Diagnosis is the cornerstone of care for temporomandibular disorders (TMD) in order to provide crucial forward momentum in terms of patient engagement and education alongside provision of appropriate management. Despite its importance diagnostic confidence for TMD is reported as low in the primary care settings. This article will consider potential reasons for this and outline the importance of diagnosis for both individuals living with the condition and clinicians. We will suggest utilisation of a pragmatic, evidence based, broad group diagnosis using simplified examination procedures and diagnostic aids to support the TMD diagnostic process for clinicians. Lalou was more interested in experimental research rather than clinical study; he returned to his native country (Romania) where he was appointed professor of pharmacology (Bucarest). After working in a private nursing home for the psychiatric ill in Fontenay-sous-Bois, A.-C. Crété. worked in that of Suresnes and finished his career in a sanatorium in Guervenan (Bretagne) which completed the social hygiene and anti-tuberculosis dispensaries. J.-A. Baronnet, L.-V. Revertégat et J.-M. Sardain were doctors who did not seem to particularly appreciate the satisfaction brought by the psychiatry specialty and later became dentists; most probably for financial reasons. Taiwan's eight dental schools are all located in the western region. This study attempted to analyze the relationship between the geographical distribution of dentists in Taiwan and their graduation school background and to develop several statistical indicators to analyze the geographical distribution of dentists. The method of the secondary data analysis was adopted to collect the open information related to the number of the practicing dentists based on their graduation schools and practice locations by cities and counties in Taiwan in January 2023. The data were used to develop several statistical indicators to analyze the geographical distribution of dentists. There were 16,533 practicing dentists in Taiwan in January 2023. Of the 16,533 dentists, 15,687 (94.88 %) graduated from domestic dental schools and 846 (5.12 %) graduated from foreign dental schools. For the school location, the coefficient of variation, Gini coefficient, population proportion index, and population density index were the largest (168.57 %, 0.47, 10.76 %, and 3927.01 people/square kilometer) in the dentists graduating from the northern dental schools. In overall, the dentists were concentrated in municipalities, cities, and the northern region of Taiwan, especially those graduating from the northern dental schools. In Taiwan, the geographical distribution of dentists is highly related to their graduation school background. Promoting a balanced distribution of dental enrollment quotas and dental student sources from different geographical locations and inducing the movement of dentists to the dentist-shortage areas may be the more feasible ways to solve the uneven geographical distribution of dentists in Taiwan. Oral diseases are a burden for developing countries such as India where owing to socioeconomic conditions cost is a factor in selecting the type of treatment. Dental care in India is provided at various levels of care including standalone clinics run by individual dental surgeons, multi-speciality dental centres providing specialist care under one roof including paediatric, prosthodontics, and so on and dental centres affiliated with hospitals having facilities for provision of operation theatre and anaesthesia services. Most dental procedures are performed as day-care procedures requiring local anaesthetics, oral, or moderate intravenous sedation and anxiolytics while maxillofacial procedures are carried out in a controlled operation theatre setting. Dental procedures are fraught with risks due to shared oral surgical space and airway leading to reduced field of visibility to dentists especially with the usage of supplementary oxygenation devices, mouth props, and so on. Various dental societies have proposed minimum standards of care for provision of anaesthesia to various dental procedures; however, there are no extant guidelines in our country. The authors propose these guidelines for provision of minimal sedation (anxiolysis) and moderate sedation/analgesia for various day care dental procedures at various levels of care, keeping the aim that no patient's life is endangered during provision of anaesthesia services at all dental locations. Most oral cancer (OC) cases are identified by family dentists in Japan. However, a significant number of patients with OC in Japan are referred to core hospitals at advanced stages. Therefore, identifying the factors that contribute to delayed referrals from family dentists to core hospitals is crucial for detecting OC in its earlier stages. The aim of this retrospective study was to identify the risk factors for referral delays from family dentists to core hospitals. The study included 63 patients with OC who were referred by family dentists to the Yamagata University Hospital between 2010 and 2022. The clinical parameters related to referral delays were retrospectively investigated using letters of reference provided by the family dentists and patient charts. Backward multiple regression analysis was performed to identify the relationships between the length of referral delay and potential risk factors. Additionally, backward multivariate logistic regression analysis was performed to examine the independent association between referral delays of >4 weeks and several clinical parameters. Multiple regression analysis revealed that misdiagnosis of malignant lesions by family dentists (P = 0.047) was significantly associated with longer referral delays. Additionally, misdiagnosis of malignant lesions by family dentists was also an independent risk factor for referral delays of >4 weeks (odds ratio, 10.387; P = 0.006). Misdiagnosis of malignant lesions by family dentists was a significant risk factor for referral delays from family dentists to core hospitals. To develop a deep-learning