Background: The use of insoles, which is increasingly widespread, can promote changes in biomechanics during running.Research Question: Can the use of insoles with various patterns of infracapital support influence factors related to the dynamic stability of the lower limbs during running on a treadmill in recreational runners?Methods: This is controlled single-blind repeated measures. Static baropodometric data were collected, as well as kinematic data for the lower limbs and electromyographic data for the gluteus maximus and gluteus medius muscles, for twelve recreational runners on a treadmill using four models of insoles (neutral and with forefoot elements - infracapital bar).Results: Neutral insoles were able to reduce laterolateral displacement, increase the displacement of the mass to the posterior, and increase the lateral rotation of the left knee and medial rotation of the right hip. Insoles with a 2 mm total infracapital bar were able to move the mass to the posterior, increase laterolateral displacement and activate the gluteus medius. Insoles with a 2 mm medial infracapital bar were able to increase the displacement of the mass to the posterior, increase the adduction of left hip and the medial rotation of right hip, and activate the gluteus medius. Insoles with a 4 mm medial infracapital element were able to move the body mass to the posterior and to the left, increase laterolateral displacement, increase the adduction of left hip, the medial rotation of right hip and the abduction of right knee.Significance: The insoles evaluated in the present study were able to modify biomechanical variables of recreational runners related to dynamic stability during running on a treadmill and static baropodometric variables. Background: From retrospective research, it is believed that children who predominantly spend their time shod have poorer foot strength and performance than those who are predominantly barefoot. Children's foot motion has been shown to be adversely affected by standard school shoes; however, the long-term effect of moderate minimalist shoes on foot strength, muscle structure and balance in children is unknown.Research Question: Does wearing moderate minimalist shoes, compared to stiff shoes, benefit a child's foot strength, muscle structure and performance over time?Methods: Seventy healthy children (9-12 yr) were randomly assigned to wear standard (control), or minimalist shoes (experimental) at school, for nine months. Cross-sectional areas (CSA) of Abductor Hallucis (AH) and Flexor Digitorum Brevis (FDB) muscles, and toe flexor strength (TFS) of hallux and lesser toes separately, were primary outcome measures. Single leg balance (SLB), Y-balance test (YBT) and standing long jump (SLJ) were secondary outcome measures. Pre- and post-intervention measurements were analysed for between group differences with ANCOVA.Results: Minimalist shoes resulted in moderate but statistically non-significant increases in muscle CSA (AH η2p =.04, FDB η2p =.05) and TFS (hallux η2p =.05, lesser toes η2p =.04). Significant moderate to large improvements in YBT in the experimental group were found in the postero-medial (P = .04, η2p =.07) and postero-lateral (P = .01, η2p =.10) directions. YBT (anterior, postero-medial and postero-lateral) was correlated with hallux TFS (R =.29,.27 and.33 respectively), lesser toes TFS (R =.28,.35 and.38 respectively) and SLJ (R =.30,.39 and.57 respectively). CSA of FDB was correlated with SLJ (R =.34) and SLB (R =.42).Significance: Wearing moderate minimalist shoes long-term improves balance in children. TFS is correlated with better balance and SLJ. Background: Individuals with bipolar disorder have a reduced life expectancy and may experience accelerated biological ageing. In individuals with bipolar disorder and healthy controls, we examined differences in age-related changes in physiology.Methods: UK Biobank recruited more than 500,000 participants, aged 37-73, between 2006 and 2010. Generalised additive models were used to examine associations between age and grip strength, cardiovascular function, body composition, lung function and heel bone mineral density.Results: The main dataset included 271,118 adults (mean age = 56.04 years; 49.60% females). We found statistically significant differences between cases and controls for grip strength, blood pressure, pulse rate and body composition, with standardised mean differences of up to -0.24 (95% CI -0.28 to -0.19). Evidence of differences in lung function, heel bone mineral density or arterial stiffness was limited. Case-control differences were most evident for age-related changes in cardiovascular function (both sexes) and body composition (females). Differences did not uniformly narrow or widen with age and differed by sex. For example, the difference in systolic blood pressure between male cases and controls was -1.3 mmHg at age 50 and widened to -4.7 mmHg at age 65. Diastolic blood pressure in female cases was 1.2 mmHg higher at age 40 and -1.2 mmHg lower at age 65.Limitations: Analyses did not distinguish between bipolar disorder subtypes. Results may not generalise to other age groups.Conclusions: Differences between bipolar disorder cases and controls were most evident for cardiovascular and body composition measures. DEPTOR plays vital roles in the regulation of cell proliferation and survival by directly modulating the activity of mTORC1/2. However, the physiological role of DEPTOR in lung tumorigenesis, as well as its clinical significance, remains elusive. In this study, we revealed that decreased DEPTOR expression correlated with increased tumor size, poor differentiation, and worse survival in patients with lung cancer. DEPTOR depletion promoted cell proliferation, survival, migration, and invasion in human lung cancer cells. Mechanistically, DEPTOR bound to the kinase domain of EGFR via its PDZ domain to inactivate EGFR signal. Thus, DEPTOR depletion not only directly activated mTORC1/2, but also relieved the inhibition of EGFR to subsequently activate mTOR signals, leading to the induction of cell proliferation and survival. Additionally, activated EGFR-mTOR signals upregulated the expression of ZEB1 and SLUG to induce epithelial-mesenchymal transition, resulting in enhanced migration and invasion. Importantly, Deptor deletion accelerated KrasG12D;p53fl/fl-induced lung tumorigenesis and shortened mouse life span via the activation of EGFR-mTOR signals. Effective treatment regimens for triple-negative breast cancer (TNBC) are relatively scarce due to a lack of specific therapeutic targets. Epidermal growth factor receptor (EGFR) signaling is highly active in TNBC and is associated with poor prognosis. Most EGFR antagonists, which significantly improve outcome in lung and colon cancer, have shown limited clinical effects in breast cancer. However, limiting EGFR expression in TNBC is a potential strategy for improving the clinical efficacy of EGFR antagonists. Here, we found that the gamma-aminobutyric acid type A receptor π subunit (GABRP), as a membrane protein enriched in TNBC stem cells, interacted with EGFR and significantly sustained its expression, resulting in stemness maintenance and chemotherapy resistance. Silencing GABRP induced down-regulation of EGFR signaling, which hindered cell stemness and enhanced sensitivity to chemotherapies, including paclitaxel, doxorubicin, and cisplatin. We also identified that retigabine, an FDA-approved drug for adjunctive treatment of seizures, increased the sensitivity of EGFR to gefitinib in gefitinib-resistant cells. Our findings show that GABRP can sustain the stemness of TNBC via modulating EGFR expression, suggesting that GABRP may be a potential therapeutic target that can address EGFR inhibitor resistance in TNBC. Background: Biomechanical analyses primarily focus on physical aspects of human movement; however, it is not understood how walking is affected while simultaneously performing a demanding cognitive task - a form of Cognitive-Motor Interference (CMI). CMI occurs when performance of a primary task (e.g. walking) is affected following the introduction of a cognitive task (e.g. visual search).Research Question: Would Visuo Motor Targeting Task (VMTT) impair visual search performance and reduce the margin of stability (MoS) at higher gait speeds?Methods: A protocol was developed to investigate responses of the neuromuscular system while performing a complex visual search task. The Computer Assisted Rehabilitation ENvironment (CAREN, Motekforce Link, Netherlands) system was used for the experimental design. Twenty male participants (Age = 24.2 ± 2.5yrs, Weight = 70.3 ± 10.6 kg, Height = 178.0 ± 9.1 cm) located and pointed towards targets in complex scenes while walking at different gait speeds (0.55, 1.11 and 1.67 m/s.) or while stationary. The cost of visual search during a Visuo Motor Targeting Task (VMTT) was based on the pointing accuracy during the visual search task.Results: A two-way repeated measure ANOVA showed that MoS in the ML direction significantly improved with increased gait speed and during the visual search task. There was also a significant interaction with MoS improvement being greater during the visual search task at high gait speeds. MoS in the AP was only affected by gait speed. Visual performance and cost of visual search were enhanced during walking versus standing up to 25 %.Significance: This study investigated CMI at different gait speeds, which may have implications in postural control, falls and other neurological disorders. Background: Reduced quadriceps function and proprioception can cause decreased mobility during stair navigation in patients with knee pain. Patients can benefit from interventions to mitigate pain and restore quadriceps function. Activating the somatosensory system via intermittent vibrational stimulation has the potential to improve stair navigation mobility in patients with knee pain by moderating quadriceps inhibition and enhancing proprioception.Research Question: What are the effects of intermittent vibrational stimulus synchronized to stair ambulation on muscle activity, kinematics, kinetics, and pain using a randomized controlled clinical trial design.Methods: Thirty-eight patients with knee pain were enrolled into a blinded cross-over study, and twenty-nine patients completed all assessments and analyses. Subjects were randomly assigned sequentially to both an active Treatment A (active) and passive Treatment B (passive) worn at the knee during ambulation for 4 weeks with a 2-week washout period between treatments.Results: Knee pain during stair navigation was significantly reduced only with Treatment A (P = 0.007). During ascent, Treatment A (active) significantly increased vastus lateralis activation (P = 0.01), increased knee flexion moment (P = 0.04) and decreased trunk flexion angles (P = 0.015) between baseline and 4-week follow-up. After using passive Treatment B, there were no significant differences in pain (P = 0.19), knee flexion moment (P = 0.09), and trunk flexion angles (P = 0.23). Changes in muscle function correlated significantly with changes in knee flexion moment and trunk flexion with Treatment A (P < 0.015). Descending differed from ascending in response to Treatment A with significantly decreased knee flexion moment(P = 