To describe the care which women with postpartum depression (PPD) in the Netherlands use for their complaints, and the impact of PPD on their general use of care for themselves and for their child, and on their participation in work.The data came from the control group of a prospective comparative study on the effectiveness of screening for PPD within the setting of Youth Health Care.We obtained data by means of two online questionnaires. Three weeks postpartum, we examined the background characteristics of the mother. Twelve months postpartum, we inquired about depression since birth, care use for depressive symptoms, general care use since birth for both mother and child, and participation in work up to 12 months postpartum. To test differences, we used chi-square and student t-tests.Of the 1049 participating women, 99 (9.4%) indicated that they had experienced depression in the year since giving birth. Of the 99 'women with PPD', 71.0% made at least some use of care aimed at their PPD complaints. Of these women with PPD, 31.3% were diagnosed with depression, and 37.7% were actually treated. Mothers with PPD used considerably more care for themselves and their child than mothers without PPD. Absenteeism from work was significantly higher among women with PPD.The limited number of women with PPDreceiving care by and the social costs entailed by PPD justify investment in routine screening and customized care pathways for these women.

Spondylodiscitis is usually caused by microorganisms, but there are also non-infectious causes.We are describing an 84-year-old man with severe pain in the side and elevated inflammation parameters. MRI of the spinal column yielded a picture suggesting spondylodiscitis. Repeated peripheral cultures and culture of a vertebral biopsy did not yield a pathogen. Intravenous antibiotics had no effect on symptoms or inflammation parameters. When the physical examination was repeated, we found arthritis in the feet and tophi. Microscopic examination of a new vertebral biopsy found urate crystals. This meant we were dealing with spondylodiscitis as manifestation of gout. Treatment with colchicine was highly successful.Spinal column gout is unknown, but seems to occur with some regularity. This disease can be symptom-free but may also lead to myelopathy or spondylodiscitis. In case of spondylodiscitis without demonstrated pathogen in patients with gout or risk factors for this, the vertebral biopsy should be evaluated for urate crystals or a dual-energy CT should be considered.

Obesity is an important risk factor for morbidity and premature death, as well as a contributing factor to psychosocial problems. The incidence of obesity has increased dramatically over the last few decades.- Obesity is considered to be a multifactorial condition in which both environmental factors and genetic factors play a part.- In approximately 5% of patients with morbid obesity, a monogenic cause can be identified. Mutations in the MC4R gene are the most frequently occurring monogenic cause of obesity.- The department of Genetics at the VU University Medical Center Amsterdam offers morbidly obese patients a diagnostic analysis of 50 obesity-associated genes. - An underlying obesity-associated genetic defect can influence patient response to certain treatments. Therefore, if the gene defect is known, it can be taken into account when considering treatment options.- The understanding of the genetics of obesity will significantly contribute to research into the development of personalized treatment options.

Op 10 augustus 2014 wilde de nationalistische groepering ‘Pro Patria’ een mars door de Haagse Schilderswijk houden om te demonstreren tegen de vermeende islamisering van deze buurt en tegen Nederlandse jihadisten die de terroristische organisatie IS ondersteunen. Dankzij politieoptreden kon een gewelddadige confrontatie tussen aanhangers van Pro Patria en bewoners van de Schilderswijk onder wie enkele radicale islamisten worden voorkomen. De bijna-confrontatie in de Schilderswijk valt samen met oplopende spanningen elders in Europa, zich uitend in verijdelde aanslagen in Duitsland en het Verenigd Koninkrijk. Dit alles vormt de aanleiding voor de Nationale Coördinator Terrorismebestrijding en Veiligheid (NCTV) om vanuit het WODC (Wetenschappelijk Onderzoek- en Documentatiecentrum) wetenschappelijk onderzoek te initiëren naar de interactie tussen de anti-islambewegingen en radicale moslims. De onderzoekers hebben hierbij de focus verlegd van bewegingen en organisaties naar velden. De twee bewegingen worden gezien als oppositionele velden, dat wil zeggen, als sociale arena’s waar gestreden wordt om de toe-eigening van economisch, sociaal en cultureel kapitaal. Ieder veld is gecentreerd rond specifieke belangen. Zowel het ‘anti-islamveld’ als het ‘radicale- islamveld’ zijn bijzonder dynamisch; groepen, organisatie en netwerken komen en gaan. Het onderzoek richtte zich daarom niet zozeer op de wisselwerking tussen organisaties als wel op de wisselwerking tussen actoren in het anti-islamveld en het radicale-islamveld.