model to help general dental practitioners diagnose periodontitis accurately and at an early stage. First, the panoramic radiographs (PARs) from the Second Affiliated Hospital of Nanchang University were input into the convolutional neural network (CNN) architecture to establish the PAR-CNN model for healthy controls and periodontitis patients. Then, the PARs from the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine were included in the second testing set to validate the effectiveness of the model with data from two centres. Heat maps were produced using a gradient-weighted class activation mapping method to visualise the regions of interest of the model. The accuracy and time required to read the PARs were compared between the model, periodontal experts, and general dental practitioners. Areas under the receiver operating characteristic curve (AUCs) were used to evaluate the performance of the model. The AUC of the PAR-CNN model was 0.843, and the AUC of the second test set was 0.793. The heat map showed that the regions of interest predicted by the model were periodontitis bone lesions. The accuracy of the model, periodontal experts, and general dental practitioners was 0.800, 0.813, and 0.693, respectively. The time required to read each PAR by periodontal experts (6.042 ± 1.148 seconds) and general dental practitioners (13.105 ± 3.153 seconds), which was significantly longer than the time required by the model (0.027 ± 0.002 seconds). The ability of the CNN model to diagnose periodontitis approached the level of periodontal experts. Antimicrobial stewardship in dentistry and antibiotic prophylaxis for tooth extraction have been areas of concern in Japan, with limited research available. This cross-sectional study utilized the regional health insurance claims database in Japan to examine antibiotic prescription trends by dentists, including antibiotic regimens, timing of prescription, and days of supply for prophylactic antibiotic use during tooth extraction. Antibiotic prophylaxis for patients with prosthetic heart valves was also investigated. Antibiotic prescriptions by dentists decreased by 7% in 2019 compared to those in 2015, with third-generation cephalosporins still accounting for 48.5% in 2019. Amoxicillin prescription increased 3.9 times in 2019, although it only accounted for 8.4% of all antibiotic prescriptions. In 2019, amoxicillin was prescribed for 17.1% of all prophylactic antibiotics associated with tooth extraction, and 80% of prophylactic antibiotics were prescribed for 3 days or more, with 85% prescribed on the day of the procedure. However, only 60–70% of patients with prosthetic heart valves received antibiotic prophylaxis. Despite the increasing trend after the implementation of the National Action Plan on antimicrobial resistance, the proportion of amoxicillin prescriptions in dentistry remains low. Antimicrobial stewardship issues related to long-term prescription and timing of administration of prophylactic antibiotics for tooth extraction should be addressed. Dentists must recognize the risks associated with high-risk patients with prosthetic heart valves who require antibiotic prophylaxis, and physicians providing valve replacement therapy should inform patients of the requirement for prophylaxis before invasive dental procedures. Improved communication can optimize treatment outcomes and patient satisfaction. Findings emphasize the need for tailored communication strategies based on patient characteristics. Implementing communication courses can enhance patient-centered care and reduce conflicts. Therefore, this study examined the feasibility of integrating doctor-patient communication education in Taiwan's dental education system. Using interviews and questionnaires, we conducted descriptive statistics and generalized linear mixed-effects model analysis on the importance of doctor-patient communication from the dentist and patient perspectives. More than 600 patient surveys and four interviewed dentists with 20+ years of experience stressed doctor-patient communication in dentistry. Patients' age and income were positively related to the emphasis on physician-patient communication but negatively associated with dental assistants' communication. Dentists valued communication education but differed in its execution and importance. It is recommended to initiate dentist-patient communication education during university studies and continue its practice to adapt to the changing societal dynamics. Individuals with higher socioeconomic status and older age show a greater appreciation for dentist-patient communication, potentially driven by self-promotion, thereby highlighting the diverse nature of doctor-patient relationships. Online courses have been widely used in all levels of education during the COVID-19 pandemic. This study explored the effectiveness of a dentist continuing education (DCE) course through the online devices in Taiwan. The practicing dentists who participated in the online course of dental radiation technology for DCE offered by the Taiwan Dental Association (TWDA) in October 2022 and in March 2023 were enrolled in this study. The composition of participating dentists was confirmed by the public inquiry system and their learning effectiveness was evaluated by a questionnaire-based survey after the online DCE class. All participating dentists (132 in October 2022 and 117 in March 2023) obtained consistent good learning outcomes in this online DCE course. Of these 249 dentists, there were 170 (68.27%) males and 79 (31.73%) females, 127 (51.00%) dental specialists and 122 (49.00%) general dentists, as well as 50 (20.08%) hospital dentists and 199 (79.92%) clinic dentists. The participation rates for this course of practicing dentists in non-municipalities (4.70%), counties (3.88%), eastern region (8.08%), and outlying islands (3.60%) were much higher than those in municipalities (0.79%), cities (1.16%), and the western region including the northern region (0.88%), central region (1.96%), and southern region (1.94%), respectively. The participating dentists express positive