0.04), hip(P = 0.02) and ankle(P = 0.04) flexion angles. Treatment B significantly reduced hip flexion angles (P = 0.005) but not knee flexion moment (P = 0.85).Significance: The results of this study suggest that intermittent vibration can improve joint motion and loading during stair navigation by enhancing quadriceps function during stair ascent and improving movement control during stair descent by modifying an adaptive flexed movement pattern in the lower limb. Aim: Inhaled nitric oxide (iNO) during cardiopulmonary resuscitation (CPR) improved systemic hemodynamics and outcomes in a preclinical model of adult in-hospital cardiac arrest (IHCA) and may also have a neuroprotective role following cardiac arrest. The primary objectives of this study were to determine if iNO during CPR would improve cerebral hemodynamics and mitochondrial function in a pediatric model of lipopolysaccharide-induced shock-associated IHCA.Methods: After lipopolysaccharide infusion and ventricular fibrillation induction, 20 1-month-old piglets received hemodynamic-directed CPR and were randomized to blinded treatment with or without iNO (80 ppm) during and after CPR. Defibrillation attempts began at 10 min with a 20-min maximum CPR duration. Cerebral tissue from animals surviving 1-h post-arrest underwent high-resolution respirometry to evaluate the mitochondrial electron transport system and immunohistochemical analyses to assess neuropathology.Results: During CPR, the iNO group had higher mean aortic pressure (41.6 ± 2.0 vs. 36.0 ± 1.4 mmHg; p = 0.005); diastolic BP (32.4 ± 2.4 vs. 27.1 ± 1.7 mmHg; p = 0.03); cerebral perfusion pressure (25.0 ± 2.6 vs. 19.1 ± 1.8 mmHg; p = 0.02); and cerebral blood flow relative to baseline (rCBF: 243.2 ± 54.1 vs. 115.5 ± 37.2%; p = 0.02). Among the 8/10 survivors in each group, the iNO group had higher mitochondrial Complex I oxidative phosphorylation in the cerebral cortex (3.60 [3.56, 3.99] vs. 3.23 [2.44, 3.46] pmol O2/s mg; p = 0.01) and hippocampus (4.79 [4.35, 5.18] vs. 3.17 [2.75, 4.58] pmol O2/s mg; p = 0.02). There were no other differences in mitochondrial respiration or brain injury between groups.Conclusions: Treatment with iNO during CPR resulted in superior systemic hemodynamics, rCBF, and cerebral mitochondrial Complex I respiration in this pediatric cardiac arrest model. Background: A growing body of quantitative evidence has been provided regarding age-related differences in plantar foot loading, multi-segment foot kinematics and muscle activity. Fundamental insight into the joint mechanics and energetics of the maturing foot has yet to be provided.Research Question/hypothesis: It was hypothesized that so-called 'biomechancial maturation' joint kinetics would be observed in children underneath the age of eight and that older age-groups would not differ from each other in these parameters.Methods: Fourty-three typically developing boys were recruited and allocated to three different age groups: 1) an early childhood group, 2) a middle childhood group, and 3) an early and late adolescence group. Multi-segment joint kinematics and kinetics of the Ankle-, Chopart-, Lisfranc- and Hallux joint were collected during barefoot walking. One-way Analysis of Covariance was conducted to examine differences among the outcome measures with group as a fixed factor and walking cadence as covariate.Results: The youngest group differed significantly from the other two age groups with respect to their ankle and chopart joint peak plantarflexion moment (p < 0.05). Ankle and chopart joint peak power generation as well as the lisfranc peak plantarflexion moment was found to be significantly lower in the youngest age group compared to the oldest group (p < 0.05). At the lisfranc joint, the youngest age group demonstrated a significantly higher peak plantarflexion velocity compared to the two older age groups (p < 0.05).Significance: This study provides novel insight into the biomechanical maturation of the developing foot which may guide clinical interventions in paediatric cohorts. Background: Falls among community-dwelling older adults are often triggered by uneven walkways. Joint coordination and its variability change with age and may place older adults at risk of falling. It is unclear how irregular surfaces impact lower-limb joint coordination and if such changes are exacerbated by aging.Research Question: To what extent do lower-limb inter-joint coordination and its variability, over flat and uneven brick walkways, differ between older and young healthy adults?Methods: A motion-capture system collected kinematic data from walking trials on flat and uneven walkways in seventeen older (72.0 ± 4.2 years) and eighteen younger (27.0 ± 4.7 years) healthy adults. Continuous relative phase analyses were performed for the Knee-Hip and Ankle-Knee joint pairs. Mean Absolute Relative Phase (MARP) quantified coordination amplitude. Deviation Phase (DP) quantified coordinative variability. Two-way mixed ANOVA's tested for effects of age, surface, and age × surface interactions.Results: Uneven surfaces prompted more in-phase MARP inter-joint coordination in adults during most gait phases (p ≤ 0.024). Age × surface interactions were observed during initial contact (Ankle-Knee: p = 0.021, Knee-Hip: p = 0.001) and loading response (Knee-Hip: p = 0.017), with post-hoc analyses showing coordination accentuated in older adults. Uneven surfaces induced higher DP in Knee-Hip (p = 0.017) and Ankle-Knee joint coupling (p < 0.001) during gait, largely independent of age. An age × surface interaction was observed during mid-swing (p = 0.050), with post-hoc analysis revealing increased variability in older adults.Significance: More in-phase and variable lower-limb gait behavior was observed on uneven walkways. These differences were accentuated in older adults during early stance phase (more tightly coordinated) and mid-swing (more variable). Background: Postural balance can be considered a conjoined parameter of gross motor performance. It is acquired in early childhood and honed until adolescence, but may also be influenced by various conditions. A simplified clinical assessment of balance and posture could be helpful in monitoring motor development or therapy particularly in pediatric patients. While analogue scales are considered unprecise and lab-based force-plate posturography lacks accessibility, we propose a novel kinematic balance assessment based on markerless 3D sensor technology.Research Question: Can balance and posture be assessed by tracking kinematic data using a single 3D motion tracking camera and are the results representative of normal motor development in a healthy pediatric cohort?Methods: A proprietary algorithm was developed and tested that uses skeletal data from the Microsoft Kinect™ V2 3D motion capture camera to calculate and track the center of mass in real time during a set of balance tasks. The algorithm tracks the distance of the COM traveled over time to calculate a balance score (COM speed). For this study, 432 school children aged 4-18 years performed 5 balance tasks and the resulting balance scores were analyzed and correlated with demographic data.Results: Preliminary experiments demonstrated that the system was able to reliably detect differences in COM speed during different balance tasks. The method showed moderate correlation with age and sex. Athletic activity positively correlated with balance skill in the age group < 8 years, but not in older children. Body mass appeared not to be correlated with balance ability.Significance: This study demonstrates that markerless 3D motion analysis can be used for the clinical assessment of coordination and balance and could potentially be used to monitor gross motor performance at the point-of-care. Background: Dravet Syndrome (DS) is a developmental and epileptic encephalopathy starting in infancy and characterised by treatment resistant epilepsy with cognitive impairment and progressive motor dysfunction. Walking becomes markedly impaired with age, but the mechanical nature of gait problems remains unclear.Research Question: What are the kinetic strategies characterised in gait of patients with DS?Methods: This case-control study compared 41 patients with DS aged 5.2-26.1 years (19 female, 22 male) to 41 typically developing (TD) peers. Three dimensional gait analysis (VICON) was performed to obtain spatiotemporal parameters, kinematics and kinetics during barefoot, level walking at self-selected walking velocity. The sagittal plane support moment was analysed using Statistical Parametric Mapping (SPM). Three DS subgroups were identified based on differences in kinetic strategies characterised by the net internal knee joint moments and trunk lean. Kinematic and kinetic time profiles of the subgroups were compared to the TD group (SPM t-test). Clinical characteristics from physical examination and parental anamnesis were compared between DS (sub)groups using non-parametric tests (Kruskal-Wallis, Wilcoxon rank-sum, Fisher's exact).Results: Support moments in stance were significantly increased in the DS group compared to TD and strongly related to minimum knee flexion in midstance. Persistent internal knee extension moments during stance were detected in a subgroup of 27 % of the patients. A second subgroup of 34 % showed forward trunk lean and attained internal knee flexion moments. The remaining 39 % had neutral or backward trunk lean with internal knee flexion moments. Subgroups differed significantly in age and functional mobility.Significance: Inefficient kinetic patterns suggested that increased muscle effort was needed to control lower limb stability. Three distinct kinetic strategies that underly kinematic deviations were identified. Clinical evaluation of gait should pay attention to knee angles, trunk lean and support moments. Background: Heeled footwear benefits people with movement disorder in the form of shoe lifts, wedges and inserts while its prolonged use causes foot injury in healthy people. There lies a need to detect parameters that affect COP progression of the foot and gait stability due to footwear.Research Question: Do we have bipedal models that can estimate gait parameters corresponding to different center of pressure (COP) trajectories?Method: In this study, we propose a COP translation model that can account for non heeled to heeled footwear. We describe the COP progression as a function of the center of mass (COM) state. This model is used to generate stable steady state walking solutions for different COP profiles. We compare these model solutions with experimental data on non-heeled and heeled-gait.Results: The bipedal model shows stability across different COP profiles. The model estimates GRF profile (R2=0.83 for 1.3 m/s ) for non heeled normal walking qualitatively and on the temporal scale. It estimates GRF due to heeled gait (R2=0.83 for 1.08 m/s) but is limited in estimation of heeled gait parameters.Significance: A bipedal model that can generate stable steady state walking solutions for different forward progressing COP profiles can help in design of foot orthotics for patients with gait disorder and understand injuries occurring due to prolonged wear of rigid heeled footwear. Background: In patients with peripheral arterial disease and presenting intermittent