Aim: Review changes in neonatal ventilation practice within our regional transport service, Embrace, identifying interventions with greatest impact on improved rates of normocapnia during transfer., Methods: Using internal transport databases and UK Neonatal Transport Group data submissions, we tracked local and national rates of ventilation and normocapnia. We correlated this with internal changes in practice, including introduction of new equipment, staffing changes, educational interventions and quality improvement projects., Results: Data demonstrated improvement in normocapnia rates benchmarked against national figures, which was not explained by changes in ventilation methods or rates, or by changes in availability of post-transfer gases. Greatest improvement was identified following introduction of transcutaneous CO 2 monitoring and ventilators enabling volume-guided ventilation strategies. Additionally, although less quantifiable, educational and quality improvement interventions, and case review mechanisms were felt to be influential., Conclusion: Volume guided ventilation and transcutaneous CO 2 monitoring have had a positive influence on the maintenance of normocapnia during transfer at Embrace Transport Service, although introduction of new equipment still presents challenges which must be overcome. Recognising the significant impact of these technologies allows for ongoing financial, time and educational investment to emphasise their importance and ensure appropriate awareness of limitations and troubleshooting options, maximising their positive impact., (© 2023 Foundation Acta Paediatrica. Background: Lung volume loss is a major risk factor for postoperative respiratory complications after general anaesthesia and mechanical ventilation. We hypothesise that spontaneous breathing without pressure support may enhance the risk for atelectasis development. Therefore, we aimed at characterising whether pressure support prevents changes in lung function in patients breathing spontaneously through laryngeal mask airway., Methods: In this randomised controlled trial, adult female patients scheduled for elective gynaecological surgery in lithotomy position were randomly assigned to the continuous spontaneous breathing group (CSB, n = 20) or to the pressure support ventilation group (PSV, n = 20) in a tertiary university hospital. Lung function measurements were carried out before anaesthesia and 1 h postoperatively by a researcher blinded to the group allocation. Lung clearance index calculated from end-expiratory lung volume turnovers as primary outcome variable was assessed by the multiple-breath nitrogen washout technique (MBW). Respiratory mechanics were measured by forced oscillations to assess parameters reflecting the small airway function and respiratory tissue stiffness., Results: MBW was successfully completed in 18 patients in both CSB and PSV groups. The decrease in end-expiratory lung volume was more pronounced in the CSB than that in the PSV group (16.6 ± 6.6 [95% CI] % vs. 7.6 ± 11.1%, p = .0259), with no significant difference in the relative changes of the lung clearance index (-0.035 ± 7.1% vs. -0.18 ± 6.6%, p = .963). The postoperative changes in small airway function and respiratory tissue stiffness were significantly lower in the PSV than in the CSB group (p < .05 for both)., Conclusions: These results suggest that pressure support ventilation protects against postoperative lung-volume loss without affecting ventilation inhomogeneity in spontaneously breathing patients with increased risk for atelectasis development., Trial Registration: NCT02986269., (© 2023 The Acta Anaesthesiologica Scandinavica Foundation. Background: Spontaneous breathing is desirable in most ventilated patients. We therefore studied the influence of isoflurane versus propofol sedation on early spontaneous breathing in ventilated surgical intensive care patients and evaluated potential mediation by opioids and arterial carbon dioxide during the first 20 h of study sedation., Methods: We included a single-center subgroup of 66 patients, who participated in a large multi-center trial assessing efficacy and safety of isoflurane sedation, with 33 patients each randomized to isoflurane or propofol sedation. Both sedatives were titrated to a sedation depth of -4 to -1 on the Richmond Agitation Sedation Scale. The primary outcome was the fraction of time during which patients breathed spontaneously., Results: Baseline characteristics of isoflurane and propofol-sedated patients were well balanced. There were no substantive differences in management or treatment aside from sedation, and isoflurane and propofol provided nearly identical sedation depths. The mean fraction of time spent spontaneously breathing was 82% [95% CI: 69, 90] in patients sedated with isoflurane compared to 35% [95% CI: 22, 51] in those assigned to propofol: median difference: 61% [95% CI: 14, 89], p < .001. After adjustments for sufentanil dose and arterial carbon dioxide partial pressure, patients sedated with isoflurane were twice as likely to breathe spontaneously than those sedated with propofol: adjusted risk ratio: 2.2 [95%CI: 1.4, 3.3], p < .001., Conclusions: Isoflurane compared to propofol sedation promotes early spontaneous breathing in deeply sedated ventilated intensive care patients. New Findings: What is