feedback on the online DCE courses, and the online DCE courses can reduce the urban-rural gap in dental education resources. Concerns about the indications for the surgical removal of third molars have not yet reached a decisive agreement. The aim of the study was to evaluate the perception and practice of Saudi' dentists when encountering third molars clinical situations including diagnosis, treatment, and postsurgical care. A cross-sectional survey was conducted and approved by the ethics research committee at Prince Sattam Bin Abdulaziz (PSAU). The survey comprised of four sets of questions with multiple-choice answers including the participants' details and type of practice (private or government), clinical awareness and attitude regarding lower third molars surgery, and the confidence level of dentists when performing third molars extraction. The survey was published online with a timeframe of three months to respond. 196 participants responded to the designated survey (55 dental interns and 141 general dental practitioners (GPs)). Most responses (80.1%) were from Riyadh province (central region). There was a significant difference between GPs and interns in terms of impacted third molars with associated cystic lesions, bony impactions, and diffuse pain (P < 0.05). Male dentists tend to prescribe antibiotics more than female dentists (P = 0.001) and GPs more than interns (P = 0.027). The results also indicated that male dentists were more confident than females when performing third molar extraction. Generally, female dentists preferred to have further training on such surgery (P < 0.05). Continuing professional development programs both during dental internship and general practice are recommended to improve the required skills of third molar extraction. [ABSTRACT FROM AUTHOR]

Medication-related osteonecrosis of the jaw (MRONJ) is a serious complication among dental patients undergoing treatment with antiresorptive medications such as bisphosphonate and denosumab. The present survey investigated the awareness and practice of dentists in the Gulf Cooperation Council (GCC) countries regarding MRONJ. This questionnaire-based study was conducted among dental practitioners in all six GCC countries. A questionnaire was designed and distributed among all potential participants via different social media platforms. SPSS version 22 was used for data analysis, and P -value <0.05 was considered statistically significant. Overall, 1685 dentists from the six GCC countries participated in the present study. The surveyed dentists revealed relatively fair practices and awareness regarding MRONJ and its prevention, with the majority reported asking their patients about history of anti-osteoporotic medications (67.8%), recording name of the medication (73.1%) and duration of treatment (75.5%). However, the majority of the participants were unconfident about the duration of drug holiday prior to dental surgical interventions (70.6%) and the overall good level of knowledge/practice related to MRONJ was just 50.6%. The regression analysis revealed that previous exposure to MRONJ cases and attending a seminar, course, meeting, or conference about osteonecrosis of the jaw were independent predictors for positive awareness/good practice regarding MRONJ (P < 0.05). The results show inadequate awareness and practices of dentists practicing in GCC countries regarding MRONJ, with significant variations among the countries. In Taiwan, dental radiology has a long history of development. However, there are very few dental radiology curricula in Taiwan's dental education system. This study explored preliminarily the dental radiology course designed for dentist continuing education in Taiwan. This study used the method of dental radiology education survey by questionnaire to evaluate participating dentists' learning outcomes by assessing their perceptions of the dental radiology course. In this study, 117 participating dentists filled out the questionnaires completely after the class of dentist continuing education. The results showed that most of participating dentists agreed that dental radiology courses were rare in dental school curriculum and dentist continuing education. Moreover, most of participating dentists found this course to be helpful in raising their basic knowledge and skill about dental radiology, their attitude towards dental radiology, and their interest in further learning of dental radiology. They were satisfied with the course. The degree of agreement for each question was high, and all the mean scores for each question were between 4.53 and 4.77. The numbers (rates) of respondents who answered as agree were between 105 and 113 (between 89.74% and 96.58%). The dental radiology course contributed to an increase in dentists' basic knowledge and skill about dental radiology and their awareness and understanding of the importance of dental radiology. Considering the effectiveness of the dental radiology course on dentists' basic knowledge/skill and attitude of dental radiology, this model shows promise for further use in dentist continuing education. The global coronavirus disease 2019 (COVID-19) pandemic continues to cause a significant impact on patient care due to its considerable transmissibility, morbidity, and mortality. The aim of this study was to investigate whether dental procedures performed at private dental clinics were associated with the occurrence of COVID-19 clusters and measures taken to prevent nosocomial infection of COVID-19. An online questionnaire-based survey on clinical activities, infection control measures, and confirmed or probable COVID-19 cases among patients and clinical staff during the pandemic (February 2020 to September 2021) was administered to dentists at private dental clinics in Nagano prefecture, Japan. Almost all dentists engaged in dental treatment were actively implementing standard precautions. Twenty-seven dental clinics provided dental care to infected patients (nine confirmed and eighteen probable), but there was no transmission from patients to staff. Ten clinics had staff who were infected, but none exhibited disease transmission from staff to patients. The results of this survey suggest that COVID-19 clusters might not occur in dental settings where appropriate protective measures are implemented. [ABSTRACT FROM AUTHOR]

