Background: Acute head and neck cancer (HNC) bleeding is a life-threatening situation that frequently presents to the emergency department (ED). The purpose of the present study was to analyze the risk factors for the 30-day mortality in patients with HNC bleeding.Methods: We included patients who presented to the ED with HNC bleeding (n = 241). Patients were divided into the survivor and nonsurvivor groups. Variables were compared, and the associated factors were examined with Cox's proportional hazard model.Results: Of the 241 patients enrolled, the most common bleeding site was the oral cavity (n = 101, 41.9%). More than half of the patients had advanced HNC stage while 41.5% had local recurrence. The proportion of active bleeding was significantly higher in the nonsurvivor group (70.5% vs. 53.3%, p = 0.038). 42.3% received blood transfusion and 5.0% required inotropic support. In total, 21.2% of the patients experienced rebleeding, and 18.3% died within 30 days. Multivariate analyses indicated that a heart rate > 100 (beats/min) (HR = 2.42; Cl 1.15-5.06; p = 0.019) and inotropic support (HR = 3.00; Cl 1.14-7.89; p = 0.026) were statistically significant independent risk factors for 30-day mortality.Conclusions: The results of this study may aid physicians in the evaluation of short-term survival in HNC bleeding patients and provide critical information for risk stratification and medical decisions. The mammalian target of rapamycin (mTOR)-sterol regulatory element-binding proteins (SREBPs) signaling promotes lipogenesis. However, mTOR inhibitors also displayed a significant side effect of hyperlipidemia. Thus, it is essential to develop mTOR-specific inhibitors to inhibit lipogenesis. Here, we screened the endogenous inhibitors of mTOR, and identified that FKBP38 as a vital regulator of lipid metabolism. FKBP38 decreased the lipid content in vitro and in vivo via suppression of the mTOR/P70S6K/SREBPs pathway. 3,5,6,7,8,3ʹ,4ʹ-Heptamethoxyflavone (HMF), a citrus flavonoid, was found to target FKBP38 to suppress the mTOR/P70S6K/SREBPs pathway, reduce lipid level, and potently ameliorate hyperlipidemia and insulin resistance in high fat diet (HFD)-fed mice. Our findings suggest that pharmacological intervention by targeting FKBP38 to suppress mTOR/P70S6K/SREBPs pathway is a potential therapeutic strategy for hyperlipidemia, and HMF could be a leading compound for development of anti-hyperlipidemia drugs. Soot particles, mainly coming from fuel combustion, affect climate forcing through absorbing light and also result in adverse human health outcomes. Though biodiesel or additives blending with diesel was considered environmentally friendly, the understanding on absorbing and oxidative capacity of soot emitted from them are still unclear. The water-soluble organic carbon (WSOC) content, surface chemical structure, light absorption and oxidative potential (OPDTT) of soot from biodiesel/diesel and chemicals/diesel blends were investigated utilizing total organic carbon analyzer, X-ray photoelectron spectrometer, ultraviolet–visible spectrophotometry and dithiothreitol (DTT) assay. The differences and correlations between soot properties were statistically analyzed. Chemicals/diesel blends soot owned significantly higher WSOC content, ratio of mass absorbing efficiency (MAE) in 250 and 365 nm (E 2 /E 3), OPDTT, and higher surface carbonyl content. Coconut biodiesel/diesel blends soot contained evidently higher aromatic carbon–oxygen single bond (Ar_C–O) content, and higher MAE 365. The individual comparison of biodiesel/diesel blends showed 20% coconut biodiesel blend owned the lowest WSOC, E 2 /E 3 and OPDTT, while highest Ar_C–O and MAE 365 , representing strongest absorbing properties. Association analysis showed OPDTT was significantly positively correlated with WSOC. Further, the evident negative correlation between MAE 365 and OPDTT was observed. Our results showed coconut biodiesel/diesel blends soot induced lower levels of oxidative potential, whereas absorption of light was higher, which have far reaching consequences on climate forcing. • The phase detection of single frame interference fringe provides the possibility of dynamic phase measurement. • Traditional image segmentation methods can not effectively segment the inverted region of phase sign. • We first applied the deep learning based on Swin Transformer structure to correct the spurious phase sign inversion in single closed-fringe demodulation. • We compared with the classic Convolutional neural network, the network based on Swin Transorfermer structure has better generalization ability. Single-shot fringe analysis draws extensive attention dues to the possibility for dynamic phase measurement. However, phase extraction from single interferogram with closed fringes is still a difficult task, the dominate reason is spurious phase sign problem. The fringe direction map is usually required to unambiguous the spurious phase sign; however, it is not a trivial process even though numerous algorithms are able to estimate the fringe direction. Swin Transformer has shown great promise as it integrates the advantages of both CNN and Transformer. In this contribute therefore, we propose to unambiguous the spurious phase sign problem using transformer based Swin-ResUnet which has strong feature extraction capabilities. Acute inflammatory or neuropsychiatric symptoms, such as headache, fatigue, anosmia, and hyposmia, sometimes persist for more than 30 days or longer than 12 weeks after infection with the Omicron variant of SARS‑CoV‑2 (hereafter referred to as COVID-19). The aim of this study was to determine whether detection of zinc concentration or vitamin D concentration could provide treatment benefits for patients with COVID-19, thus reducing the risk of them experiencing long COVID. The interval between the date of COVID-19 diagnosis and the date of visit to pulmonary department for prolonged symptoms of COVID-19 was recorded for statistical analysis. Inductively coupled plasma mass spectrometry for detecting zinc and chemiluminescence immunoassay for detecting vitamin D were performed in laboratory tests. Fifty-five patients were included. Of the participants, 29.1 % and 27.3 % had vitamin D and zinc deficiency, respectively. On average, the patients underwent long COVID treatment for 31.7 ± 17.7 days. A positive statistical correlation was observed between vitamin D and zinc concentrations (Pearson's correlation = 0.378). Compared with sufficient zinc levels, zinc deficiency was associated with a higher fibrinogen level (p < 0.05). Within 30 days, the observed vitamin D deficiency rate was only 21.4 %; after 30 days, the vitamin D deficiency rate rose to 37.0 % (McNemar's chi-square test; p < 0.05). Zinc deficiency correlates to acute and persistent inflammation and vitamin D deficiency is associated with delayed recovery in long COVID syndrome. [ABSTRACT FROM AUTHOR]

