2. Classroom Promotion of Oral Language: Outcomes from a Randomized Controlled Trial of a Whole-of-Classroom Intervention to Improve Children's Reading Achievement

- Abstract
Children need rich language learning experiences in school to build language and reading skills. Research suggests that various effective ways to support teacher provision of these experiences. The Classroom Promotion of Oral Language cluster randomized controlled trial (n = 1,360 students; 687 intervention, 673 control) examined whether a teacher professional learning intervention targeting oral language in the first years of school led to improved student outcomes compared to usual teaching practices. The intervention comprised face-to-face professional learning and ongoing support. The primary outcome was student reading ability at Grade 3; secondary outcomes included oral language, reading, and mental health at Grades 1 and 3. No differences were detected between the intervention and control arms. Implications of results and future directions are explored.

Teacher classroom instructional practice can be a powerful influence on student learning. The Classroom Promotion of Oral Language randomized controlled trial tested whether teacher professional learning focused on promoting oral language in classrooms could improve teacher knowledge and classroom instruction. This study focuses on evaluating the impact of the professional learning on teacher classroom instruction. The nature of classroom talk was used as a means of exploring theories of change and the pathways to practice change. Classroom talk was classified according to its function; organisational, doing, and learning. Teachers from early years classes (first 2 years of school) participated in 4 days of professional learning and received implementation support in their school. Teacher instruction was measured through audio-recordings of classroom talk between teachers and students. The characteristics and proportions of the functions of classroom talk demonstrated by teachers who participated in professional learning (n = 40) and those who did not (n = 38) were compared. Findings indicated no differences in the functions of classroom talk measured with Anstey's type of talk framework following participation in the professional learning; similarities in classroom instruction are discussed and implications for professional learning.

Teacher knowledge is a critical factor in student success. In the Classroom Promotion of Oral Language cluster randomized controlled trial, we aimed to determine whether a teacher professional learning intervention focused on promoting oral language led to improved teacher knowledge. We report a study of the impact of professional learning on teacher knowledge of oral language and literacy constructs. The intervention comprised four days of face-to-face professional learning for early years' teachers, and ongoing school implementation support. Teacher knowledge was measured through a survey at baseline, 12 months and 24 months (n = 40 intervention, n = 38 control). Teachers in the intervention arm made more correct responses on survey items than teachers in the control arm, at both time points. The professional learning intervention improved teacher knowledge in the short-to-medium term.

Background and objectives: The Mediterranean Diet (MD) has been recognized for its benefits for human health and sustainability for the planet, but it has considered not easy to reproduce in other populations. The United Nations Educational, Scientific and Cultural Organization (UNESCO) Chair on Health Education and Sustainable Development is fostering a research project (Planeterranea), aiming to identify a healthy dietary pattern based on local foods with the same MD features. The aim of our study is to develop a MD-based food pyramid for Asian populations. Methods: Asia was stratified into six areas according to pedo-climatic conditions. For each region a comprehensive scoping review of local crops and typical foods was conducted on several databases such as the US Department of Agriculture (USDA)'s database, the Food and Agriculture Organization of the United Nations (FAO) website, and PubMed, focusing on both plant-based and animal-based foods. Narrative review was then conducted on the identified foods to determine their nutritional composition and planetary health impact. Finally, the collected information was used to build up the Asian food pyramid with details for each respective region. Results: We proposed a food pyramid for Asian countries, guaranteeing the same nutritional intake and health benefits as MD, by considering dietary habits and typical foods of this population. From the bottom to the top, Asian fruits and vegetables present similar nutritional profile as those in MD. Whole grains (barley) may represent valid alternative to white rice. Sesame oil represents a source of unsaturated fats and an alternative to olive oil. Legumes (soybean), edible insects, mushrooms and algae, guarantee an adequate intake of plant-based proteins with a complete amino-acid profile and a low environmental impact with respect to animal-based ones. The 1920s and 1930s were decades of momentous changes and expansion in the Hindi literary sphere In this period Hindi became an established public language in print, education and politics and struggled successfully to become the future national language of India. A market for Hindi literature was first created, journals provided venues for debate and literary expression as well as professional employment, genres and styles were explored in many new directions, and new voices emerged, importantly those of women writers. The nationalist movement, too, entered a new phase which emphasized popular publics and vernacular institutions. Through the concept of 'public sphere' as expounded by Jurgen Habermas and other political scientists, this thesis analyzes those changes at the levels of institutions, actors, discourses and, to a limited extent, of audiences in their proper context and in relation to each other. Chapter 2 explores changes in the literary sphere, both its expansion chiefly through the medium of journals, and its institutionalization through a linguistic and literary agenda in the education system. Chapter 3 analyzes historical debates and narratives in order to trace the consolidation and diffusion of a nationalist historical consciousness. Chapter 4 examines the development of women's journals and the space they provided for a critique of discrimination against women and their public access, and for the exploration of women's roles and emotionality. Chapter 5 focuses on the making of Hindi's claim to be the 'national language', the strategies employed and the exclusions operated in the process of its political affirmation. Chapter 6 explores the relationship between the literary and the political spheres focusing on the role of Hindi intellectuals and political leaders. The institutional authority gained in one sphere underwrote that gained in the other, it shall be argued, and gave credence to an official nationalism that does not reflect the complexity and variety of cultural imagination and literary practices in that period.