Although many coronavirus disease 2019 (COVID-19) vaccine injections have been administered worldwide, the safety of this practice remains unclear. This study aimed to compare the rates of complications associated with COVID-19 vaccines administered by dentists with those of vaccines administered by nurses. This study aimed to evaluate the safety of a vaccination program delivered by dentists. This observational cohort study included 537 recipients of the second dose of the Pfizer COVID-19 vaccine, delivered as an intramuscular injection to the upper arm deltoid muscle by dentists or nurses at the study site. Vaccine recipients were divided into two groups according to the vaccination administrator (dentist vs. nurse groups). The rates of complications associated with intramuscular injection technique (numbness in the hand or arm at the time of the injection, vasovagal reflex at the time of the injection, vaccine-related shoulder injury, and prolonged numbness) were examined. A total of 125 vaccine recipients were included (nurse group, n = 84; dentist group, n = 41). The overall incidence rate of complications was lower in the dentist group (2.4%; 1/41) than in the nurse group (8.3%; 7/84). However, this difference was not statistically significant (P = 0.3). The applications and scope of digitization and technology in dentistry are becoming increasingly valuable right from clinical dentistry to research, student training, teaching, and laboratory techniques. Mastering 3D printing and its usage are essential for dental practitioners and dental technicians as it allows them to choose and necessarily know what is offered, as well as how to implement it in everyday practices thereby contributing to the betterment of the dental profession. The study aims to assess dental practitioners' and dental technicians' knowledge, understanding, and practices related to the use of 3D printing in dentistry. A cross-sectional questionnaire-based study was done among dental practitioners and technicians in Karnataka, India who were given access to a self-explanatory questionnaire via Google link consisting of questions that evaluated their knowledge, awareness, and practices regarding 3D printing. The Chi-square test was used for statistical analysis. A total of 380 replies were obtained after the questionnaire was circulated. Awareness regarding the use of digital technology in dentistry was known by 98.9% of practitioners and 92.7% of technicians, of which we discovered that 9.28% of practitioners and 17.7% of technicians were unfamiliar with 3D printing, which was statistically significant (p = 0.0400*). 81.6% of practitioners consider 3D printing can be used to fabricate complex design prostheses. The participants' understanding of digital dentistry and 3D printing is acceptable. The majority of dental professionals expressed an interest in adopting 3D printing and believe that there should be a forum for collecting and exchanging skills and knowledge about 3D printing. Among children younger than 18 years, the prevalence of long-term chronic diseases (LTDs) is not well known in France, nor the frequency of the use of healthcare services. This nationwide observational study focused on both topics over a 1-year period following the birth or birthday of French children in 2018 and compared the LTD status and use of healthcare. We selected children living in mainland France from the national health data system (SNDS). It includes data concerning the LTD status, which guarantees 100% reimbursement for related healthcare expenditures. We calculated the median and interquartile range (IQR) for the prevalence of LTDs and the rate of children using healthcare services at least once during the year. We included 13.211 million children (51.2% boys), of whom 4% had at least one LTD (boys: 4.6%, girls: 3.3%). Mental and behavioral disorders were the most frequent cause (1.6%). At least one visit to a general practitioner (GP) or pediatrician was found for 88% of children (median: 3, IQR: 2–6): 98% for children under 1 year of age and 81% for children aged 14–17 years. A pediatrician was visited by 17% of children, another specialist by 39%, a dentist by 37%, with peaks of about 60% at the ages of 6, 9, and 12 years; 8% visited a nurse and 7% visited a physiotherapist. At least one emergency department visit was recorded for 24% of children (42% <1 year) and one short-stay hospitalization (SSH) for 9%. Regional variations were observed. Children with LTDs more frequently used all services, such as specialist visits (50% vs. 40%), ED visits (32% vs. 23%), SSHs (26% vs. 8% and 15% vs. 4.0% for one night or more), and psychiatric hospital admissions (5% vs. 0.1%). Most children saw a GP or pediatrician during the year and children with an LTD showed more frequent use. Nevertheless, outpatient visits appeared to be underutilized with respect to recommendations or free-of-charge prevention visits, such as for dentists. More detailed studies are required to identify factors associated with the use of healthcare services in France, for example, studies including the deprivation index and regional variations. In Taiwan, the formal education for cultivating medical radiation technologists began in 1965. However, there are very few dental radiology curricula in Taiwan's medical radiology schools. We investigated mainly the appearance of dental radiology education for medical radiology students in Taiwan. We used documentary analysis, a survey of dental radiology education, and secondary data analysis in this study to find the appearance of Taiwan's dental radiology education for medical radiology students. There were currently ten medical radiology schools. Among them, five offered independent curricula in their subject schedules and another five had dental radiology education included in their general