Standard methods for studying the association between two ecologically important variables provide only a small slice of the information content of the association, but statistical approaches are available that provide comprehensive information. In particular, available approaches can reveal tail associations, that is, accentuated or reduced associations between the more extreme values of variables. We here study the nature and causes of tail associations between phenological or population‐density variables of co‐located species, and their ecological importance. We employ a simple method of measuring tail associations which we call the partial Spearman correlation. Using multidecadal, multi‐species spatiotemporal datasets on aphid first flights and marine phytoplankton population densities, we assess the potential for tail association to illuminate two major topics of study in community ecology: the stability or instability of aggregate community measures such as total community biomass and its relationship with the synchronous or compensatory dynamics of the community's constituent species; and the potential for fluctuations and trends in species phenology to result in trophic mismatches. We find that positively associated fluctuations in the population densities of co‐located species commonly show asymmetric tail associations; that is, it is common for two species' densities to be more correlated when large than when small, or vice versa. Ordinary measures of association such as correlation do not take this asymmetry into account. Likewise, positively associated fluctuations in the phenology of co‐located species also commonly show asymmetric tail associations. We provide evidence that tail associations between two or more species' population‐density or phenology time series can be inherited from mutual tail associations of these quantities with an environmental driver. We argue that our understanding of community dynamics and stability, and of phenologies of interacting species, can be meaningfully improved in future work by taking into account tail associations. Taylor's law (TL), a commonly observed and applied pattern in ecology, describes variances of population densities as related to mean densities via log(variance) = log(a) + b*log(mean). Variations among datasets in the slope, b, have been associated with multiple factors of central importance in ecology, including strength of competitive interactions and demographic rates. But these associations are not transparent, and the relative importance of these and other factors for TL slope variation is poorly studied. TL is thus a ubiquitously used indicator in ecology, the understanding of which is still opaque.The goal of this study was to provide tools to help fill this gap in understanding by providing proximate determinants of TL slopes, statistical quantities that are correlated to TL slopes but are simpler than the slope itself and are more readily linked to ecological factors.Using numeric simulations and 82 multi‐decadal population datasets, we here propose, test and apply two proximate statistical determinants of TL slopes which we argue can become key tools for understanding the nature and ecological causes of TL slope variation.We find that measures based on population skewness, coefficient of variation and synchrony are effective proximate determinants. We demonstrate their potential for application by using them to help explain covariation in slopes of spatial and temporal TL (two common types of TL).This study provides tools for understanding TL, and demonstrates their usefulness. During the 1980s, the North Sea plankton community underwent a well-documented ecosystem regime shift, including both spatial changes (northward species range shifts) and temporal changes (increases in the total abundances of warmer water species). This regime shift has been attributed to climate change. Plankton provide a link between climate and higher trophic-level organisms, which can forage on large spatial and temporal scales. It is therefore important to understand not only whether climate change affects purely spatial or temporal aspects of plankton dynamics, but also whether it affects spatiotemporal aspects such as metapopulation synchrony. If plankton synchrony is altered, higher trophic-level feeding patterns may be modified. A second motivation for investigating changes in synchrony is that the possibility of such alterations has been examined for few organisms, in spite of the fact that synchrony is ubiquitous and of major importance in ecology. This study uses correlation coefficients and spectral analysis to investigate whether synchrony changed between the periods 1959-1980 and 1989-2010. Twenty-three plankton taxa, sea surface temperature ( SST), and wind speed were examined. Results revealed that synchrony in SST and plankton was altered. Changes were idiosyncratic, and were not explained by changes in abundance. Changes