2. How do social relationships, amongst other diverse factors, shape community change initiatives in the context of climate change?

- Abstract
Guiding deliberate change in the context of climate change is a complex social process, shaped by multiple social factors over time, involving different actors, preferences and perspectives. One factor widely recognised as important is social relationships. However, understandings about how to navigate and work through social relationships to shape collective change processes are currently limited. This thesis examines how social relationships, amongst diverse other factors, shapes community change in the context of climate change. Through a series of stand-alone chapters drawing on a range of qualitative methods, overall four key insights are provided on how social relationships interact with community change initiatives in the context of climate change. These are: (1) quality of relationships; (2) multiple intersecting normative factors; (3) how particular ideas and initiatives are interpreted; and (4) the intentions guiding collective change processes as a whole. This shows that social relationships are much more than structural patterns of connections between actors. They entail qualitatively different interactive opportunity spaces shaped by multiple social identities and social norms. These factors not only guide why and how relationships develop but also how relationships shape interpretations and any actions that unfold. Social relationships are clearly important within community initiatives. Yet, they do not emerge through a static dynamic and their nature shifts over time, resulting also in changes in the way they influence how actors interpret and engage with different situations and initiatives, and then what emerges through and from these complex social processes. Actors seeking to strategically work through relationships will need nuanced understandings of what social relationships entail and which recognise the multiple different normative dimensions involved in shaping how they develop. This nuance is critical for working with complex social processes more widely and for shaping opportunities for the emergence of meaningful change over time.

Este estudio tiene como finalidad la investigación sobre el desarrollo de los ingenieros en comunicaciones y electrónica (ICE) que laboran en Servicios a la Navegación en el Espacio Aéreo Mexicano (SENEAM). Con el propósito de determinar cuáles han sido las transiciones y sus proyecciones en el entorno laboral durante su permanencia en dicho organismo, y conocer y desarrollar la curva de aprendizaje desde sus inicios profesionales como ICE en el área de Ingeniería de Servicios IDS, comunicaciones, radio ayudas o radar. Las curvas de aprendizaje son modelos empíricos que permiten estudiar las transformaciones tecnológicas como resultado del aprendizaje. Partiendo de esto, el aprendizaje es comprendido como la sabiduría adquirida por la repetición de un proceso (learning-by-doing) (Arrow, 1962). Wright (1996) publicó un artículo donde afirma que la curva de aprendizaje se observó por primera vez en 1920. Al respecto, Hirschmann (1964) comenta “que la práctica hace la perfección, que las cosas se pueden hacer mejor no solo la segunda vez, sino cada vez que lo intente”; la curva de aprendizaje es la que se encarga de cuantificar y representar gráficamente este rendimiento (Hirschmann, 1964). La SENEAM cuenta con más de 40 años en México; proporcionando servicios de ayuda a la navegación aérea para la seguridad, fluidez y orden en el espacio aéreo mexicano; garantizando calidad y eficiencia conforme a la normatividad nacional e internacional aplicable. Se espera que esta investigación dé a conocer los factores que determinan la curva de aprendizaje de los ingenieros en comunicaciones y electrónica, caso SENEAM.

RESUMO A pandemia do novo coronavírus (Covid-19) é uma realidade no Brasil e no mundo com potencial para desencadear sintomas psicológicos. O objetivo deste trabalho foi realizar uma revisão integrativa da literatura sobre saúde mental e pandemias. Foram cruzados os unitermos “saúde mental AND Covid-19 OR coronavírus OR pandemias AND intervenções em saúde mental OR psicotrópicos OR sintomatologia psiquiátrica OR psicopatologias OR fator de proteção em saúde mental” nas bases Scientific Electronic Library Online, Centro Latino-Americano e do Caribe de Informação em Ciências da Saúde e National Library of Medicine (PubMed), resultando em 1.642 artigos publicados em português, inglês e espanhol, no período de 2015 a 2020. Foram selecionados 43 artigos, de acordo com critérios de inclusão e exclusão, e todos foram publicados em 2020, referindo-se ao primeiro ano da pandemia de Covid-19. Sintomas depressivos e ansiosos, estresse e insônia foram frequentemente relatados e estiveram relacionados a ser profissional da saúde, adulto jovem, mulher; ter menor escolaridade e renda, histórico de doença crônica ou transtorno psiquiátrico e maior exposição nas mídias. Algumas intervenções psicológicas podem ser utilizadas para minimizar os impactos negativos da pandemia de Covid-19. Além disto, fatores de proteção podem ser explorados para reduzir os prejuízos na saúde mental dos indivíduos e das coletividades.