claudication (PAD-IC), the pain due to ischemia impacts gait parameters, particularly in cases of unilateral disease. Deterioration of gait parameters in a pathological context is frequently associated with increased coactivation (simultaneous activation of agonist and antagonist muscles around a joint).Research Question: Does unilateral PAD-IC affect the coactivation pattern during walking? Does the coactivation pattern change with increasing pain intensity?Method: We evaluated symptomatic and asymptomatic legs in 17 subjects with unilateral PAD-IC and 16 without PAD-IC (control group), during walking. Tibialis anterior (TA) and gastrocnemius medialis (GM) electromyographic activity, and peaks of vertical ground reaction force were recorded in this prospective study. We analyzed the coactivation index (CI(GM/TA)) during three periods (pain-free, pain and maximum pain) and phases of the gait cycle. Statistical analysis was carried out using the ANOVA procedure.Results: During single support, CI(GM/TA) increases in the symptomatic leg during the pain period (+28 %) and in the asymptomatic leg during the maximum pain period (+29 %). During second double support, CI(GM/TA) increases in the symptomatic leg only (+49 %). In these gait phases, pain elicits differences in CI(GM/TA) between legs (p < 0.05). Second peak force decreases in the symptomatic leg only (-9%) and is negatively correlated with CI(GM/TA) during the three periods (r = -0.57; -0.76 and -0.78 respectively, p < 0.05). No difference is found in the control group.Significance: The appearance and development of pain in the lower limbs is associated with a higher level of CI(GM/TA), revealing a compensatory gait pattern in PAD-IC patients. Optimal prevention, rehabilitation and re-training strategies for PAD-IC patients should take into consideration neuromuscular compensatory mechanisms between asymptomatic and symptomatic legs. Background: Natural ankle quasi-stiffness (NAS) is a key metric used to personalize orthotic and prosthetic ankle-foot devices. NAS has traditionally been defined as the average slope (i.e. linear regression) of the net ankle moment vs. ankle angle curve during stance. However, NAS appears to have nonlinear characteristics. Characterizing nonlinear NAS across a wide range of tasks will enable us to incorporate these attributes into future orthotic and prosthetic ankle-foot device designs.Research Question: Does nonlinear NAS change across multiple intensities of walking, running, and load carriage tasks?Methods: This observational study examined 22 young, healthy individuals as they walked, ran, and walked while carrying a load at three intensities (speed or load). Linear, quadratic, and cubic regressions were done on the net ankle moment vs. ankle angle curve over three phases of stance: impact, loading, and push-off. RMSE between regressions and measured data were computed to determine regression accuracy, and multilevel linear models (MLMs) were used to determine significant differences between coefficients across intensities.Results: Quadratic and cubic regressions of NAS had significantly lower RMSE than linear NAS for all phases of stance. Because of diminishing reductions in RMSE between quadratic and cubic regressions, only quadratic regression coefficients were further analyzed. Most first (linear) and second (nonlinear) order coefficients of quadratic regressions exhibited clear trends with respect to changes in walking or running speed, but not to increases in load.Significance: This was the first study to our knowledge to thoroughly characterize nonlinear NAS across multiple gait tasks and intensities. Purpose: The purpose of this study was to examine how attentional focus during training influences the effects of a 6-week hip-knee strength training program on pain, function, strength, and kinematics in males and females with Patellofemoral pain (PFP).Methods: Seventy-five males and females with PFP were randomly allocated to a group that trained with an internal focus (n = 25), a group that trained with an external focus (n = 25), or a control group (n = 25). All patients completed testing before (baseline) and after (posttest) the 6-week period. The primary outcome was pain, assessed by Visual Analog Scale (VAS). The secondary outcomes were function, hip muscles strength and lower extremity kinematics, assessed by Kujala Questionnaire, handheld dynamometer and a 2-D motion capture, respectively. All outcomes were measured at the baseline and after the 6-week intervention. Analysis of covariance was used to compare posttest outcomes among the groups while accounting for group differences in baseline performance.Results: The hip-knee strengthening exercises resulted in improved knee valgus (ES(95 % CI) = 0.43(0.26 to 0.75), p = 0.03), and external rotator strength (ES(95 % CI) = 0.51(0.12 to 0.78), p = 0.03) for males and females who trained with an external compared to internal focus.Conclusions: Our findings indicate that males and females with PFP may benefit from completing a hip-knee strengthening training program with an external focus vs. an internal focus. Physical therapists and clinicians should consider using instructions that promote an external focus when implementing hip-knee strengthening training programs with PFP patients. Background: Patellofemoral pain (PFP) is the most common running-related injury. Altered hip and knee kinematics and increases in weekly distance and running pace are often associated with PFP development and exacerbation.Research Question: Are altered movements and training load characteristics (weekly distance and running pace) relate to pain intensity or physical function level in runners with PFP?Methods: Forty recreational runners with PFP (20 males and 20 females) participated in this cross-sectional observational study. Three-dimensional hip and knee kinematics were quantified during the stance phase of running. Weekly distance was defined as the average weekly kilometers of running and running pace as the average pace of the activity measured as minutes per kilometer. A visual analogue scale was used to evaluate worst knee pain during the last week. The anterior knee pain scale (AKPS) was used to evaluate knee functional score. A Pearson correlation matrix was used to investigate the association between each dependent variable (worst pain in the last week and AKPS score) and the independent variables (knee and hip kinematics, weekly distance and running pace).Results: There was no significantly correlation between kinematic variables, pain and functional score for both males and females separately and combined. Weekly distance (km/week) was found to positively correlate to pain intensity (r = 0.452; p < 0.05) in females with PFP. A simple linear regression revealed that weekly distance was significant predictor emerged of pain in females with PFP. Females exhibited significantly greater peak hip adduction and hip adduction ROM than the males and males had significantly greater running pace compared to females.Significance: Weekly distance should be considered in the clinical context during rehabilitation of PFP in females runners aiming at pain reduction. Background: Midfoot and ankle movement dysfunction in people with diabetes mellitus and peripheral neuropathy (DMPN) is associated with midfoot deformity and increased plantar pressures during gait. If midfoot and ankle motion during heel rise and push-off of gait have similar mechanics, heel rise performance could be a clinically feasible way to identify abnormal midfoot and ankle function during gait.Research Question: Is midfoot and ankle joint motion during a heel rise associated with midfoot and ankle motion at push-off during gait in people with DMPN?Methods: Sixty adults with DMPN completed double-limb heel rise, single-limb heel rise, and walking. A modified Oxford multi-segment foot model (forefoot, hindfoot, shank) was used to analyze midfoot (forefoot on hindfoot) and ankle (hindfoot on shank) sagittal angle during heel rise and gait. Pearson correlation was used to test the relationship between heel rise and gait kinematic variables (n = 60). Additionally, we classified 60 participants into two subgroups based on midfoot and ankle position at peak heel rise: midfoot and ankle dorsiflexed (dorsiflexed; n = 23) and midfoot and ankle plantarflexed (plantarflexed; n = 20). Movement trajectories of midfoot and ankle motion during single-limb heel rise and gait of the subgroups were examined.Results: Peak double-limb heel rise and gait midfoot and ankle angles were significantly correlated (r = 0.49 and r = 0.40, respectively). Peak single-limb heel rise and gait midfoot and ankle angles were significantly correlated (r = 0.63 and r = 0.54, respectively). The dorsiflexed subgroup, identified by heel rise performance showed greater midfoot and ankle dorsiflexion during gait compared to the plantarflexed subgroup (mean difference between subgroups: midfoot 3°, ankle 3°).Significance: People with DMPN who fail to plantarflex the midfoot and ankle during heel rise have difficulty plantarflexing the midfoot and ankle during gait. Background: While mini-trampolines have been used among a variety of groups including children as an intervention tool, the motor behavior children adopt while hopping on this soft, elastic surface is unknown. Identifying coordinative structures and their stability for hopping on a mini-trampoline is imperative for recommending future interventions and determining appropriateness to populations with motor dysfunctions.Research Question: Do children demonstrate similar biomechanical and coordination patterns as adults while hopping on a mini-trampoline?Methods: Fifteen adults aged 18-35 years and 14 children aged 7-12 years completed bouts of continuous two-legged hopping in-place on a stiff surface for 10 s at a time and on a mini-trampoline for 30 s at a time. 3-D motion capture tracked whole-body movement. We evaluated whole-body vertical stiffness as a ratio of peak vertical force and peak vertical displacement, as well as spatiotemporal parameters of hopping. Coordinative structures were evaluated as continuous relative phase angles of the foot, shank, thigh, and pelvis segments.Results and Significance: Adults did not modify whole-body vertical stiffness on a mini-trampoline, while children increased whole-body vertical stiffness to compensate for the reduced surface stiffness. Both groups conserved the coordinative structure for hopping on a mini-trampoline by modulating hopping cycle timing. Moreover, children hopped with an adult-like coordinative structure, but required greater shank-thigh and thigh-pelvis out-of-phase motion. However, the consistency of their coordination was diminished compared to adults. Children aged 7-12 years old have formed a stable coordinative structure for spring-mass center-of-mass dynamics that is preserved on this soft, elastic surface. However, children might be developing control strategies for preferred whole-body vertical stiffness, particularly when required to dampen peak vertical forces. These results highlight the importance of evaluating the emerging motor behavior to manipulated environmental constraints, particularly when considering the utility and appropriateness of mini-trampoline interventions