in the synchrony of Calanus helgolandicus and Para-pseudocalanus spp appeared to be driven by changes in SST synchrony. This study is one of few to document alterations of synchrony and climate-change impacts on synchrony. Despite evidence from a number of Earth systems that abrupt temporal changes known as regime shifts are important, their nature, scale and mechanisms remain poorly documented and understood. Applying principal component analysis, change-point analysis and a sequential t-test analysis of regime shifts to 72 time series, we confirm that the 1980s regime shift represented a major change in the Earth's biophysical systems from the upper atmosphere to the depths of the ocean and from the Arctic to the Antarctic, and occurred at slightly different times around the world. Using historical climate model simulations from the Coupled Model Intercomparison Project Phase 5 ( CMIP5) and statistical modelling of historical temperatures, we then demonstrate that this event was triggered by rapid global warming from anthropogenic plus natural forcing, the latter associated with the recovery from the El Chichón volcanic eruption. The shift in temperature that occurred at this time is hypothesized as the main forcing for a cascade of abrupt environmental changes. Within the context of the last century or more, the 1980s event was unique in terms of its global scope and scale; our observed consequences imply that if unavoidable natural events such as major volcanic eruptions interact with anthropogenic warming unforeseen multiplier effects may occur. Decadal-scale regime shifts in Northwest Atlantic shelf ecosystems can be remotely forced by climate-associated atmosphere--ocean interactions in the North Atlantic and Arctic Ocean Basins. This remote climate forcing is mediated primarily by basin- and hemispheric-scale changes in ocean circulation. We review and synthesize results from process-oriented field studies and retrospective analyses of time-series data to document the linkages between climate, ocean circulation, and ecosystem dynamics. Bottom-up forcing associated with climate plays a prominent role in the dynamics of these ecosystems, comparable in importance to that of top-down forcing associated with commercial fishing. A broad perspective, one encompassing the effects of basin- and hemispheric-scale climate processes on marine ecosystems, will be critical to the sustainable management of marine living resources in the Northwest Atlantic. The Continuous Plankton Recorder survey has monitored plankton in the Northwest Atlantic at monthly intervals since 1962, with an interegnum between 1978 and 1990. In May 1999, large numbers of the Pacific diatom Neodenticula seminae were found in Continuous Plankton Recorder (CPR) samples in the Labrador Sea as the first record in the North Atlantic for more than 800 000 years. The event coincided with modifications in Arctic hydrography and circulation, increased flows of Pacific water into the Northwest Atlantic and in the previous year the exceptional occurrence of extensive ice-free water to the North of Canada. These observations indicate that N. seminae was carried in a pulse of Pacific water in 1998/early 1999 via the Canadian Arctic Archipelago and/or Fram Strait. The species occurred previously in the North Atlantic during the Pleistocene from∼1.2 to∼0.8 Ma as recorded in deep sea sediment cores. The reappearance of N. seminae in the North Atlantic is an indicator of the scale and speed of changes that are taking place in the Arctic and North Atlantic oceans as a consequence of regional climate warming. Because of the unusual nature of the event it appears that a threshold has been passed, marking a change in the circulation between the North Pacific and North Atlantic Oceans via the Arctic. Trans-Arctic migrations from the Pacific into the Atlantic are likely to occur increasingly over the next 100 years as Arctic ice continues to melt affecting Atlantic biodiversity and the biological pump with consequent feedbacks to the carbon cycle. Abstract Oceanic inflow is estimated to contribute more than 90% of the nutrient input into the North Sea. Variability in the volume, chemical properties, biological content and source of the inflowing water is thus likely to have a considerable effect on North Sea ecosystems. Changes seen in the plankton, and in particular Calanus finmarchicus and Calanus helgolandicus , over the last 40 years as measured by the Continuous Plankton Recorder survey, allow clear periods to be identified that appear to be associated with variability in inflow. Monthly estimates of inflow and outflow across a section between Orkney and Utsira in Norway as well as netflow (sum of Baltic outflow, runoff and Channel inflow), have been derived from runs of the NORWECOM model for two integrated depth intervals: surface to 150 m and >150 m. A comparison is made between the physical model output and plankton results