the central question of the study? Does respiratory support ensure blood gas homeostasis and the relevance of experimental outcomes? What is the main finding and its importance? Spontaneous breathing during surgical intervention under anaesthesia results in impaired gas exchange and loss of diaphragm muscle strength in rats. Subsequent short-term mechanical ventilation restored blood gas homeostasis and diaphragm muscle strength. Blood gas homeostasis interferes substantially with experimental conditions and may alter study results. Monitoring and maintenance of blood gas balance is required to ensure quality and relevance of physiological animal experiments., Abstract: In pre-clinical small animal studies with surgical interventions under general anaesthesia, animals are often left to breathe spontaneously. However, anaesthesia may impair respiratory functions and result in disturbed blood gas homeostasis. In turn, the disturbed blood gas homeostasis can affect physiological functions and thus unintentionally impact the experimental results. We hypothesized that short-term mechanical ventilation restores blood gas balance and physiological functions despite anaesthesia and surgical interventions. Therefore, we investigated variables of blood gas analyses and diaphragm muscle strength in rats anaesthetized with ketamine/medetomidine after tracheotomy and catheterization of the carotid artery under spontaneous breathing and after 20 min of mechanical ventilation following the same surgical intervention. Spontaneous breathing during general anaesthesia and surgical intervention resulted in unphysiological blood oxygen partial pressure (<65 mmHg) and carbon dioxide partial pressure (>55 mmHg). After subsequent short-term mechanical ventilation, blood gas partial pressures were restored to their physiological ranges. Additionally, diaphragm muscle strength of animals breathing spontaneously was lower compared to animals that received subsequent mechanical ventilation (P = 0.0063). We conclude that spontaneous breathing of rats under ketamine/medetomidine anaesthesia is not sufficient to maintain a physiological blood gas balance. Disturbed blood gas balance is related to reduced diaphragm muscle strength. Mechanical ventilation for only 20 min restores blood gas homeostasis and muscle strength. Therefore, monitoring and maintenance of blood gas balance should be conducted to ensure quality and relevance of small animal experiments., (© 2020 The Authors. Background: We investigated the haemodynamic stability of a novel porcine model of lung collapse induced by negative pressure application (NPA). A secondary aim was to study whether pulmonary shunt correlates with cardiac output (CO)., Methods: In 12 anaesthetized and relaxed supine piglets, lung collapse was induced by NPA (-50 kPa). Six animals resumed spontaneous breathing (SB) after 15 min; the other six animals were kept on mechanical ventilation (MV) at respiratory rate and tidal volume (V(T) ) that corresponded to SB. All animals were followed for 135 min with blood gas analysis and detailed haemodynamic monitoring., Results: Haemodynamics and gas exchange were stable in both groups during the experiment with arterial oxygen tension (PaO(2) )/inspired fraction of oxygen (FiO(2) ) and pulmonary artery occlusion pressure being higher, venous admixture (Q(va) /Q(t) ) and pulmonary perfusion pressure being lower in the SB group. CO was similar in both groups, showing slight decrease over time in the SB group. During MV, Q(va) /Q(t) increased with CO (slope: 4.3 %min/l; P < 0.001), but not so during SB (slope: 0.55 %min/l; P = 0.16)., Conclusions: This porcine lung collapse model is reasonably stable in terms of haemodynamics for at least 2 h irrespective of the mode of ventilation. SB achieves higher PaO(2) /FiO(2) and lower Q(va) /Q(t) compared with MV. During SB, Q(va) /Q(t) seems to be less, if at all, affected by CO compared with MV., (© 2012 The Authors. Aim: To provide data on ventilation, oxygenation and acid-base state from birth to 48 h in very preterm infants treated with lung recruitment manoeuvre and nasopharyngeal continuous positive airway pressure in the delivery room., Methods: Subjects of this prospective observational cohort study were 48 of 61 infants enrolled in a randomised controlled trial to test two lung recruitment manoeuvres after birth. The infants had received an arterial line in the delivery room. The outcome measures were data on oxygenation, ventilation and acid-base state during spontaneous breathing., Results: Data are presented as (n [%]; median [minimum-maximum]). 22 of 48 (46%) infants (gestational age, 26.4 [25.0-28.9] weeks; birth weight 870 [540-1310] g) were never intubated during the study. The FiO(2) of these infants was low (0.4 [0.21-0.45] at 45 min and 0.21 [0.21-0.5] at 48 h). PCO(2) reached its maximum at 24 (11-44) min (8 [6.4-10.8] kPa) and decreased below 6.7 kPa (median) within 3 h. The incidence of intracranial haemorrhage/periventricular leukomalacia did not increase with hypercapnia (pCO(2) > 8 kPa)., Conclusion: A transient period of hypercapnia after birth may occur in spontaneously breathing very preterm infants supported with nasopharyngeal continuous positive airway pressure in the delivery room. The incidence of cerebral damage was not increased in infants with hypercapnia.

