Lysosomal storage diseases (LSDs) form a group of more than 70 rare inherited metabolic diseases, usually caused by a malfunction of some of the lysosomal enzymes. The aim of this review is to introduce enzyme replacement therapy (ERT) which gained an indispensable role in the therapy of LSDs. ERT is based upon replacement of the defective enzyme by a recombinant protein administred in a 2-4 hour infusion once a week or once a fortnight. This therapy is currently available for seven LSDs: Gaucher disease, Fabry disease, Pompe disease and mucopolysaccharidoses I, II, IVA and VI. ERT significantly affects visceral manifestations of the diseases (hepatomegaly, splenomegaly, vital lung capacity, muscle weakness, anaemia, gastrointestinal disorders etc.), yet it is not effective in treating the central nervous system (CNS) involvement and only a small effect is observed in treating disease manifestations in bones, cartilage and heart valves. An immune reaction with clinical presentation of an allergic reaction and therapy effectiveness decrease can occur with a portion of patients. Other modalities for LSDs therapy are concisely mentioned: haematopoietic stem cell transplantation, substrate reduction therapy, chaperons and gene therapy. Conclusion: ERT slows the progression of the disease and markedly alters its natural course, improves quality of life and prolongs patients' lifespan. The limitations include insufficient effect in some tissues, possible allergic reactions and great financial demands. An essential presumption for effectiveness and success of ERT is its early initiation following quick diagnosis of the disease. [ABSTRACT FROM AUTHOR]

More than one quarter of 900 known inherited metabolic disorders (IMD) may manifest in the neonatal age. Their diagnostics is demanding. The correct and early diagnosis with subsequent therapy introduction may have a substantial impact on further prognosis of the child. Case reports of three children with manifestation of IMD in neonatal age and late diagnosis are presented. On contrary, assessment of early diagnosis saved favourable prognosis of the child with galactosemia and enabled the early introduction of adequate therapy in a child with alpha-1-antitrypsin deficit. The first girl with methymalonicaciduria manifested sepsis-like state with hyperventilation, consciousness disorder and seizures, severe metabolic acidosis and hyperam-monemia. The liver failure was the main symptom of the girl with galactosemia. The boy from the third case report with glycogenosis type Ib showed repeated hypoglycamiae. The leading symptom of mitochondrial disorder in the fourth newborn -- with ATP synthesis deficiency due to mutation in TMEM70 gene was the hypertrophic cardiomyopathy. The alpha-1-antitrypsin deficit was determined in the fifth boy with hepatopathy. The acute and life-threatening state typically occurs after period of relative health in acute intoxication type of IMD. The relations of ketonuria and significant metabolic acidosis or hyperammonemia are often present. The selective IMD screening or comprehensive metabolic investigation in the specialized center is indicated in case of suspicion on IMD based on clinical and laboratory signs. Úvod: Syndrom MELAS (Mitochondriální Ence-falomyopatie, Laktátová Acidóza a Iktu podobné příhody -- stroke-like episodes) je maternálně dědičné mitochondriální onemocnění s variabilním klinickým obrazem a věkem nástupu onemocnění. Onemocnění se může vázat s mutacemi v několika genech mitochondriální DNA (mtDNA). Jako první byla u syndromu MELAS popsána mutace 3243A>G v MTTL1 genu a je s ním nejčastěji spojována. Cílem této práce je retrospektivní zpracování klinických a laboratorních dat souboru českých pacientů s průkazem uvedené mutace diagnostikovaných za období let 1992-2012. Výsledky: Mutaci 3243A>G jsme prokázali u 42 pacientů (29 symptomatických). První příznak onemocnění se manifestoval ve věku 1-59 let, s mediánem 18 let. Průměrný věk symptomatických a asymptomatických pacientů při hodnocení se výrazně nelišil (32,8 vs. 27,3 let). U symptomatických pacientů byla vyšší průměrná hladina heteroplas-mie mutované mtDNA v krvi (25,5 vs. 13,8 %) i ve vyšetření vlasových folikulů (33,6 vs. 17,6 %). Nejčastějším příznakem byla kombinace poruchy sluchu (76 %) a svalové slabosti (52 %) nebo laboratorně prokázané myopatie (60 %). Často byl postižen zrak (48 %), nejčastěji ve formě zevní oftalmoflegie (34 %) nebo ptózy víček (21 %). Třetina pacientů měla diabetes mellitus, čtvrtina pak postižení štítné žlázy. Zvýšená hladina laktátu v klidovém stadiu onemocnění byla přítomna u 42 % pacientů. Iktu podobná příhoda se vyskytla pouze u 28 % pacientů. Závěr: Z našich výsledků vyplývá, že plně vyjádřený fenotyp syndromu MELAS byl přítomný u pouhých 17 % symptomatických pacientů s mutací 3243A>G a že je nutné o tomto onemocnění uvažovat i u všech pacientů, kde se kombinuje porucha sluchu s projevy myopatie a diabetů. Důležitým nálezem je také přítomnost oftalmoplegie a ptózy víček. Normální hladina laktátu a nepřítomnost iktu podobné příhody toto onemocnění nevylučuje. [ABSTRACT FROM AUTHOR]

Úvod: Mukopolysacharidóza II. typu (MPS II) je X-vázané vázáné onemocnění s incidencí 1:140 000. Příčinou je nízká aktivita lysozomálního enzymu iduronátsulfatázy, který se podílí na štěpení der-matan- a heparansulfátu. Mezi hlavní klinické projevy patří hrubé rysy obličeje, opakované záněty horních cest dýchacích a otitidy, porucha sluchu, hepatomegalie, pupeční a tříselné kýly, ztuhlost kloubů a opoždění psychomotorického vývoje. Principem léčby je nitrožilně podávaná náhrada chybějícího enzymu (ERT) preparátem Elaprase. Cíl: Popsat klinický průběh a efekt léčby u čtyř pacientů s MPS II. Výsledky: ERT pro MPS II je v ČR dostupná od roku 2008. Od té doby byla indikována u 3 chlapců a jednoho dospělého pacienta ve věku 6, 7, 14 a 35 let. Soubor pacientů ilustruje kontinuum klinické manifestace od závažné formy s těžkou mentální retardaci u nejmladšího chlapce, přes intermediární formu s mírným opožděním vývoje u dvou chlapců až po lehkou formu u dospělého pacienta s nadprůměrnou inteligencí. Infuzní terapie je podávaná jednou týdně, u dětí cestou venózního portu. Během 52, 37, 45 a 2 měsíců trvající terapie se nevyskytly významné nežádoucí účinky. Při terapii došlo k ústupu organomegalie a snížení vylučování mukopolysacharidů v moči (na 53 ± 8 % ve 2 měsících, 63 ± 18 % v roce a 49 ± 11 % ve dvou letech). Jelikož enzym neprostupuje hematoencefalickou bariérou, nebyl ovlivněn případný kognitivní a mentální deficit. Významnou roli v péči o pacienty s MPS II má terapie podpůrná, zejména rehabilitace a psychosociální podpora rodiny. Závěr: Mukopolysacharidóza II. typuje X-vázané lysozomální střádavé onemocnění se širokou klinickou variabilitou. Finančně náročná enzymová substituční terapie pomáhá stabilizovat jeho průběh. Background: Hereditary tyrosinemia type 1 (HT1) is a rare autosomal recessive inborn error of metabolism caused by deficiency of fumarylacetoacetate hydrolase. HT1 manifests with severe liver and kidney impairment and associates with an increased risk of liver cancer development. The aim of our study is to present a detailed clinical picture and results of biochemical and molecular genetic analyses in 11 Czech patients with HT1 diagnosed in our clinic within 1982-2006., Methods and Results: In 9 patients the disease manifested between 1.5-7 months of age with refusal to eat, failure to thrive and vomiting. In 4 children HT1 progressed to acute liver failure. One clinically healthy boy was diagnosed because of affected sister. In one boy with liver cirrhosis the diagnosis was delayed until the age of 5.5 years. In all children the biochemical investigation showed elevated liver enzymes, alpha1-fetoprotein and hypophosphatemic rickets. Metabolic investigation revealed increased plasma tyrosine level, urinary excretion of succinylacetone and in 8 measured patients also increased urinary delta-aminolevulinic acid concentration. Three patients born before 1988 died due to liver cancer development (two of them) or liver failure. The average age of our 8 living patients is 10.7 +/- 8.3 years. Mutation analysis of FAH gene confirmed the HT1 in these patients and three novel mutations were found in FAH gene: c.579C>A, c.680G>T and c.1210G>A. Clinical status in six patients is favourable on strict low protein diet combined with Orfadin therapy. However, in two children despite of the maximal available therapy lasting 2 and 10 years resp., the disease progressed towards liver cancer development and necessity of liver transplantation., Conclusions: Early diagnostics of HT1 as a part of extended newborn screening is the only possibility to further improve the prognosis of the patients. Moreover, available molecular-genetic analysis of the FAH gene enables prenatal diagnostics in affected families.