for children with motor dysfunctions. Background: Sensorimotor foot orthoses is an alternative concept, which in addition to mechanical effects, are designed to change muscle activation by altering sensory input to the plantar surface of the foot. However, there is little evidence of how these affect the kinematics of the foot during gait.Research Question: The aim of the study was to explore the immediate effect of calcaneal medial heel bars and retrocapital bars on foot kinematics during the stance phase of gait.Methods: Kinematic data were collected from 32 healthy individuals using an eight camera motion capture system and a six-degrees-of-freedom multi-segment foot model in three different orthotic conditions; calcaneal medial heel bar, retrocapital bar, and no orthosis. Vector field statistical analysis was performed to explore the effect of the orthotic conditions over the kinematic time series curves during stance phase. Peak median and interquartile ranges were also reported during the different phases of stance.Results: The calcaneal medial bar significantly decreased rearfoot eversion for the majority of the stance phase and compensatory increased midfoot eversion during the entire stance phase compared to the no orthosis condition. The retrocapital bar rotated the foot externally significantly abducting the rearfoot for the entire stance phase and the midfoot for the majority of stance phase.Significance: The calcaneal medial heel bar and retrocapital bar significantly altered the foot kinematics in a way that may benefit patients with abnormal pronation and intoeing gait. Background: Kinematic changes associated with knee osteoarthritis (OA) have been traditionally measured with camera-based gait analysis. Lately, inertial sensors have become popular for gait analysis with the advantage of being less time consuming and not requiring a dedicated laboratory.Research Question: Do spatiotemporal and discrete kinematic gait parameters measured with the inertial sensor system RehaGait® differ between the affected and unaffected side in patients with unilateral knee OA and between patients with severe knee OA and asymptomatic control subjects? Do these differences have a similar magnitude as those reported in the literature?Methods: Twenty-two patients with unilateral knee OA scheduled for total knee replacement and 46 age matched control subjects were included in this study. Spatiotemporal parameters and sagittal kinematics at the hip, knee, and ankle joint were measured using the RehaGait® system while walking at a self-selected speed for a distance of 20 m and compared between groups.Results: Patients with knee OA had slower walking speed, longer stride duration, shorter stride length and lower cadence (P < 0.001). Peak knee flexion during stance and swing was lower in the affected than the unaffected leg (-4.8° and -6.1°; P < 0.01). Peak knee flexion during stance and swing (-5.2° and -8.8°; P < 0.01) and knee range of motion during loading response and swing (-3.6° and -4.4°; P < 0.01) were lower than in the control group.Significance: These side to side differences within patients and differences between patients with knee OA and control subjects agree with known gait alterations measured with camera-based systems. Background: Gait event detection of the initial contact and toe off is essential for running gait analysis, allowing the derivation of parameters such as stance time. Heuristic-based methods exist to estimate these key gait events from tibial accelerometry. However, these methods are tailored to very specific acceleration profiles, which may offer complications when dealing with larger data sets and inherent biological variability.Research Question: Can a structured machine learning approach achieve a more accurate prediction of running gait event timings from tibial accelerometry, compared to the previously utilised heuristic approaches?Methods: Force-based event detection acted as the criterion measure in order to assess the accuracy, repeatability and sensitivity of the predicted gait events. 3D tibial acceleration and ground reaction force data from 93 rearfoot runners were captured. A heuristic method and two structured machine learning methods were employed to derive initial contact, toe off and stance time from tibial acceleration signals.Results: Both a structured perceptron model (median absolute error of stance time estimation: 10.00 ± 8.73 ms) and a structured recurrent neural network model (median absolute error of stance time estimation: 6.50 ± 5.74 ms) significantly outperformed the existing heuristic approach (median absolute error of stance time estimation: 11.25 ± 9.52 ms). Thus, results indicate that a structured recurrent neural network machine learning model offers the most accurate and consistent estimation of the gait events and its derived stance time during level overground running.Significance: The machine learning methods seem less affected by intra- and inter-subject variation within the data, allowing for accurate and efficient automated data output during rearfoot overground running. Background: There is a paucity of available biomechanical kinetic data comparing shod and barefoot conditions in children.Research Question: Do children wearing footwear have comparable gait velocity, ground reaction forces (GRF), spatiotemporal parameters, propulsive and braking impulses when compared to children walking barefoot?Methods: Seventy-five children were divided into four groups: Group 1 females aged 4-9 years old (n = 29). Group 2 females aged 3-5 years old (n = 16). Group 3 males aged 6-9 years old (n = 13). Group 4 males aged 4-8 years old (n = 17). Children walked at a self-selected pace over a walkway of force platforms. Each footwear and barefoot represented a separate condition. The order of conditions was randomized. A repeated-measures ANOVA was performed to investigate the effects of the footwear type on gait parameters in each group. Multiple comparisons with Bonferroni corrections were conducted when appropriate.Results: There were no statistical differences in velocity or in vertical and anteroposterior GRF across conditions for all groups. There was a significant effect of the footwear worn on time to loading response peak (p = 0.008), time to midstance force (p = 0.006), and time to propulsive peak (p < 0.001). For Group 3, there was a significant effect of the footwear worn on time to braking peak (p < 0.001) and time to propulsive peak (p < 0.001). Regarding impulses for Group 1, there was a significant effect of the footwear worn on the loading response impulse (p = 0.016) and terminal stance and pre-swing impulse (p = 0.001). Background: Posterior tibial tendon dysfunction (PTTD) is characterized by degeneration of this tendon leading to a flattening of the medial longitudinal arch of the foot. Foot orthoses (FOs) can be used as a treatment option, but their biomechanical effects on individuals with PTTD are not yet fully understood.Research Question: The aim of this study was to investigate the effects of three types of FOs on gait biomechanics in individuals with PTTD.Methods: Fourteen individuals were recruited with painful stage 1 or 2 PTTD based on Johnson and Strom's classification. Quantitative gait analysis of the affected limb was performed in four conditions: shoes only (Shoe), prefabricated FO (PFO), neutral custom FO (CFO) and custom varus FO (CVFO) with a 5° medial wedge and a 4 mm medial heel skive. A curve analysis, using 1D statistical parametric mapping, was undertaken to assess differences in lower limb joint motion, joint moments and muscle activity over the stance phase of gait across conditions.Results: Decreased hindfoot eversion angles, decreased ankle inversion moments and increased ankle eversion moments were observed with custom FOs compared to the Shoe and PFO conditions (p < 0.001). CFOs and CVFOs induced an increased knee abduction moment compared to Shoe (p < 0.001). No changes in hip kinematics and kinetics or in EMG activity of tested muscles were observed between conditions.Significance: Custom orthoses may be more suitable than PFOs to decrease the pathological biomechanical outcomes observed in PTTD. Decreased ankle inversion moments during the stance phase could explain why custom orthoses are effective at reducing pain in PTTD patients. Background: Inertial measurement units (IMUs) are promising tools for collecting human movement data. Model-based filtering approaches (e.g. Extended Kalman Filter) have been proposed to estimate joint angles from IMUs data but little is known about the potential of data-driven approaches.Research Question: Can deep learning models accurately predict lower limb joint angles from IMU data during gait?Methods: Lower-limb kinematic data were simultaneously measured with a marker-based motion capture system and running leggings with 5 integrated IMUs measuring acceleration and angular velocity at the pelvis, thighs and tibias. Data acquisition was performed on 27 participants (26.5 (3.9) years, 1.75 (0.07) m, 68.3 (10.0) kg) while walking at 4 and 6 km/h and running at 8, 10, 12 and 14 km/h on a treadmill. The model input consists of raw IMU data, while the output estimates the joint angles of the lower body. The model was trained with a nested k-fold cross-validation and tested considering a user-independent approach. Mean error (ME), mean absolute error (MAE) and Pearson correlation coefficient (r) were computed between the ground truth and predicted joint angles.Results: MAE for the DOFs ranged from 2.2(0.9) to 5.1(2.7)° with an average of 3.6(2.1)°. r ranged from 0.67(0.23) to 0.99(0.01) with moderate correlation (0.4≤r<0.7) was found for the hip right rotation and lumbar extension, strong correlation (0.7≤r<0.9) was found for the hip left rotation and ankle right/left inversion while all other DOFs showed very strong correlation (r≥0.9).Significance: The proposed model can reliably predict joint kinematics for walking, running and gait transitions without specific knowledge about the body characteristics of the wearer, or the position and orientation of the IMU relative to the attached segment. Background: The head, arms and trunk segment constitute a large proportion of the body's mass. Therefore, small alterations in trunk inclination may affect lower limb joint moments and muscle activation patterns. Although previous research has investigated the effect of changing frontal plane inclination of the trunk, it is not clear how increasing trunk flexion will impact on the activation of the lower limb muscles.Research Question: What is the effect of independently manipulating trunk flexion angle on lower limb kinematics, moments and muscle function?Methods: Gait analysis was carried out on 20 healthy people under four trunk flexion conditions: normal walking (NW), NW-5°, NW+5° and NW+10°. For the latter three conditions, a biofeedback approach was used to tightly control trunk flexion angle. A linear mixed model was used to investigate the effect of changing trunk flexion on joint angles, moments, and knee muscle activation.Results: There were clear increases in hip and ankle moments as trunk flexion was increased, but no change in knee moments. The results also showed a linear increase in knee flexor muscle activity and a corresponding