Background: Automatic tube compensation (ATC) is a new option to support spontaneously breathing tracheally intubated patients. We have previously demonstrated an increased respiratory comfort compared to pressure support ventilation (PSV) in volunteers. Here we characterized the breathing pattern during ATC associated with respiratory comfort in comparison to PSV. Furthermore, we studied whether ATC can be substituted by a simple modification of PSV., Methods: We exposed 10 volunteers breathing through a 7.5 mm endotracheal tube via mouthpiece to PSV with 1) immediate and 2) delayed pressure rise and to 3) ATC. Immediate changes of the respiratory pattern after mode shifts were analyzed in detail. Furthermore, the volunteers were instructed to indicate changes in comfort after transitions between these modes as increased, unchanged, or decreased., Results: Decreased comfort was associated with a substantial increase of tidal volume, minute ventilation, gas flow, and pressure. No differences in respiratory comfort were perceived between immediate and delayed pressure rise during PSV., Conclusion: PSV resulted in excessive tidal volumes and airflow, which was perceived as discomfort. This cannot be avoided by a delayed pressure rise but can be by the more comfortable ATC. ATC seems to adapt better to the ventilatory demand than PSV.

Background: The complications related to anaesthesia usually occur in the early postoperative period. Hypercapnia and hypoxaemia may result from any persistent depression of the respiratory drive relative to the metabolic demand. The purpose of this study was to compare the respiratory effects of desflurane anaesthesia with or without nitrous oxide during the period of emergence., Methods: Twenty patients scheduled for a standardised surgical procedure, laparoscopic hysterectomy, were randomly allocated to anaesthesia with 1.3 MAC of desflurane/N2O (Group 1) or desflurane alone (Group 2), with 10 patients in each group. Times of resumption of spontaneous breathing and extubation were recorded and elimination rates of carbon dioxide, end-tidal concentrations of desflurane and N2O, and blood gases were measured., Results: Spontaneous breathing was resumed in both groups when pH had decreased by about 0.07 and PaCO2 increased by about 1.4 kPa compared with the values at the end of 1.3 MAC anaesthesia with controlled normoventilation. There were no significant differences between the groups with regards to extubation time, 6 vs. 13 min, or total MAC value at extubation, 0.20 vs. 0.19 in Group 1 and 2, respectively. Neither did the groups differ in minute ventilation, end-tidal carbon dioxide, oxygen concentrations, or blood gases. CO2 elimination decreased in both groups from about 220 ml 70 kg-1 min-1 at the end of anaesthesia to a lowest value of about 160 ml 70 kg-1 min-1., Conclusion: The respiratory profiles during recovery from gynaecological laparoscopy with either desflurane/N2O or desflurane anaesthesia were similar with fast resumption of spontaneous breathing, short time to extubation, and no signs of CO2 retention.

