Title: Case study of physiotherapeutic treatment of a patient after implantation of total hip replacement Aims: The aim of this thesis is to provide a case study of physiotherapeutic treatment of a patient after implantation of a total hip replacement which had been preceded by hip dysplasia. Methods: Examination techniques and methods, which are taught at the bachelor's degree level at FTVS UK were used within this case study observed at the Malvazinky Rehabilitation Clinic in Prague. As part of the patient's treatment the medical history was established and an initinal kinesiological analysis was performed, based on which the aims of the following therapy were determined. The therapy consisted of sort tissue techniques, joint play mobilization, individual kinesiotherapy and locomotion training. Emphasis was also placed on autotherapy. At the end of the treatment, a final kinesiological analysis was performed after which the effect of the therapy was evaluated. Results: During the treatment the swelling of the lower left leg and of the surroundings of the surgical wound dissipated. The pain of the operated joint and the left thigh alleviated. The range of motion of the left hip and knee increased. There was an increase in muscle strength of the weakened muscles and the joint play was restored....

Author: Jiří Klíma Title: A case study of physiotherapeutic treatment of a patient with a diagnosis of total hip endoprosthesis Objectives: The objective of this Bachelor's thesis is to review a case study of physiotherapeutic treatment of a patient with a diagnosis of total hip endoprosthesis and become acquainted with this diagnosis from a theoretical point of view. Methods: A case study of physiotherapeutic treatment of the patient was produced during practise in the Malvazinky Rehabilitation Clinic from the 11th of January to 5th of February 2021. It included anamnesis, initial kinesiological examination, both short-term and long-term physiotherapy plans, 14 physiotherapy sessions, a final kinesiological examination and an evaluation of the effect of therapy. Results: Physical condition of the patient was improved. The postoperative oedema was reduced and condition of soft tissue around the scar was improved. The range of flexion, extension and abduction of the operated hip joint increased. Flexors, extensors and abductors have been strengthened. v anglickém jazyce This thesis strives to find relationship between human posture and the origin and progression of coxarthrosis. The main idea is built on the fact that the degenerative joint disease often arises on the basis of the chronical overload in the functional decentered position. The theoretical section is focused on trunk and hip muscles coordination that is defined in the ontogenesis. Moreover, the information about joint loading and stabilization is mentioned. The practical part consists of two case studies, which are observing subjective and objective changes after therapy. There are no local techniques in this therapy.

Title: Case study of physiotherapy treatment of a patient with total hip replacement. Objectives: The aim of this thesis was to summarize theoretical knowledge about the hip joint and total hip arthroplasty and the subsequent elaboration of a practical case study of physiotherapeutic care in a patient with this diagnosis. Methods: This thesis is divided into theoretical and practical parts. The theoretical part describes the hip joint's anatomical structure, osteoarthritis, coxarthrosis, and total hip arthroplasty. The practical part of the work contains a detailed case study of physiotherapeutic care for a patient after implantation of total hip arthroplasty. The case study includes anamnesis, entry kinesiological analysis, short-term and long-term therapeutic plan, therapy design, the record of individual therapeutic units, outcome kinesiological analysis, and evaluation of the effect of therapy. Results: Goals set by the short-term therapeutic plan have been met within the performed therapeutical units. In particular, the patient achieved a reduction in pain in the area of the operated joint, an increase in the range of motion, an increase in the muscular strength of weakened muscles, and an adjustment of the walking stereotype. Title: The case study of a physiotherapeutic treatment of a patient after reoperation of a total hip replacement. Objective: The aim of this bachelor's thesis was a comprehensive elaboration and summary of theoretical knowledge and practical skills related to physiotherapeutic care after reoperation of a total hip replacement. Method: The bachelor thesis consists of two main parts - general and special. The first, general part, contains knowledge of anatomy and kinesiology of the hip joint. It contains a chapter on coxarthrosis, its etiology and possibilities of its treatment. It also describes the various types og hip implants, the most commonly used surgical approches, indications, contraindicatios and complications of the surgery. It also includes a description of the cource of preoperative rehabilatition, early rehabilitation after surgery and subsequent medial rehabilitation. The second, special part, contains a case study of a patient after reoperatin of a total hip replacement, the course of treatment and evaulation of the effect of therapy. Results: The goals of the short - term therapeutic plan were fullfilled. The applied therapy increased the range of motion in the joints of the operated lower limb, increased muscle strength, stretched shortened muscles, affected hypertonic muscles,...

