Affordable access to healthcare including medicines is a key social policy goal in Europe. However, it has rarely been addressed in comparative social policy research. Although the concept of decommodification has already been used in the context of healthcare and sickness benefits, we argue that the scope of such studies should be expanded to medicines to understand how welfare states protect their citizens from market forces in case of illness. We examine and compare the relationship between income, other characteristics, and subjective financial burden of medicines (FBM) across five countries with universal health systems pursuing egalitarian aims (Denmark, Finland, the Netherlands, Norway and Sweden). Analyses using 2017 EU-SILC microdata and linear probability models showed large differences in the level of FBM across countries, with the highest income quintile in Finland reporting FBM more frequently than the lowest income quintile in Denmark. Finland differed from the rest by increasing probability of FBM with age. In other countries, middle-aged adults tended to be the most affected, and older adults were well-protected. The association between income and FBM was strongest in the Netherlands; however, the higher probability of FBM was skewed towards the lower quintiles in all countries. FBM and financial burden of medical care were strongly associated although FBM tended to be more common. Unmet needs for medical examination were rare and lacked sensitivity in capturing manifestations of market risk. Decommodification literature has focused healthcare services as proxy of access; nevertheless, our study shows that further functions, and broader outcomes should be examined to capture market risk. Our evidence further highlights that important differences can be found even in countries with relatively similar health policy aims. Objectives: To inform allocation decisions in any healthcare system, robust cost data are indispensable. Nevertheless, recommendations on the most appropriate valuation approaches vary or are nonexistent, and no internationally accepted gold standard exists. This costing analysis exercise aims to assess the impact and implications of different calculation methods and sources based on the unit cost of general practitioner (GP) consultations in Austria.Methods: Six costing methods for unit cost calculation were explored, following 3 Austrian methodological approaches (AT-1, AT-2, AT-3) and 3 approaches applied in 3 other European countries (Germany, The Netherlands, United Kingdom). Drawing on Austrian data, mean unit costs per GP consultation were calculated in euros for 2015.Results: Mean unit costs ranged from €15.6 to €42.6 based on the German top-down costing approach (DE) and the Austrian Physicians' Chamber's price recommendations (AT-3), respectively. The mean unit cost was estimated at €18.9 based on Austrian economic evaluations (AT-1) and €17.9 based on health insurance payment tariffs (AT-2). The Dutch top-down (NL) and the UK bottom-up approaches (UK) yielded higher estimates (NL: €25.3, UK: €29.8). Overall variation reached 173%.Conclusions: Our study is the first to systematically investigate the impact of differing calculation methods on unit cost estimates. It shows large variations with potential impact on the conclusions in an economic evaluation. Although different methodological choices may be justified by the adopted study perspective, different costing approaches introduce variation in cross-study/cross-country cost estimates, leading to decreased confidence in data quality in economic evaluations. Objective: This study examines European decision makers' consideration and use of quantitative preference data.Methods: The study reviewed quantitative preference data usage in 31 European countries to support marketing authorization, reimbursement, or pricing decisions. Use was defined as: agency guidance on preference data use, sponsor submission of preference data, or decision-maker collection of preference data. The data could be collected from any stakeholder using any method that generated quantitative estimates of preferences. Data were collected through: (1) documentary evidence identified through a literature and regulatory websites review, and via key opinion leader outreach; and (2) a survey of staff working for agencies that support or make healthcare technology decisions.Results: Preference data utilization was identified in 22 countries and at a European level. The most prevalent use (19 countries) was citizen preferences, collected using time-trade off or standard gamble methods to inform health state utility estimation. Preference data was also used to: (1) value other impact on patients, (2) incorporate non-health factors into reimbursement decisions, and (3) estimate opportunity cost. Pilot projects were identified (6 countries and at a European level), with a focus on multi-criteria decision analysis methods and choice-based methods to elicit patient preferences.Conclusion: While quantitative preference data support reimbursement and pricing decisions in most European countries, there was no utilization evidence in European-level marketing authorization decisions. While there are commonalities, a diversity of usage was identified between