Glaucoma is the leading reason for irreversible blindness. Early detection and timely treatment of glaucoma are essential for preventing visual field loss or even blindness. In clinical practice, Optical Coherence Tomography (OCT) and Visual Field (VF) exams are two widely-used and complementary techniques for diagnosing glaucoma. OCT provides quantitative measurements of the optic nerve head (ONH) structure, while VF test is the functional assessment of peripheral vision. In this paper, we propose a Deep Relation Transformer (DRT) to perform glaucoma diagnosis with OCT and VF information combined. A novel deep reasoning mechanism is proposed to explore implicit pairwise relations between OCT and VF information in global and regional manners. With the pairwise relations, a carefully-designed deep transformer mechanism is developed to enhance the representation with complementary information for each modal. Based on reasoning and transformer mechanisms, three successive modules are designed to extract and collect valuable information for glaucoma diagnosis, the global relation module, the guided regional relation module, and the interaction transformer module, namely. Moreover, we build a large dataset, namely ZOC-OCT&VF dataset, which includes 1395 OCT-VF pairs for developing and evaluating our DRT. We conduct extensive experiments to validate the effectiveness of the proposed method. Experimental results show that our method achieves 88.3% accuracy and outperforms the existing single-modal approaches with a large margin. Objective: Glaucoma is an irreversible chronic eye disease that leads to vision loss. As it can be slowed down through treatment, detecting the disease in time is important. However, many patients are unaware of the disease because it progresses slowly without easily noticeable symptoms. Currently, there is no effective method for low-cost population-based glaucoma detection or screening. Recent studies have shown that automated optic nerve head assessment from 2-D retinal fundus images is promising for low-cost glaucoma screening. In this paper, we propose a method for cup to disc ratio (CDR) assessment using 2-D retinal fundus images. Methods: In the proposed method, the optic disc is first segmented and reconstructed using a novel sparse dissimilarity-constrained coding (SDC) approach which considers both the dissimilarity constraint and the sparsity constraint from a set of reference discs with known CDRs. Subsequently, the reconstruction coefficients from the SDC are used to compute the CDR for the testing disc. Results: The proposed method has been tested for CDR assessment in a database of 650 images with CDRs manually measured by trained professionals previously. Experimental results show an average CDR error of 0.064 and correlation coefficient of 0.67 compared with the manual CDRs, better than the state-of-the-art methods. Our proposed method has also been tested for glaucoma screening. The method achieves areas under curve of 0.83 and 0.88 on datasets of 650 and 1676 images, respectively, outperforming other methods. Glaucoma is a chronic eye disease that leads to irreversible vision loss. The Cup-to-Disc Ratio (CDR) serves as the most important indicator for glaucoma screening and plays a significant role in clinical screening and early diagnosis of glaucoma. In general, obtaining CDR is subjected to measuring on manually or automatically segmented optic disc and cup. Despite great efforts have been devoted, obtaining CDR values automatically with high accuracy and robustness is still a great challenge due to the heavy overlap between optic cup and neuroretinal rim regions. In this paper, a direct CDR estimation method is proposed based on the well-designed semi-supervised learning scheme, in which CDR estimation is formulated as a general regression problem while optic disc/cup segmentation is cancelled. The method directly regresses CDR value based on the feature representation of optic nerve head via deep learning technique while bypassing intermediate segmentation. The scheme is a two-stage cascaded approach comprised of two phases: unsupervised feature representation of fundus image with a convolutional neural networks (MFPPNet) and CDR value regression by random forest regressor. The proposed scheme is validated on the challenging glaucoma dataset Direct-CSU and public ORIGA, and the experimental results demonstrate that our method can achieve a lower average CDR error of 0.0563 and a higher correlation of around 0.726 with measurement before manual segmentation of optic disc/cup by human experts. Our estimated CDR values are also tested for glaucoma screening, which achieves the areas under curve of 0.905 on dataset of 421 fundus images. We present a novel model for the mammalian retina and analyze its behavior. Our outer retina model performs band- pass spatiotemporal filtering. It