A 48-year old woman in good general health was referred to the orofacial pain clinic in a centre for special dentistry with a toothache in the premolar region of the left maxillary quadrant. The complaints had existed for 15 years and various dental treatments, including endodontic treatments, apical surgery, extraction and splint therapy, had not helped to alleviate the complaints. As a result of the fact that anti-epileptic drugs were able to reduce the pain it was concluded that this 'toothache' satisfied the criteria of an atypical odontalgia: 'toothache' with a neuropathic background.Een 48-jarige vrouw in goede algemene gezondheid presenteerde zich op de afdeling Gnathologie van een centrum voor bijzondere tandheelkunde met pijnklachten in het tweede kwadrant. De klachten bestonden reeds 15 jaar en diverse tandheelkundige behandelingen, waaronder endodontische behandelingen, chirurgische apicale endodontische behandeling, extractie en de vervaardiging van een stabilisatieopbeetplaat, hadden niet geholpen. Uit het feit dat een anti-epilepticum de pijn deed afnemen, werd geconcludeerd dat er sprake was van atypische odontalgie: ‘kiespijn’ met een neuropathische achtergrond.

OBJECTIVE: The development and testing of a questionnaire to enable anesthesiology residents to assess the training qualities oftheir clinician-educators. DESIGN: Questionnaire. METHODS: The taskforce drafted a questionnaire based on the 26 item Stanford Faculty Development Program questionnaire (SFDP26) and adapted to the Dutch situation. Following its discussion in separate meetings ofanesthesiology residents and clinical staff, the questionnaire was further edited. The resulting instrument contained 6 teaching domains and 26 items. The questionnaire was made available electronically to anesthesiology residents only. Participation was voluntary, confidential and anonymous. Group results were presented at a plenary session. Anesthesiology staff received their individual scores by e-mail; results were confidential. RESULTS: 21 residents assessed 39 anesthesiologists. A total of 423 questionnaires were completed. Factor analysis resulted in the reduction and re-grouping of the teaching domains. The internal consistency ofthe teaching domains was high (Cronbach-alpha > or = 0.86). The assessment results of the teaching qualities of the anesthesiology faculty were positive. 'Communication of goals' was the lowest scoring (mean = 3.41) and 'Professional attitude towards residents' the highest scoring teaching domain (mean = 4.07). Gender did not correlate with the assessment scores of faculty. 'Year of training' was negatively correlated with most of the teaching domains. CONCLUSION: A feasible and reliable instrument for the evaluation of Dutch clinician-educators is now available

Hypermobility of the temporomandibular joint is only noted when it interferes with smooth mandibular movements. These interferences may result from a condylar dislocation beyond the temporal eminence at maximum mouth opening. Aim of this study was to test whether the condyle of a symptomatically hypermobile temporomandibular joint is positioned more anterosuperiorly to the temporal eminence at maximum mouth opening than a condyle without hypermobility. Nine persons with a hypermobile temporomandibular joint and 9 control persons participated. Diagnostics were based upon opto-electronic mandibular movement recordings. Condylar positions at maximum mouth opening were assessed by magnetic resonance imaging. A small significant difference in condylar position was found between groups. Condyles of persons with a hypermobile temporomandibular joint moved beyond the temporal eminence. However, this was also true for nearly half of the control persons. This suggests that condylar position alone is not a sufficient condition for symptomatic hypermobility of the temporomandibular joint. Maybe, symptoms of hypermobility only become apparent in combination with a particular line of action of the masticatory muscles.