for the period 1958–99. Distinct plankton periods that appear to reflect changing inflow events are discussed in relation to hydrometeorological and earlier plankton studies over approximately the last 100 years. Abstract Recently, large-scale changes in the biogeography of calanoid copepod crustaceans have been detected in the northeastern North Atlantic Ocean and adjacent seas. Strong biogeographical shifts in all copepod assemblages were found with a northward extension of more than ° in latitude of warm-water species associated with a decrease in the number of colder-water species. These changes were attributed to regional increase in sea surface temperature. Here, we have extended these studies to examine long-term changes in phytoplankton, zooplankton and salmon in relation to hydro-meteorological forcing in the northeast Atlantic Ocean and adjacent seas. We found highly significant relationships between (1) long-term changes in all three trophic levels, (2) sea surface temperature in the northeastern Atlantic, (3) Northern Hemisphere temperature and (4) the North Atlantic Oscillation. The similarities detected between plankton, salmon, temperature and hydro-climatic parameters are also seen in their cyclical variability and in a stepwise shift that started after a pronounced increase in Northern Hemisphere Temperature anomalies at the end of the 1970s. All biological variables show a pronounced change which started after circa 1982 for euphausiids (decline), 1984 for the total abundance of small copepods (increase), 1986 for phytoplankton biomass (increase) and Calanus finmarchicus (decrease) and 1988 for salmon (decrease). This cascade of biological events led to an exceptional period, which is identified after 1986 to present and followed another shift in large-scale hydro-climatic variables and sea surface temperature. This regional temperature increase therefore appears to be an important parameter that is at present governing the dynamic equilibrium of northeast Atlantic pelagic ecosystems with possible consequences for biogeochemical processes and fisheries. Trends in basking shark (Cetorhinus maximus ) fishery catches off Achill Island, west Ireland between 1949 and 1975 were examined in relation to zooplankton (total copepod) abundance in four adjacent sea areas over a 27-year period. The numbers of basking sharks caught and copepod abundance showed downward trends and were positively correlated (r -value range, 0.44–0.74). A possible explanation for the downward trend in shark catches was that progressively fewer basking sharks occurred there between 1956 and 1975 because fewer copepods, their food resource, occurred near the surface off west Ireland over the same period. We suggest that the decline in basking sharks may have been due to a distributional shift of sharks to more productive areas, rather than a highly philopatric, localized stock that was over-exploited. In the more than 50 years that the Continuous Plankton Recorder (CPR) survey has operated on a regular monthly basis in the north-east Atlantic and North Sea, large changes have been witnessed in the planktonic ecosystem. These changes have taken the form of long-term trends in abundance for certain species or stepwise changes for others, and in many cases are correlated with a mode of climatic variability in the North Atlantic, either: (1) the North Atlantic Oscillation (NAO), a basin-scale atmospheric alteration of the pressure field between the Azores high pressure cell and the Icelandic Low; or (2) the Gulf Stream Index (GSI), which measures the latitudinal position of the north wall of the Gulf Stream. Recent work has shown that the changes in the GSI are coupled with the NAO and Pacific Southern Oscillation with a 2 year lag. The plankton variability is also possibly linked to changes observed in the distribution and flux of water masses in the surface, intermediate and deep waters of the North Atlantic. For example, in the last two decades, the extent and location of the formation of North Atlantic Deep Water, Labrador Sea Intermediate Water and Norwegian Sea intermediate and upper-layer water has altered considerably. This paper discusses the extent to which observed changes in plankton abundance and distribution may be linked to this basin-scale variability in hydrodynamics. The results are also placed within the context of global climate warming and the possible effects of the observed melting of Arctic permafrost and sea ice on the subpolar North Atlantic. The geographical and seasonal occurrences of dinoflagellate cysts at a depth of 10 m in the North Atlantic and North Sea are described from samples taken by the Continuous Plankton Recorder between June 1973 and December 1975. A special study was made of the large cyst of Polykrikos schwartzii Bütschli in the autumn of 1973 when CPR samples were supplemented by vertical sampling in the upper 500 m with a Longhurst-Hardy Plankton Recorder