There is a statutory duty for all practitioners to record information in child protection work in England. Case recording is a daily task for practitioners, yet an under-researched area of practice. This continuing professional development (CPD) paper will consider the context in which case recording takes place and highlight messages from child protection reviews and enquiries before exploring learning from contemporary research. The complexities of case recording will be considered and how practitioners can reflect upon and improve their daily case recording skills. Practitioners are encouraged to keep the child's views and experiences central and consider the long-term impact of case recording on adults who have experienced abuse and neglect in childhood. Key Practitioner Messages • Case recording is a complex and nuanced task, often missing the multiple perspectives inherent in a child's narrative. • The views, experiences and identity of individual children and young people can be lost in case recording, especially for very young children, older children and those in sibling groups. • Case recording has a support function for care experienced adults to understand their childhood histories, aiding recovery from abuse and neglect. This paper investigates the comprehensive compassionate care reform programme within the National Health Service (NHS) in England. Through a synoptic reading of policy documents, we show how 'compassion' is introduced as an overarching meta‐virtue designed to govern relationships and formal positions in health care. Invoking an 'ethics of office' perspective, mainly drawing on the thinking of Max Weber, we evaluate the promotion of compassion as a managerial technology and argue how seemingly humanistic and value‐based approaches to healthcare management might have unintended consequences for the quality of care and the conduct of health professionals that in some ways resemble and in some ways exceed those of the more traditional New Public Management measures, which the new compassion paradigm is expected to outdo. In the paper's final sections, we turn to the original work of the nursing icon Florence Nightingale to argue that compassion and other virtues should continuously be formulated and re‐formulated in relation to the role‐specific skills and duties of particular offices in the healthcare sector. ABSTRACT As there are a number of high-profile public inquiries into child death tragedies, 'information sharing' has now become a moral and political imperative across England and Wales for improving the welfare and protection of children. This paper discusses multi-agency information practices, at the stage of referral, which were observed and documented in the day-to-day practice of child protection work. Drawing on transcribed, professional narrative accounts, a 'jigsaw' metaphor is used to describe the process of piecing information together to ascertain a 'full' picture of children and families lives. However, these accounts highlight that there is something of a mismatch between the jigsaw, as articulated in the conceptual abstract accounts, and jigsaw practices operating on the ground. It is argued that abstracting professional information practices from situated contexts creates impoverished understandings of these practices. Thus, reported findings in this paper highlight the inherent complexities of jigsaw practices in the 'everyday' of child protection work, which challenges objectivist assumptions about a stability of meaning, and further highlight that the 'endpoint' of reaching a 'full' picture of a child's life is not fixed, nor does it have the same meaning for all professionals, but rather it is a complex process involving sense-making, translation in context and organizational relevance. Abstract: The present position paper provides an account of the life of Martha Entwistle, the earliest recorded convict nurse who worked within the Castle Hill Asylum. In our review of primary historical sources, Australia's first convict mental health nurse was found to be a resilient woman who endured several traumatic life experiences. Her nursing within Australia's first mental health asylum was highly valued by the superintendent of the service. She nursed in a harsh colonial environment, short of adequate resources, during an era of fast‐paced industrial and technological a change. Martha Entwistle's experiences provide a historical account of the role of the early convict mental health nurse. Her life story enables modern‐day nurses to reflect on the advances made in mental health nursing and contexts of care for nurses today. [ABSTRACT FROM AUTHOR]

Written records belie the complexity of social work practice. And yet, keeping good records is a key function for social workers in England (and elsewhere). Written records provide a future reference point for children, especially those in public care. They are foundational for the inspection of children's services. They provide practitioners and managers with an opportunity to record their thinking and decisions. They add to result from and cause much of the bureaucratic maze that practitioners have to navigate. As part of a wider study of child and family social work practice, this paper describes an analysis of more than 200 written records of supervision. These records primarily contain narrative descriptions of activity, often leading to a set of actions for the social worker to complete - what they should do next. Records of why these actions are necessary and how the social worker might undertake them are usually absent, as are records of analytical thinking or the child's views. This suggests that written records of supervision are not principally created in order to inform an understanding of the social work decision-making process; rather, they are created to demonstrate management oversight of practice and the accountability of the practitioner. Background and aims