In India, as in other parts of the world, readers' exposure to the world and to world literatures largely took place through the pages of magazines, via translations, reviews, snippets of information, survey articles, and so on. The 1950s to 1970s were the golden age of magazine publishing in Hindi. Several Hindi magazines devoted to the short story not only showcased new literary talent but also invested much effort in translating writings from foreign literatures and from other Indian languages. Competing Cold War efforts to promote literatures from their rival spheres of influence produced a profusion of literary translations in magazine and book form, on which enterprising Hindi editors freely drew. This essay focuses on the spectacular special issues curated by Kamleshwar for two Hindi story magazines to explore the nexus between the short story, the magazine, and the world. Every region of India is and has been multilingual, with speakers of different languages and speakers of multiple languages. But literary ‘multilingual locals’ are often more fragmented than we think. While multilingualism suggests interest, and proficiency, in more than one literary language and tradition, very real barriers exist in terms of written vs. oral access, mutual interaction, and social and cultural hierarchies and exclusions. What does it mean to take multilingualism seriously when studying literature? One way, this essay suggests, is to consider works on a similar topic or milieu written in the different languages and compare both their literary sensibilities and their social imaginings. Rural Awadh offers an excellent example, as the site of many intersecting processes and discourses—of shared Hindu-Muslim sociality and culture and Muslim separatism, of nostalgia for a sophisticated culture and critique of zamindari exploitation and socio-economic backwardness, as the home of Urdu and of rustic Awadhi. This essay analyses three novels written at different times about rural Awadh—one set before 1947 and the others in the wake of the Zamindari Abolition Act of 1950 and the migration of so many Muslim zamindars from Awadh, either to Pakistan or to Indian cities. The first is Qazi Abdul Sattar’s Urdu novel Shab gazida (1962), the other two are Shivaprasad Singh’s Alag alag vaitarani (1970) and the Awadh subplot in Vikram Seth’s A Suitable Boy (1993). Without making them representatives of their respective languages, by comparing these three novels I am interested in exploring how they frame and what they select of Awadh culture, how much ground and sensibility they share, and how they fit within broader traditions of ‘village writing’ in Hindi, Urdu, and Indian English.

Background: Blinding of treatment allocation from treating clinicians in neonatal randomised controlled trials can minimise performance bias, but its effectiveness is rarely assessed. Methods: To examine the effectiveness of blinding a procedural intervention from treating clinicians in a multicentre randomised controlled trial of minimally invasive surfactant therapy versus sham treatment in preterm infants of gestation 25–28 weeks with respiratory distress syndrome. The intervention (minimally invasive surfactant therapy or sham) was performed behind a screen within the first 6 h of life by a 'study team' uninvolved in clinical care including decision-making. Procedure duration and the study team's words and actions during the sham treatment mimicked those of the minimally invasive surfactant therapy procedure. Post-intervention, three clinicians completed a questionnaire regarding perceived group allocation, with the responses matched against actual intervention and categorised as correct, incorrect, or unsure. Success of blinding was calculated using validated blinding indices applied to the data overall (James index, successful blinding defined as > 0.50), or to the two treatment allocation groups (Bang index, successful blinding: −0.30 to 0.30). Blinding success was measured within staff role, and the associations between blinding success and procedural duration and oxygenation improvement post-procedure were estimated. Results: From 1345 questionnaires in relation to a procedural intervention in 485 participants, responses were categorised as correct in 441 (33%), incorrect in 142 (11%), and unsure in 762 (57%), with similar proportions for each of the response categories in the two treatment arms. The James index indicated successful blinding overall 0.67 (95% confidence interval (CI) 0.65–0.70). The Bang index was 0.28 (95% CI 0.23–0.32) in the minimally invasive surfactant therapy group and 0.17 (95% CI 0.12–0.21) in the sham arm. Neonatologists more frequently guessed the correct intervention (47%) than bedside nurses (36%), neonatal trainees (31%), and other nurses (24%). For the minimally invasive surfactant therapy intervention, the Bang index was linearly related to procedural duration and oxygenation improvement post-procedure. No evidence of such relationships was seen in the sham arm. Conclusion: Blinding of a procedural intervention from clinicians is both achievable and measurable in neonatal randomised controlled trials. [ABSTRACT FROM AUTHOR]