is comprised of two reciprocally connected resistive grids that model the cone and horizontal cell syncytia. We show analytically that its sensitivity is proportional to the space-constant-ratio of the two grids while its half-max response is set by the local average intensity. Thus, this outer retina model realizes luminance adaptation. Our inner retina model performs high-pass temporal filtering. It features slow negative feed- back whose strength is modulated by a locally computed measure of temporal contrast, modeling two kinds of amacrine cells, one narrow-field, the other wide-field. We show analytically that, when the input is spectrally pure, the corner-frequency tracks the input frequency. But when the input is broadband, the corner frequency is proportional to contrast. Thus, this inner retina model realizes temporal frequency adaptation as well as contrast gain control. We present CMOS circuit designs for our retina model in this paper as well. Neuromodulation technologies are progressing from pacemaking and sensory operations to full closed-loop control. In particular, optogenetics-the genetic modification of light sensitivity into neural tissue allows for simultaneous optical stimulation and electronic recording. This paper presents a neural interface application-specified integrated circuit (ASIC) for intelligent optoelectronic probes. The architecture is designed to enable simultaneous optical neural stimulation and electronic recording. It provides four low noise (2.08  μV) recording channels optimized for recording local field potentials (LFPs) (0.1-300 Hz bandwidth, 5 mV range, sampled 10-bit@4 kHz), which are more stable for chronic applications. For stimulation, it provides six independently addressable optical driver circuits, which can provide both intensity (8-bit resolution across a 1.1 mA range) and pulse-width modulation for high-radiance light emitting diodes (LEDs). The system includes a fully digital interface using a serial peripheral interface (SPI) protocol to allow for use with embedded controllers. The SPI interface is embedded within a finite state machine (FSM), which implements a command interpreter that can send out LFP data whilst receiving instructions to control LED emission. The circuit has been implemented in a commercially available 0.35  μm CMOS technology occupying a 1.95 mm 1.10 mm footprint for mounting onto the head of a silicon probe. Measured results are given for a variety of bench-top, in vitro and in vivo experiments, quantifying system performance and also demonstrating concurrent recording and stimulation within relevant experimental models.

Glaucoma is a serious ocular disorder for which the screening and diagnosis are carried out by the examination of the optic nerve head (ONH). The color fundus image (CFI) is the most common modality used for ocular screening. In CFI, the central region which is the optic disc and the optic cup region within the disc are examined to determine one of the important cues for glaucoma diagnosis called the optic cup-to-disc ratio (CDR). CDR calculation requires accurate segmentation of optic disc and cup. Another important cue for glaucoma progression is the variation of depth in ONH region. In this paper, we first propose a deep learning framework to estimate depth from a single fundus image. For the case of monocular retinal depth estimation, we are also plagued by the labeled data insufficiency. To overcome this problem we adopt the technique of pretraining the deep network where, instead of using a denoising autoencoder, we propose a new pretraining scheme called pseudo-depth reconstruction, which serves as a proxy task for retinal depth estimation. Empirically, we show pseudo-depth reconstruction to be a better proxy task than denoising. Our results outperform the existing techniques for depth estimation on the INSPIRE dataset. To extend the use of depth map for optic disc and cup segmentation, we propose a novel fully convolutional guided network, where, along with the color fundus image the network uses the depth map as a guide. We propose a convolutional block called multimodal feature extraction block to extract and fuse the features of the color image and the guide image. We extensively evaluate the proposed segmentation scheme on three datasets- ORIGA, RIMONEr3, and DRISHTI-GS. Flicker annoyance measurement, regardless if it is performed by processing the supplying voltage or by evaluating the light emitted by the lamp, relies on the assumption of a proper model of the behavior of the complex eye-brain system in the presence of fluctuating light. The only experimental data available are the ones obtained, for a certain incandescent lamp, by asking several people to report their perception about the flicker to which they were subjected. In this paper, the possibility of achieving an "objective" evaluation of the flicker