Platformarbeid is een relatief nieuw fenomeen. Hierbij worden ZZP-ers ingezet die op basis van feiten en omstandigheden niet zelden als werknemer aan te merken zijn. In Nederland geldt dat deze arbeidsrelaties beoordeeld worden op basis van de reguliere criteria voor de kwalificatie van de arbeidsrelatie uit het BW. De invulling van deze criteria is in beweging, enerzijds vanuit jurisprudentie en anderzijds vanuit plannen voor nieuwe wetgeving. Specifiek over platformarbeid zijn er inmiddels 3 uitspraken van de Hoge Raad, waarin in twee zaken (Deliveroo en Uber) werd geoordeeld dat de platformwerkers geen ZZP-er was, maar werknemer en 1w zaak (Helpling) waarin de platformwerker geen ZZP-er was, maar uitzendkracht. In de EU wordt momenteel gewerkt aan een richtlijn voor de verbetering van de arbeidsvoorwaarden bij platformwerk. Deze richtlijn is nog niet in werking getreden. Het doel van de richtlijn is om aan de hand van criteria vast te kunnen stellen of een dienstverband tussen de platformwerker en het platform moet worden aangenomen. Dit zou de positie van de platformwerker moeten verbeteren. De criteria die bepalen of er sprake is van een dienstverband zijn gedurende het wetgevingsproces uitgebreid ten opzichte van het initiële voorstel voor de conceptrichtlijn. Deze scriptie tracht antwoord te geven op de vraag hoe de Nederlandse situatie zich verhoudt tot de nieuwe conceptrichtlijn.

Discontinuation of antidepressant medication can be difficult due to withdrawal symptoms and relapse risk. Scientific evidence on the questions of who, when, and how to stop antidepressants is limited. In Amsterdam a multidisciplinary outpatient clinic was started to provide advice and guidance.To substantiate the design of the clinic. Central questions relate to knowing which patients are referred, the background of their request, and their experiences with the outpatient clinic.The first 51 patients of the clinic were described on the basis of file research, in addition a survey was conducted into patient experiences.Half of the patientsa id="_idTextAnchor005"/a(55%) actually started discontinuation, 39% were advised not to do so (yet). Patients at the clinic had used antidepressants for an average of 10 years, and 76% had previously attempted to stop. 21% had now successfully stopped and 25% were satisfied with a lower dose. One patient relapsed during tapering.So far, patients with long-term antidepressant use and multiple quit attempts have been referred. Our experiences are aimed at helping individual patients but can also result in more knowledge about who can stop at what moment, and how this should be done.

Verpleeg- en Verzorgingshuizen en Thuiszorg (VVT) organisaties werken veelal met cliëntportalen, die cliënten en hun naasten inzage bieden in het medisch dossier. Hoewel het gebruik van een cliëntportaal grote voordelen heeft, kan het ook leiden tot onvoorziene nadelen en ethische bezwaren, zowel bij wilsbekwame als bij wilsonbekwame cliënten. In dit artikel bespreken we enkele van deze bezwaren. We sluiten af met een aantal aanbevelingen voor de praktijk.

Een noodopgraving in 2017 op de Schultingaweg in de toenmalige gemeente Ten Boer heeft middeleeuwse skeletten van daar begraven inwoners uit het nagenoeg verdwenen dorp Lutjewolde aan het licht gebracht. When a child or adolescent is admitted to a Highamp; Intensive Care (HIC) unit for adolescents, this implies that they find themselves in a severe psychiatric crisis. The work policy aiming autonomy and recovery is a vision that has been implemented in the treatment method at the HIC unit for adolescents. This policy focuses on the recovery of young people, in which learning to understand their own story and learning taking responsibility, are essential components. Sharing these experiences from doctors and patients aims to increase knowledge about this policy. The consistent implementation of this vision and method will result in high-quality care, more recovery and reduction of urge and coercive interventions. Calls are made for a broad-based implementation of a work policy aiming autonomy and recovery in the Netherlands.

In cardiology, but also in neurology and vascular surgery, the current scientific evidence for this is still insufficient. Nevertheless, the role of pharmacogenetics will gain importance in today's medical world, where the demand for personalised medicine is on the rise. The implementation of genotyping in the clinic will nonetheless be a practical challenge due to a lack of clarity about who will bear the associated costs. In patients with coronary artery disease and a higher bleeding risk, it is valuable to determine the CYP2C19 genotype prior to treatment with clopidogrel. Pending further studies, we recommend that specialists prescribing clopidogrel should determine the CYP2C19 genotype in patients at high risk of recurrent ischemic events.