jurisdictions. Pilots suggest the potential for greater use of preference data, and for alignment between decision makers. • Homemade and untaxed alcoholic beverages are called unrecorded alcohol. • Survey data from 33 European countries were analysed. • 12% of adults drinking alcohol in the past week reported unrecorded alcohol use. • Higher prevalence (24%) was reported among high-risk alcohol users. • Unrecorded alcohol accounted for 7% of per capita consumption in 2020. Using data from 33 European countries (including all EU member states), this study aimed to 1) estimate the prevalence of unrecorded alcohol use among past-week alcohol users, 2) describe how unrecorded alcohol use is associated with drinking patterns, and 3) estimate the contribution of unrecorded alcohol to the total amount of alcohol consumed annually in these countries. Data from 25,728 adults who drank alcohol in the past week and self-reported their use of unrecorded alcohol in 2021 were analysed. Prevalence of unrecorded alcohol use in the last week was estimated for those with low, medium, and high risk drinking categorised using the WHO-recommended risk thresholds and definition of risky single occasion drinking. Prevalence estimates were weighted for the country-specific gender, age, and geographical population distribution. An adjusted weighted proportion of unrecorded drinking occasions in total drinking occasions was calculated and compared to 2020 recorded annual per capita consumption estimates. Among past-week alcohol users, the average prevalence of past-week unrecorded alcohol use was 12.1% (95% CI: 11.7-12.5%), with considerable difference between countries (min: 2.0% in Malta; max: 27.0% in Greece). Unrecorded alcohol use was much more prevalent among people with high-risk alcohol use in the past week (24.2%, 95% confidence interval [CI]: 22.9-25.5%) compared to people with low- (6.5, 95% CI: 6.0-6.9%) or medium-risk alcohol intake in the past week (13.6%, 95% CI: 12.9-14.3%). Unrecorded alcohol accounted for 7% of per capita consumption in 2020. This is the first comparable assessment of unrecorded alcohol use across EU and affiliated countries. The findings add support to the observation that availability of unrecorded alcohol may contribute to risky drinking in Europe. The observed country variations may be related to differences in country-specific pricing policies and measures to reduce the production and consumption of unrecorded alcoholic beverages. This paper proposes an economic analysis of three different types of processing in CDW (construction and demolition waste) recycling platforms, according to the sophistication of the processing technologies (current advanced, advanced and advanced sorting). The methodology that is adopted is in the economic evaluation concept of projects and is classified with a scoping study phase. In these contexts, three levels of CDW processing capabilities for recycling platforms are analyzed (100, 300 and 600 thousand tons per year). This article considers databases obtained from similar projects that have been published in the specialized literature; the data sources are primarily from the European continent. The paper shows that current advanced process has better economic performance, in terms of IRR, related to the other two processes. The IRR associated with advanced and advanced sorting processes could be raised by, (i) higher price of secondary primary material, and/or (ii) higher capacity of platforms, and/or (iii) higher sharing of secondary primary material in the total production. The first two points depend on the market conditions (prices and total quantity of CDW available) and (potential) fiscal or incentive policies. The last one depends on technological progress. Background: The adverse relation between dietary trans fatty acid (TFA) intake and coronary artery disease risk is well established. Many countries in the European Union (EU) and worldwide have implemented different policies to reduce the TFA intake of their populations. Objective: The aim of this study was to assess the added value of EU-level action by estimating the cost-effectiveness of 3 possible EU-level policy measures to reduce population dietary TFA intake. This was calculated against a reference situation of not implementing any EU-level policy (i.e., by assuming only national or selfregulatory measures). Design: We developed a mathematical model to compare different policy options at the EU level: 1) to do nothing beyond the current state (reference situation), 2) to impose mandatory TFA labeling of prepackaged foods, 3) to seek voluntary agreements toward further reducing industrially produced TFA (iTFA) content in foods, and 4) to impose a legislative limit for iTFA content in foods. Results: The model indicated that to impose an EU-level legal limit or to make voluntary agreements may, over the course of a lifetime (85 y), avoid the loss of 3.73 and 2.19 million disability-adjusted life-years (DALYs), respectively, and save >51 and 23 billion euros when compared with the reference situation. Implementing mandatory TFA labeling can also avoid the loss of 0.98 million DALYs, but this option incurs more costs than it saves compared with the reference option. Conclusions: The model indicates that