effects on a human subject is investigated. Flickermeters provide correct measurements only when the source of flicker is an incandescent filament lamp. Due to the rapid proliferation of modern lamps, there is the need to make available flickermeters that will quantify the degree of annoyance caused by any kind of lamp. It is widely accepted by the scientific community that the preferable way to reach this goal is to directly measure the emitted light instead of the voltage supplying the lamp itself (as implemented by the actual flickermeters). This paper presents the results of a study aimed at modeling the eye-brain chain in order to predict the response, and hence the sensation, of a human being to a given luminous stimulus. The model has been experimentally tested with the standard input signals recommended for testing existing flickermeters. Preliminary test results are encouraging. The authors believe that the presented theoretical model may serve as a kernel for defining a general mathematical model for predicting the response of the eye-brain system to generic luminous fluxes of known spectrum and modulation. Seeking to match the brain's computational efficiency [14], we draw inspiration from its neural circuits. To model the four main output (ganglion) cell types found in the retina, we morphed outer and inner retina circuits into a 96 × 60-photoreceptor, 3.5 × 3.3 mm², 0.35 μm-CMOS chip. Our retinomorphic chip produces spike trains for 3600 ganglion cells (GCs), and consumes 62.7 mW at 45 spikes/s/GC. This chip, which is the first silicon retina to successfully model inner retina circuitry, approaches the spatial density of the retina. We present experimental measurements showing that the chip's subthreshold current-mode circuits realize luminance adaptation, bandpass spatiotemporal filtering, temporal adaptation and contrast gain control. The four different GC outputs produced by our chip encode light onset or offset in a sustained or transient fashion, producing a quadrature-like representation. Reliable and efficient optic disk localization and segmentation are important tasks in automated retinal screening. General-purpose edge detection algorithms often fail to segment the optic disk due to fuzzy boundaries, inconsistent image contrast or missing edge features. This paper presents an algorithm for the localization and segmentation of the optic nerve head boundary in low-resolution images (about 20 μ/pixel). Optic disk localization is achieved using specialized template matching, and segmentation by a deformable contour model. The latter uses a global elliptical model and a local deformable model with variable edge-strength dependent stiffness. The algorithm is evaluated against a randomly selected database of 100 images from a diabetic screening programme. Ten images were classified as unusable; the others were of variable quality. The localization algorithm succeeded on all bar one usable image; the contour estimation algorithm was qualitatively assessed by an ophthalmologist as having Excellent-Fair performance in 83% of cases, and performs well even on blurred images. Glaucoma is the second leading cause of blindness worldwide. Often, the optic nerve head (ONH) glaucomatous damage and ONH changes occur prior to visual field loss and are observable in vivo. Thus, digital image analysis is a promising choice for detecting the onset and/or progression of glaucoma. In this paper, we present a new framework for detecting glaucomatous changes in the ONH of an eye using the method of proper orthogonal decomposition (POD). A baseline topograph subspace was constructed for each eye to describe the structure of the ONH of the eye at a reference/baseline condition using POD. Any glaucomatous changes in the ONH of the eye present during a follow-up exam were estimated by comparing the follow-up ONH topography with its baseline topograph subspace representation. Image correspondence measures of L1 -norm and L2 -norm, correlation, and image Euclidean distance (IMED) were used to quantify the ONH changes. An ONH topographic library built from the Louisiana State University Experimental Glaucoma study was used to evaluate the performance of the proposed method. The area under the receiver operating characteristic curves (AUCs) was used to compare the diagnostic performance of the POD-induced parameters with the parameters of the topographic change analysis (TCA) method. The IMED and L2 -norm parameters in the POD framework provided the highest AUC of 0.94 at 100 field of imaging and 0.91 at 15° field of imaging compared to the TCA parameters with an AUC of 0.86 and 0.88, respectively. The proposed POD framework captures the instrument measurement variability and inherent structure variability and shows promise for improving our ability to detect glaucomatous change over time in glaucoma management. Electrophysiological