Het Ministerie van LNV heeft naar aanleiding van het rapport “Zoönosen in het vizier” een aantal vragen gesteld, met nadruk op de tussen-bedrijfstransmissie. De gestelde vragen hadden betrekking op de mogelijke toekomstige oorzaken van zoönosen, verspreidingsmechanismen van zoönosen, precieze betekenis en berekening van de RH-waarde, beschikbaarheid van RH-waarden voor verschillende diersoort-pathogeen combinaties, en de rol van aanvullend onderzoek in de beantwoording van vragen over deze onderdelen. De gestelde vragen zijn door de werkgroep van deskundigen beantwoord op basis van de expertise, maar zonder volledig literatuuronderzoek. Vervolgens zijn aanvullend een aantal overwegingen gegeven die betrekking hebben op de beperkingen van de geldigheid van de antwoorden op de gestelde vragen. Daarnaast wordt kort ingegaan op recente kennisontwikkelingen die in de nabije toekomst nieuw perspectief zullen of kunnen bieden bij de beantwoording van de gestelde vragen en preventie en management van zoönotische uitbraken in een periode van transitie naar een duurzamere veehouderij.

Samenvatting Doel Het diagnosticeren en behandelen van psychische klachten bij niet-westerse migranten in de huisartsenpraktijk blijkt moeilijk. Het Brief Cultural Interview (BCI) kan helpen bij het inzicht verkrijgen in relevante culturele factoren. We onderzochten de effecten van en ervaringen met het BCI toegepast door praktijkondersteuners-geestelijke gezondheidszorg (poh’s-ggz). Methode Geclusterd effectonderzoek onder niet-westerse migranten die met psychische klachten bij de huisarts kwamen. In de interventiegroep zaten elf poh’s; vijf poh’s-ggz gaven gebruikelijke zorg. Patiënten beantwoordden bij de start van de behandeling en vier maanden later vragen over vertrouwen in de zorg, waardering van de patiënt-poh-relatie, gevoel van controle, en psychische en fysieke gezondheid. We verzamelden kwalitatieve data over de ervaringen van de poh’s-ggz en patiënten met het BCI. Resultaten In totaal werden 87 patiënten geïncludeerd. In de interventiegroep namen 31 patiënten deel aan de voor- en nameting, en in de controlegroep vijftien. De verschilscores op de uitkomstmaten verschilden niet significant tussen beide groepen. Zeven van de elf poh’s-ggz uit de interventiegroep hebben patiënten geïncludeerd. De geïnterviewde acht poh’s-ggz en negen patiënten waren positief over de meerwaarde van het BCI. Conclusie Het onderzoek heeft beperkte kennis opgeleverd over het gebruik van het BCI door de poh’s-ggz. Het BCI kan worden ingezet voor het bevorderen van cultuursensitief handelen, maar is moeilijker toepasbaar dan gedacht. Verder onderzoek is nodig naar de effecten van het BCI en de implementeerbaarheid in de huisartsenpraktijk.

SAMENVATTINGDit artikel presenteert een nieuw meetinstrument om ‘morele veerkracht’te meten. Morele veerkracht is het vermogen van een individu om om tegaan met en te herstellen van situaties die als moreel belastend wordenervaren. We gaan in op het vertaal- en aanpassingsproces van de RushtonMoral Resilience Scale (RMRS) naar het Nederlands, en de uitdagingen diewe hierin tegenkwamen. De RMRS-NL is een aanvulling op reeds bestaandeinstrumenten die morele competenties of morele stress bepalen, omdat hetde mogelijkheid biedt rechtstreeks het vermogen van zorgverleners te bepalenom goed om te gaan met de morele belasting die zij ervaren in hun werk. SUMMARYThis article presents the Dutch translation of a new scale to measure “moralresilience”, i.e. “the capacity of an individual to sustain or restore their integrityin response to moral adversity” (Rushton, 2016). We will discuss the translationand adaptation process of the Rushton Moral Resilience Scale (RMRS) tothe Dutch language and context, and the challenges we encountered inthis process. The RMRS(-NL) complements pre-existing instruments thatdetermine moral competences or moral stress, as it offers the possibility todirectly determine the capacity of caregivers to properly cope with the moralburden they experience in their work.