Fear of postoperative pain is one of the most important concern of patients undergoing total hip arthroplasty and it is therefore important to address this issue in medical, and nursing care. In this research we focus on how patients experience pain after total hip arthroplasty. The diploma thesis is divided into two consecutive parts, namely the theoretical, and empirical part. The theoretical part describes the current state of total hip replacement, coxarthrosis, and postoperative pain management. From this part it is clear that the treatment of postoperative pain is dominated by pharmacological methods. In the empirical part, hypotheses were established which dealt with the measurement of the intensity of postoperative pain after total hip replacement surgery the use of pharmacological and non-pharmacological methods of pain treatment and the timeliness of postoperative pain relief. In the practical part of the work a quantitative survey was used. A query sheet was created which was then distributed to patients in the orthopedic patient department. The query sheets were filled out in the first, third, and fifth postoperative day and participation in it was voluntary. From a non-standardized query sheet the data were analyzed by statistics and the results are shown graphically in the thesis. The research survey shows that female and male respondents rate postoperative pain in the same way. The gradual reduction of postoperative pain is also the same for both sexes of the respondents. At the initial meeting with postoperative pain the medical staff instructed patients about the possibilities of pain treatment as the use of pharmacological and non-pharmacological pain treatment. In the query sheet the 87 (81 %) respondents choose the combination of both therapies on the first postoperative day. The 101 (94 %) respondents stated that they used 3 or more methods of non-pharmacological pain treatment on the fifth day. Research has shown that patients experienced less pain after surgery every day and the treatment of the postoperative pain was successful in terms of the rate of analgesic administration by a nurse according to the doctor's instruction. The use of non-pharmacological interventions in the treatment of pain also increased every day.

It covers general information about anatomy and kinesiology of a hip joint, findings about coxarthrosis, alloplastic implantation of a hip joint and rehabilitation. Specialized part of this work presents the case study in the physical therapy on a patient post total hip endoprostheses. It includes the initial kinesiological analysis, short-term and long-term plan of physical therapy, course of physical therapy sessions, final kinesiological analysis and factual evaluation of therapy. Title: The case interpretation of physiotherapeutic treatment of a patient with total hip replacement diagnosis Aims: The aim of the thesis has been comprehensive arrangement of theoretical findings, their deepening and exercising of practical knowledge related to total hip replacement diagnosis and subsequent physiotherapeutic treatment. Summary: The thesis is divided into two parts. The first one includes theoretical knowledge of hip joint anatomy and kinesiology, furthermore coxarthrosis findings (the most often reason of necessity of hip joint alloplastic surgery), its phases, rehabilitation, surgical attitude and kinds of used total hip replacements. The other part is dedicated to a particular case of a female patient after the total hip replacement surgery, I had an opportunity to visit her during 8 treatments. At the end of the part there is a comparison of input and output kinesiological analysis and total assessment of the treatment effect. It contains information about the symptoms of the diagnosis, the surgical approaches, different types of implants and the possibilities of aftercare with a focus on physiotherapeutic treatment. Objectives: The main aim of this work was to summarize theoretical and practical knowledge about total hip replacement and to process a case study of a patient with this diagnosis. Methods: This work consists of two main parts - general and special part. The general part contains information about hip joint anatomy and kinesiology, coxarthrosis, congenital hip dysplasia, total hip replacement and rehabilitation after total hip replacement. A special part is processed in the form of a case study of a patient after implantation of total hip replacement. It includes anamnesis, entry kinesiological examination, goals of short-term and long-term therapeutic plan, description of therapeutic units, outcome kinesiological examination and a final evaluation of the effect of therapy. Results: The goals of the short-term therapeutic plan were fulfilled. During the therapy, soft tissues were relaxed, the muscle tone of the hypertonic muscles was reduced, the range of motion in the joints of the lower limbs increased, the joint play restored, the weakened muscles strengthened, the shortened muscles stretched and the gait improved. Title: The case study of a physioterapeutic treatment of a patient with a diagnosis total hip replacement. Objectives: The aim of this study is to summarize theoretical and practical knowledge of total hip replacement and to process the case study of a patient with this diagnosis. Methods: This thesis is divided into two main parts - a general one and a special one. The general part includes topics associated with a total hip replacement. It describes the structure of the hip joint, coxarthrosis, which was the indication for the surgery, different types of hip implants and the rehabilitation process. The special part contains the case study of a patient after a total hip replacement, which describes the therapy process along with a final evaluation of the effect of therapies. Results: We managed to fulfil our short term goals. Range of motion in afflicted lower limb was increased, muscle strength was increased, shortened muscles were stretched, the muscle tone in hypertonic muscles was decreased, the treatment of soft tissues was made, walking stereotype was improved and physical fitness of a patient was increased. This Bachelor thesis is focused on coxarthrosis, kinesiology and patokinesiology of hip muscles. The goal of the thesis is to describe compensatory changes in patients affected by coxarthrosis and summarize the possibilities of influencing by physiotherapy procedures.