increase in co-contraction as trunk flexion increased. Interestingly, there was a dramatic change in the profile of hamstring activity. In the medial hamstrings, this change led to a 100% increase in activation during early stance as flexion was increased by 5° from NW.Significance: This is the first study to demonstrate a strong dependence of knee flexor muscle activity on trunk flexion. This is important as people with knee osteoarthritis have been observed to walk with elevated muscle activation and this has been linked to increased joint loads. Background: Obesity and female sex are independent risk factors for knee osteoarthritis and also influence gait mechanics. However, the interaction between obesity and sex on gait mechanics is unclear, which may have implications for tailored gait modification strategies.Research Question: The purpose of this study was to examine the influence of obesity and sex on sagittal and frontal plane knee mechanics during gait in young adults.Methods: Forty-eight individuals with (BMI = 33.03 ± 0.59; sex:50 % female; age:21.9 ± 2.6 years) and 48 without obesity (BMI:21.59 ± 0.25; sex:50 % female; age:22.9 ± 3.57 years) matched on age and sex completed over-ground gait assessments at a self-selected speed. Two (BMI) by two (sex) analysis of variance was used to compare knee biomechanics during the first half of stance in the sagittal (knee flexion moment [KFM] and excursion [KFE]) and frontal plane (first peak knee adduction moment [KAM], knee varus velocity [KVV]).Results: We observed a BMI by sex interaction for normalized KFM (P = 0.03). Females had smaller normalized KFM compared to males (P = 0.03), but only in individuals without obesity. Males without obesity had larger normalized KFM compared to males with obesity (P = 0.01), while females did not differ between BMI groups. We observed main effects of sex and BMI group, where females exhibited greater normalized KAM (P < 0.01) and KVV (P < 0.01) compared to males, and individuals with obesity walked with greater KVV compared to those without obesity (P < 0.01). All absolute joint moments were greater in individuals with obesity (all P<0.01) and males had greater absolute KFM compared to females (P < 0.01).Significance: We observed sex differences in gait mechanics, however, KFM differences between males and females were only evident in individuals without obesity. Background: Although it is assumed that the presence of patellofemoral pain (PFP) may result in compensatory behaviors that can alter trunk kinematics and lower limb mechanics, the influence of the exacerbation of patellofemoral pain on trunk kinematics and lower limb mechanics during stair negotiation has not been established.Research Question: Does the exacerbation of PFP symptoms lead to altered trunk kinematics and lower limb mechanics during stair negotiation?Methods: Three-dimensional kinematics and kinetics were obtained from 45 women with PFP during stair descent and ascent. Data were obtained before and after a pain exacerbation protocol. The variables of interest were peak trunk, hip, and knee flexion, and ankle dorsiflexion; peak hip, and knee extensor, and ankle plantarflexor moments. Paired t-tests were used to compare the variables of interest before and after pain exacerbation.Results: Following pain exacerbation, there was a decrease in peak knee extensor moment during stair descent (Effect size = -0.68; p = 0.01) and stair ascent (Effect size = -0.56; p = 0.02); as well as in peak ankle dorsiflexion during stair descent (Effect size = -0.33; p = 0.01) and stair ascent (Effect size = -0.30; p = 0.01). An increase in ankle plantarflexor moment during stair descent (Effect size = 0.79; p < 0.01) and stair ascent (Effect size = 0.89; p < 0.01) was also observed. No significant differences were observed for peak trunk, hip, and knee flexion or hip extensor moment (p > 0.05).Significance: Our findings show compensatory strategies used by people with PFP in response to symptoms exacerbation that may have a negative impact on knee and ankle mechanics. Our findings also suggest that people with PFP do not seem to change their trunk, hip, and knee flexion or hip extensor moment during stair negotiation in response to symptom exacerbation. Background: Increasing the ankle plantar-flexion angle at initial contact (IC) during landing reduces the impact features associated with landing, such as the vertical ground reaction force and loading rate, potentially affecting the risk of anterior cruciate ligament (ACL) injury. However, the relationships between the ankle plantar-flexion angle at IC and the previously identified biomechanical factors related to noncontact ACL injury have not been studied.Research Question: Thus, the purpose of this study was to determine whether significant relationships exist between the ankle plantar-flexion angle at IC and the biomechanical factors related to noncontact ACL injury.Methods: The peak anterior tibial shear force, peak external knee valgus moment, peak knee valgus angle, and combined peak external knee valgus plus tibial internal rotation moments were measured in 26 individuals while performing self-selected, single-leg landing. Background: Studies of walking in those with femoroacetabular impingement syndrome have found altered pelvis and hip biomechanics. But a whole body, time-contiuous, assessment of biomechanical parameters has not been reported. Additionally, larger cam morphology has been associated with more pain, faster progression to end-stage osteoarthritis and increased cartilage damage but differences in walking biomechanics between large compared to small cam morphologies have not been assessed.Research Question: Are trunk, pelvis and lower limb biomechanics different between healthy pain-free controls and individuals with FAI syndrome and are those biomechanics different between those with larger, compared to smaller, cam morphologies?Methods: Twenty four pain-free controls were compared against 41 participants with FAI syndrome who were stratified into two groups according to their maximum alpha angle. Participants underwent three-dimensional motion capture during walking. Trunk, pelvis, and lower limb biomechanics were compared between groups using statistical parametric mapping corrected for walking speed and pain.Results: Compared to pain-free controls, participants with FAI syndrome walked with more trunk anterior tilt (mean difference 7.6°, p < 0.001) as well as less pelvic rise (3°, p < 0.001), hip abduction (-4.6°, p < 0.05) and external rotation (-6.5°, p < 0.05). They also had lower hip flexion (-0.06Nm⋅kg-1, p < 0.05), abduction (-0.07Nm⋅kg-1, p < 0.05) and ankle plantarflexion moments (-0.19Nm⋅kg-1, p < 0.001). These biomechanical differences occurred throughout the gait cycle. There were no differences in walking biomechanics according to cam morphology size.Significance: Results do not support the hypothesis that larger cam morphology is associated with larger differences in walking biomechanics but did demonstrate general differences in trunk, pelvis and lower limb biomechanics between those with FAI sydrome and pain-free controls. Altered external biomechanics are likely the result of complex sensory-motor strategy resulting from pain inhibition or impingement avoidance. Background: Progressive loading of the lower limb muscles during running on a positive pressure or reduced gravity (Alter-G™) treadmill is suggested as a rehabilitation strategy after muscle and tendon injury but the influence of running up or downhill and at higher speeds is not known, nor are the interaction effects of speed, inclination, and indicated bodyweight.Research Question: What are the lower limb EMG activation levels and cadence when running up and downhill in normal and reduced gravity?Methods: 10 recreationally active male athletes ran on a positive-pressure Alter-G™ treadmill at: 3 indicated bodyweights (60 %, 80 %, and 100 %); 5 speeds (12, 15, 18, 21, and 24 km/h); for incline, decline, and flat conditions (-15 %, -10 %, -5%, 0%, 5%, 10 %, and 15 %); while monitoring the surface EMG of 11 leg muscles as well as cadence (strides per minute).Results and Significance: Linear mixed models showed significant effect of running speed, inclination, and indicated bodyweight, with interaction effects observed. Increasing running speed was associated with the largest change in activity, with smaller effects for increasing bodyweight and inclination. Downhill running was associated with reduced activity in all muscle groups, and more tightly clustered activity patterns independent of speed. Substantial variation in sEMG activity occurred in the flat and uphill conditions. Subject responses were quite variable for sEMG, less so for cadence. For the conditions examined, increasing running speed induced the largest changes in EMG of all muscles examined with smaller changes seen for manipulations of inclination and bodyweight. Background: Supplementing animal diets with fish oil increases myocardial omega-3 polyunsaturated fatty acids [ω-3 (n-3) PUFA], lowers heart rate, and prevents malignant cardiac arrhythmias. In contrast to epidemiological reports, results of some human clinical trials and of unphysiologically high doses employed in animal studies call into question the application of dietary ω-3 PUFA for cardioprotection.Objective: This study tested the hypothesis that low ω-3 PUFA dietary thresholds for myocardial incorporation in rats, equivalent in dose to what humans derive from eating fish, can reduce heart rate and arrhythmia vulnerability.Methods: Male Sprague-Dawley rats (12-15 wk old) were fed isoenergetic diets containing 10% fat for 4-5 wk. The control diet (CON) contained 5.5% beef tallow, 2.5% sunflower seed oil, and 2% olive oil. Fish oil diets contained high-DHA tuna oil, exchanged for olive oil: 0.31% [fish oil group 1 (FO1)] (human equivalent EPA + DHA 570 mg/d); 1.25% [fish oil group 2 (FO2)] (equivalent EPA + DHA 2.3 g/d). Anaesthetized rats (pentobarbital, 60 mg/kg intraperitoneally) were subjected in vivo to 15-min cardiac ischemia by left coronary artery occlusion and then reperfusion, with arrhythmias detected by electrocardiogram.Results: Fish oil dose dependently modulated myocardial membrane fatty acids (DHA mean ± SEM: CON, 5.0 ± 0.2%; FO1, 13.1 ± 0.9%; FO2, 18.3 ± 0.4%; n = 4-5; P-trend < 0.001 ANOVA); resting heart rate (CON, 453 ± 6; FO1, 432 ± 4; FO2, 422 ± 5 bpm; n = 15-18; P-trend < 0.001); reduced ventricular fibrillation (VF) (CON, 89%; FO1, 60%; P = 0.052; FO2, 50%; n = 15-18; P = 0.013 chi square); and total arrhythmia severity (arrhythmia score: CON, 6.1 ± 0.4; FO1, 4.6 ± 0.5; FO2, 3.1 ± 0.7; n = 15-18; P-trend < 0.01) during ischemia and reperfusion (VF: Con, 86%; FO1, 22% P = 0.011; FO2, 8% P = 0.001; n = 7-12); (arrhythmia score: CON, 4.6 ± 0.3; FO1, 3.1 ± 0.3; FO2, 1.3 ± 0.3; n = 7-12; P-trend < 0.001).Conclusions: Ventricular arrhythmias were prevented and heart rate was slowed by lower ω-3 PUFA intake in rats than previously reported, equivalent to human fish consumption and associated with increased myocardial DHA. The efficacy of low-dose fish oil demonstrates biological plausibility for nutritional ω-3 fatty acid-mediated cardioprotection and suggests that effectiveness in human clinical trials may be obscured