Veel mensen met een depressieve stoornis hebben last van repetitieve, oncontroleerbare en indringende suïcidale beelden en gedachten. Deze zogenaamde 'suïcidale intrusies' kunnen mogelijk een aanknopingspunt zijn voor versterking van suïcidale ideatie en suïcidaal gedrag. Verschillende experimentele studies hebben aangetoond dat oogbewegingen tijdens het ophalen van de onaangename beelden hun levendigheid en negatieve emoties verminderden. Als toevoeging aan de reguliere behandeling kan een Eye Movement Dual Task (EMDT)-interventie mogelijk de suïcidale intrusies verminderen en daarmee het risico op suïcide verlagen. Het doel van de huidige pilotstudie onder zes deelnemers is om de bruikbaarheid, veiligheid en uitvoerbaarheid van de interventie te evalueren. De pilotstudie dient als voorbereiding op een groot multicenter gerandomiseerde klinische trial (RCT). Resultaten geven een positieve indruk van de veiligheid, verdraagbaarheid, en uitvoerbaarheid van deze additionele EMDT-interventie. Participanten gaven aan tevreden te zijn over het ontvangen van de interventie en vonden het invullen van de online vragenlijsten haalbaar. Therapeuten gaven aan dat het EMDT-onderzoeksprotocol uitvoerbaar was en gaven enkele suggesties ter verbetering en/of optimalisering. De pilotstudie liet geen aanwijzingen zien voor een toename van de ernst van de suïcidaliteit bij patiënten na de EMDT-interventie als toevoeging aan de lopende behandeling, wat erop wijst dat EMDT voldoende veilig lijkt om de effectiviteit ervan in een multicenter RCT te evalueren

Dealing with euthanasia requests can be a complex matter for a doctor. How to determine whether the due diligence criteria of the Dutch Euthanasia Act are met in cases that are not straightforward? We argue that moral case deliberation (MCD), methodically structured reflective discussions on concrete moral dilemmas, can provide support in dealing with complex euthanasia requests. In this article, we discuss a case that was talked about during a MCD (in particular the CURA method, where CURA stands for 'concentrating, postponing, reflecting and action') by a group of GPs and nurses who met in the context of a network for ambulatory palliative care.This was about an older patient with multiple chronic conditions who lost any prospects of quality of life.Among other things, it was examined whether requests could be 'well-considered' (one of the due diligence criteria) when the patients are in doubt as to when euthanasia should be carried out.The importance of recognising one's own emotions as a doctor and the quality of communication between patient and doctor were also considered.For that reason, we try to show that MCD can assist in making careful and well-considered decisions when determining a course of action in the case of complex euthanasia requests and can encourage collaborative learning processes.

With the exception of a few medical specialties, the implementation of pharmacogenetic tests in daily practice has thus far been limited. The Royal Dutch Pharmacists Association (KNMP) has developed pharmacogenetics-based therapeutic doserecommendations for 80 medicinal product combinations on the basis of a systematic literature review. Genotyping of patients can take place on a reactive or pre-emptive basis; the advantage of pre-emptive genotyping is that it provides genetic information the moment a medicinal product is prescribed. Clinical decision support software is crucial to implement pharmacogenetics into daily practice.

AimDiagnosing and treating mental health problems in non-western migrants visiting general practice appears difficult. The Brief Cultural Interview (BCI) can help by providing insight into relevant cultural factors. We examined the effects of and experiences with the BCI as implemented by mental health nurses in general practice.MethodsClustered effect study among non-western migrants who visited their general practitioner with mental health problems. Eleven mental health nurses were trained to conduct the BCI and five mental health nurses provided care as usual. At the start of treatment and four months later, patients answered questions on trust in health care, the patient-nurse relationship, self-control, physical and mental health. We collected qualitative data on the experiences of mental health nurses and patients with the BCI.ResultsIn total 84 patients were included. In the intervention group 31 patients participated at baseline and follow-up, in the control group 15. Seven out of eleven trained nurses included patients. Change scores on the outcomes did not differ significantly between groups. The eight mental health nurses and nine patients interviewed were positive about the usefulness of the BCI.ConclusionThe study has to a limited extent added to our knowledge on the BCI implemented by mental health nurses. The BCI can be used to enhance culturally sensitive care, but is not easy to implement in daily care. Future research is needed on the effects of the BCI and the ease of implementation in general practice.