medical radiology curricula. Of the 53 dental radiology teaching hospitals, 21 (39.62%) were medical centers and 25 (47.17%) were regional hospitals, and these large hospitals were concentrated in the northern region of Taiwan. These large dental radiology teaching hospitals offered more training hours of dental radiology internship for medical radiology students and had a higher availability for the medical radiology students. In Taiwan, the current status of dental radiology curricula in the medical radiology schools is indeed seriously insufficient. A dental radiology education system should be established in the future, including the innovative dental radiology curricula developed for medical radiology students and more dental radiology curricula designed for dental students. This in turn can create new career options for medical radiology students and new practice directions for medical radiation technologists, and then expand their potential involvement in dental radiology. In Taiwan, there is no independent licensing system for dental radiation technologists. A licensed medical radiation technologist who engages in dental radiology is the so-called dental radiation technologist. This study explored mainly the profile of dental radiation technologists in Taiwan. This study used the methods of documentary analysis, dental radiation manpower survey, and the secondary data analysis to find the profile of dental radiation technologists in Taiwan. There were currently 59 dental radiology departments and 101 dental radiation technologists (29 males and 72 females) in 57 hospitals and their branches. Of the 101 dental radiation technologists, 56 worked in the medical centers, 28 in the regional hospitals, and 17 in the district hospitals. More than half of the dental radiation technologists were concentrated in the medical centers (55.45%, 56/101) or the northern region of Taiwan (57.43%, 58/101), especially in the northern medical centers (30.69%, 31/101). In Taiwan, the manpower of dental radiation technologists is insufficient, and dental radiation technologists usually work in the dental departments of the hospitals. A large number of clinic dentists lack dental radiation technologists to assist in dental radiology works and the clinic dentists have to perform the dental radiology works by themselves. Therefore, a dental radiology education system should be established to design innovative dental radiology courses for radiological technology students. Currently, Taiwan has a very complete education system of oral hygiene, but there is still no licensing system for oral hygienists. This study explored mainly the profile of education system for oral hygienists in Taiwan from 2017 to 2021. This study used the documentary analysis and secondary data analysis to survey mainly the development of oral hygiene education system in Taiwan, and the changes of oral hygiene students and graduates from 2017 to 2021. In Taiwan, currently, there are 8 oral hygiene schools which offer associate, bachelor, and master degrees in oral hygiene. The oral hygiene enrollment quotas determined by the Ministry of Education increased from 343 quotas in 9 programs in 2017 to 500 quotas in 13 programs in 2021, while new oral hygiene students who completed registration increased from 282 students in 2017 to 353 students in 2021. The registration rate of the five-year training program of junior college was significantly higher than the other programs. The number of oral hygiene students with official student status increased from 604 students in 2017 to 1349 students in 2021. The total increase was 745 students from 2017 to 2021 with a total increase rate of 123.34%. In Taiwan, although the manpower of oral hygienists is important for public oral health, they usually work as dental assistants or change their careers. The development of dental assistants in Taiwan has been over a century. This study explored mainly the profile of dental assistants and the occupation survey about them in Taiwan from 2009 to 2021. This study used the documentary analysis and secondary data analysis to survey mainly the changes of manpower of dental assistants in Taiwan from 2009 to 2021, and the association of their wages with economic indicators. According to occupation survey from government, the number of dental assistants increased from 4315 in 2009–7909 in 2021. When estimated based on the data of the United States showing 1.86 dental assistants per dentist, the estimated number of dental assistants increased from 21,113 in 2009 to 28,698 in 2020. Although the mean monthly wage of dental assistants increased from 22,127 NT dollars in 2009 to 26,795 NT dollars in 2021, the proportion of the mean annual wage of dental assistants to the gross domestic product per capita decreased from 47.43% in 2009 to 34.78% in 2021. Moreover, the proportion of the mean monthly wage of dental assistants to the basic monthly wage of labor decreased from 128.05% in 2009 to 111.65% in 2021. In Taiwan, although the manpower and work content of dental assistants are important for dentists, they usually work with a low wage from 2009 to 2021. Physician burnout is now a public health crisis and dentist is a stressful professional health occupation. The purpose of this cross-sectional study was to determine the level of burnout and related factors among 423 Thai dentists working in public hospitals, southern Thailand. A self-administered questionnaire was collected through an online platform and included 5 parts: general information, work information, 22-item Maslach Burnout Inventory-Human Services Survey (MBI-HSS), 23-item Thai Effort-Reward Imbalance Questionnaire (Thai ERIQ), and the Utrecht Work Engagement Scale-9 (UWES-9). The data were analyzed for descriptive statistics – frequency, percentage, mean, standard deviation – and inferential statistics using binary logistic regression. The prevalence of medium to high level of burnout were 45.8%, 44.3%, 4.0% for emotional exhaustion, depersonalization, and low personal accomplishment, respectively. Regarding to the multivariate analysis, the