Aim: To evaluate the output of scientific publications in obstetrics and gynecology journals from Mainland China and six other countries, to compare the difference of research situation between Mainland China and the developed countries in order to measure the trend of number of publication in the field of Obstetrics and Gynecology. Methods: Information on article amounts, impact factors (IF), citation and publication in high‐impact obstetrics and gynecology journals by seven top‐ranking countries between January 1, 2007 and August 31, 2017, was extracted from Web of Science databases. Results: A total of 137 661 articles were published between 2007 and 2017 in the 98 obstetrics and gynecology journals. The cumulative IF and citations of articles from USA were the highest. The highest average citation was France and Mainland China ranked fourth. There were 52 631 papers published in the top 10 obstetrics and gynecology journals and the USA accounted for the majority of these papers (33 978, 64.6%), followed by England (8347, 15.9%). Articles from the USA were most often published in American Journal of Obstetrics and Gynecology (10658) and those from Mainland China were mostly published in Fertility and Sterility (809). Conclusion: The USA published the most articles having high IF, making the greatest contribution to the field of obstetrics and gynecology. Although the number of scientific publications from Mainland China has rapidly increased from 2007 to 2017, the quality of these publications has not been as satisfactory. It implys that Chinese current academic evaluation system should pay special attention to the quality of academic papers, instead of the number of papers published by scientists. This paper explores national patterns of entry to primary school in England over the past decade. It focuses on deferred entry (where children begin Reception with the cohort below) and delayed entry (where children miss some or all of Reception, and enter Year 1 with their 'normal' cohort). In 2014, the Department for Education's (DfE's) guidance began to be updated to clarify the 'right to request' later entry. Analyses of the National Pupil Database (NPD) show a rise in deferral rates from this point. However, rates of delayed entry remain fairly steady after this time. Variation by local authority in deferral rates has become wider over recent years. Children with special educational needs and/or disabilities (SEND), as recorded in the NPD, are now much more likely to defer entry, alongside those who would be youngest in their 'normal' cohort (August-borns). Children from families eligible for free school meals (FSM) and from families speaking English as an Additional Language (EAL) are less likely. When these factors are interacted, children with SEND from more advantaged families (proxied by FSM and EAL) are most likely. Alongside additional inequalities in patterns of deferral and delay, these findings interrogate the DfE's assessment that 'good progress has been made' in terms of 'right to request' later school entry, and that, 'the system is now working well'. They also raise wider questions around the accessibility, fairness and efficacy of such quasi-policies which confer a 'right to request' aspects of a child's education, rather than enshrining them through legislation as an unconditional, mainstreamed option for all. A twice-daily record of barometric pressure exists for Durham Observatory (54.768 °N, 1.584 °W, barometer cistern 107.3 m above mean sea level, MSL) from 23 July 1843 to 31 December 1960 and is published here for the first time. The Durham record, which is 98.7% complete, is by far the longest digital barometric pressure series in northern England and fills a very large temporal and spatial gap in the International Surface Pressure Database (ISPD: Cram et al, [2015] Geoscience Data Journal, 2, 31-46). In what is believed to be the first study of its kind, the record has been independently quality-controlled against the NOAA-CIRES-DOE Twentieth Century Reanalysis version 3 (20CRv3; Slivinski et al., [2019] Quarterly Journal of the Royal Meteorological Society, 145, 2876; Slivinski et al., [2021] Journal of Climate, 34, 1417-1438), which did not include the Durham records in its assimilation set. This paper describes the instruments used and their exposure, the sources of the record, digitization work undertaken to generate the digital time series (including quality control assessments using 20CRv3) and reduction to mean sea level pressure from station level observations, and examines consistency over the period of record against 20CRv3, concluding with a summary of monthly and annual means and extremes over the 117 year series and the details of the new dataset. Purpose: Myopia prevalence has increased in the UK at age 10‐16y, but little is known about younger children. We hypothesise that if the 'myopia epidemic' is affecting young children, then there will be increasing rates of bilateral reduced unaided vision (V) at vision screenings of children 4–5 years of age. Methods: Retrospective anonymised data from computerised vision screening at age 4–5 years were analysed from serial cross‐sectional data. Refractive error is not assessed in UK vision screening, so vision was investigated. Data were only included from schools that screened every year from 2015/16 to 2021/22. The criterion used was unaided monocular logMAR (automated letter‐by‐letter scoring) vision >0.20 in both the right and left eyes, so as to maximise the chances of detecting bilateral, moderate myopia rather than amblyopia. Results: Anonymised raw data were obtained for 359,634 screening episodes from 2075 schools. Once schools were excluded where data were not available for every year and data were cleaned, the final database comprised 110,076 episodes. The proportion (percentage and 95% CI) failing the criterion from 2015/16 to 2021/22 were 7.6 (7.2–8.0), 8.5 (8.1–8.9), 7.5 (7.1–7.9), 7.8 (7.4–8.2), 8.7 (8.1–9.2), 8.5 (7.9–9.0) and 9.3 (8.8–9.7), respectively. The slope of the regression line showed a trend for increasing rates of reduced bilateral unaided vision, consistent with increasing frequency of myopia (p = 0.06). A decreasing linear trendline was noted for children 'Under Professional Care'. Conclusions: For children 4–5 years of age, there were signs of reduced vision over the last 7 years in England. Consideration of the most likely causes support the hypothesis of increasing myopia. The increase in screening failures highlights the importance of eye care in this young population. Rationale, Aims, and Objectives: Several novel oral anticoagulants (NOACs) are licensed for atrial fibrillation (AF) treatment in the United Kingdom. We describe the incidence and mortality from ischaemic stroke and major bleeding in non‐valvular atrial fibrillation (NVAF) patients in England, including treatment patterns before/following introduction of NOACs, healthcare resource utilization (HRU), and costs post‐onset of these events. Method: Data were extracted from the UK Clinical Practice Research Datalink linked to Hospital Episode Statistics secondary care and Office for National Statistics mortality data. Results: Of 42 966 patients with a first AF record between 2011 and 2016, 9143 patients (21.3%) remained without AF (antiplatelets/antithrombotics) treatment post‐index diagnosis. The proportion of patients receiving aspirin for ≥3 months post‐index declined during the study (50.6%‐5.5%), irrespective of CHA2DS2‐VASc score, while the proportion prescribed NOACs increased (2.0%‐70.1%). Rates of ischaemic stroke per 1000 patient‐years (95% CI) were 9.4 (3.8‐15.0) with NOACs, 10.4 (8.0‐12.9) with warfarin, 20.1 (16.4‐23.8) with aspirin, 21.3 (5.3‐37.2) with other antiplatelets and 43.6 (39.3‐47.8) in patients without AF prescription. Major bleeding occurred at a similar rate with different treatments. All‐cause mortality rates were 42.8 (31.4‐54.3) with NOACs, 46.3 (41.1‐51.5) with warfarin, 56.5 (50.5‐62.4) with aspirin, 102.2 (76.2‐128.3) with other antiplatelets and 412.8 (399.6‐426.0) with no AF prescription. Mean annual National Health Service healthcare costs up to 1 year post‐index were lowest in patients receiving aspirin plus other antiplatelets without an event (£6152), and highest in patients with an event without AF prescriptions (£17 957). By extrapolation, national AF HRU in the United Kingdom in 2016 was estimated at £8‐16 billion annually. Conclusions: These data provide temporal insights into AF treatment patterns and outcomes for NVAF patients in England and highlight the need to review higher stroke risk AF patients not receiving antiplatelet/antithrombotic prescriptions. Background: Red blood cells are essential for modern medicine but managing their collection and supply to cope with fluctuating demands represents a major challenge. As deterministic models based on predicted population changes have been problematic, there remains a need for more precise and reliable prediction of use. Here, we develop three new time-series methods to predict red cell use 4 to 52 weeks ahead.Study Design and Methods: From daily aggregates of red blood cell (RBC) units issued from 2005 to 2011 from the NHS Blood and Transplant, we generated a new set of non-overlapping weekly data by summing the daily data over 7 days and derived the average blood use per week over 4-week and 52-week periods. We used three new methods for linear prediction of blood use by computing the coefficients using Minimum Mean Squared Error (MMSE) algorithm.Results: We optimized the time-window size, order of the prediction, and order of the polynomial fit for our data set. By exploiting the annual periodicity of the data, we achieved significant improvements in long-term predictions, as well as modest improvements in short-term predictions. The new methods predicted mean RBC use with a standard deviation of the percentage error of 2.5% for 4 weeks ahead and 3.4% for 52 weeks ahead.Conclusion: This paradigm allows short- and long-term prediction of RBC use and could provide reliable and precise prediction up to 52 weeks ahead to improve the efficiency of blood services and sufficiency of blood supply with reduced costs. Objectives: In persons with hepatitis C virus (HCV) infection, HIV coinfection leads to faster progression to advanced liver disease. The aim of our study was to estimate diagnosed HIV prevalence among people with evidence of current HCV infection (polymerase chain reaction positive) and examine predictors of coinfection. Methods: Adults (≥ 15 years old) with a current HCV infection reported to the Public Health England (PHE) sentinel surveillance of blood‐borne viruses were linked to the PHE national HIV database using a deterministic methodology. Descriptive and multivariate analyses were conducted. Results: Between 2008 and 2014, 5.0% (999/20 088) of adults with a current HCV infection were diagnosed with HIV coinfection. The majority acquired HIV through sex between men (441; 64.9%), followed by injecting drug use (153; 22.5%) and heterosexual contact (84; 12.4%). Of persons who were coinfected, 65.5% had been diagnosed with HIV infection > 6 months before their HCV diagnosis, 41.4% of whom had a negative anti‐HCV test between their HIV and HCV diagnoses. In a multivariable model among persons with current HCV infection, an HIV diagnosis was more likely among men [adjusted odds ratio (aOR) 3.29; 95% confidence interval (CI) 2.60–4.16] and persons of black ethnicity (aOR 3.19; 95% CI 1.36–7.46), and less likely among older adults (aOR 0.85 per 10‐year increase; 95% CI 0.79–0.92) and persons of Asian ethnicity (aOR 0.59; 95% CI 0.41–0.86). Conclusions: Our results indicate that the majority of diagnosed HIV and current HCV coinfections are among men who have sex with men. Safer sex campaigns should include awareness of transmission of HCV among MSM living with HIV. [ABSTRACT FROM AUTHOR]