The StatCap semiquantitative carbon dioxide detector was evaluated in an experimental setting. The monitor underread at progressively higher concentrations of carbon dioxide. During simulated rebreathing, progressive increases in inspired carbon dioxide resulted in the StatCap readings decreasing. It is recommended that the StatCap be used with caution in circumstances where rebreathing may occur, for example, during manual ventilation with an Ayre's T-piece.

Background: We have developed a combined indirect calorimetric and breath-by-breath capnographic device (GEM) for respiratory monitoring: oxygen consumption (VO2), carbon dioxide excretion (VCO2), respiratory quotient (RQ), energy expenditure (EE), alveolar ventilation (VA) and dead space/total ventilation (VD/VT)., Methods: The device was tested in a lung model in which VO2 was achieved by combustion of hydrogen. VCO2 was achieved by delivering CO2 into the single alveolus combustion chamber. VO2, VCO2, compliance, and anatomical dead space could be varied independently., Results: Measured VO2 was 101 +/- 3% (SD) of set value at a F1O2 < 0.6 and 101 +/- 7% at a F1O2 > 0.6 during 15 hours of testing. The corresponding VCO2 values were 99 +/- 2% and 102 +/- 7%. The GEM could with good accuracy measure accumulated energy expenditure (EE) during simulated unstable patient conditions up to a F1O2 of 0.8. At F1O2 above 0.8 VCO2 and VO2 could be estimated using a default RQ value of 0.85. On-line estimated VA and VD/VT values could be obtained at any F1O2 up to 1.0. In a test sequence with stable VO2 and VCO2 the GEM adequately followed changes in VA, induced by changes in anatomical dead space, breathing frequency and compliance., Conclusion: The overall performance of the device is satisfactory and well comparable with any equipment tested. It allows near-continuous non-invasive monitoring of EE, VO2, VCO2, VA, VD/VT in ventilated, critically ill patients, providing a rationale for ventilator settings and nutritional support.

Background: During laparoscopic cholecystectomy the arterial-end-tidal CO2 gradient (Fa-ETCO2) has been variously shown to be unchanged, increased, decreased or even negative. The goal of this study was to evaluate Fa-ETCO2, and to determine the proper contribution of VECO2 and VA in regard to the increase of FETCO2., Methods: Ventilatory patterns were studied in 15 ASA 1-2 patients (mean age +/- SD: 48.5 +/- 15.0) undergoing laparoscopic cholecystectomy, with intraperitoneal CO2 insufflation limited to 12 mmHg, 15 degrees head-up position, during general anaesthesia and controlled ventilation. The following were studied before, during and after the pneumoperitoneum: FaCO2, FETCO2, nasopharyngeal temperature; dead space ventilation, and expired volumes using the Single Breath Test for CO2. VA was calculated as the alveolar fraction of expired VT multipled by the respiratory frequency., Results: During pneumoperitoneum it is shown that: 1) Fa-ETCO2 either decreases and becomes even negative (n = 8) (P < 0.01), or stays unchanged (n = 7), but never elevates; 2) VECO2 increases (peak value: +22.6%) (P < 0.01); 3) VA is unchanged, and 4) dead space ventilation, determined in 7 patients, remains unchanged., Conclusion: We conclude that only exogenous CO2 loading, and not VA, can explain such increase in FETCO2 and FaCO2, in cases of limited CO2 insufflating pressure in ASA 1-2 patients.