by failure to exclude fish eaters. Objectives: Although the overuse injury rate of the Achilles tendon (AT) for running is high, the effect of shoe conditions on AT loading remains unclear. Hence, this study aims to determine the mechanical properties of AT during running under different shoe conditions (minimalist vs. conventional shoes).Design: This work is a controlled laboratory study.Methods: Sixteen healthy male rearfoot strike runners were recruited to complete over ground running trials at 3.33 m/s (±5%) under two shoe conditions (minimalist shoes: INOV-8 Bare-XF 210; conventional shoes: NIKE AIR ZOOM PEGASUS 34). Sagittal plane ankle kinematics and ground reaction forces were simultaneously recorded. Ankle joint mechanics (ankle joint angle and moment) and the mechanical properties (peak force, impulse, stress, strain, and their corresponding peak rate) of the AT were calculated.Results: In comparison with conventional shoes, wearing minimalist shoes showed significant changes (p < 0.05): (1) decreased strike angle (48.92 ± 9.01 vs. 41.04 ± 8.69°); (2) increased ankle moment (2.34 ± 0.44 vs. 2.55 ± 0.46 Nm/kg); (3) increased peak AT force (5.85 ± 1.22 vs. 6.24 ± 1.13 BW), AT force impulse (0.65 ± 0.13 vs. 0.70 ± 0.13 BW·s), peak AT loading rate (109.94 ± 9.33 vs. 118.84 ± 26.62 BW/s), and average loading rate (48.42 ± 15.64 vs. 54.90 ± 17.47 BW/s); (4) decreased time to peak AT force (126.31 ± 20.68 vs. 117.77 ± 17.62 ms); (5) increased AT stress (66.96 ± 14.59 vs. 71.89 ± 14.74 MPa), strain (8.19 ± 1.77 vs. 8.78 ± 1.80 %), peak AT stress rate (66.96 ± 14.59 vs. 71.89 ± 14.74 MPa/s), and strain rate (148.71 ± 48.52 vs. 167.28 ± 42.82 %/s).Conclusion: Increased AT force, loading rate, and stress were observed in runners who habitually wear conventional shoes with rearfoot strike patterns when they wore minimalist shoes. Background: The differences and relationship between joint stiffness and leg stiffness can be used to characterize the lower limb behavior during different walking speeds.Research Question: This study aimed to investigate the differences in whole leg and lower limb joint stiffness at different walking speeds and the interactions between leg and lower limb joint stiffness.Methods: Twenty-seven healthy adults, seventeen males (age: 19.6 ± 2.2 years, height: 176.0 ± 6.0 cm, mass: 69.7 ± 8.9 kg), and ten females (age: 19.1 ± 1.9 years, height: 164.0 ± 3.0 cm, mass: 59.6 ± 3.8 kg), were recruited. Dynamic leg and joint stiffness were calculated during eccentric loading from data recorded using 3D infrared motion analysis and force plates at slow, normal, and fast walking speeds. Differences in dynamic stiffness, joint angles and moments were explored between the walking speeds using Repeated Measures ANOVA with Sidak post-hoc tests. Correlations between leg, joint stiffness, and walking speed were also explored.Results: The results indicated that the leg dynamic stiffness is decreased by walking speed, however, hip and ankle joint stiffness were increased (p < 0.001) and knee stiffness was unaffected. Leg stiffness showed no correlation with hip, knee, or ankle stiffness. A positive significant correlation was seen between hip and ankle stiffness (p < 0.01) and between knee and ankle stiffness (p < 0.001), however, no correlation was seen between hip and knee stiffness.Significance: These results suggest leg stiffness is not associated with lower limb joint stiffness during eccentric loading. Background: The ankle-strategy model, where the human body is modeled as a single inverted pendulum hinged at the ankle, has been used for decades to study the dynamics and the stability of the human upright posture (UP). However, the contribution of the hip joints is critical whenever postural disturbances are considered. To account for hip contribution, a double inverted pendulum (DIP) model rotating about the ankle and hip joints has been recently proposed in our previous work but experimental validation efforts are scarce.Method: In the present study, it is investigated whether the DIP model is able to reproduce the experimentally observed frequency spectrum of the ankle and hip joint kinematic for young and elderly subjects balancing on a compliant surface. The DIP model based and experimental kinematics are compared via Fourier analysis to obtain their corresponding amplitude spectrum density (ASD) functions. Quantitative comparisons of the ASD functions are accomplished through Bland-Altman (B&A) analysis, and Pearson correlation coefficient (PCC).Results: The DIP model can reproduce part of the experimental frequency spectrum of the ankle and hip joint angle position and velocity, especially for frequencies larger than 0.35 Hz. Moreover, the model captures the decaying behavior of the experimental ASD functions as frequency increases. With respect to joint angle velocities, the highest PCC between model-based and experimental ASD functions is found for the hip joint of elderly subjects. The B&A analysis shows that the zero-difference between model-based and experimental ASD functions lies between the 95 % confidence interval, especially for the joint angle position results. These suggest that the DIP model reproduces part of the experimentally observed frequency spectrum, which validates the model to study the dynamics and stability of the human upright posture. Objective: The aim of this study was to determine the effectiveness of a novel body-weight-supported (BWS) gait training system with visual feedback, called Copernicus® (Rehalife, Italy). This computerized device provides comfortable, regular and repeatable locomotion in hemiplegic patients. Through visual real-time monitoring of gait parameters, patients are trained to transfer weight loading alternately on both feet.Design: A single-blind, randomized controlled study. A single center used a computer-generated randomization code to allocate treatments.Setting: Intensive rehabilitation unit (IRU) at the Institute S. Anna (Italy).Participants: 63 first-ever stroke patients (39 men, age: 66.1 ± 9.6 years; 61.6 % with left-sided lesion) randomly distributed into three demographically/clinically matched groups.Treatments: All groups were treated five times a week for 2 -h sessions for six consecutive weeks. The first group ("control") underwent a conventional physical therapy; the second group performed advanced BWS gait training sessions without visual feedback (Experimental VF- group); whereas the third group used BWS with visual feedback stimulation (Experimental VF+ group).Main Outcome Measures: Absolute changes were recorded using conventional clinical scales and kinematic measurement of static gait balance from baseline to follow-up.Results: Significant interaction Group*Time effects scales (F2,126 = 5.1, p-level = 0.005, η²p = 0.25; F2,126 = 4.7, p-level = 0.007, η²p = 0.19; respectively) were detected in the Functional Independence Measure and Tinetti-Balance scales. Post hoc analysis demonstrated that the recovery of motor functioning was greater for the VF + group with respect to other groups (all p's ≤ 0.001). A similar pattern of findings was also obtained with a stabilometric analysis, demonstrating a better clinical improvement in static balance after VF + treatment.Conclusion: The proposed advanced rehabilitation system with visual feedback was more effective in improving gait recovery with respect to conventional and high-tech therapies without a sensor feedback. Background: Thumb orthoses are a standard treatment modality, with substantial evidence to support its usage for multiple conditions affecting the upper extremity. Despite commonly prescribed, little is known about the immediate impact of such devices on the upper extremity, including potential modifications on motor patterns.Research Question: We aimed to determine the changes in the upper limb kinematics during the usage of thumb orthotics, comparing differences in orthotic design, length, and fabrication materials.Methods: In this cross-sectional study, subjects performed a standardized reaching task and the placing subtest of the Minnesota Manual Dexterity Test (MMDT) while wearing five unique thumb orthoses. Besides the active range of motion of the shoulder, elbow, wrist and hand joints, movement smoothness (Number of Movement Units-NMU), speed, and motion control strategies were analyzed through eight Qualisys Oqus 300 cameras (Qualisys AB, Göteborg, Sweden).Findings: Ten non-disabled, university students participated in this study. Despite differences in fabrication materials, all orthotics reduced thumb's abduction (13.3° to 4.3°), and metacarpophalangeal flexion (11.5° to 4.2°). Although orthotics impacted movement smoothness and hand function during its usage, forearm-based devices further increased the NMUs and the time required for the MMDT performance (Control: NMU = 4.8, MMDT = 58.1; Long Orthotics: NMU = 6.6, MMDT = 78.2), while short, flexible orthoses provided thumb stabilization without significant impact on upper extremity movement strategies.Significance: Although joint stabilization was similar among orthotics fabricated with rigid and flexible materials, the improved hand dexterity observed during the use of flexible devices suggests an advantage of flexible orthotics for enhanced stability and hand function. These results can assist healthcare professionals during the selection and prescription of thumb orthotics, providing information not only on the range of motion but other sensorimotor aspects involved in upper extremity movement patterns that may be affected by orthotics usage. Background: The Oxford Foot Model (OFM) and Rizzoli Foot Model (RFM) are the two most frequently used multi-segment models to measure foot kinematics. However, a comprehensive comparison of the kinematic output of these models is lacking.Research Question: What are the differences in kinematic output between OFM and RFM during normal gait and typical pathological gait patterns in healthy adults?.Methods: A combined OFM and RFM marker set was placed on the right foot of ten healthy subjects. A static standing trial and six level walking trials were collected for normal gait and for four voluntarily adopted gait types: equinus, crouch, toe-in and toe-out. Joint angles were calculated for every trial for the hindfoot relative to shank (HF-SH), forefoot relative to hindfoot (FF-HF) and hallux relative to forefoot (HX-FF). Average static joint angles of both models were compared between models. After subtracting these offsets, the remaining dynamic angles were compared using statistical parametric mapping repeated measures ANOVAs and t-tests. Furthermore, range of motion was compared between models for every angle.Results: For the static posture, RFM compared to OFM measured more plantar flexion (Δ = 6°) and internal rotation (Δ = 7°) for HF-SH, more plantar flexion (Δ = 34°) and inversion (Δ = 13°) for FF-HF and more dorsal flexion (Δ = 37°) and abduction (Δ = 12°) for HX-FF. During normal walking, kinematic differences were found in various parts of the gait cycle. Moreover, range of motion was larger in the HF-SH for OFM and in FF-HF and HX-FF for RFM. The differences between models were not the same for all gait types. Equinus and toe-out gait demonstrated most pronounced differences.Significance: Differences are