at Ocean Weather Station, INDIA. Only ten isolated records of cysts were made in the western Atlantic. All others occurred to the east of 30°W in both oceanic and neritic waters and especially in the shallow waters of the North Sea. Cysts were found in the spring plankton but were most abundant in the autumn. They have been recorded down to 500 m but occur most frequently in the top 30 m of the water column. Large aggregations of cysts in bottom sediments probably reflect concentration over many years since they cannot be explained by the low numbers found in the plankton. Generally, the distribution of cysts in the plankton is reflected in bottom sediments but concentrations in sediments may be outside the centres of production, implying movement by currents or sediment transport. A cluster analysis of the dinoflagellate cyst assemblage of sixty-eight intertidal sediment samples from the British Isles has been used to divide the coastline into eleven regions. The resulting pattern of regions is discussed in relation to hydrography and the distribution of dinoflagellates in the plankton. The subdivision of dinoflagellate regions shows a close relationship to the occurrence of chaetognath indicators of water masses around the British Isles. This suggests that cyst propagation is localized and that each water mass has its own characteristic dinoflagellate population. [ABSTRACT FROM AUTHOR]

During a 6-year cumulative experience, 163 patients with drug-refractory ventricular tachycardia/fibrillation (VT/VF) underwent automatic implantable defibrillator (AID) and/or automatic implantable cardioverter defibrillator (AICD) implantation at our institutions. Baseline preimplantation programmed electrical stimulation was performed in 160 patients revealing induction of sustained arrhythmias in 115 patients (71%), nonsustained arrhythmia in 20 patients (13%), and noninducible arrhythmia in 25 patients (16%). Operative mortality was 4.9%. At 22 ± 15 (mean ± standard deviation) months follow-up 77 patients (50%) had experienced at least one appropriate AID/AICD discharge. The actuarial incidence of sudden cardiac death (VT/VF death within 1 hour of symptoms or during sleep) in the AICD population was 1.8% and 4.1% at 1 and 2 years, respectively. Excluding patients with operative death and those responding to concomitant subendocardial resection (no longer inducible arrhythmias), the incidence of clinical arrhythmic event (AICD discharge and/or VT/VF death) in patients with sustained, nonsustained, and noninducible arrhythmias were not significantly different. We conclude that the AICD appears to reduce the expected incidence of sudden cardiac death in patients with drug-refractory VT/VF and that selection for AICD therapy should not be based on results of baseline programmed electrical stimulation alone. Refractory period extension (RPE) bas been proposed as a basic mechanism for defibrillation but it remains unclear if RPE exists at the fast rates associated with ventricular fibrillation. In 7 pentobarbital anesthetized dogs, we measured refractory periods with and without 8 ms rectangular transcardiac shocks at left ventricular pacing rates of 200-600 beats/min. To achieve these high rates, an incremental rate pacing method was used to produce pacing train timing sequences requiring 4.5-27 seconds. A variably timed premature stimulus followed the last stimulus in each pacing train. To determine refractoriness, a 128 electrode array (4 × 4 cm) was used to detect the presence, or absence of an activation sequence sweeping away from the pacing site. At each rate, a control refractory period (RPc) was measured and refractory periods were also measured for 8 and 12 V/cm shocks with coupling intervals of 60% to 90% of RPc. RPc decreased as the rate increased with a minimum RPc of 94 ms at a rate of 600 beats/min (100 ms cycle length). RPE/RPc versus shock coupling interval was similar at all pacing rates. RPE/RPc increased with increased coupling interval or higher shock intensity. We conclude that during ventricular pacing at fibrillatory rates tissue is nearly always in a refractory state: that RPE exists at fibrillatory activation rates; and that RPE/RPc versus shock coupling interval does not vary strongly with pacing rate. Objective: The mechanisms by which pharmacological agents alter electrical defibrillation ant not full understood. It has been proposed that, in addition to directly stimulating tissue, defibrillation may involve refractory period extension (BPE) produced by the shock. Accordingly, pharmacological agents might modulate defibrillation by altering EPE. This study examined the effect of Glass I and Glass III antiarrhythmic agents on RPE by transcardiac shocks. Methods: In four groups of pentobarbital anesthetized dogs, RPE was measured during rapid ventricular pacing before and after administration of either the Glass I agents flecainide (n = 7) or encainide (n = 7), the Glass