Twenty patients, scheduled for surgical resection of thoracoabdominal aortic aneurysm were divided into two groups according to the type of differential lung ventilation used during graft replacement of the descending thoracic aorta. In the high-frequency jet ventilation (HFJV) group of ten patients, HFJV was applied to the left lung once collapsed and retracted by the surgeon, the patient lying in the right lateral decubitus and being intubated by a Carlens' tube. In the continuous positive airway pressure (CPAP) group of ten patients, CPAP was applied to the left lung at the same mean airway pressure as HFJV (1 kPa). Before anaesthetic induction, an arterial and a Swan-Ganz catheter were inserted for cardiovascular monitoring. The same anaesthetic technique using fentanyl 6 micrograms.kg-1, flunitrazepam 0.02 mg.kg-1 and pancuronium 0.1 mg.kg-1 was used for each patient. Haemodynamic and respiratory measurements were made; 15 min after positioning the patients in the right lateral decubitus using two-lung ventilation; 15 min after collapse and retraction of the left lung using one-lung ventilation and 15 min after using differential lung ventilation with CPAP or HFJV. Left lung collapse with conventional one-lung ventilation induced a dramatic decrease in arterial oxygenation: PaO2/FIO2 ratio decreased from 43 +/- 6 kPa to 20 +/- 8 kPa, alveolo-arterial oxygen difference increased from 24 +/- 7 kPa to 72 +/- 11 kPa and pulmonary shunt increased from 17 +/- 2% to 37 +/- 3%. Whereas differential lung ventilation with CPAP did not improve any of the respiratory parameters measured, differential lung ventilation with HFJV, significantly increased PaO2/FIO2 ratio to 41 +/- 14 kPa.(ABSTRACT TRUNCATED AT 250 WORDS)

The effects of inverse ratio ventilation (IRV) and PEEP on dynamic hyperinflation and auto-PEEP were studied in sedated, paralysed patients with adult respiratory distress syndrome (ARDS) (n = 9) and in 10 postoperative patients after coronary artery by-pass (CABG). During volume-controlled mechanical ventilation with constant tidal volume (V(T) 12 ml.kg-1) and respiratory rate (12.min-1), two consecutive experiments were carried out: (1) with constant I:E ratio PEEP was increased in steps of 2 cmH2O (0.2 kPa) from 0 to 12 cmH2O (0 to 1.2 kPa) and (2) with no PEEP I:E ratio was changed stepwise from 1:4 to 4:1. Flow, V(T), peak airway pressure (Pmax) and static end-expiratory pressure (PEEPtot) were registered. PEEPtot was measured while occluding the airway in the end-expiration. The changes in the end-expiratory lung volume (EELV) were measured with respiratory inductive plethysmograph. We found that: (1) increasing PEEP and IRV caused a similar increase in EELV and PEEPtot in ARDS, but in CABG the increase in EELV was greater with PEEP; (2) at the same PEEPtot the increase in EELV was similar with PEEP and IRV in both groups; and (3) the reduction in Pmax was marginal during IRV. We conclude that the effect of reduced expiratory time on end-expiratory lung volume and pressure during volume controlled IRV is similar to the use of PEEP.

In order to test the hypothesis whether the breathing pattern is helpful in predicting weaning outcome in patients being weaned from mechanical ventilation, 38 patients who underwent operation for esophageal cancer were evaluated at weaning from mechanical ventilation (19 unsuccessful weanings, group U, and 19 successful weanings in age-matched patients, group S). Since all patients initially fulfilled our weaning criteria, ventilatory parameters such as tidal volume, respiratory frequency, minute ventilation, and arterial blood gas analysis showed no significant differences between the groups. The breathing pattern was registered quantitatively by means of respiratory inductive plethysmography at 3 cmH2O (0.3 kPa) of CPAP prior to weaning. The contribution of rib cage movement to tidal volume (%RC) was significantly greater in group U than in group S (P < 0.05). Indeed, 84% of the patients in group S showed %RC less than 50%, compared to only 16% of the patients in group U (P < 0.05). The results suggest that the breathing pattern is one important factor in predicting the outcome of weaning in patients after thoraco-abdominal surgery. Diaphragmatic fatigue is suspected to be the mechanism for the increase in the RC component in patients with unsuccessful weaning outcome.