there is added value of an EU-level action, either via a legal limit or through voluntary agreements, with the legal limit option producing the highest additional health benefits. Air quality and related health effects are not only affected by policies directly addressed at air pollution but also by other environmental strategies such as climate mitigation. This study addresses how different climate policy pathways indirectly bear upon air pollution in terms of improved human health in Europe. To this end, we put in perspective mitigation costs and monetised health benefits of reducing PM 2.5 (particles less than 2.5 μm in diameter) and ozone concentrations. Air quality in Europe and related health impacts were assessed using a comprehensive modelling chain, based on global and regional climate and chemistry-transport models together with a health impact assessment tool. This allows capturing both the impact of climate policy on emissions of air pollutants and the geophysical impact of climate change on air quality. Results are presented for projections at the 2050 horizon, for a set of consistent air pollution and climate policy scenarios, combined with population data from the UN's World Population Prospects, and are expressed in terms of morbidity and mortality impacts of PM 2.5 and ozone pollution and their monetised damage equivalent. The analysis shows that enforcement of current European air quality policies would effectively reduce health impacts from PM 2.5 in Europe even in the absence of climate policies (life years lost from the exposure to PM 2.5 decrease by 78% between 2005 and 2050 in the reference scenario), while impacts for ozone depend on the ambition level of international climate policies. A move towards stringent climate policies on a global scale, in addition to limiting global warming, creates co-benefits in terms of reduced health impacts (68% decrease in life years lost from the exposure to PM 2.5 and 85% decrease in premature deaths from ozone in 2050 in the mitigation scenario relative to the reference scenario) and air pollution cost savings (77%) in Europe. Against the background of the recent trend towards ever larger wind turbines at higher hub heights, this contribution determines the total technical potential and associated costs for electricity generation in Europe with large turbines, based on a GIS-based methodology employing cost-potential curves. For the EU28 and Europe technical potentials of 15 PWh/a and 20 PWh/a are determined respectively, with associated LCOEs (Levelized Costs of Electricity) of between 6 and 50 €ct/kWh and large variations between countries: the largest potentials and lowest generation costs are to be found in the UK, Poland and Sweden. The approximate required investment to meet national 2020 targets based on the National Renewable Energy Action Plans is estimated based on the model results. A comparison with the results of other studies shows significant deviations in the results, most of which can be explained through the differences in input parameters, and a comparison of the obtained results for Germany with those from a previous study under the same assumptions produced very similar results with a deviation of about 10%. Based on a data-intensive weather-driven modelling approach, technically and economically optimal designs are derived for a simplified, highly renewable pan-European electricity system, which minimise the need for backup energy, backup capacity, transmission capacity and the levelised system cost of delivered electricity. The overall cost-optimal design, based on standard cost assumptions, relies on synchronised backup across the transmission grid and comes with a renewable penetration of 50% with a rather high wind fraction of 94%. Given the current European electricity consumption, this corresponds to 600 GW rated wind power capacities, 60 GW installed solar power capacities, 320 GW conventional backup power capacity, and about five times today's installed transmission capacities. A sensitivity analysis reveals that the design and cost of the optimal system depend mostly on the assumed cost of wind capacity and fuel for backup energy. Lower costs for wind capacity, higher costs for backup energy and usage of otherwise curtailed excess electricity generation lead to a strong increase of the optimal renewable penetration. The sensitivity analysis is also used to find that a CO 2 tax of over 100 €/ton would be needed for the economic viability of carbon capture and sequestration. [ABSTRACT FROM AUTHOR]