records are a standard procedure in the study of neuronal coding in the visual system. In this paper we propose a model that can predict the electrophysiological record of electrical activity in the optic nerve evoked by visual stimuli projected on the retina. This development contemplates certain processes of coding the visual information realized in retina and how it is spread by the nerve fibers that compose the optic nerve. The study of ocular manifestations of neurodegenerative disorders, Oculomics, is a growing field of investigation for early diagnostics, enabling structural and chemical biomarkers to be monitored overtime to predict prognosis. Traumatic brain injury (TBI) triggers a cascade of events harmful to the brain, which can lead to neurodegeneration. TBI, termed the “silent epidemic” is becoming a leading cause of death and disability worldwide. There is currently no effective diagnostic tool for TBI, and yet, early-intervention is known to considerably shorten hospital stays, improve outcomes, fasten neurological recovery and lower mortality rates, highlighting the unmet need for techniques capable of rapid and accurate point-of-care diagnostics, implemented in the earliest stages. This review focuses on the latest advances in the main neuropathophysiological responses and the achievements and shortfalls of TBI diagnostic methods. Validated and emerging TBI-indicative biomarkers are outlined and linked to ocular neuro-disorders. Methods detecting structural and chemical ocular responses to TBI are categorised along with prospective chemical and physical sensing techniques. Particular attention is drawn to the potential of Raman spectroscopy as a non-invasive sensing of neurological molecular signatures in the ocular projections of the brain, laying the platform for the first tangible path towards alternative point-of-care diagnostic technologies for TBI [ABSTRACT FROM AUTHOR]

We describe an automated method to locate the optic nerve in images of the ocular fundus. Our method uses a novel algorithm we call fuzzy convergence to determine the origination of the blood vessel network. We evaluate our method using 31 images of healthy retinas and 50 images of diseased retinas, containing such diverse symptoms as tortuous vessels, choroidal neovascularization, and hemorrhages that completely obscure the actual nerve. On this difficult data set, our method achieved 89% correct detection. We also compare our method against three simpler methods, demonstrating the performance improvement. Glaucoma is a chronic eye disease, causing damage to the optic nerve; it may cause permanent vision loss. The conventional instrument methods for glaucoma detection are manual and time-consuming. Many approaches have recently been proposed for automatic glaucoma classification using retinal fundus images. However, none of the existing methods can efficiently use for early-stage glaucoma detection. In this letter, we proposed a novel method for glaucoma classification based on the newly introduced two-dimensional tensor empirical wavelet transform (2D-T-EWT). In this study, the pre-processed images are decomposed into sub-band images (SBIs) using 2D-T-EWT. Then, texture-based grey level co-occurrence matrix (GLCM), chip histogram, and moment invariant features have been extracted from decomposed SBIs. Afore, robust features have been selects and ranked using the student'st-test algorithm. Finally, trained multi-class least squares-support vector machine (MC-LS-SVM) classifier has been used for the classification. The experimental results show that our method outperformed state-of-the-art approaches for glaucoma classification. Abnormalities in pupillary light reflex can indicate optic nerve disorders that may lead to permanent visual loss if not diagnosed in an early stage. In this study, we focus on relative afferent pupillary defect (RAPD), which is based on the difference between the reactions of the eyes when they are exposed to light stimuli. Incumbent RAPD assessment methods are based on subjective practices that can lead to unreliable measurements. To eliminate subjectivity and obtain reliable measurements, we introduced an automated framework to detect RAPD. For validation, we conducted a clinical study with lab-on-a-headset, which can perform automated light reflex test. In addition to benchmarking handcrafted algorithms, we proposed a transfer learning-based approach that transformed a deep learning-based generic object recognition algorithm into a pupil detector. Based on the conducted experiments, proposed algorithm RAPDNet can achieve a sensitivity and a specificity of 90.6% over 64 test cases in a balanced set, which corresponds to an AUC of 0.929 in ROC analysis. Composite models that combine medical imaging with electronic medical records (EMR) improve predictive power when compared to traditional models