statistically significant factors associated with emotional exhaustion were age, work engagement, workplace relationship, work effort, work reward, grade point average, and workplace consultant. The factors associated with depersonalization were age, work engagement, workplace relationship, work effort, and job reselection. The factors associated with personal accomplishment were age, work engagement, work reward, and level of workplace. Introduction: Nonopioid analgesics are more effective for most oral pain, but data suggest that dental prescribing of opioids is excessive. This study evaluates the extent to which opioids exceed recommendations and the characteristics associated with opioid overprescribing by Veterans Health Administration dentists.Methods: This was a national cross-sectional study of Veterans' dental visits from 2015 to 2018. Overprescribing was defined per national guidelines as >120 morphine milligram equivalents (primary outcome). The association of dental visit and patient demographic and medical characteristics was modeled with overprescribing (defined as >120 morphine milligram equivalents) using Poisson regression with clustering by facility and patient. A secondary analysis assessed opioid prescriptions >3 days' supply. The dates of analysis were January 2020‒May 2021.Results: Of the 196,595 visits, 28.7% exceeded 120 morphine milligram equivalents. Friday visits and people with chronic oral pain or substance misuse were associated with a higher prevalence of overprescribing. Women, older Veterans, and Black and Latinx Veterans were less likely to be overprescribed than men, younger Veterans, and White Veterans, respectively. Routine dental visits had a higher prevalence of opioid overprescribing than invasive visits. Opioid overprescribing decreased over time. White Veterans were more likely to receive oxycodone and hydrocodone, whereas people of Black race and Latinx ethnicity were more likely to receive codeine and tramadol. In the secondary analysis, 68.5% of opioid prescriptions exceeded a 3-day supply.Conclusions: Nearly 1 in 3 opioids prescribed by Veterans Health Administration dentists exceed guidelines. Prescribing higher potency and quantities of opioids, especially on Fridays and to certain demographic groups, should be addressed as part of dental opioid stewardship programs. Introduction: Evidence suggests that U.S. dentists prescribe opioids excessively. There are limited national data on recent trends in opioid prescriptions by U.S. dentists. In this study, we examined trends in opioid prescribing by general dentists and dental specialists in the U.S. from 2012 to 2019.Methods: Dispensed prescriptions for oral opioid analgesics written by dentists were identified from IQVIA Longitudinal Prescription Data from January 2012 through December 2019. Autoregressive integrated moving average and joinpoint regression models described monthly population-based prescribing rates (prescriptions/100,000 individuals), dentist-based prescribing rates (prescriptions/1,000 dentists), and opioid dosages (mean daily morphine milligram equivalents/day). All analyses were performed in 2020.Results: Over the 8 years, dentists prescribed >87.2 million opioid prescriptions. Population- and dentist-based prescribing rates declined monthly by -1.97 prescriptions/100,000 individuals (95% CI= -9.98, -0.97) and -39.12 prescriptions/1,000 dentists (95% CI= -58.63, -17.65), respectively. Opioid dosages declined monthly by -0.08 morphine milligram equivalents/day (95% CI= -0.13, -0.04). Joinpoint regression identified 4 timepoints (February 2016, May 2017, December 2018, and March 2019) at which monthly prescribing rate trends were often decreasing in greater magnitude than those in the previous time segment.Conclusions: Following national trends, dentists became more conservative in prescribing opioids. A greater magnitude of decline occurred post 2016 following the implementation of strategies aimed to further regulate opioid prescribing. Understanding the factors that influence prescribing trends can aid in development of tailored resources to encourage and support a conservative approach by dentists, to prescribing opioids. Background/purpose: Taiwan government has provided population-based fluoride varnish application services for all preschool children since July 2004. This study investigated the association providing such services on dental caries experiences among schoolchildren.Methods: A cross-sectional study was conducted on schoolchildren aged 8-9 years. A questionnaire collected information on sociodemographic background, parents' oral health status, children's oral health-related behavior, and dietary habits. Dental caries was recorded through standardized oral examinations. The number of services was retrieved from the Taiwan National Health Insurance Research Database. Univariate, multivariable linear, and logistic regression analyses were performed.Results: The study involved 1246 children. The mean dental caries indices were 3.97 for decayed, extracted, and filled teeth (deft) and 0.94 for decayed, missing and filled teeth (DMFT). After adjustments for confounding factors, it was revealed that children receiving services were not associated with significantly lower deft and DMFT indices (P > 0.05). The adjusted odds ratio (OR) for untreated primary teeth of children receiving 3 or more services was 0.64 (95% CI = 0.44-0.95) compared with those who received no services (P = 0.025). However, subgroup analyses demonstrated that children in the low-risk group were mainly affected (adjusted OR = 0.36-0.89, P = 0.013).Conclusion: This study revealed that children receiving 3 or more services were associated with a 36% decreased risk of having untreated caries in primary dentition, but these children were mainly in the low-risk group. The aim of this study is to (1) examine the prevalence of human papillomavirus (HPV) vaccination in adolescents and young adults in the U.S., including those who had a dental visit in the last year but not a medical visit and (2) to determine an association between