The Extensometer is a new device capable of continuous accurate measurement of length over convex surfaces with a rapid response rate. This makes it a powerful research tool and a useful clinical instrument, particularly in the field of respiratory monitoring. This paper describes the principles behind the device. The linearity of its response was demonstrated in laboratory tests and its ability to quantify abdominal and chest wall movement was assessed in anaesthetised patients. Potential applications in the field of ventilatory pattern analysis and respiratory monitoring in anaesthesia and intensive care are discussed.

In order to investigate the importance of different inspiratory gas flow patterns in respirator treatment, eight intensive care patients were studied with breathing mechanics and five patients also with gas exchange studies. Three different inspiratory gas flow patterns were tested in randomized sequences namely, accelerating, constant and decelerating flow. All three flow patterns were generated by the same respirator. No end-inspiratory pause was used. The results point to a favourable effect on breathing mechanics of a decelerating and a constant flow when compared with an accelerating flow type. However, when the total effects on gas distribution and lung perfusion were evaluated in the gas exchange studies, no significant differences were seen between the three flow patterns.

Twenty patients, 36-78 years old, with a history of chronic obstructive lung disease, and immediately after operation mostly for cardiac disease, were treated with ipratropium bromide (IB, 15 cases) or placebo (5 cases). A metered dose inhaler with a new adapter was used during postoperative ventilator treatment. In the IB group the heart rate did not change, but the inspiratory resistance decreased and the arterial oxygen tension increased. This is considered to indicate an effect not only on large but also on small airways, or an improvement of the ventilation/perfusion relationship. The investigation also demonstrated the practical usefulness of the new adapter.

Semi-closed anaesthetic circuits are converted into the corresponding Hafnia circuits by replacing the expiratory valve by a side tube connected to an ejector flowmeter. Theoretical analysis of the Hafnia A and D circuits revealed by the fresh gas flow requirements are dependent on the inspiration/expiration time ratio. Using a ratio of 1/1.2 and a sine-wave respiratory waveform, the minimal fresh gas requirements were calculated as 2.1 (Hafnia A) and 2.5 (Hafnia D) times the respiratory minute volume. The fresh gas requirements are identical with spontaneous or controlled ventilation.

The aim of the study was to compare the effect of halothane anaesthesia on sympathetic nerve discharge in mechanically normoventilated and spontaneously breathing rats. Renal sympathetic nerve activity (rSNA), mean arterial pressure (MAP) and heart rate (HR) were measured in the conscious state and at the inspiratory halothane concentrations of 0.6%, 1.2% and 2.4% in one mechanically normoventilated and one spontaneously breathing group, while a third group was subjected to controlled hypoventilation at 1.2% halothane concentration. Halothane in blood was determined in two separate groups at 1.2%. In an additional group of spontaneously breathing rats, PaCO2 was analysed during consciousness and the halothane concentrations of 1.2% and 2.4%. There was a pronounced decrease in rSNA, MAP and HR at all levels of anaesthesia in the mechanically ventilated rats. However, rSNA, HR and MAP were significantly higher in the spontaneously breathing rats at increasing levels of halothane anaesthesia. Controlled hypoventilation at 1.2% halothane increased the variables significantly. In spontaneously breathing animals, PaCO2 increased significantly during the halothane exposure. The concentration of halothane in blood was significantly higher in the spontaneously breathing rats. Thus, the halothane-induced respiratory depression in the spontaneously breathing rats preserved rSNA during halothane anaesthesia, possibly via CO2-mediated chemoreceptor stimulation.

Two patients who had undergone emergency tracheal intubation developed signs of aspiration of ingested food. Nasendoscopy demonstrated supraglottic anaesthesia which recovered from the laryngeal margins in towards the vocal cords. It is assumed that this was caused by a neuropraxia of the internal branch of the superior laryngeal nerve, presumably as a result of trauma related to intubation.