Nas últimas décadas, o aleitamento materno exclusivo (AME) no Brasil apresentou tendência ascendente, cujos principais ganhos foram observados entre 1986 e 2006, seguindo de uma estabilização em 2013 que foi de 36,6%. O presente estudo avaliou aspectos nutricionais de lactentes de 0 a 6 meses atendidos no Programa Mãe-Bebê. Trata-se de um estudo quantitativo e retrospectivo. Foi utilizado banco de dados de puérperas e lactentes atendidos no Programa Mãe-Bebê, no período de março de 2016 até junho de 2018. Foram incluídos lactentes que receberam AME e/ou misto até o sexto mês de idade. A classificação do peso ao nascer e classificação do estado nutricional foi descrito através das curvas de crescimento de peso para o comprimento (P/C) e comprimento para idade (C/I), conforme preconizado pela OMS. A amostra foi composta por 79 lactentes e 34,2% foram amamentados exclusivamente. Ao correlacionar P/C com duração do AME não houve diferença significativa, contudo, 26,6% das crianças em AME foram classificadas eutróficos quando avaliado P/C aos 06 meses de idade, e 34,2% das crianças em AME até 180 dias estavam com comprimento adequado para a idade. Quando avaliamos o P/C observamos que 27,8% nasceram com risco para sobrepeso e após 06 meses de idade estavam classificadas em obesidade. Podemos concluir com este estudo que crianças amamentadas exclusivamente ao seio no primeiro semestre de vida apresentaram estado nutricional de eutrofia, evidenciando que o leite materno o alimento é ideal para o crescimento adequado nos seis primeiros meses de vida.

La insuficiencia hepática aguda pediátrica es un síndrome de disfunción severo y repentino de los hepatocitos que lleva a un fallo de la función de síntesis y detoxificación. Es una enfermedad grave y poco frecuente, pero potencialmente fatal, que se presenta en un niño previamente sano. La etiología se relaciona con la edad del paciente y la región geográfica de procedencia, reconociéndose el origen: metabólica, infecciosa, tóxica, autoinmune, vascular y tumoral. La causa indeterminada, donde la búsqueda etiológica es negativa, puede oscilar entre el 18 y el 47%, dependiendo del centro y acceso a la realización de los estudios etiológicos. El proceso que determina la injuria hepática no es aún bien conocido y se considera multifactorial. Depende esencialmente de la susceptibilidad del huésped, la causa y severidad del daño y la capacidad de regeneración hepática. La presentación clínica depende de la etiología, comenzando habitualmente con un episodio de hepatitis aguda, que en días o semanas eviden - cia una evolución desfavorable, profundizando la ictericia, acompañada del gran compromiso del estado general y de una severa coagulopatía. El tratamiento consiste en la implementación de medidas generales que tenderán a corregir las alteraciones metabólicas, el aspecto nutricional, y la prevención y tratamiento de todas las complicaciones que se presentan en el transcurso evolutivo como infecciosas, neurológicas, etc. Además, es fundamental implementar el tratamiento específico de aquellas enfermedades que pueden beneficiarse del mismo (hepatitis aloinmune, galactosemia, tirosinemia, infección por herpes simple, enfermedad de Wilson, etc). Sin embargo, a pesar de los avances terapéuticos, la insuficiencia hepática aguda resulta en muerte o trasplante hepático en más del 45% de los casos.

Restoration is a long-standing concept in ecology and guiding principle for many environmental institutions including the European Union's proposed Nature Restoration Law. The terms ‘restoration’ and the newer term of ‘Nature-based Solutions’ are both commonly used, sometimes interchangeably. However these concepts are not identical and understanding these differences might help us find ways to tackle the nature and climate crises more effectively. We examine the implications of the different concepts, to help shape and prioritise future action.

