Objective To investigate the efficacy and effects on cognitive function and sleep quality of Buqi Wenyang Dingkuang Decoction in treatment of mania patients with upfloating of asthenia yang type. Methods A total of 100 patients with mania who were hospitalized between January 2020 and January 2022 in the Affiliated Brain Hospital of Guangzhou Medical University were selected and divided into observation group and control group according to the random number table method (n=50, each) . Both group was given valproate sustained-release tablets and quetiapine tablets treatment, and the observation group was added with Buqi Wenyang Dingkuang Decoction. The treatment course of the 2 groups was 4 weeks. Treatment efficacy and incidence of adverse reactions were compared between two groups. The primary syndrome score, Clinical Global Impressions Scale (CGI) score, Pittsburgh Sleep Quality Index (PSQI) score and Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) score before and after treatment were compared between the two groups. Results The total effective rate was higher in the observation group than in the control group (90.00% vs 72.00%, χ2=5.263, P＜0.05) . The scores of mania, thirst not to drink, and limb cold at 4 weeks of treatment in the 2 groups were lower than those before treatment (P＜0.05) , and those of the observation group were lower than those of the control group (P＜0.01) . The BRMS scale score, CGI score, and PSQI scale score of the 2 groups at 4 weeks of treatment were lower than those before treatment (P＜0.01) , and the above-mentioned scores of the observation group were lower than those of the control group (P＜0.01) . The scores of immediate memory, spatial structure, attention, verbal function, and delayed memory in the 2 groups at 4 weeks of treatment were higher than those at pretreatment (P＜0.01) , and the scores in the observation group after treatment were higher than those in the control group (P＜0.05) . Fewer adverse reactions were observed in the observation group than those in the control group (8.00% vs 26.00%, P＜0.05) . Conclusion Buqi Wenyang Dingkuang Decoction has a certain effect on mania patients with upfloating of asthenia yang type, and can significantly improve their cognitive function and sleep quality.

This paper reported a case of patient with body dysmorphic disorder in order to improve the clinical interview skills， diagnosis and treatment of clinicians in psychiatric department. The client was a 17-year-old unmarried male， came for psychological consultation because he was not satisfied with his appearance and had low self-esteem. After the changing of learning environment， the client showed excessive attention to his appearance along with low mood. The client was diagnosed with body dysmorphic disorder， and a biological-psychosocial comprehensive intervention was recommended. Sertraline combined with quetiapine was given as biotherapy to improve his mood and cognition. The psychotherapy included mindfulness-based stress reduction and cognitive behavioral therapy. These therapies helped the client learn to relax and correct cognitive biases. In terms of social resources， family members were encouraged to pay more attention to the client’s advantages， and schoolwork management on campus was strengthen. These methods promoted the development of client’s self-confidence.

Objective *To investigate the therapeutic efficacy of metformin (MF) on glucose dysregulation and dyslipidaemia induced by quetiapine in patients with schizophrenia (SZ). Methods *A total of 83 SZ patients from Shanghai Hongkou District Mental Health Center and Shanghai Yangpu District Mental Health Center were enrolled, and randomly divided into treatment group (MF group, n=38) and control group (n=45). The control group received oral quetiapine therapy, and the MF group additionally received metformin on the basis of oral quetiapine therapy. The serum levels of glucose (GLU), total cholesterol (TC), triacylglycerol (TAG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), small density low-density lipoprotein cholesterol (sd-LDL-C) and homocysteine (HCY) were determined before treatment and 2, 4 and 8 weeks after treatment. Results *There were no statistic differences in ages, courses, body mass index, the levels of GLU, TC, TAG, HDL-C, LDL-C, sd-LDL-C and HCY between the control and MF group before treatment (all P>0.05). After 8 weeks of treatment, levels of all biomarkers elevated in the control group. The levels of GLU, TC, TAG, LDL-C, sd-LDL-C and HCY were lower after treatment, and the level of HDL-C elevated higher in the MF group. The variations of levels of HDL-C after treatment showed statistically no significant difference between the two groups (P=0.247), while statistical differences existed in other biomarkers, which indicated that the glucose-lowering and lipid-lowering efficacies in the MF group were significantly better than those in the control group. The analysis of variance suggested that, in the control group, the level of HDL-C elevated significantly after 4 and 8 weeks' treatment, having statistical differences (P=0.005 and P=0.003). The level of LDL-C elevated significantly after 2, 4 and 8 weeks' treatment (P=0.005, P= 0.019 and P=0.026), and no statistical differences were observed in other biomarkers. In the MF group, the levels of GLU, TC, TAG and sd-LDL-C were lower 2, 4 and 8 weeks after treatment, the levels of LDL-C and HCY descended 4 and 8 weeks after treatment, and no statistical differences were observed in other biomarkers. The level of HDL-C was higher after 2 weeks, treatment, but showed statistically no significant differences 4 and 8 weeks after treatment, compared to the level determined before treatment. Conclusion *Administration of MF in the SZ patients having glucose dysregulation