- Abstract
Acute severe ulcerative colitis represents serious clinical event which complicates disease course in 25% patients and it is caused byl inflammation penetrating through the whole large bowel wall. In American literature is called a fulminant colitis. These event is associated with high risk of life threatening complications including bowel perforation, toxic megacolon, sepsis with multiorgans failure and/or with tromboembolic complications. The first step in therapy is the parenteral application of corticosteroids together with an intensive and complex care, in those patients with non-response with a few days, the surgery - colectomy and ileostomy might by considered, or the rescue therapy with cyclosporine A or infliximab is indicated. The promising therapy for the future seems to be combination cyclosporine A with monoclonal anti-integrin antibody-vedolizumab. [ABSTRACT FROM AUTHOR]
