Four major double-blind randomized trials in kidney transplant patients have shown that the interleukin-2 receptor (IL-2R alpha) antagonists declizumab or basiliximab, when added to an immunosuppressive regimen consisting of cyclosporin and prednisone, reduce the incidence of acute rejections after kidney transplantation by 30-40%, during the first 6 months. Daclizumab and basiliximab are monoclonal antibodies of which the variable parts are of mouse origin and the other components of human origin. The addition of the interleukin-2 receptor antagonists was not accompanied by extra side effects. Ongoing clinical trials aim at answering the question whether the addition of daclizumab and basiliximab will allow to avoid or decrease the use of more toxic immunosuppressive drugs.

Three major double-blind trials in kidney transplantation patients have shown that mycophenolic acid (mycophenolate mofetil), added to an immunosuppressive regimen consisting of cyclosporine and prednisone, reduces the incidence of acute rejection after kidney transplantation by 50%, during the first six months. This statistically significant reduction is achieved equally with daily doses of 2 or of 3 g. In view of the fact that the side effects (diarrhoea, abdominal cramps, leukopenia) are more frequently found in the patients treated with 3 g, it is advised to prescribe 2 g mycophenolic acid. As acute rejection is a risk factor for the development of chronic rejection and because a beneficial effect of mycophenolic acid on chronic rejection in animal models has been observed, there may also be an effect on late graft loss due to chronic rejection after kidney transplantation in man.

To investigate the wishes and ideas about length of working week, part-time work, length of career and future kind of establishment, a questionnaire was sent to all Dutch registrars of internal medicine. In the opinion of more than 70 percent of the registrars, the length of a full-time working week of an internist should be less than 50 hours. Of all registrars (62% men and 38% women), 68% would decide in favour of part-time work if the length of a full-time working week of an internist were to remain at about 68 hours (common length of a working week of Dutch internists nowadays). 33% would still like to work part-time when the length of a full-time working week became as they desired (about 47 hours). The preferred length of their career was for most registrars less than 30 years. The majority preferred a tenure to a position in a partnership. In conclusion, the wishes about the future of the registrars of internal medicine differ markedly from the current situation.