This review aims to explore the needs and opportunities of research & development in the field of biowaste biorefinery in Europe. Modern industry in recent years is giving its close attention on organic waste as a new precious bioresource. Specific biowaste valorisation pathways are focusing on food processing waste, being food sector the first manufacture in Europe. Anyway they need to be further tested and validated and then transferred at the larger scale. In particular, they also need to become integrated, combining biomass pretreatments and recovery of biogenic chemicals with bioconversion processes in order to obtain a large class of chemicals. This will help to (a) use the whole biowaste, by avoiding producing residues and providing to the approach the required environmental sustainability, and (b) producing different biobased products that enter different markets, to get the possible economical sustainability of the whole biorefinery. However, the costs of the developed integrated processes might be high, mostly for the fact that the industry dealing with such issues is still underdeveloped and therefore dominated by high processing costs. Such costs can be significantly reduced by intensifying research & development on process integration and intensification. The low or no cost of starting material along with the environmental benefits coming from the concomitant biowaste disposal would offset the high capital costs for initiating such a biorefinery. Over the last years the European Union (EU) has implemented ambitious climate and energy targets for 2020, i.e. the 2020 Climate and Energy Package (Package), to become a highly efficient, low carbon economy. The Package assigns binding renewable energy sources (RES) target for each EU member countries. Indeed costs to reach Package's targets vary among EU countries, given heterogeneity among regions and different instruments implemented to spur RES deployment. The aim of our paper is to provide a policy tool for EU policy makers in the negotiation process that assigns achievable quantitative targets according to which countries are not inclined to deviate from the agreement. We compute potential extra costs that may arise when some EU countries fail to reach the assigned objective within the Package with reference to RES target. More generally, our simulation model is an instrument that allows EU policy makers to assess incentives that countries might have to participate or not to the environmental agreements. Our analysis shows that partial participation to the Package produces inefficiencies by rising costs for other participating countries to the environmental agreement. Finally, we propose a new policy mechanism based on progressive sanctions for countries that do not meet their national targets, which are proportional to the monetary value of the penalty they impose to the rest of the EU countries with their opportunistic behavior. Abstract: As with other European countries, the French automotive industry needs to improve the end-of-life vehicle (ELV) recycling rate in order to reach the minimum reuse and recovery rate required by EU directives. This paper proposes a model to investigate the potential cost and benefit of ELV glazing recycling for all value-chain stakeholders, and for the network as a whole. Key parameters of future changes, namely glass cullet price, landfill cost, network coverage and transportation costs are identified at the level of field research. A system dynamics approach is then used to simulate the net economic balance of the recycling network under different future scenarios. Abstract: The presence of price clustering in markets is taken as a sign of market inefficiency that can influence trading strategies. In this paper, we study the presence of a concentration in prices in carbon futures markets. Specifically, we analyze the European Carbon Futures Markets and test for evidence of preference for certain prices above others. Our results reveal the strong presence of price clustering in the carbon market at prices ending in digits 0 and 5. These findings support the attraction hypothesis, which endorses a significant clustering on gravitational prices, but also backs the negotiation hypothesis, which advocates greater clustering when trading costs are higher. Abstract: Objectives: The objective of the present study was to measure and compare the direct costs of intensive care unit (ICU) days at seven ICU departments in Germany, Italy, the Netherlands, and the United Kingdom by means of a standardized costing methodology. Methods: A retrospective cost analysis of ICU patients was performed from the hospital''s perspective. The standardized costing methodology was developed on the basis of the availability of data at the seven ICU departments. It entailed the application of the bottom–up approach for “hotel and nutrition” and the top–down approach for “diagnostics,” “consumables,” and “labor.” Results: Direct costs per ICU day ranged from €1168 to €2025. Even though the distribution of costs varied by cost component, labor was the most important cost driver at all departments. The costs for “labor” amounted to €1629 at department G but were fairly similar at the other departments (€711 ± 115). Conclusions: Direct costs of ICU days vary widely between the seven departments. Our standardized costing methodology could serve as a valuable instrument to compare actual cost differences, such as those resulting from differences in patient case-mix. [Copyright &y& Elsevier]

The article presents a study on the costs of different methods for the detection of coronary artery stenoses in patients with possible coronary artery disease (CAD). It used a group of patients who underwent cardiovascular magnetic resonance (CMR). It performed methods such as cost analysis and randomized controlled trials (RCTs). Results show cost savings of 50%, 25% and 23% in the public sectors of the health care systems in Germany, Great Britain and Switzerland, respectively.