present in kinematic output between OFM and RFM, which also depend on gait type. Background: Impairment of protective steps to recover balance from external perturbation is evident after stroke. Voluntary-induced stepping response (VSR) can be used to practice protective steps by instructing an individual to voluntarily lean their whole body forward until they perceive a loss of balance and automatically induce a step. However, to improve protective stepping performance, detailed characteristics of VSR in healthy persons are required.Research Question: What is the difference in VSR between healthy and persons with stroke?Methods: An observational study was conducted in 30 participants, (10 young, 10 older, and 10 persons with stroke). All participants performed VSR for 10 trials. Step length, step width, step duration, CoM position, CoM velocity, trunk-hip displacement, and strategies of response were recorded using a motion capture system and analysed using Matlab software. Statistical analysis was performed using One-way ANOVA and Chi-square.Results: On average, participants with stroke had shorter step lengths and step durations than young and older adults. Step width of older adults and participants with stroke was wider than that of young adults (p < 0.05). While multiple steps and losing balance were reported more frequently in participants with stroke than the others, the percentage of trials in which participants grasped the handrails was not significantly different between older adults and participants with stroke. CoM position, CoM velocity, and trunk-hip displacement at foot liftoff were significantly smaller in older adults and participants with stroke than young adults (p < 0.05). Participants with stroke tended to use trunk bending rather than trunk leaning strategies to generate VSR in contrast to healthy participant. The prevalence of the trunk bending strategy was also greater in older adults than young adults.Significance: Values obtained from healthy groups can be used as guidelines to set realistic goals during VSR training to improve protective steps in patients with stroke. Background: Whole-body movement adjustments during gait are common post-stroke, but comprehensive ways of quantifying and evaluating gait from a whole-body perspective are lacking.Research Question: Can novel kinematic variables related to Center of Mass (CoM) position discriminate side asymmetries as well as coordination between the upper and lower body during gait within persons post-stroke and compared to non-disabled controls?Methods: Thirty-one persons post-stroke and 41 age-matched non-disabled controls walking at their self-selected speed were recorded by 3D motion capture. The Ankle-CoM Inclination Angle (A-CoMIA) and the Head-CoM Inclination Angle (H-CoMIA) defined the angle between the CoM and the ankle and the head, respectively, in the frontal plane. These angles and their angular velocities were compared between groups, and with regard to motor impairment severity during all phases of the gait cycle (GC) using a functional interval-wise testing analysis suitable for curve data. Upper and lower body coordination was assessed using cross- correlation.Results: The A-CoMIA was symmetrical between body sides in persons post-stroke but larger compared to controls. The angular velocity of A-CoMIA also differed when compared to controls. The H-CoMIA was consistently asymmetrical in persons post-stroke and larger than in controls throughout the stance phase. There were only minor group differences in the angular velocity of H-CoMIA, with some side asymmetry in persons post-stroke. The A-CoMIA of the non-affected side, and the H- CoMIA, discriminated between persons with more severe impairments compared to those with milder impairments post-stroke. The variables showed strong cross- correlations in both groups.Significance: The A-CoMIA and Head-CoMIA discriminated post-stroke gait from non-disabled, as well as motor impairment severity. These variables with the advantageous curve analysis during the entire GC add valuable whole-body information to existing parameters of post-stroke gait analysis through assessment of symmetry and upper and lower body coordination. Background: Abnormal upper limb movements frequently affect people with acquired brain injury (ABI) during walking. Three-dimensional motion analysis (3DMA) can quantify upper limb abnormality kinematically, with composite scores condensing multiple joint axes data into a single score.Research Question: Are 3DMA-derived composite scores valid (known-groups and convergent validity), reliable and able to quantify speed-related changes in abnormal upper limb movement during walking?Methods: This observational study compared 42 adults with ABI and abnormal upper limb movements during walking with 36 healthy controls (HC) at a matched walking speed intention. Participants underwent 3DMA assessment of self-selected and fast walking speeds. Composite scores quantified the affected upper limb's kinematic abnormality. The Arm Posture Score arithmetic mean version (APSam) and 1.96 standard deviation reference-range scaled versions; the Kinematic Deviation Score mean (KDSm) and worst score (KDSw) were evaluated for association with each other and subjective abnormality rating (Pearson's 'r' correlation), test-retest reliability (intra-class correlation coefficient (ICC)), and ability to quantify speed-related changes in abnormal upper limb movement (Cohen's d effect size (ES), % change scores).Results: Very strong correlations existed between composite scores. The KDSm under-classified upper limb abnormality, whereas the KDSw captured the majority of ABI participants. All scores had moderate-strong correlations with subjective rating of abnormal upper limb movements (r = 0.54 - 0.79) and very strong test-retest reliability (ICCs > 0.81). The APSam demonstrated a 16% (ES = 0.76) walking speed-related increase in upper limb abnormality, whilst decreases were demonstrated in the KDSm 26% (ES 0.90) and KDSw 35% (ES 0.96).Significance: The APSam, KDSw, and number of abnormal joint axes comprehensively assess the whole upper limb abnormal movements, accurately classifies abnormality, and quantifies severity. This study illustrated notable presence of abnormal upper limb movements at self-selected walking speed and small increase at fast speeds. Background: Clinicians and footwear manufacturers often advise young children to wear soft-soled footwear when they are first learning to walk. There is limited evidence as to why this advice is given, and if soft-soled shoes are as close to barefoot as thought.Research Question: What are the differences in spatiotemporal measures of gait during walking and running in three common types of children's footwear with a soft-soled compared to barefoot in young children?Methods: The study used a quasi-experimental design, with the condition order randomised using a Latin square sequence. Forty-seven children were recruited (2 - 4 years). Participants walked or ran the length of a GAITrite mat in a randomized order for barefoot and soft-soled sneaker, boot and sandal conditions. Linear regression analyses were used to investigate the main effect of each soft-soled footwear compared to bare feet in the different gait parameters.Results: For walking and running trials, cadence decreased whereas step time and stride length increased in all footwear types compared to the barefoot condition. While wearing sneakers and sandals increased the stance percentage for walking and running trials, compared to barefoot, this difference was only apparent during the running trial for the boots. Likewise, although double support time increased for both the boots and sneakers in walking and running, compared to barefoot, this difference was only observed in the sandals during walking.Significance: This research found that various types of soft-soled footwear impacted gait compared to the barefoot condition, with some differences seen between walking and running trials. These findings challenge the assumption that soft-soled footwear facilitate a similar gait to barefoot walking and running, although the clinical significance of these differences is unknown. Background: Foot orthoses (FOs) have been widely prescribed to alter various lower limb disorders. FOs' geometrical design and material properties have been shown to influence their impact on foot biomechanics. New technologies such as 3D printing provide the potential to produce custom shapes and add functionalities to FOs by adding extra-components.Research Question: The purpose of this study was to determine the effect of 3D printed FOs stiffness and newly design postings on foot kinematics and plantar pressures in healthy people.Methods: Two pairs of ¾ length prefabricated 3D printed FOs were administered to 15 healthy participants with normal foot posture. FOs were of different stiffness and were designed so that extra-components, innovative flat postings, could be inserted at the rearfoot. In-shoe multi-segment foot kinematics as well as plantar pressures were recorded while participants walked on a treadmill. One-way ANOVAs using statistical non-parametric mapping were performed to estimate the effect of FOs stiffness and then the addition of postings during the stance phase of walking.Results: Increasing FOs stiffness altered frontal and transverse plane foot kinematics, especially by further reducing rearfoot eversion and increasing the rearfoot abduction. Postings had notable effect on rearfoot frontal plane kinematics, by enhancing FOs effect. Looking at plantar pressures, wearing FOs was associated with a shift of the loads from the rearfoot to the midfoot region. Higher peak pressures under the rearfoot and midfoot (up to +31.7 %) were also observed when increasing the stiffness of the FOs.Significance: 3D printing techniques offer a wide range of possibilities in terms of material properties and design, providing clinicians the opportunity to administer FOs that could be modulated according to pathologies as well as during the treatment by adding extra-components. Background: High-heeled shoes are currently preferred by women due to contemporary aesthetics. However, high-heeled shoes may increase the effort required to ascend stairs and, hence, alter biomechanical performance.Research Question: How do high-heel shoe parameters affect the pelvis position, lower extremities kinematics, and ground reaction force in young women during stair ascent motion?Methods: Stair ascent experiments were performed with 20 healthy adult women. The participants were instructed to ascend a 3-step staircase, wearing heeled shoes of different heel heights and heel types and one pair of flat shoes as the control group. Changes in lower body biomechanics were analyzed with kinematics and ground reaction force variables collected from the dominant limb. A two-way repeated ANOVA was performed to determine which variables were affected by heel type and which were affected by heel height or a combination of both.Results: As the heel height increased, an increased range of ankle dorsiflexion-plantarflexion, as well as pelvic rotation, was observed(P = 0.039 and P = 0.003, respectively). A thinner heel type displayed a larger pelvic forward tilt movement(P = 0.026)and 