last visit to the dentist and HPV vaccination status. We conducted a cross-sectional study using data from the National Health and Nutrition Examination Survey from 2015 to 2018, including participants 9–26 years. Descriptive statistical analyses were conducted to characterize the study population and calculate the prevalence of HPV vaccination in adolescents and young adults, including those who had a dental visit in the last year but not a medical visit. Logistic regression analyses were performed to examine the association between last visit to the dentist and HPV vaccination status. In total, 38.6% of participants were vaccinated for HPV, with higher prevalence of vaccination in those with the following characteristics: female, older age, higher income, higher education level, and having medical insurance. Participants who had a dental visit in the last year had an HPV vaccination rate of 40.8%. Of those who had a dental visit and were not vaccinated for HPV, 12.5% did not have a medical visit. Having a dental visit in the last year increased the odds of being vaccinated for HPV (odds ratio 1.69, confidence interval 1.26–2.28). Dentists see a significant number of adolescents and young adults who are unvaccinated for HPV in a given year and could serve as an access point for HPV vaccine delivery in the future. The postgraduate year dentist training program (PGYD) officially implemented in 2010. This study aimed to assess PGYD trainees' subjective satisfaction perception and objective competence performance according to different training institutions (either dental clinics or hospitals). A nationwide cross-sectional survey was conducted in 2018. Subjective satisfaction questionnaires from 222 PGYD trainees and 166 PGYD trainees' scores of objective structured clinical examinations (OSCEs) were collected for analysis. The t -test and logistic regression were used to compare differences between two groups. In subjective satisfaction, PGYD trainees in hospitals revealed less positive perceptions on teachers (odds ratios [ORs] range: 0.33–0.7) and on training plans and auxiliary facilities (ORs range: 0.23–0.69), but they had more opportunities to attend and present at professional meetings than those in clinics. In PGYD trainees' opinion, the optimal training period allocation was 25% in hospitals and 75% in clinics. Overall, trainees in hospitals had better OSCE scores than those in clinics (OR [95% confidence interval, CI]: 3.12 [1.68–5.79]), except for the item of "the relation between physical condition and dental treatment outcome" (0.3 [0.1–0.97]). PGYD trainees in dental clinics have more positive perceptions on teachers and on training plans and auxiliary facilities, but PGYD trainees in hospitals have better OSCE scores. PGYD trainees prefer to be trained in clinics for a better connection with future careers, while hospitals can offer better training for PGYD trainees to become independent dentists because they have better training environment and more educational resources. Introduction: It is unknown whether certain dentists account for disproportionate shares of dental opioid prescriptions and high-risk prescriptions. Identifying and characterizing such dentists could inform the targeting of initiatives to improve the appropriateness and safety of dental opioid prescribing.Methods: In May 2021, the authors conducted a cross-sectional analysis using the IQVIA Longitudinal Prescription Database, which reports dispensing from 92% of U.S. pharmacies, and 2 provider databases (IQVIA OneKey, National Plan and Provider Enumeration System). Analyses included opioid prescriptions from dentists dispensed in 2019 to patients aged >12 years. High-risk prescriptions were those considered high risk by any of 3 metrics (prescriptions to opioid-naïve patients exceeding a 3-day supply, prescriptions with daily opioid dosage ≥50 morphine milligram equivalents, opioid prescriptions with benzodiazepine overlap). Among all prescriptions and high-risk prescriptions, the authors calculated the proportion accounted for by high-volume dentists -- those with prescription counts in the 95th percentile or higher. Using logistic regression, the characteristics associated with being a high-volume dentist were identified.Results: In 2019, a total of 141,345 dentists accounted for 10,736,743 opioid prescriptions dispensed to patients aged >12 years; 4,242,634 (39.5%) were high-risk prescriptions. The 7,079 high-volume dentists, a group representing 5.0% of the 141,345 dentists, accounted for 46.9% of all prescriptions and 47.5% of high-risk prescriptions. Male sex, younger age, non‒Northeast location, and specialization in oral and maxillofacial surgery were associated with a higher risk of being a high-volume dentist.Conclusions: In 2019, high-volume dentists accounted for almost half of dental opioid prescriptions and high-risk prescriptions. Objective: This study aimed to develop, validate, and apply a scale assessing knowledge of sleep-related myths and truths and associate it with sociodemographic factors.Methods: A scale with 15 questions was created, containing statements about the characteristics of sleep and related to sleep and dentistry. Each answer ranged from 0 to 4 points, generating a total score from 0 to 60, where higher scores represented greater knowledge. A preliminary study with 200 people assessed its convergent and discriminant construct validity, internal consistency, and temporal stability. The main study included 1965 respondents over 18 years. Additionally, sociodemographic data were collected and a classification of the level of knowledge was performed. Data were analyzed with Student's t-test and one-way ANOVA (p < 0.05).Results: The questionnaire showed convergent (p < 0.001) and discriminant (p = 0.024) construct validity, internal consistency (alpha = 0.7), and temporal stability (ICC = 0.87). In the main study, 90.3% of the participants had moderate and high knowledge, with the score ranging from 24 to 58. Adults over 28 years old (p < 0.001), from the southern region of Brazil (p < 0.001), who lived in