and dyslipidaemia induced by quetiapine can significantly lower serum level of GLU, and improve the serum lipid profiles. OBJECTIVE: To probe into the efficacy and safety of quetiapine and amisulpride in treatment of mental disorder induced by elderly cerebrovascular diseases. METHODS: 82 patients with mental disorder induced by elderly cerebrovascular diseases admitted into Jinzhou Kangning Hospital from Feb. 2015 to Sep. 2016 were retrospectively selected and divided into observation group and control group according to different therapeutic options, with 41 cases in each. The control group was treated with quetiapine, while the observation group was given amisulpride, both groups received a 2-month treatment. Scores of brief psychiatric rating scale (BPRS) and scale for the assessment of negative symptoms (SANS), clinical efficacy and incidences of adverse drug reactions of two groups were observed. RESULTS: y\t half a month, 1 month and 2 months alter treatment, the BPRS score and SANS score of both groups were significantly improved than those of before treatment, with statistically significant difference (P <0.05), while no statistical significances were found in intergroup comparisons (P>0. 05); the total effective rate of observation group (95. 12%, 39/41) was significantly higher than that of the control group (78. 05%, 32/41), with statistically significant difference (P <0. 05) ; the incidence of adverse drug reactions of observation group (14. 64%, 6/41) was significantly lower than that of the control group (70.73%, 29/41), with statistically significant difference (P<0. 05). CONCLUSIONS: The efficacy of quetiapine and amisulpride in treatment of mental disorder induced bv elderly cerebrovascular diseases are both remarkable, yet the amisulpride performed better in efficacy and safety. OBJECTIVE: To probe into the clinical efficacy of sodium valproate combined with quetiapine in treatment of mania-related cognitive impairment. METHODS: 90 patients With mania-related cognitive impairment admitted into the Third People’s Hospital of Zhanjiang from Jan. 2016 to Dec. 2016 were selected to be divided into observation group and control group via the random number table, with 45 cases in each. The control group were treated with quetiapine, while the observation group were given sodium valproate combined with quetiapine. The young mania rating scale, overall impression of the score table，side effects were adopted to evaluate the cognitive function of patients. RESULTS: The total effective rate of observation group was 97. 78% (44/45)，significantly higher than that of control group [88. 89% (40/45)]，with statistically significant difference (P < 0. 05). After treatment，the young mania rating scale and the overall impression of the score table were lower than that of control group，with statistically significant difference (P < 0. 05). After treatment of 6 weeks，the side effects of observation group was (4.2 ± 1.4) scores， significantly lower than that of control group [(5.6 ±2.1) scores]，with statistically significant difference (P < 0. 05). CONCLUSIONS: Effects of valproate combined with quetiapine in treatment of mania is better than that of quetiapine alone，which can improve the cognitive impairment of patients. Objective To investigate the efficacy and safety of risperidone combined with quetiapine in the treatment behavioral and psychiatric symptoms of dementia (BPSD). Methods Ninety cases of BPSD in the geriatric department of our hospital from January' 2013 to December 2015 were randomly divided into the combination group, risperidone group and quetiapine group according to the completely random method, 30 cases in each group. The combination group was given risperidone combined with quetiapine, the risperidone group was given the risperidone therapy and quetiapine group received the quetiapine therapy. The treatment effects were evaluated by using the BEHAVE-AD before treatment and after 2-,4-,8- week treatment. The side effects scale(TESS) was used to evaluate the adverse reactions occurrence situation. Results The AD-BEHAVE scores after 2-,4-,8-week treatment in 3 groups were significantly lower than those before treatment,the difference was statistically significant (P<0.05); the effect in the combination group was similar to that in the risperidone group, the difference had no statistical significance (P>0.05) ,but the effect in the combination group was significantly superior to that in the quetiapine group(χ2=15,39,P<0.05). The occurrence rate of adverse reactions in the risperidone group was significantly higher than that in the combination group and quetiapine group, and the difference was statistically significant (P<0.05). Conclusion Risperidone combined with quetiapine in the treatment of elderly BPSD has good curative effect, small adverse reactions, which is more suitable for the patients with senile dementia. Quetiapine is a psychotropic drug. Excessive use of quetiapine may lead to drowsiness, blurred vision, respiratory depression, hypotension and extrapyramidal reactions. Acute respiratory distress syndrome (ARDS) is rare due to overdose of quetiapine. On 14 February 2020, a patients with coma, respiratory arrest and hypotension due to overdose of quetiapine were admitted to our hospital. After receiving mechanical ventilation、plasma adsorption and anti-inflammatory treatment, the patient's consciousness turned clear, the machine was successfully removed and extubated, and the patient's condition was improved and discharged from hospital. We analyzed the clinical data of the patient with quetiapine poisoning, and discussed the clinical symptoms and chest CT characteristics of ARDS caused by quetiapine poisoning, in order to improve the understanding of quetiapine poisoning and improve the success rate of rescue.