Abstract A membrane electro-bioreactor (MEBR) embracing biological treatment, electrokinetic phenomena and membrane filtration was established by applying intermittent direct current (DC) to MBR. MEBR exhibited significant improvement of treatment performance and reduction of membrane fouling. COD and total phenols removal efficiencies increased to 83.53% and 93.28% at an exposure mode of 24′-OFF/6′-ON, compared to 71.24% and 82.43% in MBR. Trans-membrane pressure increment rate declined dramatically in MEBR, which was mainly attributed to the increase of sludge floc size and decrease of zeta potential, soluble microbial products and specific resistance to filtration, resulted from electrokinetic effects such as electrocoagulation, electrophoresis, electroosmosis and electromigration of ions. It was notable that DC exposure exerted distinct evolution on microbial community, with the improvement of microbial community richness and diversity. The relative abundances of functional genera were promoted noticeably in MEBR. Background/purpose: We aim to evaluate the accuracy of the new prehospital notification criteria for patients with potential acute stroke in the prehospital setting.Methods: We conducted a retrospective observational study from March 2011 to February 2013 of potential acute stroke patients prenotified using the new criteria which were: (1) positive Cincinnati Prehospital Stroke Scale (CPSS); (2) symptom onset within 3 hours; and (3) blood glucose level > 60 mg/dL. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the new criteria were calculated and outcomes of acute stroke patients were reported. Data of all patients with stroke or transient ischemic attack (TIA) transported to the destination hospital were also obtained to evaluate the compliance of emergency medical technicians.Results: There were 2888 patients suspected of stroke by emergency medical technicians and 221 patients prenotified due to meeting the criteria. The PPV, NPV, sensitivity, and specificity of the new criteria were 76.9%, 96.6%, 64.9%, and 98.1%, respectively. Onset time > 3 hours (24/51, 47.1%) and seizure (27.5%) were the two most common conditions leading to false prenotification. Of all prenotified patients, 23.1% (51/221) received thrombolytic therapy. Hemorrhagic stroke or ischemic stroke with hemorrhagic transformation (53.8%) and minor symptoms or rapid recovery (26.9%) were the most common reasons excluding correctly prenotified patients from thrombolytic therapy.Conclusion: The accuracy of the new prehospital stroke criteria has higher PPV and specificity compared to previous CPSS validation studies. Graphical abstract Abstract Despite great advances in contemporary medicine, brain death still remains enigmatic and its cellular and molecular mechanisms unsettled. This review summarizes recent findings that substantiate the notion that PTEN/FLJ10540/PI3K/Akt cascade, the classical tumorigenic signaling pathway, is actively engaged in experimental brain stem death. These results were based on a clinically relevant animal model that employs the pesticide mevinphos as the experimental insult in Sprague-Dawley rats to mimic brain stem death in patients died of organophosphate poisoning. The neural substrate investigated is the rostral ventrolateral medulla (RVLM), a brain stem site classically known to maintain arterial pressure (AP) and is established to be the origin of a “life-and-death” signal detected from AP, which reflects brain stem cardiovascular dysregulation that precedes death. Activation of PI3K/Akt signaling pathway in the RVLM upregulates the nuclear factor-κB/nitric oxide synthase II/peroxynitrite cascade, resulting in impairment of brain stem cardiovascular regulation that leads to the loss of the “life-and-death” signal in experimental brain stem death. This process is reinforced by FLJ10540, a PI3K-association protein; and is counteracted by PTEN, a negative regulator of PI3K/Akt signaling. The concept that a classical signaling pathway in tumorigenesis is also an active player in cardiovascular dysregulation in brain stem death provides new ramifications for translational medicine. It promulgates the concept that rather than focusing on a particular disease condition, a new vista for future therapeutic strategy against both fatal eventualities should target at this common cellular cascade. As water stress, including drought and waterlogging, can severely affect plant growth, this study investigated the effects of an endophyte from the genus Epichloë on two different ecotypes of Festuca sinensis grass under five soil water conditions in a controlled greenhouse experiment. Changes in F. sinensis plants grown with (E+) and without the endophyte (E−) were evaluated as they were subjected to different water treatments (20%, 35%, 50%, 65% and 80% relative saturation moisture content, RSMC). Growth parameters such as plant height, number of tillers, blade width, stem diameter, root length, total biomass, root-shoot ratio and relative water content were determined. The results showed that drought and waterlogging significantly ( P < 0.05) inhibited the growth of F. sinensis . The presence of the endophyte significantly ( P < 0.05) increased plant growth and root-shoot ratio under drought and waterlogged conditions. In addition, the plant height, number of tillers, blade width, stem diameter and total biomass in seedlings of both ecotypes reached the maximum at 65% RSMC, which suggests the optimal water condition. These findings also show that moderate drought (35% and 50% RSMC) could promote root growth of grass seedlings.