III agent clofilium (n = 7), or vehicle (n = 5). Measurements included QRS duration during sinus rhythm and a conduction time, QTc interval and refractory period, and BPE for 4- to 10- V/cm shocks delivered 20-80 ms before the end of the tissue absolute refractory period. For the 6-V/cm shocks, the interval after the shock during which tissue remained refractory (RIAS) was also computed. Results: Drugs affected QRS duration, conduction time, QTc, and refractory period (without shocks) in accordance with their anticipated Class I and Class III actions. Without drugs, significant RPE was observed in all animals for all shocks delivered 40 ms or less before the end of the refractory period. Clofilium. encainide, and flecainide had a tendency to increase RPE but only clofilium produced a significant increase. For 6-V/cm shocks with different timings, the minimum RIAS was found to be approximately 43 ms, and occurred for shocks given 20-30 ms before the end of the refractory period. Conclusions: At drug dosages that produced moderate Class III (≈15%) or strong Class I (≈ 35 %) effects, only the Class III agent significantly increased RPE arid RIAS. Thus, in addition to altering tissue excitability, the effect of antiarrhythmic agents to increase RPE and the minimum RIAS may help explain their influence on defribrillation threshold. In 18 open-chested mongrel dogs (18.0 ± 1.7 kg) we compared three anesthetics and three methods for measuring the defibrillation threshold. Six animals were anesthetized with pentobarbital (30 mg/kg) and maintained with a pentobarbital infusion (4 mg/kg per hour). All other animals were anesthetized with sodium brevital (10 mg/kg) and maintained with either halothane gas (1.5%, N = 6) or isoflurane gas (1.8%, N = 6). In each dog. we measured the energy required for 50% successful defibrillation (E50) with: (A) a 3 reversal—up/down method; (B) a 15 shock—up/down method; and (C) a percent success method. Anesthetics and methods were selected in a balanced random order. Ventricular fibrillation was induced with 50 Hz electrical pacing. After 15 seconds, monophasic truncated exponential shocks were delivered by way of a spring-patch electrode configuration. The animal was rescued (if needed) and fibrillation/defibrillation episodes were repeated at 3 minute intervals. After each determination of the E50, the E50 was delivered in ten successive defibrillation trials to determine its actual success rate. We found no significant difference in E50 among anesthetics; a significant difference (P < 0.05) in E50 between method C (9.7 ± 2.6 joules) and method B (8.2 ± 1.6 joules); no significant difference among anesthetics or methods for the actual success rate of the E50 (45 ± 21% successful); and method A required significantly fewer fibrillation episodes and number of shocks and less cumulative energy than the other methods. We concluded that the anesthetics tested had little effect on E50 but that the method used to determine E50 could have an effect. Cellulose acetate of the hot water soluble polysaccharide extracts from whole filaments, as well as base., mid and tip segments, of marine asexual and sexual Bangia atropurpurea (Roth) C. Ag. yielded distinctive patterns which demonstrated that changes occur in the outer cell walls during sexual reproduction. Heterogeneity of the sulfated polysaccharide components isolated from outer cell walls was shown to be specifically related to sexual reproduction. Two components (Bands I and II) were detected in extracts from tips of sexual filaments, while only one (Band I) was present in the vegetative segments of all filaments and in asexual reproductive regions. Introduction: Multiple-pathway electrode configurations generally allow improved current distribution over the heart and lower defibrillation thresholds than single-pathway systems. However, current distributions using multiple pathways are largely determined by electrode type and location. We hypothesized that switching the current among multiple pathways at high frequency (HF) could allow the switching duty cycle to control the proportion of time-averaged current flowing in each pathway, thus permitting altered (possibly improved) defibrillation efficacy using the same electrodes and shock waveform. Methods and Results: In dogs, we measured the current (I50) for 50% defibrillation success using catheter electrodes in the right ventricular apex (cathode) and superior vena cava (A-pathway anode) and a subcutaneous patch on the left chest wall (B-pathway anode). In group 1 (N = 7), we measured I50S for shocks that used HF to proportion 10% to 90% of the current to the A-pathway. Shocks with 10% to 30% of the current in the A-pathway had significantly lower I50S than nonproportioned shocks using all three electrodes. However, the resistance differed among single and simultaneous pathways so energy did not necessarily parallel these changes. In group 2 (N = 6), we measured