Dit is de vierde druk van het Handboek ouderenpsychiatrie. Sinds de eerste druk in 1997 wordt dit boek hoog gewaardeerd en gezien als het Nederlandstalig standaardwerk voor de ouderenpsychiatrie. Met behulp van maar liefst 59 auteurs is deze vierde, geheel herziene druk tot stand gekomen onder redactie van de vijf hoogleraren ouderenpsychiatrie in Vlaanderen en Nederland. Veel ouderen vinden een goede kwaliteit van leven het belangrijkste. Aantasting van hun psychische gezondheid vermindert de kwaliteit van leven aanzienlijk, en vaak in grotere mate dan bij verlies van lichamelijke gezondheid. Goede herkenning en behandeling van psychiatrische stoornissen bij ouderen is daarom essentieel. De laatste klinische en wetenschappelijke inzichten van de verschillende psychiatrische stoornissen bij ouderen worden besproken, zoals geclassificeerd in de DSM-5. Eerst wordt echter ruim aandacht geschonken aan de specifieke context van de oudere mens en de ouderenpsychiatrie. Zo wordt uitgebreid ingegaan op de biologische, psychologische en sociale aspecten van veroudering, alsmede de diagnostische en therapeutische principes relevant voor het werken met ouderen. We hopen dat het boek hiermee vele professionals stimuleert te kiezen voor een carrière in de ouderenzorg en -psychiatrie. Het boek is bedoeld voor de opleiding, en bij- en nascholing van alle beroepsgroepen die werkzaam zijn in de (ouderen)psychiatrie en/of ouderenzorg. Een must-have voor alle psychiaters (in opleiding) en een waardevolle ondersteuning voor alle professionals in de gezondheidszorg die te maken hebben met psychiatrische stoornissen bij ouderen.

Introduction: Diagnosing behavioral variant frontotemporal dementia (bvFTD) is challenging because of its broad differential diagnosis and the clinical resemblance with the symptoms of psychiatric illnesses like bipolar disorder, schizophrenia, depression, obsessive compulsive disorder, kleptomania and personality disorder. The new FTD consortium criteria (2011) emphasize the importance of distinguishing possible and probable bvFTD. A significant number of possible bvFTD patients remain with normal neuroimaging results and without functional decline at long term follow up: the bvFTD phenocopy syndrome. A neurodegenerative nature of this condition is unlikely although an alternative explanation is missing. Case description: Patient D was a 52 year old man who was initially diagnosed with bvFTD according to the former Neary et al. FTD consensus criteria (1998). Clinical follow up of 3 years raised doubt on this diagnosis because of repeated neuroimaging results and the absence of functional decline. We describe the multidisciplinary diagnostic process including neurological examinations, neuropsychological examinations and psychiatric observation on a clinical ward. His neuropsychiatric syndrome could be explained by a combination of a below-average intelligence, cluster C personality traits and relational problems and the diagnosis of bvFTD was withdrawn. Conclusion: Acknowledging possible bvFTD as a clinical syndrome that might not be caused by neurodegenerative FTD can reduce the number of false positive diagnoses and is important to maintain quality in medicine. Using this new criteria a flow chart is proposed to improve diagnostic process.

We present the case of a 52-year-old man who was diagnosed with the behavioural variant of frontotemporal dementia (bvFTD) in accordance with consortium criteria from 1998. The diagnosis was questioned in the years of follow-up, since neuroimaging showed no abnormalities and no deterioration of clinical symptoms was seen. After 3 years, the diagnosis was withdrawn after the psychiatrist concluded that his low intelligence, combined with a cluster C personality and relational problems, had caused his altered behaviour. Diagnosing bvFTD is challenging because of its clinical resemblance to neurodegenerative and psychiatric illnesses. The new bvFTD consortium criteria from 2011 emphasize the importance of distinguishing possible and probable bvFTD. This can reduce the number of incorrect diagnoses. We present a flow chart based on these criteria, which could aid in improving the diagnostic process.

Electroconvulsion therapy (ECT) is the generation of an epileptic seizure by means of a brief pulse of electrical current under general anaesthesia and is used to treat psychiatric disorders. The principal indication for ECT is severe depression, with or without psychotic characteristics. ECT works quicker and is more effective than antidepressants and has a lower risk of side effects. The principle side effect is retrograde amnesia. ECT can be administered with unilateral or bilateral electrodes. In the Netherlands the unilateral electrode is used, as this probably gives rise to fewer cognitive side effects.