El desarrollo urbano bajo parámetros sostenibles indudablemente incide en la calidad de vida de los ciudadanos. Tradicionalmente, el ordenamiento urbanístico español ha focalizado sus esfuerzos en el crecimiento exponencial de la ciudad, contribuyendo al desarrollo de la construcción de viviendas. Pero este enfoque ha experimentado un cambio sustancial en la última década, hacia un modelo de planificación urbana que promueva los procesos de rehabilitación y regeneración en la ciudad ya consolidada, minimizando el crecimiento disperso, en consonancia con las exigencias europeas en materia de desarrollo sostenible. Si se consideran las condiciones físicas de multitud de edificios y barrios de nuestro país, con deficiencias importantes de habitabilidad y consumo energético, problemas de accesibilidad y carencia de servicios, la rehabilitación urbana constituye, en la actualidad, una necesidad inexcusable. En este trabajo se analiza la importancia de los procesos de rehabilitación urbana en las ciudades, cuáles son las fuentes de financiación pública que impulsan estos proyectos y cómo las cooperativas pueden protagonizar un papel relevante en este ámbito. Los desafíos que plantea la rehabilitación urbana demandan una respuesta desde las fórmulas cooperativas, que contribuyen a la cohesión económica, social y territorial en el marco de un desarrollo urbano sostenible.

This paper provides an outline of a new interdisciplinary project called FixOurFood, funded through UKRI's 'Transforming UK food systems' programme. FixOurFood aims to transform the Yorkshire food system to a regenerative food system and will work to answer two main questions: (1) What do regenerative food systems look like? (2) How can transformations be enabled so that we can achieve a regenerative food system? To answer these questions, FixOurFood will work with diverse stakeholders to change the Yorkshire food system and use the learning to inform change efforts in other parts of the UK and beyond. Our work will focus on shifting trajectories towards regenerative dynamics in three inter‐related systems of: healthy eating for young children, hybrid food economies and regenerative farming. We do this by a set of action‐orientated interventions in schools and the food economy, metrics, policies and deliverables that can be applied in Yorkshire and across the UK. This article introduces the FixOurFood project and concludes by assessing the potential impact of these interventions and the importance we attach to working with stakeholders in government, business, third sector and civil society. Ten essentials are presented for community resilience initiatives in the context of achieving a 1.5 °C world: enhance adaptability; take account of shocks and stresses; work horizontally across issues; work vertically across social scales; aggressively reduce carbon emissions; build narratives about climate change; engage directly with futures; focus on climate disadvantage; focus on processes and pathways; and encourage transformations for resilience. Together the essentials highlight that resilience initiatives seeking to retain the status quo will be detrimental when they enable societies to cling to unsustainable activities. Instead, climate resilience initiatives need to be viewed more as a process of transformative social change, where learning, power, inequities and relationships matter. Finally, there is an urgent need for researchers to shift focus away from examining the nature of resilience to accelerating learning about fostering resilience in practice.

Aims and Objectives: This report presents findings from an action research project conducted in the Scottish Borders between May 2015 and September 2016. The project aimed to:1) Support a local process of community change through building partnerships, learning and capacity building; and2) Understand the critical factors involved in facilitating the development of community resilience to climate change to draw out key levers for change nationally.The project was a collaboration between the University of Dundee, the Scottish Borders Council, Tweed Forum, Southern Uplands Partnership, International Futures Forum and the Scottish Association of Marine Sciences. It worked with three communities that had experience of flooding in the Borders council area and involved bringing together diverse organisations and community members in workshops and other activities.

The Scottish Borders Climate Resilient Communities Project (SBCRC)1 was a participatory action research project that sought to build community resilience to climate change through working in three communities with a history of flooding in the Scottish Borders. It aimed to:- Understand some of the critical factors that contribute to shaping community resilience in the context of climate disadvantage;- Understand how community resilience to climate change can be developed in different local contexts in practice by supporting and facilitating engagement between members of three local communities and other stakeholders in the Scottish Borders region, and evaluating outcomes;- Draw out lessons for policymakers and practitioners on how to support the development of community resilience in the context of climate change.The project was structured around nine workshops (three per community) that brought together different organisations (e.g. the Scottish Borders Council, local NGOs) to examine issues and develop actions to build community resilience in the context of climate change. A tenth workshop used the outcomes from the work within the communities to examine how a more integrated and synergistic national policy landscape in Scotland could be developed to enhance community resilience to climate change. An evaluation helped to inform project delivery and the overall findings.