I50S for shocks to the B-pathway alone, for nonproportioned shocks to A and B, and for shocks that proportioned 80% of the current to the B-pathway using either HF, sequential, or amplitude proportioning methods. All proportioning methods had similar I50s that were significantly lower than the I50 for nonproportioned shocks to A and B and that were comparable to shocks to the B-pathway alone. Conclusions: Shocks with most current proportioned to the B-pathway had lower defibrillation currents than nonproportioned shocks using both pathways. Thus, defibrillation efficacy was changed by HF proportioning without changing the electrodes or shock waveform. Introduction: Capacitor-discharge type waveforms are practical for defibrillation devices but may not be optimum. Discharging a capacitor as a series of high-frequency (HF) pulses may allow effective waveform shaping by modulating the pulses. This approach could lead to improved defibrillation by allowing waveforms that would otherwise be unachievable with a capacitor-discharge approach. However, little is known about defibrillation with HF. Methods and Results: In open chest pentobarbital anesthetized dogs, we measured defibrillation thresholds for continuous rectangular waveforms with 5-, 10-, and 20-msec durations and for 10- and 20-msec long series of HF rectangular pulses. HF series bad a 50% "on-time" duty cycle at 100 Hz to 20 kHz. At 1 kHz and above, defibrillation with HF required the same time-averaged current but approximately twice the peak current and energy as defibrillation with continuous waveforms having the same envelope duration. At lower frequencies, defibrillation peak current and energy approached values required for the continuous waveforms. While waveforms were not actually filtered, the heart responded as though the HF series were low-pass filtered. A filtered effective waveform model with a 3.7-sec time constant predicts these HF data and makes reasonable predictions for various continuous waveform shapes. Conclusion: Defibrillation is possible using HF pulses up to 20 kHz and has a frequency response similar to a low-pass filter. A filtered effective waveform model predicts these HF results and may help explain bow waveforms influence defibrillation efficacy. While the un-modulated HF pulsing used in this study increased defibrillation requirements, these findings support the concept that HF pulse modulation can be used to change the effective shape of a waveform, which could permit more efficacious waveform shapes and a net reduction of thresholds. Introduction: A new method for defibrillation allows two shocks to be combined to defibrillate with reduced current by adjusting their separation according to the cycle length of the fibrillation event. We investigated various aspects of this new method to better understand its characteristics and applicability to defibrillation. Methods and Results: In 48 pentobarbital-anesthetized dogs, we measured the current for 50% defibrillation success using the new method with sequences of rectangular shocks. Group 1 studied the role of shock total duration and found that two-shock sequences followed a strength-duration curve similar to, but below, that for single shocks. Group 2 studied the role of amplitude and duration balance between shocks and found that two-shock sequences with equal shocks performed best. Group 3 studied whether the new method could be used with either biphasic waveforms or sequential shock pathways. Current reduction for the combined methods equaled the product of current reduction by each method, demonstrating that these methods can be effectively combined. Group 4 extended the method to include three-shock and four-shock sequences and found that a fourth shock did not further improve defibrillation. The optimum three-shock sequence required 33% lower current (P < 0.002) and 34% greater energy (P < 0.095 = NS) than a single shock. Conclusions: The new method allows defibrillation to be distributed over several fibrillatory cycles and has an improved strength-duration relationship. Two- or three-shock sequences using equal shocks permit a substantial reduction of defibrillation current that can be combined with the reduction for biphasic and sequential methods. Introduction: We investigated a method of defibrillation in which two shocks were delivered to the same electrodes with a separation based on the cycle length of the fibrillation event (FCL). Methods and Results: In pentobarbital anesthetized dogs, a computerized system determined the FCL from the fibrillation event, computed the desired double-pulse (DP) shock separation, and immediately delivered the DP shocks. In group 1, energy for 50% success at defibrillation (E50) was measured using separations from 55% to 95% of the FCL and remeasured after administration of flecainide, clofilium, or vehicle to change the FCL. Both drugs increased FCL by = 25%. Plots of E50 versus %FCL aligned before