Key Points: Question: For preterm infants with respiratory distress syndrome supported with continuous positive airway pressure (CPAP), does administration of surfactant via a thin catheter improve survival without moderate to severe neurodevelopmental disability (NDD) at 2 years of age compared with sham treatment? Findings: In this follow-up of a randomized clinical trial of 486 infants at 25 to 28 weeks' gestation, the composite outcome of death or NDD at 2 years of age occurred in 36.3% receiving minimally invasive surfactant therapy compared with 36.1% receiving sham treatment. Meaning: In preterm infants supported with CPAP, minimally invasive surfactant therapy did not lead to a reduction in the composite outcome of death or neurodevelopmental disability at 2 years of age. Importance: The long-term effects of surfactant administration via a thin catheter (minimally invasive surfactant therapy [MIST]) in preterm infants with respiratory distress syndrome remain to be definitively clarified. Objective: To examine the effect of MIST on death or neurodevelopmental disability (NDD) at 2 years' corrected age. Design, Setting, and Participants: Follow-up study of a randomized clinical trial with blinding of clinicians and outcome assessors conducted in 33 tertiary-level neonatal intensive care units in 11 countries. The trial included 486 infants with a gestational age of 25 to 28 weeks supported with continuous positive airway pressure (CPAP). Collection of follow-up data at 2 years' corrected age was completed on December 9, 2022. Interventions: Infants assigned to MIST (n = 242) received exogenous surfactant (200 mg/kg poractant alfa) via a thin catheter; those assigned to the control group (n = 244) received sham treatment. Main Outcomes and Measures: The key secondary outcome of death or moderate to severe NDD was assessed at 2 years' corrected age. Other secondary outcomes included components of this composite outcome, as well as hospitalizations for respiratory illness and parent-reported wheezing or breathing difficulty in the first 2 years. Results: Among the 486 infants randomized, 453 had follow-up data available (median gestation, 27.3 weeks; 228 females [50.3%]); data on the key secondary outcome were available in 434 infants. Death or NDD occurred in 78 infants (36.3%) in the MIST group and 79 (36.1%) in the control group (risk difference, 0% [95% CI, −7.6% to 7.7%]; relative risk [RR], 1.0 [95% CI, 0.81-1.24]); components of this outcome did not differ significantly between groups. Secondary respiratory outcomes favored the MIST group. Hospitalization with respiratory illness occurred in 49 infants (25.1%) in the MIST group vs 78 (38.2%) in the control group (RR, 0.66 [95% CI, 0.54-0.81]) and parent-reported wheezing or breathing difficulty in 73 (40.6%) vs 104 (53.6%), respectively (RR, 0.76 [95% CI, 0.63-0.90]). Conclusions and Relevance: In this follow-up study of a randomized clinical trial of preterm infants with respiratory distress syndrome supported with CPAP, MIST compared with sham treatment did not reduce the incidence of death or NDD by 2 years of age. However, infants who received MIST had lower rates of adverse respiratory outcomes during their first 2 years of life. 28. Epidemiological Impact of Metabolic Syndrome in Overweight and Obese European Children and Adolescents: A Systematic Literature Review.