Schoolleiders staan voor de uitdaging om in hun school een professionele leercultuur te creëren, anders gezegd, om van hun school een professionele leergemeenschap te maken. In een leercultuur of professionele leergemeenschap hebben leraren de tijd en de ruimte om met en van elkaar te leren, gericht op het verbeteren van hun dagelijkse praktijk. Dit leidt tot betere leerprocessen van leerlingen, heeft een positieve impact op de professionele ontwikkeling van de leraren zelf, en draagt bij aan schoolontwikkeling.

Feasibility was assessed by compliance rates and satisfaction of patients and therapists (CSQ). Effectiveness was assessed within and between groups by the Insomnia Severity Index (ISI) and the Patient Health Questionnaire (PHQ-9) at baseline and after 6 and 12 weeks.br/RESULTS: In the intervention group 41% completed all treatment sessions and 77% completed 3 or more sessions. Patients rated the intervention positively (CSQ 23.6, range 4-32). Sleep improved in the intervention group (Cohen's d = 1.93), as well as depression (Cohen's d=1.05). In the control group there was no significant improvement. The difference between the two groups was high and significant for sleep problems (Cohen's d = 0.99) but not for depressive symptoms.br/CONCLUSION: Online CBT-I is a feasible treatment option for depressed patients in mental health care settings. There are indications that CBT-I is also effective in reducing sleep problems in more severely depressed patients.

Doel Preventieve cognitieve therapie (PCT) is een zelfhulptherapie voor de eerste lijn, ontwikkeld om terugval in een depressie te voorkomen. Methode Gerandomiseerd gecontroleerd onderzoek waarin gebruikelijke zorg (onder andere antidepressiva) werd vergeleken met gebruikelijke zorg plus PCT. Deelnemers waren 18 jaar of ouder, hadden minimaal twee depressies doorgemaakt en waren depressievrij. De primaire uitkomstmaat was terugval na twaalf maanden. De interventie bestond uit een zelfhulpboek met acht hoofdstukken, waarvan de deelnemers er elke week een bestudeerden en opdrachten maakten. Ze hadden wekelijks telefonisch contact met de poh-ggz gedurende maximaal vijftien minuten. Resultaten De 248 deelnemers werden gerandomiseerd naar een interventiegroep (n = 124) en een controlegroep (n = 124). Na twaalf maanden waren 44 deelnemers (35,5%) in de interventiegroep en 62 deelnemers (50,0%) in de controlegroep teruggevallen in een depressie (incidentieratio 0,71; 95%-BI 0,52 tot 0,97). In de interventiegroep was na twaalf maanden de ervaren kwaliteit van leven sterker toegenomen (0,04 punten op de EQ-5D; 95%-BI 0,004 tot 0,08) en waren depressieve symptomen sterker afgenomen (–2,18 punten op de QIDS-sr; 95%-BI –3,09 tot –1,27) dan in de controlegroep. De interventie had geen effect op comorbide psychopathologie, zelfredzaamheid en kwaliteit van leven gemeten met de SF-12. Conclusie Dit onderzoek bevestigt het hoge risico op terugval na een depressieve episode. Een psychologische zelfhulpinterventie met begeleiding van de poh-ggz blijkt een welkom alternatief voor gebruikelijke zorg, waaronder antidepressiva.

Due to growing discontent about the national government in the 1780’s a revolutionary movement attempted to regain the old days of the Golden Age, with its local autonomies. The effect of this Patriotic movement was the opposite of what they had intended. The creation of the Bataafse Republiek at first seemed hopeful, but under French influence the liberties that still had existed disappeared step by step. Centralization overtook the role that democracy was originally intended to play. However, at the end of this period, the situation of the countryside government had lesser changed than one should have expected. After the revolutionary turbulence in the period 1795-1798 it appeared that people with the same background as the pre-1795 rulers were still in charge in the government of 1813. Thanks to the exceptions that were made in the successive constitutions for the small communities, even a substantial local autonomy had remained. This was mostly due to the short time the Napoleonic rule had lasted. However, the path Napoleon set out was followed by his successor, king Willem I. The emphasis was still stricter placed on central, royal, government. For the first time there came a regulation for the villages. The concluding chapters throw a light on this period with its strict rules for appointments and policies. The schout became ʻburgemeesterʼ, he was the central person in the village with great local powers. It is surprising to see that even in that period of strict regulations a number of villages endeavored, and sometimes succeeded, in following their own plans. The group of people who made these local policies, still resembled very much the group that did so at the end of the eighteenth century. It can be said that the typical Dutch ʻpoldermodelʼ, what was said to have vanished after 1795 and returned not earlier than at the end of the nineteenth century, had a longer life in the Utrecht countryside.