and after drug showed that the optimum pulse separation followed the FCL. In group 2, E50s were measured for 55% to 185% FCL separations in clofilium or vehicle-treated animals. The optimum DP F50 was at 85% FCL and was not significantly different from the single-shock F50. In group 3, no differences were found when comparing the probability of success versus total energy relationships for single and optimum DP shocks. Group 4 compared E50s for single and DP shocks using a single-catheter configuration and three-electrode configurations (catheter-subdermal patch). F50s for single and DP shocks were equal using the single-catheter configuration but DP shocks required = 20% more energy in the three-electrode configurations. Conclusion: In single-pathway lead configurations, two shocks with 85% FCL separation can be reliably combined to defibrillate using the same total energy as a larger single shock. The effects of acebutolol, a new investigational cardioselective beta blocker, and propranolol on ventricular arrhythmias were compared in 14 patients with more than 30 premature ventricular contractions (PVCs) per hour. Each patient served as their own control, receiving both drugs and placebo in random sequence and in double-blind fashion, with an intervening one-week, drug-free period. Each drug was given for a two-month period, the maximum acebutolol dosage reaching 600 mg tid and the maximum propranolol dosage 80 mg tid. Seventy-two-hour ambulatory electrocardiographic monitoring assessed arrhythmia frequency for each study period. Mean PVC counts did not significantly differ during the two control periods. Acebutolol decreased mean PVC count by 65% (P < .02), with eight patients exhibiting a 70% or greater decrease. Only three patients exhibited a similar decline with propranolol. The incidence of PVCs was not significantly decreased by propranolol. Acebutolol reduced the number of couplets by 70% (P < .04), whereas propranolol did not significantly affect couplets. At the dosage of 600 mg tid, acebutolol was well tolerated, effectively suppressed total PVCs and couplets, and appeared to be more effective than propranolol administered at 80 mg tid. Lidocaine plasma levels and indocyanine green clearance were measured in five normal volunteers and eight patients admitted to the coronary care unit. All individuals received lidocaine as a 1 mg/kg bolus and a 35 μg/kg/min constant infusion for 180 minutes. Eight of the 13 (62 per cent) individuals studied (all normal volunteers and three patients) developed early, potentially subtherapeutic plasma lidocaine levels (≤ 2.4 μg/ml) within 15 minutes after starting therapy. Those individuals with subtherapeutic levels had either minimal (American Heart Association Class I) or no clinical evidence of congestive heart failure. The use of indocyanine green (ICG) clearance as an estimate of hepatic plasma flow showed that individuals with early, subtherapeutic lidocaine levels had higher ICG clearance (9.33 ± 0.32 ml/min · kg versus 2.90 ± 1.74 mi/min · kg) and shorter ICG t1/2 (2.02 ± 0.99 minutes versus 3.6 ± 0.69 minutes) and larger volume of distribution (36.1 ± 16.3 ml/kg versus 19.5 ± 12.8 ml/kg) than patients without subtherapeutic levels. This study suggests that early lidocaine kinetics may be significantly altered by clinical conditions that alter hepatic blood flow. The reappearance of arrhythmias shortly after initiating lidocaine therapy in patients without heart failure may be due to rapid hepatic clearance and subtherapeutic blood levels rather than lidocaine-resistant arrhythmias. Twiddler's syndrome, characterized by dislodgment of pacemaker leads due to twisting of pulse generators within the subcutaneous pocket with subsequent retraction of leads and loss of pacing function, has been well described in patients with permanent pacemakers implanted for bradyarrhythmias. The case presented here is the first report of a patient with an internal automatic cardioverter-defibrillator with lead dislodgment due to pulse generator rotation. The automatic implantable cardioverter-defibrillator is an electronic device designed to monitor the heart continuously, to identify malignant ventricular tachyarrhythmias, and then to deliver effective countershock to restore normal rhythm. There are two defibrillating electrodes which are also used for waveform analysis; one is located in the superior vena cava, the other is placed over the cardiac apex. A third bipolar right ventricular electrode serves for rate counting and R-wave synchronization. When ventricular fibrillation occurs, a 25 joule pulse is delivered; when ventricular tachycardia faster than a preset rate is detected, the discharge is R-wave synchronized. The device can recycle three times if required. Special batteries can deliver over 100 shocks or provide a 3-year monitoring life. Implantation of the device is made either through a thoracotomy or by a subxiphoid approach. Thus