that use imaging alone. The digitization of EMR provides potential access to a wealth of medical information, but presents new challenges in algorithm design and inference. Previous studies, such as Phenome Wide Association Study (PheWAS), have shown that EMR data can be used to investigate the relationship between genotypes and clinical conditions. Here, we introduce Phenome-Disease Association Study to extend the statistical capabilities of the PheWAS software through a custom Python package, which creates diagnostic EMR signatures to capture system-wide co-morbidities for a disease population within a given time interval. We investigate the effect of integrating these EMR signatures with radiological data to improve diagnostic classification in disease domains known to have confounding factors because of variable and complex clinical presentation. Specifically, we focus on two studies: First, a study of four major optic nerve related conditions; and second, a study of diabetes. Addition of EMR signature vectors to radiologically derived structural metrics improves the area under the curve (AUC) for diagnostic classification using elastic net regression, for diseases of the optic nerve. For glaucoma, the AUC improves from 0.71 to 0.83, for intrinsic optic nerve disease it increases from 0.72 to 0.91, for optic nerve edema it increases from 0.95 to 0.96, and for thyroid eye disease from 0.79 to 0.89. The EMR signatures recapitulate known comorbidities with diabetes, such as abnormal glucose, but do not significantly modulate image-derived features. A novel accurate and fast optic disc (OD) detection method is proposed by using vessel distribution and directional characteristics. A feature combining three vessel distribution characteristics, i.e., local vessel density, compactness, and uniformity, is designed to find possible horizontal coordinate of OD. Then, according to the global vessel direction characteristic, a General Hough Transformation is introduced to identify the vertical coordinate of OD. By confining the possible OD vertical range and by simplifying vessel structure with blocks, we greatly decrease the computational cost of the algorithm. Four public datasets have been tested. The OD localization accuracy lies from 93.8% to 99.7%, when 8–20% vessel detection results are adopted to achieve OD detection. Average computation times for STARE images are about 3.4–11.5 s, which relate to image size. The proposed method shows satisfactory robustness on both normal and diseased images. Optic nerve head (ONH) tissue properties and biomechanics remain mostly unmeasurable in the experiment. We hypothesized that these can be estimated numerically from ocular parameters measurable in vivo with optical coherence tomography (OCT). Using parametric models representing human ONHs we simulated acute intraocular pressure (IOP) increases (10 mmHg). Statistical models were fit to predict, from OCT-measurable parameters, 15 outputs, including ONH tissue properties, stresses, and deformations. The calculations were repeated adding parameters that have recently been proposed as potentially measurable with OCT. We evaluated the sensitivity of the predictions to variations in the experimental parameters. Excellent fits were obtained to predict all outputs from the experimental parameters, with cross-validated R2s between 0.957 and 0.998. Incorporating the potentially measurable parameters improved fits significantly. Predictions of tissue stiffness were accurate to within 0.66 MPa for the sclera and 0.24 MPa for the lamina cribrosa. Predictions of strains and stresses were accurate to within 0.62% and 4.9 kPa, respectively. Estimates of ONH biomechanics and tissue properties can be obtained quickly from OCT measurements using an applet that we make freely available. Future clinical applications of magnetic resonance imaging-guided high-intensity focused ultrasound (MRgHIFU) are moving toward the management of different intracranial pathologies. We sought to validate the production, safety, and efficacy of thermal injury to cranial nerves generated by MRgHIFU. In this study, five female domestic pigs underwent a standard bifrontal craniectomy under general anesthesia. Treatment was then given using an MRgHIFU system to induce hyperthermic ablative sonication (6 to 10 s; 50 to 2000 J.) Histological analyses were done to confirm nerve damage; temperature measured on the optic nerve was approximately 53.4°C (range: 39°C to 70°C.) Histology demonstrated a clear definition between a necrotic, transitional zone, and normal tissue. MRgHIFU induces targeted thermal injury to nervous tissue within a specific threshold of 50°C to 60°C with the tissue near the sonication center yielding the greatest effect; adjacent tissue showed minimal changes. Optical coherence tomography (OCT) is widely used in the assessment of retinal nerve fibre