Intrapulmonary gas distribution was studied in 10 extremely obese patients: (1) during spontaneous breathing awake; (2) during anaesthesia with controlled ventilation and zero end-expiratory pressure (ZEEP), and (3) as under (2) but with a positive end-expiratory pressure of approximately 15 cmH2O (PEEP). Gas distribution was assessed quantitatively by means of a multiple-breath nitrogen wash-out technique and subsequent fractional analysis, which permitted the calculation of nitrogen wash-out delay (NWOD). Gas distribution was also analyzed by means of a single-breath nitrogen wash-out in order to determine the slope of the "alveolar plateau." Gas distribution was within normal limits during spontaneous breathing, judged from multiple-breath as well as single-breath wash-out. With anaesthesia and ZEEP, NWOD was higher, indicating less efficient gas mixing, and the slope of the "alveolar plateau" was twice as steep as during spontaneous breathing. With PEEP, distribution of inspired gas improved (lowered NWOD and flatter slope). Theoretical considerations and clinical experiments led to the conclusion that uneven distribution in the anaesthetized obese is caused both by regional differences in the pulmonary time constants (as in obstructive lung disease) and by airway closure.

This is a review of previous studies on the effects of variations between 12 and 24 breaths per minute in ventilation frequency during artificial ventilation, minute ventilation being constant. The total material consisted of 66 healthy subjects investigated under anaesthesia with artificial ventilation and 43 patients investigated during prolonged respiratory treatment. An increase in ventilation frequency resulted in an increased ratio of dead space to tidal volume (VD/VT) and in diminished alveolar ventilation with a subsequent elevation of Paco2. The pressures in airways and alveoli were lowered. The dynamic compliance both of the lungs and of the chest wall was diminished and inspiratory resistance was slightly reduced. Intrapulmonary gas distribution was unaffected. Cardiac output was increased, as was venous admixture. Pao2 was slightly reduced. The more efficient gas exchange occurring at a low ventilation frequency makes this setting preferable in the treatment of patients with chronic obstructive lung disease, whereas a high ventilation frequency, by improving cardiac output, may be advantageous in patients with circulatory failure.

Intrapulmonary gas distribution, assessed by a multiple breath, nitrogen washout technique and expressed by the Fowler index NWOD, was studied in subjects with healthy lungs during spontaneous breathing while awake, and during mechanical ventilation under halothane anaesthesia. The distribution index rose from a mean of 32% awake to 56% during anaesthesia. An attempt was also made to differentiate between the contribution of gravitational (airway closure) and non-gravitational (diffuse airway obstruction) inhomogeneity of ventilation. This was accomplished by measurement of the slope of the alveolar plateau during a sinle breath nitrogen washout. The slope was similar awake and anaesthetized, and it increased equally under both conditions, when preinspiratory lung volume was stepped up from RV to CC, CC being the lung volume at which airways begin to close during expiration. NWOD was significantly correlated to the degree of airway closure, expressed as FRC-CC, and, less so, to inspiratory resistance. It is suggested that a less efficient ventilation distribution in anaesthetized normal subjects, as measured by NWOD, is caused rather by airway closure (gravitational inhomogeneity) than by diffuse airway obstruction (non-gravitational inhomogeneity of ventilation).

Ten patients without known cardiac or respiratory disease were investigated with breathing mechanics and gas exchange studies during anaesthesia and artifical ventilation. The effects of three different inspiratory gas flow patterns, namely, accelerating, constant and decelerating flows were studied. A decelerating flow resulted in an increase of total compliance when compared to an accelerating or a constant flow. However, at the same time, there was an increase in physiological dead space and a decrease in alveolar ventilation with a decelerating flow compared to an accelerating flow. These results seem to indicate an improved gas distribution in the greater airways with a decelerating flow pattern, but when the total effects of gas exchange were judged, the greatest benefits were with an accelerating flow.