Regulation of electronic cigarettes has moved to the top of the addiction policy agenda, as demonstrated by the recent focus across the United Kingdom on introducing age-of-sale restrictions. However, the views of those affected by such regulation remain largely unexplored. This paper presents the first detailed qualitative exploration of adolescents' perceptions of existing, and opinions about potential e-cigarette regulation. Methods Sixteen focus groups, including a total of 83 teenagers between the ages of 14 and 17 years, were conducted in deprived, mixed and affluent urban areas in Scotland and England between November 2014 and February 2015. Transcripts were imported into Nivivo 10, coded thematically and analysed. Results Participants critically considered existing evidence and competing interests in regulatory debates and demonstrated sophisticated understanding of the advantages and disadvantages of regulation. They overwhelmingly supported strong e-cigarette regulation and endorsed restrictions on sales to minors, marketing and e-cigarette use in public places. Concern about potential health harms of e-cigarette use and marketing increasing the acceptability of vaping and smoking led these adolescents to support regulation. Conclusions In focus group discussions, a sample of UK adolescents exposed to particular communications about e-cigarettes supported strict regulation of e-cigarettes, including banning sales to minors and use in indoor public areas. Stark, widening health and income inequalities in the United Kingdom underpin the need for increased support for low‐income families to access affordable and nutritious foods. Using anonymised supermarket loyalty card transaction records, this study aimed to assess how an additional Healthy Start voucher (HSV) top‐up of £2, redeemable only against fruit and vegetables (FVs), was associated with FV purchases among at‐risk households. Transaction and redemption records from 150 loyalty card‐holding households, living in northern England, who had engaged with the top‐up scheme, were analysed to assess the potential overall population impact. Using a pre‐post study design, 133 of these households' records from 2021 were compared with equivalent time periods in 2019 and 2020. Records were linked to product, customer and store data, permitting comparisons using Wilcoxon matched‐pairs sign‐ranked tests and relationships assessed with Spearman's Rho. These analyses demonstrated that 0.9 more portions of FV per day per household were purchased during the scheme compared to the 2019 baseline (p = 0.0017). The percentage of FV weight within total baskets also increased by 1.6 percentage points (p = 0.0242), although the proportional spend on FV did not change. During the scheme period, FV purchased was higher by 0.4 percentage points (p = 0.0012) and 1.6 percentage points (p = 0.0062) according to spend and weight, respectively, in top‐up redeeming baskets compared to non‐top‐up redeeming baskets with at least one FV item and was associated with 5.5 more HSV 'Suggested' FV portions (p < 0.0001). The median weight of FV purchased increased from 41.83 kg in 2019 to 54.14 kg in 2021 (p = 0.0017). However, top‐up vouchers were only redeemed on 9.1% of occasions where FV were purchased. In summary, this study provides novel data showing that safeguarding funds exclusively for FV can help to increase access to FV in low‐income households. Background: Annually in England, over 1.5 million children and young people (CYP) are admitted to hospital. However, a proportion of these CYP will experience failure to rescue (FtR), a failure to recognize, respond and escalate clinical deterioration, which can result in significant harm or death. Aim: To identify and quantify FtR episodes from emergency events at a 110‐bedded tertiary children's hospital located within a University Teaching Hospital and evaluate the impact of targeted interventions on reducing FtR. Methods: A quality improvement approach was adopted. From 170 446 patients admitted between 2011 and 2019, all emergency event calls were systematically reviewed to identify FtR episodes. Root–cause analysis was performed to identify practice deficiencies. The Plan‐Do‐Study‐Act fundamentals were used. Results: A total of 520 emergency events were reviewed over the 9‐year period. One hundred and thirty‐two (n = 132; 25%) were cardiac arrest events, with the majority occurring within the PCCU setting. Three hundred and twelve (60%) of the events were in children who had been inpatient for more than 48 hours. FtR trend declined over the study period from 23.6% in 2011 when the project commenced to 2.5% or less over the following 8 years. Conclusions: Identifying rates of FtR events from routinely collected emergency events data can be used as a patient safety measure to identify emergency concerns. This enables dynamic problem solving through delivery of strategic and targeted interventions. The proposed interventions outlined in this quality improvement study have application to critical care nursing as mechanisms for reducing unplanned admissions to paediatric critical care unit (PCCU), patient mortality, and PCCU and non‐PCCU cardiac arrests. Relevance to Clinical Practice: This study emphasises the importance in understanding the antecedence of emergency events for paediatric inpatient populations. In family settings stories, photographs and memory objects support narratives of identity and belonging. Such resources are often missing for people who were in care as children. As a result, they may be unable to fill gaps in their memories or answer simple questions about their early lives. In these circumstances, they turn to the records created about them by social workers and care providers to reconstruct personal histories. Research suggests that thousands of requests to view records for this purpose are made each year in England under the subject access provisions of data