The prevalence of overweight and obesity is continuously increasing, both in the adult and pediatric populations, posing a substantial challenge to public health. Understanding the epidemiological burden of metabolic syndrome (MetS) among children, particularly regarding its complications and long-term effects in adulthood, is crucial for identifying effective preventive measures and enhancing the clinical care of obese children. Therefore, by searching two databases, a systematic review was conducted in order to evaluate studies that specifically addressed the epidemiological MetS impact among overweight/obese European children and adolescents. Overall, 15 studies were considered. The epidemiological data concerning the MetS impact were contingent on the diagnostic criteria used and varied across countries, resulting in a prevalence range of 1.44% to 55.8%. Spanish studies were the most numerous (34%), revealing a country prevalence rate ranging from 2.5% to 19.6%. Males (prevalence range: 1.4–55.8%) and subjects with overweight/obesity (prevalence range: 12.9–55.8%) were mainly affected. Obesity emerged as the main risk factor in the MetS development and the consequent onset of cardiovascular complications and diabetes. Knowing the MetS burden and its risk factors could improve their prevention, detection, and treatment, and guide the development of targeted public health interventions to appropriately address the health needs of younger patients. Column-based liquid phase separation techniques, such as liquid chromatography (LC) in reversed phase separation mode and capillary electromigration techniques, using continuous electrolyte systems, are widely used for the identification and quantification of phenolic compounds in plants and food matrices of plant origin. This paper is the first of a two-part review article discussing fundamental and practical aspects of both LC and capillary electromigration techniques used for the analysis of phenolic compounds occurring in plant-derived food and in edible and medicinal plants. The chemical structure and distribution of the major phenolic compounds occurring in the plant kingdom, as well as the main methods used for their extraction and sample preparation, are also discussed. In 2014, two sets of Dalit literature scholars based in the West started working on two different collaborative grant projects that announced themselves as 'networks': Nicole Thiara and Judith Misrahi-Barak received AHRC funding to organise a series of programmes for the Dalit Literature Network, and Christi Merrill and her colleagues at the University of Michigan received internal funding to develop a suite of digital tools called Translation Networks (TN). In this dialogue with Francesca Orsini, Merrill uses examples from the overlap between these two different networks to describe how the TN concept mapping tool helps users visualise underlying scholarly infrastructures they depend on and in the process disrupt many of the binaries and hierarchies built into conventional archiving tools. To investigate the effects of providing rigid wrist-hand orthoses plus usual multidisciplinary care, on reducing hand impairments in children with cerebral palsy. A pragmatic, multicentre, assessor-blinded randomised controlled trial aimed to enrol 194 children aged 5–15 years, with wrist flexor Modified Ashworth Scale score ≥1. Randomisation with concealed allocation was stratified by study site and passive wrist range. The treatment group received a rigid wrist-hand orthosis, to wear ≥6 h per night for 3 years. Analysis included repeated measures mixed-effects linear regression models, using intention-to-treat principles. The trial stopped early due to insufficient recruitment: 74 children, across all Manual Ability Classification System levels, were randomised (n = 38 orthosis group; n = 36 control). Mean age was 10.2 (SD 3.1) years (orthosis group) and 9.1 (SD 2.8) years (control). Data showed some evidence that rigid wrist-hand orthosis impacted passive wrist extension with fingers extended in the first year [mean difference between-groups at 6 months: 13.15° (95%CI: 0.81–25.48°, p = 0.04); 12 months: 20.94° (95%CI: 8.20–33.69°, p = 0.001)]. Beyond 18 months, participant numbers were insufficient for conclusive findings. The study provided detailed data about short- and long-term effects of the wrist-hand orthosis and highlighted challenges in conducting large randomised controlled trials with this population. Trial Registration: Australia and New Zealand Clinical Trials Registry: U1111-1164-0572 There may be incremental benefit, for children with cerebral palsy, at 6 and 12 months on passive wrist range from wearing a rigid wrist-hand orthosis designed according to this protocol. The rigid-wrist-hand orthosis evaluated in this study, which allowed for some tailoring for individual children's presentations, differed in design from past recommendations for "resting hand" positioning. Longitudinal follow up of children with cerebral palsy prescribed a rigid wrist-hand orthosis is essential to monitor any benefit. Minor adverse events were commonly experienced when wearing the orthosis and should be discussed prior to prescription of a rigid wrist-hand orthosis. [ABSTRACT FROM AUTHOR]