Abstract: We investigated the costs of Parkinson''s Disease (PD) in 486 patients based on a survey conducted in six countries. Economic data were collected over a 6-month period and presented from the societal perspective. The total mean costs per patient ranged from EUR 2620 to EUR 9820. Direct costs totalled about 60% to 70% and indirect costs about 30% to 40% of total costs. The proportions of costs components of PD vary notably; variations were due to differences in country-specific health system characteristics, macro economic conditions, as well as frequencies of resource use and price differences. Abstract: Noise is becoming one of most important pollutants, especially in towns. Indeed, the European Community recognizes the environmental noise as one of the most important environmental problems in Europe. Therefore, there are real demands, increasing in the near future, of acoustic absorbent materials. Taking into account the conditions that these materials must present (durability, costs, noise absorption power, etc.), a new material based on petroleum coke agglomerated with cement has been developed, targeted as base for designing and construction of noise screens. Our results showed that the petroleum coke–cement system had suitable acoustic properties. Therefore, the usage of petroleum coke as acoustic absorbent material opens a new alternative, with important advantages from social and environmental viewpoints. Abstract: Background: Although there is a wealth of guidance concerning the management of patients with ventilator-associated pneumonia (VAP), compliance with recommendations concerning optimal treatment practices is highly variable. Methods: This document presents a comprehensive care bundle package addressing all aspects of VAP diagnosis and treatment in an attempt to promote guideline-compliant practices. Uniquely, the development of these care bundles used a formalized method to assess the supporting data, based on multicriteria decision analysis. Results: This system allowed the numerous VAP management parameters identified from recent European guidelines to be ranked according to defined criteria. The resulting VAP care bundles are (a) diagnosis: early chest x-rays within 1 hour, immediate reporting of respiratory secretions Gram staining, and (b) therapy: immediate treatment, empiric therapy based on local pathogens and risk factors, de-escalation, assessment of response within 72 hours, and short therapy duration if feasible. Conclusion: Adoption of these care bundles should rationalize VAP management practices and facilitate the development of consistent and guideline-compliant care processes. [Copyright &y& Elsevier]

The objective of this paper is to analyze the role of technology learning in European wind power generation in the presence of public R&D. A Cobb–Douglas cost function is employed to derive a learning curve model for wind power, thus illustrating how the investment costs for this technology are influenced by global learning-by-doing, scale effects, and a European R&D-based knowledge stock. We assume that public R&D expenses targeting wind power add to the above stock, and these R&D outlays are in turn hypothesized to be influenced by technology cost levels, the opportunity cost of public R&D as well as by government budget constraints. We estimate the learning and the R&D model, respectively, using a panel data set covering five European countries over the time period 1986–2002. The empirical results confirm the importance of both learning-by-doing and public R&D support in the cost reduction process, and governments' R&D expenses have declined in response to lowered investment costs. This is efficient in the sense that public funds are best targeted at technologies which are far from being commercial. The results also illustrate that governments in Europe have been sensitive to the opportunity cost of public R&D in the energy R&D budget process. [Copyright &y& Elsevier]

Abstract: The objective of the study is to analyse the conditions for connection of residential buildings in heat sparse areas to district heating systems in order to increase electricity production in municipal combined heat and power plants. The European electricity market has been assumed to be fully deregulated. The relation between connection of heat sparse areas, increased electricity and heat production as well as electricity prices, fuel prices and emissions rights is investigated. The results of the study show that there is potential to expand the district heating market to areas with lower heat concentrations in the cities of Gävle, Sandviken and Borlänge in Sweden, with both economic and environmental benefits. The expansion provides a substantial heat demand of approximately 181GWh/year, which results in an electricity power production of approximately 43GWh/year. Since the detached and stand-alone houses in the studied heat sparse areas have been heated either by oil boiler or by direct electricity, connection to district heating also provides a substantial reduction in emissions of CO2. The largest reductions in CO2 emissions are found to be 211ktonnes/year assuming coal-fired condensing power as marginal electricity production. Connection of heat sparse areas to district heating decrease the system costs and provide a profitability by approximately 22millionEURO/year for the studied municipalities if the price of electricity is at a European level, i.e. 110EURO/MWh. Sensitivity analysis shows, among other things, that a strong relation exists between the price of electricity and the profitability of connecting heat sparse areas to district heating systems. • Road-freight TCO compared for 5 drive-technologies in 3 applications, 