Breathing mechanics and gas exchange were studied in 10 extremely obese subjects (average weight 138 kg) prior to and during anaesthesia with mechanical ventilation. Breathing mechanics were analysed from measurements of transpulmonary pressure (during anaesthesia, trans-chest wall pressure as well) inspiratory gas flow and tidal volume. Gas exchange was studied by analysing inspired and from the Bohr equation, and the division into anatomical and alveolar dead space was arrived at by capnography. The patients were anaesthetised with neuroltpt agents and ventilated with an air-oxygen mixture. Lung compliance during spontaneous breathing was below normal and decreased further during artificial ventilation. Chest wall compliance measured during anaesthesia was within normal limits. Lung resistance was above normal during spontaneous breathing and increased further during mechanical ventilation. Total dead space was normal during spontaneous breathing and increased moderately during artificial ventilation, the increment coming mainly from alveolar dead space. A moderate hypoxaemia was recorded during spontaneous breathing, and the alveolar-arterial oxygen tension difference was slightly elevated. During anaesthesia this difference was markedly greater. It is concluded that the most probable reason for the relative hypoxaemia is right-to-left shunting.

The effect of continuous positive airway pressure during continuous mechanical (CMV + PEEP) and spontaneous (CPAP) ventilation on central haemodynamics and systemic oxygen transport was studied in 10 male patients who had undergone aortocoronary bypass graft operation 18 h earlier. With the change from CMV + PEEP 5 cmH2O to CPAP 5 cmH2O, cardiac index was found to increase from 2.58 +/- 0.44 (s.e. mean) to 2.88 +/- 0.19 l/min/m2 (P less than 0.005), and systemic oxygen transport improved from 8.5 +/- 0.6 to 9.5 +/- 1.0 ml/min/kg (P less than 0.05). Arterial oxygen tension and content did not change, but mixed venous blood oxygen tension increased from 3.5 +/- 0.2 to 4.2 +/- 0.2 kPa (P less than 0.005), reflecting the increase in cardiac output. Arteriovenous oxygen content difference decreased from 4.6 +/- 0.5 (CMV + PEEP) to 3.6 +/- 0.2 (CPAP) ml/100 ml (P less than 0.05), while total oxygen consumption remained unchanged. Mean systemic arterial pressure was found to increase from 10.8 +/- 0.4 to 11.6 +/- 0.4 kPa (P less than 0.05) and mean pulmonary arterial pressure changed from 2.2 +/- 0.1 to 2.4 +/- 0.1 kPa (P less than 0.05). Right atrial and pulmonary capillary wedge pressures did not change. Our observations suggest that, in terms of central haemodynamics and tissue oxygen supply, CPAP offers a noteworthy alternative weaning method and an alternative to CMV + PEEP in cases where therapy is prolonged and the patient is able to breathe spontaneously.

HPS is a life-threatening condition in patients with end-stage liver disease, in which intrapulmonary vascular dilatations result in intrapulmonary shunts and hypoxemia. The only successful treatment is liver transplantation. Hypoxemia may be severe prior to transplantation; however, it can worsen or become refractory after liver transplantation and result in increased post-operative mortality. Here, we present the case of a 10-month-old female infant with progressive end-stage liver disease and severe HPS, who developed refractory hypoxemia after a successful liver transplantation. After 19 days of unsuccessful attempts to reverse the hypoxemia using conventional mechanical ventilation and HFOV, the patient responded dramatically to APRV, with rapid improvement in her Pa O2 and sharp decline in her OI. She was able to begin weaning from APRV two days later and was extubated within seven days. APRV was successful in treating refractory hypoxemia in this patient with severe HPS after liver transplantation, possibly by modifying distribution of pulmonary blood flow. Although we cannot rule out coincidental natural resolution of the HPS, APRV could be a useful rescue therapy in patients with HPS and refractory hypoxemia. [ABSTRACT FROM AUTHOR]