Although many industries have already implemented technologies based on artificial intelligence (AI) in their business, the effects of new digital solutions on customer satisfaction are not yet fully known. This study aimed to evaluate the AI-based advice implemented by an Italian start-up operating in food supplements to support customer choices. The evaluation utilized the Delphi method and a questionnaire survey. This research aimed to provide companies wishing to use AI with a preliminary evaluation criterion for improving customer satisfaction through digital approaches. Research findings indicate that AI-based advice can improve customer perspectives, such as customer satisfaction and loyalty, by providing a value-added business service, diversified for each product category. However, some mistakes have emerged, which may still be a limitation in the use of AI-based advice. Therefore, this study presents an innovative approach to evaluate the performance of digital advice in traditional sectors such as the food industry. [ABSTRACT FROM AUTHOR]

Coenzyme Q
10 (CoQ10 ) bioavailability in vivo is limited due to its lipophilic nature. Moreover, a large body of evidence in the literature shows that muscle CoQ10 uptake is limited. In order to address cell specific differences in CoQ uptake, we compared cellular CoQ10 content in cultured human dermal fibroblasts and murine skeletal muscle cells that were incubated with lipoproteins from healthy volunteers and enriched with different formulations of CoQ10 following oral supplementation. Using a crossover design, eight volunteers were randomized to supplement 100 mg/daily CoQ10 for two weeks, delivered both in phytosome form (UBQ) as a lecithin formulation and in CoQ10 crystalline form. After supplementation, plasma was collected for CoQ10 determination. In the same samples, low density lipoproteins (LDL) were extracted and normalized for CoQ10 content, and 0.5 µg/mL in the medium were incubated with the two cell lines for 24 h. The results show that while both formulations were substantially equivalent in terms of plasma bioavailability in vivo, UBQ-enriched lipoproteins showed a higher bioavailability compared with crystalline CoQ10 -enriched ones both in human dermal fibroblasts (+103%) and in murine skeletal myoblasts (+48%). Our data suggest that phytosome carriers might provide a specific advantage in delivering CoQ10 to skin and muscle tissues. [ABSTRACT FROM AUTHOR]- Published

Objectives: Nurse home visiting (NHV) is widely implemented to address inequities in child and maternal health. However, few studies have examined longer-term effectiveness or delivery within universal healthcare systems. We evaluated the benefits of an Australian NHV program ("right@home") in promoting children's language and learning, general and mental health, maternal mental health and wellbeing, parenting and family relationships, at child ages 4 and 5 years. Setting and participants: Randomised controlled trial of NHV delivered via universal, child and family health services (the comparator). Pregnant women experiencing adversity (≥2 of 10 risk factors) were recruited from 10 antenatal clinics across 2 states (Victoria, Tasmania) in Australia. Intervention: Mothers in the intervention arm were offered 25 nurse home visits (mean 23·2 home visits [SD 7·4, range 1–43] received) of 60–90 minutes, commencing antenatally and continuing until children's second birthdays. Primary and secondary outcomes measured: At 4 and 5 years, outcomes were assessed via parent interview and direct assessment of children's language and learning (receptive and expressive language, phonological awareness, attention, and executive function). Outcomes were compared between intervention and usual care arms (intention to treat) using adjusted regression with robust estimation to account for nurse/site. Missing data were addressed using multiple imputation and inverse probability weighting. Results: Of 722 women enrolled in the trial, 225 of 363 (62%) intervention and 201 of 359 (56%) usual care women provided data at 5 years. Estimated group differences showed an overall pattern favouring the intervention. Statistical evidence of benefits was found across child and maternal mental health and wellbeing, parenting and family relationships with effect sizes ranging 0·01–0·27. Conclusion: An Australian NHV program promoted longer-term family functioning and wellbeing for women experiencing adversity. NHV can offer an important component of a proportionate universal system that delivers support and intervention relative to need. Approaches to world literature often think through binaries of local/global, major/minor, provincial/cosmopolitan, taking them as given positions on a single world map. To an extent, this is true of Amitav Ghosh’s prize-winning essay “The testimony of my grandfather’s bookcase” (1998), which reflects on his grandfather’s collection of world literature books to think about the relationship between his grandfather’s provincial location in Calcutta and the world. Yet in The Shadow Lines Ghosh takes a much more complex and interesting approach to space, the world, perception and narration. In the novel’s complex narration, space, time, and self always appeared mirrored through other people, times, and spaces. Places also acquire reality and meaning only after they are first narrated and imagined, often several times, and before they are experienced directly. This is a stance that has deep existential but also epistemological implications that go beyond “simply” critiquing colonial and national border-making. This essay explores how (and which) spaces become “significant” in the novel, and how the novel’s approach to space can be productive for thinking about world literature.