protection legislation. This article reports the findings of MIRRA, a participatory research project on the memory and identity dimensions of social care recordkeeping. Drawing on data collected during interviews and focus groups with adult care leavers, the study explores the motives and experiences of care‐experienced people who access their records in England. Findings show the practical and cultural challenges they face when doing so and the resulting impacts on well‐being. The study suggests that the development of person‐centred approaches to recordkeeping in social work, which focus on the perspectives and experiences of the individual, could better support the lifelong memory and identity needs of care‐experienced people. Abstract: Background: UK Hospital Trusts are charged with increasing patients’ research awareness and willingness to take part in research. This includes implementing strategies to encourage patient‐initiated enquiries about participation. Objectives: To evaluate the impact of a research statement inserted in outpatient letters in one clinical service, and to derive suggestions on potential steps towards increasing patient‐initiated recruitment. Setting: A medical outpatient clinic of a research‐active hospital trust, serving an inner‐city multi‐ethnic population across two boroughs. Methods: Pre‐intervention and post‐intervention questionnaires were administered face‐to‐face to new patients. Questionnaires included closed questions and one open comments section. Data were analysed for frequencies, with thematic coding of open‐ended responses. Results: The response rates were 87% for the pre‐intervention survey and 92% for the post‐intervention survey. In the post‐intervention survey, 85% of patients did not notice the research statement in the letter. More than half found the statement “a little unclear,” whilst one‐third considered it “clear.” Three‐quarters of respondents perceived the statement to be “a little helpful.” Only one person enquired about participating in clinical research having read the statement in the outpatient letter. Conclusion: The analysis suggests that simple, single‐solution approaches such as including research statements in outpatient letters are unlikely to be sufficient to significantly facilitate patient‐initiated recruitment. Recruitment efforts need to take into consideration the diversity of patient constituencies including the reasons they seek health care, and how patients can meaningfully access information (research literacy). Aim: The aim of this research was to investigate newly qualified nurses (NQN) experiences of starting their career in the intensive care unit (ICU). The author also investigated the opinions of senior nurses from ICU in relation to NQN in ICU. Background: Although not a new concept, NQN in ICU has little substantive research to support recruitment. There is a current demand to investigate this topic, because of retention of staff and a shortage of qualified nurses. Method: This was a comparative, qualitative study using two forms of data collection. This study was conducted over two phases. Phase I, used semi-structured interviews with eight NQN, with one participant for the pilot study. PHASE II used a focus group to interview seven senior nurses in ICU. Findings: NQN experienced anxiety about time management, accountability and socialization. Senior nurses felt NQN coped well with the demanding aspects of ICU but were aware that preceptors are under a lot of pressure. They were concerned about NQN lack of hospital knowledge. The competency-based practice highlighted the importance of record keeping and its associated legal implications. Conclusion: NQN cope well with the complexity of ICU. Having student placements in this area seem to ease this transition and reduce some 'reality shock'. They therefore seem better equipped to deal with the steep learning curve. [ABSTRACT FROM AUTHOR]

Rationale, aims and objectives Extraction of prescribing data from medical records is a common, albeit flawed, research method. Yet little is known about the processes that result in those data. This study explores the creation and use of prescribing documentation in the medical record, from the perspective of the hospital doctors who both create and use it. Methods Thirty-six hospital doctors were purposively selected for qualitative interviews, giving a maximum variability sample of grades of doctors across the range of major medical specialty areas and medical teams at a large teaching hospital in England. Results The findings suggest a number of reasons why hospital doctors fail to record prescribing decisions in the medical record. There was no set standard, record keeping was not formally taught and the hurried environment of the ward gave little time for documentation. The doctors also acknowledged that there was no need for completeness, as colleagues would be able to ‘fill in the gaps’ via an inferential process. Assumptions were made and although this was not seen as ideal, it was recognized as necessary if work was to be done efficiently. Conclusion These results reinforce the suggestion that, despite the large number of potential users, the medical record is created for those with the right privileged knowledge. This has profound implications for those without that insider knowledge who are using medical records for research purposes. The article presents information about the ASLIB Cranfield research project, which began in late 1957. The project was an investigation into the relative retrieval efficiency of four forms of indexing: the universal decimal classification as used in a classified catalog, an alphabetical subject heading system, a faceted classification and the uniterm system used in its original form as a free association of elemental terms. The material involved in the test comprised 18,000 documents from the collection of the College of Aeronautics, Cranfield, England.
