1. Treatment of acute cholecystitis in a third-trimester pregnant women

Abstract

This case report describes a 35-year-old female with acute cholecystitis 36 weeks into her pregnancy. Laparoscopic cholecystectomy was assessed not to be possible. An ultrasonic guided percutaneous transhepatic gall bladder drainage was performed resulting in immediate pain relief. The patient was discharged two days later, and the drain sat in place until a caesarian section was per--formed. A post-surgery cholangiography demonstrated stones in the gall bladder but no stones in the common bile duct. An uneventful laparoscopic cholecystectomy was carried out three months after surgery. The case report demonstrates that percutaneous transhepatic gall bladder drainage is a safe procedure to be considered in women with cholecystitis in which cholecystectomy is not possible or assumed to be associated with an unacceptable high risk.

Abstract

Vindmøller designes til at modstå virkningen af den turbulente vind, og i den forbindelse plejer man, at antage at hastighedens sandsynlighed er normalfordelt. Det synes, på baggrund af et stort antal målinger fra flere lokaliteter, at være en rimeligantagelse i fladt homogent terræn, men der findes tilfælde, typisk i bjergområder, hvor antagelsen om Gaussisk turbulens virker mere tvivlsom. Her har vindhastigheden ofte en skæv fordeling, hvor vindhuller er hyppigere end vindstød. For at simuleresådanne vindpåvirkninger, er der udviklet og implementeret en numerisk metode til at simulere kunstig turbulens. Metoden kan simulere samtidige tidsserier med en sandsynlighedsfordeling, der ikke nødvendigvis er Gaussisk, uden at forvrængespektral-fordelingen eller den rumlig kohærens. De simulerede tidsserier blev siden brugt som input til lastsimuleringsprogrammet Vestas Turbine Simulator (VTS). Herved kunne vi simuler dynamisk respons af systemer udsat henholdsvis for Gaussisk turbulensog turbulens med ekstreme men realistiske ikke-Gaussiske sandsynlighedsfordelinger. Visse udmattelseslaster på møller med aktiv pitch regulering steg op til 15% i forhold til lasterne for ren Gaussisk turbulens. Hertil skal dog siges at den uheldigevirkning afhænger af det dynamiske system, og det er tænkeligt at møllernes reguleringssystem kunne optimeres efter de lokale turbulensforhold.

The paper describes 14 tests on the shear strength of reinforced masonry beams made of tile bricks and lime/cement mortar. Besides a few tests on the strength of the interface between brick and mortar are treated. The project was supported by the Masonry Development Committee (MUR) and the interface tests were carried out at the Danish Building Research Station (SBI).

In this report a theory is developed for the calculation of crack directions, crack distances and crack widths in reinforced concrete structures subjected to biaxial stress fields.The crack direction is determined by minimization of the complementary elastic energy.For crack distances and crack widths an approximate method of calculation, based on the uniaxial theory, is given.It is shown, that the theory is in good ageement with tests.

Abstract

Artificial intelligence (AI) has the potential to increase quality and efficiency of breast cancer screening. Recent studies have provided comparative data on AI versus human performance in cancer detection with encouraging results, and commercially available AI systems are used worldwide as a clinical tool for mammography screening. There are, however, a number of methodological concerns in relation to the evaluation of AI systems. This review discusses these aspects as well as the opportunities and challenges of clinical validation and implementation in breast cancer screening practice.

Abstract

As the prognoses of both heart and cancer patients have improved along with a longer life expectancy in the general population, the prevalence of both heart- and cancer diseases is increasing. Thus, a larger proportion of cancer patients will have cardiovascular co-morbidity and an increased risk of cardiovascular complications during and after cancer treatment. In this article, the current knowledge on the prevention, monitoring and treatment of cardiotoxicity induced by medical anti-cancer treatment with focus on anthracyclines, trastuzumab and 5-fluorouracil is described.

Abstract

Optic disc oedema describes the nonspecific, localized swelling of the optic nerve head regardless of aetiology. Therefore, differentiating among the various aetiologies depends on a thorough history and knowledge of the clinical characteristics of the underlying conditions. Papilloedema strictly refers to optic disc oedema as a consequence of elevated intracranial pressure. It is usually a bilateral condition and visual function is preserved until late. Optic disc oedema caused by an anterior optic neuropathy is usually unilateral and accompanied by the loss of visual function.

Abstract

A review of patient charts in 322 in-hospital deaths in one surgical unit from July 1 2010 to June 31 2011. Overall mortality was 2.9%, with terminal cancer patients, patients unsolvable at admission and patients declining treatment comprising 35.7% of patients dying. Most fatalities (169/53%) were after acute admission (median age 78 years, 61%, ASA score > 2). Stratification of in-hospital mortality in surgical patients is feasible and meaningful. A measurable reduction in surgical mortality is probably only possible in the group of patients with acute admissions.

Abstract

Increased plasma levels of immunoglobulin E (IgE) are encountered in various clinical settings, most often infection or atopic/allergic disease. IgE, however, is neither a sensitive, nor a specific marker for any particular disease. We describe a case of a 52-year-old woman presenting with non-specific symptoms and increased plasma IgE levels on admission. Atopy/allergy, infection and malignancy were ruled out, and the only plausible cause remained a pulmonary embolism found incidentally on a computed tomography. The patient's IgE levels rapidly declined after the initiation of anticoagulant therapy.