Hydro-hazards are increasing in frequency due to climate change which has inspired a cultural change in Flood Risk Management (FRM). Uncertainty associated with climate change has resulted in a shift towards flood resilience as it helps deal with unexpected climatic perturbations that impact extreme flows. The concept of resilience has increased in popularity, leading to a multitude of definitions, measurements and applications. This paper systematically reviews the FRM literature to provide clarity on the differing perspectives of resilience and how they influence successful implementation of the concept. Our analysis assesses where FRM is positioned within three pre-defined interdisciplinary understandings of resilience. The polysemic nature of resilience has produced a multitude of different perspectives that prevent successful operationalisation. Resilience is interdisciplinary; therefore it requires integration between top-down and bottom-up FRM approaches, as well as a more holistic approach to the interdependence between temporal and spatial scales. The concept of ecosystem services has been extensively studied in recent decades. Most studies have focused on describing the specific aspects such as production, spatial extent, valuation of services and the trade-off between services. Few studies however assess the practitioners’ views on the frameworks, models or tools developed. In this paper, we report on a multi-stakeholder workshop where two tools were tested (i) the ecosystem service cascade framework was tested as a means to frame the issues and (ii) a participatory-spatial modelling method, QUICKScan, was tested as an aid to support discussion over natural resource management and planning in a multi-use landscape. A focused group discussion was utilised to determine stakeholders’ views of the cascade framework and pre- and post-workshop questionnaires quantified the stakeholders’ views of the QUICKScan method. The stakeholders identified both positive and negative aspects of both tools. The diversity of views expressed were associated with (i) the past experience of the individual with the issues discussed, (ii) the technical aspects of the tools i.e. the ability with GIS and (iii) the level of new shared knowledge they reported acquiring on the day which was related to their initial knowledge of the issue and area studied. 33. Assessing Compliance With Established Pneumonia Core Measures at a Comprehensive Cancer Center.

Healthcare organizations use Pneumonia Core Measures (PCMs) to ensure delivery of high-quality care. In this study, a multidisciplinary team was organized to optimize care and enhance compliance in a comprehensive cancer emergency center. We performed a four-phase study, three of which were interventional: intense education regarding PCM; microbiologic analysis of the pathogens responsible for the pneumonias; development and implementation of an institutional pneumonia algorithm and order set. In phase 4, we analyzed five PCMs. The percentage of pneumonia patients from whom blood cultures were obtained increased from 73% to 91% after intervention (p < .001); sputum cultures increased from baseline 24.6% to 51% (p = .004) post order-set implementation, and order-set utilization increased from 40% to 77%. We achieved the benchmark for only one PCM, PN 3a. More than 80% of patients met clinical and microbiological criteria for healthcare-associated pneumonia. We identified a gap between our patient population and some PCMs that relates to antibiotics selection. The treatment of cancer patients and pneumonia falls outside established guidelines for treating community-acquired pneumonia. Although the algorithm and order set implemented optimized care and minimized variation, national benchmarks for four of the PCMs were not met. Our findings provide information for policymakers considering pneumonia measurements for antibiotic selection in a cancer care setting. [ABSTRACT FROM AUTHOR]

- Abstract
If we want to learn how to deal with the COVID-19 pandemic, we have to embrace the complexity of this global phenomenon and capture interdependencies across scales and contexts. Yet, we still lack systematic approaches that we can use to deal holistically with the pandemic and its effects. In this Discussion, we first introduce a framework that highlights the systemic nature of the COVID-19 pandemic from the perspective of the total environment as a self-regulating and evolving system comprising of three spheres, the Geosphere, the Biosphere, and the Anthroposphere. Then, we use this framework to explore and organize information from the rapidly growing number of scientific papers, preprints, preliminary scientific reports, and journalistic pieces that give insights into the pandemic crisis. With this work, we point out that the pandemic should be understood as the result of preconditions that led to depletion of human, biological, and geochemical diversity as well as of feedback that differentially impacted the three spheres. We contend that protecting and promoting diversity, is necessary to contribute to more effective decision-making processes and policy interventions to face the current and future pandemics. The Tonic Immobility Scale has been only studied in abuse and/or sexual assault survivors, but tonic immobility (TI) also occurs after other types of traumatic events. Consequently, we modified the scale for this purpose and studied its factor structure, internal consistency, and con-vergent, divergent and discriminant validity in 392 university students, most of whom had suffered one or more traumatic events of various kinds. The Fear subscale of the Modified Tonic Immobility Scale (MTIS) showed very low internal consistency, and must be eliminated or reformu-lated. Discrimination indices of items 5 and 8 were very low, and these items must be reformulated or removed. After removing the five previous items, the shortened version of the MTIS had a one-factor structure, alt-hough model fit was not completely satisfactory. This scale showed ac-ceptable levels of reliability, convergent validity with post-traumatic symp-tomatology, divergent validity with measures of depression and anxiety, and discriminant validity differentiating between groups that had and had not experienced traumatic events. The psychometric properties of the shortened version of the MTIS should be studied further. It may also be advisable to add new items of TI in order to establish whether this is a construct with more than one dimension., 8 páginas