1st peak vertical force(P = 0.025), as well as a smaller 2nd peak vertical force (P = 0.002). With high heels, increased external rotation of the knee, inversion and plantar flexion, and flexion values of the knee were observed. We also observed decreased external rotation of the pelvis, ankle eversion, varum, and dorsiflexion.Significance: To stabilize body posture during stair ascent motion with high-heeled shoes, compensatory response including increasd pelvic range of motion and changing the joint angles of the lower extremities. Background: Low back, pelvic, and lower extremity pain are common during and after pregnancy. Understanding differences in mechanics between pregnant and non-pregnant females is a first step toward identifying potential pathological mechanisms. The primary purpose of this study was to compare joint kinetics and muscle activation during gait between females during and after pregnancy to nulliparous females.Methods: Twenty pregnant females completed testing on three occasions (second trimester, third trimester, and post-partum), while 20 matched, nulliparous controls were tested once. Motion capture, force data, and surface electromyography were averaged across seven trials during gait. Lower extremity kinematics, lower extremity moments and work normalized to pre-pregnancy body mass, work distribution, and peak and average muscle activation amplitude were calculated. Independent t-tests were conducted between pregnant and nulliparous females at each time point.Results: Compared to controls, peak hip abductor moments were greater throughout and after pregnancy. Females in second trimester also demonstrated greater sagittal negative ankle work and greater percent contribution of the ankle and smaller percent contribution of the hip to negative work. Compared to controls, during third trimester there were greater knee abductor, ankle plantarflexor, and ankle dorsiflexor moments and greater work at the ankle and total work. Several moment and work variables continued to be elevated post-partum compared to controls. Gluteus maximus muscle activation amplitude was smaller in second trimester and post-partum compared to controls.Significance: While overall joint demands were greater during and after pregnancy, there was a smaller relative sagittal utilization of the hip early in pregnancy and smaller gluteus maximus muscle amplitude during second trimester and post-partum. Because the gluteus maximus muscle contributes to force closure and dynamic stability of the low back and pelvis, relative gluteus maximus disuse, concurrent with increased joint loads, could potentially contribute to pain during and after pregnancy. Background: Mobility aids are commonly prescribed to offload an injured lower extremity. Device selection may impact stance foot loading patterns and foot health in clinical populations at risk of foot ulceration.Research Questions: Two questions motivated this study: How does device selection influence peak plantar and regional (rearfoot, mid foot and forefoot) foot forces on the stance foot? Does device selection influence peak, cumulative, and regional plantar forces within a 200 m walking trial?Methods: Twenty-one older adults walked 200 m at self-selected pace in four randomized conditions for this prospective crossover study. Participants used a walker, crutches, wheeled knee walker (WKW), and no assistive device (control condition). Plantar forces were measured using a wireless in-shoe system (Loadsol, Novel Inc., St. Paul, MN). Repeated measures analyses of variance were used to determine differences in peak and cumulative total and regional forces among walking conditions. Paired sample t-tests compared forces during first and last 30 s epochs of each condition to determine device influence over time.Results: The WKW reduced peak net forces by 0.29 and 0.35 bodyweight (BW) when compared to the walker or control condition with similar trends in all foot regions. Crutch use had similar peak forces as control. There were no differences in the number of steps taken within devices comparing first and last epochs. Crutches had a 0.04 and 0.07 BW increase in peak net and forefoot forces during the last epoch. Walker use had 66.44 BW lower cumulative forefoot forces in the last epoch.Significance: Crutches had similar stance foot loading as normal walking while a walker lowered forefoot forces at the expense of increased steps. Background: Even in mild unilateral cerebral palsy increased internal hip rotation can be noted on physical- and gait examination. The influence of spasticity on femoral growth in the transverse plane is not clear. These deviations and asymmetry in movement pattern may negatively affect efficiency of gait and cause psychological concerns about appearance.Research Question: Is increased internal hip rotation on the involved side in mild unilateral CP common and is there compensatory external pelvic rotation to keep foot progression symmetrical?Methods: This prospective study included 45 individuals with unilateral cerebral palsy, mean age 17.7 (13.0-24.0) years. All were Gross Motor Function Classification Level I. Physical examination, three-dimensional gait analysis and magnetic resonance imaging for assessment of rotational alignment was performed.Results: On physical examination internal hip rotation was mean 50.6 (SD 10.4) degrees on the involved side and 44.3 (SD 10.3) on the non-involved side, p = 0.001. In gait analysis calculating the whole gait cycle, internal hip rotation was mean 2.3 (6.2) degrees on the involved side, and on the non-involved side external 1.8 (7.6) degrees, p = 0.004. Increased external pelvic rotation was noted on the involved side, mean 2.0 (4.3) degrees with corresponding internal rotation on the non-involved side, mean 3.6 (4.4), p = 0.001. There was no difference in foot progression, p = 0.067, with mean 5.1 (8.6) and 3.9 (6.4) external respectively. Magnetic resonance imaging revealed femoral torsion on the involved side mean 17.3 (11.3) degrees compared to 11.4 (10.8) on the non-involved side, p = 0.001.Significance: Transverse plane asymmetry in the femur was noted in mild unilateral cerebral palsy. Increased anatomical and dynamic internal rotation was compensated for by external pelvic rotation. Background: Children with unilateral spastic cerebral palsy (USCP) exhibit impaired bimanual coordination, gait control, and whole body movement control. Intensive upper extremity training has been found to be effective for improving upper extremity function. However, the effectiveness of the intensive upper extremity training on whole body movement control is not known.Research Question: The present study aimed to evaluate the effects of Constraint Induced Movement Therapy (CIMT) and Hand Arm Bimanual Intensive Therapy (HABIT) on bimanual coordination and gait control during a complex whole body task.Methods: Sixteen children with congenital hemiplegia (age 6-12 years; GMFCS: I-II, MACS: I-II) were randomly assigned to either CIMT or HABIT for 6 h per day training for 15 days. Children were asked to perform two whole body tasks (walking with and without a tray carrying) while 3-D kinematic analysis was performed before and after training.Results: After training, the HABIT group increased the symmetry in height of their hands during tray carrying (more leveled tray). Both CIMT and HABIT groups decreased the lateral motion of the tray. The CIMT group increased speed and stride length after training in both the walking and tray carrying tasks. Both groups also increased their minimum toe clearance (all p < 0.05).Significance: Two types of intensive upper extremity training have provided significant improvements to whole body movement control for children with USCP. Adhering to the specificity of practice concept, HABIT improved bimanual coordination after training during the whole body tray carrying tasks. Background: Minimum Toe Clearance (MTC) is defined as the minimum vertical distance between the lowest point under the front part of the foot and the ground, during mid-swing. Low values of MTC and walking on inclines are both related to higher probability of tripping and falling in lower limb amputees. New prosthetic designs aim at improving MTC, especially on ramps, however the real effect on MTC still needs investigation. The objective of this study was then to evaluate the effect of different prosthetic designs on MTC in inclined walking.Methods: Thirteen transtibial amputees walked on a ramp using three different prostheses: non articulating ankle (NAA), articulating hydraulic ankle (AHA), and articulating hydraulic ankle with microprocessor (AHA-MP). Median MTC, coefficient of variation (CV), and tripping probability (TP) for obstacles of 10 and 15 mm were compared across ankle type in ascent and descent.Findings: When using AHA-MP, larger MTC median values for ascending (P ≤ 0.001, W = 0.58) and descending the ramp (P = 0.003, W = 0.47) were found in the prosthetic limb. Also significantly lower CV was found on the prosthetic limb for both types of AHA feet when compared to NAA for descending the ramp (P = 0.014, W = 0.45). AHA-MP showed the lowest TP for the prosthetic leg in three conditions evaluated. On the sound limb results showed the median MTC was significantly larger (P = 0.009, W = 0.43) and CV significantly lower (P = 0.005, W = 0.41) when using an AHA in ascent.Interpretation: Both AHA prosthetic designs help reduce the risk of tripping of the prosthetic limb by increasing the median MTC, lowering its variability and reducing TP for both legs when ascending and descending the ramp. For most of the conditions, AHA-MP showed the lowest TP values. Findings suggest that AHA prostheses, especially AHA-MP could reduce the risk of tripping on ramps in amputees. [ABSTRACT FROM AUTHOR]

Background: The popularity of inertial sensors in gait analysis is steadily rising. To date, an application of a wearable inertial sensor system for assessing gait in hip osteoarthritis (OA) has not been reported.Research Question: Can the known kinematic differences between patients with hip OA and asymptomatic control subjects be measured using the inertial sensor system RehaGait®?Methods: The patients group consisted of 22 patients with unilateral hip OA scheduled for total hip replacement. Forty-five age matched healthy control subjects served as control group. All subjects walked for a distance of 20 m at their self-selected speed. Spatiotemporal parameters and sagittal kinematics at the hip, knee, and ankle including range of motion (ROM) were measured using the RehaGait® system.Results: Patients with hip OA walked at a slower walking speed (-0.18 m/s, P < 0.001) and with shorter stride length (-0.16 m, P < 0.001), smaller hip ROM during stance (-11.6°, P < 0.001) and swing (-11.3°, P < 0.001) and smaller knee ROM during terminal stance and swing (-9.0° and-11.5°, P < 0.001). Patients had a smaller hip ROM during stance and swing and smaller knee ROM during terminal stance and swing in the affected compared to the unaffected side (P < 0.001).Significance: The differences in spatiotemporal and kinematic gait parameters between patients with hip OA and age matched control subjects assessed using the inertial sensor system agree with those documented for camera-based systems.
