (a) depression free days, (b) residual depressive symptoms, (c) antidepressant (ADM) usage, (d) psychiatric and medical co-morbidity, (e) quality of life, and (f) cost effectiveness? An explanatory research question asks is an increase in mindfulness skills the key mechanism of change? Methods/Design The design is a single blind, parallel RCT examining MBCT vs. m-ADM with an embedded process study. To answer the main policy research question the proposed trial compares MBCT plus ADM-tapering with m-ADM for patients with recurrent depression. Four hundred and twenty patients with recurrent major depressive disorder in full or partial remission will be recruited through primary care. Depressive relapse/recurrence over two years is the primary outcome variable. The explanatory question will be addressed in two mutually informative ways: quantitative measurement of potential mediating variables pre/post-treatment and a qualitative study of service users' views and experiences. Discussion If the results of our exploratory trial are extended to this definitive trial, MBCT will be established as an alternative approach to maintenance anti-depressants for people with a history of recurrent depression. The process studies will provide evidence about the effective components which can be used to improve MBCT and inform theory as well as other therapeutic approaches.