layer thickness (RNFLT) in glaucoma. Images are typically acquired with a circular scan around the optic nerve head. Accurate registration of OCT scans is essential for measurement reproducibility and longitudinal examination. This study developed and evaluated a special image registration algorithm to align the location of the OCT scan circles to the vessel features in the retina using probabilistic modelling that was optimised by an expectation-maximization algorithm. Evaluation of the method on 18 patients undergoing large number of scans indicated improved data acquisition and better reproducibility of measured RNFLT when scanning circles were closely matched. The proposed method enables clinicians to consider the RNFLT measurement and its scan circle location on the retina in tandem, reducing RNFLT measurement variability and assisting detection of real change of RNFLT in the longitudinal assessment of glaucoma. Optic Disc (OD) localization is an important pre-processing step that significantly simplifies subsequent segmentation of the OD and other retinal structures. Current OD localization techniques suffer from impractically-high computation times (few minutes per image). In this work, we present a fast technique that requires less than a second to localize the OD. The technique is based upon obtaining two projections of certain image features that encode the x- and y- coordinates of the OD. The resulting 1-D projections are then searched to determine the location of the OD. This avoids searching the 2-D image space and, thus, enhances the speed of the OD localization process. Image features such as retinal vessels orientation and the OD brightness are used in the current method. Four publicly-available databases, including STARE and DRIVE, are used to evaluate the proposed technique. The study of the architecture of the optic nerve head (ONH) may provide valuable information about the development and progression of glaucoma. To this end, we have generated three-dimensional datasets from monkey eyes under controlled intraocular pressure (IOP). Segmentation of the connective tissues in this area is crucial to obtain an accurate measurement of geometrical parameters and to build mechanical models. However, this segmentation is made difficult by the complicated geometry and the artifacts introduced in the dataset building process. We present a novel segmentation algorithm, based on expectation-maximization, which incorporates an anisotropic Markov random field (MRF) to introduce prior knowledge about the geometry of the structure. The structure tensor is used to characterize the predominant structure direction and the spatial coherence at each point. The algorithm, which has been validated on an artificial validation dataset that mimics our ONH datasets, shows significant improvement over an isotropic MRF. Eye movement recordings with video-based techniques have become very popular, as long as they are restricted to the horizontal and vertical movements of the eye. Reliable measurement of the torsional component of eye movements, which is especially important in the diagnosis and investigation of pathologies, has remained a coveted goal. One of the main reasons is unresolved technical difficulties in the analysis of video-based images of the eye. Based on simulations, we present solutions to two of the primary problems: a robust and reliable calibration of horizontal and vertical eye movement recordings, and the extraction of suitable iris patterns for the determination of the torsional eye position component. We use a whole-animal model and wavelength-selective optical stimulation to investigate relationships between optical stimulus characteristics and neural signaling. Light-emitting diodes are used to selectively stimulate rod and cone pathways in Rana pipiens. A suction electrode is used to make in vivo measurements of the compound action potential from the optic nerve as the wavelength and intensity of the stimulus is varied. Er:YAG lasers are being widely studied as candidates for surgical procedures in liquid environments, such as in ophthalmology. However, while the Er:YAG laser can be a precise and efficient light scalpel, this surgical method includes accompanying stress waves that must be quantified and evaluated for potential harm. In this study, Er:YAG laser-induced stress waves for free running laser pulses were measured over various consecutive treatment periods. Using a spot-poled PVDF hydrophone, measurements were acquired beneath a biological membrane submerged in a saline bath. Results yielded pressures peaks of 300-600 mbar beneath the uncut membrane, which could be harmful for the optic nerve if located directly below the treatment area. Acoustic waves representative of direct laser-liquid interactions were observed immediately following membrane rupture, and yielded much larger pressures.