capital or metropolitan areas (p < 0.001), with higher education (p < 0.001), without religion (p < 0.001), and involved in dentistry (p < 0.001) had greater knowledge than their peers.Conclusions: The scale presented good psychometric properties. Most participants had moderate and high knowledge on sleep, with a difference in knowledge related to the age, region and area of residence, education, involvement with dentistry, and religion. Primary sleep bruxism (SB) affects between 6 and 30% of children and adolescents. Its frequency increases with age, reaching a peak at 10–14 years of age, after which it drops until adulthood. The treatment of SB has not been extensively documented, resulting in a lack of reference points and legitimate concerns for dentists in their everyday practice. The aim of this literature review was to summarize the available evidence on the management of SB in children and adolescents and the efficacy of the different approaches. A systematic literature search was conducted according to PRISMA guidelines from January 2006 to December 2020 using the PubMed, The Cochrane Library, Science Direct, and Google Scholar databases. All types of SB treatment were accepted. Eight studies were selected for their protocol quality, according to the PICOS tool. Several treatment methods were assessed from the eight clinical studies selected, such as occlusal splint, palatal expansion, and pharmacological treatment. Assessment of the treatment methods was difficult because only eight studies were selected, and both the treatment methods and the study protocols used were different. The therapeutic strategies found in the literature were based on the general condition of the case. Treatment approaches were distinguished into treatments for isolated SB and those for SB associated with ventilatory disorders. The current study shows the need to define diagnostic criteria adapted to SB in children in order to improve epidemiological surveys and subsequently clinical practice. To overcome the difficulties related to SB diagnostic criteria, a multidisciplinary approach, involving pediatricians, otorhinolaryngologists, and dental practitioners, is essential for a better management and follow-up of young patients. Taiwan's two-year postgraduate year training program for dentists (PGYD) has been implemented since 2010. However, there was no relevant study for confirming the training effectiveness of PGYD trainees. This study evaluated whether different training institution attributes and different training modes for PGYD trainees could result in differences in the achievement of PGYD training purposes. We adopted the questionnaire survey to collect the views of PGYD trainees on the training effectiveness of PGYD and further evaluated whether different training institution attributes and different training modes for PGYD trainees could result in differences in the achievement of PGYD training purposes. For the "patient-centered" learning environment and the management ability to be a responsible dental clinic owner, the views of PGYD trainees in dental clinics and district hospitals were more positive than those in medical centers. For the clinical professional ability and the management ability to be a responsible dental clinic owner, the views of PGYD trainees in joint group were more positive than those in single hospital. We conclude that both hospitals and dental clinics can achieve PGYD training purposes. The smaller training institutions (dental clinics or district hospitals) are better training institution attributes and the joint group is the better training mode to achieve PGYD training purposes. The joint groups with medical centers combining many dental clinics, especially those in remote areas, may be an excellent training mode, because this training mode has the high potential to subsequently balance the distribution of dentists in Taiwan. In 1896, the first dentist opened his dental clinic in Taipei City and this also opened a new era of Taiwan's dentistry. This study tried to assess the dentist manpower in Taiwan during the Japanese colonial period from 1923 to 1924, and hence to explore the appearance of Taiwan's early dentistry. This study utilized the secondary data analysis to evaluate the numbers and distributions of dentists, physicians, and local practitioners in Taiwan from 1923 to 1924, to find the relationship among them, and further to explore the development of dentists in Taiwan at that period. The total numbers of dentists and physicians increased from 87 to 882 in 1923 to 97 and 927 in 1924, respectively. Moreover, the total number of local practitioners decreased from 583 in 1923 to 558 in 1924. Their Gini coefficients for dentists, physicians, and local practitioners were 0.18, 0.16 and 0.20 in 1923 and 0.27, 0.05 and 0.19 in 1924, respectively. From 1923 to 1924, dentists and physicians were mainly concentrated in the northern and southern regions of Taiwan, and dentists had more serious uneven distribution problem. We conclude that the uneven distribution of dentist in Taiwan is an old problem. After a hundred years of development, the number of dentists increases at a higher rate than the number of physicians and there is no shortage of dentists to date in Taiwan. This study examines the initial moments of dental visits in Korea by focusing on the dentist's very first turn that initiates interaction with the patient. Using the method of conversation analysis, 34 video recordings were transcribed and analyzed to explore the design of the dentist's first turns and how they shape the trajectory of interaction. Findings show that the dentist initiates either task-oriented or social talk in the first turn. With a task-oriented first turn, the dentist moves directly to the business of the visit, constructing the interaction as transactional/task-focused and facilitating the progression of the consultation. In contrast, with social talk, the dentist invites the patent to engage in an opening activity to produce a more social/personal encounter, postponing the launching of medical activities.