far, the device has been implanted in 160 patients with a follow-up of 42 months. Acceleration of ventricular tachycardia to a faster rhythm or to ventricular fibrillation occurred only rarely and is dealt with most successfully through recycling. Actuarial analysis of the initial 52 patients has indicated 22.9% one-year total mortality, a 52% decrease from the 48% mortality that would be expected in the same group of patients without the device; the mortality attributed to arrhythmias was only 8.5%. In conclusion, the automatic cardioverter-defibrillator can reliably identify and correct potentially lethal ventricular tachyarrhythmias, leading to a substantial increase in survival in properly selected high-risk patients. Technical advances now permit permanent implantation of devices such as antitachycardia pacemakers, cardioverters and cardioverter-defibrillators for control of ventricular tachyarrhythmias. Despite the fact that these devices have been implanted only in patients resistant to conventional medical and surgical management, their effectiveness has been very encouraging. While improved designs are anticipated, preliminary studies have clearly indicated that one of the major challenges for successful clinical use will be appropriate interfacing of the device with the pathophysiology of the patient. Failure to recognize this fact will likely lead to unsuccessful clinical application. Moreover, these new instruments appear to be complementary to, rather than a replacement for, existing medical and surgical approaches to the patient with life-threatening ventricular tachyarrhythmias. The automatic implantable defibrillator continuously monitors cardiac rhythm, identifies ventricular fibrillation and then delivers corrective defibrillatory discharges when indicated: it weighs 250 grams and has a volume of 145 cc. When a suitable arrhythmia is detected, a 25 Joule pulse is delivered through a superior vena cava catheter electrode and another electrode placed over the cardiac apex. As of March 1981, sixteen survivors of multiple cardiac arrests refractory to antiarrhythmic therapy had undergone implantation of the automatic defibrillator. There was no operative mortality and the morbidity was minimal. Electrophysiologic studies were performed before and after surgery to confirm failure of drug therapy and to ensure the device's ability to terminate malignant arrhythmias. Eight spontaneous and fourteen of the seventeen induced malignant arrhythmias were properly recognized and corrected by the device. The discharges were well tolerated by awake patients. A number of problems including recycling delays and spurious discharges have been identified and corrected. There were three late deaths with pulmonary edema noted in two patients, and asystole in one. The autopsies revealed no myocardial damage attributable to the automatic defibrillator. Although the ultimate role of this approach to prevention of sadden arrhythmic death has yet to be determined, the results obtained to date are encouraging and indicate that a useful modality for treating malignant ventricular arrhythmias has been added to our armamentarium. Deriving maps of phytoplankton taxa based on remote sensing data using bio-optical properties of phytoplankton alone is challenging. A more holistic approach was developed using artificial neural networks, incorporating ecological and geographical knowledge together with ocean color, bio-optical characteristics, and remotely sensed physical parameters. Results show that the combined remote sensing approach could discriminate four major phytoplankton functional types (diatoms, dinoflagellates, coccolithophores, and silicoflagellates) with an accuracy of more than 70%. Models indicate that the most important information for phytoplankton functional type discrimination is spatio-temporal information and sea surface temperature. Several environmental/physical variables derived from satellite and in situ data sets were used to understand the variability of coccolithophore abundance in the subarctic North Atlantic. The 7-yr (1997-2004) time-series analysis showed that the combined effects of high solar radiation, shallow mixed layer depth (<20 m), and increased temperatures explained <89% of the coccolithophore variation. The June 1998 bloom, which was associated with high light intensity, unusually high sea-surface temperature, and a very shallow mixed layer, was found to be one of the most extensive (>995,000 km²) blooms ever recorded. There was a pronounced sea-surface temperature shift in the mid-1990s with a peak in 1998, suggesting that exceptionally large blooms are caused by pronounced environmental conditions and the variability of the physical environment strongly affects the spatial extent of these blooms. Consequently, if the physical environment varies, the effects of these blooms on the atmospheric and oceanic environment will vary as well. [ABSTRACT FROM AUTHOR]