The Bachelor thesis deals with the use of low-induction magnetotherapy in case of patients with coxarthrosis. The aim of the thesis was an application of the low-induction magnetotherapy in treatment of particular patients and its subsequent evaluation. Coxarthrosis belongs to relatively often degenerative disease nowadays, increasingly frequent among younger patients. Pain and joint stiffness, particularly in the morning, and limited range of movements are usually the main symptoms, leading to constraints in performing of normal daily activities. Moreover, most natural human movement - walking is impaired by the disease. Low-induction magnetotherapy is nowadays often used method of physical therapy belonging to a non-contact low-frequency electrotherapy which has as its main task to reduce the pain in coxarthritis. The thesis consists of two main parts. The theoretical part contains a description of the anatomy of the hip joint, more specifically its construction, ligamentous apparatus, vessels and nerves, muscle apparatus, movements in the hip joint and hip joint kinesiology. Osteoarthrosis, its principles, causes, symptoms, diagnosis and treatment, and coxarthrosis in more detail are also described here. The last topic discussed in the theoretical part is low-induction magnetotherapy. Its principles, physical parameters and the effects of magnetic fields, indications and contraindications are explained. In the practical part, qualitative research method was used. The research group consisted of three patients who attended Medipont polyclinics in České Budějovice on outpatient basis. In addition to low-induction magnetotherapy, physical therapy was indicated as well. Low-induction magnetotherapy is not recommended for use as monotherapy but its combination with other treatments is reported as more effective. The research was elaborated in the form of case studies that include patients' medical history, input and output kinesiology analysis and course of individual therapy. Kinesiology analysis includes a visual examination, palpation, anthropometric and trigonometric examinations, examination of pelvis, muscle test, and examination of shortened muscles, movement stereotypes, breathing, as well as walking. During therapy, the PIR, joint mobilization, soft techniques, exercises to activate HSS, stretching and strengthening exercises for the hip joint, and exercises for the proper activation of the transverse and longitudinal arch of the foot were used in addition to the low-induction magnetotherapy. Output examination after ten therapies showed changes in health status in all three patients; for each one, however, to a different degree. For best results, I would advise to continue with this treatment in the future. Interviews with ten patients in Berta's Spa in Třeboň and Medipont polyclinics in České Budějovice were conducted to better evaluation of the effects of the low-induction magnetotherapy. Patients rated their subjective experience with application of the low-induction magnetotherapy.

The second, specialist part deals with the casuistry of a patient submitted to the implantation of an artificial hip joint, including an assessment of the effects of the chosen therapy by comparing the kinesiological analyses before and after the treatment. Title: The case study of a physiotherapeutical treatment of a patient with total hip arthroplasty Summary: The aim of this thesis is to elaborate theoretical and practical knowledge of the physiotherapy care of patients with this diagnosis. The thesis is divided into two parts. The first general part is focused on anatomical and kinesiological description of the hip and issue of an osteoarthritis and the joint arthroplasty. The second part is a special part, in which is processed a case of a patient with total hip arthroplasty. This case report includes an input kinesiology analysis, a target of the therapy, a short-term and a long-term plan, a course of therapeutic units, an output kinesiology analysis and an evaluation of the effect of the therapy. The case was processed during a continuous professional practice in the Military University Hospital Prague at the Department of Physical Medicine and Rehabilitation in the period from 22th of January to 3rd of February 2015. Práce posuzuje možnosti využití chůzových testů u pacientů s koxartrózou. Na skupině třiceti pacientů se třetím stupněm koxartrózy kyčelního kloubu bylo provedeno měření pěti různých testů: dotazník Harris Hip Score, Timed Up And Go Test (TUG), Ten Meter Walk Test - desetimetrový test chůze (10MWT), Two Minute Walk Test - dvouminutový test chůze (2MWT) a Six Minute Walk Test - šestiminutový test chůze (6MWT). Následně byly porovnány výsledky jednotlivých testů a zjištěny jejich specifické výpovědní vlastnosti. Cílem studie bylo zjistit jejich vzájemné souvislosti, limity v použití, případně navrhnout jejich úpravu a doporučit cílové skupiny pacientů pro jednotlivé druhy testů. Během měření a při zpracování studie byly zjištěny statisticky významné skutečnosti, pomocí kterých by bylo možné zpřesnit hodnocení kvality terapie a zvýšit efektivitu vyšetřování pacientů s koxartrózou. Byla prokázána shodná výpovědní hodnota testu 2MWT ve srovnání s delšími testy (6MWT) a také výhody provedení testu TUG, jako zdroje doplňujících informací k vyšetření pomocí dotazníku Harris a běžných chůzových testů.