10 countries. • A database of costs for road-freight TCO parameters is newly compiled. • Low-carbon vehicles largely competitive in light- and medium-duty segments. • Low-carbon vehicles competitive in heavy-duty segments in selected countries. • Findings indicate three important TCO parameters to drive this competitiveness. In light of the Paris Agreement, road-freight represents a critically difficult-to-abate sector. In order to meet the ambitious European transport sector emissions reduction targets, a rapid transition to zero-carbon road-freight is necessary. However, limited policy assessments indicate where and how to appropriately intervene in this sector. To support policy-makers in accelerating the zero-carbon road-freight transition, this paper examines the relative cost competitiveness between commercial vehicles of varying alternative drive-technologies through a total cost of ownership (TCO) assessment. We identify key parameters that, when targeted, enable the uptake of these more sustainable niche technologies. The assessment is based on a newly compiled database of cost parameters which were triangulated through expert interviews. The results show that cost competitiveness for low- or zero-emission niche technologies in certain application segments and European countries is exhibited already today. In particular, we find battery electric vehicles to show great promise in the light- and medium-duty segments, but also in the heavy-duty long-haul segments in countries that have enacted targeted policy measures. Three TCO parameters drive this competitiveness: tolls, fuel costs, and CAPEX subsidies. Based on our analysis, we propose that policy-makers target OPEX before CAPEX parameters as well utilize a mix of policy interventions to ensure greater reach, increased efficiency, and increased policy flexibility. Drug development performance is examined using data on clinical research projects of 10 pharmaceutical companies. In contrast to previous work on the discovery phase of pharmaceutical R&D we find a strong correlation between the diversity of firms' development efforts and the success probability of individual projects, but no effect of scale per se. Large firms' superior performance in drug development appears to be driven by returns to scope rather than returns to scale. Global energy demand is constantly increasing and recent projections show that this trend is going to continue with an average increase of 1.2% up to 2040. Since the building sector has emerged as a large energy consumer, in an effort to combat this trend, governments worldwide introduced various policies. The utilization of solar energy conversion systems are at the forefront of attention as they are considered carbon dioxide neutral and can be used in order to cover both electricity and heating load demands. Thus, these systems need to be thoroughly investigated taking into consideration the variations in the solar potential and the differences that exist in the electricity mix throughout Europe both in terms of cost, as well as of emissions, in order to assess their sustainability. In the current work a complete life cycle impact assessment is conducted for typical 4 kWp photovoltaic systems throughout Europe, and their environmental impact, sustainability, energy return on energy invested and payback period are calculated. The results highlight that although residential photovoltaic systems are considered "clean" and have a relatively low environmental impact throughout their life cycle, depending on the installation location and local electricity mix this might not be the case in the coming years. The energy return on energy invested ranges from 1.64 to almost 5 depending on location, while their simple payback period is less than 11 years in most cases, and as low as 5, without any subsidy. • Relation between the deterioration of the infrastructure in the wastewater treatment plants and the repairs and corrective maintenance cost. • The findings revealed that the repairs and corrective maintenance cost has increased in the last years for most of the facilities. • A dynamic cost model has been developed to predict the repairs and corrective maintenance cost along the time. • The model could become a useful tool to support managers in the decision making process. • This cost function could be helpful to assess the feasibility of different investment projects, such as, repairs or equipment replacement. The implementation of the Directive 91/271 / CEE resulted in the proliferation of new Wastewater Treatment Plants (WWTPs) all around Europe. These facilities, which are compound by a wide range of assets (civil works, electromechanical equipment, pipelines, etc.), are ageing and deteriorating having an impact on the efficiency of the process. It is founded that repairs and corrective maintenance costs have increased in the last years, due to the deterioration process. Therefore, the aim of the present study is to focus on the cost effects of the deterioration process of the facilities in different wastewater treatment technologies. The analysis of these impacts will allow the operators to make decisions on the convenience of applying a preventive maintenance program as well as the selection of the best time for the replacement of the equipment. These results will contribute to the technical and economic optimization of the wastewater treatment process, reducing, also its environmental effects. In order to achieve this purpose a model of the repairs and corrective maintenance cost will be developed for a group of WWTPs with extended aeration and activated sludge technologies, offering valuable results for the managers of the WWTPs in the decision making process.
